AimsHypertrophic cardiomyopathies (HCM) are caused in 30–60% of cases by mutations in cardiac sarcomere genes but can also be an expression of cardiac involvement in multi-systemic metabolic diseases, such as Anderson–Fabry disease (AFD). HCM entails a risk of sudden cardiac death (SCD) of 0.9%/year and is the most common cause of SCD in young adults. Recent studies suggested mechanical dispersion (MD) by speckle tracking echocardiography (STE) as an additional arrhythmic risk marker. The aim of the study was to evaluate left ventricle global longitudinal strain (LV-GLS) and MD, in patients with HCM or AFD cardiomyopathy, and the association with ventricular arrhythmias (V-AR).Methods and resultsWe evaluated 40 patients with HCM, 57 with AFD (12 with LV hypertrophy and 45 without), and 40 healthy subjects, between January 2014 and June 2022. We performed a comprehensive echocardiographic study and analysed systolic and diastolic functions, LV-GLS, and MD. We also analysed V-AR, including ventricular fibrillation and sustained/non-sustained ventricular tachycardia, by Holter electrocardiogram (Holter-EKG), in a subset of hypertrophic patients. Data were analysed by unpaired Student t-test or chi-square/Fisher’s exact test as appropriate and binary logistic regression (SPSS Statistics ver.26). LV-GLS was significantly lower in the V-AR group compared with patients without V-AR (median −10.2% vs. −14%, P = 0.038); MD was significantly higher in the V-AR group (85.5 ms vs. 61.1 ms, P = 0.004). V-AR were found significantly associated with MD (OR, 1.030; 95% CI, 1.003–1.058; P = 0.03).ConclusionsMD is a useful additional index in the evaluation of patients with HCM and may be a promising prognostic predictor of increased arrhythmic risk.

Background Hypertrophic Cardiomyopathy (HCM) is burdened by sudden cardiac death (SCD) risk of 0.9%/year, and is the most common cause of SCD in young adults. It is an autosomal dominant inherited disease caused by mutations in cardiac sarcomere genes, but the hypertrophic phenotype can also be an expression of cardiac involvement in multiorgan metabolic storage diseases, such as Anderson-Fabry disease (AFD). Mechanical Dispersion (MD) by Speckle-Tracking Echocardiography (STE) has recently emerged as an additional arrhythmic risk marker. Purpose Aim of the study was to evaluate LV systolic and diastolic function, global longitudinal strain (GLS) and MD by STE and analyze their association with ventricular arrhythmias in patients with HCM and AFD. Methods We included in our analysis 36 patients with HCM, 54 with AFD, of which 10 with left ventricular hypertrophy (AFD-LVH) and 44 without (AFD-N), and 27 healthy subjects. We performed a comprehensive basic echocardiographic study and analyzed GLS and MD (post-processing through EchoPAC 2.02). We also evaluated ventricular arrhythmias (V-AR), including ventricular fibrillation and sustained and non-sustained ventricular tachycardia, by Holter ECG, and the data obtained by cardiac magnetic resonance (CMR) in hypertrophic patients. Data were analyzed by unpaired Student t-test or chi-square/Fisher's exact test as appropriate, and binary logistic regression (SPSS Statistics ver.26). Results Diastolic function was impaired in HCM and AFD-LVH patients compared to control and AFD without LVH. GLS was significantly lower in the V-AR group compared to patients without V-AR (9.7±2.9 vs 14.1±4, P=0.007), MD was significantly higher in the V-AR group (111±47 vs 68.1±16, P=0.03). We exhibit the case of a 75 years-old man who undertook cardio-oncological assessment for Non-Hodgkin Lymphoma: baseline echo revealed typical features of lipomatous hypertrophy of the interatrial septum (LHIS) and an apical hypoechoic left ventricle (LV) lesion. Five years before, the patient performed MRI (Magnetic Resonance Imaging), which confirmed LHIS, but superabundant epicardial fat extended to the right ventricle and the apical junction between the two ventricles, both characterized by normal ejection fraction, and surrounded the origin of the pulmonary trunk. LV lateral and inferior walls presented lipomatous metaplasia foci and no history or ECG sign of previous myocardial injury could be pointed out. A slight band of late gadolinium enhancement at the basal infero-lateral wall of LV was highlighted. The patient referred episodes of fainting: the Holter-ECG showed pronounced bradycardia which couldn't be explained by other secondary causes. So a bicameral pace-maker was implanted. During our ambulatory evaluation, we decided to repeat MRI, which showed no difference in comparison with the previous one, so excluding malignant degeneration of the fatty mass surrounding the heart. Despite older age and the lack of sudden cardiac death family history we searched for pathognomonic arrhythmogenic cardiomyopathy (ACM) mutations. In consideration of the new evidence of the metabolic risk associated with abundant epicardial fat (EF) we asked for updated laboratory dosage of plasma glucose, LDL cholesterol and triglycerides and we recommended stricter control of blood pressure. From 4% to 52% of cardiac mass is made up of fat but only EF interacts with myocardium and coronary arteries. Anomalous growth of fat in the heart can present itself as LHIS, excessive lipomatous infiltration or cardiac lipomas (real capsulated tumors) but also ACM is histologically characterized by fibrofatty replacement of cardiomyocites. Overabundant and inflammed EF is now considered a surrogate of metabolic risk and is associated with multivessel coronary disease, the finding of chronic total occlusions (CTO) and microvascular angina. Identifying fatty heart lesions and defining their clinical and prognostic role remains an intriguing challenge. We suggest a general binary way to follow: the “mechanic one” for lipomas or lipomatous infiltration, that is paying attention to compression phenomena or the generation of arrhythmias, and the “metabolic one”, in other words optimizing cardiovascular prevention when EF is too much. A careful research of imaging features of ACM can't be omitted.

