Background. Prostate cancer (PCa) is an actual disease and a frequent oncological pathology in men. The main methods of radical treatment of patients with PCa are radical prostatectomy and radiation therapy. Radical prostatectomy s the most commonly used method of therapy in patients with localized PCa. Adjuvant hormone therapy after surgical treatment is the standard method of therapy in patients with the presence of lymph node metastases. At the same time, the standard approach of treatment of patients with metastatic PCa is combination therapy with medical (using of analogues or antagonists of luteinizing hormone-releasing hormone (LHRH) or surgical castration in combination with chemotherapy with docetaxel or new generation antiandrogens (enzalutamide or apalutamide)). Numerous studies have demonstrated the importance of achieving minimum testosterone levels at all stages of drug therapy in patients with PCa. It has also been shown that the use of LHRH analogues may be less effective to the use of LHRH antagonists (degarelix) in relation to the effectiveness of testosterone suppression. Thus, conducting a study aimed at studying the effectiveness of testosterone suppression using LHRH antagonists in various clinical situations and patient populations in real clinical practice is a very actual task.Aim. To evaluate the effectiveness and safety of castration therapy using degarelix in real clinical practice and in various clinical situations.Materials and methods. The object of an observational non-interventional study was 132 patients with PCa from 13 cancer centers of Russian Federation who were treated with LHRH antagonist degarelix. The study was non-interventional (observational), retrospectively-prospective, open multicenter and not randomized. In accordance with the design of the study, depending on the clinical situation, patients were divided into 3 groups: group A (n = 52; 39.4 %) – patients with primary metastatic hormone-sensitive PCa, who were shown to undergo combined drug treatment with castration therapy as one of the components; group B (n = 43; 32.6 %) – patients, who underwent combined hormonal and radiation treatment (ADT + radiation therapy); group C (n = 37; 28 %) – patients who underwent surgical treatment (radical prostatectomy with extended PLND) with the presence of metastases in the lymph nodes identified by the results of a morphological examination (pN1).Results and conclusion. As a result of a non-interventional observational study, high efficacy of androgen-deprivation therapy with the use of degarelix was demonstrated in relation to the suppression of testosterone and PSA in patients with primary metastatic and locally advanced PCa in various clinical situations, as well as low toxicity and satisfactory tolerability of this variant of hormonal treatment.

Objective: to study the long-term functional results of creating an orthotopic urinary reservoir from the ileocecal angle in comparison with the Studer technique of creating a urinary reservoir after radical cystectomy in women. Materials and methods. From 2004 to 2018 radical cystectomy for bladder cancer was performed in 120 women. In 71 (59.2 %) patients, an orthotopic urinary reservoir was formed: 48 (67.6 %) from the ileum, 23 (32.4 %) from segments of the colon. This study included 11 patients who had a reservoir formed from the ileocecal angle, including by their own modification, and 13 patients who had a reservoir forme by Studer. The average age of the patients was 62.6 ± 2.6 (29—82) years. The average follow-up time was 77.1 ± 10.5 (19—184) months. The presence and degree of urinary incontinence was assessed using a questionnaire and a Pad test. 12 months or more after the operation, a complex urodynamic study was performed. Results. The overall 5-year survival rate was 92.3 %: in the Studergroup it was 90.9 %, in the ileocecal angle group it was 100 % (p = 0.317). In the general population of patients, 37.5 % were completely continent. Women after plastic surgery by Studer retained urine better than after plastic surgery by ileocecal angle (46.2 % vs 27.3 %), the difference was only for mild urinary incontinence, which was more common in the ileocecal angle group. The frequency of severe urinary incontinence in both groups was the same. Nocturnal urinary incontinence was more common in patients in the Studer group (46.2 %). In the group of patients with ileocecal angle plastic surgery, there was no chronic urinary retention requiring intermittent catheterization, and in the group of Studer plastic surgery — in 1 (7.7 %) case. Analysis of urodynamic parameters revealed the presence of a greater physiological capacity of the urinary reservoir in the group of patients after plastic surgery according to Studer (p = 0.043). In the group of patients with ileocecal angle plastic surgery, higher values of the maximum pressure of the reservoir wall, “leakage pressure” and maximum expulsion pressure are observed, possibly associated with a more pronounced muscle layer of the intestinal segment used. Conclusion . The functional results of the orthotopic reservoir formedfrom the ileocecal angle are comparable to the results of urine derivation using the classical Studer method. Complex urodynamic study showed a direct influence of various characteristics of the reservoir on the nature and degree of urination disorders in patients.

