Abstract The recently detected Omicron BA.2.86 lineage contains more than 30 amino acid mutations relative to BA.2. BA.2.86 and its JN.1 derivative evade neutralization by serum antibodies of fully vaccinated individuals. In this study, we elucidate epitopes driving the immune escape of BA.2.86 and JN.1 via pseudovirus neutralization. Here we generate 33 BA.2.86 mutants, each reverting a single mutation back to BA.2. We use this library in an approach that we call reverse mutational scanning to define distinct neutralization titers against each epitope. Mutations within the receptor binding domain at K356T, V483Δ, and to a lesser extent N460K, A484K, and F486P enhance immune escape. Interestingly, 16insMPLF within the spike N-terminal domain and P621S within S1/S2 also significantly contribute to antibody escape of BA.2.86. Upon XBB.1.5 booster vaccination, neutralization titers against JN.1 and BA.2.86 improve considerably, and residual immune escape is driven by 16insMPLF, N460K, E554K, and to a lesser extent P621S, and A484K.

Background: The POCSO (Protection Of Children from Sexual Offences) Act was introduced in 2012 as a measure to safeguard and prevent interests of children affected by sexual crimes. Both medical and dental professionals are capable of identifying such cases. Aim: The aim of this study was to evaluate the knowledge and awareness of child sexual abuse related to POCSO Act among both medical and dental professionals in India. Materials and Methods: This cross-sectional questionnaire-based study was conducted on 100 medical and dental healthcare professionals employed in a single hospital for evaluation of knowledge and awareness on POCSO Act. The questionnaire containing multiple choice questions was randomly distributed among all the study participants as hard copy or via electronic generated forms. The questionnaire comprised questions related to demographics and those dealing with knowledge and attitude. Statistical Analysis: Categorical variables were represented in the form of percentages, and data analysis was done using SPSS software. Results: 67% study participants demonstrated full knowledge of POCSO, 23% had some knowledge, and 10% study subjects had absolutely no knowledge regarding this act. 7% professionals opined that the support for such affected children must come from Government Organizations, while 15% believed that Non-Governmental Organizations must support these children. 28% professionals believed that the child support and welfare committee should play a role in supporting these children. However, 50% study participants were of the opinion that the primary support must be community-based. On analyzing statistically, level of knowledge and awareness were found to be statistically significant (P = 0.05). Conclusion: There appears to be a good amount of knowledge and attitude among both medical and dental professionals regarding POCSO Act.

Abstract Phloem is the primary conduit for transporting photosynthates and signaling molecules in plants, facilitating communication between various plant organs. As an ancient vascular tissue, phloem transports sugars, proteins, and hormones from source tissues to sinks over long distances. However, this vital transport system also serves as a battlefield where plants and pathogens compete for survival. The phloem’s nutrient-rich environment offers pathogens a secure habitat, protecting them from external threats while providing ample metabolic resources. Phloem-feeding insects, bacteria, fungi, and viruses exploit this system to access nutrients, leading to widespread diseases and yield losses. These insects can also transmit pathogens, such as viruses, which can evade the plants’ defense systems, causing systemic damage throughout the transport network. This review describes the mechanisms by which pathogens invade and colonize the phloem, the plant’s defense strategies, and their dynamic interactions. Understanding the phloem’s structural intricacies, physiological functions, and defense mechanisms provides a foundation for comprehending phloem–pathogen interactions. Insights into these interactions at the molecular level are crucial for developing innovative and effective disease management strategies. Genomics, proteomics, and bioinformatics advances have elucidated the interactions between phloem defenses and pathogen offenses. Finally, this review discusses integrated disease management strategies to counteract these pathogens, paving the way for improving plant health and resilience.

Primary peritoneal serous carcinoma (PPSC) is an extremely rare tumor mostly idiopathic in nature arising from the peritoneal layers of the abdominal cavity. We present here a unique case of a 72-year-old female with a history of abdominal swelling and difficulty in micturition and defecation for 3 months. Diagnostic imaging and histopathological examinations led to the diagnosis of PPSC. Simultaneously, the patient also has a space-occupying lesion involving the left ovary which on histopathological examination was found to be mucinous cystadenoma. We present this case as PPSC with synchronous mucinous cystadenoma of the left ovary, an extremely rare phenomenon. This combination has not been reported in the literature yet to the best of our knowledge.

Background: Papillary thyroid carcinoma (PTC) is the most widespread endocrine malignancy. Among the several risk factors, Hashimoto thyroiditis (HT) is one of the factors associated with increased incidence of PTC. Both HT and PTC share several epidemiologic features including high prevalence, female predominance, and increased incidence in iodine-sufficient areas. Aims and Objectives: The aim of the study was to assess the incidence of HT and PTC coexistent in endemic population. The study also aimed to determine the association of various prognostic parameters of PTC in patients with coexistent HT. Materials and Methods: This was a retrospective study done from May 2019 to May 2024 over a period of 5 years in a tertiary care hospital of West Bengal. A total of 54 patients with primary PTC who underwent total thyroidectomy with cervical lymph node dissection (LND) were studied. Patients with primary PTC with neck LND and complete medical history were included in this study. Patients with previous thyroidectomy surgery or patients with incidental PTC diagnosed in thyroidectomy specimens done for a benign neoplasm (without LND) were excluded from the study. Results: A total of 54 cases of PTC were studied out of which 23 cases were PTC associated with HT and 31 cases were PTC without HT constituting 42.59% and 57.4%, respectively. PTC associated with HT had a greater incidence in females. In this study, our results showed that PTC with HT had a greater tendency of tumor multifocality, greater mean lymph node size, and increase rate of extranodal tumor extension. At the same time, patients of PTC associated with HT had smaller tumor size and low clinical stage. Conclusion: PTC associated with HT had a female preponderance, larger metastatic lymph node, a tendency for multifocality, and extranodal extension. At the same, they also had smaller tumor size and low clinical stage indicating a complex and controversial effect of HT on PTC.

Abstract Aedes aegypti and Aedes albopictus mosquitoes spread major vector-borne viral diseases in tropical and sub-tropical regions of the globe. In this study, we sequenced the genome of Indian Ae. aegypti and Ae. albopictus and mapped to their reference genomes. Comparative genomics were performed between our strain and the reference strains. A total of 14,416,484 single nucleotide polymorphisms (SNPs) and 156,487 insertions and deletions (InDels) were found in Ae. aegypti, and 28,940,433 SNPs and 188,987 InDels in Ae. albopictus. Particular emphasis was given to gene families involved in mosquito digestion, development, and innate immunity, which could be putative candidates for vector control. Serine protease cascades and their inhibitors called serpins, play a central role in these processes. We extracted high-impact variants in genes associated with serine proteases and serpins. This study reports for the first time a high coverage genome sequence data of an Indian Ae. albopictus mosquito. The results from this study will provide insights into Indian Aedes specific polymorphisms and the evolution of immune related genes in mosquitoes, which can serve as a resource for future comparative genomics and those pursuing the development of targeted biopesticides for effective mosquito control strategies.

