Diffusion-weighted magnetic resonance imaging (DMRI) is a potential tool to assess changes in brain connectivity and microstructure resulting from physical and occupational therapy in young children with cerebral palsy. This works was carried out to assess whether DMRI can detect changes after 36 weeks of physical and occupational therapy in the microstructure and connectivity of the brains of children with cerebral palsy and determine whether imaging findings correlate with changes in clinical measures of motor function. Five children underwent anatomical MRI and DMRI and evaluations of motor function skills at baseline and after 36 weeks of intensive or once-weekly physical and occupational Perception-Action Approach therapies. Diffusion tensor imaging and constrained spherical deconvolution methods were used to calculate fractional anisotropy (FA) and fiber orientation distribution functions (fODFs), respectively. The fODFs were used to generate tractograms of the cerebrospinal tract (CST). After 36 weeks of physical and occupational therapy, all children showed increases in motor function. No changes were observed in anatomical MRI before and after therapy but CST tractography did show small differences indicating possible altered microstructure and connectivity in the brain. FA values along the CSTs, however, showed no significant changes. Reliable longitudinal DMRI can be employed in toddler-aged children with CP and DMRI has the potential to monitor neuroplastic changes in white matter microstructure. However, there is a high variability between subjects and clinical improvements were not always correlated with measures of FA along the CST.

Abstract  The aim of this study was to evaluate a non-enhanced ultra-low-dose (ULD) abdominal–pelvic multi-detector row computerized tomography (MDCT) to assess patients with acute abdominal pain who would otherwise undergo three-view abdominal X-ray series. Institutional review board approval was obtained with waiver of informed consent. This study was Health Insurance Portability and Accountability Act-compliant. One hundred and sixty-three patients (mean age, 51 years; range, 19–82 years, M/F = 110:53) who underwent ULD MDCT were included in the study. Two subspecialty radiologists independently reviewed the images for abnormal findings and image quality parameters. The effective radiation dose was calculated for each patient and compared to standard-dose computed tomography (CT) scans of 50 matched controls. Findings were confirmed by reviewing the patients’ medical records, and statistical analysis was performed. ULD MDCT showed a high sensitivity (100%), specificity (98.5%), and positive predictive value (91.7%) for detection of free air, stones, and intestinal obstruction. For other sources of abdominal pain, the overall sensitivity, specificity, and positive predictive value were 86%, 96%, and 95%, respectively. Mean effective radiation dose from this study was 2.10 mSv (range of 0.67 to 6.64 mSv) with a 78% mean dose reduction compared to standard-dose CT. There was good inter-observer agreement (=0.4 to 0.81). ULD abdominal–pelvic MDCT provides rapid and reasonably accurate diagnostic information in patients with acute abdominal pain at a very low radiation dose. [ABSTRACT FROM AUTHOR]

Abstract  To assess whether independent evaluation of coronal and sagittal reformatted images can replace axial images for primary interpretation of multidetector row computed tomography (MDCT) images in patients with thoraco-abdominal trauma. 111 (M/F 69:42) patients with acute chest or abdominal trauma underwent 16-channel MDCT. Coronal and sagittal multiplanar reformatted (MPR) images were generated from thin-section axial images. Two radiologists independently interpreted the MPR images first followed by axial images for both imaging findings as well as adequacy of image quality. Differences between independent reader review of axial and MPR images were assessed using the Wilcoxon signed rank test. There was no significant difference in soft tissue findings identified on MPR vs axial images for either reader, p = 0.91 and 0.34, respectively. However, both readers identified more skeletal findings on the MPR as compared to the axial images, p = 0.026 and 0.040, respectively. There was no significant difference between the readers in their interpretation of axial (soft tissue, p = 0.56; skeletal, p = 0.65) or MPR (soft tissue, p = 0.32; skeletal, p = 0.65) findings. More skeletal findings were identified during the isolated review of MPR as compared to axial images alone. However, the use of MPR images alone still resulted in an unacceptably high number of missed soft tissue and even skeletal findings. A combined approach where both data sets are simultaneously available is therefore preferred. [ABSTRACT FROM AUTHOR]
