From September 2016 to September 2019, this study was carried out at the Urology and Nephrology University Hospital at Assiut University in Assiut, Egypt. On sixty individuals were divided into two therapy groups, each comprising thirty patients. The first group underwent a traditional percutaneous nephrolithotomy (PCNL), while the second group underwent a tubeless procedure. The average blood loss before and after the procedure, changes in hemoglobin, and creatinine levels did not differ statistically significantly between the two groups, according to the results. These findings demonstrates that the tubeless group had a longer surgical length, a difference that was statistically significant (P-value = 0.034*). However, there were no significant differences between the two groups with respect to the number of tracts or methods of access. Following the procedure, the patient was randomly assigned to have either a nephrostomy tube or have the tract closed without the need for a tube. Phytotherapy, which involves the use of plant extracts and natural compounds for medicinal purposes, is indeed a promising alternative for managing urinary lithiasis. Many plants have been studied for their potential to prevent and treat kidney stones, and they may offer a more natural and potentially less harmful approach compared to conventional treatments. Additionally, phytotherapy may be more cost-effective. The aim of the present study was to investigate the antilithic potential of extracts and essential oils of Saussurea costus (Falc) Lipsch in two in vivo models, one on ethylene glycol-induced calcium oxalate crystal formation and the other to assess the effects of these extracts on magnesium oxide-induced struvite crystal formation. The experiment involved the administration of different doses of aqueous and ethanolic extracts of S. costus (200 and 400 mg/kg) and essential oils (25 and 50 mg/kg) to male Wistar rats, followed by the evaluation of various physiological, biochemical and histopathological parameters. The results demonstrated that the administration of S. costus essential oils and extracts had significant effects on the rats, influencing body weight, urine volume, crystal deposition, cytobacteriological examination of urine, and serum biochemical parameters. Histopathological examinations revealed varying impacts on the kidneys and livers of the treated rats. The findings suggest that S. costus extracts and essential oils may hold promise in inhibiting calcium oxalate crystal formation in vivo and influencing various physiological and biochemical parameters in rats. Overall, the 200 mg/kg ethanolic extract of S. costus demonstrated antilithiatic efficacy, did not exhibit signs of toxicity and reduced the number of crystals in the kidneys. Furthermore, the study did not find a significant effect on reducing struvite crystals.

Objectives: To analyze the hypothesis that ureteral obstruction may activate kidney latent tuberculous though qualitative study of Urogenital Tuberculosis patients. Methods: A qualitative study was conducted using semistructured interviews in eight patients with Urogenital Tuberculosis. The progression of the disease from the initial symptoms was characterized through the analysis of the clinical and radiological data. The presence of ureteral obstruction prior to the onset of renal tuberculosis was observed in three patients. Results: Patient 1: A 58-year-old female had five episodes of acute left ureteral lithiasis in two years prior to left kidney tuberculosis. Patient 2: A 55-year-old male patient had a 1.2 cm proximal left ureteral stone and in the following six months, the diagnosis of tuberculosis was made in a nonfunctioning left kidney with ureteral thickening and stenosis. Patient 3: A 47-year-old male patient had a 1.2 cm stone in the proximal right ureter and developed urinary tuberculosis with a nonfunctioning right kidney and a contracted bladder. Conclusion: Kidney tuberculosis may appear in the same kidney that had previously suffered stone ureteral obstruction, which may have created local conditions for the activation of latent foci of renal tuberculosis.

