Zusammenfassung Einleitung Die COVID-19-Pandemie wirkt sich auch auf die medizinische Versorgung anderer Erkrankungen aus. Differenziert zwischen patient:innen- und anbieter:innenseitigen Gründen wurde untersucht, inwieweit Personen mit chronischen Erkrankungen vom Aussetzen medizinischer Versorgungsleistungen betroffen sind. Methoden Es wurde eine Querschnittstudie auf Datenbasis der Kohortenstudie Hamburg City Health Study (HCHS) durchgeführt. Die Studienpopulation bestand aus allen HCHS-Teilnehmer:innen (Stichprobe der Bevölkerung Hamburgs, 45–74 Jahre) zwischen April 2020 und November 2021. Über das „COVID-19-Modul“ der HCHS wurde die Inanspruchnahme von Versorgungsleistungen erhoben. Als Grunderkrankungen wurden u. a. Herz-Kreislauf‑, Nieren- und Lungenerkrankungen, Krebs und Diabetes mellitus betrachtet. Die Daten wurden deskriptiv und multivariat in logistischen Regressionen ausgewertet. Ergebnisse Von 2047 Teilnehmer:innen hatten 47,9 % mindestens eine Vorerkrankung. 21,4 % der Personen mit Vorerkrankungen hatten mindestens eine Versorgungsleistung ausgesetzt oder eine Terminabsage erhalten. 15,4 % gaben an, von sich aus auf einen Arztbesuch verzichtet zu haben. Fachärztliche Leistungen (Anteil 43,8 %) entfielen häufiger als hausärztliche (16,6 %). Nach Adjustierung für Alter, Geschlecht und Bildung erwiesen sich Lungen- (OR 1,80; p p Diskussion Gesundheitspolitik und Medien stehen vor der Herausforderung, mit Ängsten in der Bevölkerung vor einer Infektion so umzugehen, dass notwendige Versorgungsleistungen dadurch nicht vermieden werden.

ZusammenfassungAufgrund von diagnostischen und therapeutischen Fortschritten in der Hämatologie und Onkologie und entsprechend steigenden Überlebensaussichten ist ein stetiger Zuwachs der Gruppe von Langzeitüberlebenden mit und nach Krebs (Cancer Survivor) in Deutschland zu verzeichnen. Obwohl das bereits vorhandene deutsche Gesundheitswesen vielfältige Versorgungsangebote vorhält, die auch für Langzeitüberlebende verfügbar sind, ist die Versorgungssituation dieser Gruppe nicht zufriedenstellend. So bedarf es zum einen der Entwicklung von Orientierungshilfen für Langzeitüberlebende, zum anderen sollten neue und innovative Versorgungsprogramme für Überlebende (Survivorship-Programme) entwickelt werden. Der Beitrag gibt einen Überblick über die Problematik, definiert relevante Begrifflichkeiten und formuliert Kernaspekte für die Ausgestaltung von Survivorship-Programmen für Langzeitüberlebende nach Krebserkrankung.

In bisherigen Studien wurde als häufigste Krankheitsfolge die postvirale Fatigue (Erschöpfungssyndrom nach viraler Infektion) identifiziert. Das Ziel der Studie war die Untersuchung von Risikofaktoren für anhaltende Fatiguesymptome infolge einer COVID-19-Infektion und deren Auswirkungen bei Beschäftigten im Gesundheitswesen. Methoden Im Frühjahr 2021 wurden 4315 Versicherte der Berufsgenossenschaft für Gesundheitsdienst und Wohlfahrtspflege (BGW) für eine schriftliche Befragung zu ihrer COVID-19-Erkrankung im Jahr 2020 und den Krankheitsfolgen angeschrieben. Dabei wurden Symptome der Akutinfektion, Krankheitsfolgen, mögliche Risikofaktoren sowie der körperliche und psychische Gesundheitszustand nach der SARS-CoV-2-Infektion erhoben. Als Fatiguescreening wurde die Skala „Allgemeine Erschöpfung“ des Multidimensional Fatigue Inventory (MFI) eingesetzt. Zur Datenanalyse wurden Regressionsanalysen und multivariate Varianzanalysen berechnet. Ergebnisse 10,7 % der Befragten wiesen schwere Fatiguewerte auf. Als Risikofaktoren für eine klinische Fatiguesymptomatik konnten u. a. Vorerkrankungen der Psyche und Atemwege sowie die Schwere der Akutinfektion identifiziert werden. Weiterhin war eine schwere Long‑/Post-COVID-Fatigue mit einer höheren psychischen Belastung, einer niedrigeren gesundheitsbezogenen Lebensqualität sowie mit einer häufigeren Arbeitsunfähigkeit assoziiert. Diskussion Von schwerer Long‑/Post-COVID-Fatigue geht ein hoher Leidensdruck aus, der spezifische Rehabilitationsansätze erfordert und Sozialversicherungsträger sowie Unfallversicherer vor die Herausforderung stellt, geeignete Rehabilitationskonzepte zu entwickeln.

