The article provides a practical example of the positive experience of the students of the medical academy in the international social project on combating arterial hypertension in order to improve the level of general competence and effective intellectual and professional socialization in getting higher medical education. The phenomenon of professional socialization should be considered along with the concepts of professional orientation, professional development, professional adaptation and professional becoming. Professional socialization is the ultimate link in the professional way of the individual. The algorithm of professional socialization of students in higher education consists of the following stages: professional orientation → professional development → professional adaptation → professional becoming → professional socialization. It should be noted that a personality who successfully socializes, has a well-defined social orientation, actively strives to self-determination and self-realization in society and is able of productive adaptation to the surrounding society on the basis of knowledge about social reality. In this case, the success of socialization is connected not only with the presence of specified qualities in the person, but, first of all, with the real actions in which these qualities are realized. By mastering the roles system, students acquire socially significant qualities, develop their worldview, their goals, motives, interests, feelings, personal and socially important needs, develop different types of competencies, including social competence, which will allow them to become what they want to become. Professional development of young talented individuals is possible only in open communication and interaction systems, it is in the international social projects of medical societies that constantly exchange energy and information with the environment, and at the same time, being a source of development and prospects for preserving the life of nation. Such a model of education supposes changing the role of teacher: the transition to joint actions with the student in new situations in an open, changing, modern world.

The article considers risk factors for the development of cognitive disorders, the most powerful of which is age. The role of the cardiovascular system diseases in the formation of cognitive impairments, in particular, the development of changes in the cognitive sphere in arterial hypertension, is reflected. The main causes of the appearance and clinical variants of the manifestation of cognitive impairment in patients with arterial hypertension are highlighted. Attention is paid to morpho-functional changes in the brain, large and small cerebral vessels in the increased blood pressure and their relationship to cognitive impairments. The indications to the study of the cognitive sphere and the variants of diagnosis of cognitive impairments in patients with arterial hypertension are reflected.

The article deals with topical issues of arterial hypertension, as an interdisciplinary problem, maintaining the quality of life and the level of cognitive functions in such patients. The program of care for patients with hypertension has gained interdisciplinary importance. Of primary importanceis the problem of the lack of real knowledge and practicals kills for early diagnosis of CI in the stage of primary care of general practitioners – family medicine, therapists – those who see the patient first and have the opportunity to observe the dynamics of the cardiovascular disease. The necessity of early detection of cognitive impairment in patients with arterial hypertension is grounded, the option of correcting cognitive impairment using the combined nootropic drug Cognifen is proposed.All patients with hypertension should pay close attention to the state of higher mental functions. The management of patients with hypertension in all age groups with CI should be based on achieving adequate control of blood pressure and as soon as possible include adequate neuroprotective therapy.

The activity of gelatinases A and B in blood plasma of rats with doxorubicin-induced cardiopathy at the application of cyclooxygenases inhibitors – ketorolak, lornoxycam and celecoxib was investigated. Significant increase of activity of both gelatinases in rats with cardiopathy was identified. Under ketorolak effect gelatinase B activity increases, and gelatinase A is inhibi­ted. The opposite reaction was observed when celecoxib was applied: gelatinase B acti­vity decreases, and gelatinase A increases. Combination of bioflavonoid quercetin and COX inhibitors decreased the activity of both latent forms of gelatinases in blood plasma of rats with cardiopathy. It is concluded that the stabilization of gelatinases activity is an indicator of myocardial metabolism and additional indicator of fibrosis development.

There was analyzed the extent and structure of cognitive disorders in outpatient conditions by the data of examination of 118 hypertension patients stage II with arterial hypertension of 1-2 degrees, which were under the supervision of General Practitioners. Neuropsychological examination was carried out by MMSE ( Mini-Mental State Examination ) and FAB ( Frontal Assessment Battery ) scales, verbal memory was studied by the method of Luria, concentration and speed of sensorimotor reactions - by techniques of Schulte and Rybakov. It was found that 28.8% of patients had cognitive disorders, mainly of neurodynamic nature, in particular, the ability of concentration and speed of psychomotor reactions was reduced. The presence of verbal memory disorders of varying severity associated with hypertension was revealed. Analysis of medical records testifies to the absence of attention to this issue in rehabilitation programs of hypertensive patients at the primary stage of care. The importance of timely diagnosis of cognitive disorders as a marker of early disorders of cerebral circulation and the functional state of the brain, the target organ in hypertension of 1-2 degrees was demonstrated.

Determining the level of blood pepsinogens has important clinical significance in the formation of risk groups for gastric cancer and erosive/ulcerative lesions of the gastric mucosa. A comparison of imported and domestic kits for testing the level of pepsinogens was performed. Russian diagnostic kits in comparison with foreign ones proved to show similar sensitivity and specificity.

Serum contents of 7 cytokines (IL-1, IL-1beta, IL-6, IL-8, TNF-alpha, INF-gamma, INF-alpha) were examined in 87 children aged 14-17 years according to presence of antibodies against Helicobacter pylori and cytotoxic CagA protein. There was a lack of difference in cytokine levels between infected and non-infected children. Thus H. pylori infection in children in contrast to adults does not cause changes in systemic cytokine secretion.

