1. Studying the Interaction of Viteral Substitutes with the Surface of Acrylic Intraocular Lenses

Abstract

Purpose of the study: In vitro to study the degree of adhesion of vitreous substitutes, such as silicone oil of various viscosities and perfluorodecalin (PFOS), to hydrophobic acrylic polymer IOLs and evaluate the interaction of vitreous substitutes with the model MIOL-Soft-23 in the clinic. Material and methods. IOLs of the model MIOL-Soft-23 were taken for the experiment. MIOL-Soft-23 are included in the RPR-20 set. IOL of the model MIOL-Soft-23 is made by photopolymerization from a biocompatible spatially cross-linked hydrophobic acrylic polymer based on oligomers and monomers of the methacrylic series with filters in the ultraviolet region of the spectrum from 19 to 23 D. For the experiment, silicone oils of different viscosities were taken: RS-OIL 2000 (Alchimia), SIL-5000-S (DORC), Oxane 5700 (Bausch & Lomb), PFOS (perfluorodecalin) Dk-line (Bausch & Lomb). Also, saline sodium chloride 0.9 % (Solofarm) was used. Results. Revealed a decrease in the degree of silicone adhesion to the surface of acrylic hydrophobic IOLs as the temperature rises, not less than 1.5 %. The absence of dependence of the degree of adhesion on the degree of viscosity of silicone oil was confirmed, which corresponds to the literature data. For the first time, the average percentage of adhesion of perfluorodecalin to the IOL surface data were obtained, in particular, the MIOL-Soft-23 model. The range of values ranged from 0.7 to 7.2 %. The average coverage percentage is 1.9 ± 1.3 % (exposure at 37 ± 1 °C for up to 14 days) and 3.4 ± 1.5 % (exposure at room temperature for up to 14 days). This confirms the need for careful intraoperative monitoring of the completeness of removal of PFOS droplets from the IOL surface. Conclusion. The presence of the risk of adhesion of residual drops of silicone oil and PFOS to the surface of the IOL necessitates careful intraoperative control of the completeness of the removal of vitreous body substitutes, as well as further search for the best ways to eliminate this complication. The conducted complex of studies makes it possible to recommend the use of IOL model MIOL-Soft-23 in complicated cases of vitreal surgery.

Abstract

The Spin Physics Detector, a universal facility for studying the nucleon spin structure and other spin-related phenomena with polarized proton and deuteron beams, is proposed to be placed in one of the two interaction points of the NICA collider that is under construction at the Joint Institute for Nuclear Research (Dubna, Russia). At the heart of the project there is huge experience with polarized beams at JINR. The main objective of the proposed experiment is the comprehensive study of the unpolarized and polarized gluon content of the nucleon. Spin measurements at the Spin Physics Detector at the NICA collider have bright perspectives to make a unique contribution and challenge our understanding of the spin structure of the nucleon. In this document the Conceptual Design of the Spin Physics Detector is presented.

Abstract

The objective: to determine the quality of knowledges about the Helsinki Declaration on Patient Safety among Russian anesthesiologists and intensivists and how widely its vision and standards have been adopted in clinical practice.Subjects and Methods. The study design involved the creation of 44-item online and offline questionnaire. The questions were divided into three blocks: personal information, data on hospitals where respondents work, and questions about implementation of the Helsinki protocol in their practice. Some of the questions required open answers. Persons with higher medical education who have completed residency and/or internship in anesthesiology and resuscitation and are working in this field in the Russian Federation were invited to participate in the survey.Results. 140 (21.5%) respondents answered all the question of the questionnaire. Of those surveyed, 76.4% were familiar with the Helsinki Declaration on Patient Safety, but only 17.1% felt they had sufficient knowledge of the concept. 43.6% of the respondents apply the Helsinki Declaration on Patient Safety to their clinical practice, while 49.3% of the respondents had difficulty answering whether they follow the Declaration in their work or not. The study showed a satisfactory level of compliance with mandatory standards for monitoring in the perioperative period but all the EBA-recommended standards are applied in only 23% of the hospitals. 26.4% of the respondents use the Safe Surgery Checklist prepared by the World Health Organization. 58.6% of the respondents use the reporting/notification system for adverse events or critical conditions in the perioperative period.Conclusion. The survey has shown that many of the surveyed anesthesiologists and intensivists have good knowledge of the Helsinki Declaration on Patient Safety and successfully apply it to their clinical practice but some doctors do it unconsciously, not understanding what standards they follow. Organization of additional educational programs could help physicians to improve their knowledge and raise their awareness in order to provide safer patient care. We also suggest introduction of unified checklists and national reporting systems for adverse events or critical conditions in the perioperative period.

Abstract

Pregnant and postpartum women are at a higher risk of infection with SARS-CoV-2 as well as a higher risk of adverse outcomes for the mother and fetus. Standard approaches to the management of COVID-19-associated multiple organ dysfunction may not always be implemented in this category of patients. In the clinical case of a patient, who developed multiple organ dysfunction syndrome (severe ARDS, coagulopathy) associated with COVID-19 in the postpartum period, we demonstrate the successful use of combined extracorporeal life support that included veno-venous extracorporeal membrane oxygenation, therapeutic plasma exchange and renal replacement therapy with the universal oXiris set.

