Abstract

The article discusses a case of successful treatment of a patient with refractory anti-myelin-associated glycoprotein antibody neuropathy using autologous hematopoietic stem cell transplantation (AHSCT). The patient initially presented with sensory symptoms and walking difficulties, which worsened over time despite previous treatments. AHSCT led to significant improvements in upper limb function and nerve conduction, while lower limb symptoms remained unchanged. The study highlights the potential of AHSCT as a therapeutic option for patients with this rare disorder, emphasizing the importance of considering treatment before major axonal loss occurs. Abstract

The letter to the editor published in Leukemia & Lymphoma discusses a rare case of HHV8-related Castleman disease presenting with nephrotic syndrome and pauci-immune glomerulonephritis in a 78-year-old Caucasian man. The patient had a history of chronic hepatitis C and presented with various symptoms, including lymphadenopathy, splenomegaly, edema, and purpuric lesions. Extensive diagnostic work-up led to the diagnosis of multicentric HHV-8+ Castleman disease related pauci-immune systemic vasculitis, and treatment with steroids and Rituximab resulted in significant improvement in the patient's condition. The authors emphasize the importance of considering Castleman disease in the diagnostic work-up of ANCA-negative pauci-immune glomerulonephritis, especially in the presence of tumoral syndrome or cytopenia. Abstract

Primary effusion lymphoma (PEL) is a rare B‐cell non‐Hodgkin lymphoma associated with Kaposi Sarcoma‐associated herpesvirus (KSHV/HHV8) infection. Lymphoma cells are coinfected with Epstein‐Barr virus (EBV) in 60−80% of cases. Tools allowing a reliable PEL diagnosis are lacking. This study reports PEL diagnosis in 4 patients using a Flow‐Fluorescence in situ hybridization (FlowFISH) technique that allowed detection of differentially expressed EBV and HHV8 transcripts within the same sample, revealing viral heterogeneity of the disease. Moreover, infected cells exhibited variable expressions of CD19, CD38, CD40, and CD138. Therefore, FlowFISH is a promising tool to diagnose and characterize complex viral lymphoproliferations. En l’absence de recommandation nationale ou internationale spécifique, le groupe Maladies Auto-Immunes et Thérapie Cellulaire (MATHEC) a rédigé des propositions de vaccinations encadrant les procédures de greffe de cellules hématopoïétiques autologues pour maladie auto-immune, incluant des propositions spécifiques au SARS-Cov-2.
