Diastolic dysfunction represents an important pathophysiological intermediate between hypertension and heart failure. In the last two decades, the prevalence of heart failure patients having normal or near normal ejection fraction (EF) has increased to around 60 %. It thus poses a great morbidity and mortality risk to the population. In view of present scenario of high prevalence, lack of evidence-based therapy, and limited clinical trials, this study aimed to evaluate how a Unani formulation affects the improvement of the left ventricular diastolic function. This clinical trial was set up as a randomized, placebo-controlled study involving 35 participants, with 18 individuals in the test group and 17 in the control group. Test group received 3.5 g of a polyherbal Unani formulation in capsule form along with 35 mL of an extract of Borago officinalis L. (Arq-e-Gaozaban), divided into two doses after meals. Meanwhile, the control group received a placebo in the same manner over an eight-week period. Follow-ups were conducted every 15 days to assess both subjective and objective parameters in all participants. The test formulation shows significant improvement in dyspnea and diastolic function from baseline to the end of trial (p<0.05), slight improvement in palpitations (p>0.05) and highly significant improvement in easy fatigability (p=0.001) as compared to the control. The present study shows the effectiveness of the test drug in enhancing the diastolic function of left ventricle and alleviating other symptoms associated with ventricular diastolic dysfunction. Nevertheless, additional research with longer follow-up durations is necessary to clarify its efficacy and establish optimal treatment approaches for ventricular diastolic dysfunction in Unani medicine. Within the management of health care, the traditional system of medicine remains indispensable. The Unani system of medicine treats a variety of illnesses with a range of medications derived from plants. Historically, people have utilized sesamum indicum seeds as a medicinal ingredient. Various parts of the plant have been used by traditional physicians. However, the dried seed and oil are commonly used as Till in Unani Medicine. The plant has a very long history of use as a medicinal herb. It can be used on its own or in compound pharmacopeial compositions with other medications. Till Safeed is an annual plant of family Pedaliaceae and this review article aims to describe morphological characteristics, phytochemistry, ethnobotanical uses and therapeutic properties of Till Safeed. So that subsequent study on any novel therapeutic activity based on phytochemistry can be conducted with greater simplicity and to support the revalidation of the drug's scientifically claimed actions as mentioned in classical literatures. We systematically searched classical Unani literature, online data sources (PubMed, Google scholar, Elsevier, Science Direct and Research Gate) and offline encyclopaedia and books on medicinal plants for the relevant data on Till Safeed. Polyunsaturated fatty acids, phytosterols, tocopherols, essential minerals, and a special class of phenylpropanoid chemicals called lignans--which include sesamin, sesamol, and sesamolin--are among the bioactive ingredients found in till seeds. The pharmacological characteristics of sesame lignans include. Objective: The present review article focuses on exploring the wide dimensions of barley including botanical description, nutritional composition, phytochemistry, experimental pharmacology, and clinical studies along with its traditional uses in Unani medicine. Background: Barley (Hordeum vulgare L.) which is referred to as 'Jao' in the Unani system of medicine has a long history of utilization as a food and medicine. It belongs to family Poaceae and is considered as one of the founder crops of old-world agriculture. Methodology: A manual literature survey of classical Unani texts was conducted to collect the information available on 'Jao'. Besides, a comprehensive search of electronic databases like PubMed, Google Scholar, and Science Direct, was carried out to collect all the available information regarding its phytochemical, pharmacological and clinical studies. Result: In Unani medicine, barley is an important drug commonly used in many disorders of the respiratory system, gastrointestinal system, musculoskeletal system, skin, etc. It is used to treat obesity, diabetes, headache, pain and inflammation of the throat, acute swelling of joints and gout, chronic swellings, diarrhoea, fever, skin eruptions, hyperpigmentation of the skin, etc. Recent studies revealed that barley constitutes beta-glucan and a myriad of bioactive compounds that play numerous biological activities such as hypoglycemic, hypocholesterolemic, anti-inflammatory, antioxidant, anti-obesity, anticancer, and wound healing activities. Background: Amenorrhea is defined as the cessation of menstruation; it is a significant clinical sign of reproductive dysfunction that affects 1.5-3% of women of childbearing age and is a leading cause of female infertility. The increasing incidence is attributed to better healthcare access and awareness. The Unani system of medicine offers a distinctive perspective on reproductive ailments, yet its terminology and concepts often diverge from modern medical understandings. Research Problem and Aim: This study aims to bridge the gap between classical Unani literature and contemporary medicine by exploring the etiopathogenesis (asbāb wa mahiyatul marazi) of Iβtibḥs al-Tamth (amenorrhea) and comparing it with conventional medical definition, types, causes and its pathogenesis, thereby enhancing understanding of it from both perspectives. Methodology: A systematic review was conducted in 2023, that includes: The primary and main sources of collection of material were from classical published books, manuscripts and their translations. The second type of sources of collection of material were from different types of available published and unpublished literature related to study topic i.e. journals, proceedings, thesis, reports, souvenir, dissertations etc. The third type of sources were from authentic digital material i.e. trusted online websites i.e. PubMed, internet archive etc. Results/Findings: The research revealed a structured understanding of Iḥtibās al-Tamth in Unani literature, highlighting its nuanced classification and etiological factors in comparison to conventional medicine. The findings underscore the potential for integrating Unani insights into contemporary gynaecological practices. Implications: This exploration contributes to a more comprehensive understanding of Iḥtibās al- Tamth (amenorrhea), suggesting that Unani medicine's holistic approach may offer alternative treatment avenues that could mitigate the side effects associated with conventional hormonal therapies. Ilaj bit Tadbeer, a vital therapeutic approach in the Unani system of medicine, focuses on modifying six essential elements to restore health. This approach encompasses various regimenal therapies such as cupping (Hijāma), leeching (Ta'līq or irsal-e- 'Alaq), venesection (Fasd), massage (Dalk), irrigation (Natūl), emesis (Qay'), diuresis (Idrār-i-Bawl), diaphoresis (Tariq), concoction and purgation (Mundij and Mu'shil therapy), medicated steam (Bukhoor), cauterization (Kaiyy), and enema (Huqna). Fasd is a crucial regimen in Ilaj Bit Tadbeer, serving as a method to eliminate morbid material from the body to treat diseases or maintain good health. With a history of almost 3000 years, Fasd has been effectively utilized in treating various ailments across different eras. Although venesection, or Fasd, has a historical significance, its modern application has declined due to the lack of scientific validation. Background & objectives: Vector-borne diseases pose substantial global health challenges, with a notable impact on India. Effective vector control strategies are crucial to mitigating these diseases as there are no effective vaccines available. Synthetic chemicals are widely used but concerns about safety and environmental impact have prompted exploration of alternative natural solutions. Unani medicine offers a potential source of natural products. This study aims to systematically review classical Unani texts and contemporary scientific literature to evaluate the vector control potential of Unani medicines. Methods: A comprehensive review of classical Unani texts was carried out to compile data on herbal and mineral substances suggested in Unani medicine for control of insects. Furthermore, electronic databases like PubMed, Google Scholar, and Science Direct were explored for research studies evaluating the insect repellent, larvicidal, and insecticidal activities of the Unani medicines. Time restriction was not imposed to ensure comprehensive retrieval of pertinent and up-to-date information. Results: From classical Unani texts, 29 drugs were identified for control of insects, primarily of plant origin from diverse botanical families. Recent scientific investigations have confirmed the insect-repellent, insecticidal, and larvicidal activities of 19 of these Unani medicines against various insect species, including mosquitoes. Interpretation & conclusion: These findings underscore the potential of Unani medicines as a natural source for vector control. However, further research is essential to explore mechanism, delivery methods, and optimal concentrations for developing effective vector-control formulations.

