Amaç: Kütanöz advers reaksiyonlar, epilepside anti-nöbet ilaçlarının (ANİ) kesilmesinin en önemli nedenlerinden biridir.Ancak bu tür kesilmeler nöbetlerin artmasına neden olabilir. Bu çalışma, çocuklarda ANİ ile ilişkili döküntü nüksü içinrisk faktörlerini araştırmaktadır.Yöntem: ANİ’lere bağlı tek döküntü gelişen hasta grubu (Grup 1), döküntüsü tekrarlayan hasta grubu (Grup 2) ve kontrolgrubundan oluşan retrospektif vaka-kontrol çalışmasında Grup 1 ve Grup 2’deki hastaların demografik ve klinik özellikleri döküntü tekrarı açısından karşılaştırıldı.Bulgular: Grup 1, Grup 2 ve kontrol grubu sırasıyla 112, 33 ve 166 hastadan oluştu. Objective: One of the most frequently investigated epigenetic factors in the pathogenesis of atopic dermatitis (AD) is food. There are different views on the effects of micronutrients on AD development and disease severity. In our study, we aimed to determine the feeding style and micronutrient levels and to investigate their relationship with AD clinical scores in infants with AD.Materials and Methods: We determined the serum vitamin A, vitamin B12, vitamin D, vitamin E, zinc, and iron levels of infants with AD at the time of presentation to our pediatric allergy clinic. We recorded the feeding types and AD clinical scores of the infants. The relationship between micronutrient levels, nutrition type, and clinical scores of the patients was evaluated.Results: A total of 63 patients, 34 (54%) male, with a median age of 6 (min-max: 2-21) months were included in the study. Breastfeeding (82.5%) was the most preferred feeding method. We did not detect any difference in terms of micronutrient levels between the feeding types. Micronutrients other than vitamin D and zinc were not correlated with disease severity. Moreover, low vitamin D levels were common despite the group receiving prophylaxis.Conclusion: Prevention of vitamin D and zinc deficiencies in patients diagnosed with AD may be beneficial in reducing the severity of the disease. It may be useful to investigate whether low vitamin D in patients with AD is a cause or a result.

Cystic fibrosis is a life-shortening disease characterized by the mutation of the salt transporter cystic fibrosis transmembrane conductance regulator (CFTR) protein. Elexacaftor/tezacaftor/ivacaftor treatment, which has been used since 2019, has had effects such as improving the quality of life, improving respiratory functions, and reducing the rate of pulmonary exacerbations. The real-life side effects of the drug are still emerging due to the new use of modulator therapies and their new spread around the world. There are no proven protocols regarding these side effects yet. Strategies for managing adverse drug reactions to these drugs are important to ensure that as many patients as possible benefit from them.

Objective:Urticariais a condition characterized by the development of blisters (hives), angioedema, or both. Acute urticaria is the appearance of rashes lasting 6 weeks or less, angioedema, or both. It is recommended to use the urticaria control test to evaluate disease control in patients with CSU. This study aimed to evaluate the factors affecting urticaria control in patients followed up with the diagnosis of chronic spontaneous urticaria.Material and Methods:Urticaria control results of children diagnosed with chronicurticaria were evaluated retrospectively and factors that could affect urticaria control were investigated.Results:Relationships between the urticaria control test and age, gender, onset time of the complaint, family history of atopy, and laboratory values such as anti-nuclear antibody (ANA) and c4 were examined.Conclusion:Although chronic urticaria has a mild course in children, parameters such as gender, age, and ANA positivity may affect the control of the disease.Keywords:Pediatrics, chronic urticaria, urticaria control test