Abstract

Purpose: to design a method of active drying ofa fragment of internal limiting membrane (ILM) to achieve its strong adhesion to the retina in the surgical treatment of idiopathic macular hole (MH). Material and methods.The study included 10 patients (10 eyes) with large idiopathic MH. The best corrected visual acuity ranged from 0.03 to 0.1. All patients underwent active drying of inverted ILM fragment during MH surgical treatment by additional air supply through a long blunt cannula. The follow-up was up to 3 months after surgery. Results. The inverted ILM fragment retained a stable position without displacement or deformation, and had dense adhesion to the retina in all cases during the follow-up. According to SOCT, improvement of the anatomical condition of the retina in the foveal area was achieved in all patients; the closure of the MH with the preservation of the defect at the level of the ellipsoid zone of the photoreceptors of the retina was noted. Final visual acuity was from 0.2 to 0.4 (0.26±0.08 in average). Central sensitivity increased to 22.36 ± 2.29 dB, total sensitivity rose to 23.7 ± 1.43 dB. Absolute scotoma was found to disappear in the center of the fovea. Conclusions. Further investigations on larger clinical material are necessary for a reliable assessment of the effectiveness of the method. Abstract

It has been studied the efficiency of ethylmethylhydroxypyridine succinate in patients with angina FC II, depending on the level of anxiety. There was high positive correlation between the number of angina attacks and high level of anxiety in patients with angina FC II. Also, there were some special effects of ethylmethylhydroxypyridine succinate as quantity of attacks, blood pressure level, and the frequency and nature of cardiac arrhythmias, heart rate variability, life quality and, as a consequence, frequency of seeking care.

Abstract

Purpose. Is to consider the organizational aspects of outpatient cataract surgery in the Kaluga branch of the S. Fyodorov Eye Microsurgery Federal State Institutuion and evaluate its effectiveness.Material and methods. The outpatient cataract surgery established in Kaluga branch is a type of hospital-replacing forms and involves hospitalization, surgery, nearest postoperative period and the patient’s discharged in the same day, that is not required inpatient medical care. The modern technologies in cataract surgery actively use in clinical practice of the Kaluga branch of Eye Microsurgery, that allow to perform operations on an outpatient basis.Results. During 2015-2016 in the Kaluga branch of Eye Microsurgery in 12 357 cataract surgeries were performed, including 11 368 (92%) in outpatient regime. The number of surgeries increases by 36% as compared with the same period before the outpatient surgical care began to use. The clinical and functional results of the outpatient cataract surgery are high, despite the absence of postoperative follow-up in inpatient regime. Besides, the majority of patients are satisfied with the absence of hospitalization and possibility to be at home after undergoing surgery. Alongside with the clinical effectiveness the outpatient treatment is resulted in considerable economic effect.Conclusions. Outpatient medical surgical care allows to perform major volume of operative treatment, ensuring the effectiveness and safety of cataract surgery and is a promising direction of the healthcare development.

Abstract

This study was aimed to systematize magnetic resonance imaging (MRI) presentation of toxic leukoencephalopathy, to find the correlation between method of central nervous system (CNS) leukemia prevention and changes on MRI, to find relationship between existence leukoencephalopathy on imaging and neurocognitive deficits in pediatric patients after anti-leukemic therapy. Brain MRI data of 48 children, who underwent a therapy course according to the ALL-MB intermediate risk protocol, was evaluated. In accordance with two arms of this protocol, they received either radiation therapy, or additional intrathecal administration of chemotherapeutic agents as a prevention of CNS leukemia. Also, neurocognitive tests were performed. According to the results of the performed investigation, 10 (50%) out of 20 children, who received cranial irradiation and 18 (66.6%) out of 27 patients, who received only intrathecal therapy demonstrated abnormal brain changes (leukoencephalopathy) according to MRI data. Leukoencephalopathy was mostly presented by diffuse zones and localized predominantly in the frontal and temporal lobes. There was no correlation between method of CNS prevention and the existence of leukoencephalopathy on MRI. The analysis of our data did not show significant differences in brain damage and severity of cognitive impairment depending on the type of prevention of CNS leukemia. Moreover, in this study no statistical correlation was found between leukoencephalopathy on MRI and neurocognitive impairment according to clinical tests data. Further long-term prospective studies and examinations should be performed to assess late neurotoxic effects.

Abstract

Diagnosis of spinal leptomeningeal metastases in children with brain tumors is currently performed using magnetic resonance imaging (MRI) with contrast enhancement. In some cases, non-specific contrast agent accumulation along the meninges is detected on MRI in the postoperative period, which can be misdiagnosed as metastases, or can obscure true metastasis to the spinal cord. The article presents a clinical example of this phenomenon.

