Abstract Background The efficacy of low-dose medicine (LDM) in childhood mild/moderate eczema is not known. We conducted a double-blind, two-stage, randomized, placebo-controlled clinical trial, lasting 23 months, to address this issue. Method Eighty children with chronic mild/moderate eczema were randomly allocated to Group A (placebo) or Group B (treatment group; Galium-Heel®, a low-dose multicomponent medicine based upon natural substances; Guna-Interleukin 12 and Guna-Interferon-γ administered twice a day for six non-consecutive months for each stage). LDM is characterized by the use of biological molecules, such as cytokines, neuropeptides, growth factors, hormones at very low concentrations, which correspond to physiological levels within the human body. The dosage of the cytokines used in this trial (IFN-γ and IL-12) is 10 fg/ml. The SCORAD index was evaluated by the same operator: subjects with a SCORAD index below 20 were considered to have mild eczema (61/80; mean: 10.79), whereas a SCORAD index between 20–50 indicated moderate eczema (19/80; mean: 26.84). The data of 66/80 children were analyzed in stage 1 and those of 62/66 children in stage 2. The primary outcome measure was reduction of eczema severity assessed by the SCORAD index. Secondary outcomes were disease-free interval, and treatment safety and tolerability. Results The decrease in disease severity was greater in Group B than in Group A already in stage 1 (a decrease 63.9% versus 53.2%), but the difference was not significant (p = 0.16). Moreover, subjective symptoms (itching and sleep disturbances) initially decreased and then worsened in Group A, whereas itching decreased linearly and sleep disturbances decreased significantly (p=0.049) in Group B. Conclusions Preliminary evidence suggests potential benefit, but further work is needed to validate this approach. Trial registration The trial was registered with EudraCT number 2010–018640-13 through the database of the National Clinical Trials Monitoring Centre Database (Osservatorio delle Sperimentazioni Cliniche, OsSC) of the Italian Medicines Agency.

Abstract Background Mycoplasma genitalium (MG), a sexually transmitted infection (STI), has emerged as a common cause of non-gonococcal urethritis and cervicitis worldwide, with documented resistance to commonly used antibiotics including doxycycline and azithromycin. Data in Ghana regarding the prevalence of MG is limited. Methods This retrospective study investigated MG presence and macrolide resistance among patients who previously reported to selected clinics for STI symptoms between December 2012 and June 2020. Samples were screened for MG and mutations associated with azithromycin resistance were investigated using Nucleic Acid Amplification Testing (NAAT) including the Resistance Plus MG® kit from SpeeDx and the LightMix® kit for MG, combined with the Modular Mycoplasma Macrolide from TIB Molbiol. Results A total of 1,015 samples were screened, out of which MG infection rate by TIB Molbiol and SpeeDx were 3.1% and 3.4%, respectively. The mutation responsible for macrolide resistance was detected in one MG positive sample by both assays. Both diagnostic tests revealed no significant association between MG infection and socio-demographic characteristics, clinical symptoms, gonorrhea, and chlamydia infection status. There was no significant difference in the mycoplasma percentage positivity rate detected using SpeeDx (3.4%) and TIB Molbiol (3.1%). Conclusions While not commonly tested as a cause of STI symptoms, MG is widespread in Ghana, exhibiting symptoms and prevalence comparable to those in other countries and linked to antimicrobial resistance. Future research using various molecular techniques is essential to monitor resistance trends and guide future antibiotic choices.

Acute gastroenteritis caused by viruses is one of the leading causes of infantile morbidity. The aim of the present study was to investigate the presence of human caliciviruses of the genera norovirus and sapovirus in children up to 3 years of age with acute gastroenteritis from low-income communities in the city of Salvador, Brazil. This study is an extension of previous work carried out to establish the profile of the most prevalent enteric pathogens present in these communities. In this report, 139 fecal samples, collected from July 2001 to January 2002 were analyzed by RT-PCR and 13 (9%) were positive for human caliciviruses. By sequencing, seven isolates were characterized as norovirus genogroup GII and one as sapovirus genotype GII/1. Sequencing of the previously detected group-A rotaviruses and human astroviruses was also performed and revealed the circulation of rotavirus group A genotypes G1P[8] and G9P[8], and human astrovirus genotypes 6, 7, and 8. No mixed infection was observed. Community-based studies provide geographically representative information on disease burden. However, there are only a few reports in developing countries concerning the genotypes of the most important gastroenteric viruses detected in such communities. The present findings demonstrate the wide diversity of genotypes of the most important viruses responsible for acute gastroenteritis circulating in low-income communities.

