Objectives: This study aimed to ascertain utility and vision-related quality of life in patients awaiting access to specialist eye care. A secondary aim was to evaluate the association of utility indices with demographic profile and waiting time. Methods: Consecutive patients that had been waiting for ophthalmology care answered the 25-item National Eye Institute Visual Function Questionnaire (NEI VFQ-25). The questionnaire was administered when patients arrived at the clinics for their first visit. We derived a utility index (VFQ-UI) from the patients' responses, then calculated the correlation between this index and waiting time and compared utility across demographic subgroups stratified by age, sex, and care setting. Results: 536 individuals participated in the study (mean age 52.9±16.6 years; 370 women, 69% women). The median utility index was 0.85 (interquartile range [IQR] 0.70–0.92; minimum 0.40, maximum 0.97). The mean VFQ-25 score was 70.88±14.59. Utility correlated weakly and nonsignificantly with waiting time (-0.05, P = 0.24). It did not vary across age groups (P = 0.85) or care settings (P = 0.77). Utility was significantly lower for women (0.84, IQR 0.70–0.92) than men (0.87, IQR 0.73–0.93, P = 0.03), but the magnitude of this difference was small (Cohen's d = 0.13). Conclusion: Patients awaiting access to ophthalmology care had a utility index of 0.85 on a scale of 0 to 1. This measurement was not previously reported in the literature. This cross-sectional study aims to describe doubts regarding the diagnosis and treatment of syphilis in pregnancy among primary care professionals in a telehealth service. All teleconsultations (TCs) offered through TelessaúdeRS-UFRGS to primary health care (PHC) services in the state of Rio Grande do Sul between 2018 and 2021 involving syphilis in pregnancy were included. A total of 356 (TCs) were analyzed. The main doubts about syphilis during pregnancy raised by primary care professionals were related to the need for retreatment (35%), diagnostic definition (23%) and initial treatment (16%). In addition, 95% of TCs were suitable for diagnosing and treating syphilis based on the 2020 Brazilian Ministry of Health guideline. This study suggests that TCs can identify failures in the diagnosis and treatment of public health problems and support decision making in PHC involving syphilis in pregnancy. [ABSTRACT FROM AUTHOR]

Background:The factors necessitating the need for referrals for in-person evaluations by a dermatologist are not adequately understood and have not been studied using automated text mining so far. The objective of this study was to compare the prevalence of required in-person dermatologist care in the presence or absence of certain clinical features. Methods:Observational cross-sectional study of 11,661 teledermatology reports made from February 2017 to March 2020. Results:The need for dermoscopy was associated with a 348% increase in the possibility of referral for in-person dermatologist evaluations (prevalence ratio [PR]: 4.48, 95% confidence interval [CI]: 4.17–4.82). Infectious diseases were associated with a 64% lower possibility of referral (PR: 0.36, 95% CI: 0.30–0.43). Discussion:Some lesions and poorly documented cases are challenging to assess remotely. This study presents a different approach to research more detailed data from teledermatology reports, using text mining, and points out the risk magnitude for demanding dermatologic in-person care of which feature analyzed. As limitations, the variables related to lesion location, size, and extension were not analyzed and the dictionaries used were originally in Brazilian Portuguese. Conclusions:Teledermatology seems sufficient for the management of 75% of clinical cases, especially acute in young patients with inflammatory or infectious lesions. Referrals for in-person dermatologist consultations were not only strongly associated with the need for dermoscopy, but also for therapeutic reasons like surgical procedures, phototherapy, and the use of some systemic medications. Introduction:Nurses have proven to be fundamental for the expansion and consolidation of primary health care (PHC), as well as the development of digital health strategies. We explored the results of a synchronous telephone teleconsultations service between professionals for nurses in Brazil. Methods:This is a cross-sectional study. We retrieved data from teleconsultations registry. All teleconsultations answered by the team of nurses between September 2018 and July 2021 were analyzed regarding the reasons (according to International Classification of Primary Care, 2nd edition—ICPC-2) and decisions of the teleconsultation. Results:There were 9,273 phone teleconsultations registered in the period, requested by 3,125 nurses from all states throughout the country, of which 56.9% called once and 15.9% used the teleconsultations at least 4 times. We found 362 different reasons for solicitations, which were classified according to the ICPC-2 chapters. The most frequent codes were respiratory (25.9%), general and unspecified (21.2%), and skin (21.2%), which corresponded to 68% of the total sample. Most teleconsultations (66.9%) had as outcome the maintenance of the case at PHC. Conclusion:Teleconsultations are widely used and address a broad number of situations. This service may improve the quality of Brazilian PHC and promote the development of clinical reasoning and critical thinking by nurses. [ABSTRACT FROM AUTHOR]