Abstract

The incidence of leukemia in children in various regions of the Russian Federation ranges from 1.9 to 4.1 per 100 000 of children aged 0 to 14 years. If the average annual morbidity of children is below the Russian (3.03) or European (4.0) standard, it is possible that some cases of leukemia and other oncological and oncohematological diseases are not reported. Clinical-laboratory and instrumental manifestations of malignant diseases can be similar to other conditions. It should be borne in mind that the most frequent childhood diseases are infections. Fever combined with bone pain, unexplained hepato- and/or splenomegaly, lymphadenopathy, changes in the cellular composition of blood and increased LDH should be the reasons to exclude malignancy.

11. DELIVERY OF DRUGS TO THE POSTERIOR EYE SEGMENT USING BIODEGRADABLE MULTILAYER IMPLANT

Abstract

Purpose. To develop a new method of intraocular drug delivery to the posterior segment of the eye using a new biodegradable multilayer sustained-release implant.Material and methods. We fabricated a multilayer biodegradable intravitreal implant, which has a tube form, 4.0mm length and 0.3mm diameter, that is composed of polyvinylpyrrolidone, lactic acid and glycosaminoglycans. Implant consists of 15 layers with the thickness of each layer 10μm. Layers are connected by transverse septums. The degradation time of the implant depends on a transverse septum quantity. The intravitreal empty implant was placed in 5ml of 0.9% NaCl solution in a closed vial at 37° C (5 implants). The replacement of 3.0ml of balanced salt solution (BSS) with the maintenance of the specified temperature and volume was made once a day. Measurements of the implant time resorption were carried out from the beginning of the experiment to the full visual disappearance of the implant in the test-tube.Results. The full resorption time of the empty implant was 31 days. The presence of water leads to hydrolysis of the implant polymer matrix. The degradation rate depends on the quantity of cross-linking between the layers of the implant and can be changed variably. Scanning electron microscopy has shown the laminated structure of the implant, presence of the transverse septums between each layer. In the developed delivery system the drug is uniformly distributed in each of saturated layer. The release of drug substance from the implant occurs as a result of polymer degradation and diffusion of the substance into the surrounding fluid.Conclusion. The implant for drug delivery to the posterior segment of the eye is developed, it allows to prolong the stay of drugs in the vitreous cavity. The new fabricated biodegradable multilayer intravitreal implant provides periodical release of active agent without its therapeutic concentration overdose.

Abstract

Мета роботи. Проведення порівняльного аналізу асортименту лікарських препаратів, які застосовують для лікування епілепсії відповідно до положень нормативних документів в Україні та у Великій Британії. Матеріали і методи. Під час дослідження використано офіційні дані нормативно-правового регулювання фармакотерапії епілепсії, зокрема, рекомендації Британського Національного Формуляру (оновлення квітень 2018 р.), Державного формуляру лікарських засобів (випуск 11, наказ МОЗ України від 18.04.2019 р. № 892), Уніфікованого клінічного протоколу первинної, екстреної, вторинної (спеціалізованої) та третинної (високоспеціалізованої) медичної допомоги епілепсії у дорослих (наказ МОЗ України від 17.04.2014 р. № 276). Були використані методи: логічний, системно-аналітичний, порівняльний аналіз та узагальнення інформації Результати й обговлрення. За результатами аналізу даних Британського Національного Формуляру визначено, що для лікування хворих на епілепсію рекомендовано 19 протиепілептичних препаратів за міжнародною непатентованою назвою. При зіставленні протиепілептичних препаратів із препаратами, що увійшли до вітчизняних нормативно-правових положень, які регулюють надання якісної медичної та фармацевтичної допомоги хворим, встановлено ряд розбіжностей. Дані відмінності включають як відсутність певних лікарських препаратів за міжнародною непатентованою назвою, так і різні підходи до формування ліній фармакотерапії хворих. За результатами порівняльного аналізу складу асортименту рекомендованих протиепілептичних препаратів Британським Національним Формуляром з Державним формуляром лікарських засобів встановлено, що до складу вітчизняного увійшли лише 9 препаратів за міжнародною непатентованою назвою. Дослідження протиепілептичних препаратів згідно з даними Державного формуляру лікарських засобів, дають підстави стверджувати про домінування лікарських препаратів іноземного виробництва (68 % від загальної кількості представлених протиепілептичних препаратів). Проаналізовано фірми-виробники лікарських препаратів, встановлено лідери серед іноземних та вітчизняних компаній. Висновки. Встановлено ряд відмінностей між рекомендаціями Британського Національного Формуляру та Державного формуляру лікарських засобів, а також уніфікованим клінічним протоколом лікування хворих на епілепсію. Так, у Державному формулярі лікарських засобів присутні лише 9 препаратів протиепілептичної дії з 19 лікарських препаратів, які зазначені у складі Британського Національного Формуляру. До вітчизняного протоколу лікування включено 12 препаратів за міжнародною непатентованою назвою. Аналіз даних Державного формуляру протиепілептичних препаратів показав, що загальна кількість представлених лікарських препаратів складала 315 торгових найменуваннь з урахуванням форм випуску, основну частку даного асортименту формували препарати іноземних компаній, співвідношення становило 68 % до 32 %.