Introduction: urinary stone is a multifactorial disease characterized by the presence of concretions in the urinary tract. The diagnosis of urinary lithiasis is based on clinical and imaging findings. The objective of this study was to determine the profile of patients suffering from urolithiasis in the city of Yaounde. Methods: this was a descriptive observational cross-sectional study with retrospective data collection. Patients with an age > 15 years and patients diagnosed with lithiasis confirmed by medical imaging were included in our study. The variables studied were sociodemographic, clinical and paraclinical. Results: a total of 120 patients were included in our study. The mean age was 40.46±12.62 years with extremes from 19 to 74 years. Male gender was predominant at 60.8% (n=73). Renal colic was the circumstance found in 67.5% (n=81). Physical examination was normal in 55.8% (n=67). Insufficient hydration was found in 45% (n=61) of cases. Abdominal scanner was the most performed examination in our study in 50.7% (n=61). The median stone density was 731 Hounsfield units [346; 1183.5]. The stones were predominantly located in the upper tract. The left renal topography was predominant in 35% (n=42) of cases. Urine culture revealed Escherichia Coli at 60% (n=15). Blood calcium, phosphorus and uricemia were requested in 15.8% (n=19), 0.8% (n=1) and 12.5% (n=15) of cases, respectively. The results were normal. In the absence of a laboratory specialized in biochemical analysis of calculi, a minority of patients (n=3) benefited from spectrophotometry. Conclusion: urinary lithiasis is a disease of men in their forties. Renal colic is the main revealing sign. The diagnosis is revealed by the abdominal scanner in half of the cases. Objective: Artificial neural networks (ANNs) are widely applied in medicine, since they substantially increase the sensitivity and specificity of the diagnosis, classification, and the prognosis of a medical condition. In this study, we constructed an ANN to evaluate several parameters of extracorporeal shockwave lithotripsy (ESWL), such as the outcome and safety of the procedure. Methods: Patients with urinary lithiasis suitable for ESWL treatment were enrolled. An ANN was designed using MATLAB. Medical data were collected from all patients and 12 nodes were used as inputs. Conventional statistical analysis was also performed. Results: Finally, 716 patients were included in our study. Univariate analysis revealed that diabetes and hydronephrosis were positively correlated with ESWL complications. Regarding efficacy, univariate analysis revealed that stone location, stone size, the number and density of shockwaves delivered, and the presence of a stent in the ureter were independent factors of the ESWL outcome. This was further confirmed when adjusted for sex and age in a multivariate analysis. The performance of the ANN at the end of the training state reached 98.72%. The four basic ratios (sensitivity, specificity, positive predictive value, and negative predictive value) were calculated for both training and evaluation data sets. The performance of the ANN at the end of the evaluation state was 81.43%. Conclusion: Our ANN achieved high score in predicting the outcome and the side effects of the ESWL treatment for urinary stones.

Objective: To present a new technique of double‐j stent (DJ) placement during laparoscopic transperitoneal ureterolithotomy (LUL). Patients and methods: Following the extraction of the stone, a 6 French DJ open‐end stent is prepared: two straight‐tip hydrophilic guidewires are inserted into the appropriate lateral holes of the stent, as identified by the preoperative evaluation of the CT scan. Approximately 5 centimeters of each wire protrude from the proximal and distal ends of the stent to straighten its terminal curl, thus resembling the wings of a flying seagull. The remaining proximal portions of both guide wires are left within each guidewire dispenser. The two ends of the stent are grasped together in a U‐fashion and inserted into the abdomen through a 10mm port. Once in the abdomen, the longer segment of the stent is inserted and pushed into the ureterotomy until it reaches the target site. The guide wire is then removed. The same procedure is repeated for the other end of the stent. A brief literature review on the currents techniques of laparoscopic DJ placement is also presented. Results: Analyzing the outcomes of 21 LUL, the "seagull" technique is time‐saving and safe. No perioperative complications were encountered. There is no risk of enlarging or tearing the ureterotomy and no need for patient replacement, extra cystoscopic or ureteroscopic procedures as well as of using modified guidewires and closed‐tip stents. The aim of this study is to evaluate the antibacterial activity of four organic extracts: petroleum ether (PE), dichloromethane (DCM), methanol (MeOH), ethanol (EtOH) and aqueous extracts (digestion, decoction, maceration, and infusion), prepared from the leaves of the aromatic plant Myrtus communis. Phytochemical screening revealed a richness of this plant in secondary metabolites such as flavonoids, phenolic acids, anthraquinones, tannins, sterols, and terpenes. The antimicrobial activity of Myrtus communis extracts was evaluated in vitro by the agar diffusion method on two strains of bacteria commonly encountered in urinary tract infections and lithiasis: Staphylococcus aureus ATCC (25923), Proteus mirabilis reference strain ATCC (43862) and Proteus mirabilis clinical strain. All these strains were found to be sensitive to the extracts studied. The minimum inhibitory concentrations (MIC) recorded varied from 3.75 to 15 mg/ml, while the minimum bactericidal concentrations (MBC) were between 7.5 and 30 mg/ml. This activity is essentially bactericidal in nature. The analyses showed the presence of phenolic groups with antibacterial capabilities. The obtained results proved an interesting antibacterial activity of the majority of Myrtus communis extracts, translated by zones of inhibition of important diameter: extracts of methanol (28 mm), petroleum ether (28 mm) and aqueous solution (25 mm) towards Proteus mirabilis reference strain ATCC (43862), Staphylococcus aureus ATCC (25923) and Proteus mirabilis clinical strain, respectively. Pyonephrosis is a suppurative infection of the kidney caused by ureteral obstruction. It can lead to kidney failure, septic shock, and death. Thus, it requires prompt assessment and appropriate management. We report a case of a 63-year-old male with giant pyonephrosis contained 10 liters of pus and spontaneously ruptured in the adjacent muscles. This clinical case illustrates the value of computed tomography scan in the diagnosis and management of an uncommon upper urinary tract infection and its complications.

ABSTRACT Urolithiasis has a high incidence among confined sheep. It is multifactorial and may cause economic damage. Our aim was to determine the capacity of urinary acidification using ammonium chloride in sheep. Twenty-five 3-month-old male sheep were confined and randomly divided into three groups; the G200 and G500 groups received 200mg/kg/GW and 500mg/kg/GW of ammonium chloride daily for 56 consecutive days, respectively, whereas the CG group did not receive ammonium chloride. Sampling times and clinical evaluation were performed weekly, starting from the 14th day of confinement (M1 or immediately before administering ammonium chloride) until the 17th day (M9) of the feedlot. Hemogasometry, biochemical examination of serum urea and creatinine concentration and ultrasound evaluation of the urinary tract were performed. The urinalysis indicated a higher incidence of ammonium magnesium phosphate crystals at the beginning of the study, showing a migration to urate crystal formation, mainly in the G500 group because of urinary acidification. No hemogasometric, serum biochemistry, ruminal fluid, or ultrasonographic changes were observed. Urinary acidification was achieved and maintained after M7 during the administration of ammonium chloride in the G500 group, but not in the other study groups.

Aim: To assess the effectiveness of non-invasive instrumental diagnostics in the detection of urolithiasis, particularly ultrasonography and computed tomography. Materials and methods. Ultrasonic diagnosis and computed tomography were performed in 78 and 104 patients, respectively. All patients were diagnosed with urolithiasis of different localization. The study was conducted at the Regional Clinical Hospital of Shymkent city for the period 2017-2018. Also, a study of medical documentation and questionary observations were carried out with subsequent statistical processing. Results. Various calculi were detected in 27 patients using ultrasonography in addition to the other symptoms of urolithiasis: change in the size and configuration of the kidney, the expansion of the pyelocaliceal complex and compaction of the parenchyma of the affected organ. 48.3% of the total number of stones were found on the right kidney, the remaining stones (51.7%) were found in the left kidney. In the upper, middle and lower groups of pyramids there were detected 10.7%, 32.2% and 28.6% of calculi, respectively. In the pelvis of the right and left kidneys, 17.8% and 10% calculi were found, respectively, and in the calyx-pelvis complex of the right (10.7%) and left (20%) kidneys. Conclusion. The analysis of the effectiveness of the ultrasound and computed tomography was demonstrated that the sensitivity, specificity and accuracy of computed tomography were higher than these parameters of ultrasound, which was also confirmed by the results of computation of the receiver operating characteristic curve and the area under this curve.

