Abstract

To the Editor: Nonvalvular atrial fibrillation (NVAF) is the most common sustained arrhythmia encountered in clinical practice and is associated with a 5-fold increased risk for stroke (1). Moreover, patients with NVAF often suffer from atherosclerotic complications such as acute myocardial infarction (AMI) (2). Peripheral artery disease (PAD) is an established marker of systemic atherosclerosis but its prevalence in NVAF is still unclear. We reasoned that inclusion of ankle-brachial index (ABI), which is an established tool for diagnosis of PAD (3), in the CHA2DS2-VASc (4) score would better define the prevalence of vascular disease. To address this issue, the Italian Society of Internal Medicine (SIMI) established an Italian registry documenting ABI in NVAF patients. The Atrial Fibrillation Registry for the ARAPACIS (Ankle-brachial Index Prevalence Assessment: Collaborative Italian Study) study is an independent research project involving all Regional Councils of SIMI. The first objective of the study was to estimate the prevalence of ABI ≤0.90 in NVAF patients. Consecutive patients with NVAF referred to internal medicine wards were eligible for the enrollment. Enrollment started in October 2010 and continued until October 30, 2012. Patients were enrolled if they were 18 years or older and had a diagnosis of NVAF, recording during the qualifying admission/consultation or in the preceding 12 months, and if it was possible to obtain the ABI measurement. Exclusion criteria included the following: acquired or congenital valvular AF, active cancer, disease with life expectancy 0.90 (93% vs. 82%; p < 0.0001). Logistic regression analysis demonstrated that ABI ≤0.90 was significantly associated with a smoking habit (odds ratio [OR]: 1.99; 95% confidence interval [CI]: 1.48 to 2.66; p < 0.0001), diabetes (OR: 1.93; 95% CI: 1.51 to 2.46; p < 0.0001), age class 65 to 74 years (OR: 2.05; 95% CI: 1.40 to 3.07; p < 0.0001), age class ≥75 years (OR: 3.12; 95% CI: 2.16 to 4.61; p < 0.0001), and history of previous transient ischemic attack/stroke (OR: 1.64; 95% CI: 1.20 to 2.24; p = 0.002). Vascular disease, as assessed by the history elements of CHA2DS2VASc score, was recorded in 17.3% of patients; inclusion of ABI ≤0.90 in the definition of vascular disease yielded a total prevalence of 33%. A higher prevalence of vascular disease was detected if ABI ≤0.90 was included in the CHA2DS2VASc score (Fig. 1). CHA2DS2VASc including ABI ≤0.90 was more associated with previous stroke (43%; OR: 1.85; 95% CI: 1.41 to 2.44; p < 0.0001) compared to CHA2DS2VASc with ABI 0.91 to 1.39 (23%; OR: 1.52; 95% CI: 1.10 to 2.11; p = 0.0117). To the best of our knowledge, there is no large-scale study that specifically examined the prevalence of ABI ≤0.90 in NVAF. In our population, 21% had ABI ≤0.90 indicating that NVAF is often associated with systemic atherosclerosis. The CHADS2 has been recently refined with the CHA2DS2-VASc score, which includes vascular disease as documented by a history of AMI, symptomatic PAD, or detection of atherosclerotic plaque in the aortic arch (4). Comparison of vascular prevalence as assessed by CHA2DS2-VASc score and/or ABI ≤0.90 is of interest to define the potentially positive impact of measuring ABI in the management of NVAF patients. Inclusion of ABI ≤0.90 in the definition of vascular disease greatly increased the prevalence of vascular disease, which increased from 17.3% (based on history alone) to 33% (based on ABI) in the entire population. If ABI ≤0.90 was encompassed in the definition of vascular disease of CHA2DS2-VASc score the prevalence of vascular disease increased in every risk class. Inclusion of ABI ≤0.90 in the CHA2DS2-VASc score allowed us to better define the risk profile of NVAF patients with an up-grading of the risk score in each CHA2DS2-VASc score category. This may have important therapeutic implications if the new score could be tested prospectively, as a higher number of NVAF patients would potentially be candidates for an anticoagulant treatment by measuring ABI. A prospective study is, therefore, necessary to validate the risk score of this new definition of vascular disease. In conclusion, this study provides the first evidence that one-fifth of NVAF patients had an ABI ≤0.90, indicating that it may represent a simple and cheap method to better define the prevalence of vascular disease in NVAF.