Background The term “Multiple Primary Malignant Neoplasms (MPMNs)” refers to two or more unrelated primary malignant neoplasms that originate from single or different organs and occur in one patient. MPMNs have been divided into synchronous and metachronous based on time duration after first malignancy. Materials and Methods This was a hospital-based retrospective study conducted at a tertiary cancer institute in Northeast India. Clinicopathological factors of patients with multiple primary malignancies with at least one female genital tract malignancy attending the gynecological oncology outpatient department were observed. Those with ambiguous status of primary malignancy and incomplete treatment of first primary malignancy were excluded from the study. Results A total of 57 patients with MPMN, including one case of triple primary malignancy, were included in the study. 59.18% of cases had metachronous, and 40.81% had synchronous malignancies. The median time to the development of second primary malignancy was 60 months. Among the first diagnosed malignancies, cervix was the most common site (26.5%), followed by endometrium (20.4%) and ovary (14.28%), whereas ovarian malignancy was more commonly diagnosed second malignancy (38.77%), followed by endometrium (14.28%) and cervix (10.2%). In an analysis of synchronous malignancies, the most common genital tract involvement was seen with endometrium and ovary, with a predominance of low-grade endometrioid histology in 75% of cases. Conclusions As the cancer survivor population continues to increase in future, these patients must be comprehensively evaluated on follow-up, and a cognizance of prior treatment taken should be kept. In addition, it is vital that the clinicians keep a lookout for high-risk population in which genetic testing may be beneficial.

Granulomatous response associated with breast cancer and within the draining lymph nodes is an extremely rare phenomenon. In this study, we are presenting a series of 4 cases of invasive ductal carcinoma showing both caseating and noncaseating granulomas within the ipsilateral draining axillary lymph nodes. We also discussed the pathogenesis of granulomatous response and its clinicopathologic significance.

Abstract Objective: This study analyzed the practice of switching intravenous antibiotics to oral dosage form in a tertiary care hospital of Nepal. Design: A prospective observational study was performed among patients admitted to medical/surgical wards in a private tertiary care hospital of Nepal. Methods: Hospitalized adult patients who received IV antibiotics for at least 48 hours and met the eligibility criteria were enrolled in the study. The detailed information on use of antibiotics such as indication, duration, type time of switch etc. were collected and analyzed. Results: Among 335 patients, 282(83.9%) met the eligibility criteria for intravenous (IV) to oral conversion however, only 18.7% of patients were switched to oral regimen. Step-down conversion was the common type of IV to oral switch. Almost all patients were empirically treated with β-lactams antibiotics (n = 327). There was significant association between the antibiotic class and duration of IV therapy (P < 0.001). The length of hospital stays and duration of IV antibiotics therapy was lower in the timely converted group than in the non-converted groups (P < 0.001). The duration of IV antibiotics therapy was strongly correlated with duration of hospital stay (r = 0.743, P < 0.001). Conclusion: The findings revealed a low prevalence of conversion from IV antibiotics to oral, despite a higher percentage of patients meeting the eligibility criteria for conversion. There is a need for the implementation of structured program to review patients on IV antibiotics and promote timely conversion to oral once they meet the conversion criteria.

ABSTRACT Background and Objectives Screening of rural women of Assam by careHPV test for high‐risk HPV (hr‐HPV) DNA and Papanicolaou (PAP) test for abnormal cytology. Method This prospective cross‐sectional study included 480 non‐pregnant women participants aged 20–70 years from Kamrup District, Assam. Two cervical scrap samples were obtained from eligible enrolled women. The Hr‐HPV DNA test by CareHPV was performed with one cervical scrap, and a second cervical scrap sample was used for the Papanicolaou (PAP) test. The statistical analysis was done using RStudio for variables. A p‐value 3 numbers of children birth (p = 0.003). Conclusion We found that the positive hr‐HPV DNA status among rural women in Kamrup, Assam, was significantly associated with alcohol consumption, tobacco chewing, irregular menstruation, urogenital tract infection, and more than three children birth. The abnormal cytology outcome was also substantially associated with tobacco chewing and alcohol consumption.

The widespread use of pesticides and the formation of by-products on the gradual decomposition of these pesticides have led to environmental pollution, which in turn has caused harm to both human and ecosystem health. Pesticides have been found in water bodies worldwide and are a cause of concern. Photocatalytic reactions have received significant attention in the past few decades for the breakdown of pesticides. Different parameters were studied, including the effects of pH, kinetics, dose, and regeneration. The UV–vis spectroscopy results suggest that the g-C3N4/GO/La2O3 nanocomposite is a superior reusable photocatalyst for the degradation of chlorpyrifos (CPF) compared to pure g-C3N4 and GO/ g-C3N4. This is demonstrated by the fact that the g-C3N4/GO/La2O3 nanocomposite outperforms both of these materials. The increased photocatalytic performance may be attributed to a balance between the band gap, morphology, crystalline quality, and surface area, all of which may be slowing down the electron-hole recombination rates. This may be due to the enhanced photocatalytic performance. In addition, the feasible processes were outlined from radical quenching studies, and the results clearly indicate that the presence of more OH radicals plays an essential role in the process of efficient photodegradation using novel g-C3N4/GO/La2O3 nanocomposites.

Purpose: To report a rare case of a pediatric dacryolith masquerading as congenital nasolacrimal duct obstruction (CNLDO). Observations: A two-year-old male child presented with history of intermittent epiphora and discharge since the age of six months. Clinical evaluation demonstrated raised tear meniscus height and delayed fluorescein dye disappearance test in the right eye. Lacrimal irrigation of the right eye under general anesthesia demonstrated 90 % regurgitation (subjectively) of mucoid fluid with a hard stop. Nasal endoscopy examination demonstrated a dacryolith obstructing the opening of the nasolacrimal duct (NLD) in the inferior meatus. The dacryolith was teased out of the NLD and following its removal the lacrimal irrigation was freely patent. At six-months post operative follow up, epiphora resolved and the child was asymptomatic. Conclusions and importance: While cases of canaliculitis is uncommon in pediatric age group, it is rare to find a NLD dacryolith in a toddler. To the best of the authors’ knowledge, there are few prior reports on pediatric NLD dacryolith masquerading as CNLDO in a toddler (1–3 years).

Snakebites profoundly impact the rural population of tropical nations, leading to significant socio-economic repercussions. Polyvalent antivenom (PAV) therapy faces several limitations, including intra-specific variations and poor efficacy against some major toxins and low molecular mass, poorly immunogenic toxins, which contribute to increased mortality and morbidity rates. Innovative strategies for developing novel antivenoms are continuously explored to address these challenges. The present study focuses on designing of 17 epitope-string toxin-specific peptide immunogens from pharmacologically active major and/or poorly immunogenic toxins (snake venom metalloprotease, Kunitz-type serine protease inhibitor, phospholipase A2, three-finger toxin) from the venom of the ‘Big Four’ venomous snakes and Naja kaouthia (NK) in India. These custom peptide antibodies demonstrated robust immuno-reactivity against the venoms ‘Big Four’ and NK. When these antibodies were supplemented with commercial PAV at a defined ratio (formulated polyvalent antivenom or FPAV), it significantly enhanced the neutralization of snake venom enzymes and in vivo neutralization of lethality and pharmacological activities such as haemorrhage, necrosis, pro-coagulant, defibrinogenation, and myotoxicity of ‘Big Four’ and NK venoms compared to PAV in mice. The present study highlights a promising strategy for developing next-generation antivenoms using synthetic peptide-based immunogens, offering a targeted approach to address the limitations of current antivenom therapy.

