Abstract

Global health education (GHE) continues to be a growing initiative in many medical schools across the world. This focus is no longer limited to participants from high-income countries and has expanded to institutions and students from low- and middle-income settings. With this shift has come a need to develop meaningful curricula through engagement between educators and learners who represent the sending institutions and the diverse settings in which GHE takes place. The Bellagio Global Health Education Initiative (BGHEI) was founded to create a space for such debate and discussion and to generate guidelines towards a universal curriculum for global health. In this article, we describe the development and process of our work and outline six overarching principles that ought to be considered when adopting an inclusive approach to GHE curriculum development.

11. Evaluating Airport Terminals from the Users’ Perspective: Are Service, Experience, Liking, and Satisfaction Equivalent?

Abstract

In the era of globalization and the deregulation of the air transportation industry, public and private stakeholders have been increasingly interested in making airports attractive for passengers and airlines. Multiple service-like indicators (i.e., satisfaction, experience, liking, and quality of service) have been used to evaluate transportation infrastructure and systems, including airport terminals. Despite literature providing a clear definition of the different indicators separately, the similarities and differences are not clear, nor is it clear whether they are equivalent and could be used interchangeably. The purpose of this paper is to further understanding of the roots and essence of the four frequently used service indicators (SIs) in airport terminals—namely satisfaction, liking, quality of service, and experience—by comparing regressors. Using a travelers’ perception survey (N= 377) at the domestic terminal in Bogotá’s El Dorado Airport, we found that, despite their correlation, the four service proxies were, in essence, different from each other conceptually, meaning that they have to be carefully chosen for the successful evaluation of a terminal. Each SI comprises different categories, representing a unique user perception. Recognizing and acknowledging that all these terms are different furthers the transparency in defining what each indicator measures.

Abstract

La cuarta revolución industrial se presenta, en la actualidad, como un proceso de transformación y evolución productiva. El comportamiento globalizado de los mercados, las nuevas tendencias tecnológicas y el auge de innovadoras metodologías, han transformado la industria en un espacio de interacción interdisciplinar para la toma de decisiones organizacionales. Uno de estos espacios es la generación, obtención y análisis de los datos obtenidos de acuerdo con la relación hombre-máquina, también conocido en la actualidad como el Big Data. El presente artículo de investigación descriptiva tiene por objetivo exponer escenarios de aplicación, control y análisis de técnicas, tecnologías y metodologías asociados al Big Data en tres sectores principales: salud, financiero y transporte y logística. Los resultados obtenidos permiten evidenciar el impacto y relevancia con relación al mundo Big Data y sus aplicaciones en los sectores industriales de estudio como eje de reflexión y repercusiones profesionales. Se concluye que la integración de hardware y software en el campo del Big Data se hace indispensable en pro de la mejora de la calidad de servicio de teleasistencia en los pacientes, así como en los servicios logísticos y financieros.