Abstract

In an era of advanced diagnostics, metastasis to cervical lymph nodes from an occult primary tumor is a rare clinical entity and accounts for approximately 3% of head and neck malignancies. Histologically, two thirds of cases are squamous cell carcinomas (SCCs), with other tissue types less common in the neck. With modern imaging and tissue examinations, a primary tumor initially undetected on physical examination is revealed in >50% of patients and the site of the index primary can be predicted with a high level of probability. In the present review, the range and limitations of diagnostic procedures are summarized and the optimal diagnostic workup is proposed. Initial preferred diagnostic procedures are a fine???needle aspiration biopsy (FNAB) and imaging. This allows directed surgical biopsy (such as tonsillectomy), based on the preliminary findings, and prevents misinterpretation of postsurgical images. When no primary lesion is suggested after imaging and panendoscopy, and for patients without a history of smoking and alcohol abuse, molecular profiling of an FNAB sample for human papillomavirus (HPV) and/or Epstein???Barr virus (EBV) is important. Abstract

Squamous cell carcinoma of the hypopharynx represents a distinct clinical entity. Most patients present with significant comorbidities and advanced???stage disease. The overall survival is relatively poor because of high rates of regional and distant metastasis at presentation or early in the course of the disease. A multidisciplinary approach is crucial in the overall management of these patients to achieve the best results and maintain or improve functional results. Traditionally, operable hypopharyngeal cancer has been treated by total (occasionally partial) laryngectomy and partial or circumferential pharyngectomy, followed by reconstruction and postoperative radiotherapy in most cases. Efforts to preserve speech and swallowing function in the surgical treatment of hypopharyngeal (and laryngeal) cancer have resulted in a declining use of total laryngopharyngectomy and improved reconstructive efforts, including microvascular free tissue transfer. There are many surgical, as well as nonsurgical, options available for organ and function preservation, which report equally effective tumor control and survival. The selection of appropriate treatment is of crucial importance in the achievement of optimal results for these patients. In this article, several aspects of surgical and nonsurgical approaches in the treatment of hypopharyngeal cancer are discussed. Future studies must be carefully designed within clearly defined populations and use uniform terminology and standardized functional assessment and declare appropriate patient or disease endpoints. These studies should focus on improvement of resultsx, without increasing patient morbidity. In this respect, technical improvements in radiotherapy such as intensity???modulated radiotherapy, advances in supportive care, and incorporation of newer systemic agents such as targeted therapy, are relevant developments. ?? 2010 Wiley Periodicals, Inc. Abstract

Background Parotidectomy is a common procedure and Frey's syndrome (gustatory sweating) is a common side effect. The current literature was assessed concerning the effectiveness of the sternocleidomastoid muscle (SCM) flap to prevent Frey's syndrome after parotidectomy. Methods A bibliography search was conducted for studies published between 1966 and 2010 and included randomized controlled trials (RCTs) or cohort studies with patients undergoing parotidectomy with facial nerve preservation. The outcome measures of particular interest were the incidence of Frey's syndrome and cosmetic impairment. Results In all, 12 studies were selected (1 meta???analysis of all interventions to prevent Frey's syndrome, 2 RCTs, and 9 cohort studies). The trials were too heterogeneous to perform a meta???analysis on the effect of the SCM flap. The results reported by the authors of each study suggest an objective decrease in Frey's syndrome when the SCM flap was used, but there was no difference in the patients' subjective reporting of symptoms. However, this conclusion is prone to the biases inherent in these studies, and thus overall it is impossible to make any recommendation. Conclusion Current reported evidence is inconclusive as to the use of SCM muscle flap as an intervention to prevent Frey's syndrome following parotid surgery. ?? 2011 Wiley Periodicals, Inc. Abstract

Evidence???based medicine integrates the best available data in decision making, with the goal of minimizing physicians' and patients' subjectivity. In 2006, the American Society of Clinical Oncology edited clinical practice guidelines for the use of larynx preservation strategies. The objective of this review was to evaluate the current levels of evidence for glottic squamous cell carcinoma. Current guidelines for early stage glottic cancer are based on low???level evidence. Conservation surgery (open or transoral) and radiation therapy are all valid options for T1 and selected T2 lesions. For advanced lesions, surgery and combined chemotherapy and radiation are options. High???level evidence favors combined chemotherapy and radiation therapy or altered fractionation radiation therapy as nonsurgical strategies for organ preservation, compared with radiation therapy alone. The optimal combination of chemotherapy, targeted therapy, and radiation therapy remains to be demonstrated, however, and for high???volume tumors, total laryngectomy may still be warranted. ?? 2011 Wiley Periodicals, Inc. L'autore esplora l'universo bibliografico attraverso un approccio che utilizza strumenti mutuati dalla filosofia e dalle scienze cognitive. Gli studi sul funzionamento del linguaggio e della mente umana possono presentare analisi valide anche per le realtà rappresentate dal catalogo, dalla raccolta documentaria e dagli utenti. Il linguaggio naturale è stato assimilato da alcune branche della filosofia del linguaggio ad un "gioco" che segue determinate regole, le quali hanno a loro volta uno stretto rapporto con il contesto sociale in cui tale gioco è giocato. Allo stesso modo, il catalogo richiede normative e standard catalografici che tengano conto delle finalità della biblioteca in cui il catalogatore opera e delle caratteristiche dell'utenza a cui la biblioteca si rivolge. Non esiste catalogo che possa rappresentare l'universo dei documenti, ma cataloghi che rappresentano diversi "universi", multiformemente collegati. La classificazione del sapere non è una classificazione della realtà e la sua struttura è solo uno dei possibili indici finalizzati a un'organizzazione delle informazioni contenute nei documenti. The studies on the operation of language and human mind can offer valid analysis for the realities represented in library catalogues, documentation collections, and users. Natural language has been ingested by some branches of Philosophy of Language as a "game" which follows specific rules in a very close relation with the respective social contest. In the same way, the catalogue requires standards that takes into account the purposes of a library and the peculiarity of its users. A research on cataloguing should consider a deep understanding on the functions and on the use of a library (rather than looking for unique models), without renouncing to the role of the library as a social institution, and of the library science as an academic subject.