Introduction: Infection with tuberculosis is a worldwide health concern. Nearly every organ in the body can be affected by Extrapulmonary Tuberculosis (EPTB). A simple diagnosis of Pulmonary Tuberculosis (PTB) can be made using radiography and a positive sputum test. On the other hand, diagnosing EPTB is difficult due to the peculiar presentation of the lesions, which are paucibacillary and exhibit varied sensitivity and specificity in different diagnostic tests across different infected tissues. Aim: To analyse the varied clinical presentations of tuberculosis infection and to assess several diagnostic techniques for detecting pulmonary and extrapulmonary tuberculosis in reference to the Composite Reference Standard (CRS). Materials and Methods: The present cross-sectional study was carried out in the Department of Pathology and Microbiology, PRM Medical College and Hospital (in the tuberculosis endemic zone of Baripada), North Odisha, India, between April 2018 and November 2022. Clinical presentations included lumps, ulcers, inflammation and lesions resembling neoplasia. All age groups of patients suffering from pulmonary and extrapulmonary tuberculosis were included. A total of 400 tuberculosis cases were detected. Fine needle aspiration was performed on the lesions after considering the patient’s age, gender, lesion location and relevant radiological studies. Biopsy samples were handled in a similar manner. To detect mycobacterial nucleic acid, all 400 cytology cases and biopsy samples were processed using the Cartridge-based Nucleic Acid Amplification Test (CBNAAT) and stained with Ziehl-Neelsen (ZN) stain for Acid-fast Bacilli (AFB) identification. A CRS was defined as the presence of bacilli in CBNAAT and/or AFB along with morphologic evidence such as caseating granulomas. To assess sensitivity and diagnostic effectiveness, CRS was used in conjunction with procedures from cytology, histopathology, AFB detection and CBNAAT. The p-value for McNemar’s test was less than 0.05, indicating statistical significance across all analyses. Results: The analysis revealed a male-to-female ratio of 1:1.02, with more females than males over the age of 20 years affected. Pulmonary tuberculosis cases were 39 (9.75%), while extrapulmonary TB cases were 361 (90.25%). The largest percentage of extrapulmonary tuberculosis cases (n=134, 33.5%) involved lymph nodes, with skeletal involvement following closely behind (n=64, 16%). Mycobacterium tuberculosis adversely affected nearly every organ in the body. The most prevalent pattern in histology was necrotising granulomatous inflammation, observed in 85 (69.6%) patients. Similarly, a type II pattern (epithelioid granuloma with caseous necrosis) was seen in 166 (59.7%) cytosmear cases. The sensitivity and diagnostic accuracy of four tests were assessed using the CRS; favourable results were obtained for FNAC (72.2%), histological examination (100%), ZN staining (20.5%), and CBNAAT (85.6%). With a diagnostic accuracy of 92.8%, CBNAAT outperformed FNAC and histology, which both achieved 86% accuracy. Conclusion: The prevalence of pulmonary and extrapulmonary tuberculosis is extremely high among the people of North Odisha. The present study suggests a combined approach of understanding clinical presentation, conducting pathological investigations, and detecting bacilli to recognise atypical presentations of EPTB at an early stage, given the restricted resources and facilities in peripheral hospitals.

BackgroundFrailty leads to reduced physical activity can cause increased fall risk. This contributes to accelerated aging processes, leading to adverse health outcomes and reduced quality of life. We have developed and piloted the design, usability, safety, and feasibility of a gaming-based cognitive-motor (CogXergaming) tele-exercise protocol in prefrail older adults. ObjectiveThis pilot randomized control trial tested preliminary feasibility and effectiveness of the CogXergaming telehealth protocol for improving physical function. MethodsCommunity-dwelling, prefrail older adults were randomly assigned to CogXergaming (n=13) or a control group (n=14). The CogXergaming group received supervised tele-exercises in a gaming format for 6 weeks (3 sessions per week) comprising 18 sessions lasting 90 minutes each. Control group participants participated in a Matter of Balance (MOB), an 8-week, once-a-week structured 90-minute tele-session that has been shown to reduce the fear of falling and increase physical activity. Feasibility of training was obtained by computing the median duration of training sessions for the CogXergaming group. Effectiveness was assessed using dynamic balance control (Four Square Step Test), subjective self-efficacy (Activities-Specific Balance Confidence scale), gait function (Tinetti Performance Oriented Mobility Assessment), muscle strength (30-second chair stand test), and endurance (2-minute step in-place test). ResultsOf the 45 participants enrolled in the study, 4 participants from CogXergaming group and 5 from MOB group lost contact after signing the consent form and did not receive their respective intervention. Eighteen participants were randomized to each group. In the CogXergaming group, 15 (83%) completed the intervention, with 3 (16%) dropping out in the first week. In the MOB group, 16 (88%) completed the program, with 2 (11%) withdrawing during the first week. In addition, there was a significant time group interaction for Four Square Step Test (F1,21=5.55, P=.03), Tinetti Performance Oriented Mobility Assessment (F1,25=4.16, P=.05), and 30-second chair stand test (F1,21=5.06, P=.03), with a significant improvement in these measures for the CogXergaming group post training, compared with no change observed in the MOB group. ConclusionsThese pilot findings indicate that CogXergaming is feasible and applicable in prefrail older adults. Such game-based protocols can be beneficial in improving physical function among community-dwelling, prefrail older adults, however, the efficacy of such training requires further investigation. Objective: To assess the prevalence and determine the factors associated with Internet Addiction among Indian dental students. Material and Methods: A cross-sectional study was conducted among 250 dental students from different academic years of the Institute of Dental Sciences, Bhubaneswar. Information on demographic characteristics, purpose, and duration of Internet use was recorded. Internet Addiction was assessed using Young’s 20-item Internet Addiction Test (IAT) questionnaire. Chi-square, One-way analysis of variance (ANOVA), and Pearson’s Correlation tests were used for statistical analysis. The level of significance was set at p≤0.05. Results: The prevalence of Internet Addiction was found to be 20.4%. Third-year students showed the highest mean Internet Addiction (46.74 ± 10.26) score. Social networking was the main purpose of Internet use. Bivariate analysis showed that daily hours of Internet use were strongly associated with Internet Addiction, which was statistically significant (p=0.033). The major domains identified in our study towards the contribution of Internet Addiction were Dependence (10.74 ± 2.79) followed by Overuse (10.52 ± 2.58), Emotional state (9.70 ± 3.10), Inhibiting the Flow of Life (8.16 ± 2.74) and Non-restraint (6.37 ± 1.98). Conclusion: Internet Addiction often coexists with other psychological problems. Therefore, early interventions should be implemented, and students should be aware of the adverse effects of spending unnecessary time online.

Objective: To evaluate the antimicrobial activity of soyabean and pumpkin seeds methanolic extract against oral microbiome. Material and Methods: An in vitro study compared the antimicrobial activity of methanolic extract of soyabean and pumpkin seed along with 0.2% chlorhexidine (positive control) by Agar well diffusion method against Streptococcus mutans, Streptococcus pyogenes, Staphylococcus aureus, Lactobacillus and Candida albicans. Suitable dilutions of each seed extract determined Minimum Inhibitory Concentration (MIC) and Minimum Bactericidal Concentration (MBC). Qualitative phytochemical analysis and Thin Layer Chromatography (TLC) was done to determine the phytochemical components. Results: Both the methanolic extracts showed the highest zone of inhibition, 26±0.31 mm and 24±0.82 mm, followed by 22±0.87 mm and 21±0.55 mm against Streptococcus mutans and Streptococcus pyogenes, respectively. The zone of inhibition was higher at concentrations in both extracts, which was not statistically significant (p>0.05). MIC values fluctuated from 1.56 to 4.5 mg/ml and 3.5 to 5.0 mg/ml for soyabean and pumpkin seed extract, respectively. Phytochemical analysis showed the presence of alkaloids, glycosides, terpenoids, tannins, and flavonoids. Conclusion: Both extracts were effective against S. mutans and S. pyogenes and contain compounds with therapeutic potential.

