Abstract

Objective: This study aims to investigate physicians’ perceptions of stroke prevention management and patients’ knowledge of AF and warfarin therapy in Hong Kong. Method: Three different questionnaires modified from 4 studies from foreign countries were used to assess physicians’ perceptions of stroke prevention management, patients’ knowledge of AF and patients’ knowledge of warfarin therapy respectively. Different score systems were developed to compare the results with the foreign countries the 4 studies investigated. Physicians’ characteristics and patients’ demographics were also used in statistical analysis to find any association with the findings. Result: Sixty two physicians with different specialties, 70 warfarin users with AF and 44 warfarin users without AF were recruited in the present study. The average score of Hong Kong physicians in the knowledge of stroke prevention therapy in AF patients was lower than that of Australian family physicians (HK 2.48 vs. AUS 4.02 out of 7, p < 0.001). The mean scores of Hong Kong AF patients in the knowledge of AF were lower than that of UK (HK 1.16 vs. UK 2.24 out of 4, p < 0.001) and that of Finland (HK 2.77 vs. FIN 5.94 out of 8, p < 0.001) respectively. The mean score of Hong Kong AF patients in the knowledge of warfarin therapy was significantly lower than that of US AF patients (HK 2.39 vs. US 3.92, p < 0.001). Conclusion: Hong Kong physicians’ knowledge level of stroke prevention management was on average lower than Australian family physicians’, and Hong Kong patients’ knowledge of AF and warfarin therapy was in general poor, and poorer than that of UK and Finland patients, and US patients respectively. Interventions should be done to improve both physicians’ and patients’ knowledge in order to better utilize warfarin for AF patients.

Abstract

Objective: Patients undergoing double and triple valve procedures represent a complex group and early outcomes have shown high mortality in some series (6.9% for first time surgeries: Society of Cardiothoracic Surgeons in Great Britain and Ireland Database 2008). Method: We studied early clinical outcomes in patients undergoing double and triple valve surgery in Prince of Wales Hospital from 1/2006 to 12/2010. Patients’ demographics, automatically generated risk scores, intra and post-operative variables were recorded and collected prospectively. Result: In the study period, 1557 patients underwent cardiac surgery in our institution and 188 patients (12.2%) required double or triple valve surgery. 55% were female and the mean age was 57 (range 20-81). Preoperative logistic EuroSCORE was mean 5.96. Fifteen patients (7.9%) had undergone previous cardiac surgery. 4.3% of patients had associated coronary artery disease and required addition of coronary bypass procedure. Ninety patients presented with Class III/IV NYHA symptoms (47.8%) and 22 (11.7%) had evidence of severe ongoing heart failure. Nineteen patients (10.1%) required urgent or emergency surgery. 2.7% had evidence of pre-operative renal impairment and 4.8% had associated obstructive airways disease. Five patients (2.7%) presented with previous permanent neurological deficit, 7 patients (3.7%) had previously had a neurological event with full recovery and 22.9% had evidence of only fair or poor left ventricular ejection fraction. Mortality was 4.8% (9 patients) for the overall group and for elective operations mortality was 3.6% (6 patients). Reoperation for bleeding was 5.3% overall and 4 patients (2.1%) developed a new post-operative stroke. Three patients (1.6%) required new post-operative dialysis on a temporary basis. Conclusion: Patient requiring double and triple valve surgeries represent a heterogeneous and complex group of patients. They are a significant portion of patients undergoing valve surgery in our Institution. Outcomes in comparison to pre-operative risk scores and International Databases are satisfactory.

Abstract

Objectives: In regurgitant tricuspid aortic valves, cusp prolapse may be isolated or associated with dilatation of the proximal aorta. Newly appearing cusp prolapse can also appear after an aortic valve sparing operation (AVSO) and be responsible for residual aortic regurgitation. In this report, we describe our experience in repairing prolapsing aortic cusps in 44 patients with aortic regurgitation. Methods: Between 1996 and 2003, 260 patients had aortic valve repair or valve sparing procedures in our department. All patients had peri-operative TEE. Prolapse of one or more of the aortic cusps was identified by TEE and confirmed by careful surgical inspection before and after valve sparing surgery. Forty-four patients with cusp prolapse were identified. Fifteen had an isolated prolapse, with a normal root (group I), 18 had cusp prolapse associated with dilatation of the proximal aorta (group IIa), and 11 had a newly appearing prolapse after AVSO (group IIb). Correction of the prolapsing cusp was achieved by either free edge plication, triangular resection or resuspension with PTFE. This procedure was associated with an aortic annuloplasty in group I, and with AVSO in groups II and III. Results: Post-operative TEE showed AR trivial or grade I regurgitation. At a mean of 23 months follow-up, one patient with recurrent regurgitation required an aortic valve replacement with a homograft. All remaining patients were in NYHA class I or II. Echocardiography confirmed the durability of the valve repair. Conclusions: Among the common causes of aortic regurgitation, isolated cusp prolapse is frequent and is amenable to surgical repair with excellent mid-term results. In particular, in patents who are potential candidates for AVSO, identification and correction of an associated prolapse, either pre-existing or secondary to the AVSO procedure, may further extend the indications for this technique, increase its success rates and improve its long-term outcome.
