Abstract: Pour remédier au nombre insuffisant de maîtres qualifiés pour enseigner les langues vivantes à l’école primaire en France, de nombreux efforts ont été déployés. Par exemple, dès l’année 2001 une formation de dominante langue a été mise en place dans les IUFM. Elle permet aux stagiaires intéressés par les langues d’améliorer leurs compétences. Cependant, cette formation n’est qu’optionnelle pour le moment. Nous considérons que l’autoformation peut constituer une alternative pour répondre aux besoins des PE (Professeurs des écoles). Cet article décrit une expérience d’autoformation menée à l’IUFM de Nancy en 2004-2005. To increase the insufficient number of primary school teachers qualified to teach foreign languages in primary schools in France, several efforts were made by the Ministry of Education. For example, in 2001 a module called “Dominante Langues vivantes” was created in the Teacher Training Colleges to enable the students interested in languages to improve their competences. However, this module is only optional for second year students. We consider that self-directed learning can constitute an alternative to meet the needs of primary school teachers. This article describes an experiment of self-directed learning carried out in the Teacher Training Colleges of Nancy in 2004-2005.

Abstract: International audience; Over the last few years, several works have proposed deep learning architectures to learn dynamical systems from observation data with no or little knowledge of the underlying physics. A line of work relies on learning representations where the dynamics of the underlying phenomenon can be described by a linear operator, based on the Koopman operator theory. However, despite being able to provide reliable long-term predictions for some dynamical systems in ideal situations, the methods proposed so far have limitations, such as requiring to discretize intrinsically continuous dynamical systems, leading to data loss, especially when handling incomplete or sparsely sampled data. Here, we propose a new deep Koopman framework that represents dynamics in an intrinsically continuous way, leading to better performance on limited training data, as exemplified on several datasets arising from dynamical systems.

Abstract: We introduce a scalar elliptic equation defined on a boundary layer given by Π 2 × [0, z top ], where Π 2 is a two dimensional torus, with an eddy vertical eddy viscosity of order z α , α ∈ [0, 1], an homogeneous boundary condition at z = 0, and a Robin condition at z = z top. We show the existence of weak solutions to this boundary problem, distinguishing the cases 0 ≤ α < 1 and α = 1. Then we carry out several numerical simulations, showing the ability of our model to accuratly reproduce profiles close to those predicted by the Monin-Oboukhov theory, by calculating stabilizing functions.

Abstract: In many applications, one encounters signals that lie on manifolds rather than a Euclidean space. In particular, covariance matrices are examples of ubiquitous mathematical objects that have a non Euclidean structure. The application of Euclidean methods to integrate differential equations lying on such objects does not respect the geometry of the manifold, which can cause many numerical issues. In this paper, we propose to use Lie group methods to define geometry-preserving numerical integration schemes on the manifold of symmetric positive definite matrices. These can be applied to a number of differential equations on covariance matrices of practical interest. We show that they are more stable and robust than other classical or naive integration schemes on an example.

Abstract: International audience; We study the shape of convective rolls in the Marine Atmospheric Boundary Layer from Synthetic Aperture Radar images of the ocean. We propose a multiscale analysis with structure functions which allow an easy generalization to analyse high-order statistics and so to finely describe the shape of the rolls. The two main results are : 1) second order structure function characterizes the wavelength and direction of rolls just like correlation or power spectrum do, 2) high order statistics can be studied with skewness and Flatness which characterize the asymmetry and intermittency of rolls respectively. From the best of our knowledge, this is the first time that the asymmetry and intermittency of rolls is shown from radar images of the ocean surface.

Abstract: Advanced age is an independent poor prognostic factor of diffuse large B-cell lymphoma (DLBCL). PMitCEBO (mitoxantrone, cyclophosphamide, etoposide, vincristine, bleomycin, and prednisolone) is an alternative to the cyclophosphamide, doxorubicin, vincristine, and prednisolone regimen to decrease side effects in elderly patients. Many studies have shown prognostic value of an interim FDG PET-CT to predict survival. A recent consensus (ICML, Lugano 2013) has suggested using the 5-point scale Deauville criteria instead of those of the International Harmonization Project (IHP) to visually assess the response on interim PET. The objective of this study was to evaluate the prognostic value of an interim FDG PET-CT in patients older than 60 with treated DLBCL and to compare IHP and 5-PS Deauville visual interpretation to predict survival.Forty-eight patients (mean age 73.2±5.2 years) treated by R-PMitCEBO for DLBCL undergoing FDG PET-CT before and after 3 cycles of treatment were retrospectively included. Event-free survival and overall survival were determined by Kaplan-Meier method and compared with interim PET-CT results using IHP and 5-PS Deauville criteria.Interim PET results using 5-PS Deauville criteria were significantly correlated with EFS (P0.0001) and OS (P=0.001) whereas they were moderately correlated with EFS (P=0.046) and not with OS (P=0.106) using IHP criteria. Two-year EFS and OS rates were 86.5% and 89.2%, respectively, for patients in 1-3 score group, and 27.3% and 36.4%, respectively, for patients in ≥4 score group using the Deauville criteria.Our results confirmed the prognostic value of an interim PET-CT in elderly patients with DLBCL and the better performance of the 5-PS Deauville criteria.

