Objectives:This study aims to define and compare sensory phenotypes in cervical radiculopathy patients exhibiting neu-ropathic pain (NP) components with healthy volunteers using clinical examination and quantitative sensory test (QST) findings. Another aim of the study is to show whether symptomatic components of the pain detect questionnaire (PDQ) are correlated with the QST findings, which may help clinicians indicate patients with sensory abnormalities without the use of specialized tests.Methods: Fifty-seven participants were included in the study, including patients with NP (n=20) and healthy volunteers (n=37). After obtaining the sociodemographic and clinical data of the participants, the PDQ was performed in patients with pain followed by QST analysis in all participants.Results: Analyses between painful and non-painful extremities yielded no differences in all groups for QST (p>0.05). Amaç: Tıbbi teknolojideki ve monitörizasyondaki gelişmeler günübirlik cerrahi uygulamalarında hızlı bir artışa neden oldu. Bu çalışmada günübirlik anestezi sonuçlarımızın sunulması hedeflendi. Gereç ve Yöntem: Günübirlik cerrahi geçiren 907 hasta prospektif gözlemsel olarak çalışmaya alındı. Acil hastalar, ameliyathane dışı anestezi uygulamaları ve anestezi ekibinin eşlik etmediği operasyonlar çalışma dışı bırakıldı. Hastalara yapılacak işleme göre genel anestezi, rejyonal anestezi veya sedo- analjezi uygulandı. Komplikasyonlarla ilişkili risk faktörleri ile demografik veriler, anestezi yöntemi, intraoperatif ve postoperatif değişkenler arasında istatiksel anlamlılık araştırıldı. Bulgular: Hastanemizde Şubat 2020’de 2194 hasta operasyona alındı. Hastalardan 907’si (%41,3) günübirlik kriterlerini karşılamaktaydı. Hastaların %26,8’ini (243 hasta) pediyatrik hastalar oluşturmaktaydı. Göz hastalıkları (%67) en sık günübirlik operasyon yapılan klinik oldu. İntratrakeal genel anestezi (%36,7), en sık uygulanan anestezi şekliydi. İntraoperatif ve postoperatif komplikasyon sırasıyla hastaların %2,9’da ve %1,9’da gelişti. Operasyonun endoskopik yapılması (OR: 4,357 [1,909- 9,943], p?0,001) ve bazal kalp hızı yükseklikleri (OR:1,030 [1,009-1,052], p=0,006) peroperatif komplikasyonlar ile ilişkiliydi. Sonuç: Operasyonun endoskopik uygulanması ve bazal kalp hızı yüksekliği peroperatif komplikasyon ile ilişkili iken; anestezi türü ile komplikasyon arasında anlamlı ilişki olmadığı saptandı. Hızlı ve kısa etkili anesteziklerin, analjeziklerin ve nöromuskuler blokörlerin yanı sıra geliştirilmiş monitörizasyon yöntemlerinin kullanımı perioperatif anestezi komplikasyonlarını azaltacaktır.

Objectives: the aim of this study was to evaluate the validity and reliability of the Turkish version of the Brief Pain InventoryShort Form for patients with chronic nonmalignant pain. Methods: An analytical design was used. A total of 192 patients were included in the study. A demographic questionnaire and the Brief Pain Inventory-Short Form were used to collect data. Content validity was assessed by experts and construct validity was tested using exploratory factor analysis. Reliability analyses estimated the internal consistency and test-retest reliability. Cronbach’s alpha and the item-total correlations were calculated for the subscales to examine internal consistency. Results: Exploratory factor analysis yielded 2 factors: pain severity and pain interference, which accounted for 68.81% of the total variance. the coefficient alpha of both subscales demonstrated good internal consistency. the item-total correlations of the scale ranged between 0.56 and 0.87. the test-retest reliability was r=0.774 for pain severity and r=0.808 for pain interference (p=0.001). Conclusion: the Turkish version of the Brief Pain Inventory-Short Form is a valid and reliable instrument to assess chronic nonmalignant pain., Amaç: Bu çalışmanın amacı, kronik nonmaling ağrısı olan hastalarda Brief Ağrı Envanteri Kısa Formu’nun Türkçe versiyonunun geçerliğini ve güvenirliğini değerlendirmektir. Gereç ve Yöntem: Analitik çalışma deseni kullanıldı. Çalışmaya 192 hasta dahil edildi. Veri toplamada sosyodemografik soru formu ve Brief Ağrı Envanteri Kısa Formu kullanıldı. Envanterin geçerliğini değerlendirmek için içerik ve yapı geçerliği kullanılmıştır. Içerik geçerliği uzman görüşü ile değerlendirildi. Yapı geçerliği için açımlayıcı faktör analizi kullanılmıştır. Güvenirlik analizi için iç tutarlılık ve test tekrar test güvenirlik analizleri yapılmıştır. İç tutarlılık için alt boyutların Cronbach alfa ve maddetoplam korelasyonları hesaplandı. Bulgular: Açımlayıcı faktör analizi 2 faktör göstermiştir, ağrı şiddeti ve ağrı girişimi. İki faktör toplam varyansin %68.81’ini açıklamıştır. Her iki alt boyutun alfa katsaysı iyi iç tutarlılık göstermiştir. Ölçeğin madde toplam korelasyonları 0.56 ile 0.87 arasındadır. Test–retest güvenirliği ağrı şiddeti için r=0.774 ve ağrı girişimi için r=0.808’dir (p=0.001). Sonuç: Brief Ağrı Envanteri Kısa Formu’nun Türkçe versiyonu, kronik nonmaling ağrıyı değerlendirmek için güvenilir ve geçerli bir araçtır.

