PURPOSE. To compare hypotensive efficiency and safety of generic latanoprost 0.005 % (Glaumax, “KEVELT AS”, Estonia) and original latanoprost 0.005 % (Xalatan, “Pfizer Inc”, USA) in glaucoma patients. METHODS. The study included patients with newly diagnosed primary open-angle glaucoma or patients with primary open-angle glaucoma on carbonic anhydrase inhibitor monotherapy 4 days after cease of treatment (the “wash-out” period). Study patients were divided into 2 groups. Group 1 consisted of 25 patients (30 eyes) with initial and advanced stages of newly diagnosed POAG and initial corneal-compensated IOP (IOPcc) of 25.15 ± 1.48 mm Hg. This group of patients was prescribed instillations of 0.005 % Glaumax eye drops 1 time per day. Group 2 also included 25 patients (30 eyes) with initial stage of newly diagnosed POAG and initial corneal-compensated IOP (IOPcc) of 25.08 ± 1.42 mm Hg. Group 2 patients were prescribed instillations of 0.005 % Xalatan eye drops 1 time per day. IOPcc was measured initially after the washout period, then 1 and 2 months after the start of the new therapy. Additionally, all patients were assessed for tear film break up time (Norn’s test) at each follow-up point. RESULTS. After 1 month, IOPcc in groups 1 and 2 was 17.36 ± 1.39 and 18.09 ± 1.45 mm Hg, respectively; after 2 month, 17.87 ± 1.30 and 18.54 mm Hg, respectively. The Norn’s test results in groups 1 and 2 after 1 month were 8 (7; 9.75) and 8 (6; 9.75) seconds, after 2 months it was 8.5 (8; 9) and 8 (7; 10) seconds, respectively, which corresponded to the initial stage of the "dry eye" syndrome. No statistically significant intergroup differences were found in any of the cases. CONCLUSION. Xalatan 0.005 and Glaumax 0.005 have comparable efficacy and tolerability, and both help achieve persistent stabilization of IOP. The option for a single-instillation regimen is convenient for the patients.

To evaluate the efficacy and safety of intramuscular Retinalamin for retinoprotection in patients with open-angle glaucoma and normalized intraocular pressure (IOP).The study included 180 patients (355 eyes) randomized into the main (n=90) and control groups (n=90). The patients of the main group received intramuscular Retinalamin injections; the course was repeated 6 months later. Patient examination was performed at 1, 3, 6, 7, 9 and 12 months.Vision acuity did not change in the main group after the treatment courses (p=0.3732, p=0.6862), nor in the control group (p=0.7751). IOP didn't have significant changes during the whole course of the study neither in the main group (p=0.7632), nor in the control group (p=0.3921). MD index in the main group has increased from -5.52±2.76 to -4.82±2.73 dB (measurements from 6 visits: p=0.0391, p=0.0201, p=0.0302, p=0.3708, p=0.0151, p=0.0353). Control group showed negative MD trend (from -3.51±1.84 to -4.60±2.61 dB; p=0.0012). PSD index has changed from 4.63±1.60 to 4.05±1.43 dB (p=0.0081) in the main group, and from 3.73±1.19 to 4.29±1.53 dB (p=0.0027) in the control group. Average thickness of retinal nerve fiber layer (RNFL) and the volume of neuroretinal rim were stable in both the main (p=0.8039, p=0.9005) and the control groups (p=0.7448, p=0.9620). Ganglion cell complex (GCC) thickness remained stable in the main group (p=0.0377), but has decreased in the control group (p=0.0250). P50 amplitude and latency were stable in the main group (6.54±2.61-6.53±2.64 µV, p=0.0479; 48.39±3.69-50.86±4.09 ms, p=0.0271), while in the control group P50 amplitude has decreased (p=0,0031) and the latency has increased (p=0,0194). In the main group, N95 amplitude has stabilized (p=0.0141) with worsened latency (p=0.0492). N95 amplitude in the control group has worsened (p=0.0195), while latency has stabilized (p=0.3401).Systemic use of Retinalamin has significant retinoprotective effect confirmed by the dynamics of morphological and functional parameters in patients with POAG and IOP compensation.