Giriş: Konjenital Plazminojen eksikliği(KPE), tüm mukozal yüzeylerde, özellikle konjonktiva da fibrinaçısından zengin psödomembran oluşumu ile karakterizedir. Solunum sistemi tutulumu yaygındır;fibrin sıklıkla üst veya alt solunum yollarını tıkayarak ölüme neden olur. Pekçok tedavi yöntemidenenmesine rağmen, özellikle solunum sistemi tutulumunda henüz kesin bir tedavi yoktur.Burada, ciddi solunum bulgularıyla başvuran, sağ akciğer total atelektazisi saptanan ve yeni bir tedavi yönteminindenendiği KPE tanılı olgumuzu sunmak istedik.Gereç-Yöntem: Radyolojik görüntülemede sağ akciğer tamamen kollabe, trakeal bronşial ağaçta yaygın yeryer kalsifikasyon içeren polipoid yumuşak doku dansiteleri ve atelektazi ve bronşiektaziler izlendi. Non-invazifmekanik ventilasyon ve antibiyotik tedavisine yanıt vermeyin hastaya her gün uygulanmak üzere intra-venöz tazedonmuş plazma (TDP) başlandı. Rijit bronkoskopide trakeobronşiyal ağaç ve sağ ana bronş, diffüz fibrin plaklarıile tıkalı olarak görüldü. Forseps ile kalsifik fibrin membranlar çıkarıldı. TDP ve doku plazminojen aktivatörü (tPA)bronkoskopik olarak sağ ve sol ana bronşlara uygulandı ve aspire edildi. Son olarak, intratrakeal olarak 10 mL TDPverildi ve prosedür aspirasyon olmadan sonlandırıldı. Hastanın atelektazisinin düzelmemesi üzerine 10 gün sonraaynı prosedür ikinci kez yapıldı.Tartışma: KPE solunum sistemi tutulumunun tedavisi oldukça zordur ve hastalıkla ilişkili mortalitenin en önemlinedenidir. Şiddetli vakalarda, fibrin plakları dikkatlice çıkarılmalıdır; ancak bu yeni lezyon oluşumunu tetikleyebilir.Solunum yollarını tıkayan fibrin plakları çıkarılmadan önce TDP ile plazminojen seviyelerinin artırılması sonderece önemlidir. Konjenital plazminojen eksikliği olan hastalarda akciğer tutulumunun neden olduğu mortaliteoldukça yüksektir.Sonuç: İntratrakeal tPA ve TDP, ciddi solunum sıkıntısı olan hastalar için kullanılabilir bir tedavi yöntemidir