Background: The dynamicity and mobility of the population in a mass gathering setting pose a challenge to traditional disease surveillance methods and strain the local health services. Velankanni is one of the most sacred Christian pilgrimage places located in Nagapattinam, Tamil Nadu, India. We participated in the Velankanni festival to describe the public health preparedness, surveillance, and response activities carried out during the festival. Methods: This was a cross-sectional study. We reviewed the national and international guidelines and published literature and discussed with the key stakeholders. We developed a checklist to observe public health preparedness activities. We facilitated the staff and monitored the activities by the implementers. We established the syndromic surveillance in the designated locations of the event and used tracker software to capture the data. Emergency medical teams were formed with trained health personnel to respond to medical emergencies. Results: The team monitored all the public health activities. There are 59 primary care public health facilities and nine ambulatory Mobile Medical Units, with 160 medical officers available at the site. Of the 16,169 persons who attended the medical camps, 9863 (61%) were males and 8408 (52%) were aged 15–44. Acute diarrheal disease was the most frequent of the reported syndromes, followed by injuries, acute febrile illness, and animal bites. Conclusions: There was no outbreak of any disease either identified or reported. Our findings suggest that risk assessments should be used, and establishing an Incident Command Center is vital for executing command and control mechanisms during mass gatherings.

Context: Reported literatures revealed the problems of access and misconceptions, low contraceptive usage and high unmet need for family planning among the tribes. Aims: Our aim was to find out the prevalence of contraceptive usage, unmet need and their determinants among tribal married reproductive women. Settings and Design: A community-based, observational, analytical research using cross-sectional design was conducted among 290 tribal women of the Nadia district of West Bengal. Subjects and Methods: After obtaining ethical clearance, an interviewer-administered pre-designed, structured proforma was used to collect information on socio-demographic attributes, marital history, contraceptives and fertility status, unmet needs and the reasons for not using contraceptives. Statistical Analysis Used: Bi-variate analysis was used to know the differences and a two-tailed significance test with a P value of 0.05 or less was considered statistically significant. Results: The present study revealed a high prevalence of teenage marriage (60.7%) and pregnancy (27.6%). Every one in six tribal women was illiterate. The contraceptive prevalence rate was 43.8%. Tubectomy (38.6%) was the most common method (38.6%) and unfelt need (19.6%) was the most common reason for non-use. Age, occupation of women, social class, living status, marital duration, number of living children and desire to have a future child were found to be associated with the current contraceptive use. Social class, number of living children, contraceptive awareness, source and place of availability, abortion history, opposition from family and husband, and husband participation all were significantly associated with the unmet need for family planning. Conclusion: In-depth counselling focusing on achieving a satisfactory level of awareness and acceptance of family planning methods may help to achieve reproductive health by reducing unmet needs.

Background: Despite advancements in treatment, patients with Parkinson’s disease (PD) experience a range of symptoms that affect their quality of life. There is a need to integrate neuropalliative care into standard care. The aim of the study is to understand the psychosocial functioning in persons with PD and explore their caregivers’ burden. Methods: The study utilizes a mixed-methods design where 50 patient–caregiver dyads attending the outpatient services of the movement disorder clinic at a tertiary care hospital were measured on psychosocial functioning and caregiver burden and palliative care outcomes for a period of 6 months. Focus group discussions were conducted with 18 patient–caregiver dyads to understand the needs of palliative care. Results: It was found that caregiver burden was positively correlated with palliative care outcomes scores of patients (r = 0.586) and caregivers (r = 0.675) and psychosocial functioning was positively correlated with palliative care outcomes of patients (r = 0.708). The psychosocial functioning score was higher among female patients (indicating worse functioning) than males, and female caregivers experienced significantly higher caregiver burden. The qualitative findings reveal that there is a substantial gap in awareness about palliative care, lack of information, presence of stigmatizing beliefs, and lack of adequate accessibility to palliative facilities. Conclusion: The study lays the foundation for future PD neuropalliative care research, guiding interventions, and exploration of regional variations in PD experiences in India. There is a need to address caregiver burden in PD in India.

This research focuses on the urban expansion occurring in the Kamrup Metropolitan District—an area experiencing significant urbanization—with the aim of understanding its patterns and projecting future growth. The research covers the period from 2000 to 2022 and projects growth up to 2052, providing insights for sustainable urban planning. The study utilizes the maximum likelihood method for land use/land cover (LULC) delineation and the Shannon entropy technique for assessing urban sprawl. Additionally, it integrates the cellular automata (CA)-Markov model and the analytical hierarchy process (AHP) for future projections. The results indicate a considerable shift from non-built-up to built-up areas, with the proportion of built-up areas expected to reach 36.2% by 2032 and 40.54% by 2052. These findings emphasize the importance of strategic urban management and sustainable planning. The study recommends adaptive urban planning strategies and highlights the value of integrating the CA Markov model and AHP for policymakers and urban planners. This can contribute to the discourse on sustainable urban development and informed decision-making.

Objective: Detecting and measuring changes in longitudinal fundus imaging is key to monitoring disease progression in chronic ophthalmic diseases, such as glaucoma and macular degeneration. Clinicians assess changes in disease status by either independently reviewing or manually juxtaposing longitudinally acquired color fundus photos (CFPs). Distinguishing variations in image acquisition due to camera orientation, zoom, and exposure from true disease-related changes can be challenging. This makes manual image evaluation variable and subjective, potentially impacting clinical decision-making. We introduce our deep learning (DL) pipeline, “EyeLiner,” for registering, or aligning, 2-dimensional CFPs. Improved alignment of longitudinal image pairs may compensate for differences that are due to camera orientation while preserving pathological changes. Design: EyeLiner registers a “moving” image to a “fixed” image using a DL-based keypoint matching algorithm. Participants: We evaluate EyeLiner on 3 longitudinal data sets: Fundus Image REgistration (FIRE), sequential images for glaucoma forecast (SIGF), and our internal glaucoma data set from the Colorado Ophthalmology Research Information System (CORIS). Methods: Anatomical keypoints along the retinal blood vessels were detected from the moving and fixed images using a convolutional neural network and subsequently matched using a transformer-based algorithm. Finally, transformation parameters were learned using the corresponding keypoints. Main Outcome Measures: We computed the mean distance (MD) between manually annotated keypoints from the fixed and the registered moving image. For comparison to existing state-of-the-art retinal registration approaches, we used the mean area under the curve (AUC) metric introduced in the FIRE data set study. Results: EyeLiner effectively aligns longitudinal image pairs from FIRE, SIGF, and CORIS, as qualitatively evaluated through registration checkerboards and flicker animations. Quantitative results show that the MD decreased for this model after alignment from 321.32 to 3.74 pixels for FIRE, 9.86 to 2.03 pixels for CORIS, and 25.23 to 5.94 pixels for SIGF. We also obtained an AUC of 0.85, 0.94, and 0.84 on FIRE, CORIS, and SIGF, respectively, beating the current state-of-the-art SuperRetina (AUCFIRE = 0.76, AUCCORIS = 0.83, AUCSIGF = 0.74). Conclusions: Our pipeline demonstrates improved alignment of image pairs in comparison to the current state-of-the-art methods on 3 separate data sets. We envision that this method will enable clinicians to align image pairs and better visualize changes in disease over time. Indian red scorpion Mesobuthus tamulus is responsible for substantial mortality in India and Sri Lanka; however, no specific diagnostic method is available to detect the venom of this scorpion in envenomed plasma or body fluid. Therefore, we have proposed a novel, simple, and rapid method for detecting M. tamulus venom (MTV) in the plasma of envenomed animals using polyclonal antibodies (PAb) raised against three modified custom peptides representing the antigenic epitopes of K+ (Tamapin) and Na+ (α-neurotoxin) channel toxins, the two major MTV toxins identified by proteomic analysis. The optimum PAb formulation containing PAb 1, 2, and 3 in proportion (1:1:1, w/w/w) acted synergistically, demonstrating significantly higher immunological recognition of MTV than anti-scorpion antivenom (developed against native toxins) and individual antibodies against peptide immunogens. The PAb formulation could detect MTV optimally in envenomed rat plasma (intravenous and subcutaneous routes) at 30–60 min post-injection. The acetonitrile precipitation method developed in this study to augment the MTV detection sensitivity enriched the low molecular mass peptide toxins in envenomed rat plasma, which was ascertained by mass spectrometry analysis. The gold nanoparticles conjugated PAb formulation, characterised by biophysical techniques such as Fourier transform infrared spectroscopy (FTIR) and transmission electron microscopy (TEM), demonstrated their interaction with low molecular mass MTV peptide toxins in envenomed rat plasma. This interaction results in the accumulation of the gold nanoparticles, thus leading to signal change in absorbance spectra that can be discerned within 10 min. From a standard curve of MTV spiked plasma, the quantity of MTV in envenomed rat plasma could be determined by gold nanoparticle-PAb formulation conjugate.

