1. Relevance of blood tumor markers in inpatients with significant involuntary weight loss and elevated levels of inflammation biomarkers

Abstract: Abstract Purpose To assess the diagnostic performance of a panel of standard tumor markers (TMs) in patients hospitalized with significant involuntary weight loss (IWL) and elevated levels of inflammation biomarkers, and a combination of the TM panel and the finding of the computed tomography (CT) scan. Methods We conducted a retrospective study in the internal medicine department at Amiens-Picardie University Medical Center (Amiens, France) between January 1st, 2015, and November 1st, 2021. The inclusion criteria were age 18 or over, significant IWL (≥ 5 kg over 6 months), elevated inflammation biomarkers (e.g. C-reactive protein), and assay data on two or more standard TMs (carcinoembryonic antigen (CEA), carbohydrate antigen (CA) 19 − 9, CA 15 − 3, CA 125, neuron-specific enolase (NSE), alpha-fetoprotein (AFP), calcitonin, and prostate-specific antigen (PSA)). The result of each TM assay was interpreted qualitatively (as positive or negative), according to our central laboratory’s usual thresholds. Results Cancer was diagnosed in 50 (37.0%) of the 135 patients included. Positivity for one or more TMs had a positive predictive value (PPV) of 0.55 [0.43–0.66], and a negative predictive value (NPV) of 0.84 [0.75–0.93] for cancer diagnosis. When combined with the presence of suspicious CT findings (e.g. a mass, enlarged lymph nodes and/or effusion), positivity for one or more TMs had a PPV of 0.92 [0.08–0.30]. In the absence of suspicious CT findings, a fully negative TM panel had an NPV of 0.96 [0.89-1.00]. Conclusion A negative TM panel argues against the presence of a cancer, especially in the absence of suspicious CT findings.

Abstract: Objectives We aimed to describe patients with autoimmune diseases (AID) developing invasive fungal disease (IFD) and identify factors associated with short-term mortality.Methods We analysed cases of IFD associated with AID from the surveillance network of invasive fungal diseases (Réseau de surveillance des infections fongiques invasives, RESSIF) registry of the French national reference centre for invasive mycoses. We studied association of AID-specific treatments with 30-day mortality. We analysed total lymphocyte and CD4-T cell counts in patients with Pneumocystis jirovecii pneumonia (PCP).Results From 2012 to 2018, 549 individuals with IFD and AID were included, mainly with PCP (n=227, 41.3%), fungemia (n=167, 30.4%) and invasive aspergillosis (n=84, 15.5%). Rheumatoid arthritis (RA) and anti-neutrophil cytoplasmic antibodies (ANCA)-associated vasculitides (AAV) were the most frequent AID in PCP (n=55 and 25, respectively) and invasive aspergillosis (n=15 and 10, respectively), inflammatory bowel diseases (IBDs) were predominant in fungemia (n=36). At IFD diagnosis, 365 (66.5%) patients received glucocorticoids (GCs), 285 (51.9%) immunosuppressants, 42 (7.7%) tumor necrosis factor (TNF)-α blockers, 75 (13.7%) other biologics. Abstract: Background: Pasteurella spp. can lead to fatal infections in humans. Objective: To assess prognostic factors of invasive pasteurellosis. Methods: We conducted a single retrospective cohort study of local versus invasive Pasteurella infections from January 1, 2005, to December 31, 2018, in the Amiens-Picardie University Hospital, France. Abstract: Abstract Cutaneous involvement in multiple myeloma with extramedullary disease is rare. We report the case of a refractory multiple myeloma patient who developed a cutaneous lesion. Histopathology revealed dermal immature plasma cell infiltrate with a lack of CD138 expression. This cutaneous location was associated with an aggressive clinical course and short survival.