Toll-like receptors (TLRs) play a key role in the response of innate and adaptive immune system to microbial and endogenous ligands. Inflammation is a common feature in end-stage renal disease (ESRD) patients; however, the mechanisms/factors triggering the inflammatory process are still poorly clarified. Our aim was to analyze the impact of the c.-1486T>C and c.896A>G polymorphisms in TLR9 and TLR4 genes, respectively, on the inflammatory response of ESRD patients. Clinical and laboratory evaluation was carried out on 184 ESRD patients. Polymerase chain reaction followed by restriction fragmens length polymorphisms (PCR-RFLP) was employed for genotyping of TLR-4 c.896A>G and TLR-9 c.-1486T>C polymorphisms. The prevalence of AA and AG of TLR4 c.896A>G polymorphism in ESRD patients was 97.8% and 2.2%, respectively. None of the individuals showed a homozygous TLR4 polymorphism. Concerning the TLR9 c.-1486T>C polymorphism, we found that ESRD patients showed a prevalence of TC and CC genotypes of 57.1% and 20.6%, respectively. We found that the heterozygous patients for the TLR4 c.896A>G polymorphism presented an increased level in lymphocyte count, a decrease in neutrophil/lymphocyte ratio and in serum levels of hepcidin. Regarding the TLR9 c.-1486T>C polymorphism, we found that it is associated with decreased white blood cell and neutrophil counts, ferritin and CRP serum levels, and with an increase in serum levels of creatinine. Our data suggest that the presence of the studied polymorphisms is associated with a decreased inflammatory response in ESRD patients under hemodialysis, and, thus its presence might have beneficial effects in ESRD patients. Moreover, our data provide new insights in the role of TLR polymorphisms in renal disease, which might have impact in the near future for the development of innovative therapies.

The Amazon basin plays a crucial role in the global hydrological cycle and the climate system. Removal of latent heat from the surface covered by the tropical forest through evapotranspiration is a key process that still requires further research due to the complex nature of the involved processes, lack of observations and different model assumptions. Here we present an assessment of the consistency between different latent heat fluxes datasets through an indirect comparison against the daily amplitude of surface temperature and vegetation status estimated from satellite observations. Our study is based on the hypothesis that the observational satellite data can be used to provide hints on how realistically fluxes are represented in different datasets. Results evidence that datasets diverge inside the basin in both space and time, but it is possible to figure out areas under water-limited conditions, especially around the borders of the basin and some regions over eastern/southeastern Amazonia. In despite of these differences, a clear link between daily amplitude of surface temperature, leaf area index and latent heat flux can be observed over particular areas and seasons, where also correlations reach values closer to −0.98 (0.94) for surface temperature (leaf area index) indicating that satellite observations are suitable for assessing the representation of the partitioning of energy fluxes in models and widely used datasets.

IntroductionGonorrhoea is a major public health concern. With the global emergence and spread of resistance to last-line antibiotic treatment options, gonorrhoea threatens to be untreatable in the future. Therefore, this study performed whole genome characterization of Neisseria gonorrhoeae collected in Ghana to identify lineages of circulating strains as well as their phenotypic and genotypic antimicrobial resistance (AMR) profiles.MethodsWhole genome sequencing (WGS) was performed on 56 isolates using both the Oxford Nanopore MinION and Illumina MiSeq sequencing platforms. The Comprehensive Antimicrobial Resistance Database (CARD) and PUBMLST.org/neisseria databases were used to catalogue chromosomal and plasmid genes implicated in AMR. The core genome multi-locus sequence typing (cgMLST) approach was used for comparative genomics analysis.Results and DiscussionIn vitro resistance measured by the E-test method revealed 100%, 91.0% and 85.7% resistance to tetracycline, penicillin and ciprofloxacin, respectively. A total of 22 sequence types (STs) were identified by multilocus sequence typing (MLST), with ST-14422 (n = 10), ST-1927 (n = 8) and ST-11210 (n = 7) being the most prevalent. Six novel STs were also identified (ST-15634, 15636-15639 and 15641). All isolates harboured chromosomal AMR determinants that confer resistance to beta-lactam antimicrobials and tetracycline. A single cefixime-resistant strain, that belongs to N. gonorrhoeae multiantigen sequence type (NG-MAST) ST1407, a type associated with widespread cephalosporin resistance was identified. Neisseria gonorrhoeae Sequence Typing for Antimicrobial Resistance (NG-STAR), identified 29 unique sequence types, with ST-464 (n = 8) and the novel ST-3366 (n = 8) being the most prevalent. Notably, 20 of the 29 STs were novel, indicative of the unique nature of molecular AMR determinants in the Ghanaian strains. Plasmids were highly prevalent: pTetM and pblaTEM were found in 96% and 92% of isolates, respectively. The TEM-135 allele, which is an amino acid change away from producing a stable extended-spectrum β-lactamase that could result in complete cephalosporin resistance, was identified in 28.5% of the isolates. Using WGS, we characterized N. gonorrhoeae strains from Ghana, giving a snapshot of the current state of gonococcal AMR in the country and highlighting the need for constant genomic surveillance.