Цель - оценить эффективность и безопасность применения Ретиналамина с целью ретинопротекции у пациентов с первичной открытоугольной глаукомой (ПОУГ) с нормализованным внутриглазным давлением (ВГД) при внутримышечном введении. Материал и методы. В исследовании приняли участие 180 пациентов (355 глаз), рандомизированных в основную (n=90) и контрольную (n=90) группы. Пациентам основной группы проводили курс внутримышечных инъекций Ретиналамина; курс повторяли через 6 мес. Обследование осуществляли на 1, 3, 6, 7, 9 и 12-м месяце. Результаты. В основной группе острота зрения после курсов лечения не изменилась (p=0,3732, p=0,6862). В контрольной группе изменений также не наблюдалось (p=0,7751). Уровень ВГД не изменился ни в основной (p=0,7632), ни в контрольной группе (p=0,3921). В основной группе индекс MD достоверно увеличился с –5,52±2,76 до –4,82±2,73 дБ (ряд изменений на 6 визитах: p=0,0391, p=0,0201, p=0,0302, p=0,3708, p=0,0151, p=0,0353). В контрольной группе наблюдалась отрицательная динамика (от –3,51±1,84 до –4,60±2,61 дБ; p=0,0012). Индекс PSD в основной группе изменился с 4,63±1,60 до 4,05±1,43 дБ (p=0,0081), в контрольной - с 3,73±1,19 до 4,29±1,53 дБ (p=0,0027). Средняя толщина нервных волокон сетчатки и объем нейроретинального пояска не изменялись ни в основной (p=0,8039, p=0,9005), ни в контрольной (p=0,7448, p=0,9620) группе. Комплекс ганглиозных клеток сетчатки стабилизировался в основной (p=0,0377), истончался в контрольной (p=0,0250) группе. В основной группе амплитуда и латентность Р50 стабилизировались (6,54±2,61-6,53±2,64 мкВ, p=0,0479; 48,39±3,69-50,86±4,09 мс, p=0,0271). В контрольной группе амплитуда Р50 снижалась (p=0,0031), латентность возрастала (p=0,0194). В основной группе амплитуда N95 стабилизировалась (p=0,0141), латентность снизилась (p=0,0492). В контрольной группе ухудшалась амплитуда N95 (p=0,0195), латентность стабилизировалась (p=0,3401). Заключение. Системное применение Ретиналамина оказывает достоверный ретинопротекторный эффект, подтверждаемый динамикой морфофункциональных показателей при ПОУГ с нормализованным ВГД.

A great number of patients with primary open-angle glaucoma require combination therapy with several hypotensive drugs of different pharmacological groups to efficiently compensate the intraocular pressure (IOP) and prevent further development of glaucoma. Although glaucoma treatment usually starts with monotherapy, several preparations administered simultaneously have stronger hypotensive effect further increased by their additive action. However, the increased frequency of instillations may lead to drug washout and decreased patient compliance. Fixed dose combinations (FDCs) are a pharmacological form of eye drops that combines two or more active drugs in a single dosage form. The frequency of FDCs administration does not exceed two per day, which is convenient for long-term application and helps improve patient compliance. FDCs tolerability tends to be higher than that of simultaneous instillations of separate monotherapy drugs, while the hypotensive in clinical trials is comparable, in some real practice trials is superior. A review of existing studies showed that FDCs are superior to monotherapy in terms of cost-effectiveness. The use of FDCs was found to be beneficial for glaucoma treatment from the viewpoints of both reaching the target IOP for preserving visual functions in glaucoma patients, and minimizing the long-term treatment costs.Многим пациентам, принимающим гипотензивные препараты с целью компенсации внутриглазного давления (ВГД) и лечения глаукомы, необходима комбинация препаратов разных фармакологических групп. Несмотря на то что лечение глаукомы начинается с монотерапии, несколько препаратов оказывают больший гипотензивный эффект и аддитивное действие друг на друга. Тем не менее увеличение кратности инстилляций приводит к эффекту вымывания и снижению приверженности к лечению у пациентов. Фиксированные комбинации являются фармакологической формой глазных капель, которая предполагает содержание в одном флаконе нескольких действующих веществ. При использовании фиксированных комбинаций частота закапывания не превышает двух раз, что удобно при длительном применении и увеличивает приверженность к лечению. Переносимость фиксированных комбинаций лучше, чем сочетанное применение монопрепаратов. При этом в клинических исследованиях они дают сравнимый, а по данным исследований реальной практики превосходящий гипотензивный эффект. Обзор имеющихся исследований свидетельствует о лучшей экономической эффективности фиксированных комбинаций по сравнению с монопрепаратами. С позиции достижения целевого ВГД для сохранения зрительных функций у пациентов с глаукомой и оптимизации расходов на длительное лечение, применение фиксированных комбинаций является важнейшим аспектом лечения глаукомы.

