Abstract

Understanding consumer trophic status and long‐term dietary changes can yield information about impacts of altered habitats on their ecology. In Hawai'i, coastal ecosystems have been significantly modified by the introduction of invasive seaweeds and mangroves, high nutrient load and overfishing, but so far, much is still to be understood about how these changes have affected the green turtle (Chelonia mydas).This study analyzed stable carbon, nitrogen, and sulfur isotope ratios in the bone tissue of modern and museum specimens of green turtles collected from 1901 to 2020 in Oahu and the North‐western Hawaiian Islands to understand how their isotopic niche has changed through time, a crucial step towards restoring the ecological role of a formerly decimated species.The standard ellipse size and the total area of the convex hull of the isotopic niche of green turtles in three periods (1901–1951, 1992–2008, and 2018–2020) were calculated.The stable isotope values of ancient green turtles (1901–1951) suggest that they relied heavily on macroalgae even before the introduction of exotic species and eutrophication promoted the development of algal pastures.However, a few ancient green turtles relied heavily on seagrasses, and others complemented their macroalgae‐based diets with significant amounts of animal matter. Such diet specialists were missing from the sample of current green sea turtles, suggesting that these foraging strategies are less common or perhaps even absent in the current population. The results suggest that green turtles have converged on the use of the most abundant resource, red macroalgae, likely because of the homogenization and simplification of coastal habitats and food webs due to anthropogenic influences.Restoring the population size of herbivorous fishes and a more diverse ecosystem structure may be necessary to recover the array of trophic strategies formerly present in the Hawaiian green turtle population. [eng] Hawaiian green turtles Chelonia mydas were heavily exploited for their fat, meat and eggs by the Polynesians in the pre-contact era and continued to be commercially exploited until 1978 when they were officially protected. These conservation measures allowed its population to rebound, although it is still considered threatened by the Endangered Species Act. Hawaiian green turtles have been the focus of intense research, but surprisingly, little is known about its trophic ecology and how it might have changed due to the anthropogenic impacts in the coastal ecosystems of the archipelago. This thesis aims to better understand the current habitat use and diet of green turtles in the Hawaiian Islands and track possible historical changes in their ecological niche. Underwater censuses in Oahu and the Kona coast revealed that green turtles had a strong preference for shallow, flat platforms covered with dense macroalgal pastures. Green turtle abundance was much lower in coral reefs, where they also had a modest contribution to the total biomass of herbivores, dominated by sea urchins and fishes. Not surprisingly, the stable isotope ratios of C, N and S in the epidermis of modern green turtles from east Oahu and the Kona coast confirmed a macroalgae- dominated diet, but seagrasses and mangroves had also relevant contributions to their diet in east Oahu, as well as fish in the Kona coast. Furthermore, the ontogenetic diet shift associated with the settlement of juvenile green turtles in neritic habitats is faster in eastern Oahu than in the Kona coast, perhaps because of the higher availability of macroalgae in the former. The stable isotope ratios of C, N and S in the squamosal and the ribs of the same green turtle individuals revealed similar patterns of geographic and ontogenetic variability, hence confirming that unprocessed bone samples are informative of diet prior to death. This is the base for retrospective analysis using museum specimens. However, mixing models using the trophic discrimination factor (TDF) derived experimentally for cortical bone yielded unreliable results when used on unprocessed bone samples, suggesting that trabecular bone has a different TDF value. This is relevant, because the skulls and carapaces preserved at museums are made of skeletal elements with a thick core or trabecular bone. Despite such limitation, the stable isotope ratios of C, N, and S in the skeletal elements of green turtles preserved in museums revealed minor changes in the isotopic niche of green turtles from east Oahu during the past 120 years. Nevertheless, the breadth of the isotopic niche decreased in the most recent years, indicating that ancient green turtles exhibited a broader diversity of individual foraging strategies, with a few individuals relying mostly on seagrasses and others consuming substantial amounts of animal matter. These trophic specialists are currently gone from eastern Oahu, where green turtles have converged on the use of the most abundant resource, red macroalgae, probably because of the homogenization and simplification of coastal habitats Nevertheless, results confirm that macroalgae were the staple diet for the majority of the green turtle population before the introduction of exotic red macroalgae and hence it is a trait characteristic of the Hawaiian population. To understand the relationship between diet and the morphology of the skull and the mandible, geometric and traditional morphometrics were used, comparing skulls and mandibles of populations relying mostly on seagrasses and populations relying mostly on macroalgae. Results showed that macroalgae consumers have longer and narrower skulls than seagrass consumers, the former is more suitable for selective browsing and suction feeding and the latter is better adapted for grazing and stronger bite force. The skull morphology of Hawaiian green turtles fits that general pattern but is different from that of the green turtles inhabiting the Mexican Pacific, although both are genetically related.