Inter-specific hybridization is a key strategy in modern crop improvement, aiming to integrate desirable traits from wild species into cultivated backgrounds. This study delves into the evaluation and identification of advanced inter-specific derivatives (IDs) derived from crosses of cultivated chickpea with Cicer reticulatum and C. echinospermum. The primary aim was to incorporate desirable yield enhancement traits, disease resistance, and nutritional quality traits into cultivated chickpea. The IDs were assessed during rabi 2021-22 and 2022-23 in the northern plains zone of India. Significant amount of genetic variability was observed for key agro-morphological traits having high heritability and genetic advance. Superior derivatives were identified for early flowering, high seed yield, and resistance to Ascochyta blight, Botrytis grey mould, and Fusarium wilt. Significant variability for crude protein and total soluble sugar content was also observed among the derivatives. The findings highlight the potential of utilizing wild Cicer species to broaden the genetic base of cultivated chickpea for the development of robust, high-yielding, disease-resistant varieties with improved nutritional traits suitable for diverse environmental conditions. The superior derivatives identified in this study hold promise for future breeding programmes for improving productivity, disease resistance and nutritional quality.

Secure communication in the digital age is necessary; securing audio data becomes very critical since this is normally transmitted across susceptible networks. Traditional approaches to encryption, like Advanced Encryption Standard and Rivest–Shamir–Adleman, are pretty solid but mostly too computationally intensive for real-time audio applications. This paper presents a new audio encryption scheme using chaotic systems, characterized by high sensitivity to initial conditions, pseudo-randomness, and determinism. These properties make chaotic systems ideal for generating keys for cryptographic purposes. Indeed, complex keys that would be nearly impossible to reverse-engineer without exact initial parameters can be obtained with them. This methodology first digitizes the audio signal and subsequently encrypts each sample according to chaotic sequences from multiple systems, like 2D Memristive Hyperchaotic Maps and 3D Hyperchaotic Quadratic Maps. The encryption processes enormously increase the randomness of the encrypted audio and blind the original data, demonstrating the strength of the chaotic systems in securing audio information.Simulation of this encryption approach confirms its effectuality in providing strong security for audio data while maintaining efficiency, thus being suitable for real-time applications. The developed encryption schemes show high resistance to differential attacks, while the quality of the encrypted audio remains acceptable after transformation. This result can demonstrate chaotic systems’ potential to provide a secure and efficient solution for audio communications, which has broad applications in telecommunications, military communications, and confidential conferencing systems. This work, described in the paper, thus satisfies the growing demand for robust security in digital communications and opens up avenues of future research toward chaotic parameter optimization and an extension to other forms of data, enhancing the security features in all types of digital communication channels.

Key Clinical Message Patients diagnosed with cancer post‐treatment are prone to have recurrent disease. Regular follow‐up of these patients enables early recognition and treatment. A tissue diagnosis before starting treatment is imperative to avoid misdiagnosis and management. Given their immunosuppression, maintaining good nutrition, body hygiene, and clean surroundings is essential to prevent the most common urinary tract infection to rare urinary myiasis infection. Cochliomyia hominivorax, Chrysomya bezziana, and Wohlfahrtia magnifica are the most prevalent flies causing myiasis in human beings. Risk factors for urinary myiasis are open or untreated wounds and debris around the genital area. Specific flies, ticks, and mosquitoes are responsible for myiasis in tropical or subtropical countries, when it is usually not transmitted from human to human. Flies lay their eggs near the urethra, and the larvae hatch and migrate to the bladder. Increased urinary frequency, haematuria, and lower abdominal pain are the most commonly found presenting symptoms. Simple preventive measures can avoid these complications. So, prevention is better than cure.

Abstract Leafy vegetables comprises of the majority of food intake around the world. A nuanced understanding of their phytonutrients, bioavailability and post-harvest processing will aid in understanding their utility in human health better. Plant foods have a variety of dietary phytonutrients beneficial to us. With a lot of diversity in the variety of these leafy vegetables, it is of utmost importance as consumers to understand their benefits, functional properties, post processing changes that occurs until it reaches us. Some of the most popular green leafy vegetables include spinach, cabbage, lettuce and mustard greens. In this review, we provide a summary of the phytonutrients in such leafy greens with a detailed description of its bioavailability of nutrients, role of bio fortification, changes during harvest and post-harvest processing. As a low calorie food item, green leafy vegetables are ideal candidates to add valuable nutrients into our daily diets, and spinach especially is known to have multiple therapeutic implications in human health. Post-harvest processing may include addition of nutrients, increasing bio availability of important constituents, assessing effect of fertilizers and growth promoting factors on their nutrient content. All of these parameters need to be studied in depth to improve their beneficial effect in human nutrition and diet. Graphical Abstract

Aim: (1) To compare the temperature rise in the pulp chamber with different resin materials used for making provisional fixed partial dentures in anterior and posterior region while using Polyvinylsiloxane impression materials as matrix. (2) To identify a superior provisionalization material based on the amount of heat dissipated suitable for anterior and posterior provisional fixed partial denture fabrication. Settings and Design: Temporary crowns and bridges are integral to Fixed Prosthodontics. It has been observed that conventional fixed prosthesis temporisation materials release heat due to the exothermic polymerisation reaction. When such a provisional material is directly let to set on a vital tooth, the heat transfer causes irreversible changes in the pulp tissue depending of the degree of change. Hence, this study observes amount of heat generation in various materials during temporisation procedure, by simulating similar conditions. Materials and Methods: Two Models were fabricated, one simulating missing lateral incisor (Model A) and another simulating missing first molar (Model B). Intact maxillary central incisors and canine for Model A and intact mandibular Second Premolar and Second Molar were selected to act as abutments. These abutment teeth were fitted with the tip of a K-type Thermocouple inside their pulp chambers and these were connected to a digital thermometer. Five temporisation materials were chosen for fabrication of temporary crowns through Direct technique. (1) polymethy methacrylate (Self Cure acrylic), (2) bisacryl composite (Protemp 4), (3) visible light cure urethane dimethacrylate (Revotec LC), (4) barium glass and fumed silica infused methacrylate (Dentsply Integrity) and (5)nano-hybrid composite (VOCO Structur 3). Ten observations were made for each provisional material on each model. During each observation, temperature rise was recorded at 30s interval from the time of application, through the peak and till a decrease in temperature is observed. Polyvinyl siloxane was used as matrix for all except light cure resin, where polypropylene sheet was used. Statistical Analysis Used: Anova test used for statistical. Results: ANOVA test revealed that there was a significant difference in the temperature changes associated with the provisional restorative materials used. Among the five, polymethy methacrylate (self cure resin) showed the maximum rise in temperature, followed by bisacryl composite (Protemp 4), visible light cure urethane dimethacrylate (Revotec LC), barium glass and fumed silica infused methacrylate (Dentsply Integrity) and nano-hybrid composite (VOCO Structur 3). There was no comparable difference between Model A and B but an overall reduction of temperature rise was observed in model B. Conclusion: VOCO Structur 3 showed the least temperature rise in the pulp chamber, and overall temperature rise was less for model B which can be attributed to the residual dentin thickness.