Abstract: We conducted a retrospective study on all cases of pneumococcal septic arthritis (SA) in patients >18 years of age reported to the Picardie Regional Pneumococcal Network in France during 2005–2016. Among 1,062 cases of invasive pneumococcal disease, we observed 16 (1.5%) SA cases. Although SA is uncommon in adult patients, the prevalence of pneumococcal SA in the Picardie region increased from 0.69% during 2005–2010 to 2.47% during 2011–2016 after introduction of the pneumococcal 13-valent conjugate vaccine. We highlight the emergence of SA cases caused by the 23B serotype, which is not covered in the vaccine.

Abstract: Objective To compare characteristics, pregnancies and treatments during pregnancies of seronegative and seropositive antiphospholipid syndrome (APS), to analyse factors associated with obstetrical outcome.Patients and methods Inclusion criteria were: (1) thrombotic and/or obstetrical APS (Sydney criteria); (2) absence of conventional antiphospholipid antibodies (APL); (3) at least one persistent non-conventional APL among IgA anticardiolipin antibodies, IgA anti-B2GPI, anti-vimentin G/M, anti-annexin V G/M, anti-phosphatidylethanolamine G/M and anti-phosphatidylserine/prothrombin G/M antibodies. The exclusion criteria were: (1) systemic lupus erythematosus ( SLE) or SLE-like disease; and (2) other connective tissue disease.Results A total of 187 women (mean 33±5 years) with seronegative APS were included from 14 centres in Austria, Spain, Italy, Slovenia and France and compared with 285 patients with seropositive APS. Seronegative APS has more obstetrical rather than thrombotic phenotypes, with only 6% of venous thrombosis in comparison to seropositive APS. Cumulative incidence of adverse obstetrical events was similar in seronegative and seropositive APS patients, although higher rates of intrauterine deaths (15% vs 5%; p=0.03), of preeclampsia (7% vs 16%, p=0.048) and lower live birth term (36±3 vs 38±3 weeks of gestation; p=0.04) were noted in seropositive APS. Abstract: Chronic stimulation by infectious pathogens or self-antigen glucosylsphingosine (GlcSph) can lead to monoclonal gammopathy of undetermined significance (MGUS) and multiple myeloma (MM). Novel assays such as the multiplex infectious antigen microarray (MIAA) and GlcSph assays, permit identification of targets for >60% purified monoclonal immunoglobulins (Igs). Searching for additional targets, we selected 28 purified monoclonal Igs whose antigen was not represented on the MIAA and GlcSph assays; their specificity of recognition was then analyzed using microarrays consisting of 3760 B-cell epitopes from 196 pathogens. The peptide sequences PALTAVETG and PALTAAETG of the VP1 coat proteins of human poliovirus 1/3 and coxsackievirus B1/B3, respectively, were specifically recognized by 6/28 monoclonal Igs. Re-analysis of patient cohorts showed that purified monoclonal Igs from 10/155 MGUS/SM (6.5%) and 3/147 MM (2.0%) bound to the PALTAVETG or PALTAAETG epitopes. Altogether, PALTAV/AETG-initiated MGUS are not rare and few seem to evolve toward myeloma.

Abstract: Noninfectious manifestations-allergy, autoimmunity/inflammation, lymphoproliferation, and malignancies-are known to exist in many primary immunodeficiency diseases (PID) and to participate in prognosis.To obtain a global view on their occurrence, we retrieved data from a retrospective cohort of 1375 patients included in the French National Reference Center for Primary Immune Deficiencies (CEREDIH) for whom we had a 10-year follow-up since inclusion in the registry.These patients were followed for 10 years (2009-2018) by specialized centers in university hospitals. This study showed that 20.1% of patients without prior curative therapy (n = 1163) developed at least 1 manifestation (event) encompassing 277 events.Autoimmune/inflammatory events (n = 138) and malignancies (n = 85) affected all age classes and virtually all PID diagnostic groups. They were associated with a risk of death that occurred in 195 patients (14.2%) and were found to be causal in 43% of cases. Malignancies (odds ratio, 5.62; 95% confidence interval, 3.66-8.62) and autoimmunity (odds ratio, 1.9; 95% confidence interval, 1.27-2.84) were clearly identified as risk factors for lethality. Patients who underwent curative therapy (mostly allogeneic hematopoietic stem cell transplantation, with a few cases of gene therapy or thymus transplantation) before the 10-year study period (n = 212) had comparatively reduced but still detectable clinical manifestations (n = 16) leading to death in 9.4% of them.This study points to the frequency and severity of noninfectious manifestations in various PID groups across all age groups. These results warrant further prospective analysis to better assess their consequences and to adapt therapy, notably indication of curative therapy.

