Background: This study aims to analyze the prevalence of diseases of the genitourinary system in the Republic of Kazakhstan from 2015 to 2022. Methods: We used the information from the primary registration database of the incidence of diseases of the genitourinary system in the regions of Almaty of the Republic of Kazakhstan. The data on the number of patients with diseases of the genitourinary system were obtained from the compilation “Health of the Population of the Republic of Kazakhstan and the Activities of Healthcare Organizations for the years 2015–2022''. The provision of the population of Kazakhstan with urological profile beds and urologists was analyzed. Results: The highest morbidity rate is observed in Northern Kazakhstan - 4984.8 cases per 100,000 people. The lowest morbidity rate is observed in Atyrau - 1619.8 cases per 100,000 population, which may indicate better health conditions in this region compared to others. Over time, the number of cases increased, reaching a peak in 2018 with 4471.9 cases. The forecast for 2029 shows that the number of cases will be 1888.3. Morbidity started at 3852.3 cases per 100,000 population in 2015. In the following years, morbidity decreases and reaches 2889.6 cases in 2022. In 2016, an increase of 382.7 cases was noted, while in 2019, there was a decrease of 353 cases. Conclusion: These data indicate the need to develop effective preventive measures and screening strategies for the high-risk population.

Abstract Urogenital emergencies demand immediate attention within the field of emergency medicine, encompassing a range of critical conditions from ectopic pregnancies to kidney stones. Timely diagnosis and treatment are vital to prevent potential mortality and morbidity. However, due to the sensitive nature of these disorders and the cultural taboos surrounding them, accessing prompt medical care can be challenging. To bridge this gap, innovative gamification-based learning techniques, such as the Learning Urogenital Diseases in Oddity (LUDO), have been introduced for emergency medicine residents (Chou, What is gamification? Yukai Chou: Gamification and behavioral design, n.d.; Gamification '13: Proceedings of the first international conference on gameful design, research, and applications, 2013). LUDO is a timed, gamified exercise that offers residents an interactive and engaging platform to enhance their clinical knowledge related to urogenital disorders. Adapted from the well-known board game, LUDO fosters learning, collaboration, and cooperation among residents. This format is highly customizable and can be utilized by various learning groups. Participants, emergency medicine residents from different academic years, formed four teams, each distinguished by a unique color. The exercise utilized simple and accessible materials, including a LUDO board, LED stopwatch, laptop, colored hats, and a desk bell. Teams advanced their tokens through the board by correctly answering urogenital disorder-related questions within a specified time frame. LUDO’s objectives extended beyond token movement, assessing essential skills such as teamwork, time management, resource utilization, and strategic decision-making. The option to seek external resources, limited to five times per team, added an element of strategy. Facilitators evaluated participants’ performance through questionnaires and Likert scales. The results revealed that LUDO effectively promoted teamwork, communication, leadership, and problem-solving among emergency medicine residents. Resident feedback was overwhelmingly positive, with interest in adopting this format for other educational modules. The gamified approach encouraged engagement and motivation, with immediate feedback ensuring continuous learning. In conclusion, the incorporation of the LUDO gamified format provides an enjoyable and interactive learning experience for emergency medicine residents. It enhances engagement, fosters teamwork, and facilitates the rapid assimilation of crucial knowledge related to urogenital diseases. This adaptable approach holds promise for improving resident training in various clinical scenarios.

This study was aimed to evaluated effects of smokeless tobacco on the uterine wall of female rats of Swiss albino species. This was a controlled experimental study with three groups of adult female Swiss albino rats. The study was conducted over a period of 31 days, at Animal House, Husbandry, and Veterinary Sciences at Sindh Agricultural University, Tandojam, and the Department of Anatomy, Isra University, Faculty of Medicine & Allied Medical Sciences, Hyderabad, Sindh, Pakistan. Thirty adult female Swiss albino rats were randomly assigned to three groups: Group A (control, no treatment), Group B (treated with 5% smokeless tobacco), and Group C (treated with 10% smokeless tobacco). Blood samples were collected on the 31st day to analyze Follicle-stimulating hormone (FSH) and Luteinizing hormone (LH) levels in the serum. After euthanization, uteruses were removed and weighed, followed by histological examination using Hematoxylin & Eosin and Mason's trichrome stains. The experimental groups (B and C) exhibited a significant reduction in uterine weight and decreased serum levels of FSH & LH. Histological examination showed cystically dilated submucosal glands and pronounced atrophy in the uteri of the experimental groups. Additionally, the myometrial wall thickness was also decreased. The use of smokeless tobacco negatively affects the uterine wall of female Swiss albino rats, resulting in histological changes and decreased serum levels of FSH and LH.