14. D-region electron density and effective recombination coefficients during twilight – experimental data and modelling during solar proton events

Abstract

Accurate measurements of electron density in the lower D-region (below 70 km altitude) are rarely made. This applies both with regard to measurements by ground-based facilities and by sounding rockets, and during both quiet conditions and conditions of energetic electron precipitation. Deep penetration into the atmosphere of high-energy solar proton fluxes (during solar proton events, SPE) produces extra ionisation in the whole D-region, including the lower altitudes, which gives favourable conditions for accurate measurements using ground-based facilities. In this study we show that electron densities measured with two ground-based facilities at almost the same latitude but slightly different longitudes, provide a valuable tool for validation of model computations. The two techniques used are incoherent scatter of radio waves (by the EISCAT 224 MHz radar in Tromsø, Norway, 69.6° N, 19.3° E), and partial reflection of radio-waves (by the 2.8 MHz radar near Murmansk, Russia, 69.0° N, 35.7° E). Both radars give accurate electron density values during SPE, from heights 57–60 km and upward with the EISCAT radar and between 55–70 km with the partial reflection technique. Near noon, there is little difference in the solar zenith angle between the two locations and both methods give approximately the same values of electron density at the overlapping heights. During twilight, when the difference in solar zenith angles increases, electron density values diverge. When both radars are in night conditions (solar zenith angle >99°) electron densities at the overlapping altitudes again become equal. We use the joint measurements to validate model computations of the ionospheric parameters f+, λ, αeff and their variations during solar proton events. These parameters are important characteristics of the lower ionosphere structure which cannot be determined by other methods.

15. The role of atomic oxygen concentration in the ionization balance of the lower ionosphere during solar proton events

Abstract

The influence of atomic oxygen concentration on the height distribution of the main positive and negative ions and on electron density in the mesosphere is studied for the conditions prevailing during the solar proton event on 17 January 2005. It is shown by numerical modeling that the electron and ion density profiles are strongly dependent on the choice of the atomic oxygen profile. Experimental measurements of the electron density are used as the criterion for choosing the atomic oxygen profile in the mesosphere. With the help of modeling, the atomic oxygen profile in the daytime in the winter mesosphere is found to lead to a model electron density profile best matching the electron density profile obtained experimentally. As a result, with the help of modeling, we find the atomic oxygen profiles at various solar zenith angles in the winter mesosphere which lead to model electron density profiles matching the electron density profiles obtained experimentally. Alteration of the atomic oxygen concentration leads to a redistribution of the abundance of both positive and negative ion constituents, with changes in their total concentrations and transition heights. In consequence this results in changes of the electron density and effective recombination coefficient. For conditions of low concentration of atomic oxygen (during a solar proton event), the formation of cluster ions is the key process determining electron and ion densities at altitudes up to 77 km. The complex negative CO3− ion is formed up to about 74 km and the final NO3− ion, which is stable in relation to the atomic oxygen, is the dominant negative ion up to 74 km. As a result the transition heights between cluster ions and molecular ions and between negative ions and electron density are located at 77 km and 66 km, respectively.

Abstract

Aim. To analyze the current situation with the antibacterial drugs turnover, as well as to develop effective ways to solve the problem of growing resistance of bacteria to antibiotics, including determination of the active role of a pharmacist in this process. Materials and methods. The analysis of the current situation in the domestic system ofpharmaceutical supply of the population was carried out on the basis of a survey of pharmaceutical specialists directly involved in the process of dispensing antibacterial drugs from pharmacies in the outpatient setting. In accordance with the aim of the study a questionnaire was developed for interviewing pharmaceutical specialists. The questionnaire included the aim and objectives of the survey. During the survey, the confidential terms of use of the information received were notified. The required size of the respondent sample was calculated taking into account the expected reliability of the results obtained. The questionnaire proposed included closed questions that provided data on the state of the problem of the rational dispensing of antibacterial drugs, as well as open-ended questions revealing the individual attitude of the respondents to the problems of the rational drug consumption from the pharmacotherapeutic group indicated. The questions contained in the questionnaire were divided into two groups: the basic ones allowing us to obtain the necessary information from the respondent and the control ones indirectly confirming the accuracy of answers to basic questions and allowing exclusion of unclear answers. Results. The article presents the results of a survey of pharmaceutical specialists on the problem of the rational supply and consumption of antibacterial drugs and the assessment of their role in organization of an effective prescription model for this group of drugs in Ukraine. It has been determined that a significant majority of respondents consider it necessity to sell antibacterial drugs strictly by prescriptions (90.42 %), but, in practice, approximately the same number of respondents (90.73 %) still dispenses antibacterial drugs without a prescription: “Sometimes” (67.73 %) or “Always” (23.0%). Respondents consider as important measure to implement a set of measures to disseminate knowledge about the rationality of using antibiotics (84.98 %) through the pharmacy network. At the same time, a large majority of respondents (90.10 %) transfer responsibility for the proper consumption of antibacterial drugs onto doctors. According to the opinion of the respondents (42.49 %), the development of an appropriate target program with the involvement of pharmacists and doctors should be the main direction of implementing the strategy of the rational use of antibacterial drugs in Ukraine. Among the factors contributing to the unsystematic use of antibacterial drugs, the leading position by the number of respondents were such answer options as “Thoughtless self-medication of the population” (79.23 %), “Violation of prescription dispensing” (62.62 %) and “Availability of antibiotics in the pharmacy” (47.92 %). Conclusions. According to the results of the study it has been found that the complexity and controversial nature of the problem of the rational consumption of antibacterial drugs in Ukraine requires a comprehensive solution involving all subjects of the pharmaceutical supply system of the population.