Abstract: The pandemic of Coronavirus disease 2019 (COVID-19), caused by a new severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has spotlighted the link between viral infection and autoimmunity. In this review, we focus on coronavirus-induced autoimmunity based on evidence from experimental animal models, SARS-CoV infection with in vitro studies of molecular mimicry and COVID-19 with several clinical reports of autoimmune manifestations of this disease. Further studies will be needed to better characterize the role of SARS-CoV-2 in the development of autoimmunity.

13. Value of Positron Emission Tomography Coupled With Computed Tomography for the Diagnosis of Inflammatory Syndrome of Unknown Origin in an Internal Medicine Department

Abstract: Objective: To evaluate the usefulness of positron emission tomography (PET) coupled with computed tomography (CT) in the diagnostic workup for inflammatory syndrome of undetermined origin (IUO) and to determine the diagnostic delay in an internal medicine department. Patients and methods: We retrospectively studied a cohort of patients for whom a PET/CT scan had been prescribed in an indication of IUO in an internal medicine department (Amiens University Medical Center, Amiens, France) between October 2004 and April 2017. The patients were grouped according to the PET/CT findings: very useful (enabling an immediate diagnosis), useful, not useful, and misleading. Results: We analyzed 144 patients. The median (interquartile range) age was 67.7 years (55.8-75.8 years). The final diagnosis was an infectious disease in 19 patients (13.2%), cancer in 23 (16%), inflammatory disease in 48 (33%), and miscellaneous diseases in 12 (8.3%). No diagnosis was made in 29.2% of the cases; half of the remaining had a spontaneously favorable outcome. Fever was observed in 63 patients (43%). Positron emission tomography coupled with CT was determined to be very useful in 19 patients (13.2%), useful in 37 (25.7%), not useful in 63 (43.7%), and misleading in 25 (17.4%). Abstract: Receptor Activator of NFκB Ligand (RANKL) and its decoy receptor osteoprotegerin (OPG) have been shown to play a role not only in bone remodeling but also in inflammation, arterial calcification and atherosclerotic plaque rupture. In human smooth muscle cells, Cu(2+)-oxidized LDL (CuLDL) 10-50 μg/ml increased reactive oxygen species (ROS) and RANKL level in a dose-dependent manner, whereas OPG level was not affected. The lipid extract of CuLDL reproduced the effects of the whole particle. Vivit, an inhibitor of the transcription factor NFAT, reduced the CuLDL-induced increase in RANKL, whereas PKA and NFκB inhibitors were ineffective. LDL oxidized by myeloperoxidase (MPO-LDL), or other pro-oxidant conditions such as ultraviolet A (UVA) irradiation, incubation with H2O2 or with buthionine sulfoximine (BSO), an inhibitor of glutathione synthesis, also induced an oxidative stress and enhanced RANKL level. The increase in RANKL in pro-oxidant conditions was also observed in fibroblasts and endothelial cells. Since RANKL is involved in myocardial inflammation, vascular calcification and plaque rupture, this study highlights a new mechanism whereby OxLDL might, by generation of an oxidative stress, exert a deleterious effect on different cell types of the arterial wall.

