Objective: To characterize the clinical-epidemiological profile of patients admitted to the adult intensive care unit of a teaching hospital. Method: Descriptive epidemiological study, with a quantitative approach, cross-sectional and retrospective nature. The research was carried out at the teaching hospital, in an adult intensive care unit between January and December 2021. Results: 153 people were hospitalized. There was a predominance of males (68.6%). The average age was 53 years (standard deviation=20). Regarding the municipality of residence, Maringá predominated (39.9%). The average length of stay was 17 days (median=11; standard deviation=18). Regarding the primary cause of admission, it was noted that the most frequent causes were trauma (14.4%). Regarding the outcome of hospitalization, although the majority of participants were discharged from the sector (58.8%), there was an important proportion who died (37.3%). Conclusion: Predominance of males, with an average age of 53 years, from the city of Maringá, hospitalized for around 17 days due to trauma and/or fractures, which required intubation and without vasoactive drugs and hemodialysis, with a more rise in the sector is frequent, although the mortality rate was 37%, raising a warning for managers. Objetivo: Evaluar el efecto de la movilización activa temprana (MAT) en comparación con el cuidado habitual activo (CHA) en sujetos adultos críticamente enfermos respecto de la fuerza muscular y el estado funcional. Materiales y métodos: Se realizó una revisión sistemática y un metaanálisis. Se incluyeron ensayos clínicos aleatorizados que compararon la MAT con el CHA en sujetos >18 años, de ambos sexos, en estado crítico durante o después del período de ventilación mecánica en la unidad de cuidados intensivos (UCI). La búsqueda se realizó en las bases de datos Cochrane, Medline/Pubmed, Embase (Elsevier), SciELO, LILACS/BVS, www.controlled-trials.com/, clinicaltrials.gov/, www.who.int/ictrp/en/. Se analizó el riesgo de sesgo mediante la herramienta RoB 1. Se generó una síntesis cualitativa y cuantitativa (metaanálisis) de los resultados. Registro: OSF: https://bit.ly/430fF72. Resultados: Se incluyeron siete estudios, con un total de 1905 sujetos. La mayoría de los sujetos eran del sexo masculino (57 %) y tenían entre 54 y 66 años. Los desenlaces reportados mostraron baja certeza de evidencia, según el sistema de clasificación de la evaluación, desarrollo y valoración de las recomendaciones (GRADE, por sus siglas en inglés). En el metaanálisis del estado funcional al alta hospitalaria, se observó un efecto a favor del grupo MAT, con una diferencia de medias estandarizada de 0,26 (IC 95 % 0,11-0,41) y baja heterogeneidad (I2 = 0 %). Conclusión: Con una baja certeza de evidencia, el efecto de la MAT en sujetos adultos críticamente enfermos en la UCI podría mejorar el estado funcional al alta hospitalaria en comparación con el CHA.

