The ultimate goal is to ensure optimal prevention and care for work-related pulmonary diseases through collaboration among specialists and the registration of patient data in a national database. Abstract

This article discusses the clinical efficacy of tezepelumab, a monoclonal antibody, in non-type 2 (T2) asthma patients. Tezepelumab targets thymic stromal lymphopoietin (TLSP), a biomarker associated with airway inflammation in non-T2 asthma. Previous research has shown that tezepelumab can improve lung function, asthma control, and quality of life in uncontrolled asthma patients with low baseline blood eosinophil counts. This case series examines the effects of tezepelumab in pre-defined non-T2 asthma patients, demonstrating improvements in lung function, asthma control, and quality of life in the majority of patients. Abstract

Background: Asthma is a common disease characterized by chronic inflammation of the lower airways, bronchial hyperactivity, and (reversible) airway obstruction. The Global Initiative of Asthma Guideline recommends a flowchart to diagnose asthma with first-step spirometry with reversibility and a bronchial challenge test (BPT) with histamine or methacholine as a second step [1]. The BPT is considered burdensome, time-consuming for patients and staff, can cause side effects, and is expensive. In addition, this test strongly encumbers lung function capacity. Elevated Nitric Oxide (NO) is associated with airway eosinophilic inflammation in asthma patients and can be measured in exhaled air with the Fractional exhaled (Fe) NO-test. This low-burden FeNO-test could be used as an ‘add-on’ test in asthma diagnostics [2, 3]. Methods and analysis: This multi-center prospective study (Trial number: NCT06230458) compares the ‘standard asthma diagnostic work-up’ (spirometry with reversibility and BPT) to the ‘new asthma diagnostics work-up’ (FeNO-test as an intermediate step between the spirometry with reversibility and the BPT), intending to determine the impact of the FeNO-based strategy, in terms of the number of avoided BPTs, cost-effectiveness and reduced burden to the patient and health care. The cost reduction of incorporating the FeNO-test in the new diagnostic algorithm will be established by the number of theoretically avoided BPT. The decrease in burden will be studied by calculating differences in the Visual Analogue Scale (VAS) -score and Asthma Quality of Life Questionnaire (AQLQ) -score after the BPT and FeNO-test with an independent T-test. The accuracy of the FeNO-test will be calculated by comparing the FeNO-test outcomes to the (gold standard) BPTs outcomes in terms of sensitivity and specificity. Abstract

Purpose: Rotational malalignment (≥ 10°) is a frequent pitfall of intramedullary-nailing of tibial shaft fractures. This study aimed to develop an intraoperative fluoroscopy protocol, coined 'C-Arm Rotational View (CARV)', to significantly reduce the risk for rotational malalignment and to test its clinical feasibility. Methods: A cadaver and clinical feasibility study was conducted to develop the CARV-technique, that included a standardized intraoperative fluoroscopy sequence of predefined landmarks on the uninjured and injured leg in which the rotation of the C-arm was used to verify for rotational alignment. A mid-shaft tibia fracture was simulated in a cadaver and an unlocked intramedullary-nail was inserted. Random degrees of rotational malalignment were applied using a hand-held goniometer via reference wires at the fracture site. Ten surgeons, blinded for the applied rotation, performed rotational corrections according to (1) current clinical practice after single-leg and dual-leg draping, and (2) according to the CARV-protocol. The primary outcome measure was the accuracy of the corrections relative to neutral tibial alignment. The CARV-protocol was tested in a small clinical cohort. Results: In total, 180 rotational corrections were performed by 10 surgeons. Correction according to clinical practice using single-leg and dual-leg draping resulted in a median difference of, respectively, 10.0° (IQR 5.0°) and 10.0° (IQR 5.0°) relative to neutral alignment. Single-leg and dual-leg draping resulted in malalignment (≥10°) in, respectively, 67% and 58% of the corrections. Standardized correction using the CARV resulted in a median difference of 5.0° (IQR 5.0°) relative to neutral alignment, with only 12% categorized as malalignment (≥10°). The incidence of rotational malalignment after application of the CARV decreased from 67% and 58% to 12% (p = <0.001). Both consultants and residents successfully applied the CARV-protocol. Finally, three clinical patients with a tibial shaft fracture were treated according to the CARV-protocol, resulting all in acceptable alignment (<10°) based on postoperative CT-measurements. Conclusion: This study introduces an easy-to-use and clinically feasible standardized intraoperative fluoroscopy protocol coined 'C-arm rotational view (CARV)' to minimize the risk for rotational malalignment following intramedullary-nailing of tibial shaft fractures. Abstract

