Background More than 30 years have passed since the first laparoscopic inguinal hernioplasty but nowadays less than 5% of patients are currently operated by this technique as recent Spanish national data suggest. Objective To present our experience 2 years after implementation of laparoscopic inguinal hernioplasty in our local hospital and highlight the challenges we faced. Methods Our hospital deals with a high volume of abdominal wall surgery and has more than 20 years experience in ambulatory surgery. A multidisciplinary clinical pathway was implemented. Strict selection criteria were established and pelvitrainer sessions were done. Results From January 2021 until December 2022, 196 laparoscopic repairs were performed: 167 transabdominal preperitoneal (TAPP), 29 totally extraperitoneal (TEP). The main challenge was the learning curve. 5 surgeons with experience in laparoscopic surgery were trained. Until now 2 completed their learning curve for TAPP and began training in TEP approach while the others still need senior assistance. The duration of laparoscopic hernioplasty, the need for deep relaxation and the increased carbon dioxide absorption were the biggest concerns for the Anesthesia team. The cost of new materials could be afforded, even though different sutures and types of mesh fixation devices were used until getting the non-invasive one. Surgical outcomes were good, with few minor complications. Moreover the outpatient rate could be maintained but the average time in waiting list for laparoscopic approach was increased. Conclusion The implementation of a laparoscopic hernioplasty program is feasible and safe but faces many challenges, especially due to long learning curve.

Background Lateral incisional hernia surgery represents an underappreciated challenge for general surgeons. McBurney's incisional hernia (L3) after appendectomy is rare, with an incidence between 0.1 and 0.9%. Several studies have found fewer postoperative complications with the laparoscopic approach compared with open approach. Objective To show a video of a transabdominal preperitoneal (TAPP) approach for McBurney's incisional hernia associated with a small right lateral inguinal hernia. Method We report a case of a 52 years old man, with previous open appendectomy in 2013, diagnosed with McBurney's incisional hernia (L3W2R0) and small right inguinal hernia (L1M0F0P). The patient was scheduled for laparoscopic TAPP incisional hernia repair. Results The patient was operated as ambulatory surgery with no intraoperative complications. No recurrence nor other complications detected during short, 4th month, follow up after surgery. Conclusions Minimal invasive hernia repair with preperitoneal mesh of lateral abdominal hernias cause less postoperative pain, have a short hospital stay, lower incidence of surgical site and mesh infection, and possibly can reduce recurrence as a wider mesh overlap of the defect is possible. This technique has shown to be feasible and reproducible in selected cases.

A pecuária leiteira orgânica é considerada incipiente no Brasil e sua expansão representa uma grande oportunidade para os produtores, visto o potencial de produção do país e o valor agregado ao produto, que é superior ao convencional. A demanda por informações sobre o setor dificulta a formulação de políticas públicas e a adesão de novos produtores ao sistema de produção. Na pecuária orgânica, a produção do leite deve seguir as técnicas de produção regulamentadas pela legislação vigente, instituída pelo Ministério da Agricultura, Pecuária e Abastecimento (MAPA), com garantia da sua qualidade e rastreabilidade por meio da certificação. Compreender a regulamentação é um desafio para os produtores orgânicos e, por outro lado, é imprescindível direcionar o manejo e uso de insumos com base na legislação para atender ao regulamento técnico e garantir que os processos se adequem aos princípios da Agricultura Orgânica. A regulamentação nacional para produção orgânica se mantém em processo de aprimoramento contínuo por meio de atualizações. Dessa forma, é importante que os produtores se mantenham atualizados. Objetiva-se apresentar o cenário atual da produção de orgânicos no Brasil, com enfoque na pecuária leiteira orgânica e sua regulamentação, de acordo com a legislação vigente.