Background: Interruptions have been reported to contribute to clinical errors and procedural failures. Objective: To analyze the interruptions experienced by nurses during the preparation and administration of high-risk medications. Methodology: A cross-sectional study was conducted in an intensive care and inpatient unit. The interruptions experienced by nurses during the medication process were observed through two checklists. The sample was selected by convenience in April-May 2019. Descriptive statistics was used to analyze quantitative data in IBM SPSS Statistics software, version 24.0, while content analysis was used to analyze qualitative data. Results: In 193 medication processes, there were 137 interruptions. Other members of the healthcare team initiated most interruptions through conversations. These interruptions were mostly negative and occurred during the preparation phase. The multitasking strategy was used to manage them. Conclusion: Interruptions during the medication process were primarily associated with professional and social communications. Objective: to verify the content validity of the nursing diagnosis proposal Ocular dryness in adult patients admitted to an intensive care unit. Materials and methods: this is a methodological study of content validation of the nursing diagnosis proposal Ocular dryness, operationalized through a focus group. The selection of specialists took into account the clinical and/or academic experience in the area of nursing diagnoses and/or ocular dryness and/or dry eye and/or ocular health, as well as the time of performance with the theme. Thirteen nurses who met the criteria described above were invited, of whom 10 agreed to participate. The validation approach was by consensus. Data were analyzed using descriptive statistics, level of expertise and analysis of diagnostic elements. Results: after the final consensus of the nurse judges in relation to the diagnostic elements, a proposal of the diagnosis Ocular dryness was defined based on the validity of content with a new definition, 14 defining characteristics, 9 related factors, 2 populations at risk and 20 associated conditions. In addition, after judging the coherence of the elements in relation to the diagnostic structure, the judges issued the consensus in relation to the conceptual and operational definitions. Conclusions: the study allowed verifying the validity of the content by judges of the nursing diagnosis proposal Ocular dryness in patients in intensive care units, which favors the diagnostic reasoning of nurses and the planning of effective interventions related to this diagnosis, allowing the management of the patients in order to provide ocular integrity. Introduction: at present, when the use of information and technology is essential for decision making, the data has become a resource of a lot of value to boost the success of hospital entities and improve the quality of life of the people. Objective: describe the "Dynamic data set generator for intensive care service" software. Methods: technological development research in the Intensive Care Service of the "Arnaldo Milián Castro" Hospital in the 2017 to 2022 period. The defined population were all the specialists (12) of the intensive therapy room that used the system since January of January 2017 to December 2022. Results: it allows to collect clinical data of patients from the intensive therapy room and generate dynamic data sets for intensive care specialists and data analysis. Conclusions: system in exploitation since 2017, the data sets generated have been used for the application of automatic learning techniques, which has enhanced medical research and timely decision making. Marco contextual: Se ha reportado la participación de distracciones en errores clínicos y fallos de procedimiento. Objetivo: Analizar las distracciones del personal de enfermería durante la preparación y administración de fármacos de alto riesgo. Metodología: Estudio transversal desarrollado en una unidad de cuidados intensivos y una unidad de hospitalización. Se observaron distracciones del personal de enfermería durante el proceso de medicación através de dos listas de control. La muestra fue seleccionada por conveniencia (abril-mayo 2019). Los datos cuantitativos se analizaron mediante estadística descriptiva (IBM SPSS Statistics, versión 24.0). Los datos cualitativos se analizaron mediante análisis de contenido. Resultados: Hubo 137 distracciones en 193 procesos de medicación. La mayoría de las distracciones fueron iniciadas por otros miembros del equipo sanitario a través de conversaciones. La mayoría se produjeron en la fase de preparación y fueron negativas y se gestionaron mediante la estrategia multitarea. Conclusión: Las distracciones durante el proceso de medicación se referían principalmente a las comunicaciones profesionales y sociales. La importancia de esas distracciones variaba en función de la fase del proceso.

Introducción: desde 2006, la encuesta del nutritionDay liderada por la European Society of Clinical Nutrition and Metabolism (ESPEN) y la Universidad de Viena ha permitido evaluar las prácticas nutricionales en pacientes en unidades de cuidados intensivo (UCI) y salas generales. En este artículo se analizan la estructura de las UCI y se examinan las características clínicas y nutricionales de pacientes críticamente enfermos en Colombia. Método: estudio observacional de corte transversal que involucró a 13 UCI y 183 pacientes que participaron en el nutri- tionDay 2021. Se llevó a cabo un análisis descriptivo de variables estructurales y demográficas, clínicas y nutricionales de los pacientes utilizando el software estadístico Stata. Resultados: el 84,6 % de las UCI eran polivalentes, y las patologías cardíacas (27,3 %) y neurológicas (18,8 %) fueron las más frecuentes. La edad media fue de 61,5 años, el 56,9 % de sexo masculino y un peso promedio de 66 kg. El 69,2 % de las instituciones contaba con una unidad de nutrición, el 100 % tenía profesionales en nutrición y dietética, y el 92,3 % tenía procedimientos de atención nutricional por escrito. En cuanto a las prácticas nutricionales, el 92,3 % de las UCI evalúa el estado nutricional utilizando parámetros de laboratorio y el 76,9 % con peso/talla. La nutrición enteral se utilizó con mayor frecuencia (33,5 %) en comparación con la nutrición parenteral (8,1 %), con una duración promedio alrededor de 4 días. El promedio de estancia fue de 11 días en la UCI y 22,5 días en el hospital. Conclusiones: este estudio identificó debilidades en la estructura e implementación de los protocolos de nutrición en las UCI. Esto resalta la necesidad de promover prácticas adecuadas para la intervención nutricional en pacientes críticos lo que podría impactar positivamente en los resultados de salud y en la eficiencia de los costos de atención médica en el futuro.

