Vascular anomalies are a group of lesions originating from blood vessels and lymphatics with varying histology and clinical behavior. They form the most common congenital anomalies in infants and children. The reconstructive method for the facial region defects varies depending on the size and position of the defect, as well as the patient's age, health status and aesthetic concerns. In this case, the authors present a male patient who underwent a wide resection of his mustache area because of vascular malformation and reconstructed with hair-bearing temporal artery island flap.A 42-year-old male patient presented with swelling and ongoing color change in the cheek and lip area on the left side of the face. His examination revealed a vascular malformation extending from the preauricular region to the left side of the nose, to the upper lip and to the medial canthal region. Superficial temporal artery island flap was designed to reconstruct the upper lip area while full thickness skin graft planned for the defect on nose and cheek.STA island flap has been applied in many areas such as forehead, eyebrow, eyelid, cheek, and nose reconstruction due to its advantages such as providing good color and texture harmony, reliable and constant pedicle, simple and fast dissection, wide rotation arc and low donor area morbidity. Superficial temporal artery island flap should be considered as a favorable reconstruction option in upper and middle face region for moderate and small sized defects.

Lenfanjiyoma Sirkumskriptum yüzeyel lokalize bir lenfanjiom varyantıdır.Karakteristik klinik prezentasyonu kümelenmiş vezikül yada papüloveziküllerşeklindedir. Çoğu lenfanjiyomlar konjenital gelişmesine rağmen erişkin yaşlardada görülebilmektedir.Bu yazıda kliniğimizde tedavi ettiğimiz sağ aksillerbölgede ve meme başında nüks etmiş bir lenfanjioma sirkumskriptum olgususunulmuştur. Lenfanjiyomasirkumskriptum hastalığının tekrarlama ve ikincil malignite gelişebilmesinedeniyle doğru tanı ve tedavi çok önemlidir. Çeşitli tedavi seçenekleriolmasına karşın özellikle rekürren vakalar için cerrahi eksizyon altın standartolup hastaların takibe alınması elzemdir., Lymphangioma Circumscriptum is a superficially localized variant oflymphangioma. The characteristic clinical presentation is a grouping ofvesicles or papulovesicles. Though most lymphangiomas develop congenitally, thelymphangioma circumscriptum subtype is known to present also in adults.Wereport a case of recurrent lymphangioma circumscriptum on the right axillaryand periareolar area treated in our clinics. Accurate diagnosis and treatment is crucial in cases of lymphangiomacircumscriptive because of thepossibility of recurrence of this disease and the possibility of developingmalignancy secondarily. Besides the various treatment modalities, surgical excision is the goldstandard especially for recurrent cases and the patients need to befollowed-up.

Amaç: Hidradenitis süpürativa apokrin terbezlerinde yerleşen ağrılı, akıntılı apse oluşumları, sinüs, fistül ve skarlarile çevre dokulara yayılan tekrarlayıcı bir hastalıktır. Etyolojisi kesin olarak bilinmemekle birliktegenetik yatkınlık, hormonal faktörler, obezite ve sigara kullanımı suçlananfaktörlerdir. Erken dönem lezyonlara medikal tedavi uygulanırken, kronik,komplike olgularda tedavi cerrahidir. Bununla birlikte nüksler görülebilir vereeksizyon gerekebilir. Bu çalışmada farklı anatomik bölgelerde bulunan veklinik olarak farklı güvenlik sınırıyla eksizyon uygulanmış hidradenitis süpürativaolgularındaki nüks ve diğer komplikasyon oranlarının araştırılmasıamaçlanmıştır.GereçVe Yöntemler: Bu çalışmada Ocak 2000- Mart 2017 tarihleri arasında plastikcerrahi polikliniğine müracat eden, medikal tedaviye dirençli, kronik,rekürrens gösteren ve cerrahi tedavi uygulanan 45 hasta lezyonların yerleşimyeri, uygulanan cerrahi