The aim of the study was to evaluate the application of global longitudinal strain (GLS) and myocardial work (MW) at rest and during exercise in healthy sedentary or trained participants, to test their ability to improve echocardiographic information and to complement prescribing exercise, cardiac screening, or rehabilitation programs.Thirty healthy males were divided into three groups of 10, sedentary (G1), resistance (G2) and power (G3) athletes, underwent a standard clinical evaluation protocol and exercise stress testing echocardiography.During stress, all showed increased left ventricular ejection fraction and mitral annulus tissue Doppler (E'). G1 showed a decrease in left atrial volume (LAVi) as opposed to an increase in G3. E/E 'a decrease in G2, unlike the increase in G3. All groups showed increase of Strain (GLS average AV, Longitudinal LS, Medio-Basal MB Apical AP), global constructive work (GCW), and Global wasted work. G1 showed increase for global work efficiency, G2 and G3 for global work index (GWI). G3 showed a greater variation of E/E', LAVi, GWI and GCW compared to G1 and G2, greater of GLS AV, LS-AP compared to G2. Only G3 showed differences for GLS AV versus LS-AP. The relative regional strain ratio showed a greater value in G3 versus G1 at the end of stress compared to rest.The new echocardiographic applications to study the physiological adaptation could open new perspectives for the diagnostic and therapeutic development through the prescription of personalized exercises and screening and follow-up of the early pathological changes of the athlete's heart.

In rocky shore systems, sessile macrobenthic assemblages may act as “ecosystem engineers” for many smaller benthic organisms. Thus, the influence of macrobenthic coverage on the diversity and assemblage structure of the harpacticoid copepod fauna was investigated in the rocky shores of a Marine Protect Area (MPA) in the Ligurian Sea (NW, Mediterranean Sea). Two sampling sites were investigated in two seasons at three different depths on both sub-vertical and inclined reefs. A total of 61 species of copepods mainly represented by Miraciidae, Laophontidae, Longipediidae and Thalestridae were found. The complex micro-topography of these substrata provided a wide variety of niches for many species with different lifestyles that suggests the important role of rocky shores to ensure the functioning of coastal ecosystems. The harpacticoid assemblage structure seemed mainly influenced by season and depth. The temporal spread observed is likely one of the underlying mechanisms of niche segregation that allows many species to co-occur in this specific environment along with a subordinate spatial segregation corresponding to the depth gradient. The results seem to support the hypothesis that the different species composition of the “ecosystem engineer” (and consequently its structure changes) are relevant in structuring the copepod assemblages. The comparison with previous data on general meiofauna underlines that higher surrogacy of the taxonomic identification could be used to study rocky shore communities, but the rich diversity that these systems host can only be understood at the lower taxonomic levels. The same holds for future evaluations of impact of environmental changes (including MPA regulations) on meiofaunal assemblages.