Abstract: International audience; Assessing genetic connectivity among populations in high gene flow species is sometimes insufficient to evaluate demographic connectivity. Genetic differentiation quickly becomes zero as soon as a few dozen migrants are exchanged per generation. This provides little information to determine whether migration can ensure demographic coupling. The resulting difficulties in delineating conservation units for the management of commercially exploited marine fish species are well illustrated in the case of the European sea bass (Dicentrarchus labrax). Previous attempts to assess connectivity patterns in the northeast Atlantic have been hampered by a lack of spatial genetic structure. In contrast, mark-recapture data suggested low migration rates between regional spawning areas. Here, we show how a spatial gradient of introgressed Mediterranean ancestry across the northeast Atlantic reflects cryptic patterns of genetic and demographic connectivity. Using a 1K SNP chip dataset in 827 individuals sampled from Portugal to the North Sea, we found null overall genetic differentiation across the northeast Atlantic. We however detected a subtle latitudinal admixture gradient originating at the edge of the contact zone with the Mediterranean sea bass lineage. Two significant breaks in the ancestry gradient at the tip of Galicia and northern Brittany indicated barriers to effective dispersal between demographically distinct units. Moreover, a northward expansion signal in Irish and North Seas was revealed by the surfing of rare Mediterranean alleles at the edge of the species range. Our results show that introgressed ancestry gradients offer a powerful alternative to assess genetic and demographic connectivity when the neutral migration-drift balance is not informative.

Abstract: Tremendous progress has been made recently in imaging the contacts between intra-cellular organelles, which are thought to be mediated by soluble tethers. However, they are still difficult to study in cellulo, and reconstituting them in vitro is a standing challenge. Here we take a mimetic approach to study Giant unilamellar vesicles (GUVs) and supported lipid bilayers (SLBs) interacting via single- (or double-) stranded DNA sequences of two different lengths. Like intra-cellular tethers which may reside in the cytosol when unbound, the DNA-tethers are soluble, but can insert into the membrane with the help of cholesterol moieties found at their extremities. Tether-exchange between the bulk “cytosol” and the GUV/SLB membrane leads to a novel statistical ensemble in which the entire system equilibrates together, rather than individual GUVs behaving as separate closed systems. As a consequence, adhesion between the GUV and the SLB is driven by collective entropic effects amplified by tether shape changes associated with membrane bridging. A direct experimental consequence is an unusual dependence on tether-concentration, which becomes an important control parameter at low concentrations, while tether length/flexibility are less important. The establishment of this fundamentally different interaction between two membranes suggests that in physiological conditions, the regulation of contact formation inside cells may be very different from the case of the much studied ligand-receptor mediated cell adhesion.

Abstract: International audience; Gastric cancer (GC) is highly deadly. Three-dimensional (3D) cancer cell cultures, known as spheroids, better mimic tumor microenvironment (TME) than standard 2D cultures. Cancer-associated fibroblasts (CAF), a major cellular component of TME, promote or restrain cancer cell proliferation, invasion and resistance to drugs. We established spheroids from two human GC cell lines mixed with human primary CAF. Spheroid organization, analyzed by two-photon microscopy, showed CAF in AGS/CAF spheroids clustered in the center, but dispersed throughout in HGT-1/CAF spheroids. Such differences may reflect clonal specificities of GC cell lines and point to the fact that GC should be considered as a highly personalized disease.

Abstract: Objective Male sex is one of the determinants of severe coronavirus diseas-e-2019 (COVID-19). We aimed to characterize sex differences in severe outcomes in adults with diabetes hospitalized for COVID-19. Methods We performed a sex-stratified analysis of clinical and biological features and outcomes (i.e. invasive mechanical ventilation (IMV), death, intensive care unit (ICU) admission and home discharge at day 7 (D7) or day 28 (D28)) in 2380 patients with diabetes hospitalized for COVID-19 and included in the nationwide CORONADO observational study (NCT04324736). Results The study population was predominantly male (63.5%). After multiple adjustments, female sex was negatively associated with the primary outcome (IMV and/or death, OR: 0.66 (0.49–0.88)), death (OR: 0.49 (0.30–0.79)) and ICU admission (OR: 0.57 (0.43–0.77)) at D7 but only with ICU admission (OR: 0.58 (0.43–0.77)) at D28. Older age and a history of microvascular complications were predictors of death at D28 in both sexes, while chronic obstructive pulmonary disease (COPD) was predictive of death in women only. At admission, C-reactive protein (CRP), aspartate amino transferase (AST) and estimated glomerular filtration rate (eGFR), according to the CKD-EPI formula predicted death in both sexes. Lymphocytopenia was an independent predictor of death in women only, while thrombocytopenia and elevated plasma glucose concentration were predictors of death in men only. Conclusions In patients with diabetes admitted for COVID-19, female sex was associated with lower incidence of early severe outcomes, but did not influence the overall in-hospital mortality, suggesting that diabetes mitigates the female protection from COVID-19 severity. Sex-associated biological determinants may be useful to optimize COVID-19 prevention and management in women and men.