This study conducted stable isotope analysis (δ13C, δ15N, and δ34S) on the epidermis and two skeletal elements (rib and squamosal bones) of Hawaiian green turtles (Chelonia mydas) and putative diet items obtained from two neritic sites: the Kona/Kohala coast and Oahu. Turtle tissues were collected in 2018–2020 and diet samples in 2018, 2019, and 2021. The effect of body size and sampling locality on individual bulk tissue isotope values was evaluated, and stable isotope mixing models based on δ13C, δ15N, and δ34S values from those tissues and four groups of food sources were used to reconstruct diet histories of the turtles. Mixing models indicated that green turtles along the Kona/Kohala coast consumed an omnivorous diet, whereas those from Oahu had an herbivorous diet. These diet make-ups are consistent with published gut content analyses. However, mixing models using the stable isotope ratios in rib and squamosal bone failed to yield reasonable diet histories, probably due to inadequacies of the applied trophic discrimination factor (TDF), a key model parameter. These results further establish that stable isotope ratios in the epidermis can be used effectively to study green turtle diet, but also reveal that more validation—and establishment of appropriate TDFs—is needed before bone can be used reliably to assess green turtle diet. Abstract

PURPOSE: We performed a nationwide registry-based analysis to describe the clinical outcome of adults patients with Philadelphia chromosome-positive acute lymphoblastic leukemia (Ph+ ALL) who underwent an allogeneic hematopoietic stem cell transplantation (HSCT) after a TKI-based treatment. PATIENTS AND RESULTS: A total of 441 patients were included in the study. The median age at HSCT was 44 years (range 18-70). All the 441 patients (100%) received TKI before the HSCT (performed between 2005 and 2016). Of these patients, 404 (92%) were in cytologic complete remission (CR), while the remaining 37 (8%) had an active disease at the time of HSCT. Molecular minimal residual disease (MRD) was negative in 147 patients (36%) at the time of HSCT. The donor was unrelated in 46% of cases. The prevalent source of stem cells was peripheral blood (70%). The conditioning regimen was myeloablative in 82% of cases (TBI-based in 50%) and included ATG in 51% of cases. With a median follow-up after HSCT of 39.4 months (range: 1-145), the overall survival (OS) probability at 1, 2 and 5 years was 69.6%, 61.1% and 50.3%, respectively, with a median OS of 62 months. Progression-free survival (PFS) at 1, 2 and 5 years was 60.2%, 52.1% and 43.7%, respectively. OS and PFS were significantly better in patients with CR and MRD-negative at the time of transplant compared with those of patients with CR but MRD-positive (50% OS not reached vs. 36 months, P=0.015; 50% PFS not reached vs. 26 months, P=0.003). The subgroup of MRD-negative patients both at HSCT and at 3 months after HSCT had a better outcome (5 years OS rate 70%). Conversely, the 37 patients who underwent a HSCT with active Ph+ ALL had a median OS and PFS of 7 and 5 months, respectively. The 5 years cumulative incidence of relapse was significantly lower in MRD-negative patients (19.5% vs. 35.4%, P=0.001). Abstract