Numerous anthropogenic activities take place within or nearby ports, thereby increasing environmental pressure in coastal zone that become the final destination of many environmental contaminants. Thus, port areas are crucial to understand the possible long-term effects of contaminants on metazoans that are at the base of the food webs. The port of Vado Ligure is the outlet to the sea of one of the largest industrial areas of the West Liguria (NW Mediterranean). Sediment quality was evaluated in twenty-eight stations at increasing distance from the port. The nematode fauna was characterised (i.e., assemblage composition, taxonomic diversity and functional traits) and organic matter quantity and composition and contaminant concentrations (heavy-metals, polycyclic aromatic hydrocarbons and heavy, long-chain hydrocarbons) were measured. The area resulted mainly polluted by mercury, followed by arsenic, zinc, and polycyclic aromatic hydrocarbons. The faunal distribution (both from a taxonomical and functional point of view) appeared influenced by the contamination, as well as by sediment granulometry and organic matter content and composition. According to nematode assemblages and environmental variables, two groups of stations showed the most marked differences: one located in front of the Vado Ligure port, characterised by the most contaminated stations, the highest organic matter content, and fine sediments, and the other one, close to Bergeggi Marine Protected Area, characterised by more pristine stations, low organic matter content and coarser sediments. A suite of nematode genera that may be considered as bioindicators of pollution and organic enrichment was selected. Maturity index (MI) resulted negatively affected by contamination, suggesting that the different proportion of the nematode life-strategies is useful in assessing the environmental quality of sediments. The ecological quality status of the sampled stations obtained using abiotic and biotic descriptors (i.e., contamination level, organic matter content and nematode diversity and maturity index) was compared. Both the abiotic and nematode variables showed a general consistency for the stations of the two groups characterised by the highest and the lowest levels of contamination and organic enrichment.

Myocardial dysfunction are the most concerning cardiovascular complications of cancer therapies with a poor prognosis, so it’s critical to detect subclinical cardiac abnormalities in order to start cardioprotective therapy early or increased surveillance frequency. Global longitudinal strain (GLS) by echocardiography is an excellent tool for assessing regional and global left ventricular (LV) function. Mechanical dispersion (MD) reflects heterogeneous myocardial contraction, evaluated in many cardiopathies. We evaluated subclinical myocardial dysfunction by GLS and MD using 2D Speckle-tracking Echo, in order to established if MD could be a predictor of ventricular dysfunction in the field of Cardiotoxicity (CTX). Were enrolled 42 women with breast cancer chemotherapy-treated and underwent to Echo evaluation during 3- and 6-months follow-up, compared to evaluation performed before starting chemotherapy (T0). Depending on chemotherapy type were identified 2 groups: Anthracyclines ± Taxol treated (group 1) and Anti-HER2 treated (group 2). CTX diagnosis was made according ESC criteria: LVEF 10% or GLS decrease >15% compared to previous check. At three months, 28% patients (p We believe that MD is a predictor of ventricular dysfunction earlier than GLS during Anti-HER2 treatment, so in this field MD could integrates information obtained from GLS about subclinical dysfunction.