Introduction: Kidney diseases have shown a rising upward trend in the last few decades due to the increase in incidence of associated chronic diseases like diabetes, hypertension. However the frequency and spectrum of renal pathology in autopsy specimens is not well documented or overlooked by physicians as well as pathologists. Aim: To analyse the spectrum of various types of renal pathologies, based on the histopathological analysis of renal tissue where autopsies were performed routinely with no history of renal diseases. Materials and Methods: This cross-sectional study was carried out over a period of two years from June 2021 to June 2023 in the Department of Pathology of SCB Medical College and Hospital, Cuttack, Odisha, India, and consisted of 84 cases of well-preserved renal autopsies. The autopsy samples were routinely processed and stained by Haematoxylin and Eosin (H&E) and then reported. Special stain of Periodic Acid Schiff (PAS) was done. Gross and microscopic findings were studied. The study was compared with other relevant studies. The data was analysed using Microsoft Excel 2019. Results: A total of 84 cases were included in the study. The male:female ratio was 1.27:1. The age group with the highest number of cases (35 cases) was 21-40 years. Among the 84 cases the commonest pathology was seen in tubules (40 cases, 47.62%) and glomeruli (7 cases, 8.33%), followed by vascular pathologies (5 cases, 5.95%) and interstitial lesions (4 cases, 4.76%). Renal cell carcinoma was incidentally detected in two cases and Wilms tumour in one case. Five cases had normal histology. Conclusion: The study illuminates the various renal lesions on autopsy and showed tubular lesions were more common than glomerular lesions in renal autopsy. Renal lesions are more common in males. The focus should be to develop more effective diagnostic methods for renal lesions so that intervention can be fast and sharp resulting in less mortality.

Abstract Background Meniscal root tears can lead to early knee osteoarthritis and pain. This study aimed (1) to compare clinical and radiological outcomes between patients who underwent arthroscopic meniscal root repair after meniscal root tears and those who received non-surgical treatment, and (2) to identify whether baseline MRI findings could be potential predictors for future treatment strategies. Methods Patients with meniscal root tears were identified from our picture archiving and communication system from 2016 to 2020. Two radiologists reviewed radiographs and MRI studies using Kellgren-Lawrence (KL) grading and a modified Whole Organ MRI Scoring (WORMS) at baseline and follow-up. The median (interquartile range [IQR]) of follow-up radiographs and MRI studies were 134 (44–443) days and 502 (260–1176) days, respectively. MR images were assessed for root tear-related findings. Pain scores using visual analogue scale (VAS) and management strategies (non-surgical vs. arthroscopic root repair) were also collected. Chi-squared tests and independent t-tests were used to assess differences regarding clinical and imaging variables between treatment groups. Abstract Background To contribute to the discovery of new microbial strains with metabolic and physiological robustness and develop them into successful chasses, Paracoccus pantotrophus DSM 2944, a Gram-negative bacterium from the phylum Alphaproteobacteria and the family Rhodobacteraceae, was chosen. The strain possesses an innate ability to tolerate high salt concentrations. It utilizes diverse substrates, including cheap and renewable feedstocks, such as C1 and C2 compounds. Also, it can consume short-chain alkanes, predominately found in hydrocarbon-rich environments, making it a potential bioremediation agent. The demonstrated metabolic versatility, coupled with the synthesis of the biodegradable polymer polyhydroxyalkanoate, positions this microbial strain as a noteworthy candidate for advancing the principles of a circular bioeconomy. Results The study aims to follow the chassis roadmap, as depicted by Calero and Nikel, and de Lorenzo, to transform wild-type P. pantotrophus DSM 2944 into a proficient SynBio (Synthetic Biology) chassis. The initial findings highlight the antibiotic resistance profile of this prospective SynBio chassis. Subsequently, the best origin of replication (ori) was identified as RK2. In contrast, the non-replicative ori R6K was selected for the development of a suicide plasmid necessary for genome integration or gene deletion. Moreover, when assessing the most effective method for gene transfer, it was observed that conjugation had superior efficiency compared to electroporation, while transformation by heat shock was ineffective. Robust host fitness was demonstrated by stable plasmid maintenance, while standardized gene expression using an array of synthetic promoters could be shown. pEMG-based scarless gene deletion was successfully adapted, allowing gene deletion and integration. The successful integration of a gene cassette for terephthalic acid degradation is showcased. The resulting strain can grow on both monomers of polyethylene terephthalate (PET), with an increased growth rate achieved through adaptive laboratory evolution. Conclusion The chassis roadmap for the development of P. pantotrophus DSM 2944 into a proficient SynBio chassis was implemented. The presented genetic toolkit allows genome editing and therewith the possibility to exploit Paracoccus for a myriad of applications.

Commonly used adenine-induced chronic kidney disease (CKD) murine models often employ C57BL/6 mice; however, this strain has inherent limitations due to its natural resistance to developing key pathological features of CKD, such as tubulointerstitial fibrosis and inflammation. There have been attempts to overcome these barriers by using multiple concentrations of adenine-supplemented diets or by performing prolonged experiments up to 20 weeks. Here, we demonstrate that SKH1 Elite mice develop clinically relevant CKD phenotypes (e.g., polyuria, proteinuria, inflammation, and renal fibrosis) over the course of only 6 weeks of low-dose (0.15%) adenine supplementation. As a docile, immunocompetent, and hairless strain, SKH1 Elite mice offer several logistical advantages over C57BL/6 mice, including ease of handling and the ability to study dermal conditions, which are often secondary to CKD.

Alzheimer’s disease (AD) is a complex neurodegenerative disorder that is classically defined by the extracellular deposition of senile plaques rich in amyloid-beta (Aβ) protein and the intracellular accumulation of neurofibrillary tangles (NFTs) that are rich in aberrantly modified tau protein. In addition to aggregative and proteostatic abnormalities, neurons affected by AD also frequently possess dysfunctional mitochondria and disrupted mitochondrial maintenance, such as the inability to eliminate damaged mitochondria via mitophagy. Decades have been spent interrogating the etiopathogenesis of AD, and contributions from model organism research have aided in developing a more fundamental understanding of molecular dysfunction caused by Aβ and toxic tau aggregates. The soil nematode C. elegans is a genetic model organism that has been widely used for interrogating neurodegenerative mechanisms including AD. In this review, we discuss the advantages and limitations of the many C. elegans AD models, with a special focus and discussion on how mitochondrial quality control pathways (namely mitophagy) may contribute to AD development. We also summarize evidence on how targeting mitophagy has been therapeutically beneficial in AD. Lastly, we delineate possible mechanisms that can work alone or in concert to ultimately lead to mitophagy impairment in neurons and may contribute to AD etiopathology.