Abstract: Background: Multiple myeloma (MM) is characterized by the accumulation of malignant plasma cells, which secrete >30g/L of monoclonal immunoglobulin (mc Ig). MM is always preceded by the usually asymptomatic stage called "monoclonal gammapathy of undetermined significance" (MGUS). Presently the detection of a mc Ig does not lead to analysis of its specificity of antigen (Ag) recognition. Yet certain infectious pathogens are associated with the development of B-cell malignancy: for instance, Epstein-Barr virus (EBV) and Burkitt lymphoma, Helicobacter pylori (H. pylori) and MALT lymphoma. Objective: Our aim was to determine the frequency of recognition by purified mc Ig from MGUS and MM patients, of infectious pathogens associated with chronic infection and B-cell malignancy. Methods: We used the "Multiplex Infectious Antigen Array" (MIAA)test set up by the team to study the specificity of purified mc IgG against 8 infectious pathogens: hepatitis C virus (HCV), EBV, H. pylori, varicella zoster virus (VZV), cytomegalovirus (CMV), herpesvirus simplex 1 (HSV-1), HSV-2, and Toxoplasma gondii. Second or third methods were used to verify the specificity of mc IgG (western blotting, ELISA, or another multiplex array). Sialylation of mc Ig, which reflects chronic inflammation, was also studied, via ELISAs and Western blotting, for 98 MGUS and MM patients and 43 controls. Results: The specificity of purified mc IgG was analyzed for 291 patients (60 MGUS, including 4 associated with a B-cell lymphoma; 108 MM; for the 123 other patients the diagnosis of MGUS or MM was not available). Purified mc IgG were found to be specific for one of the 8 infectious pathogens of the MIAA test for 17.5% of patients (51/291: 25 MGUS, 1 SMM, and 25 MM). Purified mc Ig specifically recognized EBV (n=27, including 8 MGUS, 1 SMM and 18 MM ; the Ag recognized were EBNA-1, 25 cases; and VCA, 2 cases); HCV (10 cases: 9 MGUS and 1 MM) ; HSV-1 (7 cases: 5 MGUS including 3 associated with a B-cell malignancy, and 2 MM) ; H. pylori (5 cases: 2 MGUS including 1 associated with a B-cell malignancy, and 3 MM); VZV (1 MM); and CMV (1 MGUS). The Ag most frequently recognized by purified mc IgG was EBV EBNA-1; MM patients with EBNA-1-specific mc IgG represented 10% of the MM in this series. Consistent with a pro-inflammatory effect, for 81.6% of MGUS and MM patients (80/98), the purified mc IgG was found to be hyposialylated. Moreover, purified polyclonal Ig were also hyposialylated for 37% of MM (compared to Conclusion: At least 6 infectious pathogens (EBV, HCV, H. pylori, HSV, VZV, CMV) may be involved in the pathogenesis of MGUS and MM with mc IgG. Importantly, EBV, HCV, H. pylori and HSV are known as oncogenic pathogens associated with solid cancer and/or B-cell malignancy. Finally, for 37% of the MM patients examined, both the polyclonal and mc Ig were found to be hyposialylated, which suggests that inflammation existed prior to clonality and MM. Abstract: International audience; BACKGROUND:Primary immunodeficiencies (PIDs) are inherited diseases associated with a considerable increase in susceptibility to infections. It is known that PIDs can also predispose to cancer and immune diseases, including allergy, autoimmunity, and inflammation.OBJECTIVE:We aimed at determining the incidence of autoimmunity and inflammation in patients with PIDs.METHODS:We have retrospectively screened 2183 consecutive cases of PID in the Centre de Référence Déficits Immunitaires Héréditaires registry (CEREDIH; the French national PID registry) for the occurrence of autoimmunity and inflammation.RESULTS:One or more autoimmune and inflammatory complications were noted in 26.2% of patients, with a risk of onset throughout the patient's lifetime. The risk of autoimmune cytopenia was at least 120 times higher than in the general population, the risk of inflammatory bowel disease in children was 80 times higher, and the risk of other autoimmune manifestations was approximately 10 times higher. Remarkably, all types of PIDs were associated with a risk of autoimmune and inflammatory complications, although the greatest risk was associated with T-cell PIDs and common variable immunodeficiency. The occurrence of autoimmune disease is a negative prognostic factor for survival.CONCLUSIONS:Our results provide the basis for a detailed prospective evaluation of autoimmunity and inflammation in the context of PIDs, with a view to accurately assessing these risks and describing the possible effect of medical intervention.