- Abstract
Objective: To evaluate the association of comprehensive assessment scores with mortality in geriatric patients of a national reference hospital in Peru. The study: A prospective cohort study was conducted. Patients over 60 years of age were included during the period from November 1, 2021 to June 30, 2022. The data was collected from the clinical history. SOFA, KATZ, FAST and Charlson Index scores were included to evaluate the association with mortality. Findings: 93 participants were found. The prevalence of mortality was 32.3%. In the simple regression, an association was observed between SOFA score greater than or equal to 12 (RR:15.3), FAST score (RR:2.29) and the Charlson index (RR:2.46) with intrahospital mortality. In the multiple regression adjusted for age and sex, only the SOFA score maintained statistical significance (RR: 13.42 p<0.001). Conclusion: The SOFA score has a strong association with in-hospital mortality in geriatric patients admitted to an intensive care unit in Peru. [ABSTRACT FROM AUTHOR]

La lesión renal aguda (LRA) es una entidad común en los pacientes críticamente enfermos. Dicha entidad puede ocurrir entre el 30?% y el 60?% de los pacientes en la unidad de cuidados intensivos (UCI), confiriéndoles un mal pronóstico clínico. Muchos pacientes en estado crítico con LRA son sometidos a terapia de reemplazo renal (TRR), sin embargo, en ausencia de indicaciones absolutas como edema agudo de pulmón refractario a diuréticos y trastornos metabólicos importantes como uremia, acidosis e hiperkalemia grave, el momento óptimo para el inicio de la TRR con el fin de mejorar los desenlaces clínicos sigue siendo incierto y aún es motivo de debate. En los últimos años, múltiples estudios se han centrado en determinar el momento idóneo para el inicio de la TRR en el contexto del paciente crítico con LRA. Varios estudios observacionales y pequeños ensayos controlados aleatorizados sugieren que el inicio temprano de la TRR puede mejorar la supervivencia, sin embargo, ensayos más recientes no han encontrado diferencias estadísticamente significativas en términos de mortalidad entre una estrategia temprana y una tardía para el inicio de la TRR en pacientes críticos con LRA grave.

25. Prevention of ventilator-associated pneumonia by a nose care program combining with oral care among patients hospitalized in intensive care units: a single-blind randomized controlled trial