O uso da teleoftalmologia pode ser uma estratégia viável para rastrear e diagnosticar as principais doenças oculares em pacientes na atenção primária à saúde. O objetivo deste estudo é descrever a realização de 30.315 telediagnósticos em oftalmologia em pacientes da atenção primária e a resolutividade desse serviço. Estudo transversal para avaliar os telediagnósticos realizados em pacientes atendidos nos oito pontos remotos do projeto TeleOftalmo entre 2 de janeiro de 2018 a 31 de dezembro de 2020. Foram avaliadas as características demográficas dos pacientes, os motivos de encaminhamento, os diagnósticos realizados conforme faixa etária e a resolutividade do telediagnóstico. Os pacientes eram, na maioria, do sexo feminino (66,1%), adultos (70,3%) e encaminhados ao telediagnóstico principalmente devido à baixa acuidade visual (60,5%). Os erros refrativos foram o diagnóstico mais prevalente em todas as faixas etárias. A presbiopia foi a doença ocular mais prevalente em adultos (65,4%) e idosos (64%), seguida da catarata (41,3%) e suspeita de glaucoma (10,6%) em idosos. Dos 30.315 pacientes que realizaram o telediagnóstico, 70,5% tiveram suas queixas oculares totalmente solucionadas, sem a necessidade de encaminhamento ao oftalmologista presencial. A telemedicina pode ser resolutiva para as doenças oculares mais prevalentes na população, aumentando a oferta de diagnósticos, qualificando e auxiliando na redução das filas de espera por atendimento oftalmológico. The use of teleophthalmology may be a viable strategy to track and to diagnose major eye diseases primary health care patients. This study aimed to describe the performance of 30,315 telediagnoses in ophthalmology in primary care patients and the case management of this service. This is a cross-sectional study to evaluate the telediagnosis performed in patients treated at the eight remote points of the TeleOftalmo project from January 2nd, 2018, to December 31st, 2020. The patients’ demographic characteristics, the reasons for referral, the diagnoses made according to age group, and the case management of the telediagnosis were evaluated. Most patients were female (66.1%), adults (70.3%), and referred to telediagnosis mainly due to reduced visual acuity (60.5%). Refractive errors were the most prevalent diagnosis in all age groups. Presbyopia was the most prevalent eye disease in adults (65.4%) and older adults (64%), followed by cataracts (41.3%) and suspected glaucoma (10.6%) in older adults. In total, 30,315 patients underwent telediagnosis, 70.5% had their ocular complaints fully resolved, without the need for referral to an in-person ophthalmologist. Telemedicine can be resolutory for the most prevalent eye diseases in the population, increasing the supply of diagnoses, qualifying and assisting in reducing waiting lines for ophthalmologic care. La teleoftalmología puede ser una estrategia viable para el tamizaje y diagnóstico de las principales enfermedades oculares en pacientes en la atención primaria de salud. El objetivo de este estudio es describir la realización de 30.315 telediagnósticos en oftalmología en pacientes de atención primaria y la resolución de este servicio. Estudio transversal para evaluar los telediagnósticos realizados a pacientes en los ocho puntos de atención remota del proyecto TeleOftalmo entre el 2 de enero de 2018 y el 31 de diciembre de 2020. Se evaluaron las características demográficas de los pacientes, los motivos de derivación, los diagnósticos realizados según el grupo de edad y la resolución de los telediagnósticos. Los pacientes eran en su mayoría mujeres (66,1%), adultos (70,3%) y remitidos para telediagnóstico, principalmente por baja agudeza visual (60,5%). Los errores de refracción fueron el diagnóstico más prevalente en todos los grupos de edad. La presbicia fue la enfermedad ocular más frecuente en adultos (65,4%) y ancianos (64%), seguida de catarata (41,3%) y sospecha de glaucoma (10,6%) en ancianos. De los 30.315 pacientes que realizaron el telediagnóstico, el 70,5% tuvo sus quejas oculares totalmente resueltas, sin necesidad de derivación presencial al oftalmólogo. La telemedicina puede ser una herramienta útil para detectar las enfermedades oculares más prevalentes en la población debido al aumento de los diagnósticos y la ayuda que produce al reducir la lista de espera para atención oftalmológica.