Introduction: Chronic non-communicable diseases (CNCDs) are a group of diseases characterized by having the highest mortality and morbidity rates in the world. Despite the increase in new drug technologies, the use of medicinal plants as an aid in therapy for these diseases is a widespread reality. However, most plant species contain inorganic crystals of calcium oxalate, a product of plant metabolism, which has several functions in plant tissues. For the human species, its ingestion is associated with the arising of kidney problems such as urinary lithiasis, especially in people who have a predisposition to these conditions. Aim: To determine the amount of oxalic acid in plant species, which are used in the treatment of CNCDs. Methodology: After collection and characterization the plant species, aqueous and acidic extracts were obtained from 0.5 g of the plant drug of each species and, subsequently, were titrated with a standardized 0.02 mol.L-1 potassium permanganate solution and the concentrations of oxalic acid were expressed in g/100 g of dry vegetable drug. Result: The data obtained from the concentration of oxalic acid ranged from 4.58 ± 0.09 g/100 g to 17.21 ± 0.07 g/100 g and demonstrated that the concentrations from acid extraction are higher compared to the aqueous extraction, for all vegetables species analyzed. Methodological optimization was performed for the species that showed the highest results, Psidium guajava and Artocarpus heterophyllus. Nowadays people worldwide uses several medicinal plants especially for curing urinary lithiasis. Ammivisnaga is extensively distributed medicinal plant used for management of numerous diseases including urinary lithiasis. Cymbopogonproximus or called Halfa-bar suggested for use in medicine for the ejection of ureteric and renal calculi. Ammivisnagaand Halfa-barfound in Asia and several parts of the world. This study aimed at investigating and comparing both Ammivisnaga and Halfa-barextracts for their antiurolithicactivity using a mature female rabbit. Forty-two rabbits randomly divided into four groups. Group A negative control, group Bpositive control (chemically induced urolithiasis), group Clike group B but treated with Ammivisnaga extract 5 ml orally twice daily for 2 weeks and group Dlike group B but treated with Proximol (Halfa Bar extract) 5 ml twice daily for 2 weeks also.GC-MS analysis of alcoholic extract of AmmiVisnaga revealed the detection of main alkaloids Khellin, Visnagin, adulicin and enidimine, withothercompounds inlower quantity. The results show that group of induced urinary lithiasis for 14 days and 21 days showed clear body weight loss, higher consumption of drinking water and increased morning urine excretion. In addition to significant increase of blood urea, uric acid, serum creatinine and calcium levels. After 14 days of treatment with AmmiVisnaga extract and proximol we noticed reversed body weight loss and overcome low blood urea, serum uric acid, creatinine and calcium. Furthermore, urinary lithiasisgroups showed clear crystallization compared to the control urine samples, but after 14 days treatment with AmmiVisnaga and proximol extracts, there was nearly clearance of the urine and disappearance of the crystals from the urine. In conclusion, AmmiVisnaga and Proximol are safe and good diuretic herbs causes improvement in the body weight, high clearance in urine crystals and amelioration in kidney function compared to untreated group. ABSTRACT Introduction: to evaluate urinary biochemical alterations related to urolithogenesis processes after sleeve gastrectomy (SG). Materials and methods : prospective study with 32 individuals without previous diagnosis of urolithiasis who underwent SG. A 24-h urine test was collected seven days prior to surgery and at 6-month follow-up. The studied variables were urine volume, urinary pH, oxalate, calcium, citrate, and magnesium and calcium oxalate super saturation (CaOx SS). INTRODUCTION Pediatric urinary lithiasis (urolithiasis) is an important health issue linked to urinary metabolic disorders. In the United States alone, annual costs associated with urolithiasis are $229 million for hospital admissions and $146 million for emergency care. OBJECTIVE Identify urinary metabolic disorders in Cuban pediatric patients with urolithiasis and better understand the relationship of age, demographic and anthropometric variables to urinary metabolic disorders strongly associated with urolithiasis. METHODS We carried out a descriptive, cross-sectional study. The study universe was comprised of Cuban patients aged 2 to 19 years with urinary lithiasis who underwent renal metabolic studies at the Dr Abelardo Buch López Nephrology Institute in Havana, Cuba, from 2008 through 2019. All data were obtained from reports of the aforementioned metabolic studies. We collected the following variables: age, sex, nutritional status, urinary volume, plasma and urinary creatine