Condensed Matter - Abstract
The double barrier resonant tunneling diode exhibits complex spatio-temporal patterns including low-dimensional chaos when operated in an active external circuit. We demonstrate how autosynchronization by time--delayed feedback control can be used to select and stabilize specific current density patterns in a noninvasive way. We compare the efficiency of different control schemes involving feedback in either local spatial or global degrees of freedom. The numerically obtained Floquet exponents are explained by analytical results from linear stability analysis., Comment: 10 pages, 16 figures

Purpose: Proton therapy is a limited resource, which is not available to all patients who may benefit from it. We investigate combined proton-photon treatments, in which some fractions are delivered with protons and the remaining fractions with photons, as an approach to maximize the benefit of limited proton therapy resources at a population level. Methods: To quantify differences in normal tissue complication probability (NTCP) between protons and photons, we consider a cohort of 45 head-and-neck cancer patients for which IMRT and IMPT plans were previously created, in combination with NTCP models for xerostomia and dysphagia considered in the Netherlands for proton patient selection. Assuming limited availability of proton slots, we develop methods to optimally assign proton fractions in combined proton-photon treatments to minimize the average NTCP on a population level. Such combined treatments are compared to patient selection strategies in which patients are assigned to single-modality proton or photon treatments. Results: There is a benefit of combined proton-photon treatments over patient selection due to the nonlinearity of NTCP functions, i.e. the initial proton fractions are the most beneficial whereas additional proton fractions have a decreasing benefit when a flatter part of the NTCP curve is reached. This effect was small for the patient cohort and NTCP models considered, but may be larger if dose-response relationships are better known. In addition, when proton slots are limited, patient selection methods face a tradeoff between leaving slots unused and blocking slots for future patients who may have a larger benefit. Combined proton-photon treatments with flexible proton slot assignment provide a method to make optimal use of all available resources. Here, we investigate combined proton-photon treatments as an approach to optimally use the limited PT resources and maximize the benefit of PT at a population level. As an example, we consider a clinic offering both photons and protons and a scenario, in which only limited PT slots are available per day for treating head and neck cancer (HNC) patients. Materials and methods: We assume a fixed number of available proton slots per day and, on average, 2 new HNC patients per week, each receiving 30 fractions over 6 weeks. We designed a slot allocation model that selects, on a daily basis, those patients currently under treatment who benefit the most from a proton treatment on the respective day. The remaining patients on that day receive a photon fraction. The model is based on modern normal tissue complication probability (NTCP) models (e.g. for xerostomia [2]). This daily slot allocation strategy is compared (in terms of average NTCP values) to a threshold-based PT patient selection in which patients are selected for whole PT treatment if a slot is available at start of fractionated treatment and their ΔNTCP exceeds a threshold (5%, 10%, 15%). To simulate many patients, the doses in relevant OARs (e.g. contralateral parotid gland) are sampled from a 2D gaussian distribution (Figure 1) derived from the OAR doses of 45 HNC patients for which IMRT and IMPT plans were previously created [1] and rescaled to a standard of care prescription (1.8 Gy to PTV, 2.3 Gy to GTV). Results: The daily slot allocation strategy leads to a higher reduction of the average NTCP values for xerostomia than the threshold-based PT patient selection as shown in Figure 2 for any number of available proton slot per day. If all patients receive only photons or only protons, the average NTCP values for xerostomia are 16.9% and 6.3%, respectively. If 3 proton slots are available per day for HNC patients, the average NTCP value for xerostomia is 12.5% for the daily slot allocation strategy and 14.0% for the threshold-based PT patient selection which would select patients with 10% ΔNTCP threshold. The NTCP benefit of 1.5% can be explained by two considerations: 1) combined proton-photon treatments make optimal use of all proton slots whereas patient selection strategies face a trade-off between leaving slots unused or blocking slots for future patients with higher benefit; 2) on the convex part of the NTCP curve, the first proton fractions delivered are the most beneficial. Conclusion: Limited proton therapy resources can be more efficiently utilized, from a global health system perspective, with combined proton-photon treatments with daily allocation of proton slots compared to single-modality treatments with optimal patient selection. Radiotherapy treatment planning studies contribute significantly to advances and improvements in radiation treatment of cancer patients. They are a pivotal step to support and facilitate the introduction of novel techniques into clinical practice, or as a first step before clinical trials can be carried out. There have been numerous examples published in the literature that demonstrated the feasibility of such techniques as IMRT, VMAT, IMPT, or that compared different treatment methods (e.g. non-coplanar vs coplanar treatment), or investigated planning approaches (e.g. automated planning). However, for a planning study to generate trustworthy new knowledge and give confidence in applying its findings, then its design, execution and reporting all need to meet high scientific standards. - Abstract
