Abstract

BackgroundMultisystem inflammatory syndrome in children (MIS-C), a hyperinflammatory condition associated with SARS-CoV-2 infection, has emerged as a serious illness in children worldwide. Immunoglobulin or glucocorticoids, or both, are currently recommended treatments.MethodsThe Best Available Treatment Study evaluated immunomodulatory treatments for MIS-C in an international observational cohort. Analysis of the first 614 patients was previously reported. In this propensity-weighted cohort study, clinical and outcome data from children with suspected or proven MIS-C were collected onto a web-based Research Electronic Data Capture database. After excluding neonates and incomplete or duplicate records, inverse probability weighting was used to compare primary treatments with intravenous immunoglobulin, intravenous immunoglobulin plus glucocorticoids, or glucocorticoids alone, using intravenous immunoglobulin as the reference treatment. Primary outcomes were a composite of inotropic or ventilator support from the second day after treatment initiation, or death, and time to improvement on an ordinal clinical severity scale. Secondary outcomes included treatment escalation, clinical deterioration, fever, and coronary artery aneurysm occurrence and resolution. This study is registered with the ISRCTN registry, ISRCTN69546370.FindingsWe enrolled 2101 children (aged 0 months to 19 years) with clinically diagnosed MIS-C from 39 countries between June 14, 2020, and April 25, 2022, and, following exclusions, 2009 patients were included for analysis (median age 8·0 years [IQR 4·2-11·4], 1191 [59·3%] male and 818 [40·7%] female, and 825 [41·1%] White). 680 (33·8%) patients received primary treatment with intravenous immunoglobulin, 698 (34·7%) with intravenous immunoglobulin plus glucocorticoids, 487 (24·2%) with glucocorticoids alone; 59 (2·9%) patients received other combinations, including biologicals, and 85 (4·2%) patients received no immunomodulators. There were no significant differences between treatments for primary outcomes for the 1586 patients with complete baseline and outcome data that were considered for primary analysis. Adjusted odds ratios for ventilation, inotropic support, or death were 1·09 (95% CI 0·75-1·58; corrected p value=1·00) for intravenous immunoglobulin plus glucocorticoids and 0·93 (0·58-1·47; corrected p value=1·00) for glucocorticoids alone, versus intravenous immunoglobulin alone. Adjusted average hazard ratios for time to improvement were 1·04 (95% CI 0·91-1·20; corrected p value=1·00) for intravenous immunoglobulin plus glucocorticoids, and 0·84 (0·70-1·00; corrected p value=0·22) for glucocorticoids alone, versus intravenous immunoglobulin alone. Abstract

In many situations, we would like to hear desired sound events (SEs) while being able to ignore interference. Target sound extraction (TSE) tackles this problem by estimating the audio signal of the sounds of target SE classes in a mixture of sounds while suppressing all other sounds. We can achieve this with a neural network that extracts the target SEs by conditioning it on clues representing the target SE classes. Two types of clues have been proposed, i.e., target SE class labels and enrollment audio samples (or audio queries), which are pre-recorded audio samples of sounds from the target SE classes. Systems based on SE class labels can directly optimize embedding vectors representing the SE classes, resulting in high extraction performance. However, extending these systems to extract new SE classes not encountered during training is not easy. Enrollment-based approaches extract SEs by finding sounds in the mixtures that share similar characteristics to the enrollment audio samples. These approaches do not explicitly rely on SE class definitions and can thus handle new SE classes. In this paper, we introduce a TSE framework, SoundBeam, that combines the advantages of both approaches. 10. Few-shot learning of new sound classes for target sound extraction

Abstract

Target sound extraction consists of extracting the sound of a target acoustic event (AE) class from a mixture of AE sounds. It can be realized using a neural network that extracts the target sound conditioned on a 1-hot vector that represents the desired AE class. With this approach, embedding vectors associated with the AE classes are directly optimized for the extraction of sound classes seen during training. However, it is not easy to extend this framework to new AE classes, i.e. unseen during training. Recently, speech, music, or AE sound extraction based on enrollment audio of the desired sound offers the potential of extracting any target sound in a mixture given only a short audio signal of a similar sound. In this work, we propose combining 1-hot- and enrollment-based target sound extraction, allowing optimal performance for seen AE classes and simple extension to new classes. In experiments with synthesized sound mixtures generated with the Freesound Dataset (FSD) datasets, we demonstrate the benefit of the combined framework for both seen and new AE classes. Abstract