concentrations; and calcium, uric acid, oxalate and citrate urinary excretions collected during a 24-hour period. We included results of urinary cystine tests and urine mini-cultures. We obtained frequency distributions for categorical and qualitative variables and calculated means and standard deviations for quantitative variables. We also evaluated homogeneity of metabolic disorders between children and adolescents. RESULTS We studied 1592 pediatric patients, of whom 67.7% (1078/1592) were adolescents. The main metabolic disorders included hypercalciuria (39.1%; 622/1592), decreased urinary flow (22.4%; 357/1592) and hypocitraturia (18.2%; 289/1592). Hypercalciuria, hypocitraturia and hyperoxaluria were more common in children, while decreased urinary flow and hyperuricosuria were more common in adolescents. Hyperuricosuria was more frequent in male patients (6.3%; 40/639 vs. 1.8%; 8/439) and had the greatest impact on lithogenesis. Hypercalciuria was more frequent in undernourished children (62.5%; 30/48) than in overweight children (21.7%; 10/46), or those with obesity (33.3%; 15/45). CONCLUSIONS The main metabolic disorders among Cuban pediatric patients with urinary lithiasis are: hypercalciuria, decreased urinary flow and hypocitraturia. Hypercalciuria, hypocitraturia and hyperoxaluria are more common in children, and decreased urinary flow and hyperuricosuria are more common in adolescents. The purpose is to present our preliminary results where ultra-mini PCNL (UMPCNL) with the new 120-watt laser with the anti-retropulsion (Lumenis® MOSES Pulse™120H Holmium: YAG laser) was used for the first time to our knowledge. Twelve patients underwent ultra-mini PCNL in prone position under general anesthesia using a 12-F nephroscope with a 14-F Access sheath in our tertiary center. The fragmentation was performed with a 500 μm laser fiber using the 120-watt Lumenis® MOSES Pulse™120H Holmium: YAG laser). Efficacy was considered in terms of stone-free rates (SFR), complication rate, duration of the operation, and hospital stay. Our SFR was 91.6% with 11 patients out of 12 being completely free of any residual stone. The duration of the operation was 86.4 ± 36.8 (40-165) min, whereas the mean laser time (real stone fragmentation laser time) was 755.7 ± 954.7(241-3425) sec. The total laser energy used was 39.7 ± 52 KJoules (11.3-182). The fluoroscopy time and radiation doses were 358.5 ± 180.4 (154-750) sec and 64.7 ± 41.2 (14.7-159.0) mGy, respectively. The mean reduction in levels of hemoglobin postoperatively was 0.6 ± 0.3 (0.1-0.9) g/dL, and no complications were observed. The combination of UMPCNL with the new 120-watt laser and the unique anti-retropulsion technology (Lumenis® MOSES Pulse™120H Holmium: YAG laser) delivered very promising results and it could be the future of PCNL. Purpose: To assess the influence of holmium laser cystolitholapaxy (HLC) concomitantly with holmium laser prostate enucleation (HoLEP) on patients with benign prostatic hyperplasia (BPH) presenting bladder calculi. Materials and Methods: We present a retrospective analysis of patients with BPH (with or without concomitant HLC) at three Spanish centers. Intraoperative variables (e.g., time and resected tissue), changes in functional parameters of the prostate, and frequency of complications (intraoperative, early postoperative, and at 12 months) in patients with and without HLC were compared. Results: The analysis included 963 patients aged 48 to 91 years, of which 54 (5.6%) underwent HLC to treat vesical lithiasis. Mean (range) prostate size (measured by transrectal ultrasound) was 79 (43–173) g and 91 (35–247) g for patients with and without concomitant HLC, respectively (p = 0.080). All bladder calculi were effectively removed. No significant differences were found regarding enucleation and morcellation times, but total operation time was significantly higher in patients with HLC: mean (standard deviation [SD]) of 78 (27) minutes vs 95 (41) minutes (p < 0.001). Three patients underwent conversion to open surgery because of bladder perforation, all of them from the group without HLC. Rates of intraoperative, early, and 12-month complications were similar in both groups. No significant differences in International Prostate Symptom Scale, maximum flow rate (Qmax), and mean flow were observed between groups 12 months after surgery. Conclusions: Simultaneous HoLEP and HLC increases the total operation time, but does not influence the risk of clinically relevant perioperative and postoperative complications. • Foreign bodies, such as suture materials, mesh, cotton swab, Hem-o-lok clips, metallic clips, coils used for angioembolization etc. are rare causes of urinary tract stone disease. • Clinicians should be aware of the possibility of migrated hem-o-lok clips serving as a nidus for urinary tract stone formation in patients who have undergone endoscopic PN. • Attention to suture tension during endoscopic partial