Objective : to identify the most important factors influencing the development of complications after radical cystectomy (RCE). Materials and methods . The results of 182 RCEs performed in September 2014 and December 2016 were analyzed. The patients included 152 (83.5 %) men and 30 (16.5 %) women. The patients’ age ranged from 31 to 84 years (mean age 62.3 years). An orthotopic urinary reservoir was formed in 138 (75.8 %) patients. Postoperative complications were evaluated according to the Clavien–Dindo classification. The calculations were made with a computer program for SPSS Statistics 16. Results. No complications were recorded in 84 (46.2 %) patients within 30 days after RCE. The development of various postoperative adverse reactions was observed in 98 (53.8 %) patients. The most common complications were gastrointestinal (26.9 %) and infectious (25.8 %). Sixteen (8.8 %) patients developed grade I complications; grades II, III, IV, and V complications were seen in 53 (29.1 %), 21 (11.5 %), 5 (2.7 %), in 3 patients, respectively. Objective . Study of 90 day complications after solitary kidney resection and determination of their dependence on resection complexity level in accordance with the RENAL nephrometry scoring system. Materials and methods . Seventy (70) resections due to solitary kidney tumors were performed in 65 patients (34 males and 31 females). Mean age was 59.1 years (40–75 years). Fifty-five (55) patients had renal cell carcinoma, 10 patients had benign tumors. According to the RENAL scoring system, in 17 (24 %) cases resection complexity was 4–6 points, in 21 (30 %) cases it was 7–9 points, in 32 (45 %) cases it was 10–12 points. Mean tumor diameter was 4.4 (1.5–10.0) cm. Kidney resection without ischemia was performed in 3 patients, with segmental ischemia – in 39 cases. In 28 (40 %) cases general ischemia was used, its mean duration was 18 (6–48) minutes. Local hypothermia was used in 5 patients. Statistical analysis was performed using SPSS Statistics 16. Results. Analysis of 90 day complications revealed no complications in 41 (58.6 %) cases, severity grade I complications were observed in 2 (2.9 %) cases, grade II – in 20 (28.6 %), grade III – in 6 (8.6 %); there were no grade IV and V complications. Objective. Analysis of factors affecting results of inferior vena cava (IVC) thrombectomy in patients with renal cell carcinoma. Materials and methods . Nephrectomy with thrombectomy from IVC was performed in 53 patients (31 (58.5 %) males and 22 (41.5 %) females) with extended thrombosis. Patient age was 35–76 years (mean age 58.7 ± 10.1 years). In 36 (68 %) patients stage Т3b was determined (mean thrombus length 5.28 ±0.32 cm), in 17 (32 %) patients – stage Т3с (mean thrombus length – 13.00 ±0.71 cm). Postoperative complications were evaluated in accordance with the Clavien–Dindo classification. Calculations were performed using statistics software SPSS Statistics 16. Results. No intraoperative complications were observed, mean blood loss was 714 (200–4000) ml. Use of the Cell Saver system allowed to perform reinfusion in the majority of patients. Blood transfusion of 200–500 ml was necessary in 15 patients. In 32 (60.3 %) patients no complications were observed. Grade I complications were observed in 2 (3.7 %) patients, grade II – in 15 (28.3 %) patients, IIIА grade – in 2 (3.7 %) patients, and grade IIIВ – in 1 (1.8 %) patient; there wasn’t any grade IV complications. Two (3.8 %) patients died of pulmonary artery thromboembolism. Correlation analysis has shown a high dependency of complications on thrombus size (r = 0.7) and blood loss volume (r = 0.6), and low dependency on tumor size (r = 0.44); complications weren’t affected by patients’ age (r = 0.1) or Karnofsky status (r = 0.0). Median overall survival was 48 months, 3-year survival was 57.1 %, median progression-free survival was 46 months, 3-year progression-free survival was 56.8 %; 90-day morbidity was absent. Three-year overall survival for рN0 was significantly higher than for рN1 (69 % vs. 25 %; р = 0.014). There was no difference in terms of survival between patients with supradiaphragmatic and subdiaphragmatic thrombi (р = 0.14). Conclusion . Results of our study show that surgical treatment of patients with renal cell carcinoma and extended IVC thrombosis is effective.

