International audience; BackgroundAntenatal betamethasone is recommended before preterm delivery to accelerate fetal lung maturation. However, reports of growth and neurodevelopmental dose-related side-effects suggest that the current dose (12 mg plus 12 mg, 24 h apart) might be too high. We therefore investigated whether a half dose would be non-inferior to the current full dose for preventing respiratory distress syndrome.MethodsWe designed a randomised, multicentre, double-blind, placebo-controlled, non-inferiority trial in 37 level 3 referral perinatal centres in France. Eligible participants were pregnant women aged 18 years or older with a singleton fetus at risk of preterm delivery and already treated with the first injection of antenatal betamethasone (11·4 mg) before 32 weeks’ gestation. We used a computer-generated code producing permuted blocks of varying sizes to randomly assign (1:1) women to receive either a placebo (half-dose group) or a second 11·4 mg betamethasone injection (full-dose group) 24 h later. Randomisation was stratified by gestational age (before or after 28 weeks). Participants, clinicians, and study staff were masked to the treatment allocation. The primary outcome was the need for exogenous intratracheal surfactant within 48 h after birth. Non-inferiority would be shown if the higher limit of the 95% CI for the between-group difference between the half-dose and full-dose groups in the primary endpoint was less than 4 percentage points (corresponding to a maximum relative risk of 1·20). Four interim analyses monitoring the primary and the secondary safety outcomes were done during the study period, using a sequential data analysis method that provided futility and non-inferiority stopping rules and checked for type I and II errors. Interim analyses were done in the intention-to-treat population. This trial was registered with ClinicalTrials.gov, NCT02897076.FindingsBetween Jan 2, 2017, and Oct 9, 2019, 3244 women were randomly assigned to the half-dose (n=1620 [49·9%]) or the full-dose group (n=1624 [50·1%]); 48 women withdrew consent, 30 fetuses were stillborn, 16 neonates were lost to follow-up, and 9 neonates died before evaluation, so that 3141 neonates remained for analysis. In the intention-to-treat analysis, the primary outcome occurred in 313 (20·0%) of 1567 neonates in the half-dose group and 276 (17·5%) of 1574 neonates in the full-dose group (risk difference 2·4%, 95% CI –0·3 to 5·2); thus non-inferiority was not shown. The per-protocol analysis also did not show non-inferiority (risk difference 2·2%, 95% CI –0·6 to 5·1). No between-group differences appeared in the rates of neonatal death, grade 3–4 intraventricular haemorrhage, stage ≥2 necrotising enterocolitis, severe retinopathy of prematurity, or bronchopulmonary dysplasia.InterpretationBecause non-inferiority of the half-dose compared with the full-dose regimen was not shown, our results do not support practice changes towards antenatal betamethasone dose reduction.

Archaeal sequences have been detected in human colostrum and milk, but no studies have determined whether living archaea are present in either of these fluids. Methanogenic archaea are neglected since they are not detected by usual molecular and culture methods. By using improved DNA detection protocols and microbial culture techniques associated with antioxidants previously developed in our center, we investigated the presence of methanogenic archaea using culture and specific Methanobrevibacter smithii and Methanobrevibacter oralis real-time PCR in human colostrum and milk. M. smithii was isolated from 3 colostrum and 5 milk (day 10) samples. M. oralis was isolated from 1 milk sample. For 2 strains, the genome was sequenced, and the rhizome was similar to that of strains previously isolated from the human mouth and gut. M. smithii was detected in the colostrum or milk of 5/13 (38%) and 37/127 (29%) mothers by culture and qPCR, respectively. The different distribution of maternal body mass index according to the detection of M. smithii suggested an association with maternal metabolic phenotype. M. oralis was not detected by molecular methods. Our results suggest that breastfeeding may contribute to the vertical transmission of these microorganisms and may be essential to seed the infant’s microbiota with these neglected critical commensals from the first hour of life.

Background:Two studies have demonstrated an analgesic effect of maternal milk odor in preterm neonates, without specifying the method of olfactory stimulation.Research aim:This study aimed to assess the analgesic effect of maternal milk odor in preterm neonates by using a standardized method of olfactory stimulation.Methods:This trial was prospective, randomized, controlled, double blinded, and centrally administered. The inclusion criteria for breastfed infants included being born between 30 and 36 weeks + 6 days gestational age and being less than 10 days postnatal age. There were two groups: (a) A maternal milk odor group underwent a venipuncture with a diffuser emitting their own mother’s milk odor and (2) a control group underwent a venipuncture with an odorless diffuser. The primary outcome was the Premature Infant Pain Profile (PIPP) score, with secondary outcomes being the French scale of neonatal pain—Douleur Aiguë du Nouveau-né (DAN) scale—and crying duration. All neonates were given a dummy.Results:Our study included 16 neonates in the maternal milk odor group and 17 in the control group. Neonates exposed to their own mother’s milk odor had a significantly lower median PIPP score during venipuncture compared with the control group (6.3 [interquartile range (IQR) = 5-10] versus 12.0 [IQR = 7-13], p = .03). There was no significant difference between the DAN scores in the two groups ( p = .06). Maternal milk odor significantly reduced crying duration after venipuncture (0 [IQR = 0-0] versus 0 [IQR = 0-18], p = .04).Conclusion:Maternal milk odor has an analgesic effect on preterm neonates.