nephrectomy may reduce the risk of clip migration. • Laser lithotripsy of the calculous cortical rim around the Hem-o-lok clip(s) and removal of the denuded foreign body under direct endoscopic visualization are strongly advisable, since Hem-o-lok clips are usually SWL-resistant. Hem-o-lok clip migration into the ipsilateral collecting system and formation of a distal ureteral stone after robot-assited partial nephrectomy (PN) is a rare condition of stone disease. A 48-year-old male presented with hematuria. Physical examination was unremarkable. Urinalysis showed presence of red blood cells. Serum multiple analysis and ultrasonography were within normal limits. Contrast enhanced abdominal computerized tomography scan revealed an 8-mm right distal ureteral stone, which was not associated with ipsilateral hydronephrosis. With the widespread adoption of minimally invasive surgery, it is not uncommon to utilize foreign bodies as surgical facilitators which also have the potential to migrate to the urothelium-lined urinary tract and act as a stone nidus when used for endoscopic PN Clinicians should be aware of the possibility of migrated hem-o-lok clips serving as a nidus for urinary tract stone formation in patients who have undergone endoscopic PN. Extracorporeal shock wave lithotripsy-related pain is the largest limiting factor in this technique. Our study aimed to compare the effectiveness of different types of analgesics for pain management used during ESWL sessions. We conducted a prospective study of 300 patients with urinary lithiasis justifying ESWL treatment. The patients were randomized to three groups: group I, included 100 patients who received intramuscular injection of 2cc of physiological saline solution (placebo), group II included 100 patients who received intramuscular injection of ketoprofen 100mg while group III included 100 patients who received lidocaine and prilocaine topical cream. Visual Analog Scale (VAS) was used to assess pain 10 minutes after and at the end of the session. Mean VAS score 10 minutes after and at the end of ESWL session was 3.7 and 4.91 respectively. There was no significant difference among the three groups with respect to: epidemiological data (age, sex, BMI, patient's history) and the characteristics of the renal stone (side, size, location, presence or not of double-J ureteral catheter). Eleven patients in the Group I terminated treatment early, with a significant difference compared to the other groups (p=0.003). VAS score 10 minutes after and at the end of ESWL session was statistically higher in Group I compared to Groups II and III (p < 0.001). Moreover, ESWL session was significantly more effective in Groups (II and III) compared to Group I (p<0.001). Pain treatment is necessary during ESWL sessions. Objectives: To analyze the hypothesis that ureteral obstruction may activate kidney latent tuberculous though qualitative study of Urogenital Tuberculosis patients., Methods: A qualitative study was conducted using semistructured interviews in eight patients with Urogenital Tuberculosis. The progression of the disease from the initial symptoms was characterized through the analysis of the clinical and radiological data. The presence of ureteral obstruction prior to the onset of renal tuberculosis was observed in three patients., Results: Patient 1: A 58-year-old female had five episodes of acute left ureteral lithiasis in two years prior to left kidney tuberculosis. Patient 2: A 55-year-old male patient had a 1.2 cm proximal left ureteral stone and in the following six months, the diagnosis of tuberculosis was made in a nonfunctioning left kidney with ureteral thickening and stenosis. Introduction: Bladder stones constitute around 5% of bladder stones in the developed countries. Holmium laser lithotripsy has revolutionised the treatment of urinary lithiasis. Aim: The aim of this study was to report the outcome of transurethral cystolithotripsy with Holmium Laser under Local Anaesthesia (LA) as a day care procedure in patients with bladder stones. Materials and Methods: Patients with bladder stone greater than 1.5cm attending urology Outpatient Department underwent transurethral cystolithotripsy with Holmium Laser under LA as day care procedure. The results were analysed on aspects of peri-operative pain, completion of procedure, stone clearance, hospital stay, complications and patient compliance. Results: A total of 85 patients with bladder stone ≥1.5cm underwent transurethral cystolithotripsy LA. The mean age of the patient was 52±7 years. There were 80 males. The mean size of stone was 3±1.2cm. Mean operation time was 40±10 minutes. Complete stone clearance was achieved in all the patients. None of the patients required hospital stay following the procedure. Conclusion: Transurethral holmium laser lithotripsy is an effective and safe procedure for large bladder stones. This procedure can be easily performed as a day care procedure.

