3. Autologous Free Reverse Inlay Vein Graft in The Treatment of Cripple Hypospadias; An Experimental Rabbit Model.

Objective: The aim of this study is using the autologous reverse inlay jugular vein graft as a substitute for defective urethral plate and the incorporation of the graft to the urethral plate. Cripple hypospadias are the patients who have a history of previous multiple unsuccessful hypospadias surgery and still having anatomical and functional complications. Insufficient local or support tissue for the repair of defective urethral plate is always a problem. Material and Methods: In the study fourteen male 8-10 months old which is weighting four kg, New Zealand type rabbit are divided into two equal groups. Juguler vein graft (1st group) and buccal mucosal graft (2nd group) harvested and transferred to defective area in the urethral plate as a free flap. Results: In the histopatologic examination, epithelization, fibrosis and inflammation degrees were evaluated between the groups and in-groups. In the histopatological examination of the 1st group, uroepithelization is complete or nearly complete, fibrotic activity is (+) and the inflammation was found minimal. In the second group after 21st day, the uroepithelization on the graft is nearly thirty percent, fibrotic activity is (++) and the inflammation is also found minimal. Conclusion: From the results of the study, in the treatment of defective urethra in cripple hypospadias cases, reverse free juguler vein graft can be used as an alternative for buccal mucosa graft as a substitution material which has been used for a long time. Background: Primary urethral cancer is defined as a tumor whose first lesion is located in the urethra; is very uncommon, represents less than 1% of malignant tumors and 5% of malignant tumors of the urinary system. The extension of this process to the prostate gland creates obstructive and irritative urinary symptoms and acute conditions such as macroscopic hematuria. Objective: To present the case of a patient with Primary urethral carcinoma in its urothelial variety with macroscopic hematuria as clinical presentation. Case presentation: Clinical case of a 75-year-old male patient, with macroscopic hematuria as a clinical presentation of primary cancer of the urethra in its urothelial variety, diagnosed anatomopathologically during the hematuria study. Conclusions: The case presented alerts the scientific community that the diagnosis of primary urethral cancer should be considered in patients with macroscopic hematuria in the absence of obstructive or irritative urologic manifestations, although it is a rare sign as clinical presentation of this disease.

Introducción. La fractura de pene es una emergencia urológica rara, se desconoce la incidencia real dado el subregistro causado por la reducida consulta de los pacientes, resultado del embarazoso contexto. En Estados Unidos alcanza un 38% donde la etiología de índole sexual predomina. En Colombia hay escasos reportes publicados. El diagnóstico de esta entidad es netamente clínico. El objetivo de este trabajo es reportar una evolución satisfactoria en términos de función miccional y eréctil en un paciente sometido a reconstrucción cavernosa y uretral temprana en rotura bilateral de origen traumático mediante una técnica quirúrgica poco convencional, de acuerdo con lo hallado en la literatura. Presentación del caso. Paciente masculino de 30 años, previamente sano, quien consultó al servicio de urgencias por uretrorragia, edema y dolor peneano secundario a trauma contuso del mismo durante relación sexual. El reporte ecográfico mostró disrupción en túnica albugínea del cuerpo esponjoso, hematoma y aparente transección uretral; se realizó exploración quirúrgica 6 horas después, con evidencia de fractura de cuerpos cavernosos, laceración del 40% de la circunferencia, transección completa de la uretra y cuerpo esponjoso. Se realizó rafia de cuerpos cavernosos y uretroplastia término terminal de uretra bulbar, con evolución clínica satisfactoria. Discusión. La reconstrucción quirúrgica en menos de 24 horas en fractura de pene disminuye significativamente la estancia hospitalaria y complicaciones precoces; asimismo, desciende el riesgo de disfunción eréctil, erecciones dolorosas y problemas miccionales. No hay publicaciones que comparen resultados a largo plazo entre los dos abordajes quirúrgicos. Conclusiones. La fractura de pene es una patología poco frecuente con diagnóstico clínico, la cual debe manejarse de manera temprana por cualquier urólogo y, en caso de ser posible, con experiencia reconstructiva y excelentes resultados en la función sexual y miccional. The aim of this work is to report a satisfactory evolution in terms of voiding and erectile function in a patient who underwent early cavernous and urethral reconstruction in bilateral rupture of traumatic origin by means of an unconventional surgical technique, in accordance with what has been found in the literature. Case Presentation. A 30-year-old male patient, previously healthy, consulted the emergency department for urethrorrhagia, edema and penile pain secondary to blunt trauma to the penis during sexual intercourse. The ultrasound report showed disruption in the tunica albuginea of the corpus spongiosum, hematoma and apparent urethral transection; surgical exploration was performed 6 hours later, with evidence of fracture of the corpora cavernosa, laceration of 40% of the circumference, complete transection of the urethra and corpus spongiosum. Sutures to the corpora cavernosa and end-to-end urethroplasty of the bulbar urethra were performed, with satisfactory clinical evolution. Discussion. Surgical reconstruction in less than 24 hours in penile fractures significantly reduces hospital stay and early complications; it also reduces the risk of erectile dysfunction, painful erections and voiding problems. There are no publications comparing long-term results between the two surgical approaches. Conclusions. Penile fracture is a rare pathology with clinical diagnosis, which should be managed early by any urologist and, if possible, with reconstructive experience and excellent results in sexual and voiding function. Introdução. A fratura peniana é uma emergência urológica rara, a real incidência é desconhecida dada a subnotificação causada pela reduzida consulta de pacientes, resultado do contexto constrangedor. Nos Estados Unidos chega a 38% onde predomina a etiologia de natureza sexual. Na Colômbia há poucos relatórios publicados. O diagnóstico desta entidade é puramente clínico. O objetivo deste trabalho é relatar uma evolução satisfatória da função miccional e erétil em um paciente submetido à reconstrução cavernosa e uretral precoce em ruptura bilateral de origem traumática por meio de técnica cirúrgica não convencional, de acordo com o que foi encontrado na literatura. Apresentação do caso. Paciente do sexo masculino, 30 anos, previamente saudável, procurou o pronto-socorro por uretrorragia, edema e dor peniana secundária a trauma contuso durante a relação sexual. O relatório do ultrassom mostrou ruptura na túnica albugínea do corpo esponjoso, hematoma e transecção uretral aparente; a exploração cirúrgica foi realizada 6 horas depois, com evidência de fratura dos corpos cavernosos, laceração de 40% da circunferência, transecção completa da uretra e corpo esponjoso. Foram realizadas ráfia dos corpos cavernosos e uretroplastia término-terminal da uretra bulbar, com evolução clínica satisfatória. Discussão. A reconstrução cirúrgica em menos de 24 horas nas fraturas penianas reduz significativamente o tempo de internação e as complicações precoces. Da mesma forma, diminui o risco de disfunção erétil, ereções dolorosas e problemas de micção. Não há publicações comparando os resultados a longo prazo entre as duas abordagens cirúrgicas. Conclusões. A fratura peniana é uma patologia rara com diagnóstico clínico, que deve ser tratada precocemente por qualquer urologista e, se possível, com experiência reconstrutiva e excelentes resultados na função sexual e miccional.