Prevalence of atrophic gastritis in various population of Siberia with serological tests was studied. Representative samples of Novosibirsk adult population and also urban and rural population of Yakutia were examined. 348 persons at the age more than 45 years (180 males and 168 females) were studied. Concentration of pepsinogen I, gastrin 17 and antibodies to Helicobacter pilori in blood serum was estimated with immune-enzyme analysis ("Biohit GastroPanel", "Biohit", Finland). In addition, domestic test-systems were used for detection cytotoxic (expressing CagA-protein) strains H. pylori. Level of markers (pepsinogen, gastrin, antibodies to Helicobacter pilori and antibodies to CagA H. pilori) in observing populations had no difference between males and females, and also did not depend on age. Occurrence of atrophy in body of stomach in Novosibirsk population, urban and rural population of Yakutia was 10.1, 16.7 and 25.6% respectively, and in antral part--10.7, 25.6 and 8.9% respectively. Total atrophy was registered in 1% in all groups. Helicobacter infection was detected in 78-88% of population. Domestic immune-enzyme test-systems were comparable with data of histological examination and demonstrated greater sensitivity at H. pylori detection vs. foreign. High prevalence of atrophic gastritis in various groups of Siberia population was noticed, which must be was bounded with great level of H. pylori infection in population.

The authors describe a current approach to the laboratory diagnosis of chronic gastritis, by using the plates of serological tests: pepsinogen I (PG-I), gastrin 17, and antibodies to Helicobacter pylori (HP). These tests and a questionnaire were used to examine 168 persons aged 45-70 years, who were a random population sample. Almost a fourth of the adult population was observed to have pronounced gastric mucosal atrophic changes, which might be associated with the high prevalence of HP infection. The concentration of PG-I is high in the persons infected with HP, its cytotoxic strains in particular, its elevated level servers as a valid marker of peptic ulcer disease and gastroesophageal reflux.

The objective of this study was to assess the prevalence of gastroesophageal reflux symptoms (GERS) in 403 adolescents at the age of 14-17 according to the Helicobacter pylori status in a two-year prospective school-based survey. Initially the H. pylori infection was revealed in 55.3%. GERS were reported by 16.1% of H. pylori negative subjects, 17.3% of H. pylori positive but CagA negative adolescents, and by 30.3% of those being CagA positive. Over 2 years, among 275 subjects without initial GERS, symptoms appeared in 43 (15.6%), being 10.8%, 18.8%, and 21.0% in three groups under the study, respectively. The H. pylori infection (especially with CagA positive strains) in older children results in GERS, which can be explained by antral gastritis with a subsequent increase of acid secretion.

To study the prevalence of CagA-positive strains of Helicobacter pylori in Novosibirsk residents and to assess their relation to some clinical parameters.Random sample of the population (218 males and 220 females aged 25-64 years) was examined. Antibodies against Helicobacter pylori and CagA protein were tested in the sera using enzyme immunoassay. Gastrointestinal symptoms were evaluated with questionnaire.The prevalence of H. pylori infection was 88%, among H. pylori-positive persons CagA-positivity was revealed in 58%. Presence of antibodies to CagA protein was associated with dyspepsia (OR = 2.5, 95% CI 1.2-2.5) and history of peptic ulcer (OR = 3.6, 95% CI 1.0-14.5). CagA-positive subjects more frequently reported bowel disturbances, diarrhea, in particular (OR = 2.9, 95% CI 1.2-7.4).The data showed similar distribution of H. pylori and CagA-positivity among adult population in Russia and Eastern European countries. CagA-positivity is associated with peptic ulcer and dyspepsia.

During prophylactic examination of blood sera taken from the members of 59 families by the enzyme immunoassay, antibodies to H. pylori and CagA protein were determined. As shown in this study, the children of non-infected mothers proved to be infected in 6.3% of cases and the children of infected mothers, in 72.1% of cases (p0.001). The children of non-infected fathers were H. pylori-positive in 71.4% and those of infected fathers, in 58.4% of cases. The CagA status was found to coincide in mothers and their children (p = 0.01), but not in fathers and their children. These data indicate that children acquire H. pylori infection from the members of their family, mainly from their mothers.

The concentration of pepsinogen I as well as antibodies to Helicobacter pylori and CagA protein were determined in the blood serum in 84 schoolchildren at the age of 14-17 using the immune-enzyme analysis. The level of pepsinogen I did not depend on the sex, age, gastroenterological symptoms and ulcer occurrence in parents. However, the concentration of pepsinogen was higher in teenagers infected with H. pylori (89.6 ug/l) than in non-infected ones (69.4 ug/l, p = 0.012), this increase taking place mostly due to strains containing CagA.

1. The properties and structure of certain calcium, lithium, barium-aluminum, and silica gel greases that have been studied are not too sensitive to the addition of water. A slight decrease can be noted in the yield values and viscosity at above-freezing temperatures, with an increase in viscosity at below-freezing temperatures. 2. The addition of water has a strong influence on mechanical stability and sometimes on the viscosity at −15°C for the complex calcium grease Uniol-1, and on the structure and properties of the sodium-calcium grease 1–13. The addition of 5% water softens the 1–13 grease, but an increase in water concentration to 20% gives a very marked hardening of the grease.

Experiments were carried out on white mice. Sera were obtained from rabbits immunized with 1B and II fractions ofP.pestis, and with the water-insoluble residue of the EV strain; only the latter serum possessed therapeutic and protective properties. Normal serum, and sera from rabbits immunized with the two other antigens showed no differences with respect to these properties. The sera of rabbits treated with the EV strain did not differ significantly from the sera of others immunized with fractions 1B and II ofP.pestis with respect to the power to neutralize toxin.
