Abstract

STUDY QUESTION Can anti-Müllerian hormone (AMH) help predict how many oocytes will be retrieved following double stimulation (DuoStim)? SUMMARY ANSWER A simple clinical tool can use serum AMH values to predict ovarian response following DuoStim in IVF cycles. WHAT IS ALREADY KNOWN The knowledge that multiple follicular waves arise during a single ovarian cycle has led to the introduction of unconventional ovarian stimulation protocols. The DuoStim protocol involves two successive ovarian stimulations performed during a single ovarian cycle and has been proposed as an approach for patients with poor ovarian response and for medical fertility preservation. As AMH has been used as a marker of ovarian reserve and stimulation response, the current study aimed to investigate the diagnostic performance of AMH in predicting the number of retrieved oocytes following DuoStim. STUDY DESIGN, SIZE, DURATION This is a retrospective observational study involving 116 patients who received IVF treatment from January 2021 to September 2022. PARTICIPANTS/MATERIALS, SETTING, METHODS The study was conducted at a private IVF centre. Only patients who had their AMH measured prior to treatment and had complete patient records regarding their clinical and IVF/ICSI cycle characteristics were included. The primary outcome was the correlation between AMH values and the number of oocytes retrieved following DuoStim. Parametric and non-parametric tests were used to compare baseline characteristics and outcomes. Spearman's R was used to analyse correlations between variables, while the C statistic was used to calculate the diagnostic performance of AMH. MAIN RESULTS AND THE ROLE OF CHANCE AMH levels were significantly correlated with the total number of oocytes retrieved after the DuoStim (R 0.61; CI 0.44–0.70; P  < 0.0001). The difference in the total number of oocytes retrieved between the first (median 4 oocytes, interquartile range (IQR) 2–6) and second (median 6 oocytes, IQR 3.2–8) stimulation was statistically significant (P  < 0.0001). However, there was no significant difference in the number of mature oocytes that were retrieved (median of 3 and 4 in the first and second stimulations, respectively). After the first stimulation, 68% of patients had at least one blastocyst available, while after the second stimulation, 74% did (NS). Based on linear regression, each 0.25 ng/ml increase in basal AMH corresponds to one additional oocyte recovered at the end of both stimulations (R 2: 0.32, P  < 0.0001). LIMITATIONS, REASONS FOR CAUTION The results are limited owing to the observational nature of the study and the number of participants. WIDER IMPLICATIONS OF THE FINDINGS Counselling infertile couples regarding the intermediate outcome of IVF (i.e. number of retrieved oocytes) is one of the most demanding tasks that clinicians face. To our knowledge, this is the first study that provides an easy-to-use clinical tool that enables the quantitative prediction of ovarian response following DuoStim, based on serum AMH values. Abstract

A survey was carried out on management practices, diet charac¬teristics, milk yield, and fatty acid profile in buffalo farming. Thirty-nine farms (about 3% of the buffalo farms of Campania Region) were voluntarily enrolled in the study. To be eligible to be included, the farms need to use the same total mixed ration (TMR) for all lactating cows. Each farm was visited twice, at least 10 days apart, to record management practices, ration composi¬tion, and milk yield along with sampling TMR and milk. Chemical composition, in vitro digestibility of DM (IVDMD) and NDF (IVNDFD) based on a 48-h incubation in a Daisy II system, and particle size distribution by means of the Penn State Forage Particle Separator were determined for TMR. Milk samples were analyzed for fat and protein (Milkoscan 605), somatic cell count (SCC-Fossomatic250), and fatty acid (FA) composition by trans-esterification reaction using capillary gas chromatography. Descriptive statistics, Pearson’s correlation and linear regression analyses were performed by using SAS statistical package. Herds averaged 136 ± 73 lactating buffaloes producing about 8.6 ± 1.7 kg at 8.45 ± 0.47% fat, 4.72 ± 0.37% protein, SCC 265.16 ± 150.81*103. Most of the farms used a combination of hays, silages, and con¬centrates while only 6 on them did not use ensiled forages. A larger variability was observed for ration characteristics. The TMR averaged 51.9 ± 8.5% DM, crude protein 13.9 ± 1.9% DM, and NDF 57.3 ± 5.4% DM. Particle size analysis showed that 45.6% of the TMR was retained on the 19-mm sieve (range 12.3–78.7%), 20.4% on the 8-mm sieve (2.4–50.5%), 10.4% on the 4-mm sieve (3.2–20.7%), and 23.0% on the bottom pan (5.4–22.7%). Weak relationships were found between particle size and milk yield and TMR composition, while IVNDFD was positively related to milk fat (R2 0.75, p < .001). Saturated FA averaged 70.3 ± 3.8, mono¬unsaturated FA 26.1 ± 1.3, and polyunsaturated FA 3.6 ± 0.5, data in line with previous reports. Weak relationships were found between TMR composition, TMR particle size and FA composition of milk fat. Overall, the survey highlighted that TMR particles larger than 19 mm were dramatically higher than the recom¬mended values for lactating cows. Nevertheless, the low incidence of metabolic disorders and the satisfactory milk yield suggests the need of establishing particle size ranges of TMR specific for buffaloes.