Introduction: Telemedicine facilitates the care in health by distance. This health technology has shown good clinical results and user satisfaction. The satisfaction is a main indication of service quality, your evaluation allows changes in the quality of care, identifies problems and enables better management and behaviors of health professionals. The increase in the use of telemedicine in the world has emerged in a need to understand the quality of this service. Objective: To translate, culturally adapt and validate a questionnaire to assess the satisfaction of patients treated by telemedicine. Methods: The version adapted after evaluation by a committee of judges was used in a pre-test with 30 patients seen in the project. The results of the pre-test were evaluated in order to obtain an adequate version of the instrument. In addition, this questionnaire was applied to a sample of 141 patients treated via telemedicine. Analysis of internal consistency and construct validation were performed. Results: The Global Content Validity Coefficient (CVC) score was 0.942, demonstrating the questions' clarity, relevance and relevance. The instrument showed internal consistency with a standardized Cronbach’s alpha of 0.6; considered acceptable. The exploratory factor analysis showed a KMO of 0.56 and Bartlett's sphericity test showed a value of 0.001. Conclusions: The Brazilian version of the Questionnaire for the Evaluation of Patient Satisfaction Via Telemedicine (QAS-Tele) is an easy and viable instrument for the evaluation of the satisfaction of patients treated by telemedicine. Introducción: La telemedicina facilita el acceso a la atención a los pacientes. Esta tecnología ha demostrado buenos resultados clínicos y en la satisfacción de los usuarios. La satisfacción es uno de los principales indicativos de la calidad de los servicios, su evaluación permite cambios en la calidad de la atención, identifica problemas y posibilita mejor gestión y comportamiento de los profesionales. El aumento del uso de la telemedicina en el mundo ha sugerido el entendimiento de la cualidad de esos servicios. Objetivo: Traducir, adaptar culturalmente y validar un cuestionario para evaluación de la satisfacción de pacientes atendidos por telemedicina. Métodos: Se desarrolló una evaluación por un comité de jueces con la versión adaptada y después se llevó a cabo un pre-test con 30 pacientes atendidos en el proyecto TeleOftalmo. La prueba previa ocurrió para obtener una versión adecuada del instrumento. Además, se aplicó en una muestra de 141 pacientes atendidos vía telemedicina. Análisis de consistencia interna y de validación de constructo fueron realizadas. Resultados: El coeficiente de validez de contenido (CVC) Global de 0,942, demostrando claridad, pertinencia y relevancia de las cuestiones. El instrumento presentó consistencia interna con alfa de Cronbach estandarizado de 0,6; considerado aceptable. El análisis factorial exploratorio presentó un KMO de 0,56 la prueba de esfericidad de Bartlett presentó valor de 0,001. Conclusiones: El Cuestionario Brasileño de Evaluación de la Satisfacción de los Pacientes Atendidos por Telemedicina (QAS-Tele) es fácil y viable para evaluar la satisfacción de los pacientes atendidos por telemedicina. Introdução: A telemedicina facilita o acesso ao cuidado para os pacientes. Essa tecnologia tem apresentado bons resultados clínicos e de satisfação dos usuários. A satisfação é um dos principais indicadores de qualidade dos serviços, e sua avaliação permite mudanças na qualidade da prestação de cuidados, identifica problemas e viabiliza a melhor gestão e os melhores comportamentos dos profissionais de saúde. Do aumento do uso da telemedicina no mundo emergiu a necessidade de entendimento da qualidade desse serviço. Objetivo: Traduzir, adaptar culturalmente e validar um questionário para avaliação da satisfação de pacientes atendidos por telemedicina. Análises de consistência interna e de validação de constructo foram realizadas. Resultados: O coeficiente de validade de conteúdo (CVC) global foi 0,942, demonstrando clareza, pertinência e relevância das questões. O instrumento apresentou consistência interna com alfa de Cronbach estandardizado de 0,6, considerado aceitável. A análise fatorial exploratória apresentou critério de Kaiser-Meyer-Olkin de adequação de amostragem de 0,56 e o teste de esfericidade de Bartlett apresentou valor de 0,001. Conclusões: A versão brasileira do Questionário de Avaliação da Satisfação de Pacientes Atendidos via Telemedicina (QAS-Tele) é um instrumento fácil e viável para a avaliação da satisfação dos pacientes atendidos por telemedicina.