Abstract Cardioprotective Unani drug, Saad Kufi (Cyperus scariosus R.Br.), mentioned by the intellectual colossus Ibn Sina (Avicenna) a thousand years back in his book, “Kitab al-Adviya al-Qalbia” and still widely used by Unani physicians, was selected for experimental study. The main objective of the study was to explore the trial drug ex vivo for validation, hemodynamic elucidation, and molecular mechanism identification for the improvement of Unani therapeutics and to identify a safe and effective novel cardiovascular drug for mainstream medicine. Five doses of 50% ethanolic extract of the physiochemically standardized root were studied in a normal rat heart, on a semi-automated Langendorff apparatus supported with an advanced data acquisition system, perfused with carbogenated non-recirculating Kreb’s-Hensleit solution, at constant flow mode, by continuous recording. The effect of 6 repetitions (n = 6) of each dose was studied with the following parameters on the Lab chart pro version: heart rate, left ventricular developed pressure, cardiac work, and coronary pressure. The significance of the difference was determined by the student’s “t” test. Results of all doses of Saad Kufi showed a significant increase in the above parameters. The maximum increase was observed by Dose III. It could be concluded that Unani’s clinical use of Saad Kufi (C. scariosus R.Br) as cardioprotective was validated and higher doses were indicated to be clinically optimal. Hemodynamically and molecular mechanism-wise, it is indicated to be a sympathomimetic or inotropic-like agent and possibly a direct vasodilator. Since existing long-term treatment of heart failure is not done by cardiac stimulants or positive inotropic and chronotropic agents, the possible future clinical demonstration of therapeutic improvement in heart failure will bring Saad Kufi as a novel drug in mainstream medicine.

Depression is a multifactorial disease with unknown etiology affecting globally. It's the second most significant reason for infirmity in 2020, affecting about 50 million people worldwide, with 80% living in developing nations. Recently, a surge in depression research has been witnessed, resulting in a multitude of emerging techniques developed for prediction, evaluation, detection, classification, localization, and treatment. The main purpose of this study is to determine the volume of depression research conducted on different aspects such as genetics, proteins, hormones, oxidative stress, inflammation, mitochondrial dysfunction, and associations with other mental disorders like anxiety and stress using traditional and medical intelligence (medical with AI). In addition, it also designs a comprehensive survey on detection, treatment planning, and genetic predisposition, along with future recommendations. This work is designed through different methods, including a systematic mapping process, literature review, and network visualization. In addition, we also used VOSviewer software and some authentic databases such as Google Scholar, Scopus, PubMed, and Web of Science for data collection, analysis, and designing comprehensive picture of the study. We analyzed 60 articles related to medical intelligence, including 47 from machine learning with 513,767 subjects (mean ± SD = 10,931.212 ± 35,624.372) and 13 from deep learning with 37,917 subjects (mean ± SD = 3159.75 ± 6285.57). Additionally, we also found that stressors impact the brain's cognitive and autonomic functioning, resulting in increased production of catecholamine, decreased cholinergic and glucocorticoid activity, with increased cortisol. These factors lead to chronic inflammation and hinder the brain's normal functioning, leading to depression, anxiety, and cardiovascular disorders. In the brain, reactive oxygen species (ROS) production is increased by IL-6 stimulation and mitochondrial cytochrome c oxidase is inhibited by nitric oxide, a potent inhibitor. Proteins, lipids, oxidative phosphorylation enzymes, and mtDNA are further disposed to oxidative impairment in the mitochondria. Consequently, mitochondrial dysfunction exacerbates oxidative stress, impairs mitochondrial DNA (mtDNA) or deletions of mtDNA, increases intracellular Ca2+ levels, changes in fission/fusion and mitochondrial morphology, and lastly leads to neuronal death. This study highlights the multidisciplinary approaches to depression with different aspects using traditional and medical intelligence. Background: In general practice, venous ulcers (stasis ulcers) are the most common and recurrent leg ulcers. Ignorance of it can have a severe detrimental effect on one's quality of life, cause gangrene, and even the limb amputation. Venous ulcers are the most dangerous and disabling side effect of chronic venous insufficiency. Leech therapy seems to be effective for the treatment of varicose ulcers due to its salivary secretions which it injects during bloodsucking. The saliva of leeches contains certain biochemical with vaso-dilating, anticoagulant, anesthetic, thrombolytic, antibiotic, analgesic, and anti-inflammatory properties. As a result, Irsal-e-Alaq (Hirudotherapy), one of the Unani regimens, can effectively control it. Objective: The purpose of this case report is to provide insight into the effects of Irsal-e-Alaq (Hirudotherapy) on venous ulcers. Intervention: A 60-year-old male has a complaintof ulceration in his left lower leg near the medial malleolus with swelling, pain, discharge, and darkening of the skin of the left lower limb. Four leeches were applied once every week for 2 months. Efficacy of leech therapy was assessed on every 15th day by pain /leg discomfort on 4 point scale ranging from 0-3, limb girth at calf, ankle & foot, pigmentation color on 4 point scale, pigmentation area, pain in ulcer on 4 point scale from 0-3, ulcer depth, PUSH Score. Hb% was assessed every 15th day to check anemia, colour flow Doppler was done to assess any change in vein anatomy. Result: In this case report, the ulcers were completely healed, and also showed remarkable improvement in other variables. Malkangni, also known as Celastrus paniculatus Willd., is a herb widely used in the Unani system of medicine for its neuropharmacological properties. The seeds and oil of Malkangni have therapeutic benefits, including enhancing memory and cognitive functions. Malkangni exhibits a range of pharmacological activities, including neuroprotective, analgesic, anti-inflammatory, and antioxidant effects. It has shown potential to improve learning and memory, sedation, and anti-convulsive properties and treat conditions such as leprosy, rheumatism, diarrhea, and arthritis. The diverse pharmacological activities of Malkangni suggest its potential in treating various health conditions. This review aims to describe the medicinal properties and therapeutic applications of Malkangni through the perspective of Unani medicine and its phytochemical and pharmacological studies. • Dental plaque is the main factor in initiating and progressing oral diseases. • Inflammatory periodontal diseases (periodontitis and gingivitis) are significant public health-related problems. • Dentifrices and mouthwashes are supplements for brushing and flossing, reducing plaque and calculus and limiting gingivitis. • The efficacy and safety of this herbal mouthwash in maintaining oral health were assessed and compared with 0.2% Chlorhexidine. • Herbal mouthwash showed a sustainable effect. Oral health is a vital indicator of overall well-being, quality of life, and general health, with historical roots in Unani medicine (i.e. preventive measures, oral hygiene, and treatment). This study aims to assess the efficacy and safety of prepared herbal mouthwash in maintaining oral health among school children. 