Abstract: We report the case of an 80-year-old woman who presented one episode of cardiopulmonary arrest and two episodes of acute airway obstruction. We found in this patient the presence of tracheomalacia caused by megaesophagus compression secondary to achalasia probably responsible for episodes of acute airway obstruction and cardiopulmonary arrest.

Abstract: Background Receptor activator of nuclear factor kappa-light-chain-enhancer of activated B cell ligand/osteoprotegerin ratio is of crucial importance in osteoclast differentiation and thus in bone dysregulation diseases. Methods Receptor activator of nuclear factor kappa-light-chain-enhancer of activated B cell ligand and osteoprotegerin were determined under oxidized low density lipoprotein treatment of human osteoblast-like cells. The involvement of oxidative stress, of the extracellular signal regulated kinase and of the transcription factors nuclear factor kappa-light-chain-enhancer of activated B cells and nuclear factor of activated T cells was demonstrated. Results Cu 2+ -oxidized low density lipoprotein increased cell-associated and extracellular receptor activator of nuclear factor kappa-light-chain-enhancer of activated B cell ligand levels whereas osteoprotegerin levels were not affected. The increase in receptor activator of nuclear factor kappa-light-chain-enhancer of activated B cell ligand was parallel to the generation of reactive oxygen species provoked by Cu 2+ -oxidized low density lipoprotein. The lipid extract of Cu 2+ -oxidized low density lipoprotein, together with other forms of oxidized low density lipoproteins such as smooth muscle cell-oxidized low density lipoprotein and myeloperoxidase-oxidized low density lipoprotein, also induced an increase in reactive oxygen species and cell-associated receptor activator of nuclear factor kappa-light-chain-enhancer of activated B cell ligand. The effect of Cu 2+ -oxidized low density lipoprotein was prevented by the antioxidant vitamin E, and mimicked by the prooxidant compounds hydrogen peroxide and buthionine sulfoximine. Inhibitors of mitogen activated protein kinase/extracellular signal regulated kinase (PD 98059), nuclear factor kappa-light-chain-enhancer of activated B cells (Ro 106-9920) and nuclear factor of activated T cells (Vivit) reduced the effect of Cu 2+ -oxidized low density lipoprotein on receptor activator of nuclear factor kappa-light-chain-enhancer of activated B cell ligand expression. Cu 2+ -oxidized low density lipoprotein signaling was also reduced by vitamin E. General significance This work describes a new molecular mechanism and elucidates the signaling pathway whereby oxidized low density lipoprotein, by means of its lipid moiety, can modulate the crosstalk between osteoblasts/osteoclasts and bone remodeling, leading to an eventual risk of osteoporosis.

19. Plasma Exchange in the Management of Catastrophic Antiphospholipid Syndrome

Abstract: Objective. Report of a case of catastrophic antiphospholipid syndrome (CAPS) with multiple organ involvement leading to a life-threatening condition despite early combination corticosteroid and heparin therapy. Initiation of plasma exchange led to rapid improvement of the patient’s general condition. Design. Case report. Setting. University teaching hospital medical intensive care unit. Patient. Single case: 52-year-old man hospitalized for catastrophic antiphospholipid syndrome (CAPS) with cardiac, renal, and cutaneous involvement. Despite early methylprednisolone and heparin therapy, the patient’s condition progressively deteriorated, resulting in acute renal failure, right adrenal hemorrhage, and pulmonary involvement, leading to acute respiratory distress on day 6, requiring high-flow nasal cannula oxygen therapy with FiO2 of 1.0. Interventions. Plasma exchange was started on day 6. Endpoints and Main Results. A marked improvement of the patient’s general condition was observed after initiation of plasma exchange, with successful weaning of oxygen therapy and normalization of platelet count, troponin, and serum creatinine within four days. Conclusions. This case illustrates the efficacy of plasma exchange in CAPS and the difficulty for physicians to determine the optimal timing of plasma exchange.