Treatment planning in radiotherapy distinguishes three target volume concepts: the gross tumor volume (GTV), the clinical target volume (CTV), and the planning target volume (PTV). Over time, GTV definition and PTV margins have improved through the development of novel imaging techniques and better image guidance, respectively. CTV definition is sometimes considered the weakest element in the planning process. CTV definition is particularly complex since the extension of microscopic disease cannot be seen using currently available in-vivo imaging techniques. Instead, CTV definition has to incorporate knowledge of the patterns of tumor progression. While CTV delineation has largely been considered the domain of radiation oncologists, this paper, arising from a 2019 ESTRO Physics research workshop, discusses the contributions that medical physics and computer science can make by developing computational methods to support CTV definition. First, we overview the role of image segmentation algorithms, which may in part automate CTV delineation through segmentation of lymph node stations or normal tissues representing anatomical boundaries of microscopic tumor progression. The recent success of deep convolutional neural networks has also enabled learning entire CTV delineations from examples. Second, we discuss the use of mathematical models of tumor progression for CTV definition, using as example the application of glioma growth models to facilitate GTV-to-CTV expansion for glioblastoma that is consistent with neuroanatomy. We further consider statistical machine learning models to quantify lymphatic metastatic progression of tumors, which may eventually improve elective CTV definition. Lastly, we discuss approaches to incorporate uncertainty in CTV definition into treatment plan optimization as well as general limitations of the CTV concept in the case of infiltrating tumors without natural boundaries.

Combined treatment can be motivated by the consideration that, on the convex part of the NTCP curve, the first proton fractions are the most beneficial. We assume a situation of limited proton slot availability and develop methods to distribute those limited slots over a patient cohort optimally in order to optimize the benefit of proton therapy at a population level. Material and Methods: We consider a cohort of 45 head and neck cancer patients for which IMRT and IMPT plans were previously created [1]. NTCP models for relevant side effects (e.g. xerostomia) were used to calculate the NTCP values for all the plans. We investigate a 30 fraction simultaneous integrated boost (SIB) scheme (1.8 Gy to the PTV, 2.3 Gy to the GTV) and a sequential boost (SEQ) scheme with a 25 fraction base plan (2 Gy to the PTV) and a 10 fraction boost plan (2 Gy to the GTV). Under the assumption that, due to limited resources, only a small percentage of the total number of fractions can be delivered with protons, an integer programming algorithm was applied to determine the optimal number of proton fractions per patient that minimizes the total number of expected complications over the patient cohort. Results: Figure 1a shows the NTCP values for xerostomia in the IMRT and IMPT plans for the SIB scheme for all patients. Figures 1b shows the optimal allocation of proton slots for the situation where 20% of all fractions are delivered with protons. The patients with the highest ∆NTCP value (IMRT-IMPT) receive the largest number of proton fractions. The average xerostomia NTCP value from all 45 patients for the SIB scheme for combined treatment equals 13.0%. For the single-modality treatment, where the 20% of patients with the highest ∆NTCP are selected for proton therapy, the average NTCP equals 13.2%. For the SEQ scheme, the average NTCP values for xerostomia equal 13.6% and 14.2% for the combined and the single-modality treatment, respectively. Figure 1c shows the corresponding proton slot allocation for the SEQ scheme, indicating that only 3 patients receive proton slots for the boost plan whereas most proton slots are used for base plans. To achieve an average NTCP of 14.2%, combined treatment would require only 265 (16.8%) proton fractions instead of 315 (20%). Similar results were obtained for NTCP models for dysphagia and aspiration. Conclusion: Combined proton-photon treatments with optimized allocation of proton slots increase the benefit of proton therapy on the population level compared to single-modality treatments with optimal proton patient selection. However, the benefit is small for the SIB scheme. A larger benefit is observed for the sequential boost scheme, where combined treatments can exploit that some patients benefit from proton boost plans and others from proton base plans.