Amaç: Palyatif bakımda yaşanan etik sorunlara odaklanan mevcut literatürün gözden geçirilmesi ve çalışmalardan elde edilen verilerin sistematik biçimde incelenmesidir. Gereç ve Yöntemler: Çalışmanın evrenini, 1 Ocak 2008-1 Temmuz 2018 tarihleri arasında “Google Scholar”, “Pubmed”, “Science Direct”, “Proquest”, “Cochrane”,” EBSCOhost”, “Ovid”, “Web of Science”, “Clinical Key”, “Google Akademik” ulusal ve uluslararası veri tabanları taranarak ulaşılan makaleler oluşturmuştur. İncelemede “etik sorunlar”, “etik ikilemler”, “etik çatışmalar”, “etik problemler”, “palyatif bakım”, “palyatif bakım ünitesi”, Türkçe ve İngilizce anahtar kelimeleriyle palyatif bakımda karşılaşılan etik sorunlara ilişkin verileri içeren, yayın dili Türkçe ve İngilizce olan, son 10 yılda yayımlanmış ve tam metnine ulaşılabilen makaleler seçilmiştir. Bulgular: Bu makalelerden araştırma kriterlerini karşılayan toplam 31 çalışma incelemenin örneklemini oluşturmuştur. Taramalardan elde edilen sonuçlar doğrultusunda; kaynak eksikliği, ilaç yönetimi, iletişim, hasta-hasta yakını-sağlık profesyoneli arasındaki ilişkiler, sedasyon yönetimi gibi çeşitli başlıklarda etik sorunlar yaşandığı görülmüştür. Sonuç: Bu sistematik çalışma sonucunda; palyatif bakımda çeşitli boyutlarda etik sorunların yaşandığı, bu etik sorunlardan en çok etik ikilemlerin meydana geldiği, etik ilke ve değer ihlallerinin olduğu, etik ikilemlerin genellikle tedavi sürecine ilişkin olduğu, sıklıkla da kaynak eksikliği nedeni ile etik çatışmaların yaşandığı ve özerklik ilkesinin sıklıkla ihlal edildiği görülmüştür., Objective: the purpose of this study was to review the present literature focusing on ethical problems in palliative care and to examine the data obtained from the studies systematically. Material and Methods: the universe of the study consisted of articles published between 1 January 2008-1 July 2018, that were accessed by screening national and international databases that were “Google Scholar”, “Pubmed”, “Science Direct”, “Proquest”, “Cochrane”, “EBSCOhost”, “Ovid”, “Web of Science”, “Clinical Key”. Articles that were reached by using the keywords “ethical issues”,“ethical dilemmas”, “ethical conflicts”, ”ethical problems”, “ palliative care”, “palliative care unit” that included data related to the reports on newborns and medication errors in newborns, that were published in English and Turkish in the last decade and that could be found in the form of full text were included in the study. Results: A total of 31 studies that met the research criteria from these articles constituted the sample of the study. According to the results obtained from the scans; it has been seen that ethical problems are experienced in various topics such as lack of resources, drug management, communication, patient-patient relatives-health professionals and sedation management. Conclusion: As a result of this systematic review; in the palliative care, it is seen that ethical problems are experienced in various dimensions, ethical issues are the most common ethical dilemmas, there are ethical principles and values violations, ethical dilemmas are generally related to the treatment process, there are often ethical conflicts due to lack of resources and the principle of autonomy is frequently violated.

EgeUn###, Introduction: To investigate the validity and reliability of the "Global Pain Scale" for Turkish population. Methods: The cross-sectional study was conducted at the Algology outpatient clinic of a university hospital in Izmir, Turkey, between March and December 2015, and comprised patients with chronic pain aged at least 18 years. Linguistic equivalence, content validity and construct validity were used for establishing the validity of the Global Pain Scale, while the Content Validity Index was used for the assessment of expert views. SPSS 16was used for data analysis. Results: Of the 222 subjects, 142(64%) were females. Overall mean age of the sample was 54.22 +/- 13.79 years. Cronbach's alpha coefficient for the entire scale was 0.95. Total item correlation coefficients of the items in the scale ranged between 0.502 and 0.794, and no items were removed from the scale. Conclusion: The Global Pain Scale was found to have adequate validity and reliability indicators, and can be used with confidence in patients experiencing chronic pain.

In addition, providing pain relief with SCS can reduce immobilization and complications related to immobilization. The present case describes pulmonary embolism (PE) that occurred in patient being treated with SCS for post-laminectomy syndrome. The possibility of PE must be kept in mind while treating patients with SCS.

Nevertheless there are a lot of diff iculties (be drived from medical doctors and health employees, patients and care givers, health care system) to access enough pain treatment for the cancer patients in our country. We should mention especially the opiophobia and lack of palliative care system among them.