Background: Although the association of urinary lithiasis and urinary tract malformation is not rare, their management poses challenges. The aim of this study was to evaluate the relationship between urolithiasis and malformations of the urinary system. There were 34 patients (19 males and 15 females) with a mean age of 4.8 years (range, 2 months to 14 years). All patients had urinary lithiasis with a urinary tract malformation. Abdominal pain was the most frequent clinical symptom (38%). Urinary infection was found in 7 patients (21%) and macroscopic haematuria was present in 10 patients (29%). The most frequent urinary tract malformations were megaureter (8 cases), uretero-pelvic junction obstruction (7 cases) and vesico-ureteric reflux (8 cases), but its malformative origin could not be confirmed. Treatment consisted of lithiasis extraction in 32 cases associated with specific treatment of the uropathy in 27 cases. Postoperative outcome was uneventful in all cases. In fact, urinary lithiasis and urinary tract malformation association is not rare. Indeed, 9-34% of urinary lithiasis are noted to be associated with urinary tract malformation. Positive diagnosis relies specifically on kidney ultrasound, intravenous urography, and urethrocystography. Treatment depends on the type of urinary tract malformation, localisation and size of the urinary lithiasis. Conclusion: In conclusion, urinary lithiasis and urinary tract malformation association is a frequent eventuality. Surgical intervention is the usual mode of treatment.

Background: Outpatient surgery is critical to improve health care costs. The aim of the study was to prospectively evaluate the results of outpatient treatment of upper tract urinary stones by rigid or flexible ureteroscopy in a routine care setting.Methods: A database was created at the creation of the outpatient surgery department. 87 patients underwent 100 ureteroscopic procedures for urinary lithiasis from June 2013 to March 2015.Results: Most of our patients were male with 53 men (sex ratio M/F 1.13), with a mean age of 52.9 ± 15 years old (23.4-82.4). 44 % of ureteroscopies performed were flexible ureteroscopies, 31 % rigid ureteroscopies and 25 % associated rigid and flexible ureteroscopies. The average stone load was 10.1 ± 5.7 mm (2-30) The mean operating time was 58.3 ± 21.1 min (20-150). 82.9 % of patients had a single urinary stone and 17.1 % (n = 14) had 2 or more. 114 stones were treated, 57,1 % intrarenal. There were 6 (6 %) postoperative complications: three Clavien stage 2 infections; three Clavien stage 3b complications (two renal colics requiring ureteral stenting 48 h after discharge and 1 symptomatic perirenal urinoma 48 h after discharge). There was one intraoperative complication (1 %): a ureteral wound with contrast leakage. The rate of transfer to conventional hospitalization was 2.2 %. Stone size influenced the stone-free status (p < 0.0001) and the need for more than one session. There was a significant correlation between operative time and stone size above 10 mm (p < 0.0001).Conclusions: Flexible and rigid ureteroscopy are safe and efficient procedures for upper urinary tract stones and can be carried out in an outpatient department. "nPercutaneous nephrolithotomy (PNL) is commonly used to treat patients with complex renal calculi. Placing a nephrostomy tube is the last step after completing PNL. Significant early postoperative discomfort after percutaneous procedure is usually secondary to nephrostomy tubes. The goal of this study is to evaluate the safety of tubeless PNL. A total of 45 patients with mean age of 46.6 years entered the study. Tubeless PNL was performed in 28 patients and 17 patients were treated with standard PNL. The results of 2 groups were compared with t test. In both groups, PNL was performed successfully without any significant complication. Postoperative hospitalization in standard group was 3.71 day and in tubeless group was 1.65 day that significant statistical difference was observed (P < 0.05). Analgesic dose using in standard group was 101.56 mg (pethidine) vs 99.07 mg (pethidine) in tubeless group, with no significant statistical difference. There wasn't any organ trauma. Rate of complications, including hematuria, extravasation, fever, UTI and urosepsis, didn't have any significant statistical difference in two groups. It seems that tubeless PNL may be an effective and safe method in renal stone treatment in selected patients. In comparison with standard PNL, tubeless PNL has some benefits including reduction the length of hospitalization. Further studies on more patients are needed to determine the advantages of this technique.