Introduction: Severe asthma is associated with a serious disease burden, partially caused by limitations in activity and work impairment. Aims and objectives: This study aims to relate treatment with biologics targeting IL-5/5Ra to work productivity and activity in the long term in a real-world context. Material and methods: This is a registry-based multi-center cohort study evaluating data from adults with severe eosinophilic asthma included in the Dutch Register of Adult Patients with Severe Asthma for Optimal DIsease management (RAPSODI). Patients that started with anti-IL-5/5Ra biologics and completed the work productivity and activity improvement questionnaire, were included. Study and patient characteristics were compared between the employed and unemployed patients. Work productivity and activity impairment are related to accompanying improvements in clinical outcomes. Results: At baseline, 91 of 137 patients (66%) were employed which remained stable throughout the follow-up period. Patients in the working age category were younger and had significantly better asthma control (p = 0.02). Mean overall work impairment due to health decreased significantly from 25.5% (SD2.6) to 17.6% (SD 2.8) during 12 months anti-IL-5/5Ra biologics treatment (P = 0.010). There was a significant association between ACQ6 and overall work improvement after targeted therapy (β = 8.7, CI 2.1–15.4, P = 0.01). The improvement of asthma control of 0.5 points on the asthma Control Questionnaire was associated with an overall work impairment of −9%. Conclusions: Work productivity and activity in severe eosinophilic asthma improved after starting anti-IL-5/5Ra biologics. Clinically relevant improvement in asthma control was associated with an overall work impairment score of −9% in this study.

Hierbij worden in achtereenvolgende fases de trillingseffecten van de Voorkeursalternatief (VKA), Voorkeursoplossing en het DO+/uitvoeringsontwerp van de dijkversterking in beeld gebracht. Deze berekeningen en analyses zijn er op gericht om een goede risico inschatting voor de geplande uitvoeringswerkzaamheden te kunnen maken.

21. NNN-compensatieplan dijkversterking KIJK : Krachtige IJsseldijken Krimpenerwaard

Abstract

Dit is het compensatieplan Natuurnetwerk Nederland van de dijkversterking van 10,5 km dijk aan de oostzijde van de Hollandsche IJssel tussen Krimpen aan den IJssel, Ouderkerk aan den IJssel en Gouderak. De dijkversterking wordt aangepakt in het project Krachtige IJsseldijken Krimpenerwaard (KIJK). De dijk langs de Hollandsche IJssel voldoet niet aan de norm. De dijk biedt op veel plekken onvoldoende stabiliteit en is ook niet hoog genoeg. Hoogheemraadschap van Schieland en de Krimpenerwaard (HHSK) wil daarom de meest urgente 10,5 km dijk tussen Krimpen aan den IJssel, Ouderkerk aan den IJssel en Gouderak versterken om de kans op overstroming terug te brengen tot het wettelijke beschermingsniveau. Voor de dijkversterking wordt een Projectplan Waterwet vastgesteld waarvoor ook een MER is opgesteld. Dit compensatieplan is als bijlage opgenomen bij het Projectplan Waterwet. De compensatieopgave als gevolg van de dijkversterking is in het MER-traject in eerste instantie bepaald aan de hand van het voorlopig ontwerp van de dijkversterking.

Abstract

Assyrian imperialism is closely associated with the practice of mass deportation. This practice has been explained by recourse to many different motivations. But can we hope to pinpoint the logic informing deportation rather than merely identifying its advantages? This paper surveys the evidence of deportation in the Levant in the period 745–620 b.c.e. Focusing on deportation in this circumscribed time and place enables a more concentrated account of its use. Deportation is generally argued to have served three broad ends: bolstering the supply of human resources in the Assyrian heartland, meeting particular strategic needs, and dealing with dissent. This paper finds that despite the many uses of deportation, it was first and foremost a punitive instrument intended to curb resistance to Assyrian hegemony. This punitive dimension constituted the foundation of Assyrian deportation in the Levant in the age of Assyrian hegemony.

