Abstract

Summary: Background: Surgical care holds significant importance in healthcare, especially in low and middle-income countries, as at least 50% of the 4.2 million deaths within the initial 30 days following surgery take place in these countries. The Lancet Commission on Global Surgery proposed six indicators to enhance surgical care. In Colombia, studies have been made using secondary data. However, strategies to reduce perioperative mortality have not been implemented. This study aims to describe the fourth indicator, perioperative mortality rate (POMR), with primary data in Colombia. Methods: A multicentre prospective cohort study was conducted across 54 centres (hospitals) in Colombia. Each centre selected a 7-day recruitment period between 05/2022 and 01/2023. Inclusion criteria involved patients over 18 years of age undergoing surgical procedures in operating rooms. Data quality was ensured through a verification guideline and statistical analysis using mixed-effects multilevel modelling with a case mix analysis of mortality by procedure-related, patient-related, and hospital-related conditions. Findings: 3807 patients were included with a median age of 48 (IQR 32–64), 80.3% were classified as ASA I or II, and 27% of the procedures had a low-surgical complexity. Leading procedures were Orthopedics (19.2%) and Gynaecology/Obstetrics (17.7%). According to the Clavien–Dindo scale, postoperative complications were distributed in major complications (11.7%, 10.68–12.76) and any complication (31.6%, 30.09–33.07). POMR stood at 1.9% (1.48–2.37), with elective and emergency surgery mortalities at 0.7% (0.40–1.23) and 3% (2.3–3.89) respectively. Interpretation: The POMR was higher than the ratio reported in previous national studies, even when patients had a low–risk profile and low-complexity procedures. The present research represents significant public health progress with valuable insights for national decision-makers to improve the quality of surgical care. Abstract

Microfluidics is an interdisciplinary field that encompasses both science and engineering, which aims to design and fabricate devices capable of manipulating extremely low volumes of fluids on a microscale level. The central objective of microfluidics is to provide high precision and accuracy while using minimal reagents and equipment. The benefits of this approach include greater control over experimental conditions, faster analysis, and improved experimental reproducibility. Microfluidic devices, also known as labs-on-a-chip (LOCs), have emerged as potential instruments for optimizing operations and decreasing costs in various of industries, including pharmaceutical, medical, food, and cosmetics. However, the high price of conventional prototypes for LOCs devices, generated in clean room facilities, has increased the demand for inexpensive alternatives. Polymers, paper, and hydrogels are some of the materials that can be utilized to create the inexpensive microfluidic devices covered in this article. In addition, we highlighted different manufacturing techniques, such as soft lithography, laser plotting, and 3D printing, that are suitable for creating LOCs. The selection of materials and fabrication techniques will depend on the specific requirements and applications of each individual LOC. This article aims to provide a comprehensive overview of the numerous alternatives for the development of low-cost LOCs to service industries such as pharmaceuticals, chemicals, food, and biomedicine.

Abstract

In Chile, despite a series of laws to improve working conditions for domestic workers, informal employment persists. Using quantitative and qualitative methods, this article examines why the informality of work is so persistent. We find that existing policies do not take into account the multidimensional nature of work informality, or the power dynamics between workers and employers that determine the conditions of domestic work. By studying these dynamics, we find that some female domestic workers are particularly vulnerable, like those with urgent economic needs or with unstable work arrangements. Future efforts to formalize the sector should take into consideration women workers’ heterogeneous abilities to claim their labor rights.

Abstract

Objetivos: determinar la asociación estadística entre el diagnóstico de enfermedad hepática alcohólica según Registros Individuales de Prestaciones de Servicios de Salud (RIPS) y ventas de alcohol por departamento, 2012. Métodos: se realizó un estudio ecológico transversal, comparando tasas de prevalencia estimadas por RIPS (códigos CIE K700-9) y población mayor de 18 años (DANE), con ventas de licores nacionales, licores importados y cervezas, convertidas a unidades de alcohol, según datos de la Federación Nacional de Departamentos. Resultados: las tasa de enfermedad hepática alcohólica nacional fue 10,7 por 100 000 con las tasas más elevadas en Santander (27,1), Risaralda (19,9) y Boyacá (15,0). Después de retirar los datos de departamentos con resultados poco confiables o incompletos, la correlación de Pearson para tasa de enfermedad y ventas de unidades de alcohol fue de 0,6. Conclusiones: existe una correlación positiva entre la tasa de enfermedad hepática alcohólica y las ventas de alcohol por departamento.