GİRİŞ VE AMAÇ: Yüksek immunglobilin E düzeyi alerjik hastalıklar, bazı immun yetmezlikler ve parazitenfeksiyonları ile ilişkili olduğu bilinmektedir. Astım tanılı hastalarda çok yüksek IgE seviyesinin( >1000IU/ml) etkileri ve kliniğe yansımaları yeterince aydınlatılmış değildir. Astımlı çocuklarda çok yüksek IgEdüzeyine eşlik eden alerjik duyarlanmalar ve diğer immunglobilinler ile arasındaki korelasyon vehastaların demografik özelliklerini araştırmayı amaçladık.YÖNTEM: Ocak 2015-Eylül 2020 arasında IgE ölçümü yapılmış çocuklar değerlendirildi ve IgE>1000IU/ml olan olgulardan çocuk alerji polikliniğinden astım tanısı ile izlenen hastalar çalışmaya dâhil edildi.Elektronik kayıt sistemi kullanılarak hastaların demografik verileri, deri prik testleri ve/veya spesifik IgEölçümleri ve immunglobilin düzeyleri kaydedildi.BULGULAR: IgE ölçümü yapılan 13256 çocuk elektronik kayıtlardan taranarak IgE>1000 IU/ml olan303 hastadan(%2,29) astım tanılı 126(%41,6) hasta saptandı. IgE medyan: 1530 (1023-2484) idi.Hastaların 85(%67,5).’i erkekti. Yüksek IgE ve erkek cinsiyet arasında anlamlı ilişki mevcuttu.(χ²(1)=15,365 p: U: 1265, p:0,013). Olguların 66 (%52,8)’unda eozinofili mevcuttu. Eozinofil medyan 500(0-1600) 103/uLbelirlendi. IgE düzeyi ile eozinofil sayısı ve diğer immunglobilinler arasında korelasyon saptanmadı.(Eozinofil için p: 0,08 IgA için p: 0,584, IgG için p:0,689, IgM için p:0,394) 67(%53,1) olguda sadeceastım, 56(%44,4) olguda astım haricinde diğer alerjik hastalıklar da mevcuttu. 44(%35)’ünde alerjik rinit,3(%2,3) olguda astım dışında farklı komorbid hastalıklar belirlendi. Tek alerjen duyarlılığı 47(%45,6)olguda ve en sık ev tozuydu(n:26/%55,3)). Çoklu alerjen duyarlılığı 56 (%56,4) olguda belirlendi. Bütünolgular içinde ev tozu 57(%55,3) olguda, polen duyarlılığı 53 (%51,4), hayvan 40(%38,8), mantar11(%10,6), besin 3(%2,9) olguda saptandı. Polen duyarlılığı(n:53) olan olgularda en sık ot poleni(n:44/%83) ve zeytin ağacı poleni(n:33/%%62) duyarlılığı mevcuttu. Hayvan alerjeni duyarlılığı olanbütün olgularda (n:40) kedi duyarlılığı vardı. IgE düzey ölçümlerinin mevsimi ile saptanan alerjenduyarlılığı arasında bir ilişki belirlenmedi.TARTIŞMA VE SONUÇ: Çalışmamızda astımlı hastalarda en sık ev tozu alerjisi, polen alerjisi ve kedialerjisi saptanmıştır. Alerjenlerin yüksek olduğu mevsimler ile IgE ilişkisi gözlenmemiştir. Yüksek IgE ileastım hastalık şiddetiyle ilgili çalışmalara ihtiyaç vardır. Çok yüksek IgE düzeyi saptandığında alerji dışınedenlerin araştırılması önerilse de atopik hastalıklarda da IgE düzeyi çok yükselebilir.

GİRİŞ: Yüksek serum immünoglobulin (Ig) E seviyeleri alerjik hastalıklar, paraziter enfeksiyonlar ve bazı bağışıklıkyetersizlikleri ile ilişkilidir; bununla birlikte, düşük IgE düzeylerinin insan bağışıklık sistemi üzerindeki potansiyeletkileri ve klinik etkileri iyi bilinmemektedir. Bu çalışma, çocuklarda ve yetişkinlerde çok düşük IgE seviyelerineeşlik eden bozuklukları belirlemeyi amaçlamaktadır.GEREÇ-YÖNTEM: Ocak 2015-Eylül 2020 tarihleri arasında IgE düzeyi belirlenen hastalar incelendi ve IgE düzeyi< 2 IU/mL olan hastalar bu çalışmaya dahil edildi. Hastaların demografik verileri, immünoglobulin düzeyleri,otoantikor sonuçları ve hastaların tanıları hastanemizin elektronik kayıt sisteminden kaydedildi.BULGULAR: 34.809 hastada (21.875 çocuk, 12.934 yetişkin) IgE seviyeleri ölçüldü ve 130 hastanın IgE seviyeleri < 2IU/mL idi. 57 hasta çocuk (%0,26); 73 hasta yetişkin (%0,56) idi. 34’ünde malign bir hastalık vardı (9’u çocuk) (%26),20’si (3’ü çocuk) otoimmün hastalıklar (%15.4) ve 17’si (14’ü çocuk) immünyetmezlik (%13.1) idi. Çocuklarda ensık nedenler diğer hastalıklar, immün yetmezlik ve malignite, yetişkinlerde malignite, otoimmün bozukluklar vediğer hastalıklardı. IgE seviyesi, diğer immünoglobulinlerin seviyeleri ile herhangi bir korelasyon göstermedi.SONUÇ: Nadir olmakla birlikte, düşük bir IgE düzeyinin malignitelere, otoimmün bozukluklara ve immünyetmezliklere eşlik ettiği gösterilmiştir. IgE düzeyleri çok düşük olan hastalar sistemik bozukluklar açısındandikkatle izlenmelidir.