Introdução: no final do ano de 2019 casos de uma pneumonia atípica causando grave adoecimento na cidade de Wuhan, na China, chamaram a atenção do mundo e as consequências catastróficas da Doença causada pelo Novo Coronavírus de 2019 (COVID-19) ainda estão em desenvolvimento no primeiro semestre de 2022. Com as restrições necessárias para conter o vírus, os serviços de saúde precisaram se reinventar para não deixar de atender seus pacientes ambulatoriais. Comparada ao resto do mundo, mas principalmente aos países de alto desenvolvimento humano, a legislação brasileira encontrava-se, no início da pandemia por COVID-19, vinte anos atrasada, não permitindo a realização de consultas médicas remotas. Entre os meses de março e abril de 2020, tanto uma portaria do Ministério da Saúde, quanto uma lei federal autorizaram a realização de atendimentos a distância enquanto durassem os efeitos desta pandemia. Várias características permitiram ao Hospital de Clínicas de Porto Alegre (HCPA) posicionar-se na vanguarda do oferecimento de consultas remotas durante a pandemia, tais como proximidade institucional com o Núcleo Técnico-Científico de Telessaúde do Rio Grande do Sul, da Universidade Federal do Rio Grande do Sul, presença de sistemas de prontuários e de gestão informatizados e em atualização constante há décadas e apoio institucional por parte da direção do Hospital. O impacto das consultas remotas tem sido avaliado em diversas dimensões e estudos tem demonstrado efeitos positivos na redução da emissão de gases de efeito estufa com intuito de melhorar a saúde planetária. Método: esta tese é composta por três estudos. Um estudo descritivo das características que permitiram ao HCPA a rápida implantação da nova modalidade de atendimento; um estudo transversal que apresenta a produção e as características das consultas remotas realizadas pelo HCPA ao longo do ano de 2020; e um estudo transversal que estimou a redução da pegada de carbono com a realização dessas consultas no primeiro ano da pandemia. Resultados: realizou-se entre 17 de março e 31 de dezembro de 2020 um total de 52.878 consultas remotas atendidas por 849 profissionais diferentes de variadas especialidades médicas, profissões da saúde e níveis de formação. A soma das distâncias deixadas de percorrer com a realização dessas consultas foi de 4.108.208 quilômetros que geraram a redução da emissão de 939,64 toneladas de gases de carbono na atmosfera. Conclusões: Os estudos dessa tese demonstram que é factível para um hospital público universitário como o HCPA implantar as consultas remotas em larga escala e em diversas especialidades médicas e profissões da saúde de forma ágil e abrangente, beneficiando pacientes com redução de deslocamentos e consequentemente a saúde planetária. Introduction: at the end of 2019, cases of an atypical pneumonia causing serious illness in the city of Wuhan, China, drew the world's attention and the catastrophic consequences of the Disease caused by the Novel Coronavirus 2019 (COVID-19) are still developing in the first semester of 2022. With the restrictions necessary to contain the virus, health services had to reinvent themselves in order not to stop serving their outpatients. Compared to the rest of the world, but especially to high human development countries, Brazilian legislation was, at the beginning of the COVID-19 pandemic, twenty years late, not allowing remote medical consultations. Between March and April 2020, both an ordinance from the Ministry of Health and a federal law authorized remote care for the duration of the effects of this pandemic. Several characteristics allowed the Hospital de Clínicas de Porto Alegre (HCPA) to position itself at the forefront of offering remote consultations during the pandemic, such as institutional proximity to the Technical-Scientific Telehealth Center of Rio Grande do Sul, at the Federal University of Rio Grande do Sul, presence of computerized medical records and management systems that have been constantly