Background Complications of urogenital schistosomiasis include acute inflammatory and chronic fibrotic changes within the urogenital tract. Disease burden of this neglected tropical disease is often underestimated, as only active, urine egg-patent Schistosoma infection is formally considered. Previous studies have focussed on short-term effects of praziquantel treatment on urinary tract pathology, demonstrating that acute inflammation is reversible. However, the reversibility of chronic changes is less well studied. Methods Our study compared, at two time points 14 y apart, urine egg-patent infection and urinary tract pathology in a cohort of women living in a highly endemic area having intermittent praziquantel treatment(s). In 2014 we matched 93 women to their findings in a previous study in 2000. Results Between 2000 and 2014 the rate of egg-patent infection decreased from 34% (95% confidence interval [CI] 25 to 44) to 9% (95% CI 3 to 14). However, urinary tract pathology increased from 15% (95% CI 8 to 22) to 19% (95% CI 11 to 27), with the greatest increase seen in bladder thickening and shape abnormality. Conclusions Despite praziquantel treatment, fibrosis from chronic schistosomiasis outlasts the presence of active infection, continuing to cause lasting morbidity. We suggest that future efforts to eliminate persistent morbidity attributable to schistosomiasis should include intensified disease management. [ABSTRACT FROM AUTHOR]

Since May 2022, there has been a worldwide outbreak of Mpox infection, a zoonotic viral disease caused by the monkeypox virus, outside of central and west Africa where it is endemic. In this report, we describe a case of a 23-year-old African American male patient with HIV infection and normal CD4 counts who presented with painful, evolving skin lesions at the base of his penis which were found to be a manifestation of Mpox. Early recognition of the symptomatology caused by Mpox is important for urologists as cases of this virus continue to rise in the United States.