Ampiyem plevral sıvının bakteriyel invazyonu sonucu plevral aralıkta püy birikmesidir. En sıkkarşılaşılan ajanlar Pnömokok, S.aureus ve H.influenzadır. Çocuklarda ampiyem sıklıkla bakteriyel pnömonilerinyetersiz veya geç tedavisi sonucu gelişebilir. Erken dönemde sadece antibiyoterapi ile tedaviedilebilir. Geç dönemde tedavide tüp torakoskopi ve fibrinolitik ajan kullanımı, yanıt alınamaz ise videoyardımlı torakoskopik cerrahi (VATS) yapılması, son basamak olarakta açık torakotomi ile dekortikasyonönerilmektedir. Ampiyeme sık neden olan önlenebilir enfeksiyon ajanlarından korunmada, aşılarınyapılması için gösterilen her çaba çocuk ölümlerini ve hastalık morbiditesini azaltmada etkin roloynayacaktır.

In December 2019, a previously unknown type of coronavirus was detected in China and named as "severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2)". The World Health Organization has named the SARS-CoV-2 related as coronavirus disease-2019 (COVID-19) and declared it as a pandemic. There is a limited data about the COVID-19 disease for the pediatric patients. In this study, it was aimed to evaluate the epidemiological, clinical, laboratory and radiologic findings, treatment and clinical outcomes of patients admitted to the pediatric emergency department with the suspicion of COVID-19. Between March 11 and dune 16, 2020, patients aged between 1 month-18 years admitted to the pediatric emergency department and who have an indication for sampling for the polymerase chain reaction (PCR) method with the suspicion of COVID-19 according to the current guidelines published by the Ministry of Health were included in the study. The demographic characteristics, symptoms, durations and the history of contact with the suspected/definite COVID-19 cases were questioned in the patients with positive results. Physical examination, laboratory and imaging data of the patients were recorded. According to clinical severity, patients were divided into five groups. Treatment methods, ward/intensive care unit admission, length of stay at hospital, and prognosis were recorded. Of the 237 patients included in the study, 45 (18.9%) of the samples were positive and 192 (81.1%) were negative. There was a history of contact with COVID-19 positive case in 38 (85.6%) of COVID-19 PCR positive patients. The mean time for onset of symptoms after contact was 3.5 +/- 1.7 days. Twenty-one of the patients (46.6%) were asymptomatic and the most common symptom was fever (34.1%) and cough (27.3%). Of the patients whose laboratory tests were requested, lymphopenia wasdetected in 50% and 52.3% of procalcitonin, 23.5% of C-reactive protein and 64.7% of D-dimer values were found to be high. Chest radiography was obtained from 45.4% of the patients; 90.0% were evaluated as normal, bronchovascular change, pleural effusion and consolidation were detected in one of each (5.0%) patient. Thorax computed tomography (CT) was obtained from 4 (9.0%) patients. One patient had normal CT findings, two patients had consolidation, one patient had peripheral ground-glass appearance and one patient had pleural effusion. Antibiotics were started in 38.6% of the patients and the most commonly used antibiotic was azithromycin (34.1%). Oseltamivir was started in one (2.3%) patient, and 10 (24.7%) patients were treated with hydroxychloroquine. There were no serious and critical cases according to the clinical severity. Pediatric patients constitute a small part of COVID-19 individuals in the community, and a significant part of them are asymptomatic, and patients who are symptomatic present with a mild clinic. In our study, most of the patients had a history of contact with COVID-19 positive cases, therefore, it should be questioned when evaluating a pediatric patient. There were no specific findings for COVID-19 positive patients in terms of laboratory and radiology.
