Hypertensive heart disease with biventricular cardiac failure is not common in clinical practice. This diagnosis requires an extensive diagnostic search. We present the clinical case of the male patient of 38 aged. He was admitted to the clinic with heart failure 3-4 NYHA class. EchoCG revealed symmetric hypertrophy of the left ventricle up to 18 mm without its dilatation, a decrease in ejection fraction up to 42%, restrictive hemodynamics, overload of the right chambers, severe pulmonary hypertension (60 mm Hg). The clinical status included persistent arterial hypertension (180-220 and 120-150 mm Hg), effusion in both pleural cavities and pericardium, ascites, renal failure. During examination (multispiral computed tomography, magnetic resonance imaging, scintigraphy with 131I-MIBG), bladder paraganglioma was diagnosed (normatenafrin 1468 μg/day). The resection of the tumor was performed, according to immunohistochemical research – neuroendocrine carcinoma, G1. After 3 months a partial regression of hypertension and cardiac failure was observed with the preservation of a high level of creatinine. The criteria and differential diagnosis of the hypertensive heart disease and the syndrome of primary myocardial hypertrophy, diagnostics of the urinary bladder paraganglioma, complex mechanisms of myocardial damage within the pheochromocytoma and its prognosis are discussed.

Results are given for development and use in industry of new import-substitution innovative laser technology providing good component, assembly, and unit operating properties for petrochemical and gas production machine equipment. The new technology complex developed has been certified by Gazprom company and used successfully with considerable economic effect at Gazprom daughter enterprises in refurbishing gaspumping units and compressor equipment.

The structure and mechanical properties of nickel alloy IN738LC with a level of high-temperature strength and corrosion resistance similar to that of domestic alloy 3MI-3U are studied after long-term operation and reconditioning heat treatment, i.e., homogenizing at different temperatures.

We have measured the reflection coefficients and threshold pump energies for phase conjugation via stimulated Brillouin scattering (SBS) in various chemical compounds and binary solutions. We have studied the possibility of controlling the parameters of SBS phase conjugation by purely chemical means.

The authors study the pharmacokinetics of dextrans with various molecular masses modified by fluorescein isothiocyanate (FITC) using a radioisotope method. The radionuclide /sup 125/I was selectively bound to a FITC residue attached to the polysaccharide by electrochemical iodination under potentiostatic conditions. In the experiments, dextrans modified by FITC were labeled with /sup 125/I (DF-/sup 125/I) by electrochemical iodination. The separation of DF-/sup 125/I and FITC from ionic forms of the radionuclide not bound to the polymer was carried out. The properties of the samples obtained are presented. The radioactivity accumulated in the rate organs and urine studied are shown. The features of DF-/sup 125/I behavior in the blood and liver are examined.

The quantitative content of nicotinic acid diethylamide in nikethamide and the physicochemical characteristics of the preparation, stabilized by the addition to the solution of 1% sodium sulfite and metabisulfite and radiation sterilized (γ irradiation with60Co, dose of 2.5 Mrd), corresponded with the requirements of the SP of the USSR after 3 years storage under the usual conditions. 2. The pharmacological activity and toxicity of stabilized nikethamide solution, irradiated with doses up to 2.5 Mrd, did not differ from the analogous properties of the unstabilized preparation during 3 years storage.

The method of treating injection solutions with ionizing radiation is of undoubted interest in connection with the ability of radiation to destroy microorganisms without raising the temperature of the main substance. This method is known in the literature by the name of the cold or radiation method of sterilization. The size of the sterilizing dose for injection solutions is taken as 25 kGy in [i] in the majority of cases. However, not all drugs by far may be subjected to the action of a sterilizing dose of ionizing radiation since radiation damage may occur in the irradiated solutions [2, 3, 4, 6].