Abstract

El presente artículo aborda el conflicto de la participación sindical, empleo y trabajo de cuidados no remunerado, de trabajadoras de dos gremios del sector servicios en Chile. Dicho análisis se lleva a cabo en el contexto del aumento de la sindicalización femenina en el país, indagando en las complejidades y conflictos de la responsabilidad conjunta de actividades de cuidados no remunerados, empleo y participación sindical, así como en las estrategias quelas mujeres desarrollan para enfrentarlas. El trabajo aporta en una dirección distinta de las escasas investigaciones nacionales actuales acerca de la participación sindical femenina, que ponen el acento en su exclusión y marginación.Para ello se llevan a cabo entrevistas semiestructuradas y grupales, a mujeres sindicalistas de distintas edades de la Región Metropolitana. El trabajo muestra el desarrollo de tácticas y estrategias de resistencia para enfrentar el conflicto por parte de las mujeres en los tres ámbitos señalados, lo que incide en el posicionamiento en el ámbito sindical, acompañado de reordenamientos en el ámbito doméstico y un proceso de creciente empoderamiento.

8. Retórica del cuerpo en Santa Bárbara de Baltasar Vargas de Figueroa

Abstract

La vivencia del cuerpo, entendida como una experiencia cultural, varía de acuerdo a la construcción histórica, política, social y religiosa a la que cada individuo se vea expuesto. Este escrito tiene como propósito comprender la relevancia de las representaciones visuales sobre la retórica corporal y sensorial que construyó la Iglesia durante la época colonial en el Nuevo Reino de Granada (Colombia). Partiendo del contraste de una pintura del martirio santoral, la “Muerte de Santa Bárbara” de Baltasar Vargas de Figueroa, analizada iconológicamente aplicando el método de Panofsky, se pretende evidenciar que este discurso es antecedente de algunas praxis corporales de la actualidad, demostrando así, que el cuerpo es una continua construcción social.

Abstract

In the context of the COVID‐19 pandemic, personal protective measures (e.g., social distancing, handwashing, and mask wearing) have been adopted as a cornerstone to limit the spread of the disease. Yet, the effectiveness of these measures depends on people's levels of adherence. In this article, we apply social‐psychological research to the study of compliance with personal protective measures during the COVID‐19 pandemic in Chile. We consider three possible models underlying adherence: (1) sociodemographic and socioeconomic factors, (2) instrumental factors, and (3) normative factors. We draw on data from a longitudinal nonrepresentative panel study (Study 1, n = 32,304) and a cross‐sectional representative survey (Study 2, n = 1,078) to explore the impact of these different factors on personal protective measures compliance. Findings show the strongest support for the role of instrumental and normative factors, in that people who comply with protective measures report to a greater extent that relatives and friends comply too and tend to perceive high risk of COVID‐19. We finish by proposing policy recommendations to promote effective strategies to contain the spread of the virus. 10. Class identity in times of social mobilization and labor union revitalization: evidence from the case of Chile (2009–2019)

Abstract

Over the past decade, there has been a revival of social protest and labor union activity in Chile. In this article, we examine the effects of this phenomenon to analyze its influence on working-class identity. Using International Social Survey Programme surveys from 2009 and 2019, we investigate whether class location and union membership affect people’s subjective identification with the working class and how that effect may have changed over the decade. Our findings suggest that subjects who are situated in a ‘subordinated’ class position (unskilled workers or informal self-employed workers) are more likely to identify with the working class compared to subjects located in a privileged class position (employers, experts, or managers). However, surprisingly, our analysis does not indicate that working-class identity is reinforced by union membership. In addition, our results do not demonstrate that the effect of class or union membership has strengthened over the past decade. At the end of this article, we offer some possible explanations for these findings.
