1. Unified Framework of Forced Magnetic Reconnection and Alfven Resonance

Abstract: A unified linear theory that includes forced reconnection as a particular case of Alfv\'en resonance is presented. We consider a generalized Taylor problem in which a sheared magnetic field is subject to a time-dependent boundary perturbation oscillating at frequency $\omega_0$. By analyzing the asymptotic time response of the system, the theory demonstrates that the Alfv\'en resonance is due to the residues at the resonant poles, in the complex frequency plane, introduced by the boundary perturbation. Alfv\'en resonance transitions towards forced reconnection, described by the constant-psi regime for (normalized) times $t\gg S^{1/3}$, when the forcing frequency of the boundary perturbation is $\omega_0\ll S^{-1/3}$, allowing the coupling of the Alfv\'en resonances across the neutral line with the reconnecting mode, as originally suggested in [1]. Additionally, it is shown that even if forced reconnection develops for finite, albeit small, frequencies, the reconnection rate and reconnected flux are strongly reduced for frequencies $\omega_0\gg S^{-3/5}$.

Abstract: The upgrade of the CMS experiment for the high luminosity operation of the LHC comprises the replacement of the current endcap calorimeter by a high granularity sampling calorimeter (HGCAL). The electromagnetic section of the HGCAL is based on silicon sensors interspersed between lead and copper (or copper tungsten) absorbers. The hadronic section uses layers of stainless steel as an absorbing medium and silicon sensors as an active medium in the regions of high radiation exposure, and scintillator tiles directly readout by silicon photomultipliers in the remaining regions. As part of the development of the detector and its readout electronic components, a section of a silicon-based HGCAL prototype detector along with a section of the CALICE AHCAL prototype was exposed to muons, electrons and charged pions in beam test experiments at the H2 beamline at the CERN SPS in October 2018. The AHCAL uses the same technology as foreseen for the HGCAL but with much finer longitudinal segmentation. The performance of the calorimeters in terms of energy response and resolution, longitudinal and transverse shower profiles is studied using negatively charged pions, and is compared to GEANT4 predictions. Abstract: The Compact Muon Solenoid Collaboration is designing a new high-granularity endcap calorimeter, HGCAL, to be installed later this decade. As part of this development work, a prototype system was built, with an electromagnetic section consisting of 14 double-sided structures, providing 28 sampling layers. Each sampling layer has an hexagonal module, where a multipad large-area silicon sensor is glued between an electronics circuit board and a metal baseplate. The sensor pads of approximately 1 cm$^2$ are wire-bonded to the circuit board and are readout by custom integrated circuits. The prototype was extensively tested with beams at CERN's Super Proton Synchrotron in 2018. Based on the data collected with beams of positrons, with energies ranging from 20 to 300 GeV, measurements of the energy resolution and linearity, the position and angular resolutions, and the shower shapes are presented and compared to a detailed Geant4 simulation.

Abstract: As part of its HL-LHC upgrade program, the CMS Collaboration is developing a High Granularity Calorimeter (CE) to replace the existing endcap calorimeters. The CE is a sampling calorimeter with unprecedented transverse and longitudinal readout for both electromagnetic (CE-E) and hadronic (CE-H) compartments. The calorimeter will be built with $\sim$30,000 hexagonal silicon modules. Prototype modules have been constructed with 6-inch hexagonal silicon sensors with cell areas of 1.1~$cm^2$, and the SKIROC2-CMS readout ASIC. Beam tests of different sampling configurations were conducted with the prototype modules at DESY and CERN in 2017 and 2018. Abstract: The CMS experiment at the CERN LHC will be upgraded to accommodate the 5-fold increase in the instantaneous luminosity expected at the High-Luminosity LHC (HL-LHC). Concomitant with this increase will be an increase in the number of interactions in each bunch crossing and a significant increase in the total ionising dose and fluence. One part of this upgrade is the replacement of the current endcap calorimeters with a high granularity sampling calorimeter equipped with silicon sensors, designed to manage the high collision rates. As part of the development of this calorimeter, a series of beam tests have been conducted with different sampling configurations using prototype segmented silicon detectors. In the most recent of these tests, conducted in late 2018 at the CERN SPS, the performance of a prototype calorimeter equipped with ${\approx}12,000\rm{~channels}$ of silicon sensors was studied with beams of high-energy electrons, pions and muons. This paper describes the custom-built scalable data acquisition system that was built with readily available FPGA mezzanines and low-cost Raspberry PI computers.