Aims Long-term sequelae may occur after SARS-CoV-2 infection. We comprehensively assessed organ-specific functions in individuals after mild to moderate SARS-CoV-2 infection compared with controls from the general population. Methods and results Four hundred and forty-three mainly non-hospitalized individuals were examined in median 9.6 months after the first positive SARS-CoV-2 test and matched for age, sex, and education with 1328 controls from a population-based German cohort. We assessed pulmonary, cardiac, vascular, renal, and neurological status, as well as patient-related outcomes. Bodyplethysmography documented mildly lower total lung volume (regression coefficient −3.24, adjusted P = 0.014) and higher specific airway resistance (regression coefficient 8.11, adjusted P = 0.001) after SARS-CoV-2 infection. Cardiac assessment revealed slightly lower measures of left (regression coefficient for left ventricular ejection fraction on transthoracic echocardiography −0.93, adjusted P = 0.015) and right ventricular function and higher concentrations of cardiac biomarkers (factor 1.14 for high-sensitivity troponin, 1.41 for N-terminal pro-B-type natriuretic peptide, adjusted P ≤ 0.01) in post-SARS-CoV-2 patients compared with matched controls, but no significant differences in cardiac magnetic resonance imaging findings. Purpose Anxiety is an accompanying symptom in cancer patients that can have a negative impact on patients. The aim of the present analyses is to determine the prevalence of anxiety, taking into account sociodemographic and medical variables, and to determine the odds ratio for the occurrence of anxiety in cancer patients compared to general population. Methods In this secondary analyses, we included 4,020 adult cancer patients during and after treatment from a multi-center epidemiological study from 5 regions in Germany in different treatment settings and a comparison group consisting of 10,000 people from the general population in Germany. Anxiety was measured with the Generalized Anxiety Disorder (GAD-7) questionnaire. In multivariate analyses adjusted for age and sex, we calculated the odds of being anxious. Results The prevalence of anxiety was observed to be 13.8% (GAD-7 ≥ 10). The level of anxiety was significant higher for patients in rehabilitation, compared to patients during inpatient and outpatient treatment (p = .013). Comparison with the general population yielded a 2.7-fold increased risk for anxiety among cancer patients (95% CI 2.4–3.1; p Conclusion The results highlight the importance of identifying anxiety in cancer patients.