Abstract: Resume Introduction les internes en medecine constituent une population a risque de troubles reactionnels au travail. L’objet de cette etude etait de qualifier par la narration les facteurs percus comme positifs et ceux percus comme negatifs au travail pour les internes d’une universite francaise. Materiel et methode Il s’agissait d’une etude qualitative, selon la theorie ancree par thematisation continue, aupres d’internes de medecine (generale et specialisee). Resultats La variation maximale, en fonction de criteres professionnels et sociaux, et la saturation des donnees de codage ont ete atteintes apres 10 entretiens non diriges. Les internes ont exprime l’importance des relations au travail, au-dessus de toute autre dimension professionnelle, qu’il s’agisse des relations avec les autres medecins (tuteurs ou autres internes) que celles avec les paramedicaux. Les difficultes organisationnelles et la charge de travail semblaient passer au second plan, et surtout se derouler lors des stages en hopital universitaire. Conclusion Ces donnees permettaient de mieux considerer les resultats des etudes quantitatives et de mieux comprendre le vecu des internes en medecine. Des axes de prevention peuvent se degager pour les universites, les hopitaux et les medecins accueillants des internes.

Abstract: International audience; Among the multitude of optical polarization contrasts that can be observed in complex biological specimens, linear diattenuation (LD) imaging has received little attention. It is indeed challenging to image LD with basic polarizing microscopes because it is often relatively small in comparison with linear retardance (LR). In addition, interpretation of LD images is not straightforward when experiments are conducted in the visible range because LD can be produced by both dichroism and anisotropic scattering. Mueller polarimetry is a powerful implementation of polarization sensing able to differentiate and measure the anisotropies of specimens. In this article, near infrared transmission Mueller scanning microscopy is used to image LD in thin biological specimen sections made of various proteins with unprecedented resolution and sensitivity. The near infrared spectral range makes it possible to lower the contribution of dichroism to the total linear diattenuation in order to highlight anisotropic scattering. Pixel-by-pixel comparison of LD images with LR and multiphoton images demonstrates that LD is produced by under-resolved structures that are not revealed by other means, notably within the sarcomere of skeletal muscles. LD microscopy appears as a powerful tool to provide new insights into the macro-molecular organization of biological specimens at the sub-microscopic scale without labelling.

She had not traveled recently. General health status was good. She denied fever, dyspnea, chest pain, and extra-pulmonary symptoms.

Abstract: Background We aimed to determine the prevalence of residual pulmonary vascular obstruction (RPVO) after symptomatic pulmonary embolism (PE) and to identify risk factors for RPVO. Methods On the basis of a prospective cohort of patients with a documented symptomatic venous thromboembolism, we included patients who had an acute PE and underwent a ventilation/perfusion lung scan at 3 to 24 months during the follow-up after PE. RPVO score was assessed for each patient. Initial pulmonary vascular obstruction at PE diagnosis was also assessed when available. Univariable and multivariable analyses were performed with preselected data to identify predictors for persistent defect defined as RPVO ≥ 5%. Results Among the 537 included patients, 278 (51.8%) had RPVO ≥ 5%, and 191 (35.6%) had RPVO ≥ 10%. In primary multivariate analysis on overall population, age ≥ 65 years (odds ratio [OR] 2.25, 95% CI, 1.45–3.52) and chronic respiratory failure (OR 3.19, 95% CI, 1.22–10.04) were independent predictors of RPVO ≥ 5%. In secondary multivariate analysis restricted to 256 patients with available initial pulmonary vascular obstruction score at index PE (IPVO), age ≥ 65 years (OR 2.78, 95% CI, 1.41–5.53), unprovoked PE (OR 2.11, 95% CI, 1.11–4.07) and IPVO ≥ 20% (OR 2.94, 95% CI, 1.68–5.20) were found to be independent risk factors for RVPO ≥5%. Conclusion In this selected population of patients with an acute PE, age ≥ 65 years, unprovoked PE and IPVO ≥ 20% at PE diagnosis appeared to be risk factors for residual pulmonary vascular obstruction measured at three to 24 months.

Abstract: International audience; Objectives: We aimed at describing placental abruption in our county and at evaluating factors associated with poor fetal outcome.Study design: In this case-control study, women with placental abruption were identified from two databases of Brest University Hospital between January 2013 and December 2018.Main outcome measures: Placental histological findings, course of pregnancies, maternal and fetal characteristics were described and compared between cases (placental abruption with stillbirth or neonatal death) and controls.Results: We identified 135 placental abruption, of whom 24.4% were complicated with stillbirth and 6.5% with neonatal death. Forty percent of women were smokers and 14.1% had a history of vasculoplacental disorder. Pregnancies were complicated with 42.2% of pre-eclampsia and 43% of intrauterine growth restriction. Cases were associated with more autoimmune diseases in mother (20.0% versus 3.2%, P = 0.003), more aspirin or heparin use during pregnancy (20.0% versus 6.3%, P = 0.03), less pre-eclampsia (25.0% versus 49.5%, P = 0.01) and more deliveries ≤ 34 weeks of gestation (80.0% versus 43.2%, P = 0.0001) than controls. Placentas from cases showed more placental indentation ≥ 30% (42.5% versus 5.3%, P < 0.0001) and less histological chronic inflammation, especially less chronic chorioamniotitis (2.5% versus 24.2%, P = 0.002) than controls. In multivariate analysis, factors negatively associated with poor fetal outcome were placental histological chronic inflammation (P = 0.01) and macroscopic infarcts (P = 0.01).Conclusions: Poor fetal outcome is negatively associated with certain placental histological chronic lesions, but not with pre-eclampsia, what suggests various pathophysiological processes among placental abruption.