The most common clinical symptom of HPV is the condyloma acuminata, characterized by type 6 and 11 and constitutes 90% of the infections. However, 12% of the smear specimens were found to have HPV types 16 and 18, which are actually oncogenic. This virüs which frequently infects external genitalia, infects mucocutaneous component much less frequently likewise urethra as a part of mucocutaneous component. We present a case of A 58-year-old menapousal woman consulted with gynecological and obstetrics department due to pathology report of condyloma accumunata of previously excited mass from anal region by a general surgeon. At the examination of the patient, two white clitoral condyloma with white colored tiny condyloma on the right side of the urethra were seen, both HPV 16 and 18 were detected aftetwards. Urethral condyloma acuminata of atypical location at urethral mucosa which coincided with vulvar condyloma, can be a clue for oncogenic HPV infection. We aimed to discuss the mucosal involvement of HPV which is rarer but has a high probability of being oncogenic

BACKGROUND: The primary carcinoma of the urethra was described for the first time in the year 1834, it represents less than 0.5% of malignant neoplasms in men, it is subdivided into primary and secondary, when it is located for the first time in this site it represents the primary form, the Secondary occurs when there is a history of previous injury to the urethra or any site of the urinary tract. CLINICAL CASE: We present the case of a 27-year-old diabetic male, who began two months before the biopsy due to obstructive urinary symptoms, hematuria and dysuria was reported as a poorly differentiated primary adenocarcinoma of the urethra with a signet ring cell component, CK 7 positive, in which a primary lesion in another genitourinary site, prostate, lung, and gastrointestinal tract was ruled out. CONCLUSION: This case is interesting because it is a rare and aggressive tumor, in stage T2 due to invasion of prostate tissue. Although the prognosis of the disease is poor, some patients may benefit from surgical resection, especially those with tumors originating in the proximal urethra; chemotherapy should be discussed with a multidisciplinary team. The International Continence Society defines urinary incontinence (UI) as the involuntary loss of urine, a potentially treatable condition with a decrease in the quality of life of women in different aspects, including sexual health with their partner and their social well-being. It has been described that only 44.9% of women consult, having higher rates of depression and anxiety. The objective of this research was to determine the occurrence of intraoperative and postoperative complications with the performance of transobturator and retropubic taping at the Central Military Hospital in Bogota during the period 2015 to 2020. An observational, descriptive, retrospective, retrospective. All patients with diagnoses of urinary incontinence, both stress, emergency, and/or mixed, operated with tension-free mid-urethral tapes by retropubic or transobturator route were included. The most prevalent complications after the surgical procedure were pain (96.8 %), followed by urinary tract infection or expulsion of the band (8.6 %), among other less frequent complications such as hematomas, dyspareunia, and abscesses. Most patients obtained an adequate clinical response in the postoperative period, 77 % did not require additional management. Mediourethral tapes provide a good result with a low incidence of surgical complications and recurrence, improving the patient's quality of life concerning her urinary incontinence pathology. Introducción. El prolapso uretral es una entidad poco común, con una incidencia estimada de 1 en 3000 mujeres. Se presenta cuando la mucosa uretral sobresale espontáneamente más allá del meato uretral. Es una patología poco diagnosticada dada su baja frecuencia y de allí la importancia de conocer sobre su presentación, diagnóstico y tratamiento. El objetivo de este artículo fue presentar el caso de una paciente de 10 años con diagnóstico de prolapso uretral y su manejo quirúrgico. Caso clínico. Paciente femenina de 10 años, que consultó por cuadro clínico de 1 año de evolución consistente en dolor en región urogenital, que se irradiaba a hipogastrio, asociado a pujo y disuria, a quien se le diagnosticó prolapso uretral y se realizó corrección quirúrgica de mucosa uretral prolapsada mediante técnica de Kelly-Burnham modificada. Conclusión. El prolapso uretral es una entidad que con frecuencia es diagnosticada erróneamente pues su diagnóstico es eminentemente clínico. Si bien se ha descrito el tratamiento médico en primera instancia, éste tiene una alta tasa de recurrencia, por lo que en estos casos se prefiere la resección quirúrgica del tejido prolapsado.