Objective: Ventilator-associated pneumonia (VAP) is one of the most common ICU-acquired infections. Preventing nasal canal colonization through an effective nasal cavity care, along with oral care seem to be an important issue. Methods: This single-blind randomized controlled trial was conducted on 31 patients in each intervention and control group in Alzahra and Kashani hospitals, Iran. The interventional program was considered every 8 h for 5 days. It included cleaning the nasal cavities with cotton swabs soaked in sterile normal saline, then 2 puffs of 65% sodium chloride nasal spray were used for each nostril, and finally the nasal canal was moistened with a swab dipped in Veramin gel (0.5 ml into each nostril). For the control group, routine nasal care including cleaning the outer nostrils was offered. Oral care in 2 groups was performed according to the standard protocol. Data collected through demographic and clinical questionnaire as well as modified pulmonary infection clinical scale. The chi-square and independent tests were used to determine the homogeneity of basic characteristics. Also, we estimated and compared the incidence of VAP between 2 groups by chi-square test. Results: The incidence of VAP was not statistically different in the intervention and control groups on the third day after intervention (29.1% vs. 32.3%, respectively, P = .915), while this rate on the sixth day was significantly lesser in the intervention than control (32.3 vs. 58%, P = .041). Conclusion: The present nasal care program along with oral care is an effective strategy to prevent VAP. Resumen: Objetivo: La neumonía asociada al ventilador (NAV) es una de las infecciones adquiridas en la UCI más comunes. La prevención de la colonización del canal nasal a través de un cuidado efectivo de la cavidad nasal, junto con el cuidado oral, parece ser un tema importante. Métodos: Este ensayo controlado aleatorio simple ciego se realizó en 31 pacientes en cada grupo de intervención y control en los hospitales de Alzahra y Kashani, Irán. Se consideró el programa intervencionista cada 8 horas durante 5 días. Incluyó la limpieza de las fosas nasales con hisopos empapados en solución salina normal estéril, luego se aplicaron dos inhalaciones de cloruro de sodio al 65% en aerosol nasal por cada fosa nasal y finalmente se humedeció el canal nasal con un hisopo humedecido en gel Veramin (0,5 ml en cada uno). fosa nasal). Para el grupo de control, se ofreció atención nasal de rutina, incluida la limpieza de las fosas nasales externas. El cuidado oral en dos grupos se realizó de acuerdo con el protocolo estándar. Además, estimamos y comparamos la incidencia de VAP entre dos grupos mediante la prueba de chi cuadrado. Resultados: La incidencia de VAP no fue estadísticamente diferente en los grupos de intervención y control al tercer día después de la intervención (29,1% vs. 32,3%, respectivamente, p = 0,915), mientras que esta tasa al sexto día fue significativamente menor en el grupo de intervención. que el control (32,3 vs. 58%, p = 0,041). Conclusión: El presente programa de cuidado nasal junto con el cuidado oral es una estrategia efectiva para prevenir la VAP.

El objetivo del presente estudio fue describir las características de los pacientes pediátricos (entre 28 días y 14 años) hospitalizados con COVID-19 durante la tercera ola (variante ómicron) en el Hospital Nacional Edgardo Rebagliati Martins (HNERM) (Lima, Perú). En esta cohorte retrospectiva, se revisaron las historias clínicas de 122 pacientes pediátricos que fueron atendidos en el HNERM entre enero e inicios de abril del 2022 (55% varones, mediana de edad de 5 años), 77,9% durante el primer mes, y la mitad presentaron alguna comorbilidad. Se hospitalizaron principalmente por dificultad respiratoria, comorbilidad descompensada y deshidratación. El 6,6% ingresaron a cuidados intensivos, 4,9% a ventilación mecánica invasiva, 5,7% requirieron algún vasoactivo y 1,6% fallecieron. Los fármacos más usados fueron antibióticos (43,4%) y corticoides (27,1%). En conclusión, se encontró un incremento rápido de hospitalizaciones en comparación con las olas anteriores, la mayoría con evolución favorable, y con un amplio uso empírico de antibióticos.

27. Efectos del decúbito prono en síndrome de distrés respiratorio agudo secundario a COVID-19: experiencia en un hospital de alta complejidad