110 schoolchildren were randomly assigned to the test group (treated with herbal mouthwash - Anacyclus pyrenthrum DC, Punica granutum (pericarp), Capparis spinosa (root bark), and Quercus infectoria Oliv (galls)) and the control group (treated with 0.2 % Chlorhexidine Mouthwash) for 30-days. The response was assessed by DMFT, Salivary pH, Oral hygiene index-simplified (OHI-S), Plaque index (Loe & Silness) (PI), Plaque index simplified (O'Leary et al., 1972) (PI-S), Gingival Index (Löe-Silness) (GI), and Bleeding on probing (BOP) on baseline and 30th day GI and PI were further assessed on 60th, 90th and 120th days to determine the sustainable effect of the intervention. After treatment, Mean OHI-S, PI, PI-S, GI and BOP significantly reduced on the 30th day from baseline (p ≤ 0.001) in both groups. After discontinuing intervention, both groups showed significantly lower PI and GI scores on the 120th day from baseline. (p < 0.001). Cervical spondylosis is a chronic degenerative process of the cervical spine characterized by pain in neck, degenerative changes in intervertebral disc and osteophyte formation. Cervical spondylosis is translated as Wajaʻ ur Raqaba , a type of joint pain. The present study was aimed to evaluate the effect of wet cupping in the pain management of cervical spondylosis. This Open, randomized, controlled, clinical study was conducted on 44 patients. Subjects in the test group (n = 22) received a series of three-staged wet cupping treatment, performed on 0, 7th and 14th day. Subjects in the control group (n = 22) received 12 sittings of Transcutaneous Electrical Nerve Stimulation (TENS): 6 sittings per week for two weeks. The objective findings of treatment were assessed with the help of VAS, Neck Disability Index (NDI) and Cervical range of motion. Intra group comparison in test group from baseline to 21st day were found highly significant (p < 0.001) in terms of VAS, NDI, Flexion, Extension and Left rotation score. While in Right rotation, Left rotation and Left lateral flexion score were found moderately significant (p < 0.01). Statistically significant difference was observed between two groups at 21st day in VAS scale, NDI, and Cervical range of motion score (p < 0.001). Ḥijāma Bish Sharṭ was found better in the management of pain due to cervical spondylosis than TENS. It can be concluded that Ḥijāma Bish Sharṭ may a better option for the pain management of cervical spondylosis. Background: Since ancient times, Unani physicians have controlled certain illnesses by altering the metabolic process using a variety of therapeutic approaches. Some treatment tenets have been promoted by Ibne-sina (Avicenna). Regimenal therapy is a fundamental approach to treatment that aims to eradicate sick matter, control excessive production, control its spread, and resolve it by specialised approaches, ultimately restoring humoral equilibrium. Hijama (Cupping) is one such operation. The Arabic word "Hijamat" (from the phrase "Hajm") means "volume", but it is also used to refer to sucking. Additionally, it might indicate "to minimise", "to return to basic size", or "to reduce the volume". One of the most prevalent symptoms of Irq-al-nasa (Sciatica) is lower limb neuralgic pain. This illness has the potential to become persistent and chronic, with significant socioeconomic consequences. Its overall incidence is estimated to be between 13% and 40%. Objective: Mainstream therapists primarily treat Sciatica with medication, surgery, or occasionally both. There should be an efficient substitute for evidence-based medicine because of its negative impacts, high cost, and diseasecentric approach to treatment. This review focused on the concept of dementia in the Unani system of medicine and comprehensive, updated information on Majoon Vaj about the phytochemistry, nootropic, CNS activities and provide insights into potential opportunities for future research. The classical literature on Majoon Vaj for its anti-dementic properties, and therapeutic uses were gathered from nearly thirteen classical Unani books including Unani Pharmacopoeia. The information of pharmacognosy, phytochemical and pharmacological activities of Majoon Vaj and its ingredient was collected by browsing the Internet (PubMed, ScienceDirect, Wiley online library, Google Scholar, ResearchGate). The relevant primary sources were probed, analysed, and included in this review. The keywords used to browse were Majoon Vaj, Dementia, Nootropic, Acorus calamus, Piper nigram, Zingiber officinalis, Nigella sativa, Carum carvi, Plumbago zeylanica, and β-asarone. Relevant Sources were gathered up to July 2021, and the chemical structures were drawn using ACD/ChemSketch software. The species name and synonyms were checked with WFO (2021): World Flora online (http://www.worldfloraonline.org) an updated version of 'The Plant List.' Majoon Vaj contains an excess of bioactive compounds e.g., alkaloids, phenols, flavonoids, tannins, diterpenes, coumarins, carbohydrates, and fixed oils and its ingredients possess broad pharmacological properties, including cognitive-enhancing, neuroprotective, anti-inflammatory, antioxidant and antimicrobial properties. The literature of Unani medicine is quite rich in discussing the pathophysiological basis of memory disorders. It argues that memory, retention, and retrieval are regulated by a complex process involving various faculties. Background: Eczema, is a common chronic skin condition that can lead to recurrent infections and poor quality of life if left untreated. This paper presents the case of a 30-year-old male patient, who came to the outpatient department (OPD) of the Kashmir Tibbia College Hospital and Research Centre, Shilvath, Bandipora, Kashmir, India. Clinical presentation: His chief complaints were lesions on the palm of his left hand for one year. The patient also complains that his skin is black and thickened with severe itching on the affected area. Diagnosis: The patient was diagnosed clinically with hand eczema resembling Nar-e-Farsi in the Unani system of medicine. Interventions: For 30 days, Marham Kafoor was topically applied twice a day at a dose of 8-10 gm. Outcome: The patient was assessed before, the 15th day and 30th day of treatment based on the EASI, POEM, and VAS. Results: After the intervention, significant improvement was observed in all scales from baseline to 30th day. EASI changes from 18 (moderate) to 5 (mild), POEM changes from 19 (severe) to 7(mild), and VAS from 8 (severe) to 1 (mild). Milk is the most necessary food for humans because it provides almost all nutrients such as the most nutritious protein, calcium, essential fatty acids, water-soluble vitamins, and other bioactive compounds responsible for maintaining health and other physiological functions. Milk has also been used effectively in the Unani system of medicine for centuries to improve complexion and to treat other ailments (especially dermatological disorders). For skin brightening, Unani physicians mentioned the use of the paste made up of powdered tukhm