Aim The Spanish Incisional Hernia Surgery Registry (EVEREG) is promoted by the Abdominal Wall section of the Spanish Association of Surgeons, starting data collection in July 2012 and currently has more than 13,500 cases. The objective of this study is to validate the data collected through a pilot audit process Material & Methods A sample of hospitals participating in the EVEREG registry since 2012 is selected. Patients registered in these centers in the period 2012–2019 are included. A stratified random sampling is carried out, with the inclusion of 10% of registered cases per center with a minimum of 20. In the 2020–2021 period, a selection of researchers, different from that of the center to be audited, checks the concordance between the registry data and the data collected in the patient's clinical history, either in person or online. Results 330 patients have been analyzed, out of a total of 2673 registered, in 9 participating centers. The average accuracy has been 95.7% (99.1–79.35%). With 1.5% incorrect data and 2.28% missing data Conclusions This is the first validity study of an abdominal wall registry The accuracy greater than 95% obtained allows us to determine an excellent quality in the data collection of the EVEREG registry. We believe that the confirmation of these data in all the centers participating in the registry would guarantee the quality of the studies carried out and their comparison with other international registries

Context. The globular cluster (GC) system of Circinus galaxy has not been probed previously partly because of the location of the galaxy at - 3.8$^\circ$ Galactic latitude which suffers severely from interstellar extinction, stellar crowding, and Galactic foreground contamination. However, the deep near-infrared (NIR) photometry by the VISTA Variables in the Via L\'actea Extended Survey (VVVX) in combination with the precise astrometry of Gaia EDR3 allow us to map GCs in this region. Aims. Our long-term goal is to study and characterise the distributions of GCs and Ultra-compact dwarfs of Circinus galaxy which is the nearest Seyfert II galaxy. Here we conduct the first pilot search for GCs in this galaxy. Methods. We use NIR VVVX photometry in combination with Gaia EDR3 astrometric features such as astrometric excess noise and BP/RP excess factor to build the first homogeneous catalogue of GCs in Circinus galaxy. A robust combination of selection criteria allows us to effectively clean interlopers from our sample. Results. We report the detection of$\sim$ 70 GC candidates in this galaxy at a 3 $\sigma$ confidence level. They show a bimodal colour distribution with the blue peak at (G-Ks)$_0$ = 0.985$\pm$0.127 mag with a dispersion of 0.211$\pm$0.091 mag and the red peak at (G-Ks)$_0$ = 1.625$\pm$0.177 mag with a dispersion of 0.482$\pm$0.114 mag. A GC specific frequency (S$_N$) of 1.3$\pm$0.2 was derived for the galaxy, and we estimated a total population of 120$\pm$40 GCs. Based on the projected radial distribution it appears that Circinus has a different distribution of GC candidates than MW and M31. Conclusions. We demonstrate that Circinus galaxy hosts a sizeable number of cluster candidates. A contaminação de alimentos por microrganismos patogênicos ou deteriorantes, como fungos e bactérias, é um problema importante para a indústria, pois leva a defeitos sensoriais, desperdício alimentar e econômico, além da possibilidade de causar problemas de saúde para o consumidor final. Atualmente uma grande variedade de conservantes químicos vem sendo usada pelas indústrias e pelos pequenos produtores para retardar a deterioração dos alimentos, contudo, tais compostos tendem a não serem bem aceitos pelos consumidores, os quais estão à procura de produtos mais naturais. Deste modo, este trabalho visa investigar a possibilidade das bactérias ácido lática (BAL) serem usadas como bioconservantes. Foram analisados os mais relevantes estudos publicados relacionados ao tema proposto originalmente na língua inglesa nos últimos 5 anos, tendo como referência a base de dados MedLine (National Library of Medicine). Este estudo demonstrou por meio de diversas fontes científicas que linhagens selecionadas de BAL podem apresentar propriedades biotecnológicas que as qualificam para serem usadas como bioconservantes.

Bactérias ácido láticas (BAL), por possuírem diversas propriedades funcionais, têm recebido destaque no cenário científico e tecnológico por sua aplicabilidade nas áreas farmacêutica e alimentícia. Um gargalo importante para exploração biotecnológica destes microrganismos está relacionado ao elevado custo para processamento e manutenção da viabilidade das células por longos períodos (cadeia de frios e/ou liofilização). O processo de secagem por atomização se apresenta como uma alternativa economicamente viável para a produção desses microrganismos como bioinsumo em escala industrial, entretanto, é um processo altamente linhagem dependente. O objetivo deste trabalho foi prospectar linhagens de BAL resistentes ao estresse térmico visando à identificação de microrganismos mais propensos a sobreviver ao processo de secagem por atomização. Cem linhagens de BAL foram submetidas ao estresse térmico de 60 °C ± 1 °C por 5 min e em seguida a taxa de sobrevivência foi avaliada por meio da técnica de microgota. Com reduções logarítmicas variando de 0,28 a 1,96 e contagens de 7,47 log UFC/mL a 8,92 log UFC/mL, 20 BAL foram capazes de resistir ao tratamento térmico e serão capazes de prosseguir para etapas seguintes de estudos visando futura aplicação industrial.