The present research aims to identify the main benefits and limitations that arise in Ecuadorian organizations when accessing the certification of the INEN Quality Seal, analyzing their competitive environment with the obtained results. The identification was carried out through semi-structured interviews with a segment that holds the certification. These interviews were conducted with personnel related to quality, such as managers, directors, and supervisors. Due to their experience, factors obtained not only from certification but also from management systems were further explored, as well as the theoretical validation of the external and internal benefits that ISO offers in its implementation. The identified benefits include: exportation, increased competitiveness, product improvement, enhancement of customer expectations, among others. Among the limitations are: implementation and adaptation of laboratories, search for equipment and technology that meet standards, trained personnel, excessive documentation, costs, regulations, among others. These limitations are timely overcome during the certification process. The analysis converges on the opportunities found in differentiating their products with a quality endorsement, resulting in satisfied customers, competitive openings to new markets, including exportation, and process improvement. The present threats remain constant due to new competitors who prioritize price, leading to the loss of potential customers, which becomes significant over time. Suppliers have not posed an evident threat as there is control, prioritizing the quality of raw materials.

Abstract

La eventual desconexión entre las preferencias de los votantes y las políticas concretas es de especial interés en Estados descentralizados, donde cualquier iniciativa política también puede verse como una erosión o un refuerzo del autogobierno. El objetivo de este artículo es aplicar este marco conceptual a la armonización del Impuesto sobre Sucesiones y Donaciones español (ISD), contrastando econométricamente si los ciudadanos que viven en comunidades autónomas que lideraron una carrera fiscal a la baja en este impuesto o en aquéllas más proclives al autogobierno se oponen en mayor medida que el resto de españoles a la armonización tributaria de este impuesto. Empíricamente, explotamos los datos del Barómetro Fiscal de 2019 del Instituto de Estudios Fiscales. Según nuestros principales resultados, los ciudadanos que viven en comunidades autónomas con un menor ISD y los ciudadanos que viven en algunas regiones donde gobiernan partidos nacionalistas tienen incluso más probabilidades de favorecer la armonización., The eventual disconnection between voters ́ preferences and specific policies is of special concern in decentralized countries, where any policy initiative might be simultaneously seen either as an erosion or a reinforcement of self-rule. The aim of this paper is to apply this framework to the harmonization of the Spanish Inheritance and Gift Tax (IGT). We econometrically contrast whether citizens ́ living in Autonomous Communities (ACs) that led a tax race to the bottom regarding this tax or in those that exert a greater taste for political autonomy are more likely to oppose a more nationally uniform tax than the rest of Spaniards. Empirically, we exploit data from the 2019 wave of the Institute for Fiscal Studies Fiscal Barometer. According to our main results, citizens living in low taxation ACs and citizens living in some more pro-self-rule regions are even more likely to favor harmonization.

Abstract

Manufacturing and testing of pharmaceutical products frequently occur in multiple facilities within a company's network. It is of interest to demonstrate equivalence among the alternative testing/manufacturing facilities to ensure product consistency and quality regardless of the facility where it was manufactured/tested. In the Frequentist framework, equivalence testing is well established when comparing two labs or manufacturing facilities; however, when considering more than two labs or production sites, the Frequentist approach may not always offer appropriate or interpretable estimates for demonstrating equivalence among all of them simultaneously. This paper demonstrates the utility of Bayesian methods to the equivalence assessment of multiple groups means, with a comparison against traditional Frequentist methods. We conclude that a Bayesian strategy is very useful for addressing the problem of multi-group equivalence. While it is not our intention to argue that Bayesian methods should always replace Frequentist ones, we show that among the advantages of a Bayesian analysis is that it provides a more nuanced understanding of the degree of similarity among sites than the hypothesis testing underpinning the Frequentist approach.