Background: Our study presents the first assessment of the relationships between environmental heterogeneity and the palm community at the landscape scale at the northern boundary of tropical rainforest distribution in America. Question: How does landscape-level heterogeneity (edaphic, topographic as well as the density, diversity, and tree biomass) shapes the palm community in a protected reserve affected by defaunation? Study site and date: The study was carried out in Los Tuxtlas Tropical Biology Station, Veracruz, Mexico (2020). Methods: We delineated landscape units (LUs) based on soil and geomorphology to characterize the biophysical mosaic. We built rank-abundance curves to compare the palm richness and their abundance among LUs. Permutational multivariate analysis of variance (PERMANOVA) and nonmetric multidimensional scaling (NMDS) were applied to detect shifts in the palm community across LUs. Results: Integrating abiotic and biotic components by delimiting LUs allowed us to evaluate the multifactorial effect of environmental heterogeneity on the palm community. The density and composition of the palms changed significantly among the LUs, influenced by soil coarse fragments, elevation, slope inclination, and annual precipitation. Tree richness and density had a significant effect on the palm community only when analyzed in combination with the soil-topography variables. Conclusions: The results reveal that there is high edaphic and topographic heterogeneity that influences palm species distribution at the landscape scale. The environmental heterogeneity approach allows capturing novel abiotic variation to explain the distribution patterns of palm species and their coexistence in highly diverse tropical communities.

Oil spill detection and mapping using deep learning (OSDMDL) is crucial for assessing its impact on coastal and marine ecosystems. A novel approach was employed in this study to evaluate the scientific literature in this field through bibliometric analysis and literature review. The Scopus database was used to evaluate the relevant scientific literature in this field, followed by a bibliometric analysis to extract additional information, such as architecture type, country collaboration, and most cited papers. The findings highlight significant advancements in oil detection at sea, with a strong correlation between technological evolution in detection methods and improved remote sensing data acquisition. Multilayer perceptrons (MLP) emerged as the most prominent neural network architecture in 11 studies, followed by a convolutional neural network (CNN) in 5 studies. U-Net, DeepLabv3+, and fully convolutional network (FCN) were each used in three studies, demonstrating their relative significance too. The analysis provides insights into collaboration, interdisciplinarity, and research methodology and contributes to the development of more effective policies, strategies, and technologies for mitigating the environmental impact of oil spills in OSDMDL.

The effective reproduction number, [Formula: see text], plays a key role in the study of infectious diseases, indicating the current average number of new infections caused by an infected individual in an epidemic process. Estimation methods for the time evolution of [Formula: see text], using incidence data, rely on the generation interval distribution, g(τ), which is usually obtained from empirical data or theoretical studies using simple epidemic models. However, for systems that present heterogeneity, either on the host population or in the expression of the disease, there is a lack of data and of a suitable general methodology to obtain g(τ). In this work, we use mathematical models to bridge this gap. We present a general methodology for obtaining explicit expressions of the reproduction numbers and the generation interval distributions, within and between model sub-compartments provided by an arbitrary compartmental model. Additionally, we present the appropriate expressions to evaluate those reproduction numbers using incidence data. To highlight the relevance of such methodology, we apply it to the spread of COVID-19 in municipalities of the state of Rio de Janeiro, Brazil. Using two meta-population models, we estimate the reproduction numbers and the contributions of each municipality in the generation of cases in all others.

Objective: To describe the characteristics of children and adolescentes with chronic diseases of outpatient clinics at a tertiary university hospital. Methods: A cross-sectional study was performed with 16,237 patients with chronic diseases followed-up in one year. The data were collected through the electronic system, according to the number of physician appointments in 23 pediatric specialties. Patients were divided in two groups: children (0–9 years) and adolescents (10–19 years). Early (10–14 years) and late (15–19 years) adolescent groups were also analyzed. Results: Of the total sample, 56% were children and 46% were adolescents.
