Abstract

La expresión “calidad de vida”es una de las más utilizadas en todos los campos de la acción humana. Existen múltiples estrategias para lograr avanzar en la percepción del ser humano hacia la calidad de vida, y con el presente trabajo de grado se busca fortalecer estas estrategias para la información, educación y comunicación en salud incluyente y accesible. Su intencionalidad es la de aportar al desarrollo de la autonomía individual y colectiva en la determinación de los estilos de vida y en la garantía del derecho a la salud, las cuales no solo serán dirigidas hacia el usuario crónico pluripatológico como tal, sino también a su núcleo familiar y personal asistencial de la Sociedad Médica Rionegro S.A. (SOMER S.A.), dentro del programa “Cuidarte Siempre”, con el fin de fortalecer elementos claves y básicos para la prevención de la enfermedad, promoción de la salud, y herramientas de autocuidado que les permitan a los profesionales dar continuidad a los tratamientos establecidos, adoptando conductas saludables para la óptima recuperación de los pacientes y, de esta manera, lograr mejoría en la calidad de vida, disminuir complicaciones, disminuir hospitalizaciones, hechos que impactan directamente a la optimización de recursos y mejoría en la prestación de servicios de salud. The expression "quality of life" is one of the most used in all fields of human action. There are multiple strategies to achieve progress in the perception of the human being towards the quality of life, and with this degree work we seek to strengthen these strategies for inclusive and accessible health information, education, and communication. Its intention is to contribute to the development of individual and collective autonomy in the determination of lifestyles and in the guarantee of the right to health, which will not only be directed towards the multipathological chronic user as such, but also to their family nucleus and care staff of the Medical Society Rionegro S.A. (SOMER S.A.), within the "Cuidarte Siempre" program, in order to strengthen key and basic elements for disease prevention, health promotion, and self-care tools that allow professionals to continue with established treatments, adopting healthy behaviors for the optimal recovery of patients and, in this way, achieve improvement in the quality of life, reduce complications, reduce hospitalizations, facts that directly impact the optimization of resources and improvement in the provision of health services. 22. Supracricoid partial laryngectomy with cricohyoidoepiglottopexy in patients with radiation therapy failure

Abstract

Abstract Background To assess functional results, complications, and success of larynx preservation in patients with recurrent squamous cell carcinoma after radiotherapy. Methods From a database of 40 patients who underwent supracricoid partial laryngectomy (SCPL) with cricohyoidoepiglottopexy (CHEP) from June 2001 to April 2006, eight patients were treated previously with radiotherapy due to squamous cell carcinoma of the glottic region and were treated for recurrence at the site of the primary cancer. Results SCPL with CHEP was performed in six men and two women with a mean age of 67 years due to recurrence and/or persistence at a mean time of 30 months postradiotherapy (in case #8 after concomitant chemoradiotherapy). Bilateral neck dissection at levels II-V was performed in six patients. Only case #8 presented metastasis in one node. In case #5, Delphian node was positive. It was possible to preserve both arytenoids in five cases. Definitive surgical margins were negative. Complications were encountered in seven patients. Follow-up was on average 44 months (range: 20-67 months). Organ preservation in this series was 75%, and local control was 87%. Overall 5-year survival was 50%. Conclusions In selected patient with persistence and/or recurrence after radiotherapy due to cancer of the larynx, SCPL with CHEP seems to be feasible with acceptable local control and toxicity. Complications may occur as in previously non-irradiated patients. These complications must be treated conservatively to avoid altering laryngeal function.

23. Referential iconicity in music and speech within and across sensory modalities

Abstract

Musical meaning is multifaceted: both highly sensory and yet often abstract, able to cross cultural boundaries and yet embedded in specific traditions. For the most part it is not denotational (Monelle, 1991). Nevertheless, in “programmatic music”, musical themes are intended to refer to worldly objects and events on the basis of iconic (and indexical) grounds. Such non-arbitrariness of the sound-sign (Sonesson, 2013) appears to apply to speech as well, where research has established that the iconicity in question is subtle, but systematic enough to be detectible by both adults and children (Ahlner and Zlatev, 2010; Imai and Kita, 2014). Very often, it operates across sensory modalities, so that for example a sound form like lulu is linked to round shapes, while titti is associated with sharp and hard ones. This thesis investigates how referential iconicity in speech operates in relation to music, taking into account different kinds of iconicity, unimodality and cross-modality and finally cultural background. To address these aspects, an experiment in which 21 Swedish and 21 Chinese native speakers had to match musical fragments or spoken word-forms to referents (represented by schematic pictures) was designed. It included two different conditions. In one there were two sound-stimuli and two referents (more contrastive). In the other, a single sound-stimulus was to be matched to one of four alternative referents (less contrastive). The results showed that there was no significant difference between the overall results for music and linguistic tasks, indicating that the psychological, interpretive processes involved are not limited to a single cognitive domain, or semiotic system. As expected, the more contrastive condition was easier for both groups, showing that cultural background played little role for making the appropriate cross-modal mapping when the choice was so constrained.