Purpose In clinical cancer care, distress screening is recommended to identify highly burdened patients in objective need for psychosocial support to improve psychological distress and quality of life and to enhance patient empowerment. It is however unclear whether distress screeners are suitable for psychosocial care planning and thus whether they can predict the willingness that is need, intention, and utilization, to seek psychosocial support. Methods In a secondary analysis of a cluster intervention study, we assessed cancer patients with three distress screeners (DT, PHQ-9, GAD-7) at baseline. The willingness to seek psychosocial support services was assessed binary for psychosocial services at 3 and 6 months. Logistic regression models were applied to examine the predictive effect of the screeners on need, intention, and utilization. We corrected all models for multiple testing. Results The 660 patients included in the study were on average 60 years, 54% were male. At the 3- and 6-month follow-up, 353 and 259 patients participated, respectively. The screeners were best in predicting the need for support (OR reaching up to 1.15, 1.20, and 1.22 for the PHQ-9, GAD-7, and DT respectively). The intention was predicted by the PHQ-9 and GAD-7, whereas utilization of psychosocial support services was not predicted by the screeners. Conclusion The three distress screeners might be useful in psychosocial care planning, as they are able to predict the need and to some degree the intention to seek psychosocial support. Future research needs to examine potential barriers and supporting factors that may explain utilization of psychosocial support. We evaluated an electronic psycho-oncological adaptive screening (EPAS) which assesses objective indicators of care needs and subjectively perceived care needs and subsequently provides patient feedback with individualized recommendations about psychosocial care services. Methods Patients were assessed within clusters, i.e., different oncological facilities of the competence network of the University Cancer Center Hamburg (UCCH). Patients in the intervention arm underwent the screening, controls received standard care. Patients were assessed at baseline (t0), 3-month (t1), and 6-month (t2) follow-up. Outcomes included information level and use of/access to nine psychosocial services at UCCH, well-being (GAD-7, PHQ-9, SF-8), and treatment satisfaction (SCCC). Conditional linear and logistic regressions were used to identify screening effects at t1 and t2. Results Of 1320 eligible patients across 11 clusters, 660 were included (50%). The average age was 60 years; 46% were female. The intervention was associated with increased information level for all psychosocial services at t1 and t2 (all p p ≤ .02), and better evaluation of access (e.g., more recommendations for services provided by physicians, p p = .02). Conclusions EPAS may improve information about psychosocial services as well as utilization of and access to these services. The effect on information level seems not to be generalizable to other aspects of oncological care. Future studies should incorporate novel technologies and condense the procedure to its core factors. Implications for Cancer Survivors: The screening may help to enhance self-management competencies among cancer survivors. Ras proteins are implicated in some of the most common life-threatening cancers. Despite intense research during the past three decades, progress towards small-molecule inhibitors of mutant Ras proteins still has been limited. Only recently has significant progress been made, in particular with ligands for binding sites located in the switch II and between the switch I and switch II region of K-Ras4B. OBJECTIVE Cancer is associated with an increased risk for completed suicide. We explored subtypes of thoughts of death, death wishes, suicidal ideation and behavior and their association with mental disorders and demographic and disease-related characteristics. METHODS We studied 2,141 cancer patients with the standardized Composite International Diagnostic Interview-Oncology (CIDI-O). Assessment included 4-weeks-prevalences of thoughts of death, wish to die, suicidal ideation, suicide plans, and lifetime suicide attempts. We further assessed 4-weeks-prevalences of mood, anxiety, adjustment, somatoform, substance use, and disorders due to general medical condition. We conducted latent class analyses (LCA). RESULTS The LCA identified three classes with distinct patterns of suicidality. Class 1 (89.0% of the sample) showed no suicidality. Class 2 (6.9%) was characterized by thoughts of death without suicidal ideation. Class 3 (4.1%) was characterized by thoughts of death, suicidal ideation, and suicide plans. Death wishes occurred in both classes 2 and 3. Dies zeigt sich u. a. bei den Anlässen für Arztbesuche, bei der Häufigkeit der Krankschreibungen und in der Zahl der vorzeitigen Berentungen. In allen Bereichen sind die Zahlen in den letzten Jahren deutlich angestiegen. Gleichzeitig haben sich die ambulanten und stationären Angebote bei gesundheitsbezogenen psychischen Problemen in dieser Zeit erheblich erhöht.