Urethral process, sigmoid flexure, urethra at the pelvic arch and neck of urinary bladder in goats are very much prone to form obstructive urolithiasis. The present study was carried out to establish baseline work on the normal measurement of the urethral passage and different segments of penis of uncastrated and castrated Black Bengal (BB) goat which will be supportive for obstructive urolithiasis management. Different segment of penis along with urethra of 25 uncastrated and 25 castrated male Black Bengal goats were collected from different slaughter house of Chattogram City Corporation. The length and diameter of male urethra and different segment of penis in uncastrated buck were significantly higher (p<.001) than castrated buck. The length of the pelvic urethra was 5.88 ± 0.111 cm and 4.58 ± 0.15 cm in uncastrated and castrated buck respectively. The length of the neck of urinary bladder; root, body, sigmoid flexure, glans penis and urethral process of the penis in uncastrated and castrated buck were 0.688 ± 0.156 cm and 0.38 ± 0.048 cm, 5.02 ± 0.09 cm and 4.24±0.10 cm, 23.48 ± 0.37 cm and 22.74 ± 0.43 cm, 9.2 ± 0.107 cm and 8.18 ± 0.29 cm, 1.92 ± 0.032 cm and 1.44 ± 0.10 cm, 2.32 ± 0.029 cm and 1.78 ± 0.07 cm respectively. The diameter of the neck of urinary bladder, root, body, sigmoid flexure, glans penis and urethral process of the penis in uncastrated and castrated buck were 1.94 ± 0.041 cm and 1.44 ± 0.05 cm, 3.7 ± 0.049 cm and 3.3 ± 0.144 cm, 2.63 ± 0.066 cm and 1.86 ± 0.08 cm, 2.77 ± 0.90 cm and 2.18 ± 0.07 cm, 1.81 ± 0.034 cm and 1.4 ± 0.06 cm, 0.66 ± 0.014 cm and 0.56 ± 0.05 cm respectively. Finally, total lengths of penis in uncastrated and castrated buck were 30.42±0.459 cm and 28.42±0.553 cm respectively. The length and diameter of the neck of the urinary bladder, root, sigmoid flexure, urethral process of the penis are clinically more important for obstructive urolithiasis. [ABSTRACT FROM AUTHOR]

It is aimed to review the literature regarding the clinical evaluation of female urinary incontinence in gynecologic consultation. An exploration of the literature in several different electronic databases, books printed texts in indexed journals and recognized scientific societies, to identify relevant literature was trying to find the best evidence in the clinical assessment of urine leakage in gynecological consultation. They are not allowing sufficient scientific documents suggest which is the most specific clinical examination in the assessment of incontinence in gynecologic consultation, but the correlation of findings from each other, facilitates the same. The literature shows that the methods of clinical evaluation of urinary incontinence in gynecologic consultation play an important role in the correct diagnosis of patients and proper treatment thereof; therefore, it is necessary to turn our eyes to the appropriate clinical evaluation in gynecological consultation.

Objective: Posterior urethral trauma is rare type of trauma that is particularly difficult to treat in children. Here, we have presented our experience with tubularised buccal mucosal graft (BMG) repairs for posterior pelvic fracture urethral distraction defects (PFUDDs) in children. Material and Methods: We retrospectively evaluated three cases of tubularised BMG repairs for PFUDDs in our clinic between 2009 and 2016. The BMGs were 3 cm in length, on average, and sutured between the stump of the proximal prostatic urethra and the distal bulbar urethra. All of the patients underwent urethrography, urethroscopy and uroflowmetry assessments postoperatively. Results: The mean patient age was 9 (6-14) years and all three patients were male. Two of the three patients with posterior urethral injuries were operated on at least twice at another centre and referred to our clinic with PFUDDs. All three patients underwent tubularised BMG urethroplasties for the PFUDDs. We noted urethral integrity in the cystoscopic evaluations of two of the patients, with no strictures, and these two patients were fully continent. A proximal anastomotic contracture was noted in the third patient and a revision was required. Conclusion: A tubularised BMG should be used to prevent penile shortening and anastomotic tension in a PFUDD. [ABSTRACT FROM AUTHOR]

In this work, a case of urethral prolapse was studied and it is characterized by a protrusion of the urethral mucosa and the external orifice of the urethra at the end of the penis, being a pathophysiology that affects young male dogs, being able to observe a genetic predisposition of races brachycephalic. The diagnosis is elucidated mainly by clinical examination, where the protruding urethral mucosa is visualized and surgical treatment is recommended when it is observed that the case is recurrent or that the alterations of the urethral mucosa are irreversible, it is recommended that the most effective treatment of this condition is the technique of urethral prolapse resection and anastomosis. The objective of this study was to report a case of urethral prolapse in an English Bulldog canine patient who was treated at a private clinic in the city of Fortaleza, Ceará, a one year old 18 kg Bulldog, with diagnosis of urethral prolapse. Introduction: Post-traumatic rupture of the posterior urethra is a serious injury that can compromise the micturition and erectile prognosis of the often-young patient. The management of this lesion is still controversial, leaving the choice between early endoscopic realignment or suprapubic catheterization with deferred urethroplasty. The objective of this study was to report our clinical experience and outcomes with early endoscopic realignment (EER) for patients with pelvic fracture urethral injury., Patients and Methods: We underwent a retrospective review of patients with pelvic fracture associated urethral injury who underwent EER from 2010 to 2020. Preoperative, perioperative, and postoperative outcome data were collected. Complications for the surgical procedure were analyzed, as well as postoperative stenosis, urinary incontinence and erectile dysfunction. The primary endpoint was success, defined as satisfying micturition with no urethral stricture at the time of last follow-up., Results: Early endoscopic realignment was performed in 26 patients managed for complete post-traumatic posterior urethral rupture. The median age was 26 (16-39) years. The most common mechanism of urethral injury was road traffic accidents in 69.23% of cases. The most common urethral injury was grade 4 in 23 patients (88.46%). The median time to endoscopic realignment was 8 days (3-18). The median time to postoperative bladder catheterization was 22 (10-32) days. The median follow-up time was 34 (18-54) months. Ten patients developed urethral stricture during follow-up: 7 (26.92%) were treated with one or two internal cold blade urethrotomies, 3 required urethroplasty. There were no urethroplasty failures after a first endoscopic realignment. Two patients reported severe stress urinary incontinence. The median IIEF-5 score at the date of last news was 23 (17-25)., Conclusion: Early endoscopic realignment allows some patients to avoid a heavier surgical treatment, and doesn't compromise the realization of a later urethroplasty., (Copyright © 2023 The Authors. 32. Transverse preputial island flap in the ventral coverage of Neourethroplasties and Ortophaloplasties for correction of male proximal hypospadias