Abstract: Metabolic myopathies comprise a diverse group of inborn errors of intermediary metabolism affecting skeletal muscle, and often present clinically as an inability to perform normal exercise. Our aim was to use the maximal mechanical performances achieved during two functional tests, isometric handgrip test and cycloergometer, to identify metabolic myopathies among patients consulting for exercise-induced myalgia. Eighty-three patients with exercise-induced myalgia and intolerance were evaluated, with twenty-three of them having a metabolic myopathy (McArdle, n = 9; complete myoadenylate deaminase deficiency, n = 10; respiratory chain deficiency, n = 4) and sixty patients with non-metabolic myalgia. In all patients, maximal power (MP) was determined during a progressive exercise test on a cycloergometer and maximal voluntary contraction force (MVC) was assessed using a handgrip dynamometer. The ratio between percent-predicted values for MVC and MP was calculated for each subject (MVC%pred:MP%pred ratio). Abstract: In the context of raw materials valorization, reducing pollution and producing more energy, the world is undergoing a revolutionary shift towards biodegradable materials. Biosourced materials used for biocomposites gain more and more attention regarding different properties and are means of improving properties, especially mechanical ones. The aim of this research was to evaluate the effect of various enzymatic treatments (xylanase, pectinase and xylanase + pectinase) on chemical structures, morphology and mechanical properties of date palm fiber (DPF) and reinforced DPF composites. Two types of palm residues, namely: palm and trunk, were subject of several experiments. The reaction time of fiber-enzyme treatments and enzyme activities were optimized by measuring the liberated reduced sugar. The treatments effect on the fibers surface was also examined by a Scanning Electron Microscope. Different fiber contents and fiber treatments substantially improved mechanical properties (Tension and impact resistance). Results have shown that an enzyme treatment for 8 h is enough to liberate the maximum of amorphous components. The treatment combining pectinase + xylanase enzymes contributed to more fibrillose structures and proper surface which indicates an effective elimination of lignin, hemicellulose and extractive fractions. The palm variety exhibits relatively high values of cellulose compared to trunk fiber. It was found that enzymatically treated date palm fibers enhance the composite materials rigidity by about 42 % and 29 % for trunk and palm fibers, respectively. Sustainable products with such characteristics could enhance the development and progress of sustainable industries.

Abstract: Several studies have shown that adequate bioclimatic information is of major importance for mapping ecological niches or for modelling the distribution ranges of species and communities, particularly from a climate change perspective [1,2]. However, in France, there are few data sources that provide consistent information, available data being produced at low spatial resolution and based on classification systems that are not suitable for mapping French ecological systems. This paper presents bioclimatic maps produced on Metropolitan France and based on the Worldwide Bioclimatic Classification System, which are called Global Bioclimatics [3]. This data paper documents a set of variables that includes 23 bioclimatic maps generated according to the Worldwide Bioclimatic Classification System. These maps describe current bioclimatic conditions in Metropolitan France at a resolution of 30 arc-seconds. Climatic parameters and bioclimatic indices usually used for the analysis or modelling of species and communities' distribution, and bioclimatic typological units, were calculated using the temperature and precipitation data derived from the WorldClim 2 model. These maps can be used in GIS or models by researchers for mapping ecological conditions, but can also provide natural resource managers with analytical tools to assess Nature conservation policies.

Abstract: International audience; The population of patients with congenital heart disease (CHD) is continuously increasing, and a significant proportion of these patients will experience arrhythmias because of the underlying congenital heart defect itself or as a consequence of interventional or surgical treatment. Arrhythmias are a leading cause of mortality, morbidity and impaired quality of life in adults with CHD. Arrhythmias may also occur in children with or without CHD. In light of the unique issues, challenges and considerations involved in managing arrhythmias in this growing, ageing and heterogeneous patient population and in children, it appears both timely and essential to critically appraise and synthesize optimal treatment strategies. The introduction of catheter ablation techniques has greatly improved the treatment of cardiac arrhythmias. However, catheter ablation in adults or children with CHD and in children without CHD is more technically demanding, potentially causing various complications, and thus requires a high level of expertise to maximize success rates and minimize complication rates. As French recommendations regarding required technical competence and equipment are lacking in this situation, the Working Group of Pacing and Electrophysiology of the French Society of Cardiology and the Affiliate Group of Paediatric and Adult Congenital Cardiology have decided to produce a common position paper compiled from expert opinions from cardiac electrophysiology and paediatric cardiology. The paper details the features of an interventional cardiac electrophysiology centre that are required for ablation procedures in adults with CHD and in children, the importance of being able to diagnose, monitor and manage complications associated with ablations in these patients and the supplemental hospital-based resources required, such as anaesthesia, surgical back-up, intensive care, haemodynamic assistance and imaging. Lastly, the need for quality evaluations and French registries of ablations in these populations is discussed. The purpose of this consensus statement is therefore to define optimal conditions for the delivery of invasive care regarding ablation of arrhythmias in adults with CHD and in children, and to provide expert and - when possible - evidence-based recommendations on best practice for catheter-based ablation procedures in these specific populations.