To study the effectiveness of monotherapy with bimatoprost in patients with decompensated intraocular pressure (IOP).90 patients (132 eyes) with stage I-II glaucoma and decompensated IOP previously treated with timolol, latanoprost and travoprost were included in the study. Average IOP at the beginning of the study was 25.4±2.5 mmHg. All patients had their hypotensive medications substituted by bimatoprost. The patients were examined during the initial appointment, as well as after 4 and 12 weeks.IOP decrease was highest in the group of patients who had been treated with timolol: initially IOP was 26.2±1.8 mmHg, after 4 weeks - 21.0±2.2 mmHg, after 12 weeks - 20.8±1.9 mmHg (p0.001). The initial IOP of patients who had been treated with latanoprost was 24.8±2.9 mmHg, 21.8±2.4 mmHg after 4 weeks, and 21.6±2.3 mmHg after 12 weeks (p0.001). Patients who had been treated with travoprost had 25.6±2.2 mmHg initially, 23.0±2.5 mmHg after 4 weeks, and 23.2±2.6 mmHg after 12 weeks (p0.001). By the end of the study IOP has decreased by 5.4, 3.2 and 2.4 mmHg in the groups of patients who had been treated with timolol, latanoprost and travoprost, respectively.Bimatoprost can be used as monotherapy if another hypotensive drug in monotherapy is insufficient for IOP compensation.Цель - изучить эффективность монотерапии биматопростом у пациентов с декомпенсированным внутриглазным давлением (ВГД). Материал и методы. В исследование включено 90 пациентов (132 глаза) с глаукомой I-II стадии и декомпенсированным ВГД, принимающих тимолол, латанопрост или травопрост. Средний уровень ВГД на момент начала исследования составлял 25,4±2,5 мм рт.ст. Всем больным заменяли инстиллируемый препарат на биматопрост. Пациентов наблюдали на первом визите, через 4 и 12 нед. Результаты. В группе пациентов, изначально принимавших тимолол, снижение уровня ВГД оказалось наиболее выраженным: перед началом лечения значения ВГД составляли 26,2±1,8 мм рт.ст., через 4 нед - 21,0±2,2 мм рт.ст., через 12 нед - 20,8±1,9 мм рт.ст (p0,001). У пациентов, принимавших латанопрост, начальный уровень ВГД составил 24,8±2,9 мм рт.ст., через 4 нед - 21,8±2,4 мм рт.ст., через 12 нед - 21,6±2,3 мм рт.ст (p0,001). В группе травопроста изначально показатели ВГД составляли 25,6±2,2 мм рт.ст., спустя 4 нед - 23,0±2,5 мм рт.ст., через 12 нед - 23,2±2,6 мм рт.ст. (p0,001). К концу исследования снижение уровня ВГД в группах пациентов, ранее принимавших тимолол, латанопрост и травопрост, составило 5,4; 3,2 и 2,4 мм рт.ст. соответственно. Заключение. Биматопрост может применяться в монотерапии при недостаточно эффективной монотерапии другим антиглаукомным препаратом.