e kharpazah (seeds of musk melon), aarad e nakhud (powdered chickpea) each 2 parts, arad e baqla(powdered broad beans), aarad e jau (powdered barley) each 1 part, adas muqashshar (powdered lentil), nishasta (corn flour), kateera (tragacanth gum) each half part, tukhm e turb (radish seeds) 1/4th part, za’fran (saffron) a little and milk at night on the face and advice to wash the face with the decoction of banafsha (sweet violet) and post kharpaza (musk melon fruit epicarp) in the morning. Milk has several therapeutic actions too e.g. Musammin e Badan (adipogenous), Muhallil (anti-inflammatory), Mugharri (glutinous- Mugharri drug is a cold and dry substance with a small amount of viscid moisture that coats the surface and constricts pores. It moisturizes the skin and makes it appear lighter), Mohsin e laun (fairness activity), Ghassal (irrigator), Tiryaq (antidote), and Muqawwi (general tonic), etc. There are some scientific reports on the anti-hyperpigmentation effect of cow milk in which a paste of cow milk and powdered roasted red lentils was tested to enhance the complexion. Common cold and flu are frequently occurring in humans, if not managed, can cause secondary infections and other complications. It may be minimized by boosting immunity, which can be done by Unani Medicine. Objective of this study is to evaluate in vitro immunomodulatory and antimicrobial activity and safety of granules of Naque Nazla, a famous Unani infusion / decoction. Naque Nazla granules (NNG) were tested for its immunomodulatory activity by lymphocyte proliferation assay and macrophage phagocytosis and antimicrobial activity by well diffusion method. LC-MS and GC-MS was done for standardization. Acute toxicity study was carried as per OECD guidelines 423, to evaluate safety of granules. THP-1 cells study displayed EC50 value of 997.6 µg/mL with a proliferation of 53.95% at 1280 µg/mL. Phagocytosis assay displayed 67.85% phagocytosis at 1280 µg/mL, antimicrobial activity was tested against common respiratory tract bacteria which showed susceptibility against Klebsiella pneumoniae, Salmonella spp., Staphylococcus aureus, and Streptococcus pyogenes. LC-MS and GC-MS fingerprinting data was set in and revealed the presence of many phytochemicals. Acute toxicity study displayed the drug safe up to 2000 mg/kg. Background and objectives: As microbes are developing resistance to antibiotics, natural, botanical drugs or traditional herbal medicine are presently being studied with an eye of great curiosity and hope. Hence, complementary and alternative treatments for uncomplicated pelvic inflammatory disease (uPID) are explored for their efficacy. Therefore, this study determined the therapeutic efficacy and safety of Sesamum indicum Linn seeds with Rosa damascena Mill Oil in uPID with standard control. Additionally, we analyzed the data with machine learning. Materials and methods: We included 60 participants in a double-blind, doubledummy, randomized standard-controlled study. Participants in the Sesame and Rose oil group (SR group) (n = 30) received 14 days course of black sesame powder (5 gm) mixed with rose oil (10 mL) per vaginum at bedtime once daily plus placebo capsules orally. The standard group (SC), received doxycycline 100 mg twice and metronidazole 400 mg thrice orally plus placebo per vaginum for the same duration. The primary outcome was a clinical cure at post-intervention for visual analogue scale (VAS) for lower abdominal pain (LAP), and McCormack pain scale (McPS) for abdominal-pelvic tenderness. The secondary outcome included white blood cells (WBC) cells in the vaginal wet mount test, safety profile, and health-related quality of life assessed by SF-12. In addition, we used AdaBoost (AB), Naïve Bayes (NB), and Decision Tree (DT) classifiers in this study to analyze the experimental data. Results: The clinical cure for LAP and McPS in the SR vs SC group was 82.85% vs 81.48% and 83.85% vs 81.60% on Day 15 respectively. On Day 15, pus cells less than 10 in the SR vs SC group were 86.6% vs 76.6% respectively. No adverse effects were reported in both groups. The improvement in total SF-12 score on Day 30 for the SR vs SC group was 82.79% vs 80.04% respectively. In addition, our Naive Bayes classifier based on the leave-one-out model achieved the maximum accuracy (68.30%) for the classification of both groups of uPID. Conclusion: We concluded that the SR group is cost-effective, safer, and efficacious for curing uPID. Menstrual patterns can be an indicator of overall health and self-perception of well-being. Amenorrhoea is the absence of menses in women of reproductive agewhich may be primary or secondary. Primary amenorrhoea refers to the absence of menarche at the age of 16 and secondary amenorrhoea is the cessation of menses for at least 6 months in already cycling women. Secondary amenorrhoea is more common than primary amenorrhea. Ihtibas al-Tamth (amenorrhoea) is defined in the Unani system of medicine as the absence of monthly bleeding for more than 2 months or a decrease in the quantity of menstrual blood. The etiologies of amenorrhoea may be considered categorically as outflow tract abnormalities, primary ovarian insufficiency, hypothalamic or pituitary disorders, other endocrine gland disorders, sequelae of chronic disease, physiologic, or induced. The causes of Ihtibas al-Tamth are related to Quwwat Dafia al-Badan, Madda and Johar Khun-i-Hayd, or Ala Makhraj -i-Hayd. Su'-i-Mizaj Barid, Yabis, Harr or Harr wa Yabis, Su'-i-Mizaj Maddi or Sada can lead to Du'f al-Quwwat-i-Dafia. Abnormality in quality and quantity of Madda in Su'-i-Mizaj Maddi can also lead to ihtibas al-tamth. Amenorrhoea is not a diagnosis but a symptom indicating anatomical, genetic and neuroendocrine abnormalities. It can be determined by two different groups of causes: (a) anatomical defects of the genital organs; (b) endocrine dysfunctions. Both congenital and acquired anomalies in the structure of the uterus and vagina could produce amenorrhoea; nevertheless, in most patients, amenorrhoea is related to an ovarian malfunction. Symptoms usually associated with amenorrhoea are headache, nausea, back ache and lower abdominal pain, tiredness, and some respiratory problems. Unani physicians have paid great attention to the use of cosmetics since very ancient times. Around 4000 BC first archaeological evidence of cosmetics usage was found in ancient Egypt. The use of perfumes, oils, burning of aromatic incense, copper eye shadows, lamp black for eyes and dying of hairs red were in practice by ancient Egyptians. Unani literature is very rich in cosmeceutical formulations taking care of appearance and dealing with cosmetic diseases in humans. In Unani classical text like Kitab-ul-Mansoori, Al-Hawi-fil-Tib, Kamil-us-Sana, Al-Qanoon-fit-Tib, Zakheera-e-Khwarzam Shahi, the details of cosmetics are mentioned under the headings of Tazeeniyaat. This systematic review can be concluded that various cosmetics and cosmeceuticals prevailing in contemporary era may have been introduced or inspired by these physicians. Background: Saffron is a significant medicinal herb that has a lot of potential for use in the development of new drugs. Since ancient times, saffron (Crocus sativus L.) has been associated with various health benefits in various parts of the world. Some of these applications have been overlooked over time. However, interest in naturally