Abstract: This paper describes the experience with the calibration, reconstruction and evaluation of the timing capabilities of the CMS HGCAL prototype in the beam tests in 2018. The calibration procedure includes multiple steps and corrections ranging from tens of nanoseconds to a few hundred picoseconds. The timing performance is studied using signals from positron beam particles with energies between 20 GeV and 300 GeV. The performance is studied as a function of particle energy against an external timing reference as well as standalone by comparing the two different halves of the prototype. The timing resolution is found to be 60 ps for single-channel measurements and better than 20 ps for full showers at the highest energies, setting excellent perspectives for the HGCAL calorimeter performance at the HL-LHC.

Abstract: The Compact Muon Solenoid collaboration is designing a new high-granularity endcap calorimeter, HGCAL, to be installed later this decade. As part of this development work, a prototype system was built, with an electromagnetic section consisting of 14 double-sided structures, providing 28 sampling layers. Each sampling layer has an hexagonal module, where a multipad large-area silicon sensor is glued between an electronics circuit board and a metal baseplate. The sensor pads of approximately 1.1 cm2 are wire-bonded to the circuit board and are readout by custom integrated circuits. The prototype was extensively tested with beams at CERN's Super Proton Synchrotron in 2018. Based on the data collected with beams of positrons, with energies ranging from 20 to 300 GeV, measurements of the energy resolution and linearity, the position and angular resolutions, and the shower shapes are presented and compared to a detailed Geant4 simulation.

Abstract: The CMS experiment at the CERN LHC will be upgraded to accommodate the 5-fold increase in the instantaneous luminosity expected at the High-Luminosity LHC (HL-LHC) [1]. Concomitant with this increase will be an increase in the number of interactions in each bunch crossing and a significant increase in the total ionising dose and fluence. One part of this upgrade is the replacement of the current endcap calorimeters with a high granularity sampling calorimeter equipped with silicon sensors, designed to manage the high collision rates [2]. As part of the development of this calorimeter, a series of beam tests have been conducted with different sampling configurations using prototype segmented silicon detectors. In the most recent of these tests, conducted in late 2018 at the CERN SPS, the performance of a prototype calorimeter equipped with ~12,000 channels of silicon sensors was studied with beams of high-energy electrons, pions and muons. This paper describes the custom-built scalable data acquisition system that was built with readily available FPGA mezzanines and low-cost Raspberry Pi computers.

Abstract: As part of its HL-LHC upgrade program, the CMS collaboration is developing a High Granularity Calorimeter (CE) to replace the existing endcap calorimeters. The CE is a sampling calorimeter with unprecedented transverse and longitudinal readout for both electromagnetic (CE-E) and hadronic (CE-H) compartments. The calorimeter will be built with ~30,000 hexagonal silicon modules. Prototype modules have been constructed with 6-inch hexagonal silicon sensors with cell areas of 1.1 cm2, and the SKIROC2-CMS readout ASIC. Beam tests of different sampling configurations were conducted with the prototype modules at DESY and CERN in 2017 and 2018. This paper describes the construction and commissioning of the CE calorimeter prototype, the silicon modules used in the construction, their basic performance, and the methods used for their calibration.