Abstract

In this paper we investigate main pre-training and initialization methods of parameter values of neural networks such as pre-training using restricted Boltzmann machines, deep autoencoders, Glorot and He initialization of parameters, transfer learning and domain adaptation. Given methods are useful for finding of appropriate parameter values and initial initialization of neural network, what is necessary condition for further efficient training of deep models, because it give a possibility during training to reduce negative effects such as vanishing or explosion of gradient, overfitting, stucking in one of local minimums of loss function, etc. These methods belong to group of unsupervised training algorithms and do not need any labeling for data which will be used later for model’s training after parameters initialization. Firstly, in this paper, we analyze all these methods and describe advantages and disadvantages of each of them. Secondly, we describe results of our experiments applying these methods for solving of classification task of MNIST dataset and introduce ideas for further development and improvement of these algorithms.

It has been studied the efficiency of ethylmethylhydroxypyridine succinate in patients with angina FC II, depending on the level of anxiety. There was high positive correlation between the number of angina attacks and high level of anxiety in patients with angina FC II. Also, there were some special effects of ethylmethylhydroxypyridine succinate as quantity of attacks, blood pressure level, and the frequency and nature of cardiac arrhythmias, heart rate variability, life quality and, as a consequence, frequency of seeking care.

Abstract

The light collection of extruded scintillation strip samples with the help of WLS fibers placed in a longitudinal hole inside of the plates has been measured. The holes are filled with various liquid fillers. Measurements are performed under irradiation by cosmic muons. A method for pumping a liquid filler with a viscosity of more than 10 Pa s into the strip hole with a WLS fiber inside is devised and successfully tested.

Abstract

This study was aimed to systematize magnetic resonance imaging (MRI) presentation of toxic leukoencephalopathy, to find the correlation between method of central nervous system (CNS) leukemia prevention and changes on MRI, to find relationship between existence leukoencephalopathy on imaging and neurocognitive deficits in pediatric patients after anti-leukemic therapy. Brain MRI data of 48 children, who underwent a therapy course according to the ALL-MB intermediate risk protocol, was evaluated. In accordance with two arms of this protocol, they received either radiation therapy, or additional intrathecal administration of chemotherapeutic agents as a prevention of CNS leukemia. Also, neurocognitive tests were performed. According to the results of the performed investigation, 10 (50%) out of 20 children, who received cranial irradiation and 18 (66.6%) out of 27 patients, who received only intrathecal therapy demonstrated abnormal brain changes (leukoencephalopathy) according to MRI data. Leukoencephalopathy was mostly presented by diffuse zones and localized predominantly in the frontal and temporal lobes. There was no correlation between method of CNS prevention and the existence of leukoencephalopathy on MRI. The analysis of our data did not show significant differences in brain damage and severity of cognitive impairment depending on the type of prevention of CNS leukemia. Moreover, in this study no statistical correlation was found between leukoencephalopathy on MRI and neurocognitive impairment according to clinical tests data. Further long-term prospective studies and examinations should be performed to assess late neurotoxic effects.

Abstract

The visually driven instrumental conditioning of a single monkey (Macaca mulatta) was conducted after single-dose cranial irradiation with high-energy protons. The monkey executed saccades toward the visual stimuli and then responded by manually pressing right or left lever for stimuli in right or left half-field, respectively. The percentage of correct responses with dominant right hand exceeded the percentage of such responses with left hand and temporarily decreased two months after irradiation. A month later, the percentage of correct right-hand responses returned to the level before irradiation. No significant dynamic was found for changes in percentage of correct left-hand responses. The proton irradiation effect on right-hand responses suggests possible short-term disturbances in the eye-hand coordination for right handedness while the visual perception remains unaffected.