Employees from medical and nursing professions are at increased risk for a SARS-CoV‑2 infection and thus more frequently affected by COVID-19 sequelae. Previous studies have identified post-viral fatigue as the most common sequelae. The aim of this study was to investigate risk factors and effects induced by clinically relevant fatigue symptoms following a COVID-19 infection of healthcare workers.In the spring of 2021, 4315 insured members of the Statutory Accident Insurance and Prevention in the Health and Welfare Service were contacted for a written survey on their COVID-19 disease in 2020 and its sequelae. Information on Symptoms of acute infection, disease sequelae, and potential risk factors were collected, as well as the physical and mental health status after SARS-CoV‑2 infection. The general fatigue scale of the Multidimensional Fatigue Inventory (MFI) was used as fatigue screening. Regression analyses and multivariate analyses of variance were calculated for data analysis.Of the respondents, 10.7% showed severe fatigue symptoms. Identified risk factors for clinical fatigue symptoms included preexisting mental and respiratory conditions and severity of acute infection. AIM To compare prevalence rates of mental disorders in patients with cancer and general population controls. METHOD In two stratified nationally representative surveys, the 12-month prevalence of mental disorders was assessed in 2141 patients with cancer and 4883 general population controls by the standardized Composite International Diagnostic Interview (CIDI). We determined odds ratios (ORs) to compare the odds for mental disorders (combined and subtypes) in cancer patients with age- and gender-matched controls. RESULTS The 12-month prevalences rate for any mental disorder was significantly higher in patients with cancer compared to controls (OR 1.28, 95% CI 1.14-1.45). Prevalence rates were at least two times higher for unipolar mood disorders (major depression: OR 2.07, 95% CI 1.71-2.51; dysthymia: OR 2.93, 95% CI 2.13-4.02) and mental disorders due to a general medical condition (OR 3.31, 95% CI 2.32-4.71). There was no significant elevation for anxiety disorders overall (OR 0.95, 95% CI 0.81-1.11). Mildly elevated prevalence rates emerged for post-traumatic stress disorder (OR 1.57, 95% CI 1.11-2.23) and social phobia (OR 1.57, 95% CI 1.07-2.31), while specific phobia (OR 0.82, 95% CI 0.67-1.00) and agoraphobia (OR 0.49, 95% CI 0.33-0.73) were significantly less frequent in cancer. CONCLUSIONS While elevated depression rates reinforce the need for its systematic diagnosis and treatment, lower prevalences were unexpected given previous evidence. Whether realistic illness-related fears and worries contribute to lower occurrence of anxiety disorders with excessive fears in cancer may be of interest to future research.

AlphaScreen is one of the most widely used assay technologies in drug discovery due to its versatility, dynamic range and sensitivity. However, a presence of false positives and frequent hitters contributes to difficulties with an interpretation of measured HTS data. Although filters do exist to identify frequent hitters for AlphaScreen, they are frequently based on privileged scaffolds. The development of such filters is time consuming and requires deep domain knowledge. Recently, machine learning and artificial intelligence methods are emerging as important tools to advance drug discovery and chemoinformatics, including their application to identification of frequent hitters in screening assays. However, the relative performance and complementarity of the Machine Learning and scaffold-based techniques has not yet been comprehensively compared. In this study, we analysed filters based on the privileged scaffolds with filters built using machine learning. Our results demonstrate that machine-learning methods provide more accurate filters for identification of frequent hitters in AlphaScreen assays than scaffold-based methods and can be easily redeveloped once new data are measured. We present highly accurate models to identify frequent hitters in AlphaScreen assays.

The existing proteasome inhibitors, such as bortezomib and ixazomib, are effective in multiple myeloma, but have little activity against solid tumors. These are covalent boronic acid-based compounds and are associated with undesired side effects, mainly hematologic toxicity and peripheral neuropathy. A variety of improved covalent proteasome inhibitors have been developed, but lack of oral availability and low distribution in tumors make them ineffective in solid tumors. In addition, they still have a narrow therapeutic window due to unexpected adverse effects such as cardiac and pulmonary toxicity. We report a non-covalent and rapidly reversible proteasome inhibitor as a potential anti-cancer agent against solid tumors as well as multiple myeloma. These novel inhibitors, called QL compounds, are non-covalent chymotrypsin-like selective proteasome inhibitors that can be taken orally. They effectively inhibited tumor growth in multiple myeloma xenograft model without hematologic toxicity through improved PK properties, especially partition of compound between plasma and RBC. Moreover, this improved PK properties of the QL compounds allowed for sufficient distribution outside the blood compartment and induced tumor growth inhibition in some types of solid cancer xenograft models that were sensitive to proteasome inhibition. These results indicate that non-covalent and rapidly reversible proteasome inhibitors are an ideal strategy for multiple myeloma and potential agents for solid tumors.