Abstract: The use of 68Ga-labelled carbon nanoparticles has been proposed for lung ventilation PET/CT imaging. However, no study has assessed the physical properties of 68Ga-labelled carbon nanoparticles. The aim of this study therefore was to evaluate the shape and size of 68Ga-labelled carbon nanoparticles, and to determine the composition of the aerosol, as opposed to 99mTc-labelled carbon nanoparticles aerosol. 99mTc- and 68Ga-labelled carbon nanoparticles, stable gallium carbon nanoparticles, 0.9 % NaCl and 0.1 N HCl-based carbon nanoparticles were produced using an unmodified Technegas® generator, following the usual technique used for clinical Technegas® production. The shape and size of particles were studied by transmission electron microscopy (TEM) after decay of the radioactive samples. The composition of the 68Zn- and 99Tc-labelled carbon nanoparticles aerosols was assessed using scanning electron microscopy (SEM) coupled with energy dispersive X-ray (EDX) analysis after decay of the 68Ga- and 99mTc-labelled carbon nanoparticles, respectively. On TEM, all samples showed similar shape with hexagonally structured primary particles, agglomerated in clusters. The mean diameters of primary stable gallium carbon nanoparticles, 99Tc- and 68Zn-labelled carbon nanoparticles were 22.4 ± 10 nm, 20.9 ± 7.2 nm and 19.8 ± 11.7 nm, respectively. Using Technegas® generator in the usual clinical way, 99mTc- and 68Ga-labelled carbon nanoparticles demonstrated similar shape and diameters in the same size range size. These results support the use of 68Ga-labelled carbon nanoparticles for the assessment of regional lung ventilation function with PET imaging.

Abstract: Introduction Type II AEG is now considered as oesophageal cancer in the seventh and eighth edition of TNM classification but optimal surgical approach for these tumors remains debated. The objective of the study is to assess and compare surgical and oncological outcomes of two surgical approaches: superior polar oesogastrectomy (SPO) or total gastrectomy (TG) in patients with type II adenocarcinoma of the oesophagogastric junction (AEG). Material and methods 183 patients with type II AEG treated from 1997 to 2010 in 21 French centers by SPO or TG were included in a multicenter retrospective study. The surgical and oncological outcomes were compared between these two surgical approaches. Results A TG was performed in 64 (35%) patients whereas 119 (65%) patients were treated by SPO with transthoracic approach in 100 of them (83.2%) and transhiatal approach with cervicotomy in 19 (16.8%). Surgical outcomes were comparable between the two approaches with a postoperative mortality rate of 4.9% and a severe operative morbidity rate within 30 days of 15.3%. Median survival in patients operated on by TG was of 46 months compared to 27 months in patients treated by SPO (p = 0.118). At multivariate analysis, TG appears to be an independent good prognostic factor compared to SPO (HR = 1.847; p = 0.008). However, TG was also associated with a higher rate of incomplete resection, (12.5% vs 5.9%; p = 0.120). Conclusion When TG allows obtaining tumor-free resection margins, this approach should be preferred to SPO.

Abstract: International audience; Background - Women show greater mortality after acute myocardial infarction. We decided to investigate whether gender affects delays and impacts in-hospital mortality in a large population.Methods and results - We performed a patient-level analysis of 7 French MI registries from different regions from January 2005 to December 2012. All patients with acute STEMI were included within 12 h from symptom onset and a first medical contact with a mobile intensive care unit an emergency department of a hospital with percutaneous coronary intervention facility. Primary study outcomes were STEMI, patient and system, delays. Secondary outcome was in-hospital mortality. 16,733 patients were included with 4021 females (24%). Women were significantly older (mean age 70.6 vs 60.6), with higher diabetes (19.6% vs 15.4%) and hypertension rates (58.7% vs 38.8%). Patient delay was longer in women with adjusted mean difference of 14.4 min (p Conclusions - This overview of 16,733 real-life consecutive STEMI patients in prospective registries over an extensive period strongly indicates gender-related discrepancies, highlighting clinically relevant delays in seeking medical attention. However, higher in-hospital mortality was not totally explained by clinical characteristics or delays. Dedicated studies of specific mechanisms underlying this female disadvantage are mandatory to reduce this gender gap.

Abstract: Background Venous thromboembolism (VTE) can be the first manifestation of cancer; however, the current incidence of malignancy in unselected patients with first unprovoked VTE needs to be confirmed. Material and methods Between March 1st, 2013 and February 28th, 2015 we included and followed-up all patients living in the Brest district, France, who were seen in hospitals or the community for a first symptomatic unprovoked VTE event. The primary study outcome was the one-year incidence of cancer. Results 526 patients, mean age 66.6 ± 18.1 years, 246 (46.8%) men, were included in the study. In the year following VTE, 26 patients were diagnosed with cancer, corresponding to a one-year cumulative incidence of cancer of 5.06% (95% CI 3.47–7.35). Age ≥60, smoking and pulmonary embolism were significantly associated with cancer diagnosis in multivariate analysis. Fifty percent of cancers were patent at the time of VTE diagnosis, mostly detected on CTPA (Computed Tomographic Pulmonary Angiography) performed for pulmonary embolism assessment. After excluding patients with patent cancer at VTE diagnosis, the one-year incidence of cancer was 2.65% (95% CI: 1.55–4.52); in multivariate analysis, only current smoking was independently associated with a significant 5.4-fold increased risk for cancer diagnosis (HR 5.40; 95% CI 1.31–22.27). No cancer was diagnosed in patients aged 50 years or younger. Conclusion The one-year incidence of cancer after a first unprovoked VTE was 5.06%. Half of the cancers were diagnosed during the diagnosis procedure for pulmonary embolism using CTPA.