Phenobarbital is widely used for treatment of neonatal seizures. Its optimal use in neonates and young infants requires information regarding pharmacokinetics. The objective of this study is to characterize the absolute bioavailability of phenobarbital in neonates and young infants, a pharmacokinetic parameter which has not yet been investigated. Routine clinical pharmacokinetic data were retrospectively collected from 48 neonates and infants (weight: 0.7-10 kg; patient's postnatal age: 0-206 days; GA: 27-42 weeks) treated with phenobarbital, who were administered as intravenous or suspension by oral routes and hospitalized in a paediatric intensive care unit. Total mean dose of 4.6 mg/kg (3.1-10.6 mg/kg) per day was administered by 30-min infusion or by oral route. Pharmacokinetic analysis was performed using a nonlinear mixed-effect population model software). Data were modelled with an allometric pharmacokinetic model, using three-fourths scaling exponent for clearance (CL). The population typical mean [per cent relative standard error (%RSE)] values for CL, apparent volume of distribution (Vd ) and bioavailability (F) were 0.0054 L/H/kg (7%), 0.64 L/kg (15%) and 48.9% (22%), respectively. The interindividual variability of CL, Vd , F (%RSE) and residual variability (%RSE) was 17% (31%), 50% (27%), 39% (27%) and 7.2 mg/L (29%), respectively. The absolute bioavailability of phenobarbital in neonates and infants was estimated. The dose should be increased when switching from intravenous to oral administration.

Objectif : évaluation des pratiques professionnelles (EPP) relative au clampage tardif (CT)/milking du cordon ombilical en salle de naissance.Matériel et méthode : deux évaluations descriptives et transversales ont été réalisées : une évaluation monocentrique au sein d’un CHU et une évaluation interrégionale multicentrique. La population ciblée est composée d’obstétriciens, pédiatres, sages-femmes et puéricultrices travaillant en salle de naissance d’une maternité de type 3 pour l’enquête monocentrique. Trois questionnaires ont été administrés sous format électronique à 1 mois, 6 mois et 18 mois de la mise en place du protocole de CT pour l’étude monocentrique. La réponse au questionnaire était anonyme. Un questionnaire a été administré dans a 2eme enquête via les réseau de périnatalités. Une réponse par centre était attendu. Des statistiques descriptives ont été réalisées sur les différents items (fréquence et effectif). Pour les questions à choix multiples, les réponses ont été présentées par réponse isolée et par association de réponses. Des analyses comparatives classiques ont été effectuées (tests de Chi2). Le seuil de significativité des tests a été fixé à 5%.Résultats : 94 maternités ont été inclues à l’enquête interrégionale, 54 ont participés. Une pratique faible du CT a été mise en évidence dans 63% des établissements et 92,6% pour le milking. 79,6% des maternités n’avaient pas de protocole concernant le CT/milking. Cependant l’intérêt pour être former et intégrer a été exprimé par 93,3% des maternités. En 18mois 148 réponses aux questionnaires de l’étude monocentrique ont été recueillies. Nous avons mis en évidence une augmentation très significative de la pratique du CT (taux de CT déclaré réalisé dans 75 à 100% des accouchements : 12,2% à T0, 72,3% à T2, p=0.000). Le taux de réalisation du milking augmente significativement, sur le nombre de personne déclarant avoir déjà réalisé un milking personnellement (p=0,017) ainsi que le taux de milking réalisé lorsque le CT n’a pas été possible (p=0,000). Lorsqu’il n’était pas réalisé les raisons étaient de l’ordre d’une contre-indication médicale ou de l’oubli (oubli = 42,4% à T0, 13,6% à T2), aucune difficulté technique n’a été retenue.Conclusion : la pratique du CT est simple, peu couteuse et recommandée sur le plan international et national. Les résultats de l’enquête interrégionale ont mis en évidence le manque de pratique du CT, l’absence de protocolisation du geste dans de nombreux centres, mais un réel intérêt pour intégrer le CT aux pratiques. Les résultats de l’étude monocentrique ont permis d’identifier au fur et à mesures de l’enquête les difficultés rencontrées, et d’établir des leviers potentiels pour permettre une amélioration de la pratique et ainsi le devenir des patients à court moyen longs terme.

Thèse présentée sous la forme d'une "Thèse Article"; Objectifs : évaluer l’efficacité d’un programme d’éducation parentale précoce (PEPP) en maternité sur les connaissances parentales, l’anxiété maternelle et le recours aux soins. Design : étude prospective, monocentrique, comparative, randomisée, contrôlée, ouverte.Lieu : maternité de niveau 3 (Marseille, France).Sujets : cent mères de nouveau-né à terme, eutrophe (poids de naissance > 5ème percentile) et en bonne santé. Intervention : PEPP d’une heure délivrée à l’aide d’un support numérique audio-visuel (groupe interventionnel, n = 50), comparé aux conseils habituels prodigués en maternité à propos des soins des nouveau-nés (groupe contrôle, n = 50).Critères d’évaluation : le critère d’évaluation principal était l’évaluation des connaissances maternelles à l’aide d’un questionnaire, noté sur 20, avant (Pré-test) et après (Post-test) le PEPP. Les critères d’évaluation secondaires étaient l’évaluation des connaissances à 1 mois (Recall-test), de l’anxiété maternelle mesurée par le questionnaire STAI (avant et après le PEPP et à 1 mois) et de la pertinence du recours médical dans le premier mois de vie. Résultats : Le score Post-test était plus élevé dans le groupe intervention comparé au groupe contrôle mais cette différence n’était pas significative (15,4 vs. 14,0 ; p=0,073).