Abstract

International audience; Mutations in the sphingomyelin phosphodiesterase 1 (SMPD1) gene were reported to be associated with Parkinson's disease and dementia with Lewy bodies. In the current study, we aimed to evaluate the role of SMPD1 variants in isolated rapid eye movement sleep behavior disorder (iRBD). SMPD1 and its untranslated regions were sequenced using targeted next-generation sequencing in 959 iRBD patients and 1287 controls from European descent. Our study reports no statistically significant association of SMPD1 variants and iRBD. It is hence unlikely that SMPD1 plays a major role in iRBD.

Abstract

We propose a new model for the time evolution of livestock commodities which exhibits endogenous deterministic stochastic behaviour. The model is based on the Yoccoz-Birkeland integral equation, a model first developed for studying the time-evolution of single species with high average fertility, a relatively short mating season and density dependent reproduction rates. This equation is then coupled with a differential equation describing the price of a livestock commodity driven by the unbalance between its demand and supply. At its birth the cattle population is split into two parts: reproducing females and cattle for butchery. The relative amount of the two is determined by the spot price of the meat. We prove the existence of an attractor and we investigate numerically its properties: the strange attractor existing for the original Yoccoz-Birkeland model is persistent but its chaotic behaviour depends also from the price evolution in an essential way., 26 pages, 19 figures

Abstract

Autoimmune diseases are a complex set of diseases characterized by immune system activation and, although many progresses have been done in the last 15 years, several unmet needs in the management of these patients may be still identified. Recently, a panel of international Experts, divided in different working groups according to their clinical and scientific expertise, were asked to identify, debate and formulate a list of key unmet needs within the field of rheumatology, serving as a roadmap for research as well as support for clinicians. After a systematic review of the literature, the results and the discussions from each working group were summarised in different statements. Due to the differences among the diseases and their heterogeneity, a large number of statements was produced and voted by the Experts to reach a consensus in a plenary session. At all the steps of this process, including the initial discussions by the steering committee, the identification of the unmet needs, the expansion of the working group and finally the development of statements, a large agreement was attained. This work confirmed that several unmet needs may be identified and despite the development of new therapeutic strategies as well as a better understanding of the effects of existing therapies, many open questions still remain in this field, suggesting a research agenda for the future and specific clinical suggestions which may allow physicians to better manage those clinical conditions still lacking of scientific clarity.

In order to solve the corresponding constrained quadratic optimization problem, we make large recourse both to the smart friendly technique originally proposed by B. de Finetti in his pioneering paper and to the well known Karush-Kuhn-Tucker conditions for constrained optimization. We offer closed form results and insightful considerations about the problem. In detail, we give closed form formulas to express the efficient mean-variance retention set both in the retention space and in the mean-variance one.

Abstract

International audience; Background/Objectives Body composition and protein-energy partitioning changes are important factors of body weight regulation, but have not been studied in the context of clinical obesity treatment setting. Thus, the aim of the present study was to investigate the pattern of body weight loss, body composition, and energy partitioning changes during a 9-month multidisciplinary weight loss program and 4-month follow up and to test the associations among these changes in adolescents with severe obesity. Subjects/Methods Twenty-five adolescents (14.1 +/- 1.5 years old; 13 girls) with severe obesity joined a pediatric obesity center for a 9-month inpatient multidisciplinary weight loss program. All participants performed body composition assessment (i.e. fat mass-FM, and fat-free mass-FFM) and completed a 36-h session in indirect calorimetric chamber before the start (T0), at the end of the intervention (T1) and 4 months follow-up to the intervention (T2). The protein-energy partitioning (P ratio) was calculated as urinary nitrogen loss/total energy expenditure over 24 hours. Interventions 9-month individualized multidisciplinary weight loss program consisting of lifestyle education, psychological support, physical activity, and dietary intervention. Results Initial P ratio was positively associated with changes in body weight from T0 to T1 (p = 0.038). The changes in FFM/FM were negatively associated with body weight changes in boys (p = 0.006) from T0 to T1 and in girls (p < 0.001) from T1 to T2. Urinary nitrogen excretion (p < 0.001) and total energy expenditure (p < 0.001) significantly decreased during the weight loss program while the P ratio did not significantly change. Conclusion The present results suggest that baseline and changes in energy partitioning may be associated with changes in body weight in adolescents with severe obesity. In addition, sexual dimorphism in these patterns of change suggest the need for specific dietary and physical activity strategies in boys and girls to optimize body weight loss and to prevent or slow weight regain.