Esta tese tem como objetivo, apresentar uma técnica desenvolvida em 2001, utilizando o retalho transverso em ilha do prepúcio, como forma de cobertura da falha ventral no tratamento de hipospádias proximais após a neouretroplastia, e nas ortofaloplastias como primeiro tempo nas cirurgias em estágios. Enquanto o tratamento de hipospádias na forma distal é bem definido na literatura, há discussão acerca de qual o melhor procedimento para a forma proximal, em tempo único ou em estágios, com utilização de retalhos prepuciais tubulizados ou não, enxertos de pele ou de mucosas, necessidade e tipos de corporoplastias, aproveitamento ou não da placa uretral, tipos de cobertura da neouretra e da falha ventral, entre outros. Desde 1982, adquirimos experiência com o uso de retalhos tubulizados e duplos retalhos transversos do prepúcio, e introduzimos modificações técnicas para reduzir complicações na correção de hipospadias proximais em tempo único. Entretanto, ao constatarmos que a maioria das complicações estavam relacionadas à porção tubulizada, não ao retalho utilizado para cobertura da falha ventral, decidimos modificar nossos procedimentos e preservar a placa uretral para a formação da neouretra quando possível. Assim, a partir de 2001, passamos a utilizar o retalho em ilha transverso do prepúcio, como forma de cobertura da falha ventral tanto no tratamento de hipospádias proximais, após a neouretroplastia, quanto nas ortofaloplastias, como primeiro tempo nas cirurgias em estágios. Apresentaremos os resultados de 38 meninos portadores de hipospádia proximal, sendo que 26 foram submetidos à neouretroplastia primária pela técnica de TIP (“Tubulized Incised Plate” longo), e 12 realizaram ortofaloplastias como primeiro tempo de cirurgia estagiada. Em ambas as técnicas a cobertura da falha ventral foi realizada com retalho transverso do prepúcio, sendo utilizado desepitelização das bordas laterais e inferiores, nos casos de cirurgia em tempo único (TIPM). Nosso interesse primário foi o de avaliar tecnicamente a utilização do retalho do modo preconizado, bem como a incidência de complicações, e verificar se este tipo de xvii cobertura da neouretra alteraria os índices das principais complicações de neouretroplastias longas. A média da idade por ocasião do tratamento cirúrgico para ambos os grupos (TIPM e Ortofaloplastia para a cirurgia em estágios) foi de 38,6 meses, com um período de seguimento médio de 37,1 meses. Houve necessidade de corporoplastias em 50% das cirurgias em tempo único e em 25% nos casos de cirurgia estagiada. Não ocorreu nenhuma complicação vascular no retalho ventral, verificando-se epidermólise superficial no prepúcio dorsal remanescente em 2 dos 26 casos de tempo único. Ocorreu deiscência parcial do retalho em 3 casos de TIPM e em um de Ortofaloplastia. As complicações tardias foram de curvatura residual em apenas um caso de ortofaloplastia, sem corporoplastia, realizada na primeira cirurgia, fístula da neouretra em 3 dos 26 casos (11,5%) de TIPM, e cistos de inclusão dérmica decorrentes da desepitelização das bordas do retalho em 7 dos 26 casos (27%) de correção em tempo único. Utilizando critérios preestabelecidos para classificação de resultados, obteve-se 77% de bons e 23% de regulares no aspecto plástico, e 88,5% de bons e 11,5% regulares, como resultados funcionais para as cirurgias em tempo único. Para a cirurgia estagiada, apenas o aspecto plástico-estético foi avaliado com resultado de 92% bom e 8% regular. Após as reoperações no grupo TIPM, de 9 cirurgias em 7 pacientes, com média de 1,34 cirurgias por paciente neste grupo, obteve-se 96% de bons resultados plásticos e 100% funcionais. Conclui-se que a nas correções de hipospadias proximais, a utilização do retalho transverso em ilha do prepúcio, na cobertura ventral de ortofaloplastias e neouretroplastias, com preservação da placa uretral tubulizada, é seguro e sem complicações vasculares no retalho. Contribui também, para uma diminuição na incidência de complicações, reoperações e, ainda, apresenta uma ótima camada de pele para futura tubulização nos casos de cirurgia em estágios. This thesis objective is to present a technique developed in 2001 that uses the transverse island flap of the foreskin as a form of coverage of the ventral failure in the treatment of proximal hypospadias after neourethroplasties, and orthophaloplasties as the first stage procedure. While the treatment of distal hypospadias is well-documented in the literature, there is debate on which would be the best procedure in proximal hypospadias, if single or multiple stages procedures, the use of tubulized or not preputial flaps, use of skin or mucosal grafts, need and types of corporoplasties, preservation or not of the urethral plate, types of coverage of the neourethra and ventral failure, among others. From 1982, we have acquired experience in the use of tubulized flaps and double transverse flaps of the foreskin for proximal hypospadias forms, including our own technical modifications to decrease the complications rate in a one-stage hypospadias corrections. However, realizing that most complications were related to the tubulized portion, and not in the flap used as a cover of ventral failure, and keeping in mind the interest in preservation, we have decided to modify our procedures and preserve the urethral plate, with primary tubulization for the formation of neourethra. Therefore, since 2001, we decided to use a transverse island flap of the foreskin as a form of coverage of the ventral failure in the treatment of proximal hypospadias after neourethroplasties; similarly, this has been used in orthophaloplasties as the first-stage procedure. We present the results of 38 boys with proximal hypospadias, 26 of which were submitted to primary neourethroplasty by the Tubulized Incised Plate technique and 12 which underwent orthophaplasties as the first-stage surgery. In both techniques, the coverage of ventral failure was performed with a transverse flap of the foreskin, with deepithelialization of the lateral and inferior edges used in cases of single-stage surgery (TIPM). Our primary interest was to technically evaluate the use of the flap in the recommended way, as well as the incidence of complications, and to verify whether this type of neourethra coverage would change the rates of the main complications of long neourethroplasties. The mean age at the time of surgical treatment for both groups (TIPM and Orthophaloplasty) was 38.6 months, with a mean follow-up period of 37.1 months. There was need for corporoplasties in 50% of single and in 25% of the cases of the staged surgeries. There was no vascular complication in the ventral flap, and superficial epidermolysis occurred in the remaining dorsal foreskin in 2 of the 26 of the one-stage procedures. Partial flap dehiscence occurred in 3 cases of TIPM and in one of Orthophofaloplasty. Late complications were: residual curvature in only one case of Orthophoplasty without previous corporoplasty in the first surgery; neourethra fístula in 3 of the 26 cases (11.5%) of TIPM; and dermal inclusion cysts resulting from the of deepithelialization of the flap edges in 7 of the 26 cases (27%) of single-stage corrections. Using pre-established criteria for classification of results, 77% were classified as good and 23% as regular in the plastic aspect, and 88.5% as good and 11.5% as regular (correction of fístulas) as functional results for surgeries in the single-stage group. For the staged surgery only the plastic aesthetic aspect was evaluated with a result of 92% good and 8% regular (1 case with residual curvature). After reoperations in the TIPM group, 9 surgeries were conducted in 7 patients, with an average of 1.34 surgeries per patient in this group: 96% of good plastic and 100% functional results were obtained. We conclude that in the corrections of proximal hypospadias the use of the transverse island flap of the foreskin in the ventral coverage of orthophaplasties and neourethroplasties with preservation of the tubulized urethral plate is safe, with no vascular complications in the flap resulting from the technique. The technique contributes to a decrease in the incidence of complications and reoperations, and still represents an excellent layer of skin for future tubulization in cases of staged surgeries.