The pharmacokinetics and metabolic features of I-radionuclide labelled derivatives of aliphatic carboxylic acid were studied in experiment. The authors show the possibilities of nuclear medicine in deriving information on the activity of fatty acid beta-oxidation in the animal liver and body using compounds with C4 = C11 aliphatic chain locus.

Modern radiopharmaceuticals are used to get information on the anatomy and function of the liquor system, i. e. they make it possible to monitor a radionuclide biological behavior in vivo by external detection. Complex compounds like diethylene triamine pentacetic acid (DTPA) labeled with 111In or 99mTc are the most common radionuclides used abroad to study the liquor system pathology. The clinicoexperimental studies on the use of the 111In-Ca-DTPA complex for radionuclide cisternography elaborated in this country, took 2 years. Experiments on animals showed that 70% of the administered amount of the radionuclide left the liquor pathways 4 h later. The administration of a clinical dose 5 times as high demonstrated the drug safety. The Soviet radiopharmaceutical was evaluated by comparing under clinical conditions 3 radionuclides: 111In-DTPA (France), 99mTc-DTPA (FRG) and 111In-Ca-DTPA (USSR). The radiological picture of radionuclide distribution by the liquor pathways did not depend on the drug. Radiological signs showed a direct correlation with a disease entity.

Scintigraphy following intravenous administration of 0.1% solution of 11-(ortho-iodine-131-benzoylamino) undecoic acid to rabbits in experimental cholestasis has shown that the intensity of beta-oxidation of this fatty acid in the liver decreases on the 2nd day of cholestasis. Its biological half-life in the liver significantly increases, at an average, 1.75-fold. On the 3rd day this value changes almost 3 times. The extraction of the acid by the liver changes from the first day of cholestasis. Disturbance of renal function from the 2nd day of cholestasis has been also noted. At the same time no significant differences in the time course of changes in the content of 131I-rose bengal have been detected in 1 and 7 days of cholestasis up to the death of part of the animals. The data obtained indicate a possibility of employing the radionuclide method of investigation of change in the intensity of beta-oxidation of fatty acids with the help of the above compound to assess a degree of hepatocytic affection in cholestasis.

The irradiation of hospital linen contaminated with radioresistant microorganisms or hospital microflora with gamma radiation in a dose of 10 kGy ensures the reliable microbial decontamination of such linen. Cotton linen has been found capable of withstanding 15 irradiation cycles in a dose of 10 kGy.

Changes in the antibioticograms after a single irradiation of dry cultures of microorganisms isolated at a pharmaceutical factory were noted in 0.5 per cent of 686 microbial strains tested. Restoration of sensitivity to one or more often several antibiotics was found in the microbes with the changed antibioticograms. After a three-fold irradiation the value increased to 9 per cent. Development of the drug resistance in the microbes neither after the single nor after the three-fold irradiation was observed.

The pharmacokinetics of 99mTc and 113mIn labeled aminopolymethyl phosphonic acids was studied in experiments on animals. Noticeable osteotropism of these compounds was shown. Scans of the rabbit and rat skeleton in health and experimental pathology 1 h after i.v. administration of these compounds were obtained making it possible to reveal the affected foci.

The extension of the Q-T interval of ECG can be accompanied by loss of consciousness and even sudden death. A unified approach is required to assess the normal duration of the Q-T interval. A table of maximum normal limits of the Q-T interval is given calculated with Bazett's formula with the coefficient K equal to 0.42. It is suggested to subdivide the extensions of the Q-T intervals according to the clinical manifestations with attacks of loss of consciousness and without them and also according to aetiology: congenital syndrome Jarvel-Lange-Nielsen and that of Romano-Ward, the acquired acute (intoxications, disorders of electrolyte balance, lesions of the central nervous system, myocardial infarction, myocarditis, medicinal reactions) and the acquired chronic (diffuse lesions of the myocardium, of the brain etc.). Personal observations of syndromes of extended Q-T intervals of different aetiology are given with attacks of ventricular tachycardia of the "torsade de pointes" type and ventricular fibrillation.