Abstract

ObjectiveTo develop and validate ClinESSDAI (Clinical European League Against Rheumatism Sjögren's Syndrome Disease Activity Index), ie, ESSDAI without the biological domain.Patients and methodsThe 702 fictive vignettes derived from 96 real cases of primary Sjögren's syndrome of the ESSDAI development study were used. As for ESSDAI development, the physician assessment of disease activity (0–10 scale) was used as the ‘gold standard’ in a multivariate model for weighting domains, after removing the biological domain. The reliability, assessed by intraclass correlation coefficient (ICC) between ClinESSDAI and ESSDAI, explored if ClinESSDAI was equivalent to ESSDAI. Its psychometric (ie, measurement) properties were compared with that of ESSDAI in an independent cohort. Also, its use was evaluated on data of two clinical trials.ResultsIn multivariate modelling, all 11 domains remained significantly associated with disease activity, with slight modifications of some domain weights. Reliability between clinESSDAI and ESSDAI was excellent (ICC=0.98 and 0.99). Psychometric properties of clinESSDAI, disease activity levels and minimal clinically important improvement thresholds and its ability to detect change over time in clinical trials were very close to that of ESSDAI.ConclusionsClinESSDAI appears valid and very close to the original ESSDAI. This score provides an accurate evaluation of disease activity independent of B-cell biomarkers. It could be used in various circumstances: (i) in biological/clinical studies to avoid data collinearity, (ii) in clinical trials, as secondary endpoint, to detect change independent of biological effect of the drug, (iii) in clinical practice to assess disease activity for visits where immunological tests have not been done.

Abstract

Parsimonious games are a subset of constant sum homogeneous weighted majority games unequivocally described by their free type representation vector. We show that the minimal winning quota of parsimonious games satisfies a second order, linear, homogeneous, finite difference equation with nonconstant coefficients except for uniform games. We provide the solution of such an equation which may be thought as the generalized version of the polynomial expansion of a proper k-Fibonacci sequence. In addition we show that the minimal winning quota is a symmetric function of the representation vector; exploiting this property it is straightforward to prove that twin Parsimonious games, i.e. a couple of games whose free type representations are each other symmetric, share the same minimal winning quota.

Abstract

In a vintage paper concerning Parsimonious games, a subset of constant sum homogeneous weighted majority games, Isbell introduced a twin relationship based on transposition properties of the incidence matrices upon minimal winning coalitions of such games. A careful investigation of such properties allowed the discovery of some results on twin games presented in this paper. In detail we show that a) twin games have the same minimal winning quota and b) each Parsimonious game admits a unique balanced lottery on minimal winning coalitions, whose probabilities are given by the individual weights of its twin game.

Abstract

Context. The BL Lac object 1ES 0647+250 is one of the few distant γ-ray emitting blazars detected at very high energies (VHEs; ≳100 GeV) during a non-flaring state. It was detected with the MAGIC telescopes during a period of low activity in the years 2009−2011 as well as during three flaring activities in the years 2014, 2019, and 2020, with the highest VHE flux in the last epoch. An extensive multi-instrument data set was collected as part of several coordinated observing campaigns over these years. Aims. We aim to characterise the long-term multi-band flux variability of 1ES 0647+250, as well as its broadband spectral energy distribution (SED) during four distinct activity states selected in four different epochs, in order to constrain the physical parameters of the blazar emission region under certain assumptions. Methods. We evaluated the variability and correlation of the emission in the different energy bands with the fractional variability and the Z-transformed discrete correlation function, as well as its spectral evolution in X-rays and γ rays. Owing to the controversy in the redshift measurements of 1ES 0647+250 reported in the literature, we also estimated its distance in an indirect manner through a comparison of the GeV and TeV spectra from simultaneous observations with Fermi-LAT and MAGIC during the strongest flaring activity detected to date. Moreover, we interpret the SEDs from the four distinct activity states within the framework of one-component and two-component leptonic models, proposing specific scenarios that are able to reproduce the available multi-instrument data. Results. We find significant long-term variability, especially in X-rays and VHE γ rays. Furthermore, significant (3−4σ) correlations were found between the radio, optical, and high-energy (HE) γ-ray fluxes, with the radio emission delayed by about ∼400 days with respect to the optical and γ-ray bands. The spectral analysis reveals a harder-when-brighter trend during the non-flaring state in the X-ray domain. However, no clear patterns were observed for either the enhanced states or the HE (30 MeV < E < 100 GeV) and VHE γ-ray emission of the source. The indirect estimation of the redshift yielded a value of z = 0.45 ± 0.05, which is compatible with some of the values reported in the literature. The SEDs related to the low-activity state and the three flaring states of 1ES 0647+250 can be described reasonably well with the both one-component and two-component leptonic scenarios. Abstract

Real-world sensory-processing applications require compact, low-latency, and low-power computing systems. Enabled by their in-memory event-driven computing abilities, hybrid memristive-Complementary Metal-Oxide Semiconductor neuromorphic architectures provide an ideal hardware substrate for such tasks. To demonstrate the full potential of such systems, we propose and experimentally demonstrate an end-to-end sensory processing solution for a real-world object localization application. Drawing inspiration from the barn owl's neuroanatomy, we developed a bio-inspired, event-driven object localization system that couples state-of-the-art piezoelectric micromachined ultrasound transducer sensors to a neuromorphic resistive memories-based computational map. We present measurement results from the fabricated system comprising resistive memories-based coincidence detectors, delay line circuits, and a full-custom ultrasound sensor. We use these experimental results to calibrate our system-level simulations. These simulations are then used to estimate the angular resolution and energy efficiency of the object localization model. Abstract

