8. Exploration of the relationship between hypoxia and measures of clinical status and inflammation in children with cystic fibrosis

Abstract

Hypoxia in cystic fibrosis (CF) may occur during sleep, and also during exercise, chest exacerbations and air travel. No standardised definition of nocturnal hypoxia in CF exists. Theoretical evidence suggests hypoxia may have a deleterious impact on clinical status in CF, due to effects on upregulation of pro-inflammatory cytokines, changes in Pseudomonas aeruginosa growth patterns, and causation of pulmonary hypertension. It was hypothesised that hypoxia, and resultant inflammation would adversely affect clinical phenotype in CF. Forty-one children with CF were studied, each undergoing home oximetry before attending for a day of clinical testing (exercise testing, lung function, respiratory and skeletal muscle testing, echocardiography, and quality of life assessment). In vitro work was undertaken to assess the effects of hypoxia on cell growth and interleukin-8 (IL-8) secretion in wild-type and CF airway epithelial cells. The effects of hypoxia were compared to a known proinflammatory stimulus - lipopolysaccharide (LPS) from Pseudomonas aeruginosa. ROC statistics were used to derive the most sensitive and specific definition of sleep hypoxia in the detection of elevated levels of inflammation (WBC, CRP, neutrophil counts and IL-8 levels). This definition (SpO2 <93% for>10% sleep) was used to dichotomise the study population. Hypoxic CF subjects (n=9) had, when compared to normoxic controls (n=32): lower exercise capacity, lower BMI, lower FEV1 and FVC, elevated RV/TLC ratio, and higher Chrispin-Norman scores. Hypoxic subjects also had reduced quality of life, bone density, and increased RV thickness on echocardiogram. Hypoxic cell culture was suggested to be pro-inflammatory, with increased IL-8 production, and synergistically increased IL-8 secretion when cells were co-incubated with LPS under hypoxic conditions. Hypoxia is associated with reduced clinical well-being and increased inflammation in childhood CF. The paradigm exists of whether hypoxia is merely an endpoint of severe CF lung disease; or whether hypoxia may be a causative factor (as suggested by the in vitro work), as well as an effect of CF lung inflammation. A trial of restoration of normoxia in children with CF, with careful re-evaluation of clinically-relevant outcomes is suggested from this preliminary work.

33. Association between clusters of back and joint pain with opioid use in middle-aged community-based women: a prospective cohort study

Abstract

BACKGROUND: To determine the relationship between clusters of back pain and joint pain and prescription opioid dispensing. METHODS: Of 11,221 middle-aged participants from the Australian Longitudinal Study of Women's Health, clusters of back pain and joint pain from 2001 to 2013 were identified using group-based trajectory modelling. Prescription opioid dispensing from 2003 to 2015 was identified by linking the cohort to Pharmaceutical Benefit Scheme dispensing data. Multinomial logistic regression was used to examine the association between back pain and joint pain clusters and dispensing of prescription opioids. The proportion of opioids dispensed in the population attributable to back and join pain was calculated. RESULTS: Over 12 years, 68.5 and 72.0% women reported frequent or persistent back pain and joint pain, respectively. There were three clusters ('none or infrequent', 'frequent' and 'persistent') for both back pain and joint pain. Those in the persistent back pain cluster had a 6.33 (95%CI 4.38-9.16) times increased risk of having > 50 opioid prescriptions and those in persistent joint pain cluster had a 6.19 (95%CI 4.18-9.16) times increased risk of having > 50 opioid prescriptions. Frequent and persistent back and joint pain clusters together explained 41.7% (95%CI 34.9-47.8%) of prescription opioid dispensing. Women in the frequent and persistent back pain and joint pain clusters were less educated and reported more depression and physical inactivity. CONCLUSION: Back pain and joint pain are major contributors to opioid prescription dispensing in community-based middle-aged women. Additional approaches to reduce opioid use, targeted at those with frequent and persistent back pain and joint pain, will be important in order to reduce the use of opioids and their consequent harm in this population.

45. Clinical studies with the antiprogesterone Mifepristone (RU486) in human pregnancy termination

Abstract

Mifepristone in combination with prostaglandin was given to 135 women requesting legal termination, with up to 63 days of amenorrhoea. Various dosage regimes were employed. A 600mg dose of Mifepristone followed forty eight hours later by a prostaglandin vaginal pessary was as effective as more complex regimes given over a longer period. Complete abortion rates of greater than 90% were achieved (approaching the efficacy of the surgical alternative). The outcome of treatment was not gestation dependent. Treatment was well tolerated, acceptable to women, and associated with a low incidence of psychological morbidity. The only serious complication encountered was a 2.5% incidence of heavy bleeding requiring blood transfusion and emergency curettage. Mifepristone is also a useful cervical priming agent prior to late first trimester surgical termination. In a double blind placebo controlled study, forty women received either placebo or 600mg Mifepristone forty eight hours prior to surgery. In the treated group significant cervical dilatation was induced and significantly less force was required to dilate the cervix to allow the passage of a number 8 Karman curette. Blood loss was also significantly reduced. The treatment was well tolerated by patients. No serious complications were noted. In the second trimester, Mifepristone was given at various time intervals prior to prostaglandin induced abortion. The induction to abortion interval and doses of prostglandin required was significantly reduced in Mifepristone treated patients. There was no apparent advantage in an exposure time to Mifepristone of longer than twenty four hours. Serum hormone changes and the development of uterine activity and increased sensitivity to prostaglandins (as measured by intrauterine pressure recordings) after Mifepristone administration are presented. The results are discussed and incorporated into our understanding of the mechanisms of action of Mifepristone in human pregnancy termination.