to study the features of microbiota in patients of urological departments and to optimize the schemes of antibiotic therapy.Data of microbiological studies in patients of urological departments in 2017 years were analyzed. Isolation of pathogens was carried out using standard techniques. A disk diffusion test using Mueller-Hinton agar with BioRad disks was used to determine the sensitivity. The statistical analysis was performed using the WHONET 5.4 program. In addition, a comparison with the results of the PCR study was done.A total of 672 etiological pathogens were isolated in whole clinic in 2017 year. From those, 173 pathogens were found in the urological clinic, which accounted for 25.7% of the total number. Gram-negative bacteria predominated and its proportion was 59.23%. Gram-positive bacteria were isolated in 24.4%, and fungi were found in 16.37% of cases. The structure of pathogens isolated from stoma or obtained by catheterization was not significantly different. Most frequently isolated pathogens included @K. pneumonia (22.8%), @E. coli (21.2%), @C. albicans (17%), @P. aeruginosa (11%), @E. aerogenes (6%) and @S. aureus (3 %). @K. pneumonia usually had an unfavorable resistance pattern. E. coli isolated from urine samples had more favorable resistance pattern in comparison to pathogens obtained from stoma. The bacteria which were found in intensive care unit had the increased level of resistance.Gram-negative bacteria predominate in the urological clinic with a relatively high proportion of extended-spectrum beta-lactamase-producing organisms. Risk factors for the emergence of multi-resistant pathogens are the treatment in the intensive care unit and the presence of stomas or catheters. The PCR method, carried out in parallel with routine microbiological studies and regular analysis of the overall bacterial spectrum, allows to optimize the starting antibiotic therapy from the first day of the disease. The use of cephalosporins is not justified in the most cases. Aminoglycosides and fluoroquinolones remain effective. The use of either -Lactamase-protected penicillins or fosfomycin is possible only after obtaining the results of bacteriological study. When prescribing carbapenems, the risk of inefficiency is especially high in patients with stomas/catheters and transferred from the intensive care unit.

In 171 patients with prostate cancer before radical prostatectomy assessed the frequency and nature of sexual disorders using the IIEF and hardness scale member. Erectile function was normal in only 73 (42,7%) respondents, 96 (57,3%) had reduced erection varying degrees, and in 74 (43,8%) patients had severe erectile dysfunction. A similar trend noted by studying the hardness of erection: 86 (54,9%) patients had an erection sufficient for sexual intercourse, but only 36 (21,1%) had erectile dysfunction at all. Orgasmic function was preserved in 111 (64,9%) of the surveyed patients, and in 89 (52%) of them was high. Level of erectile function depends on the patient's age and the presence of a concomitant cardiovascular disease, such as atherosclerosis and hypertension. Only 50,9% of patients show a high interest in restoring erectile function after surgery.

Ultrastructural changes in the colorectal mucosa in response to chronic contact with the urine were demonstrated in outbred albino rat experiments. Oral correction with slightly alkaline sodium hydrocarbonate solution reduced the destructive effect of the urine on rat colorectal mucosa.

Resolution on the results of the Meeting of Experts on the treatment of castrate-resistant prostate cancer.