occurring active compounds has rekindled interest in saffron's past applications and the best applications for saffron today can be aided by knowing its various historical uses. It was prized as a culinary spice as well as a perfume, color, and medicinal herb. It was used medicinally in many different cultures to treat a wide range of ailments, from genitourinary to eye problems. In many countries, it was also used as an antidepressant and tonic substance. Objective: The purpose of this review is to provide an overview of the traditional and modern uses of Crocus sativus, with particular attention to its chemical makeup, pharmacological characteristics, and ethnobotanical description. Methodology: Scientific databases and publications about the chemical profile, biological activity, and historical use of saffron were included in the literature search. Objectives: The objectives of this study were to observe the Efficacy of unani pharmacopoeial formulation Boozidan (Pyrethrum indicum) for the management of Niqris Muzmin (Chronic gout) and to observe any concomitant and adverse effects of formulation. Methods: This single blind, randomized and standard control clinical study was conducted between April 2021 to October 2021 for a period of 28 days. 60 patients diagnosed with chronic gout were divided into two groups, 30 Test group and 30 Control group. The test group was administered with 5g boozidaan in powder form once daily with water and the patients of control group were given tablet febuxostat 80mg once daily. Patients were followed up and improvements in subjective parameters were assessed weekly at 7th, 14th, 21st, and 29th day. Objective parameters were assessed at the baseline and at 29th day. The data obtained was subjected to statistical analysis. Results: Both the test and control drug were effective in reducing Serum uric acid level, ESR and CRP levels. A significant improvement in subjective and objective parameters was observed in both test and control groups and no adverse effects were observed during and after the study. However, the test drug had superior efficacy in reducing the objective parameters and it was statistically significant (p-value<0.0001). Conclusion: Both Boozidaan and Febuxostat were safe and significantly effective in resolving the symptoms and signs of gouty arthritis and both have significant effect on reducing serum uric acid level. Background: Chronic anal fissure is recognized for its profound impact on quality of life and the propensity for treatment relapse, underscoring the need to explore safe and alternative clinical interventions. Unani medicine offers a wealth of therapeutic approaches for anal fissures, yet empirical evidence remains limited. Objective: This study aimed to assess the efficacy of the Unani formulation Marham Safeda Kafoori in treating chronic anal fissures. Methods: Seven patients with chronic anal fissures underwent a 14-day treatment with Marham Safeda Kafoori. Concurrently, two Unani interventions, namely Habb-e-Bawaseer Amya and Habb-e-Muqil, were administered for the management of constipation. Therapeutic response was assessed using a 100mm Visual Analog Scale for anal pain and burning sensations, alongside evaluating non-hemorrhoidal rectal bleeding and fissure healing status. Results: Following a two-week follow-up, pain and burning decreased significantly. Similarly, all the cases were healed completely with no instances of rectal bleeding. Conclusion: The trial formulation demonstrated promising efficacy in the management of anal fissures, with no observed adverse effects. Nevertheless, it is prudent to recommend rigorous prospective clinical investigations to validate its scientific soundness.

Regarding the importance of Mizaj for health preservation and treating diseases, it is necessary to achieve a standard tool for Mizaj identification. The purpose of this study was to design a standard self-reporting Mizaj identification questionnaire for elders. Methods: In this exploratory sequential study, criteria of Mizaj identification were extracted by reviewing PM literatures and interview with PM experts and elders. The primary questionnaire was designed and its validity and reliability were assessed, using weighted Kappa statistics, Pearson correlation coefficient (PCC) assessment, receiver operating characteristic (ROC) curve and determining the specificity and sensitivity of cut-off points. Results: Among the 101 items in the primary questionnaire, 73 items had acceptable reliability. The final 20-item questionnaire was obtained after the criterion validity and PCC assessment. The sensitivity and specificity of this questionnaire were 83% and 88% for warmness, 49% and 80% for moderate in warmness-coldness, 72% and 91% for coldness, 57% and 78% for wetness, 30% and 79% for moderate in wetness-dryness, and 81% and 67% for dryness, respectively. Conclusion: The first standard Mizaj identification questionnaire for elders was designed in this study. It is recommended as a supplementary diagnostic tool for clinicians and researchers in PM. Also, the people with age over 60 can use it to identify their own Mizaj and then, choose the suitable PM or Unani medicine lifestyle recommendations based on their Mizaj.

Background: Since the early days of the coronavirus disease-2019 (COVID-19) pandemic, a number of persistent symptoms have been observed in the patients, continuing for weeks or even months after recovery, now named as post-COVID-19 condition, or long-COVID. The commonest symptoms are neurological (cognitive decline, headache, insomnia etc.), cardiovascular afflictions (thromboembolism, hypertension etc.), respiratory problems, and general symptoms like fatigue, malaise etc. In this context, traditional medicine systems are being explored for their holistic and multi-dimensional effects, often not attainable in synthetic drugs. Purpose: This review is done to explore the potential medicaments (single raw/ natural ingredients only) in Unani system of medicine which may be beneficial in post-COVID-19 condition. Methodology: We explored four important textbooks of Unani medicine for medications beneficial for conditions similar to COVID-19, in the light of guidelines of post-febrile phase in Unani medicine. Results: After following the inclusion and exclusion criteria, a total of 159 medicaments of herbal, animal and mineral origin were identified. It was observed that 57.23 % herbs (e.g. Pistacia vera L., Pimpinella anisum L.) had efficacy in various respiratory conditions, while 38.36 % herbs (e.g. Crocus sativus L., Borago officinalis L.) were effective in various neurological conditions. Besides, many other medications are described to be effective in various ailments of gastro-intestinal tract, as vital-organ tonics, nutritive, etc. An important observation is that most herbs are effective in more than one condition. Some of the important medicaments include C. sativus, honey, Cichorium intybus L., Aucklandia costus Falc., B. officinalis, Cordia dichotoma G.Forst. etc. Pre-clinical and clinical studies on the drugs corroborate the classical Unani literature. Conclusion: Several Unani herbs can be potentially utilized in post-COVID-19 conditions due to their multi-dimensional therapeutic and protective actions. Hence, clinical research in this direction may provide a practical solution to overcome the sequelae of COVID-19.