Background and aims: Extra Corporeal Shockwave Lithotripsy (ESWL) is the treatment of choice for urinary lithiasis. Majority of patients experienced pain during ESWL. Acupuncture is a traditional method of treatment in which relieving pain is a feature. This study aimed to evaluate its therapeutic effect on diminishing the pain during ESWL. Method: This clinical trial study performed on 100 patients infected to Urinary lithiasis who were candidate to ESWL operation. They were visited by Acupuncture specialist before starting ESWL and randomly divided into two groups of True Acupuncture and Sham acupuncture. In the first group, acupuncture was carried out in 4 special points of acupuncture for 30 minutes and in the second group acupuncture was ineffectively applied to the same points for 30 minutes. Then ESWL was begun and the intensity of pain compared in two groups using the numeric 0-10 VAS scale. Results: Findings in this study showed that the pain rate mean was significantly lower in true acupuncture group (2.74 ±0.92) than Sham group (4.34 ±0.87). Also, stone size in both groups was significantly related to pain intensity during ESWL (P<0.01). As the stone is bigger, the pain will be more. Age and sex did not have any significant effect on pain. Conclusion: Applying acupuncture in patients, who have been under ESWL, reduces the pain during ESWL. [ABSTRACT FROM AUTHOR]

The holmium:yttrium aluminum garnet (Ho:YAG, holmium) laser is an intracorporeal lithotrite that is widely used in the surgical management of urinary lithiasis. The Ho:YAG laser is capable of fragmenting urinary stones of all compositions while maintaining a wide margin of safety. The 2140-nm wavelength of energy is transmitted from the generator to the stone using specialized silica optical fibers. The effectiveness of the laser can be impacted by the type of laser fiber used, the pulse energy and frequency settings, and the composition of the stone. This paper provides an overview of Ho:YAG laser fibers utilized for lithotripsy during ureteroscopy. We will also review current data regarding optimal energy settings and discuss our experience with different fragmentation techniques. [ABSTRACT FROM AUTHOR]

Purpose: To evaluate the impact of a ureteral access sheath (UAS) on stone-free (SF) rate after flexible ureteroscopy for upper urinary tract stones. Materials and methods: We retrospectively reviewed 280 patients who underwent flexible ureteroscopy (URS) for upper urinary tract stone between 2009 and 2012. Patients were divided into two groups based on whether a UAS was used ( n = 157) or not ( n = 123). SF rate was evaluated at one and three months after surgery by abdominal imaging. Quantitative and qualitative variables were compared with Student's t test and χ2 test, respectively. A logistic regression model was used to determine the predictive factors of SF status. Results: Stone size was similar in both groups (15.1 vs. 13.7 mm, p = 0.21). SF rates at one and 3 months were comparable in UAS and non-UAS groups (76 vs. 78 % and 86 vs. 87 %, p = 0.88 and 0.89, respectively). Complication rates were similar in both groups (12.7 vs. 12.1 %, p = 0.78). In multivariable analysis, stone size was the only predictive factor of SF rate ( p = 0.016). Conclusion: The routine use of a UAS did not improve SF rate in patients undergoing flexible URS for upper urinary tract calculi. [ABSTRACT FROM AUTHOR]