16. Subgroups in chronic low back pain patients - a step toward cluster-based, tailored treatment in inpatient standard care: On the need for precise targeting of treatment for chronic low back pain

Abstract: Objective: The purpose of this study was to find applicable clusters for the development of different treatment pathways in an inpatient multimodal pain-therapy setting based on the multifaceted nature of CLBP.Methods: Based on data of questionnaires (Hospital Anxiety and Depression Scale (HADS), Marburg Questionnaire on Habitual Health Findings (MFHW), quality of life assessment using the Short-Form 12 (SF 12)), a retrospective two-step cluster analysis involving a sample of chronic low back pain (CLBP) patients (N=320) was calculated. Subsequently, the clusters were precisely described and compared on the basis of further data collected during the patients' standard care: pain characteristics, socio-demographic data and the general state of health, psychological variables, therapy intensity, and Diagnosis Related Groups (DRG) data.Results: We found a three-cluster solution: little psychological interference but marginal physical and mental quality of life (Cluster 1); poor well-being, low physical quality of life, and marginal mental quality of life (Cluster 2); and heavy mental strain and marginal physical quality of life (Cluster 3).Conclusions: Similar to previous studies, our results suggest that patients suffering from CLBP differ with regard to the magnitude of mental burden and the presence of physical impairment. These differences ascertain the need for precise targeting of treatment for CLBP. Inpatient pain centers therefore should offer different multimodal therapy pathways and integrate a meaningful triage, taking into account the multifaceted nature of CLBP based on sophisticated knowledge about forms, differences, and relationships among the biopsychosocial components of CLBP., Hintergrund: Die vorliegende Studie soll als Grundlage für die gezielte Entwicklung von Cluster-basierten Behandlungspfaden im stationären Therapiesetting mit multimodaler Schmerztherapie für Patienten mit chronischen Rückenschmerzen dienen.Methoden: Basierend auf Daten aus Fragebögen (Hospital Anxiety and Depression Scale (HADS), Marburger Fragebogen zum habituellen Wohlbefinden (MFHW), Messung der Lebensqualität per Short-Form 12 (SF 12)) haben wir retrospektiv eine Two-step-Clusteranalyse unter Einbezug von N=320 Patienten mit chronischen Schmerzen berechnet. Abstract: Bacillus subtilis harboring the cat-86 constitutive plasmid pPL708C2 with an ochre mutation at the 9th codon (terc 9) was sensitive to chloramphenicol (Cm(s)) and exhibited relative thermostability when heated at 47 degrees C. Reversion to chloramphenicol resistance (Cm(r)) occurred at a frequency of 5.4 x 10(-8). All of the plasmid Cm(r) revertants tested were thermosensitive. Similarly, wild-type pPL708C2 present in B. subtilis also rendered the bacterium thermosensitive. When a nonsense mutation is introduced at codon 141, however, this terc 141 variant of pPL708C2 failed to thermosensitize B. subtilis. Another variant of pPL708C2 that produces intact yet catalytically inactive CAT-86 has both His-16 and His-17 at the active site replaced by Pro. Nevertheless, cells of B. subtilis carrying this variant were thermosensitive. Plasmid-free and pPL708C2-bearing strains did not exhibit differences in major heat shock proteins. Electron micrographs revealed a threefold increase of inclusion bodies present in a strain harboring pPL708C2 when compared with those in an isogenic plasmid-free strain.

22. Efficacy and Effectiveness of Evidence-Based Non-Self-Fitting Presets Compared to Prescription Hearing Aid Fittings and a Personal Sound Amplification Product.

Abstract: Purpose: In this study, we compare the efficacy and effectiveness of gain-frequency responses of evidence-based presets developed by our lab for over-the-counter hearing aids to conventional National Acoustic Laboratories' nonlinear fitting procedure, version 2 (NAL-NL2) gain-frequency response and to gain-frequency response of a personal sound amplification product (PSAP). We hypothesized that the hearing aids with our presets would perform better than a PSAP with poor frequency response and comparably to the hearing aid with NAL-NL2 frequency response., Method: We used a single-blinded, randomized cross-over design to compare audibility, speech recognition, sound quality, listening effort, and subjective preferences in 37 participants in laboratory settings and following field trials., Results: The presets developed in our lab showed comparable outcomes to the hearing aids with NAL-NL2 gain-frequency response in most measured domains. Performance with the presets was better than the PSAP gain-frequency response in the domains of listening effort and sound quality in laboratory testing and speech recognition in our real-world measures. We also found that most participants (54.05%) preferred our presets over the PSAPs and were willing to pay significantly more to purchase the hearing aids with our presets., Conclusion: Our evidence-based presets have better outcomes than a PSAP with a single, poorly suited frequency response while performing comparably to the clinical best-practice National Acoustic Laboratories condition.