Sesame oil has a long history of usage as a food and medicine. It is the most used oil as a medicine or as a base oil for preparations of many compound drugs in the Unani system of medicine. It has a wide range of biological activities as mentioned in Unani classical text; this review highlights its pharmacological activities and their possible mode of action. Searched many Unani classical literature online and offline and simultaneously did parallel search on databases like PubMed, and Science Direct, and extraction of data related to sesame oil, sesame seeds with its pharmacological activities, mode of action, then interpretation and summarization of all related data. Sesame oil possesses many biological activities like anti-inflammatory, antihyperlipidemic, antiatherosclerotic, hepatoprotective, antiasthmatic, analgesic, emollient, antipruritic, and wound healing effects, which were scientifically demonstrated as mentioned in Unani literature. Sesame oil has a hopeful effect on modulating diseases with no significant toxic effect; so, there is a need to identify its safety and efficacy on human subjects to develop a new potential drug.

Majoon Ushba (MU) a polyherbal preparation in a semisolid oral dosage form and Marham Raal (MR) an ointment made up of herbal and animal drugs are used for skin disorders in Unai medicine. Considering the biologically active substances with antioxidant, antimicrobial, antifungal, anti-inflammatory, and wound-healing properties of their ingredients, this study aimed to evaluate the clinical and mycological efficacy and safety of MU and MR in tinea corporis. This randomized open-labeled active-controlled clinical trial was conducted on 42 patients of tinea corporis. Patients were randomly assigned via a computer-generated list to the test group (n=21) receiving oral MU (6 gm) with topical MR and the control group (n=21) topical terbinafine hydrochloride (1%) for 40 days. The severity scores of clinical symptoms; erythema, scaling, itching, margins, and size were the primary outcome and the mycological cure (negative KOH microscopy), total score, and dermatology life quality index (DLQI) were the secondary outcomes were analyzed at baseline and at 40 days. A significant reduction was observed in clinical severity scores of erythema, scaling, and itching (P ≤ 0.05) in both groups except for margins, and size of the lesion (P ≥ 0.05). The mycological cure (P =.001) and the total score (P = 0.0002) and DLQI (P = 0.002) were statistically significant in both groups when analyzed from baseline to the end of the treatment within groups, however, the differences in these outcomes were statistically insignificant (P ≥ 0.05) on the inter-group comparison. All the safety parameters (clinical and laboratory) were in the normal range in both groups. The results demonstrated that traditional and conventional treatments were equally effective and MU and MR can be prescribed as alternative or complementary treatments for tinea corporis. Unani physicians have suggested a wide range of anti-dermatophytic remedies, although the scientific evidence is scarce. Thus, the efficacy and safety of Terminalia chebula Retz. fruit powder mixed with vinegar was compared with terbinafine hydrochloride 1% cream in the treatment of tinea corporis in order to establish the non-inferiority of test drugs. The primary outcome measures were change in the presence or absence of hyphae on KOH mount test, change in pruritus severity assessed on 100 mm VAS and change in physician's global assessment. Secondary outcome measure was change in the dermatology life quality index (DLQI). Hemograms, serum creatinine, serum bilirubin, and random blood sugar levels were measured at the baseline and after treatment to ensure the safety of the interventions. A per-protocol analysis was done on 40 participants (21 in the test group and 19 in the control group). The observed differences in the primary and secondary outcomes between the test and control groups were greater than the non-inferiority margin, signifying that the test drugs were not inferior. It may be inferred that the trial drug Terminalia chebula Retz. fruit powder mixed with vinegar is not inferior to terbinafine hydrochloride cream in the treatment of tinea corporis. Unani medicine is one of the oldest systems encompassing the medical traditions from the Babylonian and Egyptian eras to the present Indian Unani tradition and Traditional Iranian medicine. It came to the Greeks through Egyptian civilization and was refined into a rational system by Hippocrates, Galen, Razi, and Avicenna. The faithful translation of traditional practices into Arabic in the medieval era under a comprehensive frame based on logic and observations developed the research concept methodology much earlier, which was believed to be a modern concept. The present paper reviews the changing face of research methods adopted in Unani medicine.The research concept was looked upon in the classical Unani literature. Objective: Melasma is a chronic, acquired, symmetrical hyper melanosis of skin, characterized by irregular light to dark brown patches on sun-exposed areas, with a significant effect on psychological health; melasma is termed as Kalf in Unani medicine. Conventional treatments have transitory results and often carry adverse effects like skin irritation, scarring, etc. This study was planned to evaluate the safety and efficacy of a Unani pharmacopoeial formulation Tila-e-Kalf, comprising of lentil (Lens culinaris), bitter almond (Prunus amygdalus), and fig (Ficus carica), and to compare its efficacy with standard drug hydroquinone in patients of melasma.Materials and Methods: This was an 8-week open-label, standard controlled, randomized clinical study conducted on patients of epidermal melasma. The test group received Tila-e-Kalf while the control group received hydroquinone 4% cream for local application once daily. Non-specific low back pain is a leading contributor to disease burden and works absenteeism worldwide with a lifetime prevalence of 60–70% in industrialized countries. This clinical study aimed to assess the efficacy of hot fomentation with half-baked medicated bread (khubz) compared to hot water bag fomentation to alleviate pain and disability in non-specific low back pain. In this randomized-controlled study, fifty-four patients with low back pain were randomly assigned into two groups to receive either hot fomentation (Takmīd-e-haar) with half-baked medicated bread in the test group or hot water bag fomentation in the control group, on the lumbosacral region daily for 30 min for 15 consecutive days. Patients were assessed statistically using the visual analogue scale (VAS) and Oswestry disability index (ODI) at baseline, 7th and after treatment (15th day). After the intervention, statistically significant improvements (p < 0.001) were observed in VAS and ODI scores in both the groups on the intragroup comparison. The test treatment showed better efficacy in comparison to the control treatment with a mean difference of 1.75 in VAS (p < 0.0001) and 8.20 in ODI (p = 0.001). The tested intervention showed significantly better efficacy in comparison to the hot water bag fomentation probably due to the analgesic (musakkin-i-alam), anti-inflammatory (muḥallil-i-awrām), and demulcent (mulaṭṭif) properties of the ingredients of tested Unani formulation in addition to the effects of heat. It may therefore be concluded that medicated fomentation is an effective, safer, feasible, and less expensive regimen for patients with non-specific low back pain. Kasni (Cichorium intybus Linn.) is highly praised in many traditional systems of medicine for medicinal properties of its leaves, seeds and roots. It is grown in Punjab, Kashmir, Andhra Pradesh, Karnataka and Maharashtra. In traditional medicine, all parts of the plant are used as diuretic, laxative, antibilious, antipyretic, blood purification, appetizer and strengthen of the stomach. It is also used in the treatment of hepatic failure, jaundice, intermittent