26. Quasi-completeness of the class of problems on the 'weight-minimax edge'

Abstract

The tasks of multi-criteria optimization in the general formulation do not have a trivial solution, which gives rise to a multitude of approaches in determining the most “successful” solution from a certain set of solutions that satisfy the problem conditions. One of the ways of formal defining of the possible alternative solution set is to isolate the Pareto set, i.e. the set of unimprovable alternatives. The previously developed approach was applied for studying some classes of multi-criteria problems, the objective functions of which have certain properties, and its productivity was confirmed. The concept of complete problems was introduced, for which the equality of the sets of feasible solutions, the Pareto and the full set of alternatives was fulfilled. In previous works, the authors introduced the concept of quasi-completeness. In the article the class of two-criterion problems, for which the admissible solution for the first criterion has a constant number of edges, and objective function contains the criterion of weight and the criterion of the minimax edge, is distinguished. The problem on the graph of the general structure and the problem on the bichromatic graph, for which the feasible solutions have a constant number of edges, were selected as representatives of this class. A method for studying the properties and estimating the powers of an admissible set of solutions, a Pareto set and a complete set of alternatives for the problems of the selected class, has been formulated. A theorem on the quasi-completeness for the selected class problems is proved. There were obtained estimates for two representatives of this class: “about a spanning tree and a minimax edge”, “about a perfect matching on a bichromatic graph and a minimax edge”. Polynomial algorithms for solving the problems under study are proposed. Estimates of the computational complexity of these algorithms are given

Abstract

The LYSO crystals of dimensions 30 × 30 × 130 mm and 10 × 10 × 10 mm are tested with different gamma sources. Energy resolutions and linearity of the energy responses in the energy range 511–1333 keV are measured. It is found that crystals have good linearity of the energy response in the entire tested energy range. Longitudinal light response non-uniformity of the long crystal is found to be less than 3.2% within the 30–120 mm range along the crystal axis.

Abstract

The 76 strains of Clostridium related to 14 species including 28 strains of C.difficile, were isolated from 74 samples of biological material taken out of oncologic patients. The species identification of strains was implemented by MALDI-TOF mass spectrometry using technique of direct application without extraction. The 74 strains (97.4%) were identified up to specie with reliability evaluation 2.0-2.5. Among C.non-difficile the most frequently were separated C.perfringens (42%), group of C.clostridioforme (24%). The sensitivity of separated strains to anti-microbial preparations was established with E-tests technique. The different species of Clostridia had different sensitivity to antibiotics. The strains C.perfringens are sensitive to all testing antibiotics whereas species related to group C.clostridioforme are resistant to fluoroquinolones (100%)б penicillin (67%). The resistance to fluoroquinolones was observed in C.scindens, C.ramosum, C.symbiosum. The strains C.scindens, C.tertium are resistant to penicillin. Among C.difficile, 93% of strains were resistant to penicillin, 57% - to imipenem and 100% - to fluoroquinolones.

The two techniques used are incoherent scatter of radio waves (by the EISCAT 224 MHz radar in Tromsø, Norway, 69.6° N, 19.3° E), and partial reflection of radio-waves (by the 2.8 MHz radar near Murmansk, Russia, 69.0° N, 35.7° E). Both radars give accurate electron density values during SPE, from heights 57–60 km and upward with the EISCAT radar and between 55–70 km with the partial reflection technique. Near noon, there is little difference in the solar zenith angle between the two locations and both methods give approximately the same values of electron density at the overlapping heights. During twilight, when the difference in solar zenith angles increases, electron density values diverge. When both radars are in night conditions (solar zenith angle >99°) electron densities at the overlapping altitudes again become equal. We use the joint measurements to validate model computations of the ionospheric parameters f+, λ, αeff and their variations during solar proton events. These parameters are important characteristics of the lower ionosphere structure which cannot be determined by other methods.

Abstract

The influence of atomic oxygen concentration on the height distribution of the main positive and negative ions and on electron density in the mesosphere is studied for the conditions prevailing during the solar proton event on 17 January 2005. It is shown by numerical modeling that the electron and ion density profiles are strongly dependent on the choice of the atomic oxygen profile. Experimental measurements of the electron density are used as the criterion for choosing the atomic oxygen profile in the mesosphere. With the help of modeling, the atomic oxygen profile in the daytime in the winter mesosphere is found to lead to a model electron density profile best matching the electron density profile obtained experimentally. As a result, with the help of modeling, we find the atomic oxygen profiles at various solar zenith angles in the winter mesosphere which lead to model electron density profiles matching the electron density profiles obtained experimentally. Alteration of the atomic oxygen concentration leads to a redistribution of the abundance of both positive and negative ion constituents, with changes in their total concentrations and transition heights. In consequence this results in changes of the electron density and effective recombination coefficient. For conditions of low concentration of atomic oxygen (during a solar proton event), the formation of cluster ions is the key process determining electron and ion densities at altitudes up to 77 km. The complex negative CO3− ion is formed up to about 74 km and the final NO3− ion, which is stable in relation to the atomic oxygen, is the dominant negative ion up to 74 km. As a result the transition heights between cluster ions and molecular ions and between negative ions and electron density are located at 77 km and 66 km, respectively.