Anderson-Fabry disease (AFD) is a rare X-linked metabolic disorder due to deficiency in lysosomal enzyme activity of a-galactosidase A, resulting in pathological accumulation of glycosphingolipids in several tissues and a progressive multi-organ dysfunction. The Global Longitudinal Strain (GLS) of the left ventricle (LV) by Speckle-tracking echocardiography (STE) has been show to detect subclinical cardiac involvement in many cardiomyopathies. The Mechanical Dispersion (MD), derived from STE is considered able to reflect a heterogeneous myocardial contraction, evaluated in many cardiopathies. A reduction in GLS was associated with myocardial fibrosis in the subclinical stages of AFD cardiomyopathy, but there is no MD data in AFD. The aim of the study was to evaluate the distribution of GLS and MD of LV in patients with AFD. 47 consecutive AFD adult patients treated (37F, 12 M, age 45 + 17 years) were examined by a complete echocardiographic examination from a Vivid E95 ultrasound scanner (Horten, Norway). The STE post-processing was performed by Echopac 2.02, in apical long-axis, 4-chambers, and 2-chambers views to determine GLS and MD. Data were expressed as mean± standard deviation. The comparison between the AFD group and 20 normal subjects (N) was performed with unpaired T-test. Unlike the soft bottom meiofauna, meiofauna associated to hard substrata is poorly studied, despite its ecological relevance. Since communities of hard substrata are usually characterized by species with different life cycles and strategies from those of soft bottom assemblages, information on hard substrata meiofauna is still needed. In this study, sessile macrobenthos and the associated meiofaunal assemblages of two sites of Portofino (NW Mediterranean) were investigated in two seasons at three different depths on both sub-vertical and inclined reefs. The study aimed to assess the abundance, diversity and composition of the meiofauna and the factors structuring its assemblages. Moreover, as meiofauna is known to be dependent upon the substrate characteristics, the study investigated whether the meiofaunal patterns could be related to the sessile macrobenthos structure and composition, and to which extent. Macroalgae dominated the sessile macrobenthic assemblages, while Nematoda and Copepoda were the main meiofaunal groups. Meiofaunal higher-taxa richness and diversity resulted very high, due to the large number of different microhabitats offered by macroalgae. Macrobenthic assemblages were dominated by Rodophyta and Ochrophyta in summer, the latter dramatically collapsing in winter. The meiofaunal abundance and composition changed significantly with the season, consistently with the sessile macrobenthic assemblages, and resulted strongly correlated with Ochrophyta. Shaping the meiofaunal assemblages, macroalgae appeared to act as ecosystem engineer for the meiofauna.

Background Twenty-four-hour blood pressure (BP) variability is an important predictor of organ damage and cardiovascular events. Although epidemiological data are widely based on evaluation of office (clinic) BP, 24-hour ambulatory BP monitoring (ABPM) accurately assess the severity of hypertension to predict cardiovascular events in hypertensive patients, because it more accurately reflects BP load on heart and blood vessels. Conventional transthoracic echocardiography (TTE), such as M-mode and two-dimensional (2D), and more advanced techniques, such as tissue Doppler imaging (TDI) and speckle tracking echocardiography (STE), are used to identify pathological changes of the hypertensive heart disease. In addition, the study of systemic arterial compliance (SAC) predicts the impact of the arterial stiffness on the LV remodeling. Methods Fifty-eight patients (34 males and 24 females, aged 53±12 years) with hypertension for at least one year were studied using Ambulatory Blood Pressure Monitoring (ABPM), with evaluation of 24-hour, day time, night time and sleep and awake average systolic and diastolic BP, and subsequently with TTE, to evaluate the influence of the 24-hour average BP, integrated with echocardiographic parameters, on cardiovascular adaptations in hypertensive patients. ECHO parameters examined were: left ventricle (LV) mass indexed to body surface area (LVMi), LV ejection fraction (EF), left atrial volume indexed to body surface area (LAVi), mitral inflow velocities (E, A and E/A), mitral annulus velocities (S' and E') and E/E' ratio by TDI, LV global longitudinal strain (GLS) using 2D STE. SAC was derived by the ratio SVi/PP, using echocardiographic stroke volume index to body surface area (SVi) and pulse pressure (PP). Results LVMi showed a correlation with the 24-hour average BP (SBP r=0.32; DBP r=0.26), SBPd (r=0.32), DBPd (r=0.28) and SBPn (r=0.29). GLS was correlated with the 24-hour average BP (SBP r=0.30; DBP r=0.32), SBPd (r=0.32), DBPd (r=0.32) and with LVMi (r=0.42), LAVi with 24 hour SBP (r=0.28), while SAC with LVMi (r=0.29) and LAVi (r=0.33). Conclusions ABPM and TTE evaluation can be particularly useful in hypertensive patients, even with normal office BP, to highlight the influence of 24-hour blood pressure profile on LVM, and to a lesser extent, on LAV. SAC, instead, seems to be correlated both to LVM and LAV, independently of BP levels.