Background Gram-negative bacteremia (GNB) is a major cause of illness and death after hematopoietic stem cell transplantation (HSCT), and updated epidemiological investigation is advisable. Methods We prospectively evaluated the epidemiology of pre-engraftment GNB in 1118 allogeneic HSCTs (allo-HSCTs) and 1625 autologous HSCTs (auto-HSCTs) among 54 transplant centers during 2014 (SIGNB-GITMO-AMCLI study). Using logistic regression methods. we identified risk factors for GNB and evaluated the impact of GNB on the 4-month overall-survival after transplant. Results The cumulative incidence of pre-engraftment GNB was 17.3% in allo-HSCT and 9% in auto-HSCT. Escherichia coli, Klebsiella pneumoniae, and Pseudomonas aeruginosa were the most common isolates. By multivariate analysis, variables associated with GNB were a diagnosis of acute leukemia, a transplant from a HLA-mismatched donor and from cord blood, older age, and duration of severe neutropenia in allo-HSCT, and a diagnosis of lymphoma, older age, and no antibacterial prophylaxis in auto-HSCT. A pretransplant infection by a resistant pathogen was significantly associated with an increased risk of posttransplant infection by the same microorganism in allo-HSCT. Colonization by resistant gram-negative bacteria was significantly associated with an increased rate of infection by the same pathogen in both transplant procedures. GNB was independently associated with increased mortality at 4 months both in allo-HSCT (hazard ratio, 2.13; 95% confidence interval, 1.45–3.13; P Conclusions Pre-engraftment GNB is an independent factor associated with increased mortality rate at 4 months after auto-HSCT and allo-HSCT. Previous infectious history and colonization monitoring represent major indicators of GNB. Clinical Trials registration NCT02088840.

Abstract Background Beta thalassemia major is a severe inherited form of hemolytic anemia that results from ineffective erythropoiesis. Allogenic hematopoietic stem cell transplantation (HSCT) remains the only potentially curative therapy. Unfortunately, the subgroup of adult thalassemia patients with hepatomegaly, portal fibrosis and a history of irregular iron chelation have an elevated risk for transplantation-related mortality that is currently estimated to be about 29 percent. Discussion Thalassemia patients may be faced with a difficult choice: they can either continue conventional transfusion and iron chelation therapy or accept the high mortality risk of HSCT in the hope of obtaining complete recovery. Throughout the decision making process, every effort should be made to sustain and enhance autonomous choice. The concept of conscious consent becomes particularly important. The patient must be made fully aware of the favourable and adverse outcomes of HSCT. Although it is the physician's duty to illustrate the possibility of completely restoring health, considerable emphasis should be put on the adverse effects of the procedure. The physician also needs to decide whether the patient is eligible for HSCT according to the "rule of descending order". The patient must be given full details on self-care and fundamental lifestyle changes and be fully aware that he/she will be partly responsible for the outcome. Summary Only if all the aforesaid conditions are satisfied can it be considered reasonable to propose unrelated HSCT as a potential cure for high risk thalassemia patients.

Maestría en Educación desde la Diversidad, Facultad de Ciencias Sociales y Humanas. El artículo invita a pensar –sentir la vida de los pueblos indígenas en Colombia, teniendo en cuenta que en las últimas cuatro décadas, estos pueblos, que sin lugar a dudas poseen vida e historia propia, se encuentran en un proceso de reconstrucción étnica, social y territorial; pero ello no puede ocultar las fracturas internas en el pensamiento, en la organización social, en la organización política y en la integridad territorial, que son las características esenciales de la vida de dichas comunidades. Se constata que estas comunidades no son ajenas a la descomposición del tejido social y, en consecuencia, a nivel interno padecen una debilidad generalizada. Los pueblos indígenas todavía viven en el arraigo y poseen un sentido de pertenencia con referencia a sus comunidades de origen, sin embargo, vienen siendo anclados por el fenómeno social de la guerra; toda la vida de las comunidades está permeada y asociada a los impactos más profundos y destructores de la violencia estructural que vive Colombia. Esta reflexión tiene un enfoque humanista, porque la problemática colombiana y del mundo entero, no es otra, sino el “olvido de lo humano”, manifestado en la incesante violación de los derechos humanos, fenómeno al que no escapa ningún grupo poblacional de nuestra sociedad. Despite major advances in the treatment of acute promyelocytic leukemia (APL), the problem of early death (ED) remains unsolved. Alongside the currently known clinical and hematological risk factors, prognostic significance has been attributed to internal tandem duplication mutations of the fms-like tyrosine kinase-3 (FLT3-ITD), hypogranular variant morphology, and the bcr-3 isoform of PML-RARα. We describe premature death of two patients with the hypogranular variant of APL who presented remarkably high expression levels of Wilms' tumor gene (WT1). Our results point to WT1 as an important prognostic factor of ED that needs to be promptly evaluated in all newly diagnosed cases of APL.