The ultrasound pattern of hepatic and renal changes after different types of resection is depicted. Ultrasound studies were performed in 335 patients operated on for focal hepatic masses and in 72 patients operated on for focal renal masses. The authors identified 3 patient groups after hepatic resection (segment-oriented liver resection (99 patients), portal hemihepatectomy (n=163), and extended hemihepatectomy (n=73)) and 3 patient groups after renal resection (planar (n = 83), wedge (n = 104), and frontal (n = 66)). It has been established that the number and location of hepatic veins in the liver stump, which the authors have designated as a hepatic vein rule, are of basic value in defining the type of hepatic resection. To determine the type of renal resection, it is essential to assess the shape of the organ and the area of its resection. The found echosemiotic patterns allow the type of performed resection of parenchymatous organs to be correctly determined in the majority of cases.

Results of treatment of 32 patients after nephrectomy with thrombectomy for renal cell carci-noma complicated by tumor thrombosis of the inferior vena cava were analyzed. The study in-cluded patients only with II - IV clot levels: 26 patients with T3b - the length of the thrombus 5.1 +/- 1.75 cm, 6 patients with T3c - the length of a thrombus - 14.8 +/- 0.98 cm. One patient (3.1%) died due to pulmonary artery thromboembolia in immediate postoperative period. 1-2 degrees Clavier Complications were observed in 11 patients, 3-4 degree - in two (6.2%) patients. 90.4% of patients had metastases in distant organs and (or) in the regional lymph nodes, which nega-tively affected the survival rates of patients - 5-year survival rate was 36%. The presence of lymphogenous metastases, MSKCC criteria were factors affecting survival rate. Survival rates of patients with T3b and T3C levels were not statistically different. The results of postoperative systemic targeted therapy were significantly statistically better than the results of immunotherapy.

Using 60 outbred albino male rats, the changes of colonic mucosa were studied after rectal infusion of urine for 1, 3 and 5 months. In response to prolonged exposure to urine, the mucosa was found to undergo gradual atrophic changes, including the reduction of its thickness and crypt depth. Moreover, the number of goblet cells was significantly decreased. Oral correction by weak solution of sodium hydrocarbonate could slow down the progression of atrophic processes in colonic mucosa.

We present results obtained using a two-channel portable device for CP OCT based on the polarization-maintaining fiber. The device is capable of acquiring OCT images of the object in direct and orthogonal polarizations simultaneously. The OCT system has an endoscopic forward-looking probe (2.7 mm in diameter). The CP OCT imaging was done in 64 postoperative specimens, for 30 patients during cystoscopy with suspicion of urothelial neoplasia, and for 11 patients with prostatectomy due to cancer. After surgery in the tissue specimen we immediately introduced surgical suture and investigated a bladder wall by CP OCT. When comparing OCT images and histology slides we can distinguish tissue layers based on position of the suture material. CP OCT images were compared with histological data. Special staining - Sirius Red for collagen types was applied. OCT -visualization of sympathetic nerve was made during open surgery. We found, that a strong signal in orthogonal polarization is produced by structures consisting of Type I collagen. We demonstrated that endoscopic benign and early malignant pathological processes that cannot be detected by standard OCT may be differentiated by CP OCT. Sympathetic nerve of neurovascular bundle gives a strong signal in both polarizations: direct and orthogonal. Based on these CP OCT features we can visualize nerve during operation (open or endoscopic surgery) and thus preserve it.

An original method of kidney resection with the use of ultrasonic surgical destructor-aspirator is proposed. The method was applied in the treatment of 19 patients (20 renal resections and 1 tumor enucleation). No complications occurred. The use of ultrasonic destructoraspirator in renal resection enabled isolation of all the intraparenchymatous tubal structures of the kidney, each of them could be ligated and cut if visualization is adequate. This provided a complete hemostasis on the wound surface of the kidney and hemostatic suturing of the kidney wound became unnecessary. This method of renal resection reduced hospital stay of the patients because they had no hemorrhages, purulent complications and urinary fistulas.