updated for decades and institutional support from the Hospital's management. The impact of remote consultations has been evaluated in several dimensions and studies have shown positive effects in reducing the emission of greenhouse gases in order to improve planetary health. Method: This thesis consists of three studies. A descriptive study of the characteristics that allowed the HCPA to quickly implement the new service modality; a cross-sectional study that presents the production and characteristics of remote consultations carried out by the HCPA throughout 2020; and a cross-sectional study that estimated the reduction of the carbon footprint by carrying out these consultations in the first year of the pandemic. Results: A total of 52,878 remote consultations were carried out between March 17 and December 31, 2020, attended by 849 different professionals from different medical specialties, health professions and training levels. The sum of the distances left to travel with these consultations was 4,108,208 kilometers, which generated a reduction in the emission of 939.64 tons of carbon gases in the atmosphere. Conclusions: The studies of this thesis demonstrate that it is feasible for a public university hospital like the HCPA to implement remote consultations on a large scale and in various medical specialties and health professions in an agile and comprehensive way, benefiting patients with reduced travel and consequently planetary health.

Após sua descrição inicial em dezembro de 2019, a infecção causada pelo SARS-CoV-2, Covid-19, espalhou-se rapidamente pelo mundo, sendo classificada em março de 2020 como pandemia. Experiências de epidemias anteriores mostram que, de acordo com as características do agente etiológico, medidas não farmacológicas, como lavagem das mãos, higienização de superfícies, uso de máscaras, isolamento de casos confirmados, quarentena, distanciamento social e lockdown, são eficazes e fundamentais para evitar a propagação de doenças infecciosas. Informações sobre a forma de transmissão do vírus e sua dinâmica no organismo do hospedeiro permitem propor a adesão dessas estratégias. Ao avaliar evidências do uso dessas medidas no controle de doenças com dinâmica semelhante à causada pelo novo coronavírus, encontra-se embasamento para sua aplicação prática. Além disso, a experiência de outros países no enfrentamento a essa doença ao longo do ano 2020 corroboram ainda mais para a adoção dessas condutas. À luz da história mundial, das evidências científicas e das experiências de outros países, faz-se necessária a aplicação dessas estratégias no combate contra a Covid-19, enquanto as medidas complementares, como as vacinas, ainda não estão disponíveis de forma ampla para toda a população. After its initial description in December 2019, the infection caused by SARS-CoV-2, Covid-19, spread rapidly around the world, being classified in March 2020 as a pandemic. Experiences from previous epidemics show that, according to the characteristics of the etiologic agent, non-pharmacological measures such as hand washing, surface cleaning, use of masks, isolation of confirmed cases, quarantine, social distance and lockdown, are effective and essential to avoid the spread of infectious diseases. Information on the form of transmission of the virus and its dynamics in the host organism, allow us to propose the adherence of these strategies. When evaluating evidence of the use of these measures in the control of diseases with dynamics similar to that caused by the new coronavirus, there is a basis for its practical application. In addition, the experience of other countries in tackling this disease throughout 2020, further corroborates the adoption of these conducts. In the light of world history, scientific evidence and the experiences of other countries, it is necessary to apply these strategies in the fight against Covid-19, while complementary measures, such as vaccines, are not yet widely available for all the population.