Background: The urethral prolapse in dogs is a rare condition known by the protrusion of the urethral mucous membrane and the external orifice of the urethra. It is more frequently seen in young males, especially of brachycephalic breeds, for instance the English bulldog. Despite the pathophysiology of this disorder being little elucidated, it is believed that the cause is related to factors such as genetic susceptibility, excessive sexual behavior, traumas, abnormalities and urinary and prostatic problems. Due to limited reports on the subject, this paper aims to describe the clinical and surgical aspects of a case of urethral prolapse in a dog, surgically corrected. Case: Admitted to the Veterinary Hospital of the Federal University of Mato Grosso (HOVET-UFMT) a dog, American Pit Bull, 7 months old, with previous history of bleeding in the penile region and pain while urinating. In the physical exam it presented: intermittent bleeding via external ostium of the urethra, increased volume and protrusion of the distal urethral mucous membrane and the external orifice of the urethra, which was presenting a round shape mass, edematous and little congested of red-purplish coloring, evidenced by the passing of urethral probe. The diagnosis of urethral prolapse was confirmed and, after conducting laboratory tests and obtaining normal results for the species, the animal was sent to surgery. It was opted for the technique of resection and anastomosis of the protruded portion of the mucous membrane. After the anesthetic protocol, it was performed the trichotomy and antisepsis of region, the fenestrated drapes were properly positioned and the urethral catheterization was done, afterwards 3 points of support were produced with nylon thread 3-0, involving the urethra and the external portion of the penis. Subsequently, it was incised 1/3 of the protruded mucous membrane (from a support point to the other) with a pair of iris scissors and the aid of a toothless Adson clamp. Promptly the anastomotic synthesis was manufactured with a simple interrupted suture pattern. By the end of the first one third the remaining ones with go under the same procedure and in the end of the resection and anastomosis of the urethral prolapse the animal was submitted to a bilateral orchiectomy. At the immediate post-surgery it was established antibiotic therapy and the use of anti-inflammatory and painkiller, after 48 h of observation the animal was discharged from the hospital. As a therapeutic measure it was opted to continue with the use of antibiotics and anti-inflammatory, and then recommended the use of Elizabethan collar 24 h a day until the removing of the stitches. It was also recommended that the animal returned for a new evaluation thirteen days after of the procedure. Discussion: That being said, even being an unusual pathology, which the physiopathology is not completely clear, the urethral prolapse is of simple diagnosis, which is based on direct observation of the protruded mucous membrane and by obtaining information of possible factors that cause its appearance, such as genetic susceptibility, in the case of the animal from the current report, since it had the English Bulldog as genetic predecessor. Even though there are techniques less traumatic for its diminishing the chosen technique is the resection and anastomose of the protruded portion of the urethral mucous membrane, due to being simple, quick, effective and with lower rates of relapses. La hiperplasia o adenoma, es una afección benigna por el crecimiento excesivo del tejido de la próstata, que presiona la uretra y la vejiga, bloquea así el flujo de la orina creando síntomas incómodos en el tracto urinario inferior. Se pueden utilizar muchos métodos imagenológicos o uroendoscópicos para estimar el volumen de la próstata y diagnosticar la hiperplasia prostática. La ecografía transabdominal (TAUS) ha sido muy utilizada para la estimación del volumen de la próstata, aunque su uso sobreestima el volumen de la próstata. Sin embargo, la ecografía transrectal (TRUS) se considera actualmente el estándar de oro para la estimación del volumen prostático. El objetivo de la presente investigación está en aportar evidencia para el correcto diagnóstico ecográfico de hiperplasia benigna prostática.