fever and mild states of chronic skin diseases. It contains glycosides, flavonoids, saponins, caffeic acid derivatives, inulin, lactucin, sonchuside A, cichoriolide. The scientific analysis of kasni demonstrates many of the activities mentioned in Unani literature. Nevertheless, further research is needed to identify the mechanism, active constituent, and usefulness of kasni in clinical practice. Background: The most frequent and recurring type of leg ulcers seen in general practise is a venous ulcer (stasis ulcer). Its neglect has a considerable negative influence on quality of life, might result in gangrene, and may necessitate amputating the affected limb. The most serious and incapacitating complication of chronic venous insufficiency is venous ulcer. The conventional system of medicine offers limited clinical evidence for its effectiveness and usually needs amputation. Hence, it can be managed well by one of the Unani regimens, i.e., Irsal-e-Alaq (Hirudotherapy). Objective: The purpose of this case report is to provide insight into the effects of Irsal-e-Alaq (Hirudotherapy) in venous ulcer. Intervention: A 53-year-old male who has complaints of ulcer in left lower leg near lateral malleolus with swelling, pain, discharge and darkening of skin of left lower limb. Four leeches were applied once every 15 days for 45 days. Ulcer was assessed on 0th day, 15th day, 30th day and 45th day on the basis of area of the wound in a square centimetre, appearance of healthy granulation tissues, epithelization in percentage and depth of wound in centimetres, pain (VAS scoring), discharge, smell and number of wounds. Results: on the first day wound size was 4 cm long, 3 cm wide, 2 cm deep and the floor filled with dark granulation tissue with 0 % epithelization, discharge, pain and smell. On the 45th day ulcer was completely healed with 100% epithelization. Conclusion: Irsal-e-Alaq has been proven beneficial in treating venous ulcers and enhancing patients' quality of life. To determine the effectiveness of Irsal-e-Alaq in the management of venous ulcer, rigorous, controlled, randomised, blinded, and long-term follow-up studies on high sample sizes are required. The objective of the present study is to elucidate the evolution of thoughts on urine inspection from antiquity to the present day. The classical Unani literature like Liber Regius, Canon of Medicine, Liber continens, Treasure of Khwarazamshah, etc. and the ancient literature of other traditional medicines like Ayurveda were prime sources of data. Additional sources were translations and summaries of the said classical books and scholarly articles written on the topic of urine. For recent advancements in uroscopy, browsing PubMed, Google Scholar, data science, and other trusted websites was carried out. Plentiful resources were frequently looked up and aided in the selection of topics for discussion in this review. With this intent, bibliographic research on the various classical literature was thoroughly sought out. In this regard, the classical literature of various traditional medicines was of immense help. Moreover, browsing trusted websites was carried out by searching for complementary and alternative medicine for uroscopy and urinalysis. Uroscopy has a long, colourful, and interesting history. The earliest evidence of urine inspection can be found in Babylonian, Sumerian, and Ayurvedic writings. Correspondingly, uroscopy has been practised for over 6,000 years. However, with the invention of the microscope, urinalysis became a valuable diagnostic tool in clinical practices. So essentially, urine represents a mirror image of the body’s states, such as health and disease. Besides, Urinalysis has been evolving from antiquity to the present day, from time to time.

Background and aim: Irritable bowel syndrome (IBS) is one of the most common gastrointestinal disorders characterized by chronic recurrent abdominal pain related to a change in bowel habit or defecation frequency and commonly accompanied by anxiety and depression affecting about 10% population globally. Jawarish Shahi (JS) is a special dosage form prepared for gastrointestinal disorders in Unani medicine containing Phyllanthus emblica L., Terminalia chebula Retz., Coriandrum sativum L., Elettaria cardamomum (L.) Maton and Salix caprea L. Considering the antioxidant, immunomodulatory, antispasmodic analgesic, antidiarrheal, antisecretory, laxative, anti-inflammatory, anxiolytic, and antidepressant properties, the present study was aimed to evaluate the efficacy of JS in IBS. Experimental procedure: This single-arm open-labeled clinical trial was conducted on 26 male and female patients of IBS according to Rome IV criteria, aged 18–50 years with moderate symptoms. JS was given 7 g orally twice a day after meal with water for 45 days. IBS Severity Scoring Scale (IBS-SSS) was used for efficacy outcomes and the difference was analyzed from baseline to the subsequent follow-ups. Unani medicine is one of the oldest systems encompassing the medical traditions from the Babylonian and Egyptian eras to the present Indian Unani tradition and Traditional Iranian medicine. It came to the Greeks through Egyptian civilization and was refined into a rational system by Hippocrates, Galen, Razi, and Avicenna. The faithful translation of traditional practices into Arabic in the medieval era under a comprehensive frame based on logic and observations developed the research concept methodology much earlier, which was believed to be a modern concept. The present paper reviews the changing face of research methods adopted in Unani medicine. The research concept was looked upon in the classical Unani literature. Several papers in this regard were also searched using search engines, namely PubMed, google scholar, and Science Direct. The concept of research methodology is quite primitive and has undergone several transitions throughout development and advancement in science. Nature and health has always remained in consonance with each other. Various natural resources have got therapeutic purpose apart from their use in other industries. Clays from nature have been used frequently for therapeutic purposes from ancient times. The external use for stopping bleeding and their internal use for various ailments have been documented from early Greek civilization and Unani system of Medicine. Survey of literature from index journal, available Unani classical text, and authentic books and websites were carried out about therapeutic clays, their nomenclature, uses and rationale for their use was noted. Dioscorides has mentioned a number of clays in his book Materia Medica. Work of other Unani/Arab scholars AbūRayḥān al-Bīrūnī, Ibn al-Bayṭār, Hunayn Ibn Isḥāq al-ʿIbādī, Abū Bakr Muhammad ibn Zakariyyā al-Rāzī, Ibn Sīnā, Abul Qasim Khalaf ibn al-Abbās al-Zahrāwī on clays etc is been shortlisted in this work. Unani system enriched by vast literature extending from Greeks to Indian scholars bridged by the Medieval Arab scholars of medicine have left great legacy in regard to the description, use, and indication of various types of therapeutic clays, which are remedy for various diseases as a haemostatic, detergent, resolvent, astringent, wound healing etc, and also used as a cosmeceuticals and cosmetics. Dry cupping therapy (DCT) is considered beneficial in the amelioration of cervical spondylosis (CS) symptoms in Unani medicine. Therefore, the focus of this study was to ascertain the efficacy of DCT and optimal cup application time duration for CS. It was a randomized clinical trial involving 45 participants with clinically diagnosed CS. The eligible subjects were randomly categorized into three groups, each having 15 participants. Each of the three groups, i.e., A, B, and C, received DCT daily for 15 days for 8 min, 10 min, and 12 min, respectively. All the participants were evaluated at the baseline, 7th, and 15th days of the trial using the neck disability index (NDI) as well as the visual analogue scale (VAS). The baseline mean ± SD of NDI and VAS scores were significantly reduced in all the three groups at the end of the trial. Although all three groups were statistically equal in terms of NDI, group-C demonstrated greater efficacy in terms of VAS. The per-protocol analysis showed that dry cupping effectively alleviated neck pain across all treatment groups. Although, this effect on neck disability index was statistically equal in all three groups, the 12-min protocol was more successful in reducing pain. [ABSTRACT FROM AUTHOR]