We discuss the prominent role played by bilateral symmetry and modified Pascal triangles in self twin games, a subset of constant sum homogeneous weighted majority games. We show that bilateral symmetry of the free representations unequivocally identifies and characterizes this class of games and that modified Pascal triangles describe their cardinality for combinations of m and k, respectively linked through linear transforms to the key parameters n, number of players and h, number of types in the game. Besides, we derive the whole set of self twin games in the form of a genealogical tree obtained through a simple constructive procedure in which each game of a generation, corresponding to a given value of m, is able to give birth to one child or two children (depending on the parity of m), self twin games of the next generation. Abstract

Volume integral methods for the solution of eddy current problems are very appealing in practice since they require meshing only the conducting regions. However, they require the assembly and storage of a dense stiffness matrix. With the objective of cutting down assembly time and memory occupation, low-rankapproximation techniques like the Adaptive Cross Approximation (ACA) have been considered a major breakthrough. Recently, the VINCO framework has been introduced to reduce significantly memory occupation and computational time thanks to a novel factorization of the dense stiffness matrix. The aim of this paper is introducing a new matrix compression technique enabled by the VINCO framework. We compare the performance of VINCO framework approaches with state-of-the-art alternatives in terms of memory occupation,computational time and accuracy by solving benchmark eddy current problems at increasing mesh sizes; the comparisons are carried out using both direct and iterative solvers. The results clearly indicate that the so-called VINCO-FAIME approach which exploits the Fast Multipole Method (FMM) has the best performance.

Abstract

International audience; OBJECTIVE: Patients with primary Sjögren's syndrome (SS) are at greater risk of developing lymphoma. This study was undertaken to evaluate whether the Fms-like tyrosine kinase 3 ligand (Flt-3L) might be associated with lymphoma in primary SS. METHODS: Serum levels of Flt-3L were measured in 369 patients with primary SS from the French Assessment of Systemic Signs and Evolution of Sjögren's Syndrome study cohort and in 10 patients with primary SS at the time of lymphoma diagnosis in an Italian cohort. Associations between increased levels of Flt-3L and a history of lymphoma, history of previously diagnosed criteria related to a high risk of lymphoma, and greater extent of disease activity were evaluated. RESULTS: Among patients with primary SS, higher levels of Flt-3L were significantly associated with a history of lymphoma (P = 0.0001). Previous markers for risk of lymphoma development, such as presence of purpura, low levels of C4, presence of lymphocytopenia, low levels of IgM, high levels of β2 -microglobulin, and a higher primary SS disease activity score, were all associated with higher levels of Flt-3L. The levels of Flt-3L were also increased in serum obtained from patients with primary SS at the time of lymphoma diagnosis. Furthermore, the Flt-3L levels were elevated in the serum of 6 patients up to 94 months (mean 46 months) prior to the diagnosis of lymphoma. Receiver operating characteristic curve analysis showed that an Flt-3L level of 175 pg/ml was the ideal cutoff value for demonstrating an association with lymphoma (specificity 97.5%, sensitivity 44%, negative predictive value 97%). CONCLUSION: Flt-3L is associated with lymphoma in primary SS, and constitutes a good biologic marker. Higher levels of this cytokine are present several years before the diagnosis of lymphoma, and may be useful as a predictive marker of lymphoproliferative disorders in primary SS.

Abstract

Background The role of surgery for incidentally discovered diffuse incidental low-grade gliomas (iLGGs) is debatable and poorly documented in current literature. Objective The aim was to identify factors that influence survival for patients that underwent surgical resection of iLGGs in a large multicenter population. Methods Clinical, radiological, and surgical data were retrospectively analyzed in 267 patients operated for iLGG from 4 neurosurgical Centers. Univariate and multivariate analyses were performed to identify predictors of overall survival (OS) and tumor recurrence (TR). Results The OS rate was 92.41%. The 5- and 10-year estimated OS rates were 98.09% and 93.2%, respectively. OS was significantly longer for patients with a lower preoperative tumor volume (P = .001) and higher extent of resection (EOR) (P = .037), regardless the WHO-defined molecular class (P = .2). In the final model, OS was influenced only by the preoperative tumor volume (P = .006), while TR by early surgery (P = .028). A negative association was found between preoperative tumor volumes and EOR (rs = −0.44, P < .001). The median preoperative tumor volume was 15 cm3. The median EOR was 95%. Total or supratotal resection of T2-FLAIR abnormality was achieved in 61.62% of cases. Second surgery was performed in 26.22%. The median time between surgeries was 5.5 years. Histological evolution to high-grade glioma was detected in 22.85% of cases (16/70). Permanent mild deficits were observed in 3.08% of cases. Conclusions This multicenter study confirms the results of previous studies investigating surgical management of iLGGs and thereby strengthens the evidence in favor of early surgery for these lesions.