Abstract

The generic theory of social innovation appeared in early 2000. Therefore, the role of social innovation was been increased in last decades in the light of macroeconomic transformation of World economy and the great development of social networks. Under social networking new business activites appear in the globalized market, even known as social business. Most programmes of the European Commision have orientation on a high speed of knowledge transfering from scientific research system to business or public life. New organization models of business use a tremendous amount of social information and usually social networks serve for the greater impact on improving structure of business environment and implementation of digital managerial solutions. Social networking serves for production of new knowledge and creation of the new ecosystems for social innovation. The authors of this article are presenting the new aspects of social innovation performance by using the content analysis for identification the role and functions of social innovation under digitalization of business environment. The research is focused on the clarification of social networking effects and better understanding why social innovation is becoming so powerful tool for business start-ups and social communication. The content analysis is used in case to highlight the comparative aspects of social innovation in different economical activities. The Chernobyl nuclear power plant accident in Ukraine in 1986 led to widespread radioactive releases into the environment - primarily of radioiodines and cesium - heavily affecting the northern portions of the country, with settlement-averaged thyroid doses estimated to range from 10 mGy to more than 10 Gy. The increased risk of thyroid cancer among exposed children and adolescents is well established but the impact of radioactive contamination on the risk of other types of cancer is much less certain. Abstract

Abundance estimates of copepodite and adult Calanus finmarchicus, Pseudocalanus spp., Oithona atlantica, O. similis, and Oncea borealis have been obtained twice a year by PINRO for the northeastern Norwegian Sea and the Barents Sea for the 30-year period 1964–1994. The data cover the upper 50 m of the water column during spring or early summer (i.e. April–July) along five inshore–offshore transects at 6730N, 7110N, 7250N, from North Cape to Bear Island, and at Kola (3330E). Along the Kola transect, data are also available for three depth strata covering the entire water column. For this study, information is extracted from these data sets for eight years, three cold (1979, 1980, and 1981) and several intermediate and warm years (1983, 1984, 1989, 1991, 1992). Spatial distribution patterns and the demography of C. finmarchicus, the target species, are described for the study area. In the time window studied, changes in abundance of C. finmarchicus between years appear to be a large-scale phenomenon throughout the entire study area. Unlike the results of previous studies, no clear interannual relationship between water temperature and copepod abundance was found. A correspondence analysis including the smaller copepod forms, as well as temperature and salinity data, revealed a consistent regional difference in the demography of Calanus, with stages from CIV to adult more common in the southwest of the study area. Abstract

The FINUDA spectrometer, devoted to hypernuclear physics and installed on the DAΦNE two rings collider at the Laboratori Nationali di Frascati, is able to monitor the relevant machine parameters, as luminosity, collision vertexes, c.m. energy and transversal momentum boost, during the process of data taking to study hypernuclear physics without affecting it. The collider parameters relevant to optimize the machine performances to the needs of the experiment are measured both on-line and offline in a run-to-run basis, in an efficient, redundant way, allowing the continuous extraction of reliable and cross-checked information on the machine working conditions.

Abstract

The OPERA neutrino detector at the underground Gran Sasso Laboratory (LNGS) was designed to perform the first detection of neutrino oscillations in appearance mode, through the study of nu_mu to nu_tau oscillations. The apparatus consists of a lead/emulsion-film target complemented by electronic detectors. It is placed in the high-energy, long-baseline CERN to LNGS beam (CNGS) 730 km away from the neutrino source. In August 2006 a first run with CNGS neutrinos was successfully conducted. A first sample of neutrino events was collected, statistically consistent with the integrated beam intensity. After a brief description of the beam and of the various sub-detectors, we report on the achievement of this milestone, presenting the first data and some analysis results., Submitted to the New Journal of Physics

Abstract

Coronary patients and patients with essential hypertension were found to have the highest levels of glycated hemoglobin (gly-Hb), specifically, of its HbAlc fraction, as was shown in screening of 615 subjects. Measurements of blood levels of gly-Hb and gly-albumin, study of pyruvate metabolism, oral glucose tolerance test, and glucose-insulin test in 74 patients with coronary disease and essential hypertension revealed a high prevalence of latent disorders of carbohydrate metabolism in this patient population, with disorders of carbohydrate metabolism manifesting primarily by an increase of blood levels of glycated proteins and by disorders of pyruvate metabolism. Glycation of blood proteins in patients with coronary disease and essential hypertension increases blood viscosity (1.6 ± 0.02), spontaneous platelet aggregation (46.59 + 2.07% vs. 18.1 ± 1.5% in control), reduces fibrinolysis, and is conducive to a parallel increase of the concentration of blood serum lipids. These patients develop disorders of insulin secretion after glucose loading, thyroid function is reduced with the deficit of tri-iodothyronine production (1.46 + 0.07 nmol/liter vs. 1.92 ± 0.1 nmol/liter in health, p < 0.05); hydrocortisone hypersecretion is observed (746.25 ± 49.74 nmol/liter vs. 358.23 + 9.8 nmol/liter in health, p < 0.001); hormonal dysfunction favors the formation of glucose toxicity mechanism in normal glycemia when no clinical signs of impaired glucose tolerance are seen.