23. Toward a New Evidence-Based Fitting Paradigm for Over-the-Counter Hearing Aids.

Abstract: Purpose This dual-aim study was intended to develop and validate a new fitting paradigm for over-the-counter (OTC) hearing aids. Aim 1 was undertaken to create a limited set of evidence-based preconfigured gain-frequency responses ("presets") for use in OTC devices. Aim 2 tested the efficacy of the presets relative to best-practice verification and determined the best method(s) for older adults to self-select amplification from the set. Method In Aim 1, audiometry data from 267 older adults with mild-to-moderate sensorineural hearing loss were obtained from the National Health and Nutrition Examination Survey database. Using these data and the National Acoustic Laboratories-Non-Linear 2 (NAL-NL2) prescriptive formula, a set of four presets were created that can fit a large percentage of older Americans with presbycusis. In Aim 2, 37 older adults with hearing loss used five methods to select presets. The selection methods were select-by-audiogram, select-by-self-test, select-by-trying, select-by-questionnaire, and random assignment. Using a crossover design, each participant completed speech recognition testing and sound quality ratings in quiet and noise for all selection methods and a verified NAL-NL2 condition. Results The set of presets can fit 67.9% of older Americans with mild-to-moderate hearing loss (Aim 1). Controlling for hearing thresholds and sound quality ratings, liner mixed-effects models indicated that speech recognition scores for select-by-audiogram, select-by-self-test, and select-by-trying were not statistically different from the NAL-NL2 condition. Statistical analysis indicated that select-by-self-test produced outcomes most consistent with individual outcomes for the NAL-NL2 condition (Aim 2). Conclusion This newly developed fitting paradigm may provide efficacy comparable to best-practice verification and could be used in OTC hearing aids.

Abstract: Purpose This study investigates common real-ear aided response (REAR) configurations prescribed by the NAL-NL2 algorithm for older adults with hearing loss. Method A data set that is representative of the older adult U.S. population with mild-to-moderate sensorineural hearing loss was constructed from the audiometric data of 934 adults (aged 55-85 years) from the National Health and Nutrition Examination Survey years 1999-2012. Two clustering approaches were implemented to generate common REAR configurations for eight frequencies (0.25, 0.5, 1, 2, 3, 4, 6, and 8 kHz) at three input levels (55, 65, and 75 dB SPL). (a) In the REAR-based clustering approach, the National Health and Nutrition Examination Survey audiograms were first converted to REAR targets and then clustered to generate common REAR configurations. (b) In the audiogram-based clustering approach, the audiograms were first clustered into common hearing loss profiles and then converted to REAR configurations. The trade-off between the number of available REAR configurations and the percentage of the U.S. population whose hearing loss could be fit by at least one of them (i.e., percent coverage) was evaluated. Hearing loss fit was defined as less than ± 5-dB difference between an individual's REAR targets and those of the clustered REAR configuration. Results Percent coverage increases with the number of available REAR configurations, with four configurations resulting in 75% population coverage. Overall, REAR-based clustering yielded 5 percentage points better coverage on average compared to audiogram-based clustering. Conclusions The common REAR configurations can be used for programming the gain frequency responses in preconfigured over-the-counter hearing aids and provide clinically appropriate amplification settings for older adults with mild-to-moderate hearing loss.