Abstract

The aim of this communication is to present the landscape agronomy framework and how it addresses spatially explicit natural resource management in agriculture. Based on a summary of the scientific background and state of the art on how agriculture is addressed at the landscape level, the result is a conceptual model that provides a framework for observing, understanding, and supporting the actions of actors involved in the dynamics of agricultural landscapes and the design of agri-food systems. The landscape agronomy conceptual model was developed to integrate assessment and monitoring beyond the farm level and across multiple temporal and spatial scales. It was introduced in a seminal paper by Benoît and colleagues (Landscape Ecology, 27(10):2012), which highlighted the importance of an integrated approach to describing and understanding farming practices, natural resources, and landscape patterns. Abstract

International audience; Landscape Agronomy is expected to contribute to the observation, understanding and support of agriculture at the landscape level. The conceptual model of Landscape Agronomy was developed to integrate assessment and monitoring beyond the farm level and across multiple temporal and spatial scales. In this chapter, we evaluate the relevance of the Landscape Agronomy conceptual model by analysing three issues of integrated governance and public action. The three examples highlight the three parts of the conceptual model: (a) core agricultural landscape dynamics, illustrated by farmers’ collective projects of agroecological transition; (b) the three poles of the conceptual model—practices, resources and patterns—illustrated by Integrated Pest Management as an example of requirements for sustainable agroecosystem management; and (c) temporal and spatial interactions, illustrated by the concept of territorial circularity as a way to integrate landscape-based management into global issues related to nutrient cycles. Although it captures the key issues addressed by such perspectives, the Landscape Agronomy conceptual model still faces some challenges, such as explicit consideration of temporal dimensions and explicit inclusion of new (invisible) patterns emerging from newly available data and digitalisation.

No. of bitstreams: 0 Previous issue date: 2021-07-01 Background: The COVID-19 pandemic is having profound mental health consequences for many people. Concerns have been expressed that, at their most extreme, these consequences could manifest as increased suicide rates. We aimed to assess the early effect of the COVID-19 pandemic on suicide rates around the world. Methods: We sourced real-time suicide data from countries or areas within countries through a systematic internet search and recourse to our networks and the published literature. Between Sept 1 and Nov 1, 2020, we searched the official websites of these countries’ ministries of health, police agencies, and government-run statistics agencies or equivalents, using the translated search terms “suicide” and “cause of death”, before broadening the search in an attempt to identify data through other public sources. Data were included from a given country or area if they came from an official government source and were available at a monthly level from at least Jan 1, 2019, to July 31, 2020. Our internet searches were restricted to countries with more than 3 million residents for pragmatic reasons, but we relaxed this rule for countries identified through the literature and our networks. Areas within countries could also be included with populations of less than 3 million. We used an interrupted time-series analysis to model the trend in monthly suicides before COVID-19 (from at least Jan 1, 2019, to March 31, 2020) in each country or area within a country, comparing the expected number of suicides derived from the model with the observed number of suicides in the early months of the pandemic (from April 1 to July 31, 2020, in the primary analysis). Findings: We sourced data from 21 countries (16 high-income and five upper-middle-income countries), including whole-country data in ten countries and data for various areas in 11 countries). Rate ratios (RRs) and 95% CIs based on the observed versus expected numbers of suicides showed no evidence of a significant increase in risk of suicide since the pandemic began in any country or area. There was statistical evidence of a decrease in suicide compared with the expected number in 12 countries or areas: New South Wales, Australia (RR 0·81 [95% CI 0·72–0·91]); Alberta, Canada (0·80 [0·68–0·93]); British Columbia, Canada (0·76 [0·66–0·87]); Chile (0·85 [0·78–0·94]); Leipzig, Germany (0·49 [0·32–0·74]); Japan (0·94 [0·91–0·96]); New Zealand (0·79 [0·68–0·91]); South Korea (0·94 [0·92–0·97]); California, USA (0·90 [0·85–0·95]); Illinois (Cook County), USA (0·79 [0·67–0·93]); Texas (four counties), USA (0·82 [0·68–0·98]); and Ecuador (0·74 [0·67–0·82]). Interpretation: This is the first study to examine suicides occurring in the context of the COVID-19 pandemic in multiple countries. In high-income and upper-middle-income countries, suicide numbers have remained largely unchanged or declined in the early months of the pandemic compared with the expected levels based on the pre-pandemic period. Abstract

International audience; Programming plays a paramount role in many educational policies and initiatives. However, the current focus on coding skills poses a risk of giving pupils an over simplistic and impoverished idea of what programming means and involves. Their experiences would be much more significant if learning were aimed at understanding the richness of the nature of programs. In fact, programs are strange creatures that escape simple definitions. They are real, in that they affect our real lives; they are abstract, in that they process abstract entities; and they are concrete, in that they take up space in digital devices memory, and can be copied, transferred, corrupted. Thus, understanding the multifaceted nature of programs is crucial knowledge for all citizens of the digital era, and a fundamental component of such an understanding is getting a sense of how programs are created and work (i.e., the programming process). To the best of our knowledge, there is no Nature of Programs framework (e.g., a set of statements that describe what the nature of programs is), that teachers and policy makers can use to shape their practice and targets. The goal of the WG is developing such a framework, by collecting and organizing contributions from CER, CS experts, and educators.