Resumen: Introducción: Para el manejo del síndrome de distrés respiratorio agudo (SDRA) moderado a severo por COVID-19 se recomendó el uso de ventilación mecánica protectora y decúbito prono debido a la gran utilidad reportada sobre la oxigenación y mortalidad. Nuestro objetivo es describir el comportamiento gasométrico y mecánico en sujetos con SDRA por COVID-19 manejados con ventilación mecánica protectora y decúbito prono en un hospital de alta complejidad. Método: Estudio observacional. Se incluyeron sujetos ≥ 18 años con SDRA por COVID-19. Se inició ventilación mecánica protectora desde la primera conexión a ventilación invasiva, mientras que el decúbito prono inició con PaO2/FIO2 150. Se realizó un seguimiento durante y posterior al decúbito prono. Se realizó un análisis descriptivo de las características basales y comparación de medias entre grupos mediante prueba de Dunn y Friedman. La significación estadística se estableció en p 0,05 a dos colas en todos los análisis. Resultados: Se estudiaron 74 sujetos, 58% corresponden a hombres con media de 60 años. La oxigenación valorada mediante PaO2 (76 a 98 mmHg, p 0,05) y PaO2/FIO2 (100 a 161, p 0,05) muestra un incremento significativo durante la primera hora de tratamiento con estabilidad de los valores más allá de las 48 horas posteriores al prono. Los parámetros de mecánica pulmonar se mantienen constantes y dentro de rangos protectores (p = 0,18). Conclusión: La estrategia de ventilación mecánica protectora y decúbito prono igual o mayor a 48 horas, en sujetos con SDRA moderado a severo secundario a COVID-19, mejora y mantiene la oxigenación hasta 48 horas después de la supinación. Abstract: Introduction: The use of protective mechanical ventilation and prone position was recommended for the management of moderate to severe acute respiratory distress syndrome (ARDS) due to COVID-19, as a result of its reported utility on oxygenation and mortality. Our objective is to describe gasometric and mechanical behavior in subjects with ARDS due to COVID-19 managed with protective mechanical ventilation and prone position in a high complexity hospital. Method: Observational study. Subjects ≥18 years of age with ARDS due to COVID-19 were included. Protective mechanical ventilation was started from the first connection to invasive ventilation, while the prone position started with PaO2/FIO2 150. Follow-up was performed during and after the prone position. A descriptive analysis of baseline characteristics and comparison of means between groups was performed using the Dunn and Friedman test. Statistical significance corresponds to p 0.05 in all analyses. Results: 74 subjects were studied, 58% correspond to men with a mean age of 60 years. There is evidence of a significant increase in arterial oxygenation assessed by PaO2 (76 to 98 mmHg, p 0.05) and PaO2/FIO2 (100 to 161, p 0.05) during the first hour of treatment, with stability of values beyond 48 hours after supination. Pulmonary mechanics values remain constant within the established protection range (p = 0,18). Conclusion: The strategy of protective mechanical ventilation and prone position for 48 or more hours, in subjects with moderate to severe ARDS due to COVID-19, improves and maintains arterial oxygenation up to 48 hours after supination.

Objetivo: describir una microteoría para cuidados de enfermería en la prevención del delirio en personas mayores en la unidad de cuidados intensivos. Método: investigación teórica prescriptiva, basada en la subconstrucción. Se dedujeron constructos del Modelo de Adaptación de Roy y se indujeron datos del fenómeno de cuidados de enfermería en la prevención del delirium en personas mayores en cuidados intensivos, a partir de una revisión integrativa de la literatura. Resultados: la microteoría posee sistema teórico, operativo y modelo de cuidado. En el sistema teórico se utilizaron los constructos estímulo focal y contextual de Roy. A partir de ellos, se elaboraron los conceptos de cuidado focal y contextual y la variable respuesta adaptativa a la prevención. De las declaraciones relacionales se elaboraron cuatro axiomas, dos postulados, ocho proposiciones y una presuposición epistémica. En el sistema operativo se establecieron dos indicadores empíricos: el Confusion Assessment Method for Intensive Care Units y el historial demográfico/clínico de las personas mayores. Posteriormente, se produjeron dos declaraciones transformacionales, cuatro hipótesis y el modelo de cuidado representado en figura. Conclusión: la microteoría producida prescribe cuidados en la prevención del delirium en personas mayores en cuidados intensivos, por un constructo de interés para la enfermería, y permite intervenciones en el desarrollo de instrumentos que guían la actuación de la enfermería.