22. Caracterización de sonidos deglutorios adquiridos mediante auscultación cervical en sujetos sanos y con disfagia

Abstract

Swallowing is an act of high neuromuscular complexity due to the involvement of more than 30 muscle pairs and 5 cranial pairs that occurs in a short period of time. This activity is divided into 4 stages: pre-oral, oral, oropharyngeal and esophageal, an alteration in the normal development of this process can develop a symptom secondary to neuromuscular and neurogenic diseases that is known as dysphagia. Dysphagia can bring many difficulties for those who suffer from it including bronchial pneumonia, malnutrition, dehydration or even death by asphyxia. The identification of characteristics that help recognize this symptom, in addition to correctly describing the swallowing process is of great importance since the existing methods are invasive. Cervical auscultation is a technique by which information about the gothic closure can be obtained in the swallowing process using audio signals, and which can be analyzed offline. The aim of this study is to evaluate different methods of characterization of cervical auscultation signals and develop a machine learning model with manually segmented swallowing event sounds to classify between control subjects and patients with oropharyngeal dysphagia. The results showed, with a maximum accuracy of 75% that by means of signs is cervical auscultation it is possible to identify dysphageal subjects. In the same way it was possible to identify that the average potency of the swallowing segments was the feature with the best score (ROC curve) and that exists a different distribution between classes in this characteristic according to the U-Mann-whitney test to discriminate between healthy and pathologic subjects during different swallowing activities., La deglución es un acto de alta complejidad neuromuscular debido a que intervienen más de 30 pares musculares y 5 pares craneales en un periodo corto de tiempo. Esta se divide en 4 etapas: pre oral, oral, orofaríngea y esofágica, una alteración en el desarrollo normal de alguna de estas fases puede desarrollar un síntoma secundario a enfermedades neuromusculares y neurogénicas que se conoce como disfagia, esta puede traer consigo muchas dificultades para quien la padece, entre esta neumonía bronquial, desnutrición, deshidratación o incluso la muerte por asfixia. La identificación de características que ayuden a reconocer dicho síntoma, además de describir correctamente el proceso deglutorio, es de gran importancia ya que los métodos existentes son invasivos. La auscultación cervical es una técnica mediante la cual se puede obtener información del cierre glótico en el proceso deglutorio por medio de señales de audio, y que puede ser analizada de manera off line. El objetivo de este estudio es evaluar diferentes métodos de caracterización de señales de auscultación cervical y desarrollar un modelo de aprendizaje automático con sonidos de eventos deglutorios segmentados de forma manual para clasificar entre sujetos de control y pacientes con disfagia orofaríngea. Los resultados mostraron, con una exactitud máxima de 75 %, que por medio de señales de auscultación cervical es posible identificar sujetos con disfagia, de igual manera se logró identificar que la potencia media de los segmentos deglutorios fue la característica con mejor rendimiento (curva ROC) y una distribución diferente entre clases según la prueba de U-Mann-Whitney, para discriminar entre sanos y pacientes en diferentes actividades deglutorias.

Abstract

Background Robot-assisted minimally invasive heart surgery is an effective alternative when compared with classical approaches. It has a low mortality and postoperative complications and its long-term durability is comparable with conventional techniques. Aim: To report short- and long-term results with the use of a robot-assisted transthoracic approach. Patients and Methods: Review of patients undergoing heart surgery between 2015 and 2019 using a robot assisted minimally invasive technique in a single center. We analyzed demographic characteristics, surgical and early ultrasound results. Results: Thirteen procedures were reviewed, nine mitral valve repairs (MVR) in patients aged 61 ± 21 years (seven males) and four atrial septal defect (ASD) closures in patients aged from 24 to 52 years (three men). For MVR, the average extracorporeal circulation and myocardial ischemia times were 120 ± 20.9 and 89 ± 21 minutes, respectively. The median hospitalization was four days. Two cases of MVR had postoperative complications. There was no mortality. All cases showed improvement in their symptoms. Ultrasound findings showed no postoperative mitral insufficiency except in one case. Conclusions: We report very good results in both complex mitral repair and CIA closure, comparable to centers with high standards in minimally invasive robot-assisted heart surgery.

Abstract

Resumen Se presenta un caso clínico de un actinomicetoma plantar en un paciente sin factores de riesgo, cuyo diagnóstico fue realizado mediante una biopsia de tejido plantar por sospecha de una neoplasia. Dado que el paciente no respondió satisfactoriamente a la terapia de primera línea, debió completar 24 semanas de tratamiento con doxiciclina, a lo cual evolucionó favorablemente. Finalmente, se desarrolla una breve discusión sobre los micetomas plantares. A case of plantar actinomycetoma without risk factors is presented, which was diagnosed by hystopatological analysis of a foot biopsy because of the suspicion of neoplasia. Since the patient did not fully respond to the first-line therapy antibiotics, a 24-weeks doxycycline regime was started, achieving a satisfactory response. Finally, a brief discussion on plantar mycetomas is presented.