Resumen La atresia vaginal distal es una anomalia infrecuente que deriva de la falta de desarrollo de los 2/3 distales de la vagina a partir del seno urogenital. Presentamos una paciente de 14 anos con amenorrea primaria, abdominalgia, masa palpable en hipogastrio y ausencia de orificio vaginal. La ecografia abdominal y la RMN evidenciaron hematometrocolpos secundario a agenesia vaginal distal, con una distancia al perine de 5 cm, sin otras malformaciones. Se realiza un drenaje vaginal transuretral y descenso via perineal asistido por laparoscopia. Evolucion favorable con calibre vaginal adecuado y normalizacion de la menstruacion a los 6 meses.

En este trabajo presentamos la fabricación de nanopartículas de plata (Ag-NPs) por medio de ablación láser (AL) en medio acuoso, y su aplicación como elementos de realce de señales Raman a través de procesos tipo SERS. Esta metodología permite obtener suspensiones estables de NPs libres del uso de procesos químicos, con dispersiones de tamaños relativamente pequeñas y de fácil procesamiento para la implementación de sustratos SERS de sílice. A través del uso de alícuotas a diferentes concentraciones de Rodamina 6G (R6G), se demuestra que con estas Ag-NPs se alcanzan umbrales de detección de señal Raman en concentraciones del orden de nM. El estudio de la distribución de las Ag-NPs sobre un sustrato de sílice se llevó a cabo por microscopía de fuerza atómica (AFM). A su vez, se presentan algunos resultados de simulación de la magnitud de los campos generando hot spot en regiones específicas de las Ag-NPs tomando en cuenta la morfología de algunos de los aglomerados identificados por microscopía de barrido electrónico (SEM) en los sustratos SERS. En comparación con algunos reportes previos, el presente trabajo demuestra bajo umbral de detección e implementación sencilla en sustratos SERS.

SUMMARYThis article describes the clinical technique of using the transparent plastic tips of the LM-Contact Former system for obtaining tight proximal contacts in direct Class II composite resin restorations using noncontoured circumferential matrix bands.With this technique, the composite resin is pressed with the tip, which adapts intimately to the walls of the proximal box. Moreover, the total working time is reduced by filling the cavity, which remains after removing the transparent tip, with a bulk-fill flowable resin-based composite.

Recent scientific evidence and the incorporation of new drugs into the therapeutic arsenal against rosacea have made it necessary to review and update treatment criteria and strategies. To this end, a panel of 15 dermatologists, all experts in rosacea, was formed to share experiences and discuss treatment options, response criteria, and changes to treatment. Based on a critical review of the literature and a discussion of the routine practices of Spanish dermatologists, the panel proposed and debated different options, with consideration of the experience of professionals and the preferences of patients or equality criteria. Following validation of the proposals, the final recommendations were formulated and, together with the evidence from the main international guidelines and studies, used to produce this consensus document. The goal of this consensus document is to provide dermatologists with practical recommendations for the management of rosacea. Las últimas evidencias científicas y la incorporación de nuevos fármacos al arsenal terapéutico de la rosácea hacen necesario revisar y actualizar los criterios y estrategias de tratamiento. Con este fin, un grupo de 15 dermatólogos expertos en esta enfermedad aportaron y discutieron acerca de las diferentes terapias y los criterios de respuesta y cambio de tratamiento. Partiendo de la revisión crítica de la bibliografía y de la exposición de los hábitos de los dermatólogos españoles en su práctica clínica, se formularon distintas propuestas que fueron debatidas teniendo en consideración tanto la experiencia profesional como las preferencias de los pacientes o los criterios de equidad. Una vez validadas las propuestas, se formularon las recomendaciones finales que, junto con la evidencia aportada por las principales guías y estudios internacionales, dieron lugar al presente documento. El objetivo de este consenso es ofrecer al dermatólogo un enfoque práctico para abordar la rosácea.