O SARS-CoV-2, vírus causador da Covid-19, é o terceiro coronavírus a causar doença grave em humanos e que apresentou disseminação global nas duas últimas décadas. Nesse contexto, diversos departamentos nacionais de saúde pú- blica, entre eles o Ministério da Saúde do Brasil, trouxeram destaque àquilo que era, até então, considerado um serviço de apoio ao sistema de saúde: a telessaúde e a telemedicina. Pretendemos apresentar as ações realizadas por um serviço nacional de telessaúde no Brasil, tanto no suporte aos profissionais de saúde da Atenção Primá- ria à Saúde quanto a pacientes, além de discutir o potencial de reorganizar um sistema de saúde. Estudo de prevalência que sumariza as ações de telemedicina adotadas pelo TelessaúdeRS-UFRGS no período da 9ª à 27ª semana epidemiológica de 2020 para apoio aos serviços de saúde do Sistema Único de Saúde (SUS). Houve aumento de 76,8% da demanda de teleconsultorias telefônicas no período avaliado em comparação com o mesmo período em 2019, sendo 28,8% dessa demanda total decorrente de dúvidas relacionada à Covid-19. A pandemia por Covid-19 demandou rápida resposta com a organização de materiais sobre a doença, uma nova equipe para execução das atividades de telemonitoramento e teleconsultas, além da elaboração de um manual para teleconsultas na Atenção Primária à Saúde. SARS-CoV-2, the virus that causes Covid-19, is the third coronavirus to cause severe disease in humans and to spread globally in the past two decades. In this context, several national public health departments, including the Brazilian Ministry of Health, highlighted what was, until then, considered a support service to the health system: telehealth and telemedicine. We intend to present the actions carried out by a national telehealth service in Brazil, both as a Primary Health Care (PHC) support service to professionals and to patients, as well as discussing the potential to reorganize a health system. This is a prevalence study that summarizes the measures adopted by Brazilian Telehealth Center from the 9th to the 27th epidemiological weeks of 2020 to support the health services of the Brazilian Unified Health System (SUS). There was an increase of 76.8% in the demand for telephone teleconsultations during the evaluated period compared to the same period in 2019, with 28.8% of the entire demand arising from doubts related to Covid-19. New epidemiological and demographic challenges require new ways of organizing healthcare systems. This essay proposes telehealth as an organizational tool, capable of smoothing the iron triangle of health care and facilitating achievement of the triple aim, namely greater access and quality with cost reduction. The integration of telehealth into the referral process and the transition between care services increases the resolvability of primary health care (PHC), facilitates the coordination of care, promotes treatment adherence, decreases readmissions, and stimulates quaternary prevention. This essay introduces the notion of telehealth as a meta-service that confers technological density to PHC and allows it to become an effective coordinator of care, organizing the flow of information, people and supplies. In view of the innovations proposed, it is essential to evaluate the impact of existing telehealth initiatives so as to extend their applicability as meta-service. Nuevos desafíos epidemiológicos y demográficos demandan nuevas formas de organizar los sistemas de salud. El presente ensayo propone la telesalud como herramienta organizacional, capaz de suavizar el triángulo de hierro de la atención a la salud y de facilitar la búsqueda por el triple aim, por su potencial de aumento del acceso y calidad con reducción de costo. La integración de la telesalud al proceso de referencia y transición entre servicios asistenciales aumenta la capacidad resolutiva de la Atención Primaria a la Salud (APS), favorece la coordinación del cuidado, promueve la adhesión terapéutica, disminuye reinternaciones y estimula la prevención cuaternaria. Este ensayo propone la telesalud como metaservicio que confiere densidad tecnológica a la APS y permite que ella se convierta en coordinadora efectiva del cuidado, pasando a organizar el flujo de informaciones, personas e insumos. Frente a las innovaciones propuestas, es esencial evaluar el impacto de acciones ya existentes de telesalud para viabilizar su aplicación como metaservicio de salud. Novos desafios epidemiológicos e demográficos demandam novas formas de organizar os sistemas de saúde. O presente ensaio propõe a telessaúde como ferramenta organizativa, capaz de suavizar o triângulo de ferro da atenção à saúde e de facilitar a busca pelo triple aim, pelo seu potencial de aumento do acesso e qualidade com redução de custo. Materiais e métodos: O desenvolvimento do curso ocorreu entre dezembro de 2016 e dezembro de 2017. A descrição do percurso metodológico foi