yöntemler ve komplikasyonlar açısından retrospesifikolarak değerlendirildi. Hastaların 25’i erkek, 20’si kadın ve yaş ortalaması 36,7idi (15–78). Bulgular: Hidradenitis süpürativalı hastaları tedaviye yönlendiren temel sebep yaşamkalitesinin bozulması ve psikolojik sorunlara kadar uzanan sağlık problemlerinesebep olmasıdır. Hidradenitis süpürativa’da tedavi hastalığın bulguları vederecesine göre belirlenir. Erken evre hastalara genellikle medikal tedavileruygulanırken ilerlemiş vakalarda geniş cerrahi eksizyon gerekmektedir. Sonuç:İntermammarial bölgede lezyonu olan 2 hastada ve perianal yerleşimli lezyonuolan 3 hastada lezyon sınırında yapılaneksizyonlar sonrası nüks görüldü. Toplamda hastalarda nüks oranı %11,1 olaraktespit edildi ve bu hastalara daha geniş cerrahi sınırla reeksizyon yapıldı. Buçalışmanın amacı hidradenitis süpürativa tedavisinde geniş ve derin cerrahieksizyonu önemini vurgulamaktır., Objective: Hidradenitis suppurativa is a recurrent disease that spreads to the surrounding tissues with painful, smooth abscess formation, sinus, fistula and scarring located in the apocrine sweat glands. Although etiology is not known precisely, genetic predisposition, hormonal factors, obesity and smoking are the accused factors. While medical treatment is applied to early lesions, it is the treatment surgeon in chronic, complicated cases. However, recurrences may occur and re-escision may be required. In this study, we aimed to investigate the recurrence and other complication rates of hydradenitis suppurativa in different anatomical regions with excision with clinically different safety margin.Materials and Methods: In this study, 45 patients who were admitted to plastic surgery clinic between January 2000 and March 2017 with medical treatment resistant, chronic, recurrence and surgical treatment were evaluated retrospectively in terms of resection site, applied surgical methods and complications. Twenty-five of the patients were male, 20 were female and the mean age was 36,7 (15-78).Results: The main reason why patients with Hidradenitis suppuratum leads to treatment is the deterioration of the quality of life and health problems leading to psychological problems. In hydradenitis suppurativa, treatment is based on the findings and the degree of the disease. Early stage disease is usually treated with medical treatments and extensive surgical excision is needed in advanced cases.Conclusion: In 2 patients with intermammary lesions and 3 patients with perianal lesions, recurrences were observed after excision on the lesion border. In total, the recurrence rate was found to be 11.1% in patients and this disease was re-esced with wider surgical margin. The aim of this study is to emphasize the importance of extensive and profound surgical excision in the treatment of hydradenitis suppurativa.

Yanık; geçmişten bugüne insanlığın en büyük problemlerinden biri olmuştur. Yanık tedavisi zor, yüksek maliyetli ve hastalar için ağrılıdır. Yanık ayrıca kalıcı kötü kozmetik izlere yol açmakta ve ciddi iş gücü kaybına neden olmaktadır. Yanık patofizyolojisindeki gelişmelere bağlı olarak yanık ciltte oluşan yanık zonları tarif edilmiş ve tedaviler buna göre şekillenmeye başlamıştır. Bu zonlar; koagulasyon, staz ve hiperemi zonudur. Hiperemi zonu normalde kendiliğinden iyileşebilen zondur. Koagulasyon zonunun tedavisi debridman ve oluşan defektin deri grefti ile onarımıdır. İlk 24-48 saatte yanık tedavisi iyi yönetilirse staz zonu kurtarılabilir; aksi takdirde nekroze olur. İlerleyen iskemi, inflamasyon ve apoptozisin staz zonunda nekroza yol açtığı iyi bilinmektedir. Büyüme faktörleri ile fibroplazi, epitelizasyon ve neovaskülarizasyonu arttıran; antiinflamatuar özellikler gösteren plateletten zengin plazma (PRP) bu çalışmada kurtarıcı ajan olarak kullanılmıştır. Bu çalışmada; 20 adet Yeni Zelanda türü dişi