Reproductive dysfunction and urogenital malignancies represent a serious health concern in men. This is in part as a result of the absence of reliable non-invasive tests of diagnosis/prognosis. Optimizing diagnosis and predicting the patient's prognosis will affect the choice of the most appropriate treatment and therefore increase the chances of success and the result of therapy, that is, it will lead to a more personalized treatment of the patient. This review aims firstly to critically summarize the current knowledge of the reproductive roles played by extracellular vesicle small RNA components, which are typically altered in diseases affecting the male reproductive tract. The incidence of lower urinary tract symptoms increases with age. People with bladder disorders who are older than 60 years are more likely to comply and persist with treatment regimens, such as antimuscarinic medications for overactive bladder. Suggestions for these differences include lifestyle factors, greater compliance with medication prescription, or pharmacological and physiological changes to the receptors within the bladder tissue itself. This scoping review focused on the latter and sought to identify if there is recent evidence to support a prominent role for the internal lining of the urinary bladder, the bladder mucosa (urothelium with lamina propria), in age-related alterations. Although there continues to be substantial urological research in humans, animal models remain highly important for the assessment of physiological and pharmacological changes in the bladder. Recent work in this field brings insights into the overall understanding of bladder function and dysfunction. The PUBMED database was searched for studies published between 2018 and 2022. From the 25 articles identified, 10 were eligible for inclusion in the evaluation, and a risk of bias assessment was performed. Studies reported a variety of age-related alterations in the bladder mucosa. The most pronounced changes appear to be an inhibition of the mucosal barrier function and signalling pathways with ageing. Ageing also appeared to inhibit receptor systems associated with contraction inhibition, potentially leading to enhanced contractions. The overall evidence suggests that the increased prevalence of bladder contractile disorders seen in ageing may be due, in some part, to physiological alterations occurring within the bladder tissue itself. • Combined Simulation/MUTA sessions increased NP student's confidence and learning. • SP/MUTA's ranked NP students as well-prepared in history and physical exam skills. • Combined Simulation/MUTA sessions uniquely prepare NP students for clinical practice. Nurse practitioner (NP) programs use skills-based activities such as structured simulations and instructional sessions to prepare students for clinical practice. NP students often have limited opportunities to perform sensitive exams during their clinical experiences. This study combined simulation and Male Urogenital Teaching Associate (MUTA) instruction to evaluate student's preparedness, knowledge, perceived learning, and confidence in diagnosing/managing urogenital complaints. Twenty-six NP students entering their first clinical course completed a simulation/MUTA clinical encounter. The encounter included a pre-brief, simulation, urogenital examination instructional session, learning activities, and debrief. Descriptive and inferential statistics were analyzed on pre-test/post-test and post-evaluation questionnaires. Data revealed a statistically significant difference in student's confidence postintervention. Students reported an increase in knowledge and preparedness. There is a paucity of evidence on the use of simulation with MUTA instruction in NP curriculum. Background: Low-intensity shockwave therapy (LiSWT) is an emerging non-invasive and restorative therapy for erectile dysfunction (ED) with demonstrated efficacy and few adverse events. Although LiSWT has been shown to improve erectile function amongst men with ED, few studies have examined its long-term durability. We present the long-term results of a randomized controlled trial (RCT) assessing erectile function after LiSWT., Methods: A total of 30 patients with baseline ED seen at the University of Virginia were randomized to LiSWT or sham treatment. Patients in the sham group crossed over at 1 month and were unblinded. After initial trial completion, patients enrolled in the long-term outcome study were considered one combined cohort. Patients were treated twice weekly for 3 consecutive weeks with a Storz ® Duolith™ device delivering 3,000 shockwaves at 0.1 mJ/mm 2 to the distal penis, the base of the penis, and the crura. Primary outcomes were changes in Sexual Health Inventory for Men (SHIM) and Erection Hardness Score (EHS) from baseline (3 months pre-treatment) up to 36 months post-treatment. Changes in SHIM and EHS scores were evaluated using linear mixed effects models. Patient satisfaction was assessed with the Erectile Dysfunction Inventory of Treatment Satisfaction (EDITS) index., Results: The mean baseline SHIM score was 10.8±0.94. At 12-, 24-, and 36-month assessment following treatment, the mean SHIM scores were 15.6±1.27 (P<0.001), 15.0±1.14 (P<0.001), and 12.2±1.43 (P=0.31). The mean baseline EHS score was 1.87±0.17. At 12-, 24-, and 36-month assessment following treatment, the mean EHSs were 2.70±0.24 (P<0.001), 2.66±0.21 (P<0.001), and 2.29±0.26 (P=0.10). The median [interquartile range (IQR)] EDITS score was 48.9 (22.7, 74.4), indicating moderate satisfaction with LiSWT. There were no adverse events recorded., Conclusions: Our analysis demonstrates sustained long-term improvement in erectile function after LiSWT for a heterogeneous cohort. While limited by population size, the results suggest durable improvement in erectile function for the first 2 years with a peak treatment effect at 1 year. Background: In recent years, the incidence of male infertility has increased worldwide. It is necessary to study the factors that influence male infertility in each area/region for better management. Objective: To determine the factors affecting male infertility in the Iranian male population. Materials and Methods: An online search was conducted in electronic databases including PubMed, Google Scholar, SID, and Scopus to identify articles on the factors associated with male infertility, published in English and Persian. The keywords used to perform the search included "factor", "epidemiology", "causes of infertility", and "male infertility". The search was conducted without a time restriction, up to April 2020. Results: The search resulted in a total number of 691 studies. After an assessment of the articles, finally 14 studies were included in this study with a total number of 26,324 infertile males. The factors associated with male infertility included semen abnormalities, varicocele and testis disorder, smoking, exposure to heat, obesity, anabolic steroids, vascular abnormalities, anti-spermatogenesis factors, antidepressants, taking ranitidine and cimetidine, penile discharge and genital ulcers, painful micturition, occupational factors, alcohol, chronic disease, sexual disorder, Surgical and urological diseases, genetic factors and herpes infection. Among these, the semen and varicocele disorders were common in most studies. Conclusion: The present review suggests that the factors affecting male infertility in Iran are similar to those reported from other countries. Pyroptosis, a form of programmed cell death distinct from apoptosis and necrosis, is thought to be closely associated with the pathogenesis of diseases. Recently, the association between pyroptosis and urinary diseases has attracted considerable attention, and a comprehensive review focusing on this issue is not available. In this study, we reviewed the role of pyroptosis in the development and progression of benign urinary diseases and urinary malignancies. Based on this, pyroptosis has been implicated in the development of urinary diseases. Objectives: This study aimed to assess multicausal mortality due to diabetes from 2016-2018 in Spain. Specific objectives were to quantify the occurrence of diabetes as an underlying cause or as any registered cause on the death certificate., Materials and Methods: A cross-sectional descriptive study taking a multicausal approach., Results: Diabetes appears as an underlying cause of 2.3% of total deaths in Spain, and as any cause in 6.2%. In patients in whom Diabetes appears as an underlying cause on the death certificates, the 15 most frequent immediate causes are cardiovascular diseases in men(prevalence ratio 1,59)and women (PR1,31). In men, the causes associated with diabetes as any cause were skin diseases(prevalence ratio 1.33), followed by endocrine diseases(prevalence ratio 1.26)and genitourinary diseases (prevalence ratio1.14). In women, the causes associated with the presence of diabetes as any cause were endocrine (prevalence ratio 1.13)and genitourinary (prevalence ratio 1.04)diseases., Conclusions: In patients in whom diabetes appears as an underlying cause on the death certificates, the 15 most frequent immediate causes are cardiovascular diseases. In men, the causes associated with the presence of diabetes as any cause of death are skin, endocrine and genitourinary diseases.