Objetivo: mapear los instrumentos para la evaluación del riesgo de lesiones por presión en adultos en situación crítica en una unidad de terapia intensiva; identificar los indicadores de desempeño de los instrumentos y la apreciación de los usuarios con respecto al uso/limitaciones de los instrumentos. Método: scoping review. Para redactar el estudio se utilizó la extensión Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews. La investigación se realizó mediante la herramienta de búsqueda EBSCOhost en 8 bases de datos, resultando 1846 estudios, de los cuales 22 conforman la muestra. Resultados: se identificaron dos grandes grupos de instrumentos: los generalistas [Braden, Braden (ALB), Emina, Norton-MI, RAPS y Waterlow]; y los específicos (CALCULATE, Cubbin & Jackson, EVARUCI, RAPS-ICU, Song & Choi, Suriaidi y Sanada y el índice COMHON). En cuanto al valor predictivo, EVARUCI y CALCULATE mostraron los mejores resultados de indicadores de desempeño. En cuanto a las apreciaciones/limitaciones señaladas por los usuarios, destaca la escala CALCULATE, seguida de la EVARUCI y la RAPS-ICU, aunque aún necesitan ajustes futuros. Conclusión: el mapeo mostró que las evidencias son suficientes para indicar uno o más instrumentos para la evaluación del riesgo de lesiones por presión en adultos críticos en una unidad de cuidados intensivos.

Objetivo: medir el volumen urinario por medio de la ecografía vesical, realizado por una enfermera en pacientes críticos, después de la retirada de la sonda urinaria permanente y verificar los factores relacionados en la retención urinaria. Método: estudio cuantitativo, observacional y transversal, realizado con 37 pacientes críticos de ambos sexos, mayores de 18 años, con retiro de catéter vesical permanente en las últimas 48 horas. Se utilizó un cuestionario con variables sociodemográficas y clínicas y el examen ecográfico. Se utilizó un cuestionario con variables sociodemográficas y clínicas y el examen ecográfico. Los datos fueron presentados a través de distribución de frecuencias, medidas de centralidad y variabilidad, asociación mediante la prueba exacta de Fisher y, para el análisis, regresión logística binomial múltiple. Resultados: de los 37 pacientes, en su mayoría fue de sexo masculino, con una edad média de 54,9 años. La medición del volumen urinario por ecografía osciló entre 332,3 y 950 ml, y el 40,54% de los pacientes presentó retención urinaria. La retención urinaria se asoció significativamente a la aparición de infección urinaria, estreñimiento intestinal y diuresis por rebosamiento espontáneo. Los pacientes con infección del tracto urinario tenían 7,4 veces más probabilidades de tener retención urinaria. Conclusión: la ecografía vesical fue eficaz para medir el volumen urinario después de retirar el catéter urinario permanente y puede contribuir a la detección de retención urinaria.