tavşan kullanıldı. Her bir grupta on tavşan olacak şekilde iki gruba ayrılan tavşanların sağ kulaklarına kaynar suda 15 dk bekletilerek ısıtılmış pirinç plaklar 20 sn süre dokundurularak yanık oluşturuldu. Deney grubundaki tavşanların sağ kulaklarında staz zonlarına tek doz PRP enjekte edilirken; diğer gruptaki tavşanlara aynı miktarda serum fizyolojik enjekte edildi. Yanık sonrası 72.saatte sintigrafik değerlendirmeyi takiben tavşanların sağ kulakları immünohistokimyasal inceleme için eksize edildi. Histopatolojik olarak reepitelizasyon, nötrofil infiltrasyonu, fibroblast proliferasyonu, anjiogenez, makrofaj dansitesi, apoptozis oranlarının ölçümü ve Image J programı ile staz zonu alan ölçümleri yapıldı. Histopatolojik sonuçlar ve fotoanaliz sonuçları Mann Whitney U ve student-T testleri ile değerlendirildi. Deney grubunda staz zonu alan ölçümleri, reepitelizasyon, nötrofil infiltrasyonu, fibroblast proliferasyonu ve apoptozis oranları yönlerinden anlamlı bulundu. Akut yanıkta PRP uygulaması staz zonunda yüksek anti-apoptotik ve anti-inflamatuar etki göstermiştir. Staz zonunun kurtarılmasında PRP epitelyal dokuda ve bağ dokusunda yüksek rejenerasyon sağlamıştır. Sonuç olarak; PRP tedavisinin apoptozisi ve inflamasyonun yıkıcı etkilerini azaltarak staz zonunun kurtarılmasında anlamlı etkinlik göstermiştir. PRP ileri çalışmalarla değerlendirildikten sonra klinik kullanımda yer alacaktır. Burn has been one of the most important problems of mankind since early ages. Burn treatment is complicated, expensive, and painful for patients. Burn also results in cosmetically undesirable scars and serious lost time injury. According to the development on understanding pathophysiology of burn, zones are described. Based on these zones, burn treatment is configured. These zones are coagulation, stasis and hyperemia zones. The hyperemia zone can heal without any treatment. The coagulation zone should be debrided and grafted over the skin defect. The appropriate management of the stasis zone in the first 24-48 hours results in full recovery. If this can not be achieved, necrosis can occur as a result of prolonged ischemia, inflammation and apoptosis. In this study platelet rich plasma (PRP) is used as a savior agent which reduces inflammation and enhances fibroplasia, epithelization and angiogenesis in the presence of growth factors. 20 New Zealand type female rabbits were used. They were divided into two groups, each consisting of ten rabbits. The brass comb, which had gained adequate heat in boiling water for 15 minutes, pressed onto each rabbit's right auricula for 20 seconds to burn them in a pattern. Single dose of autogenous PRP was injected to stasis zone of experimental group while saline at the same amount was injected to sham group. After 72 hours of burn trauma; scintigraphic examination was carried out. Subsequently, rabbits' burnt ears were excised for immunohistochemistry assay. Histopathologic examination of reepithelization, neutrophil infiltration, fibroblast proliferation, angiogenesis and macrophage density were performed and apoptotic ratios were measured. Stasis zone area was also calculated by using the programme Image J for photoanalysis. The results were evaluated by Mann Whitney U and student-T tests. In the experimental group the vital areas of salvaged stasis zone, reepithelization, neutrophil infiltration, fibroblast proliferation and apoptotic ratios were statistically significant compared to sham group. Application of PRP on stasis zone has shown advanced anti-apoptotic and anti-inflammatory effects in acute burn. In salvage of stasis zone, PRP had maintained considerable epithelial and connective tissue regeneration. In conclusion, PRP treatment is significantly effective in salvage of stasis zone by preventing the destructive effects of inflammation and decreasing apoptosis.