Abstract: Objective: The purpose of this study was to find applicable clusters for the development of different treatment pathways in an inpatient multimodal pain-therapy setting based on the multifaceted nature of CLBP. Methods: Based on data of questionnaires (Hospital Anxiety and Depression Scale (HADS), Marburg Questionnaire on Habitual Health Findings (MFHW), quality of life assessment using the Short-Form 12 (SF 12)), a retrospective two-step cluster analysis involving a sample of chronic low back pain (CLBP) patients (N=320) was calculated. Subsequently, the clusters were precisely described and compared on the basis of further data collected during the patients' standard care: pain characteristics, socio-demographic data and the general state of health, psychological variables, therapy intensity, and Diagnosis Related Groups (DRG) data. Results: We found a three-cluster solution: little psychological interference but marginal physical and mental quality of life (Cluster 1); poor well-being, low physical quality of life, and marginal mental quality of life (Cluster 2); and heavy mental strain and marginal physical quality of life (Cluster 3). Conclusions: Similar to previous studies, our results suggest that patients suffering from CLBP differ with regard to the magnitude of mental burden and the presence of physical impairment. These differences ascertain the need for precise targeting of treatment for CLBP. Inpatient pain centers therefore should offer different multimodal therapy pathways and integrate a meaningful triage, taking into account the multifaceted nature of CLBP based on sophisticated knowledge about forms, differences, and relationships among the biopsychosocial components of CLBP., Competing Interests: The authors declare that they have no competing interests. 26. Utilization of physical therapy, speech therapy and occupational therapy by children and adolescents in Germany.Results of the cross-sectional KiGGS Wave 2 study and trends.

Abstract: Allied health services such as physical therapy, speech therapy and occupational therapy contribute to the early treatment of health disorders in children and adolescents and promote a healthy development. This article describes the utilization of these allied health services by children and adolescents in Germany and analyses its association with demographic and social factors. The analyses are based on the second wave of the German Health Interview and Examination Survey for Children and Adolescents (KiGGS Wave 2, 2014-2017) including 15,023 participants. Trends are calculated in comparison with the KiGGS baseline study (2003-2006). Within one year, 9.6% of children and adolescents in Germany use physical therapy, 6.1% speech therapy and 4.0% occupational therapy. Speech therapy and occupational therapy are used more frequently by boys than by girls. The utilization of speech therapy is highest among 3- to 6-year-olds with 15.0%. Occupational therapy (8.3%) is most frequently used by 7- to 10-year-olds and physical therapy (16.9%) by 14- to 17-year-olds. Social differences are evident mainly in the higher utilization of occupational therapy and speech therapy and a lower utilization of physical therapy by socially disadvantaged children and adolescents. 27. European Core Health Indicators - status and perspectives.