Objective: The purpose of this study is to evaluate the pharmaceutical interventions (PI) performed in PRM and the factors associated with their acceptance. Method: This is a descriptive and cross-sectional study that analyzed PIs performed on adult patients from two ICUs in Fortaleza, Brazil, in 2019. PIs were analyzed in the categories quantity of drug and pharmacological strategy, using the classification proposed by Sabater et al. The drugs were also classified by the Anatomical, Therapeutic, and Chemical Classification System and by the High Surveillance Drugs (FAV) classification. Results: 305 patients were included, most of them male (55.1 %), elderly (52.8 %) and treated in the surgical ICU (51.4 %). 1,317 PIs were analyzed and 88.0 % were accepted, being to replace one or more drugs (28.0 %), to add one or more drugs (27.7 %) and to change the dose (24.8 %) the most frequent types. The most prevalent therapeutic class was anti-infectives for systemic use (24.1 %) and AVFs were involved in 21.7 % of the PIs. An association was observed between PIs accepted and admission to the clinical ICU (p<0.0001), AVF (p=0.0013), substitution of one or more drugs (p=0.0062) and the class of blood substitutes and perfusion solutions. Introduction: The Intensive Care Unit (ICU) is an environment where critical patients must be treated by a multidisciplinary team. What becomes extremely important to recognize the clinical epidemiological profile to evaluate individually. Objective: To analyze the epidemiological profile of patients admitted to an ICU of a University Hospital and its relationship with the Fugulin scale. Method: This is a retrospective, descriptive study with a quantitative approach, which carried out an analysis of the epidemiological profile, outcomes and variables associated with morbidity and mortality, through reports of patients hospitalized from March to August 2020. Results: It was observed that most of these hospitalized patients were male, referred by the Emergency Medical Care Service, were diagnosed more frequently: acute respiratory failure, sepsis and acute renal failure, with a prevalent outcome in deaths, having been observed correlation of the Fugulin scale with the mortality and severity scores of these patients. Conclusion: In view of the complexity in the care of critically ill patients, the study demonstrates that the Fugulin scale can be an alternative in clinical practice, relating the need for care with severity and mortality to patients in an ICU. Fundamento: en el amplio contexto de la epidemia de COVID-19, las infecciones bacterianas secundarias representan un factor de riesgo importante para los resultados adversos de la entidad.Objetivo: caracterizar a los pacientes ingresados en unidad de cuidados intensivos con diagnóstico de neumonía adquirida en la comunidad y negativos a COVID-19.Métodos: estudio descriptivo, realizado entre marzo/2020 a marzo/2021, en una serie de 338 pacientes ingresados en la Unidad de Cuidados Intensivos con el antecedente de reacción en cadena de la polimerasa negativo a COVID-19, pero con diagnóstico de neumonía adquirida en la comunidad. Se analizaron variables como: edad, sexo, mes del ingreso, requerimiento de ventilación artificial mecánica y estado al egreso.Resultados: el 60,3 % de los pacientes correspondió al sexo masculino, y el grupo de edad de 60 a 79 años (50,3 %) fue el más representativo. El 31,7 % requirió ventilación artificial mecánica, y el 61,8 % permaneció ingresado menos de 48 horas. El 61,6 % fueron egresados vivos, con un promedio de estadía de 6,31 ± 10,62 días. Sumaron 218 aquellos ingresados por más de 10 días desde el inicio de los síntomas y la realización del test diagnóstico de COVID-19.Conclusiones: durante la situación epidemiológica causada por la COVID-19 ingresaron pacientes en unidades de cuidados intensivos con diagnóstico de neumonía adquirida en la comunidad no relacionada al virus del SARS-Cov-2. El tratamiento oportuno, en algunos casos ventilación mecánica artificial, incluso prolongada, contribuyó a la baja letalidad y al egreso con una calidad de vida mejor.

Las infecciones asociadas a la atención en salud, y específicamente las infecciones asociadas a los dispositivos, más frecuentes en unidades de cuidados intensivos, son un problema cada vez más frecuente y de impacto mundial. Por lo que en países de altos ingresos económicos se realizan estudios describiendo el tipo de flora más frecuente, conocen además sus perfiles de resistencia y con base en esto elaboran sus protocolos de manejo y deciden el inicio de antibióticos empíricos. En Latino-América hay poca información recopilada sobre su prevalencia e impacto clínico, así como también hay muy poca literatura o nula sobre este tipo de infecciones en el departamento del Quindío, Colombia. Por lo que no hay una descripción precisa sobre los perfiles bacterianos, ni de sus perfiles de resistencia; y las recomendaciones que rigen las conductas intrahospitalarias, se basan en estudios extranjeros, sin saber con certeza si es extrapolable a nuestra población. Debido a esto se hizo un estudio descriptivo, reporte de casos de las infecciones asociadas a la atención en salud, específicamente, infecciones asociadas a dispositivos en una unidad de cuidados intensivos generales de una clínica de tercer nivel en la ciudad de Armenia, Quindío en el periodo desde junio a diciembre de 2021, encontrando los agentes bacterianos más prevalentes en esta unidad, así como sus perfiles de resistencia basándonos en los cultivos donde se aislaron. No se contaron con estudios genéticos en de estos agentes infecciones aislados, ya que no hay evidencia de que se hayan realizado en la institución.