Abstract: The purpose of this study was to identify the incidence, causes and impact of non-adherence to oral and subcutaneous chronic treatments for patients with polycythemia vera or essential thrombocythemia. Patients receiving cytoreductive drugs for polycythemia vera or essential thrombocythemia were recruited at our institution (Observatoire Brestois des Néoplasies Myéloprolifératives registry). They completed a one-shot questionnaire designed by investigators (Etude de l’Observance Thérapeutique et des Effets Secondaires des Traitements study). Abstract: Summary Background The ability of the pulmonary embolism rule-out criteria (PERC) to exclude pulmonary embolism without further testing remains debated outside the USA, especially in the population with suspected pulmonary embolism who have a high prevalence of the condition. Our main objective was to prospectively assess the predictive value of negative PERC to rule out pulmonary embolism among European patients with low implicit clinical probability. Methods We did a multicentre, prospective, observational study in 12 emergency departments in France and Belgium. We included consecutive patients aged 18 years or older with suspected pulmonary embolism. Patients were excluded if they had already been hospitalised for more than 2 days, had curative anticoagulant therapy in progress for more than 48 h, or had a diagnosis of thromboembolic disease documented before admission to emergency department. Physicians completed a standardised case report form comprising implicit clinical probability assessment (low, moderate, or high) and a list of risk factors including criteria of the PERC rule. They were asked to follow international recommendations for diagnostic strategy, masked to PERC assessment. The primary endpoint was the proportion of patients with low implicit clinical probability and negative PERC who had venous thromboembolic events, diagnosed during initial diagnostic work-up or during 3-month follow-up, as externally adjudicated by an independent committee masked to the PERC and clinical probability assessment. The upper limit of the 95% CI around the 3-month thromboembolic risk was set at 3%. We did all analyses by intention to treat, including all patients with complete follow-up. This trial is registered with ClinicalTrials.gov, number NCT02360540. Findings Between May 1, 2015, and April 30, 2016, 1773 consecutive patients with suspected pulmonary embolism were prospectively assessed for inclusion, of whom 1757 were included. 1052 (60%) patients were classed as having low clinical probability, 49 (4·7%, 95% CI 3·5–6·1) of whom had a venous thromboembolic event. In patients with a low implicit clinical probability, 337 (32%) patients had negative PERC, of whom four (1·2%; 95% CI 0·4–2·9) went on to have a pulmonary embolism. Interpretation In European patients with low implicit clinical probability, PERC can exclude pulmonary embolism with a low percentage of false-negative results. The results of our prospective, observational study allow and justify an implementation study of the PERC rule in Europe. Funding French Ministry of Health.

Abstract: BACKGROUND: Coronary artery disease (CAD) is common in patients undergoing transcatheter aortic valve replacement (TAVR). However, the impact of CAD distribution before TAVR on short‐ and long‐term prognosis remains unclear. HYPOTHESIS: We hypothesized that the long‐term clinical impact differs according to CAD distribution in patients undergoing TAVR using the FRench Aortic National CoreValve and Edwards (FRANCE‐2) registry. METHODS: FRANCE‐2 is a national French registry including all consecutive TAVR performed between 2010 and 2012 in 34 centers. Three‐year mortality was assessed in relation to CAD status. CAD was defined as at least 1 coronary stenosis >50%. RESULTS: A total of 4201 patients were enrolled in the registry. For the present analysis, we excluded patients with a history of coronary artery bypass. CAD was reported in 1252 patients (30%). Half of the patients presented with coronary multivessel disease. CAD extent was associated with an increase in cardiovascular risk profile and in logistic EuroSCORE (European System for Cardiac Operative Risk Evaluation) (from 19.3% ± 12.8% to 21.9% ± 13.5%, P < 0.001). Mortality at 30 days and 3 years was 9% and 44%, respectively, in the overall population. In multivariate analyses, neither the presence nor the extent of CAD was associated with mortality at 3 years (presence of CAD, hazard ratio [HR]: 0.90; 95% confidence interval [CI]: 0.78‐1.07). A significant lesion of the left anterior descending (LAD) was associated with higher 3‐year mortality (HR: 1.42; 95% CI: 1.10‐1.87). CONCLUSIONS: CAD is not associated with decreased short‐ and long‐term survival in patients undergoing TAVR. The potential deleterious effect of LAD disease on long‐term survival and the need for revascularization before or at the time of TAVR should be validated in a randomized control trial.