The use of free-living marine nematodes as ecological indicators (EcoInds) of human impacts has increased greatly in Italy since 1990. This paper is a summary of the Italian research experience in the study of nematode assemblages of shallow water habitats, and provides a breakdown of the most important insights that have been obtained so far. Although nematodes are among the best candidates for the Ecological Quality (EcoQ) assessment in the benthic domain, many guidelines need to be developed and limits overcome. Italian research has certainly contributed to the achievement of this purpose with highly focused local investigations on the effects of specific stressors (riverine and sewage discharge, aquaculture, trace elements and hydrocarbons), but also to a large extent with wider analyses aimed at finding new and valuable tools for monitoring programs and useful nematode descriptors, in line with the European Directives. Currently, the Italian and foreign experiences in this field draw light to the fact that the best nematode descriptors for the EcoQ assessment are the taxonomic composition and life strategy traits. However, nematode worldwide research is running the risk of being marginalized because of the relatively narrow scope of most contemporary studies. Nematode researchers should work in a nematologist community in order to better promote the use of nematodes such as EcoInds in the era of the Water Framework Directive (WFD) and Marine Strategy Framework Directive (MSFD). In the present paper, possible steps to obtain this goal are brought to the reader’s attention and discussed.

Marine biologists have progressively increased their consciousness of the importance of meiofauna for the benthic domain in both temperate and tropical regions. After the 1998 bleaching, Maldivian reefs (Indian Ocean) have been regarded as a vulnerable ecosystem that must be carefully monitored. Accordingly, an extensive investigation of meiofaunal distribution in the reef slopes of the Maldivian archipelago has been carried out, taking into account geographical position, type of habitat (inner vs. outer slope), inclination and depth gradient. Twenty-four taxa revealed the highest meiofaunal richness ever found in Maldivian reefs. Interestingly, Thermosbenacea and Syncarida were identified, which are two taxa that have only recently been documented in the marine ecosystem. Chaetognatha were also present, which is a group that was only considered to be planktonic until 2000, when they were also discovered in the benthos. The type of habitat, affected by different hydrodynamic conditions, was the main factor influencing the meiofaunal community's structure and diversity. In detail, the outer reefs were characterized by the highest level of diversity, confirming previous observations on the rate of coral reef growth and vitality and underlining the greater vulnerability of the inner slopes. In contrast, depth only significantly affected the community structure, but not its density or diversity. Accordingly, community structure seems to be more sensitive than abundance and diversity indices when it comes to detecting depth gradients. The 10° inclination of the inner slopes revealed the most different community structure and the greatest dominance of nematodes, leading to the lowest diversity levels.

Valvular heart diseases (VHD) may be observed in patients with cancer for several reasons, including preexisting valve lesions, radiotherapy, infective endocarditis, and secondary to the left ventricle dysfunction. The incidence of VHD is especially in younger survivors treated with thoracic radiation therapy for certain malignancies, such as Hodgkin's lymphoma and breast cancer. The mechanism of radiation-induced damage to heart valves is not clear and includes diffuse fibrocalcific thickening of the valve. VHD is commonly diagnosed after a long latent period, in the context of clinical symptoms, or suspected on the basis of a new murmur. The evaluation includes identification of anatomical valve abnormalities, valve dysfunction, and assessing the functional consequences of valve dysfunction on the ventricles. Echocardiography is the optimal imaging technique for diagnostic and therapeutic management. Cardiovascular magnetic resonance and computed tomography (CT) may be used to assess the severity of VHD, but cardiac CT is mainly useful for detecting extensive calcifications of the ascending aorta. Patients exposed to mediastinal radiotherapy and minimal valve dysfunction require follow-up of 2–3 years, with moderate valve disease yearly, with severe, should be assessed for valve surgery.