Abstract

Background Coronavirus disease 2019 is a serious respiratory virus pandemic. Patient characteristics, knowledge of the COVID-19 disease, risk behaviour and mental state will differ between individuals. The primary aim of this study was to investigate these variables in patients visiting an emergency department in the Netherlands during the COVID-19 pandemic and to compare the “COVID-19 suspected” (positive and negative tested group) with the “COVID-19 not suspected” (control group) and to compare in the “COVID-19 suspected” group, the positive and negative tested patients. Methods Consecutive adult patients, visiting the emergency room at the Franciscus Gasthuis & Vlietland, Rotterdam, the Netherlands, were asked to fill out questionnaires on the abovementioned items on an iPad. The patients were either “COVID-19 suspected” (positive and negative tested group) or “COVID-19 not suspected” (control group). Results This study included a total of 159 patients, 33 (21%) tested positive, 85 (53%) negative and 41 (26%) were COVID-19 not suspected (control group). All patients in this study were generally aware of transmission risks and virulence and adhered to the non-pharmaceutical interventions. Working as a health care professional was correlated to a higher risk of SARS-Cov-2 infection (p- value 0.04). COVID-19 suspected patients had a significantly higher level of anxiety compared to COVID-19 not suspected patients (p-value < 0.001). The higher the anxiety, the more seriously hygiene measures were followed. The anxiety scores of the patients with (pulmonary) comorbidities were significantly higher than without comorbidities. Conclusion This is one of the first (large) study that investigates and compares patient characteristics, knowledge, behaviour, illness perception, and mental state with respect to COVID-19 of patients visiting the emergency room, subdivided as being suspected of having COVID-19 (positive or negative tested) and a control group not suspected of having COVID-19. All patients in this study were generally aware of transmission risks and virulence and adhered to the non-pharmaceutical interventions. COVID-19 suspected patients and patients with (pulmonary) comorbidities were significantly more anxious. However, there is no mass hysteria regarding COVID-19. The higher the degree of fear, the more carefully hygiene measures were observed. Knowledge about the coping of the population during the COVID-19 pandemic is very important, certainly also in the perspective of a possible second outbreak of COVID-19.

Item does not contain fulltext, BACKGROUND: E-bike usage is increasingly popular and concerns about e-bike-related injuries and safety have risen as more injured e-bikers attend the emergency department (ED). Traumatic brain injury (TBI) is the main cause of severe morbidity and mortality in bicycle-related accidents. This study compares the frequency and severity of TBI after an accident with an e-bike or classic bicycle among patients treated in the ED. METHODS: This was a prospective cohort study of patients with bicycle-related injuries attending the ED of a level 1 trauma centre in the Netherlands between June 2016 and May 2017. The primary outcomes were frequency and severity of TBI (defined by the Abbreviated Injury Scale head score >/=1). Injury Severity Score, surgical intervention, hospitalisation and 30-day mortality were secondary outcomes. Independent risk factors for TBI were identified with multiple logistic regression. RESULTS: We included 834 patients, of whom there were 379 e-bike and 455 classic bicycle users. The frequency of TBI was not significantly different between the e-bike and classic bicycle group (respectively, n=56, 15% vs n=73, 16%; p=0.61). After adjusting for age, gender, velocity, anticoagulation use and alcohol intoxication the OR for TBI with an e-bike compared with classic bicycle was 0.90 (95% CI 0.56 to 1.45). Independent of type of bicycle, TBI was more likely if velocity was 26-45 km/hour, OR 8.14 (95% CI 2.36 to 28.08), the patient was highly alcohol intoxicated, OR 7.02 (95% CI 2.88 to 17.08) or used anticoagulants, OR 2.18 (95% CI 1.20 to 3.97). TBI severity was similar in both groups (p=0.65): eight e-bike and seven classic bicycle accident victims had serious TBI. CONCLUSION: The frequency and severity of TBI among patients treated for bicycle-related injuries at our ED was similar for e-bike and classic bicycle users. Abstract

De IJsselmeerdijk beschermt de diepe Flevopolder tegen het water van het IJsselmeer. In 2018 heeft Waterschap Zuiderzeeland (hierna afgekort als: Zuiderzeeland) beoordeeld of de IJsselmeerdijk zo sterk is als de waterveiligheidsnormen voorschrijven. Dat blijkt niet zo te zijn. Sinds 2017 gelden voor de waterkeringen in Nederland nieuwe wettelijke waterveiligheidsnormen. Deze norm is voor Flevoland strenger dan daarvoor om in te spelen op de gevolgen van klimaatverandering en om de grotere hoeveelheid inwoners en de hogere economische waarde in Flevoland beter te beschermen. De waterkering voldoet ruim niet aan de nieuwe strengere norm die eraan gesteld is. Dat wil niet zeggen dat er op dit moment acuut een onveilige situatie is. Het betekent wel dat een dijkversterking nodig is. Het is de wettelijke taak van het waterschap om de keringen aan de normen te laten voldoen. Zuiderzeeland is daarom in 2019 gestart met dit meerjarige project Versterking IJsselmeerdijk.