Aims We aimed to determine the relation between baseline systolic blood pressure (SBP), change in SBP, and worsening renal function (WRF) in acute heart failure (AHF) patients enrolled in the Pre-RELAX-AHF trial. Abstract

At the Laboratory of High Energies (JINR, Dubna) a unique approach was elaborated with hypernuclei produced by the excitation of the beam nuclei and their decays observed at a distance of tens of cm behind the production target. While the very first experiments were carried out in the Synchrophasotron beams, more extensive hypernuclei research programme is planned for the Nuclotron accelerator and the new spectrometer created by the authors. At the time of the conference the first test run in the Nuclotron beam was in progress. Investigation of hydrogen hypernuclei isotopes is in the first line of the experimental research plans. Namely, lifetimes and production cross sections of Λ 4 H and Λ 3 H will be measured. Search for Λ 6 H is also among the tasks.

Abstract

In the HARP experiment the large-angle spectrometer is using a cylindrical TPC as main tracking and particle identification detector. The momentum scale of reconstructed tracks in the TPC is the most important systematic error for the majority of kinematic bins used for the HARP measurements of the double-differential production cross-section of charged pions in proton interactions on nuclear targets at large angle. The HARP TPC operated with a number of hardware shortfalls and operational mistakes. Thus it was important to control and characterize its momentum calibration. While it was not possible to enter a direct particle beam into the sensitive volume of the TPC to calibrate the detector, a set of physical processes and detector properties were exploited to achieve a precise calibration of the apparatus. In the following we recall the main issues concerning the momentum measurement in the HARP TPC, and describe the cross-checks made to validate the momentum scale. As a conclusion, this analysis demonstrates that the measurement of momentum is correct within the published precision of 3%., Comment: To be published by JINST

In the paper the results of numerical calculations of heterogeneity influence of active area silicon FET on development of electrothermal processes in a crystal of the transistor from the beginning of avalanche breakdown before catastrophic thermal breakdown of the device stepping at achievement of values of a lattice temperature in the current cord area, the appropriate temperature of fusion Au of substrate are presented.

Abstract

Puberty-youthful (hypothalamic) dispituitarism (PD) is a disease of adolescence and adolescence, the obligatory pathogenesis factors of which are damage to the diencephalon and hypothalamic-pituitary-endocrine dysregulation: hypercorticism, hyperaldosteronism, hyperinsulinotinotropin dysregulation, tibiotensinotropin dysregulation, gonotropinotropin dysotropin syndrome . PD is a chronic disease with a long, sometimes lifelong course. His prognosis is primarily determined by the correctness and regularity of treatment, which largely depends on the awareness of the parents about the clinic, diagnosis, therapy, possible outcomes of PD. Due to the peculiarities of adolescent psychology, patients often do not want to be treated, considering themselves healthy (this is very typical for boys), or do not seek medical help due to a severe astheno-depressive state, which is more typical of girls. Our long-term experience shows that patients come especially late in cases when the family has obese relatives and adolescent obesity does not cause serious anxiety in relatives. It is very important to conduct broader public health education on PD issues. A gross medical error should be considered a frequently encountered fact when a doctor refuses treatment for a patient with a PD, motivating: This will pass:. Indeed, in terms of severity, PD is mild, moderate, and severe. However, it should be remembered that any stressful situations, infections, traumatic brain injuries, etc. can change the subclinical course of the disease in an unfavorable direction, which must be explained to the patient and parents; and vice versa, even severe forms of PD, when a patient has IV degree obesity, can end in complete recovery with proper systematic treatment.

Abstract

Measurements of blood lipids and hormones (plasma renin, aldosterone, vasopressin, prolactin, atrial natriuretic peptide, 6-endorphine, thyrotropin, thyroid hormones) in two groups of patients suffering from obesity (group 1: 64 patients with arterial hypertension and group 2: 26 patients with normal arterial pressure) have brought the authors to a conclusion that arterial hypertension in young obese patients is an early manifestation of essential hypertension. Hormonal dysfunction in obese patients is conducive to early development of essential hypertension in cases when there is a hereditary predisposition to it.

Abstract

We have screened index cases from 25 Russian breast/ovarian cancer families for germ-line mutations in all coding exons of the BRCA1 and BRCA2 genes, using multiplex heteroduplex analysis. In addition we tested 22 patients with breast cancer diagnosed before age 40 without family history and 6 patients with bilateral breast cancer. The frequency of families with germline mutations in BRCA was 16% (4/25). One BRCA1 mutation, 5382insC, was found in three families. The results of present study, and those of a separate study of 19 breast-ovarian cancer families, suggest that BRCA1 5382insC is a founder mutation in the Russian population. Three BRCA2 mutations were found in patients with breast cancer without family history: two in young patients and one in patients with bilateral breast cancer. Four novel BRCA2 mutations were identified: three frameshift (695insT, 1528del4, 9318del4) and one nonsense (S1099X).