baseada no modelo ADDIE (Analysis, Design, Development, Implementation e Evaluation). Resultados: A análise envolveu a identificação dos profissionais da APS como públicoalvo, suas necessidades de aprimoramento e objetivos de aprendizagem. Na fase de desenho foram escolhidos os assuntos do curso: alimentação saudável, mitos e verdades, materno-infantil, alergias e intolerâncias, doenças crônicas, transtornos alimentares e terapia nutricional enteral domiciliar. Na etapa de desenvolvimento foram criados os recursos educacionais. Na implementação, houve a disponibilização na plataforma e o acompanhamento pedagógico iniciou. Na avaliação, o desempenho e satisfação foram avaliados. Conclusão: O emprego do modelo ADDIE permitiu desenvolver recursos mais adequados à realidade do público-alvo, no entanto, é necessário discutir acerca dos obstáculos durante a formulação do curso e propor estratégias para minimizá-los. Espera-se que esse trabalho contribua para compreender as limitações e potencialidades da educação a distância como estratégia de educação permanente no campo da alimentação e nutrição. Aims: This paper describes and discusses the creation of an online course on food and nutrition for graduated health professionals from Primary Health Care (PHC) throughout Brazil. Materials and methods: Course development occurred between December 2016 and December 2017. The description of the methodology was based on ADDIE (Analysis, Design, Development, Implementation and Evaluation) model. Results: The analysis involved the identification of PHC professionals as a target audience, their needs for improvement and the learning objectives. In the design phase, the topics were chosen: healthy eating, myths and truths, nutrition for mothers and infants, allergies and intolerances, non-communicable diseases, eating disorders and home enteral nutritional therapy. At the development, audio-visual and text-based educational resources were created. In the implementation, the course was made available in the learning environment and the pedagogical support begin. At the evaluation, performance and satisfaction were assessed. Conclusion: The use of ADDIE model allows developing resources appropriated to target audience, however, it is necessary to discuss the obstacles in the formulation of the course and propose strategies to minimize them. This work is expected to contribute to understand the limitations and possibilities of distance education as a continuing education strategy in the field of food and nutrition. Teleducação

Probably yes. There is no evidence to prove the safety of the use of larvicides in drinking water. However, there are few studies evaluating the effect on the population of mammals, as are the bioavailabilities and cytotoxicity reviews show that in low concentrations are safe and no genotoxic or carcinogenic effects. Larvicides evaluated and authorized for use in drinking water by the World Health Organization, intended for human consumption are: DIFLUBENZUROM, METOPRENO, NOVALUROM, Pirimifós/, PIRIPROXIFEM, ESPINOSADE, TEMEFÓS, beyond Bacillus thuringiensis israelensis (BTI). It is recommended to always use the correct dose of larvicides. Temephos is used to treat outbreaks, but should not be used in fish tanks. Approved for use in drinking water. The pyriproxyfen is an ether and there is no evidence to support that it causes damage in mammalian fetuses. BTI are recommended as larvicides for use in public health, it is a bacterium that contact with water releases toxic to the larvae of some insects. The application should be performed by trained professional. BTI has no acceptable daily intake, however the guidance of use should be 1-5mg/liter. The evaluation of chemicals of any kind should always stress the possibility of bioaccumulation. Substances that are not biodegraded are biopersistent and remain at high levels in the tissues of living things. So even if the substances are not for direct human consumption, an appraisal of its use in agriculture, river bed and pastures should be carefully evaluated. Probablemente sí. La aplicación debe ser realizada por profesionales capacitados. BTI no tiene ninguna ingesta diaria admisible, sin embargo, la orientación de uso debe ser 1-5 mg/litro. La evaluación de los productos químicos de cualquier tipo siempre se debe hacer hincapié en la posibilidad de bioacumulación. Las sustancias que no se biodegradan, son biopersistente y se mantienen en niveles altos en los tejidos de los seres vivos. A avaliação de substâncias químicas de qualquer natureza sempre deve salientar a possibilidade de bioacumulação. As substâncias que não são biodegradadas, são biopersistentes e mantêm-se em altas quantidades nos tecidos dos seres vivos. Portanto, mesmo que as substâncias não sejam para consumo humano direto a avaliação do seu uso na agricultura, leito de rio e pastos deve ser avaliada criteriosamente.