Abstract

Myeloid sarcoma is a rare tumor consisting of myeloid blasts that involve anatomic sites outside the bone marrow. Fatal prognosis is inevitable in patients with extramedullary relapse after hematopoietic stem cell transplantation (HSCT), and no standard treatments are available yet. We report the first case of extramedullary relapse after HSCT treated with a combination of daunorubicin, cytarabine, and cladribine (DAC) regimen plus radiotherapy and donor lymphocyte infusion (DLI). This treatment induced a new and durable remission in our patient. The favorable toxicity profile and the reduced cost make this combination worthy of further investigations.

Abstract

The article invites us to think, feel the life of indigenous people in Colombia, given that in the past four decades, these people, they certainly have their own life and history, are in a rebuilding process ethnic, social and territorial, but it can not hide the internal fractures in thought, in social organization, in the political and territorial integrity, which are the essential features of life in these communities. These communities are not immune to the breakdown of the social fabric and therefore suffer internally. The towns weakness Indians still live in the roots and have a sense of belonging with reference home communities, however, are being anchored by the social phenomenon of war life is permeated communities and impacts associated with deeper and destroyers of structural violence in Colombia. and the world, is nothing but the oblivion of humanity, expressed in constant violation of human rights, a phenomenon that does not escape any population group in our society. It emphasizes the urgent need to reclaim the role of indigenous and social and political subject in wartime, because even in the midst of their vulnerability are alive and dynamic individuals whose human rights have not been fully recognized., bia, teniendo en cuenta que en las últimas cuatro décadas, estos pueblos, que sin lugar a dudas poseen vida e historia propia, se encuentran en un proceso de reconstrucción étnica, social y territorial; pero ello no puede ocultar las fracturas internas en el pensamiento, en la organización social, en la organización política y en la integridad territorial, que son las características esenciales de la vida de dichas comunidades. Se constata que estas comunidades no son ajenas a la descomposición del tejido social y, en consecuencia, a nivel interno padecen una debilidad generalizada. 50. Stable isotopes provide insight into population structure and segregation in eastern North Atlantic sperm whales

Abstract

In pelagic species inhabiting large oceans, genetic differentiation tends to be mild and populations devoid of structure. However, large cetaceans have provided many examples of structuring. Here we investigate whether the sperm whale, a pelagic species with large population sizes and reputedly highly mobile, shows indication of structuring in the eastern North Atlantic, an ocean basin in which a single population is believed to occur. To do so, we examined stable isotope values in sequential growth layer groups of teeth from individuals sampled in Denmark and NW Spain. In each layer we measured oxygen-isotope ratios (delta O-18) in the inorganic component (hydroxyapatite), and nitrogen and carbon isotope ratios (delta N-15: delta C-13) in the organic component (primarily collagenous). We found significant differences between Denmark and NW Spain in delta N-15 and delta O-18 values in the layer deposited at age 3, considered to be the one best representing the baseline of the breeding ground, in delta N-15, delta C-13 and delta O-18 values in the period up to age 20, and in the ontogenetic variation of delta N-15 and delta O-18 values. These differences evidence that diet composition, use of habitat and/or migratory destinations are dissimilar between whales from the two regions and suggest that the North Atlantic population of sperm whales is more structured than traditionally accepted.