Current vaccines against COVID-19 elicit immune responses that are overall strong but wane rapidly. As a consequence, the necessary booster shots have contributed to vaccine fatigue. Hence, vaccines that would provide lasting protection against COVID-19 are needed, but are still unavailable. Cytomegaloviruses (CMVs) elicit lasting and uniquely strong immune responses. Used as vaccine vectors, they may be attractive tools that obviate the need for boosters. Therefore, we tested the murine CMV (MCMV) as a vaccine vector against COVID-19 in relevant preclinical models of immunization and challenge. We have previously developed a recombinant MCMV vaccine vector expressing the spike protein of the ancestral SARS-CoV-2 (MCMVS). In this study, we show that the MCMVS elicits a robust and lasting protection in young and aged mice. Notably, spike-specific humoral and cellular immunity was not only maintained but also even increased over a period of at least 6 months. During that time, antibody avidity continuously increased and expanded in breadth, resulting in neutralization of genetically distant variants, like Omicron BA.1. A single dose of MCMVS conferred rapid virus clearance upon challenge. Moreover, MCMVS vaccination controlled two variants of concern (VOCs), the Beta (B.1.135) and the Omicron (BA.1) variants. Thus, CMV vectors provide unique advantages over other vaccine technologies, eliciting broadly reactive and long-lasting immune responses against COVID-19.

Small non-coding RNAs (sRNAs) regulate the synthesis of virulence factors and other pathogenic traits, which enables the bacteria to survive and proliferate after host infection. While high-throughput sequencing data have proved useful in identifying sRNAs from the intergenic regions (IGRs) of the genome, it remains a challenge to present a complete genome-wide map of the expression of the sRNAs. Moreover, existing methodologies necessitate multiple dependencies for executing their algorithm and also lack a targeted approach for the de novo sRNA identification. We developed an Isolation Forest algorithm-based method and the tool Prediction Of sRNAs using Isolation Forest for the de novo identification of sRNAs from available bacterial sRNA-seq data ( http://posif.ibab.ac.in/ ). Using this framework, we predicted 1120 sRNAs and 46 small proteins in Mycobacterium tuberculosis . Besides, we highlight the context-dependent expression of novel sRNAs, their probable synthesis, and their potential relevance in stress response mechanisms manifested by M. tuberculosis.

Microplastics (MPs) have emerged as persistent toxicants in the recent decade. MPs are reported to present in different samples such as soil, water, wastewater, and human samples including placenta, urine etc. Recent studies have reported its presence in drinking water. MPs presence in the drinking water is of concern to the research because MPs are associated with several toxicities in animal models including human. The presented review is focused on understanding MPs abundance, sources, detection, analysis, and biotechnological approaches for its degradation. The paper discusses MPs sources, distribution, and transport in drinking water. In addition, it discusses the MPs identification in drinking water, and advances in biotechnological, metagenomics, system, and synthetic biology approaches for MPs degradation. Moreover, it discusses critically the major challenges associated with the MPs degradation in drinking water. Heterogeneity in the MPs size and shape makes it its identification difficult in the drinking water. Most of the methods available for MPs analysis are based on the dried samples analysis. Development of MPs in liquid samples may bring a breakthrough in the research.

The Central Kerala coast in Southwest India serves as a valuable natural archive for studies of the Late Quaternary coastal evolution, palaeoclimate, and maritime trades. The coast is an archeologically and culturally significant region in South India, located in the mouth of the longest river in Kerala, the Periyar River. The riverbank of the Periyar at its mouth is believed to host the ancient port Muziris, whose remnants are found in the areas around where the Periyar River joins the Arabian Sea. Sea level oscillations and climate variabilities in the past have played a pivotal role in the destruction of the Muziris port. Recent excavations unearthed many artifacts from nearby areas of the river confluence; however, the exact location of the port remains unresolved. The present study is an attempt to trace out the coastal evolutionary processes and palaeoclimatic conditions that prevailed in the area during the Late Quaternary period using three drilled cores retrieved from this stretch of the coastal lands. A multi-proxy approach, combining geochemical, mineralogical, palynological, and geochronological tools was used in the study. The Holocene deposit in the study area is composed mainly of sand and clay-dominant sediments that fall within an age range of 1.1–8.1 cal kyr BP, whereas the underlying sedimentary sequence dates to a maximum of 31.5–37.9 cal kyr BP. The inorganic and organic elemental compositions, together with the textural and palynological attributes, indicate that Holocene sedimentation occurred here under fluctuating environmental conditions with significant changes in climate and sea level positions. The upper part of this section bears the signature of the humid depositional environment that prevailed during the Late Holocene when sand-dominated sediments were deposited and brought in under the influence of longshore drift. The sediments from the lower part of the section carry the signature of the Holocene transgression when deposition took place under submerged conditions with considerable marine influence but receiving significant terrestrial inputs. The study highlights the location of the historically significant lost ancient port of Muziris at Kodungallur near the mouth of the Periyar River. A three-fold evolutionary model proposed for the coastal segment revealed that the location of the Muziris port was part of the Periyar River near Kodungallur, as mentioned by the historians, and the settlement near Pattanam may be considered a satellite township of the ancient Muziris port.

Background: Recurrence rates of FIGO stage IB-IIA and IIB-IVA cervical cancer 28–64 respectively. There is a scarcity of data on the recurrence recurrence pattern for unusual sites and theirrecurrence pattern for unusual sites and its association with survival and prognosis. Objective: To study overall survival in patients with distant metastasis compared to local and regional nodal metastasis. Methods: A retrospective study was done from 1/1/2017 to 30/12/22. Cervical cancer patients post primary treatments were included. Survival was analyzed with respect to 3 groups local, regional nodalconducted from 1/1/2017 to 30/12/22. Dermatomyositis (DM) presents with inflammatory myopathy and distinct skin manifestations, often linked to specific autoantibodies. Anti-transcriptional intermediary factor-1 gamma (TIF-1γ) antibodies (Abs) are typically linked to DM in older patients and malignancy in 15% to 40% of cases. We highlight a case of a 24-year-old female who presented with weakness of proximal muscles, periorbital edema, heliotrope rash, erosions on oral mucosa, and painful scaly rash on the lower extremities. Transcriptional intermediary factor-1 gamma Abs were positive, confirming inflammatory myopathy. Treatment with steroid pulse therapy and immunoglobulin led to improvement. Evaluation for malignancy yielded unremarkable results. This case underscores the importance of recognizing and managing DM with TIF-1γ Ab positive, even in atypical demographics, and highlights the need for comprehensive malignancy evaluation.

Objective: To assess sleep quality and stress level and to analyze their effect on the academic performance of undergraduate dental students. Material and Methods: A cross-sectional descriptive survey was conducted among 234 undergraduate dental students at Bhubaneswar. Pittsburgh Sleep Quality Index (PSQI), consisting of 18 items rated on a 4-point Likert scale, assessed sleep quality. Kessler Psychological Distress Scale (K10) evaluated the stress level. Google Forms collected demographic details and marks obtained in their previous examination. Chi-square, Mann-Whitney U, Kruskal-Wallis tests, and Spearman correlation coefficient were used for analysis (p≤0.05). Results: Mean PSQI and K10 scores were 4.87±4.5 and 17.60±8.5, respectively. 38% of students had poor sleep quality, and mean actual sleep hours were 8.01±1.7. One-fourth (24.8%) of subjects had moderate to severe stress. Sleep quality significantly differed among the various academic years (p=0.001). A post-hoc test showed a difference between 1st and 2nd-year students and 1st and 4th-year students. A statistically significant difference between genders regarding the components of the K10 scale was observed. A non-significant positive correlation between total marks and K10 score and a negative correlation between marks and global PSQI score was found. Conclusion: Sleep quality and stress harm the academic performance of dental students.