Abstract

Purpose: To estimate the contribution of cryopreservation to the cumulative live birth rate (CLBR) after law modification in Italy in the era of vitrification and freeze-all. Methods: The Italian National Registry performed a cycle-based data collection. Nine Italian IVF clinics were involved incorporating a total of 10,260 fresh cycles performed between January 2015 and April 2016 resulting in 9273 oocyte retrievals and 3266 subsequent warming cycles from the same oocyte retrievals performed up to December 2016. Mean female age was 37 ± 4.3 years. Primary outcome measure was CLBR per oocyte retrieval. Confounding factors were tested in multivariate regression analysis, and the relative impact of cryopreservation to the CLBR in different patient categories was calculated. Results: CLBR per oocyte retrieval was 32.6%, 26.5%, 18.7%, 13.0%, and 5.5% for women younger than 36, aged 36–39, 40–41, and older than 41 years, respectively. The total relative contribution of oocyte/embryo cryopreservation was 40.6% (95% CI 38.41–42.75). An association between maternal age, number of oocytes retrieved, fertilization rate, cryopreservation, and cumulative live birth was shown. When adjusted for confounders, a 2.3-fold increase was observed in the chance of live birth when cryopreservation was performed (OR 2.3; 95% CI 1.99–2.56). In high responder patients (> 15 oocytes retrieved) where freeze-all was applied in 67.6% of cycles to avoid the risk of hyper stimulation syndrome, the relative contribution of vitrification to the CLBR was 80.6%. Conclusions: Cryopreservation is essential in IVF and should always be available to patients to optimize success rates. Multicentric, cycle-based data analyses are crucial to provide infertile couples, clinicians, and regulatory bodies with accurate information on IVF effectiveness including fresh and cryopreserved cycles.

Abstract

The scarce implementation of automatic milking systems (AMS) in buffalo farming despite its primarily benefit, i.e. relief from the milking routine, is mainly due, beside to the initial investment, to concerns about adaptability of buffalo cows to robotic milking and a potential detrimental effect on milk quality. This study aimed to shed light on the operational and functional aspects of AMS in buffaloes and their relationships with milk yield and quality. The research was carried out over 3 years on a dairy buffalo farm equipped with both a traditional tandem parlour and four AMS, each serving about 50 cows. Efficiency of the AMS was evaluated by taking into consideration three issues: (1) yield and quality of milked milk; (2) system capacity, i.e. number of buffaloes that can be milked by AMS per year; (3) number of milking-failure visits, i.e. robot failures to attach milking cluster. Abstract

Skeletal remodelling is a continuous process during life and is still active also in extreme senescence. In the elderly, bone resorption often prevails over bone formation, causing bone loss and fragility. Elderly subjects are exposed to the risk of fractures, and loss of self-sufficiency, if considering that the proximal femur is the most frequently involved site. Bone remodelling can maintain circulating calcium within physiological ranges, at the expense of a substantial loss of this ion from the skeleton, particularly during senescence. Calcium metabolism is regulated at cellular/molecular level by a network of cytokines, growth factors, systemic hormones that act on bone in paracrine/autocrine/systemic fashion. Among the molecules involved in bone metabolism, parathyroid hormone (PTH) and vitamin D present some peculiar aspects during senescence. The osteometabolic features in a consistent group of centenarians have been evaluated. It results that a severe hypovitaminosis D was present in 99 out of 104 centenarians (25-OH vitamin D below 5 nmol/L), and that it plays an important role as a factor inducing a vicious circle involving hypocalcemia, secondary hyperparathyroidism, together with biochemical features indicating a consistent bone loss. Serum C-terminal cross-linking telopeptide, a specific marker of bone resorption was elevated in 92% of these subjects. Moreover, it has been found that several femoral fractures had occurred after 90 years of age. These data offer a rational for the possible prevention of elevated bone turnover, bone loss and consequently the reduction of osteoporotic fractures and fractures-induced disability, in the oldest olds, through the simple supplementation with calcium and vitamin D.

Abstract

From November 2009 to February 2010, twenty-one buffalo bulls were used to evaluate the effect of group size on behavioural and immune responses. Fourteen bulls (group HS) were group-housed in an indoor slatted floor pen with an outdoor paddock (3.2+3.2 m2/animal). Other 7 (group LS) were group-housed in a similar pen having the same space allowance per animal. The animals at beginning of experiment were aged 455±72 d and weighed 337±67 kg. Animals were subjected to seven sessions of instantaneous scan sampling at ~15-d intervals. Observations were made every 10 min over a 5-h period (1000 to 1500 h). Behavioural variables were expressed as proportions of subjects observed in each category of posture and activity. In addition, rapid behaviours such as agonistic, social, and reproductive interactions, social licking, and self-grooming were recorded continuously. These variables were expressed as number of interactions per animal per hour. Phytohaemagglutinin (PHA, 1 mg/animal; intradermal injection) was used to assess in vivo cellular immunity, whereas 5 mg of keyhole limpet hemocyanin were injected subcutaneously to evaluate antibody response. The immune responses of group HS were assessed only on seven animals chosen at random. Behavioural data were analysed with a two-way ANOVA (group size and period of observation). In vivo cellular immunity response was subjected to ANOVA with one factor (group size). Antibody response data were analysed using a linear mixed model for repeated measures. When more animals were involved in eating activity an higher number of aggressive interactions were observed compared to the period away from feeding (0.96 vs 0.56; SE 0.13). Immune responses were not affected by group size. It can be concluded that group size had not a detrimental effect on buffalo bull welfare.

Abstract

The Italian multicentric study on centenarians (IMSC) was aimed at assessing the level of preservation of the sense of taste, and at estimating to what extent the recognition of various gustative stimuli can give satisfaction and information regarding the surrounding environment for the centenarians. Taste sensitivity has been qualitatively established in a group of 126 Italian centenarians (mean age 101.9+/-1.4 years) and compared to that of a group of 100 elderly subjects (mean age 70.5+/-5.0 years). All the individuals included in the study groups had a mini mental state examination (MMSE) score larger than 19. The results revealed that taste sensitivity is significantly reduced in the centenarians; as a matter of fact, the elderly control subjects are able to perceive taste stimuli in 84.25+/-3.40% of the testing, while the centenarians were successful only in 74.23+/-6.19% of the experiments (P
