In this article, a smart visual acuity measurement (VAM) system is designed and implemented. Hardware of the proposed VAM system consists of two parts: a wireless remote controller, and a high-resolution LCD controlled through a Raspberry-Pi mini-computer. In the remote controller, a 3.5" graphical LCD with a touch screen is used as a human-machine interface. When a point is pressed on the touch screen, the unique identifier (ID) code of that point as well as its page number is transmitted to the Raspberry-Pi. In the Raspberry-Pi, data are received and processed by a smart application coded in visual studio software. Then, the commanded tasks are executed by the Raspberry-Pi's operating system. Numerous charts, characters, and pictures are stored in the proposed VAM system to provide various VAM options while the size of the optotypes is adjusted automatically based on the distance of the patient from the LCD. The performance of the proposed VAM system is examined practically under the supervision of an expert optometrist where the results indicate that visual acuity, astigmatism, and color blindness of patients can be examined precisely through the proposed VAM system in an easier and more comfortable manner. The main aim of this work is to present the results of the shape design optimization process of an interior permanent-magnet synchronous motor. The shape design optimization process is accomplished based on the variations of the rotor structure using the Taguchi method. The time-stepping finite-element method is used for analysis of the motor. It is found that the optimal two-layer Machaon design has the lowest torque pulsation compared with the other structures. Finally, the optimized Machaon and preliminary one-layer interior permanent-magnet synchronous motors, which are amended from industrial three-phase induction motors, are manufactured. The aim of the study was to evaluate and collect current evidence on the effect of antibiotics in pretreatment of pouchitis after restorative ileal pouch anal anastomosis (IPAA). Pubmed, Embase, Web of Science, Scopus, and Cochrane Library databases were searched between 1966 and July 2008; and relevant clinical trials extracted, reviewed, and validated according to the study protocol. The outcome of interest was clinical improvement after treatment. Nine randomized, placebo-controlled clinical trials were found relevant and studied but 3 of them with 70 patients were entered into meta-analysis. Pooling of the results from these trials yielded an odds ratio of 15.96 with a 95% CI of 4.20–60.70, indicating a significant OR (p<0.0001) in treatment group in comparison to the placebo group. In conclusion, the meta-analysis confirms benefit of antibiotics in management of pouchitis after IPAA operation. [ABSTRACT FROM AUTHOR]

Solitary rectal ulcer syndrome (SRUS) is a syndrome with symptoms such as rectal bleeding, obstructed defecation, straining at stool and incomplete evacuation, and rectal polyps. In the present study, the clinical features of SRUS among Iranian patients is reviewed. Records of 112 patients diagnosed with SRUS between 1997 and 2007 admitted to a special coloproctology clinic have been analyzed retrospectively. Of 112 patients with SRUS, 61 were male (54.4%) and 51 female (45.6%) with a mean age of 32.2 years (range, 16–64 years). The mean interval between onset of symptoms and final diagnosis of SRUS was 3.8 years (range, 1–14 years). Rectal bleeding (67%) was the most common symptom in both genders, while a feeling of fullness was the least common symptom, observed only in one woman (2%). Incontinence was observed in 4 women (7.8%) and 3 men (4.9%), comprising 6.25% of the total clinical presentations. There were 38 patients (33.9%) with mucosal prolapse and 2 patients (1.8%) with total rectal prolapse. In conclusion, rectal bleeding in the presence of constipation or diarrhea is the main sign for diagnosis of SRUS. This syndrome is usually misdiagnosed; it is frequently confused with inflammatory bowel disease (IBD) because the rectal macroscopic and microscopic lesions and true polyps of rectum are similar. We suggest that most of patients who are treating for IBD and true polyps without any response are suffering from SRUS. Regarding misdiagnosis between SRUS and IBD or rectal polyp, the exact prevalence of SRUS has been mistakenly underestimated. [ABSTRACT FROM AUTHOR]