La fractura de pene es una emergencia urológica rara, se desconoce la incidencia real dado el subregistro causado por la reducida consulta de los pacientes, resultado del embarazoso contexto. En Estados Unidos alcanza un 38% donde la etiología de índole sexual predomina. En Colombia hay escasos reportes publicados. El diagnóstico de esta entidad es netamente clínico. El objetivo de este trabajo es reportar una evolución satisfactoria en términos de función miccional y eréctil en un paciente sometido a reconstrucción cavernosa y uretral temprana en rotura bilateral de origen traumático mediante una técnica quirúrgica poco convencional, de acuerdo con lo hallado en la literatura. Presentación del caso. Paciente masculino de 30 años, previamente sano, quien consultó al servicio de urgencias por uretrorragia, edema y dolor peneano secundario a trauma contuso del mismo durante relación sexual. El reporte ecográfico mostró disrupción en túnica albugínea del cuerpo esponjoso, hematoma y aparente transección uretral; se realizó exploración quirúrgica 6 horas después, con evidencia de fractura de cuerpos cavernosos, laceración del 40% de la circunferencia, transección completa de la uretra y cuerpo esponjoso. Se realizó rafia de cuerpos cavernosos y uretroplastia término terminal de uretra bulbar, con evolución clínica satisfactoria. Discusión. La reconstrucción quirúrgica en menos de 24 horas en fractura de pene disminuye significativamente la estancia hospitalaria y complicaciones precoces; asimismo, desciende el riesgo de disfunción eréctil, erecciones dolorosas y problemas miccionales. No hay publicaciones que comparen resultados a largo plazo entre los dos abordajes quirúrgicos. Conclusiones. La fractura de pene es una patología poco frecuente con diagnóstico clínico, la cual debe manejarse de manera temprana por cualquier urólogo y, en caso de ser posible, con experiencia reconstructiva y excelentes resultados en la función sexual y miccional. Penile fracture is a rare urological emergency, the real incidence is unknown given the underreporting caused by the reduced consultation of patients, resulting from the embarrassing context. In the United States it reaches 38%, where sexual etiology predominates. In Colombia there are few published reports. The diagnosis of this entity is purely clinical. The aim of this work is to report a satisfactory evolution in terms of voiding and erectile function in a patient who underwent early cavernous and urethral reconstruction in bilateral rupture of traumatic origin by means of an unconventional surgical technique, in accordance with what has been found in the literature. Case Presentation. A 30-year-old male patient, previously healthy, consulted the emergency department for urethrorrhagia, edema and penile pain secondary to blunt trauma to the penis during sexual intercourse. The ultrasound report showed disruption in the tunica albuginea of the corpus spongiosum, hematoma and apparent urethral transection; surgical exploration was performed 6 hours later, with evidence of fracture of the corpora cavernosa, laceration of 40% of the circumference, complete transection of the urethra and corpus spongiosum. Sutures to the corpora cavernosa and end-to-end urethroplasty of the bulbar urethra were performed, with satisfactory clinical evolution. Discussion. Surgical reconstruction in less than 24 hours in penile fractures significantly reduces hospital stay and early complications; it also reduces the risk of erectile dysfunction, painful erections and voiding problems. There are no publications comparing long-term results between the two surgical approaches. Conclusions. Penile fracture is a rare pathology with clinical diagnosis, which should be managed early by any urologist and, if possible, with reconstructive experience and excellent results in sexual and voiding function.

A prevalência de incontinência urinária em gestantes diabéticas é significantemente elevada e persiste por até dois anos após o parto cesárea, podendo ser a sequela mais frequente da hiperglicemia gestacional comparada a outras complicações. Dessa forma, identificar os fatores de risco para o desenvolvimento da incontinência urinária em diabéticas é o principal objetivo na prevenção dessa condição tão comum. Pesquisas recentes apontam que não apenas o músculo uretral mas também a matriz extracelular uretral desempenham papel importante no mecanismo da continência urinária. Os trabalhos do nosso grupo de pesquisa evidenciaram que, em ratas, o diabetes durante a prenhez lesa a matriz extracelular e o músculo estriado uretral, o que pode explicar a alta prevalência de incontinência e disfunção do assoalho pélvico em mulheres com diabetes mellitus gestacional. O diabetes exerce efeito sobre a expressão, organização e alteração dos componentes da matriz extracelular em diversos órgãos, e a remodelação do tecido e a fibrose parecem ser uma consequência direta dele. Assim, a compreensão do impacto de fatores de risco modificáveis, como o diabetes, permitirá que, utilizando estratégias preventivas, reduzamos as taxas de incontinência urinária, bem como os custos de assistência à saúde, e melhoremos a qualidade de vida das mulheres, especialmente na gestação e no pós-parto.