Non-alcoholic Fatty Liver Disease (NAFLD) is one of the diseases that have evolved lately into a major challenge for gastroenterologists. Although, the term NAFLD has not been familiar to the medical world since long, other conditions resembling the presentation of NAFLD have been there since primitive times. It is a reversible condition of the liver, wherein large vacuoles of triglyceride fat accumulates in liver cells via the process of steatosis, despite any evidence of excessive alcohol consumption. In the developed countries NAFLD is reported to be the most common liver disorder, with a worldwide prevalence of 6–35%, in India its prevalence has been increasing gradually. Unani physicians have described liver as one of the principal organs of the body. It is the primary source of natural faculties, where the functions of digestion, concoction, absorption and excretion are performed, normally temperament of liver is hot and moist which can get converted to cold due to mutable dietary habits, consumption of fatty and cold food in abundance etc. In Unani System of Medicine, NAFLD has not been described as such, but it can be studied under Su'-i-Mizāj Kabid Bārid due to correlation of most of the symptoms. Its management mainly consists of elimination of morbid matter which is accumulated in the liver and correction of Su'-i-Mizāj Kabid Bārid by using drugs having opposite temperament (Ilaj bil zid). This article aims to discuss the impact of air quality on human health, measures to achieve the goal of good indoor air quality and proposed benefits of interventions of Unani Medicine with an evidence-based approach. The significance of air quality on the health of the community cannot be denied. Recent evidences from WHO illustrated data on severe air pollutants and their impacts on human health ranges from minor upper respiratory irritation to chronic respiratory ailments including lung carcinoma and heart disease associated with premature mortality and reduced life expectancy. In Unani Medicine, air has been included in the list of factors, which are six in number and play the central role in prevention of diseases and maintenance of health. Air is considered as the medium of most of the extrinsic factors such as chemical and biological pollutants affecting health and their exposure results in short and long-term health issues. The literature of Unani Medicine proposes many simple and effective measures, which help to improve indoor and outdoor air quality. The goal of outdoor clean air is achieved through implementation of measures to tackle the source of pollution, while indoor clean air is attained through various means e.g., fumigation with herbal drugs. Hence, an extensive literature survey on Unani reserve was conducted to collect information about the concept of air discussed under the heading of six essential factors and its implication in prevention of diseases and maintenance of health. Further, research databases such as Pub Med, Google Scholar, and Science-Direct were broadly searched for evidence on the efficacy of herbals mentioned in Unani literature for the indoor air purification and subsequent air quality improvement. Recent studies showed good air quality leads to decrease in mortality, particularly of respiratory and cardiovascular deaths whereas poor air quality results in a variety of diseases. Unani scholars prescribed several regimens such as Bukhoor (Fumigation), Sa'oot (Nasal instillation) and use of Abeer (Perfumes) and Nadd (Incense) for the improvement of air quality. Likewise various herbal fumigants and sprays containing drugs like mī'a sā'ila (Liquidambar orientalis Mill.), mastagi (Pistacia lentiscus L.), mushk (Moschus moschiferus L.), loban (Styrax benzoides W. G. Craib), ābnoos (Diospyros ebenum J. Koenig ex Retz), zā'fran (Crocus sativus L.) and sirka (vinegar) etc. has been well explained and used exclusively for air purification and improvement of AQI. Therefore, in the present scenario of altered air quality, we forward certain measures described in Unani system of medicine for health promotion and protection. Unani medicine relies on drugs of natural origin for the treatment of various medical conditions. It is practised throughout Asia and particularly in India. The rich natural drug asset of Unani medicine includes both single and compound drugs. Habb-e-Azaraqi is a polyherbal Unani formulation used clinically since decades for the management of various neurological disorders like facial palsy, paralysis, gout and arthritis. The explicit nature of Habb-e-Azaraqi rationalizes its use in nervine disorders. The main ingredient of Habb-e-Azaraqi is Azaraqi mudabbar (detoxified Strychnos nuxvomica L.) which is known for its diverse medicinal uses. Liver plays a major role in metabolism and excretion of xenobiotics. Liver dysfunction is a major health problem that challenges health care professionals, pharmaceutical industry and drug regulatory agencies. The most common symptoms are fatigue and discomfort in abdomen while in patients who are obese with BMI > 25, about one third have metabolic syndromes. The present clinical research was conducted in Regional Research Institute of Unani Medicine, Aligarh. The patients under trial were selected from GOPD of institute for obesity and it was observed that out of total registered patients' approx 28% have elevated liver enzymes from its normal range without any sign and symptom of hepatitis etc. Total 23 patients were selected from the study whose liver enzymes (SGOT, SGPT and ALP) were high at the baseline. Unani pharmacopoeial drug Jawārish bisbāsā 7 g daily was prescribed with lukewarm water in the morning and evening empty stomach for 8 weeks. At the end of study the result were compared with base line. It was observed that Jawārish bisbāsā significantly reduced the liver enzymes, e.g. SGOT, SGPT, Alkaline Phosphatase in comparison to baseline values. To validate the concept of abadāl-i-adwiya (drug substitution) by evaluation of physicochemical standardization and hepatoprotective activity of Aristolochia rotunda & its substitute, Curcuma Zedoaria in albino Wistar rats. Physicochemical standardization by estimation of moisture content, ash values and extractive values were carried out using standard methods. Hepatotoxicity was induced in albino Wistar rats using CCl4 1 mL/kg s. c. on alternate day for 14 days. Group I was served as Plain control and Group II as Negative control. Group III was administered silymarin 50 mg/kg p. o. while Group IV received HAE of A. rotunda 89.64 mg/kg p. o., and Group V was administered HAE of C. Zedoaria 45.73 mg/kg p. o. At the end of the study, serum bilirubin, AST (SGOT), ALT (SGPT) and ALP were estimated. The histopathology of liver was also carried out. The physicochemical parameters of both test drugs viz. moisture content, total ash, acid insoluble ash and water soluble ash were found within normal limit. The total serum bilirubin, direct bilirubin, AST (SGOT), ALT (SGPT) levels were significantly decreased in Test groups A and B when compared to the Negative and Standard controls. The microscopic examination of liver collected from animals of Group IV and Group V revealed significant recovery from hepatic toxicity compared to the Negative control. The study experimentation has revealed that C. Zedoaria may be used as a substitute for A. rotunda in the treatment of liver diseases. However, the outcome has to be further corroborated with phytochemical evaluation and clinical trials of both the drugs.