Using first-principles calculations, in this piece of work, authors have investigated the physical properties of Ra2LaNbO6 double perovskite by employing the linearized augmented plane wave (LAPW) method. Structural and electronic properties are determined by using LDA, GGA (WC and PBE), LDA + mBJ, and GGA + mBJ potentials. We have found that Ra2LaNbO6 is an indirect band gap (Eg = 2.4 eV) semiconductor. Its elastic and thermodynamic parameters demonstrate its stability. Its optical study indicates that this material opens the door to its applications in optical devices such as photodetectors, solar cells, superlenses, optical fibers, filters, electromagnetic shielding devices, photovoltaic devices, etc. This material is very good for its practical implementation in thermoelectric devices as both p- and n-type material and extends the interest of experimentalists for further investigations. Thus, Ra2LaNbO6 is found thermodynamically stable and identified as a potential candidate for photovoltaic and thermoelectric devices.

Background subtraction is a crucial stage in many visual surveillance systems. The prime objective of any such system is to detect local changes, and the system could be utilized to face many real-life challenges. Most of the existing methods have addressed the problems of moderate and fast-moving object detection. However, very few literature have addressed the issues of slow moving object detection and these methods need further improvement to enhance the efficacy of detection. Hence, within this article, our significant endeavor involved identifying moving objects in challenging videos through an encoder-decoder architectural design, incorporating an enhanced VGG-19 model alongside a feature pooling framework. The proposed algorithm has various folds of novelties: a pre-trained VGG-19 architecture is modified and is used as an encoder with a transfer learning mechanism. The proposed model learns the weights of the improved VGG-19 model by a transfer-learning mechanism which enhances the model’s efficacy. The proposed encoder is designed using a smaller number of layers to extract crucial fine and coarse scale features necessary for detecting the moving objects. The feature pooling framework (FPF) employed is a hybridization of a max-pooling layer, a convolutional layer, and multiple convolutional layers with distinct sampling rates to retain the multi-scale and multi-dimensional features at different scales. The decoder network consists of stacked convolution layers projecting from feature to image space effectively. The developed technique’s efficacy is demonstrated against thirty-six state-of-the-art (SOTA) methods. The outcomes acquired by the developed technique are corroborated using subjective as well as objective analysis, which shows superior performance against other SOTA techniques. Additionally, the proposed model demonstrates enhanced accuracy when applied to unseen configurations. Further, the proposed technique (MOD-CVS) attained adequate efficiency for slow, moderate, and fast-moving objects simultaneously.

Background: The pandemic of COVID-19 has created an unprecedented burden on the healthcare system and medical communities resulting in new public health challenges. On the other hand, in tropical countries, another concern arises due to the similar spectrum of clinical manifestations between COVID-19 and dengue fever. Thus, the neglected tropical disease ‘Dengue’ writhed with more inattention. This study aims to find the effect of the COVID-19 pandemic on dengue infections in endemic areas of West Bengal, India, and their combined impact on public health. The alterations in circulating dengue serotype and their genomic diversity in different COVID-19 waves were also monitored. Methods: A total of 1782 patients were included in this study. Dengue NS1 ELISA, molecular serotyping, genotyping and their phylogenetic analysis were performed. GISaided analysis of various dengue serotypes and hotspot identification for risk maps of severe dengue in endemic zones were done. The burden of dengue fever and the sustainability of their viral strains with changing meteorological parameters in parallel to COVID-19 waves were analyzed. Results: Co-circulation of all the four dengue serotypes with rapid change in the pattern of prevalent serotype DENV4 (Genotype-I) in the year 2020 and DENV3 (Genotype-III) in 2021 were observed, in parallel to different circulating strains of COVID-19. Spatiotemporal distribution of DENV using Geographic Information System (GIS) applications observed a serotypic shift and hotspot mapping for risk analysis detected Kolkata as a dengue hotspot, which has also reported the maximum number of COVID-19 cases. Conclusion: This study indicates the increased fitness of circulating dengue virus strains with optimal virulence as per changing environmental conditions and the inhabitant’s immunity. The high infectivity rate of both the RNA viruses and considering.the consequences of severe dengue and COVID-19 in the population of the same geographical settings is an alarming risk.

The prevalence and distribution of pathotypes and mating types of Ascochyta rabiei (Pass.) Labr. were evaluated during the winter (October to March) seasons of 2017-18 and 2018-19. Forty-five A. rabiei isolates were obtained from six different states. From the survey results, the state of Punjab recorded the highest disease rating (Rating 9) compared with other chickpea-growing areas. The β-tubulin gene was amplified (330 bp) from all the isolates, and sequence analysis was performed. The phylogenetic tree produced by cluster analysis of β-tubulin gene sequences differentiated the 44 A. rabiei isolates. Using multiplex PCR, mating types were identified. From the mating type (MAT) study of all the A. rabiei isolates, amplification was observed at the 490-bp size, which corresponds to mating type 2 (MAT1-2). The four pathotypes (I, II, III, IV) and six physiological races were delineated based on the differential reaction of the disease. The most widely distributed pathotype was pathotype III (52.27%) among the surveyed regions of North Indian states, except Uttarakhand (where only pathotype II was recorded), followed by the least aggressive pathotype I (22.7%), highly aggressive pathotype IV (15.9%), and aggressive pathotype II (9.1%). Among the six physiological races identified, the most common races were race 5 (25%), followed by race 1 (22.73%), race 6 (18.18%), race 4 (18.18%), race 3 (9.09%), and race 2 (6.82%). Because of the prevalence of a single mating type among the surveyed locations in India, we opine that there may be less diversity in A. rabiei compared with isolates reported from other countries. Abstract The COVID lockdown presented an interesting opportunity to study the anthropogenic emissions from different sectors under relatively cleaner conditions in India. The complex interplays of power production, industry, and transport could be dissected due to the significantly reduced influence of the latter two emission sources. Here, based on measurements of cloud condensation nuclei (CCN) activity and chemical composition of atmospheric aerosols during the lockdown, we report an episodic event resulting from distinct meteorological conditions. This event was marked by rapid growth and high hygroscopicity of new aerosol particles formed in the SO2 plume from a large coal-fired power plant in Southern India. These sulfate-rich particles had high CCN activity and number concentration, indicating high cloud-forming potential. Examining the sensitivity of CCN properties under relatively clean conditions provides important new clues to delineate the contributions of different anthropogenic emission sectors and further to understand their perturbations of past and future climate forcing.

Fluctuations in the number of regulatory molecules and differences in timings of molecular events can generate variation in gene expression among genetically identical cells in the same environmental condition. This variation, termed as expression noise, can create differences in metabolic state and cellular functions, leading to phenotypic heterogeneity. Expression noise and phenotypic heterogeneity have been recognized as important contributors to intra-tumor heterogeneity, and have been associated with cancer growth, progression, and therapy resistance. However, how expression noise changes with cancer progression in actual cancer patients has remained poorly explored. Such an analysis, through identification of genes with increasing expression noise, can provide valuable insights into generation of intra-tumor heterogeneity, and could have important implications for understanding immune-suppression, drug tolerance and therapy resistance. In this work, we performed a genome-wide identification of changes in gene expression noise with cancer progression using single-cell RNA-seq data of lung adenocarcinoma patients at different stages of cancer. We identified 37 genes in epithelial cells that showed an increasing noise trend with cancer progression, many of which were also associated with cancer growth, EMT and therapy resistance. We found that expression of several of these genes was positively associated with expression of mitochondrial genes, suggesting an important role of mitochondria in generation of heterogeneity. In addition, we uncovered substantial differences in sample-specific noise profiles which could have implications for personalized prognosis and treatment.
