14. Current practice of rapid sequence induction in the United Kingdom : a national survey

Abstract

Background \ud The ‘classical’ technique of rapid sequence induction (RSI) was described in 1970. With the introduction of new drugs and equipment in recent years, a wide variation in this technique has been used. The role of cricoid pressure is controversial due to the lack of scientific evidence. Moreover, gentle mask ventilation has been recommended in situations such as obesity and critically ill patients, to prevent hypoxaemia during the apnoeic period. In identifying multiple techniques, we conducted a national postal survey to establish the current practice of RSI in the UK.\ud \ud Methods \ud A survey consisting of 17 questions was created and posted to 255 National Health Service (NHS) hospitals in the UK. We included two copies of the questionnaire in each envelope; one to be completed by the airway lead and the other by a trainee in the same department. The difference in responses from consultants and trainees were assessed using the chi-squared test and the Fisher's exact test.\ud \ud Results \ud In total we received 272 responses (response rate 53%) of which 266 (58% from consultants and 42% from trainees) were analysed. A majority of the respondents (67.7%) pre-oxygenated by monitoring end-tidal oxygen concentration and 76.3% of the respondents use 20–25° head up tilt for all RSIs. Propofol is the most commonly used induction agent (63.9% of all respondents). Opioid has been used by 80.5% of respondents and only 17.7% of respondents use suxamethonium for all cases and others choose rocuronium or suxamethonium based on clinical situation. Although 92% of anaesthetists use cricoid pressure, 83% of them never objectively measure the force used. During apnoeic period 17% of the respondents use gentle mask ventilation.\ud \ud Conclusions \ud Our survey demonstrated a persistent variation in the practice of RSI amongst the anaesthetists in the UK. The ‘classical’ technique of RSI is now seldom used. Increasing awareness of optimising oxygenation during apnoeic period and wider use of newer drugs such as propofol, rocuronium, and opioids has driven the current practice of RSI.

Abstract

Tribological Processes in the gear cutting of cylindrical gears on cutting elements of hob milling tools develop under specific conditions. The particular requirements under which the contacts between the hob milling tools and the workpiece material are made, make it difficult to become familiar with the tribological processes on them so that today there is more knowledge about their consequences on the machined surface of the teeth and the cutting elements of the hob milling tool, and less about their nature. In this paper, based on the experimental investigations, the wear analysis of three-treads uncoated and coated integral hob milling tools in the gear cutting of cylindrical gears is given. This paper aims to determine the pressure on the soil, by simulating real field conditions in the laboratory. Experiments were conducted using a complex testing system in accelerated regime, which can simulate the static pressure at compression of the tires on the soil (stationary machinery). Abstract

This paper presents a theoretical model for prediction of the stress state in agricultural soil under the agricultural tires of a heavy-duty tractor, using the Finite Element Method. Using an acquisition data system and pressure sensors, the theoretical model was experimentally verified in the laboratory. This study highlights the major advantage of using FEM models, which consists mainly in the spectacular reduction of costs for experimental testing and a considerable reduction in the time needed to analyse the possibility of the occurrence of artificial compaction phenomenon in the agricultural soil under heavy-duty tractors. Abstract

Head and neck position is one of the factors which can be associated with difficult videolaryngoscopy and tracheal intubation. This prospective randomised clinical trial compared 'sniffing' and neutral positions using a channelled (KingVision® ) and a non-channelled (C-MAC® D-blade) videolaryngoscope in 200 adult patients randomly allocated into four groups (KingVision 'sniffing', KingVision neutral, C-MAC 'sniffing' and C-MAC neutral). The primary outcome was the ease of tracheal intubation using the modified intubation difficulty scale (mIDS) score. Laryngoscopy time, intubation time, laryngoscopic view using the percentage of glottic opening (POGO) score and success rate of tracheal intubation were secondary outcomes. The median (IQR [range]) modified difficulty scale scores for the four groups, respectively, were 0 (0-1 [0-3]), 0 (0-1 [0-4]), 1 (0-1 [0-5]) and 0 (0-1 [0-3]; p = 0.384). There was no significant difference in laryngoscopy time (p = 0.020), intubation time (p = 0.272) and success rate (p = 0.968) between the groups. The percentage of glottic opening score was lower for C-MAC neutral group as compared with other three groups (p = 0.01). There was no significant difference in the ease of intubation between the 'sniffing' and the neutral position when using the KingVision and the C-MAC videolaryngoscopes. Therefore, either of the two positions could be used with these types of videolaryngoscopes, if deemed advantageous for the patient. [ABSTRACT FROM AUTHOR]

Some types of structures made of orthogonal cable networks are considered. Improved technique for analysis regarding the determination of the axial force in the cables are proposed. Some modification of the outline shape and the anchoring mode meant to increase the safety rank are also mentioned. Aspects of dynamic effects generated by the wind on these roofs, referring to the flutter and resonance phenomena, are discussed.

Nowadays, the destruction and pollution of our environment has produced a growing awareness worldwide of the need for more effective wastewater treatment. Sedimentation is the oldest and most widely used unit operation in water and wastewater treatments. Sedimentation is the process of removing solid particles heavier than water by gravity settling. The terms sedimentation, settling, and clarification are used interchangeably. The unit sedimentation basin may also be referred to as a sedimentation tank, clarifier, settling basin, or settling tank. In wastewater treatment, sedimentation is used to remove both inorganic and organic materials which are settleable in continuous-flow conditions. Abstract

Vegetable oil industry is one of the most important industries in the world, due to the large number of uses of vegetable oil in different fields. Thus, the vegetable oil obtaining from oilseeds, it is a very important operation in the industry. The most common methods of obtaining oil are mechanical pressing and solvent extraction, from which the mechanical pressing operation presenting several important advantages. This led to intensive study of pressing process and more specifically to the study of factors that influence the efficiency of the press, characterized in especially by the recovered oil from the oleaginous material. In this study it is presented a mathematical model of oil extraction process using a screw press obtained by dimensional analysis.