41. Hipospadia perineal em um cão: relato de caso

A hipospadia é uma anomalia congênita da genitália externa, na qual a uretra peniana termina ventral e caudalmente à sua abertura normal. Pode ser classificada, com base na localização da abertura uretral, como glandular, peniana, escrotal, perineal e anal. O objetivo deste trabalho é relatar um caso raro de hipospadia perineal, em que são abordados seus aspectos clínicos e terapêuticos, em um cão macho, sem raça definida, com seis meses de idade. Um canino com histórico de suspeita de hermafroditismo e presença de um orifício, logo abaixo do ânus, pelo qual urinava foi atendido no Hospital de Clínicas Veterinárias da Universidade Federal do Rio Grande do Sul (UFRGS). Durante o exame físico, foi observado o meato uretral localizado ventralmente ao ânus, além de alterações no prepúcio, pênis e escroto. Foram realizados exames de triagem, tais como: hemograma completo, urinálise, ecografia abdominal e uretrocistografia. O cão foi encaminhado para a cirurgia, quando foi submetido à amputação peniana e prepucial total e também à orquiectomia. O canino recebeu antibioticoterapia, analgésicos, anti-inflamatório e limpezas diárias dos pontos e da região perineal e apresentou excelente recuperação pós-operatória. Após 30 dias da cirurgia, o paciente retornou ao hospital, e foi verificada completa cicatrização da ferida cirúrgica, sem sinais de infecção urinária e inflamação ou assaduras na região perineal. A hipospadia perineal é um caso raro de anomalia uretral congênita, ainda pouco documentada em medicina veterinária. O procedimento cirúrgico realizado demonstrou ser importante, uma vez que removeu toda a genitália externa defeituosa, onde, futuramente, poderiam desenvolver-se infecções e/ou inflamações. Com isso, minimizou as complicações dessa doença e melhorou a qualidade de vida do paciente.

Resumen Introducción. El prolapso uretral es una entidad poco común, con una incidencia estimada de 1 en 3000 mujeres. Se presenta cuando la mucosa uretral sobresale espontáneamente más allá del meato uretral. Es una patología poco diagnosticada dada su baja frecuencia y de allí la importancia de conocer sobre su presentación, diagnóstico y tratamiento. El objetivo de este artículo fue presentar el caso de una paciente de 10 años con diagnóstico de prolapso uretral y su manejo quirúrgico. Caso clínico. Paciente femenina de 10 años, que consultó por cuadro clínico de 1 año de evolución consistente en dolor en región urogenital, que se irradiaba a hipogastrio, asociado a pujo y disuria, a quien se le diagnosticó prolapso uretral y se realizó corrección quirúrgica de mucosa uretral prolapsada mediante técnica de Kelly-Burnham modificada. Conclusión. El prolapso uretral es una entidad que con frecuencia es diagnosticada erróneamente pues su diagnóstico es eminentemente clínico. Si bien se ha descrito el tratamiento médico en primera instancia, éste tiene una alta tasa de recurrencia, por lo que en estos casos se prefiere la resección quirúrgica del tejido prolapsado. Abstract Introduction. Urethral prolapse is a rare entity, with an estimated incidence of 1 in 3,000 women. It occurs when the urethral mucosa spontaneously protrudes beyond the urethral meatus. It is a poorly diagnosed pathology given its low frequency and hence the importance of knowing about its presentation, diagnosis and treatment. The objective of this article was to present the case of a 10-year-old patient with a diagnosis of urethral prolapse and its surgical management. Clinical case. A 10-year-old female patient, who consulted for a 1-year clinical picture consisting of pain in the urogenital region, radiating to the hypogastrium, associated with pushing and dysuria, who was diagnosed with urethral prolapse and a surgical correction of the urethral mucosa was performed prolapsed by modified Kelly-Burnham technique. Conclusion. Urethral prolapse is an entity that is frequently misdiagnosed because its diagnosis is eminently clinical. Although medical treatment has been described in the first instance, it has a high recurrence rate, so surgical resection of the prolapsed tissue is preferred in these cases.

