Objetivo: Evaluar asociación y la capacidad de predicción de los niveles de medicamento anti-TNF α y de anticuerpos contra anti-TNF α con relación a la respuesta clínica en pacientes con Artritis Reumatoide (AR) y Espondiloartritis (EAS) tratados con terapia biológica anti- TNF α en el Hospital Militar Central. Metodología: Estudio observacional analítico de cohorte. Pacientes con AR y EAS tratados con Adalimumab (ADA), Etanercept (ETN) e Infliximab (IFX) entre el 2012 y 2015. Muestreo a conveniencia. Se efectúo análisis bivariado y se calculó la chi-cuadrado como medida de asociación entre la concentración de anti TNF y los anticuerpos contra anti TNF de cada biológico, con las variables de desenlace clínico DAS28 (28 joint Disease activity Score) por VSG y PCR, CDAI (Clinical Disease activity Index), SDAI (Simple Disease activity index) y HAQ-DI (Health Assessment Questionnaire Disability Index) en AR, y BASDAI (Bath Ankylosing Spondylitis Disease Activity Index) y BASFI (Bath Ankylosing Spondylitis Functional Index) en EAS. Mediante regresión lineal múltiple se evaluó si la concentración de medicamento y los anticuerpos predecían la respuesta clínica para AR y EAS. En una cohorte de pacientes con AR y EAS se realizó una comparación de mediciones clínicas por criterios de respuesta EULAR y BASDAI50 respectivamente, para valorar la tasa de respuesta al tratamiento. Resultados: n= 51 para AR, n=48 para EAS. Los niveles totales de medicamentos (p=0,08, p=0,042, p= 0,047) especialmente ADA e IFX en AR así como los de IFX (p= 0,034) en las EAS se asociaron con actividad. Los anticuerpos contra anti-TNF no se asociaron con la respuesta clínica en pacientes AR, pero, en EAS si hubo asociación ente los niveles totales de anticuerpos contra anti-TNF y la actividad de la enfermedad (p=0,027). Los niveles de medicamentos anti-TNF y los anticuerpos contra anti-TNF no predijeron la respuesta clínica en los pacientes con AR ni en las EAS. En la cohorte evaluada la mayoría de los pacientes con AR tuvieron respuesta moderada y los de EAS pobre respuesta. Conclusión: Los niveles de medicamentos anti-TNF y los anticuerpos contra anti-TNF se asocian con la actividad de la enfermedad pero no la predicen. In a cohort of patients with AR and SpA were performed a comparison of clinical measurements by EULAR response criteria and BASDAI50 respectively, to assess the rate of response to treatment. Results: n = 51 for AR, n = 48 for SpA. Total levels of drugs (p=0,08, p=0,042, p= 0,047) especially ADA and IFX for AR, as well as those of IFX (p= 0,034) for SpA were associated with the activity. Anti-TNF antibodies were not associated with clinical response in RA patients, but in SpA there was an association between total levels of antibodies against anti-TNF and the disease activity(p=0,027). Levels of anti-TNF drugs and anti-TNF antibodies did not predict the clinical response in patients with RA or SpA. In the cohort evaluated the majority of patients with RA had moderate response and SpA had poor response. Conclusion: The levels of anti-TNF drugs and anti-TNF antibodies are associated with disease activity, but they do not predict it.

Periodontitis and rheumatoid arthritis (RA) are chronic destructive inflammatory diseases with significant worldwide prevalence. They are characterized by inflammatory lesions adjacent to bone destruction involving connective tissue and bone. Dickkopf-1 (DKK-1) is a major regulator of bone mass; however, their involvement in local bone resorption is largely unknown. The aim of this study was to assess the utility of DKK-1 levels in gingival crevicular fluid (GCF) and serum as a potential predictor of bone loss in periodontal disease and/or early RA.