Abstract: Purpose: Inadequate clinical target volume (CTV) definition is likely to be a major contributing factor to local recurrence (LR) rate after radiotherapy. Our aims were to identify sites of prostate cancer LR in biochemical recurrence post-prostatectomy using 18F-Fluorocholine (18F-FCH) positron emission tomography/computed tomography (PET/CT) and to compare different CTV-delineation guidelines in a cohort of postoperative patients.Material and methods: Thirty-six patients presenting with LR within the prostatic bed on 18F-FCH PET/CT between 10/2011 and 06/2016 were included in this retrospective study. Median PSA at the time of 18F-FCH PET/CT was 2.7 ng/mL (0.8–9.4) and median PSA doubling time was 11 months (3–28). Abstract: Direct oral anticoagulants (DOACs) have been approved to treat and prevent thrombotic events. However, they are not yet labeled for use in patients with active cancers. Myeloproliferative neoplasms (MPNs) are clonal chronic disorders with a high incidence of thrombotic events, for which low-dose aspirin (LDA) is the standard drug treatment. We analyzed efficacy and safety of DOACs prescription in patients treated for MPNs. An MPN database, the OBENE registry, was established at our institution. We collected biological and clinical data from diagnosis to last follow-up for every patient included in this study. Thrombotic and hemorrhagic events and hematologic evolutions were categorized as major events in the database. Of the 760 MPN patients in the OBENE registry, 25 (3.3%) were treated with a DOAC. Median follow-up duration was 2.1 years (0.12-4.3 years). The reasons for prescribing DOACs were atrial fibrillation and thrombotic events for 13 and 12 patients, respectively. We only observed one thrombotic event (4%) and three major hemorrhagic events (12%). A case-control study did not detect a significant difference in thrombotic or hemorrhagic events in patients treated with LDA and DOACs. These preliminary results suggest that DOACs may be highly efficient and safe for use in MPN patients.

Abstract: Introduction The optimal management of oral contraception and menstrual bleeding during treatment of venous thromboembolism (VTE) is largely unknown. We aimed to elicit expert opinion and compare that to current practice as assessed by a world-wide international web-based survey among physicians. Methods 10 international thrombosis experts and 10 abnormal uterine bleeding experts independently completed a questionnaire containing three hypothetical patient cases each with four different scenarios, and additional queries covering different severities of VTE, patient circumstances, hormonal contraceptives and both thrombotic and bleeding complications. The consensus percentage was set a priori at ≥ 70%. The same questionnaire with randomized case scenarios was presented to international physicians via newsletters of the ISTH and national scientific communities. Differences between the expert groups and daily clinical care were assessed. Results Expert recommendations were divergent and differed in several important points from clinical practice. In contrast to common practice in which contraceptives are discontinued at the moment of a VTE diagnosis, the thrombosis experts agreed to continue oral contraception (OC) during the anticoagulation treatment period. Also, experts reached consensus on treating patients with anticoagulation-associated abnormal uterine bleeding with tranexamic acid, although this is not supported by strong evidence from the literature. No consensus was reached on the optimal anticoagulant drug class. Conclusions International experts' opinions on handling of contraceptives and management of anticoagulant-associated abnormal uterine bleeding in female VTE patients are divergent and management in clinical practice is heterogeneous. There is a great need of further studies on these topics.

These computations are based on finite element (FE) simulations of polycrystalline aggregates made of grains with hexagonal crystalline structure. A detailed statistical analysis has been performed for a specific grain located at the center of the aggregate by varying its neighboring grains. Comparisons are performed at the overall and local scales between simulations using the affine extension of the self-consistent scheme and the FE results for tensile and creep loading tests.

Abstract: Background: The aim was to describe genetic, clinical and morphological features in a large, multicentre European cohort of patients with SPINK1 related pancreatitis, in comparison with patients with idiopathic pancreatitis (IP). Methods: All SPINK1 mutation carriers with pancreatic symptoms from two French and one English centers were included. Patients with IP were included in a control group. Abstract: International audience; Cationic amphiphiles featuring two thioether functions in each lipid chain of bicatenar cationic amphiphiles are reported here for the first time. The physicochemical properties and transfection abilities of these new amphiphiles were compared with those of already reported analogues featuring either (i) saturated, (ii) unsaturated or (iii) mono-thioether containing lipid chains. The homogeneity of the series of new compounds allowed to clearly underscore the effect of bis-thioether containing lipid chains. This study shows that besides previous strategies based on unsaturation or ramification, the incorporation of two thioether functions per lipid chain constitutes an original complementary alternative to tune the supramolecular properties of amphiphilic compounds. The potential of this strategy was evaluated in the context of gene delivery and report that two cationic amphiphiles (i. e. 4 a and 4 b) can be proposed as new efficient transfection reagents.