It is aimed to review the literature regarding the clinical evaluation of female urinary incontinence in gynecologic consultation. An exploration of the literature in several different electronic databases, books printed texts in indexed journals and recognized scientific societies, to identify relevant literature was trying to find the best evidence in the clinical assessment of urine leakage in gynecological consultation. They are not allowing sufficient scientific documents suggest which is the most specific clinical examination in the assessment of incontinence in gynecologic consultation, but the correlation of findings from each other, facilitates the same. The literature shows that the methods of clinical evaluation of urinary incontinence in gynecologic consultation play an important role in the correct diagnosis of patients and proper treatment thereof; therefore, it is necessary to turn our eyes to the appropriate clinical evaluation in gynecological consultation. OBJETIVO: Avaliar os efeitos da eletroestimulação (ES) do assoalho pélvico na uretra de ratas. MÉTODOS: Quarenta ratas adultas foram distribuídas, randomicamente, em quatro grupos com dez animais cada: Ctrl - sem intervenção; Sham - não foi submetido a ES, recebeu um eletrodo dentro da vagina; Exp6 - submetido a seis sessões de ES do assoalho pélvico; e Exp12 - submetido a 12 sessões de ES do assoalho pélvico. Ao final do experimento, todos os animais foram anestesiados, e o terço médio da uretra foi retirado, fixado em líquido de Bouin e processado para estudo histomorfométrico. Alguns cortes foram corados pela hematoxilina e eosina, para descrição morfológica e morfométrica, e outros, pelo picrosirius red, para avaliação do colágeno total. As espessuras da camada muscular e do epitélio foram obtidas, nos 4 quadrantes da uretra, pela realização de 20 medições em cada animal. O número de vasos sanguíneos presentes na lâmina própria foi obtido nos quatro quadrantes, em uma área de 10³ mm² por quadrante, sendo as imagens obtidas pelo programa de análise de imagens AxioVision® REL 4.3 (Carl Zeiss). A proporção de colágeno e de fibras musculares foi obtida de duas imagens por quadrante, de cada lâmina da uretra corada pelo picrosirius red, com auxílio do programa Imagelab®. Os dados foram submetidos à análise de variância (ANOVA) e ao teste de comparações múltiplas de Tukey-Kramer (pExp6==Ctrl==Sham; *pPURPOSE: To evaluate the effects of electrical stimulation (ES) of the pelvic floor on the urethra of female rats. METHODS: Forty adult rats were divided at random into four groups of ten animals each: Ctrl - without intervention; Sham - not submitted to ES, but with an electrode inserted into the vagina; Exp6 - submitted to six sessions of ES of the pelvic floor, and Exp12 - submitted to 12 sessions of ES of the pelvic floor. At the end of the experiment, all animals were anesthetized and the middle third of the urethra was removed, fixed in Bouin's fluid and processed for histomorphometric study. Sections were stained with hematoxylin and eosin for morphological and morphometric description, and others were stained with picrosirius red for the quantitation of total collagen. The thicknesses of the muscle layer and of the epithelium were determined, in 4 quadrants of the urethra, by performing 20 measurements per animal. The number of blood vessels present in the lamina propria was counted in the four quadrants over an area of 10³ µm² per quadrant and the images were obtained using the image analysis program AxioVision® REL 4.3 (Carl Zeiss). The collagen and muscle fiber ratios in the urethrae were calculated from two images per quadrant of every slice stained with picrosirius red, employing the Imagelab® Program. Resumen Purpose El tratamiento mínimamente invasivo de la estrechez uretral tiene altas tasas de recurrencia y re-operación a largo plazo, no obstante, encuestas realizadas en otros países han demostrado que los urólogos tienen poca experiencia con la uretroplastia abierta y hay una preferencia a la utilización de las terapias endoscópicas mínimamente invasivas. El objetivo de este estudio, es describir patrones de práctica del tratamiento de la estrechez de uretra anterior en nuestro país. Métodos Se trata de un estudio observacional descriptivo y para ello se realizó un cuestionario adaptado a nuestro contexto nacional, basado en varios estudios previamente realizados acerca de la experiencia en Urología reconstructiva. Ese cuestionario incluía información sobre la edad, nivel de experiencia en urología general, la experiencia en urología reconstructiva, escenario de la práctica y las técnicas preferidas para el manejo de las estrecheces uretrales pendulares y bulbares. La información fue almacenada de forma anónima, los datos fueron analizados mediante el paquete estadístico SPSS y se realizó un análisis de distribución de frecuencias. Resultados Se obtuvieron 106 respuestas de los urólogos encuestados. Para el tratamiento de la estrechez uretral pendular la mayoría de los urólogos prefiere el manejo endoscópico mínimamente invasivo, seguido de uretroplastia con injerto con porcentajes de 69,9% y 25,5% respectivamente. Solo el 5% prefiere derivar a sus pacientes a un centro especializado. Para la estrechez de la uretra bulbar se prefiere las técnicas mínimamente invasivas, uretroplastia y remisión a un centro especializado en un 44,3%, 41,5% y 14,2% respectivamente. La población más joven y con formación urológica más reciente tiende a hacer más a menudo la uretroplastia con injerto y menos manejo endoscópico, específicamente la uretrotomía interna. En las ciudades intermedias, hay una predilección por el tratamiento endoscópico, especialmente uretrotomía interna. Conclusiones El enfoque de tratamiento mínimamente invasivo de la estrechez uretral es el más frecuentemente elegido a pesar de sus pobres tasas de éxito a largo plazo. Es de destacar que las nuevas generaciones de urólogos muestran más interés y dominio de las técnicas abiertas, tratamiento estándar hoy en día y con bajas tasas de recaídas y reoperación a largo plazo.

A dog with abdominal trauma had an urethrorectal fistula and secundary bilateral hidronephrosis and hydroureter, identified by urethrocistography and excretory urography. According to our researches, only two cases of traumatic urethrorectal fistula had been reported until now in veterinary medicine.Um cão com trauma abdominal desenvolveu fístula uretroretal, hidronefrose e hidroureter bilaterais secundários, identificados por uretrocistografia e urografia excretora. Na literatura veterinária, há somente dois casos de fístulas uretroretais traumáticas descritos até o momento.

Se presenta un caso de una paciente femenina de la raza blanca, de 34 años de edad, con antecedentes de sepsis urinaria baja a recurrente, acude a la consulta externa del Hospital General Docente Abel Santamaría Cuadrado, sintiendo un ardor y dolor al orinar, disminución en el calibre del chorro de la orina, pujos y tenesmo vesical. Al examen físico se comprueba un tumor que emerge través del meato uretral, vascularizada color rojo violáceo, situada en la uretra anterior. Se realizan los exámenes complementarios e imageneológicos, los cuales se encuentran en los rangos del valor normal. Se realiza un tratamiento quirúrgico (exéresis de la tumoración, electrofulguración) y biopsia, resultando ser un carcinoma transcicional micro invasor con invasión linfática, completamente removido, y evolución clínica satisfactoria actualmente curada.A 34 year-old, Caucasian-female patient presenting a recurrent, low urinary sepsis came to the out-patient clinic at "Abel Santamaria Cuadrado" University Hospital complaining of burning and pain during the passing of urine, a diminution of urine stream and vesical tenesmus. At physical examination a tumor emerging from the urethral meatus was observed, having a red-purplish vascularization located in the anterior urethra. Complementary tests and imaging examinations were performed which ranges showed normal values. Surgical treatment (removal of the tumor and electro- fulguration) and a biopsy were performed which resulted in a micro-invasive transitional carcinoma having a lymphatic invasion completely removed, the patient showed a satisfactory clinical progress, currently cured.