Abstract: Background: The European Core Health Indicators (ECHI) are a key source of comparable health information for the European Union (EU) and its Member States (MS). The ECHI shortlist contains 88 indicators which were developed by experts from MS and international organisations. Most indicators are derived from data sources at the EU's statistical office (Eurostat), the World Health Organisation (WHO) and the Organisation for Economic Co-operation and Development (OECD) and are available for most MS. The remaining indicators on the shortlist are at different stages of conceptual and/or methodological development. The indicators have been reviewed in the past against scientific developments, changes in data collections and emerging policy needs, yet not as part of a systematic and sustainable procedure. There is also no regular inventory of problems met by the MS in collecting the necessary data. Work package 4 of the BRIDGE Health project aimed at updating and improving the existing ECHI-indicator knowledge and expertise and at strengthening the scientific base that supports the effective development and use of health indicators for health policy evaluation and prioritization by the EU and its MS. The aim of this paper is to present a first overview of its outcomes and to explore issues concerning the ECHI data availability, content and policy relevance, update process and accessibility to stakeholders, in light of working towards a sustainable future., Methods: Two surveys were conducted within the framework of the BRIDGE Health project to reassess the status of the ECHI shortlist. The first survey focused on data availability in EU MS, candidate countries and European Free Trade Association (EFTA) countries. The second survey evaluated current needs and criteria with respect to content and policy relevance of the ECHI shortlist. Exploring potential new indicator topics was part of both surveys. All evaluations were supported by an advisory network of national and international experts., Results: Of the 36 countries (EU MS, candidate and EFTA countries) contacted for the data availability mapping, 23 countries (63%) participated in the survey. Data availability from preferred data sources varied between chapters. Availability was highest for the chapter on demography and socio-economic situation, followed by the chapter on health status, where data were available for most indicators from more than 90% of the participating countries. Problems experienced by MS relating to the incorporation of ECHI into their health systems were also identified through the survey. Findings from the survey on policy relevance point at the need for strengthening the links with policy (priorities) and for exploring a possible format change of the list to accommodate actionability. It also showed support for embedding ECHI in a sustainable health information structure; this may practically be aided by a web-based single point of access to an information repository., Conclusion: Policy relevance is an essential but not systematically developed criterion for the inclusion of indicators into the ECHI shortlist. Data availability is crucial for the actual implementation of indicators and has considerably increased for ECHI in the last decade. The data availability mapping provides a structured overview of the current status of data availability for implemented indicators. The ECHI shortlist can contribute to the collection of comparable policy-relevant health data in Europe, foster evidence-based public health and contribute to Member States learning from each other. Abstract: Osteopontin (OPN) is a key cytokine involved in T-cell activation in multiple sclerosis (MS). We investigated whether polymorphism of the osteopontin gene affects MS occurrence and clinical course in a Polish population. Disability in 100 MS patients was evaluated using the Expanded Disability Status Scale (EDSS). Genotype and allele frequencies at exons 6 and 7 were examined by PCR. Using appropriate statistical tests, the distribution of variables was tested and means ± SD compared. Genotype distribution and allele frequency differences between patients and control individuals were not statistically significant. No association of OPN with susceptibility to MS was found in the Polish population. The EDSS score was higher in 8090 T/T + 9250 C/C patients than in 8090 C/C + 9250 C/C MS patients (p = 0.0120), and the disability in 8090 C/C + 9250 C/T MS patients was higher than in 8090 C/C + 9250 C/C MS patients (p = 0.0137). Logistic regression analysis revealed age to be an independent factor influencing disability. The polymorphisms of the OPN gene in positions 8090 T/T + 9250 C/C, 8090 C/C + 9250 C/T, and 8090 C/T + 9250 C/T were linked with higher levels of disability in MS patients.

Abstract: A case of Trichosporon beigelii prosthetic valve endocarditis is described. Prosthetic valve endocarditis developed in the patient, a 58-year-old woman with a history of rheumatic heart disease, 10 months after mitral valve surgery. A large left atrial fungus ball was present. Cultures of blood and valvular tissue were positive for T. beigelii. The organism was sensitive to amphotericin B, 5-fluorocytosine, ketoconazole, and miconazole.

31. An automated system for the study of pulsatile hormone secretion in the immature rat.

Abstract: A procedure is described for prolonged sampling of blood from unrestrained immature rats which overcomes some of the problems associated with contemporary techniques. 1 or 2 days before sampling, the animal is fitted with two indwelling catheters, one inserted into the right jugular vein and the other into a femoral vein. On the day of the experiment blood is continuously withdrawn from the jugular vein (30 microliter/min) using a peristaltic pump and is dispensed into sample tubes every 5 min by means of an automatic fraction collector. A second channel of the same pump continuously infuses a blood replacement mixture (at 37 degrees C) into the femoral vein thereby keeping the animal's blood volume constant. In the present study this technique was used to sample blood from both ovariectomized and immature, intact female rats for as long as 5-7 h. The procedure did not appear to disturb the normal locomotion, feeding, drinking and sleeping behavior of the animals. Plasma luteinizing hormone (LH) profiles obtained from the ovariectomized rats showed a normal pulsatile pattern of LH secretion. The frequency and amplitude of the pulses was not affected, even when the animals were bled on 2 consecutive days. Moreover, the procedure permitted accurate detection and characterization of pulsatile LH release in intact, immature rats although the episodes of LH secretion were more variable and much less pronounced. It is believed that this novel sampling technique could prove to be a valuable tool in the study of pulsatile hormone secretion in small laboratory rodents.