Introduction. The COVID-19 pandemic, in its beginnings, caused a great impact with a high morbidity and mortality in which health teams, especially nursing, had to face great vital organizational changes in the way of caring. Objective: To understand from an interdisciplinary perspective the coping of nurses in an Adult Intensive Care Unit of a public hospital in Valparaíso, Chile, when caring for people diagnosed with COVID-19 during the year 2020 of the pandemic. Methodology: Qualitative study with a narrative design, which convenience sampling allowed the selection of 8 nurses who participated voluntarily after signing the informed consent. The collection of information was carried out through a semi-structured interview recorded virtually by videoconference, with prior authorization from the hospital's Scientific Ethics Committee. Results: The stressors were identified: Seeing people die, fear of infecting others, organizational changes and the uncertainty about the disease. To deal with them, they had personal and environmental resources, using coping strategies focused on the problem and emotion, which allowed them to adapt and focus the care of the person served. Conclusions: Nurses faced caring with emotional intelligence and resilience, which added to structural changes and the restructuring of their role, allowing them to adapt to overcome the contingency situation. Introduction: Mechanical ventilation is an essential part of the therapeutic care for patients admitted to intensive care units. Objective: To characterize clinical and epidemiologically the patients treated with invasive ventilation. Methods: An observational, descriptive and cross-sectional study of 108 patients treated with invasive mechanical ventilation was carried out in the Intensive Cares Unit of Orlando Pantoja Tamayo General Provincial Hospital in Contramaestre municipality, in Santiago de Cuba province, from January to December, 2022. In the statistical analysis, the absolute frequency and the percentage were used for the qualitative variables, as well as the mean and the standard deviation for the quantitative ones, in addition to the chisquare test with a confidence level of 95% to demonstrate association between these variables. Results: In the series, there was a prevalence of the female patients (55.5%) at advanced ages (47.3%) with malnutrition (52.8%), mechanical ventilation during more than 96 hours (63.9%), and a punctuation higher than 2 (60.5%) according to the scale of organic failure; 30.6% presented complications and 48.1% died. Pneumonia was the most frequent cause in beginning invasive ventilation (36.1%). Conclusions: There was a high frequency of females at advanced ages and with malnutrition. Besides, the sequential evaluation of organic failure was related to mortality and the causes that led to invasive ventilation were associated with death and complications. [ABSTRACT FROM AUTHOR]

- Abstract
Aims: Identify factors that interfere in the provision of oral hygiene care, developed by nurses, to orotracheally intubated patients in intensive care units, on primary scientific evidence. Methods: A systematic literature review study, developed according to the Joanna Briggs Institute protocol. To obtain the articles, a search on B-On? and PubMed? was made. The terms used in the search took into account the vocabulary indexed to the Medical Subject Headings (MeSH) database, having been structured according to Boolean operators, with the following combination in English: "Oral Hygiene" AND "Pneumonia, Ventilator-Associated" OR "Pneumonia, Ventilator Associated" AND "Critical Care Nursing". Articles published between December 2017 and December 2020, in English, Portuguese and Spanish, available in full, published in peer-reviewed scientific journals (peer review) and which fit the PICO question developed for the study, were defined as limiting. Results: After applying the inclusion and exclusion criteria, eight articles of a primary and qualitative nature were included in the review that address the factors that influence the provision of oral hygiene care by nurses to patients undergoing orotracheal intubation. Conclusions: The oral hygiene care practices provided to users under orotracheal intubation, by nurses, are influenced by their knowledge, attitudes, available resources, training and institutional policies, as well as by factors related to the user that are mainly related to difficulties in access to the oral cavity. Objective: To evaluate the pulmonary severity of COVID-19 patients throug the SOFA score computed with pulmonary involvement in Chest Computed Tomography. Method: This is a descriptive epidemiological study conducted out in an Intensive Care Unit, which aimed to study the pulmonary treatment of COVID-19 patients through the calculation of the pulmonary SOFA score relating to Chest Tomography and whether these were related to clinical treatment. Results: The study population consisted of 704 patients, of which 43.7% were women and 56.2% men, with a mean age of 61 years and a mean hospitalization time of 13 days. Most patients had a pulmonary behavior of 75%, a pulmonary SOFA score of 2 and a PaO²/FiO² ratio between 100 and 200. Conclusion: Patients who had more extensive pulmonary involvement/consequently had a lower PaO²/FiO² ratio and remained longer hospitalized with a higher incidence of death. [ABSTRACT FROM AUTHOR]