6 páginas Introducción La enfermedad de Still de inicio en la edad adulta es un trastorno inflamatorio sistémico poco común de etiología desconocida que se caracteriza por la tríada clásica de fiebres altas y punzantes persistentes, dolor articular y una erupción irregular distintiva de color salmón; sin embargo, la afectación de múltiples órganos puede estar presente. Descripción del caso Una mujer de 40 años previamente sana fue remitida a nuestro hospital con 7 días de fiebre alta y artralgia generalizada.El examen físico reveló edema angioneurótico detectado en plantas de los pies, palmas y lengua y placas urticadas de color rojo generalizadas en distribución simétrica que afectaban a los brazos , manos dorsales, pecho superior e inferior y espalda. Seguido 5 días después por fiebre, el paciente presentó disnea, tos e hipoxemia, los estudios de imagen mostraron consolidación unilateral y derrame pleural. La broncoscopia con lavado broncoalveolar y biopsia de piel fueron compatibles con urticaria neutrofílica. Se excluyeron los trastornos hematológicos, infecciones y otras enfermedades autoinmunes. Discusión y evaluación El diagnóstico de la enfermedad de Still del adulto puede resultar muy difícil. No hay pruebas específicas y generalmente se confía en un complejo de síntomas y en la erupción típica bien descrita que se observa en la mayoría de los pacientes. En los últimos años, sin embargo, se han informado otras manifestaciones cutáneas de la enfermedad de Still del adulto, pero no son tan bien conocidas. Conclusiones La evidencia de manifestaciones raras está aumentando y la presentación clínica temprana del síndrome de Still del adulto es extremadamente variable, lo que dificulta el diagnóstico. Por este motivo, los datos sobre la presentación clínica precoz de la enfermedad son de interés. Reportamos el primer caso de enfermedad de Still aguda de inicio en el adulto con la asociación de hemorragia pulmonar, urticaria y angioedema, incluyendo una manifestación sistémica rara como reacción leucemoide. Introduction: Adult-onset Still’s disease is a rare systemic inflammatory disorder of unknown aetiology characterized by the classic triad of persistent high spiking fevers, joint pain and a distinctive salmon-colored bumpy rash however, the multiorgan involvement can be present. Case description: A 40-year-old woman previously healthy was referred to our hospital with 7 days of high fever and generalized arthralgia, The physical exam revealed angioneurotic edema detected on soles, palms and tongue and widespread red, urticated plaques in a symmetrical distribution affecting the arms, dorsal hands, upper and lower chest and back. Followed 5 days later by fever, the patient presented dyspnea, cough and hypoxemia, the imaging studies showed unilateral consolidation and pleural effusion. The bronchoscopy with bronchoalveolar lavage and skin biopsy were consistent with neutrophilic urticarial. The hematological disorders, infections and other autoimmune diseases were excluded. Discussion and evaluation: The diagnosis of adult-onset Still’s disease can be very difficult. There are no specific tests and reliance is usually placed on a symptom complex and the well described typical rash seen in most patients. In recent years, however, other cutaneous manifestations of Adult-onset Still’s disease have been reported but these are not so well known. Conclusions: The evidence of rare manifestations is growing and the early clinical presentation of Adult-onset Still’s is extremely variable, making diagnosis difficult. For this reason, data on early clinical presentation of the disease are of interest. We reported the first case of acute Adult-onset Still’s disease with the association of pulmonary hemorrhage, urticaria and angioedema including a rare systemic manifestation as leukemoid reaction.

Actualmente se percibe una necesidad apremiante en la identificación y validación de biomarcadores que reflejen tempranamente el inicio de actividad lúpica o que se conviertan en predictores de la misma. La actividad clínica del lupus eritematoso sistémico (LES) es ondulante a lo largo del tiempo y la actividad subyacente persistente lleva a daño tisular. Este daño es reflejo de cambios irreversibles en la función y estructura orgánica, por lo que la prevención, más que el tratamiento, debería ser la meta de cualquier terapia en LES y así lograr disminuir la morbimortalidad y los costos directos e indirectos causados por la enfermedad. Es necesario encontrar biomarcadores no invasivos de actividad lúpica que no solo permitan tomar de forma oportuna decisiones terapéuticas, sino que también se correlacionen con los desenlaces clínicos y sean útiles en los ensayos clínicos, permitiendo acortar el tiempo del desarrollo de estos estudios. Este artículo pretende buscar la evidencia que se tiene con respecto a los nuevos biomarcadores existentes para actividad de la enfermedad en LES y su utilidad actual y futura, enfatizando en la necesidad de buscar nuevas moléculas que permitan un diagnóstico más precoz de la actividad de la enfermedad. There is a need for the identification and validation of biomarkers that reflect the early onset of lupus activity or may be predictors of this. The clinical activity of systemic lupus erythematosus (SLE) fluctuates over time and the underlying activity leads to persistent tissue damage. This damage is a reflection of irreversible changes in the function and organic structure, so prevention, rather than treatment, should be the goal of any therapy in SLE.This will reduce morbidity, mortality, direct and indirect costs caused by the disease. It is necessary to find biomarkers of lupus activity that not only allow making treatment decisions in the short term, but also to correlate with clinical outcomes. These could also be useful in clinical trials and may shorten the duration of these studies. This article aims to find evidence on new biomarkers for SLE disease activity, and their current and future use. Emphasis will be made on the need to find new molecules for an early diagnosis of disease activity.

Presentamos el caso de una mujer de 32 años, con dos años de evolución de lesiones en piel tipo placas, induradas, nodulares, dolorosas, inicialmente en miembros inferiores con posterior compromiso generalizado, con evidencia de lesiones cutáneas sugestivas de dermatomiositis y biopsia de piel que reporta calcinosis; inicialmente sin evidencia clínica ni paraclínica que sugiriera compromiso muscular inflamatorio asociado. La resonancia magnética nuclear de músculos y la biopsia muscular confirmaron la presencia de este compromiso, definiendo los diagnósticos de dermatomiositis hipomiopática y calcinosis universal, dos espectros de una patología con baja frecuencia de presentación en la población adulta. The case is presented of a 32 year-old woman with indurated, raised and painful plaques in the skin of 2 years onset. It began in the legs, but later with a generalized extension to other regions, with evidence of cutaneous lesions very suggestive of dermatomyositis. The skin biopsy reported as calcinosis; initially without clinical evidence of inflammatory muscle compromise. A Nuclear Magnetic Resonance study and muscle biopsy showed this compromise, leading to the diagnosis of hypomyopathic dermatomyositis and universal calcinosis, two pathologies with rare presentation in an adult patient.