The Water Framework Directive (WFD) strengthened the need for the protection and enhancement of Ecological Quality Status (EQS) in water bodies of EU Member States. However, in terms of the EQS assessment, there is a lack of detailed studies that assess the consequences of dredging, especially concerning macrobenthic communities. Therefore, the main objective of this study was to evaluate the EQS assessment, as important need to WFD, based on diversity and ecological quality indices after dredging activity. Data was collected in Mondego estuary, Portugal, between October 2007 and November 2008, within and around the area to be dredged (3 ha): one dredging occasion and ten sampling dates along one year. Physical–chemical changes were examined. The relative performance of Margalef, Shannon–Wienner, AMBI–AZTI Marine Biotic Index and the multimetric BAT –Benthic Assessment Tool (BAT) were analyzed before and after dredging. In the first days, dredging increased the share of fine and coarse sand sediment and had changed TSS, POM and SOM, affecting the macrofauna assemblages. Two months after the dredging, the number of taxa, density, diversity and ecological indices showed similar values to pre-dredging conditions and one year later the number of species and density values almost duplicated. However, the macrofauna composition and structure were still significantly different in relation to pre-dredging and the biomass values did not return to the original values. Nevertheless, BAT was sensitive to the changes that occurred in the area, showing that the system recovered to pre-dredging values after one year, in terms of EQS. The present work reinforced the robustness of BAT as an accurate ecological quality assessment tool.

Meiofauna in sediments colonized by Posidonia oceanica in the Ligurian Sea (NW Mediterranean) was investigated to evaluate its use as an early warning indicator of environmental disturbance. Descriptors commonly used in seagrass health assessment are mainly related to the plant (e.g., phenological parameters) and/or to the meadow (e.g., structural parameters) and are subjected to long-term response times. Conversely, meiofauna is considered a good bioindicator with a rapid response to environmental variations, due to its main features (e.g., short life cycle, scarce mobility, presence/absence of tolerant/sensitive species). Meiofaunal community, sediment characteristics and organic matter descriptors in three meadows located in urbanized coastal areas were contrasted with those observed in three meadows located within Marine Protected Areas. Samplings were carried out in two distinct periods, at the beginning and at the end of the summer season, in order to individuate early changes in the meiofaunal assemblages. The meiofauna and, particularly, the nematode assemblage composition and diversity, highlighted differences among meadows at the end of the summer not detectable by organic matter descriptors and structural parameters of the meadow (e.g. shoot density). Nematodes are, thus, proposed as appropriate biological quality elements (BQEs) able to show early responses to environmental disturbance.

Nematode biomass and allometric attributes (size spectra, body length and width, morphotypes) were measured and related to the environmental quality of sediments of the Genoa-Voltri commercial harbour (Ligurian Sea, NW Mediterranean) to investigate their possible use as ecological indicators. The sediment quality was defined by measuring the level of organic enrichment (quantity and biochemical composition of sedimentary organic matter) and oxygen stress (redox potential). Nematode biomass spectra (NBS) proved to be extremely valuable in determining differences in the environmental quality of sediments on a hundred-metre spatial scale. High peaks of the NBS were observed in the more organic-rich and oxygen-stressed stations probably in relation to a lower diversity of the nematode communities in these stations, with the predominance of tolerant genera such as Paracomesoma and Sabatieria. Among allometric variables, the length was found to be negatively correlated with oxygen concentrations and positively with TOM percentages, whilst the morphotype length/width ratio (L/W) resulted negatively related to oxygen concentrations and organic matter quality (protein:carbohydrate ratio), suggesting that these allometric attributes represent indicators of the functional adaptation of nematodes to the changing environmental conditions. We suggest that in contrast to time-consuming and expertise-requiring nematode taxonomic analysis, biomass and allometric attributes analysis can provide a simpler but comparable tool to assess sediment quality and environmental heterogeneity of harbour ecosystems.

The spatial distribution and structure of nematode assemblages in the area surrounding the harbour of Vado Ligure (Savona, NW Mediterranean) were studied in relation to the influence of natural and anthropogenic environmental factors. Stations were selected following an “anthropogenic gradient” from sites located near the city centre and its harbour to more pristine and distant sites. Sediment quality was determined by considering both sediment granulometric and chemical parameters (hydrocarbons, heavy metals, total organic matter, proteins, carbohydrates) as well as nematode abundance, diversity, life strategies, trophic structure and assemblage composition. A high correlation between environmental characteristics and the nematode response was found. On the basis of the comparison of these results, which identified three distinct sub-areas associated with different levels of environmental quality, a set of nematode indicator genera was selected for the future evaluation of quality status.