Objetivo: Discutir a associação entre microcefalia e a infecção materna por Zika Vírus. A microcefalia é o tamanho da cabeça menor do que o esperado em comparação com bebês do mesmo sexo e idade. Entre as causas conhecidas, estão as infecções congênitas. O aumento de casos entre outubro e novembro de 2015 no nordeste brasileiro, que coincidiu com a presença da circulação de novo vírus no país, em maio do mesmo ano, criou a hipótese de associação entre a microcefalia e a infecção materna durante a gravidez. O Zika Vírus é um arbovírus similar ao da Febre Amarela e da Dengue, transmitido principalmente através da picada do Aedes aegypti. A provável transmissão por relação sexual e transfusão de sangue, além de outros vetores como o Aedes albopictus e possivelmente até o pernilongo (Culex sp) aumentam a necessidade de cuidados preventivos em relação à infecção. O exame para detecção viral idealmente é realizado até o quinto dia após o início dos sintomas. Sorologias ainda não são amplamente disponíveis no Brasil. Métodos: Revisão narrativa da literatura. Conclusão: A associação entre casos de microcefalia e o Zika Vírus é embasada nos relatos de relação têmporo-espacial, padrão de alterações neurológicas associado a malformações congênitas, presença do RNA viral no líquido amniótico e nos tecidos de fetos. As respostas definitivas de causalidade serão possíveis após pesquisas e disponibilidade de exames laboratoriais. As evidências até agora apoiam fortemente esta hipótese e todas as medidas preventivas devem ser estimuladas. Objective: To discuss the association between microcephaly and intrauterine infection by Zika virus. Microcephaly occurs when a child is born with a head smaller than expected when compared to babies of the same sex and age. Known causes of microcephaly include congenital infections. The increase in the number of microcephaly cases in Northeast Brazil between October and November 2015, which coincided with the emergence of Zika virus in the country in May of the same year, led to the hypothesis of an association between microcephaly and intrauterine Zika virus infection. Zika is an arbovirus that is closely related to yellow fever and dengue viruses. Aedes aegypti mosquitoes are the primary vector of transmission. Possible transmission through sexual contact and blood transfusion, as well as the implication of other vectors, such as Aedes albopictus and even Culex sp increases the need for preventive action. The test for viral detection is ideally performed before the 5th day following the onset of symptoms. Serology tests are not yet widely available in Brazil. Methods: We performed a narrative literature review. Conclusion: The hypothesis of an association between microcephaly and Zika virus is based on reports of spatial/temporal relationship, pattern of neurologic alterations associated with congenital malformations, and findings of viral RNA in amniotic fluid and fetal tissue. Definitive conclusions about the causality can only be reached after further research and availability of laboratory tests. The current evidence strongly supports the association between microcephaly and Zika infection, and all preventive measures must be stimulated. Objetivo: Discutir las asociaciones entre microcefalia e infección materna por Virus Zika. La microcefalia es el tamaño de la cabeza menor de lo esperado en comparación con los bebés del mismo sexo y edad. Entre las causas conocidas están las infecciones congénitas. El aumento de casos entre octubre y noviembre de 2015 en el nordeste de Brasil, que coincidió con la presencia de la nueva circulación del virus en el país en mayo del mismo año, creó la hipótesis de asociación entre la microcefalia y la infección de la madre por Virus Zika durante el embarazo. El virus Zika es un arbovirus similar al dengue y la fiebre amarilla. El virus se transmite a través de la picadura del mosquito Aedes aegypti. La probable transmisión por vía sexual y por la transfusión de sangre - además de otros vectores como el Aedes albopictus y posiblemente el mosquito Culex sp - aumentan la necesidad de atención preventiva contra la infección. El cuadro clínico es benigno, autolimitado, caracterizado por erupción maculopapular asociado con otros síntomas tales como conjuntivitis, artralgia y la inflamación de las articulaciones. El examen para la detección del virus se realiza idealmente por el quinto día después de la aparición de los síntomas. Las pruebas serológicas no están ampliamente disponibles en Brasil. Métodos: Revisión de literatura. Conclusión: La asociación entre los casos de microcefalia y el virus Zika se basa en informes de patrón de relación temporo-espacial de los trastornos neurológicos asociados con malformaciones congénitas, el ARN viral presente en el líquido amniótico y tejidos de fetos. Las respuestas definitivas de causalidad serán posibles después de la investigación y la disponibilidad de pruebas de laboratorio. Hasta ahora, las evidencias apoyan firmemente esta hipótesis y todas las medidas preventivas deben ser estimuladas. Telemedicina