The teaching of quantum physics is challenging, not the least because teachers must overcome the traditional narrative approach, students must gain a conceptual understanding of fundamentals, and citizens must become aware of quantum technologies. Quantum games are powerful tools to overcome obstacles and push one’s limits without fear of failure. We report on a pilot study involving twenty high-school student volunteers, consisting of a compact intervention module on the concepts of quantum states, properties, measurement, superposition, and entanglement within the framework of the Model of Educational Reconstruction, followed by playing a game, quantum TiqTaqToe. The outcomes of this research-based learning environment are discussed via the qualitative analysis of students’ answers to two open questionnaires. We find that students grasped the concepts of superposition and, with special awareness, entanglement, the game proving effective to help students experience their implications in quantum behavior. Theinformal and stimulating tournament atmosphere favored intertwining of the game with learning goals. Our central message is that the use of quantum game tools fits a teaching/learning environment in manners often not well understood in the literature; it enhances awareness of the nature of new and non-intuitive concepts, increases complementarity with other languages within the process of thinking about physics, boosts student engagement, and improves intervention efficiency and effectiveness.

Abstract

The landscape is widely identified as a relevant target both by integrative policies and across the disciplines dealing with resource management and territorial planning. Landscape agronomy promotes a greater involvement of agricultural sciences into this arena by increasing the attention on the dynamics relating the farming practices to the natural resources and the temporal and spatial patterns of land covers. This book covers the background that improved the transdisciplinary interface of agronomy with spatially-explicit disciplines like landscape ecology and geography both in research and in training programs, in addition to some experiences of participative landscape management. Abstract

The best stem cell source for T-cell replete human leukocyte antigen (HLA)-haploidentical transplantation with post-transplant cyclophosphamide (PTCy) remains to be determined. In this European Society for Blood and Marrow Transplantation retrospective study, we analyzed the impact of stem cell source on leukemia-free survival (LFS) in adult patients with primary refractory or relapsed acute myeloid leukemia (AML) given grafts from HLA-haploidentical donors with PTCy as graft-versus-host disease (GVHD) prophylaxis. A total of 668 patients (249 bone marrow [BM] and 419 peripheral blood stem cells [PBSC] recipients) met the inclusion criteria. The use of PBSC was associated with a higher incidence of grade II-IV (HR = 1.59, p = .029) and grade III-IV (HR = 2.08, p = .013) acute GVHD. There was a statistical interaction between patient age and the impact of stem cell source for LFS (p < .01). In multivariate Cox models, among patients = 55 years of age, the use of PBSC versus BM was associated with higher non-relapse mortality (NRM) (HR = 1.7, p = .01), lower LFS (HR = 1.37, p = .026) and lower overall survival (HR = 1.33, p = .044). In conclusions, our data suggest that in patients >= 55 years of age with active AML at HLA-haploidentical transplantation, the use of BM instead of PBSC as stem cell source results in lower NRM and better LFS. In contrast among younger patients, the use of PBSC results in at least a comparable LFS.

Abstract

PURPOSE 18F-Fluorodeoxyglucose (FDG) positron emission tomography (PET)/computed tomography (CT) is currently the standard technique to define minimal residual disease (MRD) status outside the bone marrow (BM) in patients with multiple myeloma (MM). This study aimed to define criteria for PET complete metabolic response after therapy, jointly analyzing a subgroup of newly diagnosed transplantation-eligible patients with MM enrolled in two independent European randomized phase III trials (IFM/DFCI2009 and EMN02/HO95). PATIENTS AND METHODS Two hundred twenty-eight patients were observed for a median of 62.9 months. By study design, PET/CT scans were performed at baseline and before starting maintenance (premaintenance [PM]). The five-point Deauville scale (DS) was applied to describe BM (BM score [BMS]) and focal lesion (FL; FL score [FS]) uptake and tested a posteriori in uni- and multivariable analyses for their impact on clinical outcomes. RESULTS At baseline, 78% of patients had FLs (11% extramedullary), 80% with an FS ≥ 4. All patients had BM diffuse uptake (35.5% with BMS ≥ 4). At PM, 31% of patients had visually detectable FLs (2% extramedullary), 24% and 67.7% of them with an FS of 3 and ≥ 4, respectively. At PM, 98% of patients retained residual BM diffuse uptake, which was significantly lower than at baseline (mainly between BMS 2 and 3, BMS was ≥ 4 in only 8.7% of patients). By both uni- and multivariable analysis, FS and BMS < 4 were associated with prolonged progression-free survival (PFS) and overall survival (OS) at PM (OS: hazard ratio [HR], 0.6 and 0.47, respectively; PFS: HR, 0.36 and 0.24, respectively) CONCLUSION FL and BM FDG uptake lower than the liver background after therapy was an independent predictor for improved PFS and OS and can be proposed as the standardized criterion of PET complete metabolic response, confirming the value of the DS for patients with MM.