20. Risque infectieux associé au daratumumab en traitement du myélome

Abstract: Le daratumumab, anticorps monoclonal ciblant le CD38, fait partie des nouvelles thérapies ayant transformé l'approche thérapeutique du myélome ces dernières années, mais son impact sur l'immunité et le risque infectieux reste incertain. L'objectif de l'étude était d'évaluer le risque d'infections notamment opportunistes chez les patients traités par daratumumab pour un myélome. Nous avons réalisé une étude rétrospective, monocentrique concernant les patients traités par daratumumab pour un myélome dans notre centre entre le 01/01/2012 et le 20/09/2019. Au total 75 patients ont été inclus. Sous daratumumab, 69% des patients ont présenté au moins une infection, la majorité étaient d'origine bactérienne (39,9%) et localisées aux voies respiratoires (63,6%). 9,3% des patients ont présenté au moins une infection opportuniste (une pneumocystose, une nocardiose, une aspergillose et six candidoses). Le taux d'incidence d'infections globales sous daratumumab était de 176 pour 100 patients-années (IC 95 [149,72205,54]) contre 65 pour 100 patients-années (IC 95 [57,06-74,06]) avant daratumumab. Le taux d'incidence d'infections opportunistes sous daratumumab était de 10 pour 100 patients-années (IC 95 [5,9-20,4]) contre 6 pour 100 patients-années avant daratumumab (IC 95 [3,9-9,1]). 72% des patients avaient une prophylaxie antibactérienne, 86,7% une prophylaxie antipneumocystose, 98,7 % une prophylaxie par aciclovir, 20% étaient vaccinés contre la grippe, 26,7% contre le pneumocoque et 10,7% contre l'Haemophilus influenzae. L'augmentation du taux d'infections globales mais aussi opportunistes sous daratumumab en traitement de myélome en rechute doit amener les cliniciens à être vigilants à la prescription des antibioprophylaxies et des vaccinations actuellement recommandées.

21. Analyse du mesencephalic astrocyte-derived neurotrophic factor (MANF) dans le contexte du myelome multiple

Abstract: Le Myélome Multiple (MM) est une hémopathie maligne caractérisée par la prolifération clonale de plasmocytes sécrétant une paraprotéine. A l’instar d’autres maladies médiées par le stress du réticulum endoplasmique (RE), l’Unfolded protein response (UPR) a un rôle clé dans la pathogénèse du MM. Nous avons exploré la possibilité d’utiliser Mesencephalic astrocyte-derived neurotrophic factor (MANF), protéine ubiquitaire sécrétée lors d’un stress du RE, comme biomarqueur dans le MM. Selon le National Cancer Institute-60, la lignée myélomateuse RPMI 8226 est une des lignées tumorales exprimant le plus l’ARNm de MANF. In vitro, elle exprime et sécrète la protéine MANF de façon significativement plus importante que d’autres lignées de MM (MM1.S) et de tumeurs solides. Le taux sérique médian de MANF était de 4,81 [3,237] ng/ml chez 73 patients avec une gammapathie monoclonale de signification indéterminée et de 5,615 [7,595] ng/ml chez 68 patients avec un MM non appariés (p=0,23). Aucune différence significative n’était retrouvée par rapport à une population de sujets sains (30 femmes et 30 hommes) âgés de 20 à 80 ans. Chez les sujets sains, la concentration médiane sérique de MANF diminuait de façon significative avec l’âge (p=0,0077) et était plus élevée chez les hommes (4,84 [3,02] ng/ml versus 8,96 [7,49] ng/ml, p=0,028). Notre étude n’a pas permis de valider MANF comme biomarqueur d’intérêt chez les patients atteints d’un MM mais invite à explorer son implication possible dans la tumorigénèse. Des études complémentaires sont également nécessaires pour préciser les facteurs physiologiques influençant le taux circulant de MANF chez l’Homme.