Abstract: Objectives Manual systematic literature reviews are becoming increasingly challenging due to the sharp rise in publications. The primary objective of this literature review was to compare manual and computer software using artificial intelligence retrieval of publications on the cutaneous manifestations of primary SS, but we also evaluated the prevalence of cutaneous manifestations in primary SS. Methods We compared manual searching and searching with the in-house computer software BIbliography BOT (BIBOT) designed for article retrieval and analysis. Both methods were used for a systematic literature review on a complex topic, i.e. the cutaneous manifestations of primary SS. Reproducibility was estimated by computing Cohen’s κ coefficients and was interpreted as follows: slight, 0–0.20; fair, 0.21–0.40; moderate, 0.41–0.60; substantial, 0.61–0.80; and almost perfect, 0.81–1. Results The manual search retrieved 855 articles and BIBOT 1042 articles. In all, 202 articles were then selected by applying exclusion criteria. Among them, 155 were retrieved by both methods, 33 by manual search only, and 14 by BIBOT only. Reliability (κ = 0.84) was almost perfect. Further selection was performed by reading the 202 articles. Cohort sizes and the nature and prevalence of cutaneous manifestations varied across publications. In all, we found 52 cutaneous manifestations reported in primary SS patients. The most described ones were cutaneous vasculitis (561 patients), xerosis (651 patients) and annular erythema (215 patients). Conclusion Among the final selection of 202 articles, 155/202 (77%) were found by the two methods but BIBOT was faster and automatically classified the articles in a chart. Combining the two methods retrieved the largest number of publications.

Abstract: Hemophilia is a rare hemorrhagic disorder caused by clotting factor deficiencies that leads to a less efficient coagulation system. Treatments of this pathology rely on a patient’s subjective assessment which reflects a need for a laboratory assay able to predict the clinical patient phenotype. According to the literature, global assays such as thrombin generation (TG), are good predictors of bleeding episodes and therefore seem to be good candidates to fit this need. Nevertheless, the result of the TG assay, known as thrombogram, is difficult to interpret for non-expert clinicians. In this paper, we present a machine learning-based clinical decision support system which goal is to help clinical decision making. In doing so, we have adopted several approaches in order to evaluate well-known machine learning algorithms, in terms of accuracy and robustness, on a thrombogram database generated using numerical simulations. Obtained results, 95.57% of accuracy using a cascade of a SVM and MLPs to classify all categories and 98.10% of accuracy for the binary case hemophilia A/B, prove that our proposal can efficiently diagnose hemophilia.

Abstract: Lightweight sandwich panels with composite facesheets and foam core have high impact energy absorption capability and are widely employed in multifunctional applications such as aircraft and marine structures. The dynamic behaviour of sandwich panels is typically studied for impact loading at normal angle of incidence but the structures are more frequently loaded at some oblique angle or with a complex tri-dimensional trajectory in real engineering situations. The damage area and damage modes for these trajectories are significantly different and it is not sufficient to study only normal impacts. There are well established experimental protocols for normal or oblique impact tests using devices like the drop tower, but impacts with complex trajectory are difficult to characterise experimentally. In this paper, a Gough-Stewart platform with six degrees of freedom has been modified to develop an original tri-dimensional impact device called Hexapod. The trajectory is defined to an impactor attached to the seventh jack of the Hexapod to study the response of sandwich plates to impact loading with complex trajectories. The applicability of the newly developed device is demonstrated by studying parabolic impact with different trajectories on sandwich plates with Kevlar facesheets and Rohacell foam core. The time history of vertical and horizontal components of force is measured using tri-axial load cell and strain history is obtained from Digital Image Correlation of a high speed camera images. The results of the parabolic impact show the importance of shear behaviour of the foam in the progression of damage in the sandwich panels. Additionally, the response of the sandwich panels to parabolic impact was simulated numerically using explicit finite element code LS-DYNA. The results of the FE model are compared with experimental data in terms of the force history and strain contours.

Abstract: International audience; Introduction The main current research interest in the field of composite materials consists in minimizing the environmental impact of both materials and manufacturing. Having a good control of (bio)composite manufacturing implies a good knowledge of wetting phenomena occurring during impregnation of (natural) fibres by thermoset or thermoplastic polymers. It is known that capillary effects, and particularly capillary parameters like molten polymer surface tension and fibre surface energy, play an important role on impregnation. These effects coupled with the viscous ones are dominant during composite manufacturing [1,2]. The visco-capillary balance in dynamic wetting can be described by a hydrodynamic approach. In the literature, the de Gennes and Cox-Voinov theories [3] were used to fit experimental data (obtained mainly by optical methods) of liquid drops spreading on model substrates at standard conditions (room temperature). This procedure is not suitable in the case of fibres wetted by molten polymers for composite applications. Moreover, the influence of temperature and polymer molecular weight still remain unknown. The aim of this study was to set an experimental procedure that can be applied to fibres and polymers to investigate wetting dynamic and identify physical parameters such as a slip length, that can be inserted into numerical models to predict flow process [4]. The experimental procedure was validated on cellulosic films with different liquids. Paraffin oils are totally wetting liquids and they were used in order to evaluate the influence of temperature on wetting dynamics. Polyethylene glycols (PEGs) are partially wetting liquids; they were used to evaluate the effect of molecular weights (Mn) on wetting dynamics of molten polymers. An independent characterisation of the solid and of the liquids was carried out as well as solid/liquid dynamic wetting tests. Hydrodynamic models were used to fit the dynamic contact angle data. The results are meaningful for a better understanding of the influence of temperature, viscosity and liquid molecular weight on the dynamic contact angles and on capillary phenomena existing during the impregnation of fibrous reinforcements in composite processing.
