Orthostatic dizziness/vertigo (ODV) is characterized by lightheadedness owing to postural changes. To measure the endolymphatic space (ELS)/total fluid space (TFS) volume ratio and the distribution rate of endolymphatic fluid (ELF) of patients with ODV and compare them with those of control subjects (CS). This study included 22 patients (44 ears) with ODV and 52 controls (104 ears, CS). The ELS/TFS volume ratio (%) and distribution rate (%) of the inner ear components were measured using 3-dimensional magnetic resonance imaging. In the ODV group, the mean ELS/TFS volume ratios of the cochlea, vestibule, and semi-circular canals (SCCs) were 12.1%, 18.6%, and 18.1%, respectively; the mean ELS distribution rates for the cochlea, vestibule, and SCCs were 27.3%, 26.2%, and 46.6%, respectively. The ELS distribution rate of the vestibule was significantly lower (p <.01) and the ELS distribution rate of the SCCs was significantly higher in the ODV than in the CS group (p <.01). The ELS distribution rate in the vestibule + SCCs among patients with ODV did not differ from that in the CS; ELF in the vestibule moved to the SCCs, and a large amount of ELF was distributed only in the SCCs. Objective. The asymmetry ratio of ocular vestibular-evoked myogenic potential (oVEMP) >40% is interpreted as augmented or reduced response depending on whether the nI-pI amplitude of the lesion ear is larger or smaller than that of the opposite ear, respectively. This study compared unilateral Ménière disease patients with augmented oVEMPs vs reduced oVEMPs to elucidate the mechanism of augmented oVEMP.Study Design. Case series with chart review.Setting. University hospital.Methods. Forty patients with unilateral definite Ménière disease were enrolled in this study, including 20 patients with augmented oVEMPs and another 20 patients with reduced oVEMPs in the hydropic side. All patients underwent audiometry, caloric test, and oVEMP and cervical VEMP (cVEMP) tests via bone-conducted vibration stimuli. Then, the oVEMP and cVEMP test results were compared with the stage of Ménière disease, respectively.Results. The augmented group had earlier nI and pI latencies and larger nI-pI amplitude of oVEMPs compared with the reduced group. Caloric test also revealed a significant difference in abnormal responses between the augmented and reduced groups. However, both groups did not differ significantly in the abnormal percentage of cVEMP test results. A significant trend to decline in the prevalence of augmented oVEMPs was noted from stages I to III-IV but not in that of abnormal cVEMPs.Conclusion. The augmented oVEMPs have earlier latencies and larger amplitudes compared with the reduced oVEMPs, indicating that a relatively larger population of intact utricular afferents is activated during the early stage of Ménière disease. Conclusion: Blockage of the endolymphatic duct is a significant finding in Meniere's disease. The position of the utriculo-endolymphatic valve (UEV) and blockage of the ductus reuniens in the temporal bones were not found to be directly indicative of Meniere's disease. Objective: Comparison of blockage of the longitudinal flow of endolymph between ears affected by Meniere's disease and normal ears. Methods: We examined 21 temporal bones from 13 subjects who had Meniere's disease and 21 normal temporal bones from 12 controls. Results: The endolymphatic duct was blocked in five (23%) ears affected by Meniere's disease ( p == 0.016). The utricular duct was blocked in 16 (76%) ears affected by Meniere's disease and 11 (52%) normal ears ( p == 0.112). The saccular duct was blocked in 6 (28%) of ears affected by Meniere's disease and 16 (76%) normal ears ( p == 0.001). The first description of the presence of a utriculo-endolymphatic valve in human fetuses was given by Bast in 1928. Since then this valve-like structure is called Bast’s valve. Its exact function has not yet been established. The general opinion is that it has a protective function by having the possibility to separate the superior endolymphatic compartments of the labyrinth from the inferior compartment. Phylogenetically seen birds are the first vertebrates with a cochlear duct and a distinct inferior and superior part of the labyrinth. The utriculo-endolymphatic valve was discovered by Bast in 1928. The function of Bast’s valve is still unclear. By means of orthogonal-plane fluorescence optical sectioning (OPFOS) microscopy 3D-reconstructions of the valve and its surrounding region are depicted. The shape of the duct at the utricular side is that of a flattened funnel. In the direction of the endolymphatic duct and sac this funnel runs into a very narrow duct. The valve itself has a rigid ‘arch-like’ configuration. The opposing thin, one cell-layer thick, utricular membrane is highly compliant. We propose that opening and closure of the valve occurs through movement of the flexible base/utricular membrane away from and toward the relatively rigid valve lip. [ABSTRACT FROM AUTHOR]