22. Dosage de l'annexine A2 au cours du lupus érythémateux systémique

Abstract: Introduction : l’Annexine A2 (ANXA2), protéine liant les phospholipides membranaires, est un ligand du C1q à la surface des cellules apoptotiques. Des anticorps anti-ANXA2 ont été détectés chez des patients atteints de Lupus Érythémateux Systémique (LES), maladie auto-immune en partie due à un défaut de clairance des corps apoptotiques. Notre étude visait à corréler les taux d’ANXA2 de patients suivis pour LES, au phénotype clinique et à l’activité du LES, évaluée par le SLEDAI (Systemic Lupus Erythematosus Disease Activity Index) et certains biomarqueurs. Matériel et méthodes : l’ANXA2 sérique et urinaire a été dosée avec un kit commercial ELISA sandwich chez 43 patients lupiques recrutés prospectivement dans notre service. Les dosages sériques ont été comparés à ceux de témoins appariés par âge et par sexe. Résultats : quatorze patients avaient une atteinte rénale, 8 une atteinte neurologique centrale, 23 une atteinte cardiovasculaire, et 11 patients un syndrome des anti-phospholipides (SAPL) associé. Les taux médians d’ANXA2 sérique des patients (5,7 ng/ml) et des témoins (3,8 ng/ml) ne différaient pas (p = 0,166). Les taux sériques d’ANXA2 ne sont pas associés à l’atteinte clinique. L’ANXA2 urinaire est corrélée à l’atteinte neurologique centrale (p = 0,017) et au SAPL (p = 0,045). Le SLEDAI est corrélé à l’ANXA2 urinaire (ρ = 0,32 ; p = 0,045) mais pas à l’ANXA2 sérique. L’ANXA2 n’est pas corrélée aux biomarqueurs étudiés d’activité du LES. Conclusion : l’ANXA2 sérique n’est corrélée ni au phénotype clinique, ni à l’activité clinico-biologique du LES. L’utilisation de l’ANXA2 urinaire comme marqueur d’activité du LES est à valider par des études de plus grand effectif.

23. Prévalence et signification clinique des anticorps anti-annexine A2 au cours de la polyarthrite rhumatoïde

Abstract: Introduction : le pannus synovial impliqué dans la destruction articulaire des patients atteints de polyarthrite rhumatoïde (PR) est la résultante d’une néo-angiogenèse au sein de la membrane synoviale. L'annexine A2 joue un rôle primordial dans la néo-angiogenèse et facilite les érosions dans des modèles murins d’arthrite induite au collagène. La prévalence des IgG anti-ANXA2 (aANXA2) dans la PR est de 10%, augmentant à 14% en cas d’atteinte érosive. Les objectifs de notre étude étaient de déterminer la prévalence des aANXA2 dans la PR et dans d’autres rhumatismes inflammatoires et de rechercher une association entre la présence des aANXA2 et les formes sévères de PR. Patients et méthodes : il s’agissait d’une étude rétrospective, transversale et monocentrique. Les données cliniques étaient recueillies dans le dossier médical informatisé des patients. Les aANXA2 ont été détectés par une technique ELISA. Résultats : 154 patients ont été inclus dans l’étude : 48 PR, 25 spondyloarthrites (SA), 4 rhumatismes psoriasiques (RPs), 12 syndromes de Gougerot-Sjögren (SGS), 40 pseudo-polyarthrites rhizoméliques (PPR), 25 autres rhumatismes inflammatoires, et 65 témoins sains. La prévalence des aANXA2 IgG était de 20.8% dans la PR, 8% dans les SA, 8.3% dans le SGS, 10% dans la PPR, 16% dans le groupe des autres rhumatismes inflammatoires, et 6.2% chez les témoins sains. En analyse univariée, il existait une différence significative entre les patients atteints de PR et les témoins sains, concernant l’âge, le sexe et les aANXA2 IgG. Après régression logistique sur l’âge et le sexe, les aANXA2 IgG restaient associés à la PR (Chi-square à 5.8705, p = 0.0154). Il n’y avait pas de corrélation entre les aANXA2 et les formes sévères de PR. Conclusion : notre étude a montré une association entre les aANXA2 IgG et la PR, avec une prévalence de 20.8%.
