6. A New Self-Expandable Metallic Stent with Low Axial Force and a High Axial Force Zero-Border Shows a Very Low Perforation Rate in Malignant Colorectal Obstruction: A Japanese Multicenter Prospective Study.

Abstract

Background: Recently, there has been a significant increase in the utilization of self-expandable metallic stents (SEMSs) for treating malignant colorectal obstructions through colorectal stenting. The mechanical properties of SEMSs are usually considered to affect clinical outcomes of patients with malignant colorectal obstructions. Methods: This single-arm, prospective, multicenter study of SEMS with a lower axial force and high axial force zero-border included 200 patients with malignant colorectal obstruction. Technical and clinical success, stent patency, and adverse events associated with SEMS placement were evaluated. Results: One patient was excluded, and 199 patients were evaluated. The treatment intent was bridge-to-surgery in 129 and palliation in 70 patients. Technical and clinical success rates were 99.5% and 97.0%, respectively. The percentage of the ColoRectal Obstruction Scoring System scores of 3 or higher improved significantly from 19.2% before placement to 93.9% after placement. Clinical success was not achieved in five patients due to insufficient stent expansion in four patients and stent occlusion in one patient. Only one patient underwent emergency surgery for perforation of the proximal colon, far from where the stent was placed; the rescue procedure was not performed, despite no improvement in proximal dilatation due to insufficient stent expansion. Among the palliation cohort, 15 patients received chemotherapy, including molecular-targeted agents such as bevacizumab. There were no fatal cases related to stent placement. Conclusions: For management of malignant colorectal obstruction, this newly developed SEMS with low axial force and a high axial force zero-border showed high technical and clinical success rates, and an extremely low perforation rate (0.5%). Abstract

Case Report, Full-thickness rectal prolapse (FTRP) is common among elderly adults and greatly impairs quality of life. We report successful use of laparoscopic suture rectopexy to treat FTRP in 5 patients aged 90-100 years. The median age of patients was 92 years (range, 90-100 years). Three of the 5 patients had recurrent FTRP that was previously managed by a transperineal procedure. All patients had massive protrusion of the rectal wall (range 50-150 mm), and FTRP greatly adversely affected quality of life. No severe comorbidities were noted in any patient preoperatively. All patients had an American Society of Anesthesiologists physical status classification of II. Laparoscopic suture rectopexy for all patients was selected after consultation with anesthesiologists. Although there were no deaths, medical complications were observed in 3 patients: 2 developed mild heart failure (Clavien-Dindo classification, grade I) and one had AU20aspiration pneumonitis (Clavien-Dindo classification, grade III). There were no complications associated with laparoscopic suture rectopexy. All patients were able to return home after surgery. Our experience suggests that laparoscopic suture rectopexy without resection is beneficial for selected patients older than 90 years who are active and in good general health. However, when treating very elderly patients, cautious evaluation of operative risks and careful perioperative management are required in order to avoid surgical complications.

Abstract

Abstract Metastasis to the abdominal wall including port sites after laparoscopic surgery for colorectal cancer is rare. Resection of metastatic lesions may lead to greater survival benefit if the abdominal wall metastasis is the only manifestation of recurrent disease. A 57-year-old man, who underwent laparoscopic surgery for advanced mucinous adenocarcinoma of the cecum 6 years prior, developed a nodule in the surgical wound at the lower right abdomen. Although tumor markers were within normal limits, the metastasis to the abdominal wall and abdominal cavity from the previous cecal cancer was suspected. An abdominal computed tomography scan did not provide detective evidence of metastasis. 18F-fluorodeoxyglucose positron emission/computed tomography (18F-FDG PET/CT) was therefore performed, which demonstrated increased 18F-fluorodeoxyglucose uptake (maximum standardized uptake value: 3.1) in the small abdominal wall nodule alone. Histopathological examination of the resected nodule confirmed the diagnosis of metastatic mucinous adenocarcinoma. Prognosis of intestinal mucinous adenocarcinoma is reported to be poorer than that of non-mucinous adenocarcinoma. In conclusion, this case suggests an important role of 18F-FDG PET/CT in early diagnosis and decision-making regarding therapy for recurrent disease in cases where a firm diagnosis of recurrent colorectal cancer is difficult to make.

Abstract

UNLABELLED The indications for laparoscopic hepatectomy were limited; generally, tumors smaller than 5 cm serve as proper indications. Here, we initially report on a patient with huge hepatocellular carcinoma (HCC) sized 15 cm in the left lateral segment who was treated by laparoscopy-assisted technique. SURGICAL TECHNIQUE 3 trocars were inserted under pneumoperitoneum. The attached ligament was divided and mobilization of the liver could be performed with laparoscopic coagulating system. In accomplishing this maneuver, upper median skin incision of 7 cm was made. The left lateral segment was exposed to be lifted up the tape around the liver. Dissecting sealer (DS30) was used for transection of the liver parenchyma. The relatively large branched vessels and ducts were ligated and transected by direct view from upper median 7-cm incision. Segment 2 and 3 Glisson's sheaths and left hepatic vein were divided using an endolinear stapler. A Hand Port system laparotomy device was installed under pneumoperitoneum, the resected liver maneuvered into a suitable sized plastic bag by endoscopic view. Extraction of the undivided specimen was performed, thus enabling histologic review. Operation time was 170 minutes and operative blood loss was 100 g. The tumor was a 15x12x9 cm in size. Oral intake and ambulation was on the first day; 7 days after the surgery patient was discharged with an uneventful postoperative course. Because of the specific characteristics of HCCs such as their high recurrence rate, the most important goals in HCC treatment are curability and minimal invasiveness. Laparoscopic hepatectomy in this case is beneficial for the patients' quality of life as a minimally invasive operation.

Abstract

The sensitivity and specificity of a new automated electrochemiluminescence immunoassay system, Elecsys® Anti-p53 (Elecsys), were compared with that of the conventional serum anti-p53 antibody (s-p53-Ab) enzyme-linked immunosorbent assay kit [MESACUP anti-p53 test (MESACUP)]. Elecsys and MESACUP were used to analyze the levels of s-p53-Abs in patients with esophageal, colorectal and breast cancer. A total of 532 controls and 288, 235 and 329 patients with esophageal, colorectal and breast cancer, respectively, were enrolled. Additionally, the sera of patients with benign diseases of the esophagus, colorectal system and breast, patients with autoimmune diseases and healthy volunteers were analyzed as controls. Sensitivity and specificity were compared between the two assay systems. Positive agreement rates were 58.7% in all samples, 71.2% in esophageal samples, 73.6% in colorectal samples and 35.1% in breast samples. Negative agreement rates for the different cancer types were ≥97.1% and the overall agreement rates were ≥92.3%. When the specificities of the two assays were aligned for all samples, Elecsys demonstrated higher sensitivities for all types of analyzed cancer together, as well as for esophageal, colorectal and breast cancer, respectively. Although positive concordance between the two assay systems was low in terms of specificity, Elecsys had a higher sensitivity than the MESACUP. [ABSTRACT FROM AUTHOR]