Abstract

This study reports the development of a sensitive magnetic bead-based enzyme-linked immunoassay (MELISA) for the pan-reactive detection of the Influenza A virus. The assay combines immunomagnetic beads and biotin-nanoparticle-based detection to quantify a highly conserved viral nucleoprotein in virus lysates. At the capture step, monoclonal antibody-coated magnetic microbeads were used to bind and concentrate the nucleoprotein in samples. The colorimetric detection signal was amplified using biotinylated silica nanoparticles (NP). These nanoparticles were functionalized on the surface with short DNA spacers bearing biotin groups by an automated supported synthesis method performed on nano-on-micro assemblies with a DNA/RNA synthesizer. A biotin-nanoparticle and immunomagnetic bead-based assay was developed. We succeeded in detecting Influenza A viruses directly in the lysis buffer supplemented with 10% saliva to simulate the clinical context. The biotin-nanoparticle amplification step enabled detection limits as low as 3 × 103 PFU mL−1 and 4 × 104 PFU mL−1 to be achieved for the H1N1 and H3N2 strains respectively. In contrast, a classical ELISA test based on the same antibody sandwich showed detection limit of 1.2 × 107 PFU mL−1 for H1N1. The new enhanced MELISA proved to be specific, as no cross-reactivity was found with a porcine respiratory virus (PRRSV). Abstract

At the end of October 2018, a storm of unprecedented strength severely damaged the forests of the eastern sector of the Italian Alps. The affected forest area covers 42,500 ha. The president of one of the damaged regions asked for help from the University of Padua. After eight months of discussion, the authors of this article wrote a consensus text. The sometimes asper debate brought to light some crucial aspects: 1) even experienced specialists may have various opinions based on scientific knowledge that lead to conflicting proposals for action. For some of them there is evidence that to restore a destroyed natural environment it is more judicious to do nothing; 2) the soil corresponds to a living structure and every ecosystem’s management should be based on it; 3) faced with a catastrophe, people and politicians find themselves unarmed, also because they rarely have the scientific background to understand natural processes. Yet politicians are the only persons who make the key decisions that drive the economy in play and therefore determine the near future of our planet. This article is an attempt to respond directly to a governor with a degree in animal production science, who formally and prudently asked a university department called “Land, Environment, Agriculture and Forestry” for help before taking decisions; 4) the authors also propose an artistic interpretation of facts (uncontrolled storm) and conclusions (listen to the soil). Briefly, the authors identify the soil as an indispensable source for the renewal of the destroyed forest, give indications on how to prepare a map of the soils of the damaged region, and suggest to anchor on this soil map a series of silvicultural and soil management actions that will promote the soil conservation and the faster recovery of the natural dynamic stability and resilience. Abstract

International audience; KRAS is one of the most mutated oncogenes and still considered an undruggable target. An alternative strategy would consist in targeting its gene rather than the protein, specifically the formation of G-quadruplexes (G4) in its promoter. G4 are secondary structures implicated in biological processes, which can be formed among G-rich DNA (or RNA) sequences. Here we have studied the major conforma-tions of the commonly known KRAS 32R, or simply 32R, a 32 residue sequence within the KRAS Nucle-ase Hypersensitive Element (NHE) region. We have determined the structure of the two major stable con-formers that 32R can adopt and which display slow equilibrium (>ms) with each other. By using different biophysical methods, we found that the nucleotides G9, G25, G28 and G32 are particularly implicated in the exchange between these two conformations. We also showed that a triad at the 3 end further stabilizes one of the G4 conformations, while the second conformer remains more flexible and less stable.

Abstract

Objective: Rapid eye movement sleep behavior disorder (RBD) is a prodromal synucleinopathy, as >80% will eventually convert to overt synucleinopathy. We performed an in-depth analysis of the SNCA locus to identify RBD-specific risk variants. Methods: Full sequencing and genotyping of SNCA was performed in isolated/idiopathic RBD (iRBD, n = 1,076), Parkinson disease (PD, n = 1,013), dementia with Lewy bodies (DLB, n = 415), and control subjects (n = 6,155). The iRBD cases were diagnosed with RBD prior to neurodegeneration, although some have since converted. A replication cohort from 23andMe of PD patients with probable RBD (pRBD) was also analyzed (n = 1,782 cases; n = 131,250 controls). Adjusted logistic regression models and meta-analyses were performed. Effects on conversion rate were analyzed in 432 RBD patients with available data using Kaplan–Meier survival analysis. Results: A 5′-region SNCA variant (rs10005233) was associated with iRBD (odds ratio [OR] = 1.43, p = 1.1E-08), which was replicated in pRBD. This variant is in linkage disequilibrium (LD) with other 5′ risk variants across the different synucleinopathies. An independent iRBD-specific suggestive association (rs11732740) was detected at the 3′ of SNCA (OR = 1.32, p = 4.7E-04, not statistically significant after Bonferroni correction). Homozygous carriers of both iRBD-specific SNPs were at highly increased risk for iRBD (OR = 5.74, p = 2E-06). The known top PD-associated variant (3′ variant rs356182) had an opposite direction of effect in iRBD compared to PD. Interpretation: There is a distinct pattern of association at the SNCA locus in RBD as compared to PD, with an opposite direction of effect at the 3′ of SNCA. Several 5′ SNCA variants are associated with iRBD and with pRBD in overt synucleinopathies. ANN NEUROL 2020;87:584–598.
