Patient safety is acknowledged as a primary aim of anesthesiology. Anesthesia records constitute the main document of the intraoperative course of anesthesia administration. In this paper, we postulate that anesthesia record systems should be based on an integral tamper-proof design and provide specific technology characteristics to ensure data immutability, accessibility and transparency. Issues and limitations regarding current anesthesia record technologies are reviewed. We introduce a novel anesthesia record system designed for patient safety optimization which integrates dedicated hardware, blockchain technology and decentralized storage solutions. We propose an oracle network in which anesthesiologists run independent Sybil-resistant nodes which broadcast biosensor time series to decentralized storage systems and generate proofs of existence on public blockchains. Records are biometrically signed and incorporate information on the temporo-spatial relation between the anesthetized patient and the professional in charge through a unique personal-transponder wearable device. Compatibility for data science and machine learning implementation are discussed. Finally, we evaluate future impact and technological potential.

Abstract: Introduction: Post-traumatic stress disorder (PTSD) has been proposed as a risk factor for migraine chronification. The aim of this study was to investigate the frequency of PTSD and traumatic life events (TE) in patients with episodic migraine (EM) and chronic migraine (CM) and their impact on clinical parameters, other comorbidities, and migraine biomarkers. Material and methods: Patients with EM and CM ICHD-3Beta according to the International Classification of Headache Disorders (third edition; beta version) were recruited at a headache unit and a primary care centre. We used validated questionnaires to investigate PTSD, TE, cranial autonomic symptoms, comorbidities (depression, anxiety, and fatigue), disability, migraine impact, and quality of life. Baseline serum levels of CGRP, VIP, and PACAP were determined by ELISA. Results: The study included 116 patients: 35 with EM and 81 with CM. Nineteen presented refractory migraine (RM). PTSD was detected in 23 patients (19.8%): 20 with CM and 3 with EM (chi-square: P=.046; Fisher T: P=.073). Neither the frequency of any TE nor the number of TEs per patient were different between CM and EM. A total of 5/19 patients with RM had experienced sexual violence (vs 2/97 with non-RM; P=.002). PTSD was associated with more autonomic symptoms; higher scores on anxiety, depression, and fatigue scales; and poorer quality of life; and it did not change neuropeptide levels. Conclusions: Our results suggest that PTSD is frequent in patients with migraine, and especially CM, in our setting; history of sexual violence is particularly frequent in patients with RM. PTSD has a negative impact on migraine, with higher numbers of comorbidities and poorer quality of life; therefore, further research is needed in this patient group.

The study of organisations’ information security cultures has attracted scholars as well as healthcare services industry to research the topic and find appropriate tools and approaches to develop a positive culture. The vast majority of studies in Saudi national health services are on the use of technology to protect and secure health services information. On the other hand, there is a lack of research on the role and impact of an organisation’s cultural dimensions on information security. This research investigated and analysed the role and impact of cultural dimensions on information security in Saudi Arabia health service. Hypotheses were tested and two surveys were carried out in order to collect data and information from three major hospitals in Saudi Arabia (SA). The first survey identified the main cultural-dimension problems in SA health services and developed an initial information security culture framework model. The second survey evaluated and tested the developed framework model to test its usefulness, reliability and applicability. The model is based on human behaviour theory, where the individual’s attitude is the key element of the individual’s intention to behave as well as of his or her actual behaviour. The research identified a set of cultural and sub-cultural dimensions in SA health information security and services.

Standardized measures of behavior can be powerful tools for assessing the impact of whale watching activities on natural populations of cetaceans. To determine the possible impact of tourism on dolphins between a period without whale watching (1989–1992) (T1) and a period with whale watching (2010–2020) (T2), we examined the changes in the rate of surface behaviors, the group size of long-time resident bottlenose dolphins living in the waters of the Humboldt Current off Chile, and for T2 alone, we compared these differences between two localities, the Punta de Choros and Chañaral de Aceituno coves. We observed a significant decrease in the group size of the resident population and in the frequency of surface events associated with the absence and presence of tourism. For T2, we observed significant differences for the frequency of surface events between the Chañaral de Aceituno and Punta de Choros coves and differences in the frequency of surface events at different hours of the day. This was associated with the number of vessels at the time of the encounter. In addition, we observed for T2 that the most observed instantaneous response of the dolphins to the presence of tourist vessels was to avoid the boats, while approaching the boats was the least observed response. The number of vessels present in each dolphin encounter was the most important variable for our model as it explains these differences. These results show that tourism vessels have a significant impact on dolphin behavior and sociability, while the same population of dolphins have different spatial and temporal responses to different impacts of tourism. Further studies are needed to establish whether changes in the rate of surface behaviors are associated with higher levels of stress in dolphins and with effects on their health and reproductive success in the long term.

Aim: To analyze the frequency and variables related to inappropriate rivaroxaban dosage in clinical practice and its impact on outcomes after 2 years. Materials & methods: Postauthorization, observational, multicenter study, in which atrial fibrillation patients, treated with rivaroxaban ≥6 months were included. Results: A total of 1421 patients (74.2 ± 9.7 years, CHA 2 DS 2 -VASc 3.5 ± 1.6) were included. Overall, 22.9% received rivaroxaban 15 mg. The proper dose of rivaroxaban was taken by 83.3% (9.7% underdosed, 7.0% overdosed). Older age and renal insufficiency were associated with inadequate rivaroxaban dosage. There was a trend toward higher all-cause mortality among underdosed patients (adjusted hazard ratio 1.39; 95% CI 0.75–2.58), and more bleedings in overdosed patients (2.29 vs 0.80 events/100 patient-years; p = 0.14). Conclusion: In clinical practice, rivaroxaban is properly dosed in most patients.

This research focuses on the effects of autonomy on motivation and year 7’s perspectives on choice-based art projects. It explores the implications and possible contradictions of autonomy in the context of a diverse, ‘requiring improvement’ school in East Anglia. Data from questionnaires, interviews and participant observation revealed that a choice-based project had a positive impact on students’ attitudes towards materials, outcomes, teacher exemplars, perceptions of competence, general interest and external recognition. Implications for future research and classroom practice are also discussed.

- Abstract
The East-Asian aphid Tinocallis (Tinocallis) zelkowae is recorded for the first time in the Iberian Peninsula on Zelkova serrata (Ulmaceae) in the Viveros Gardens of the city of Valencia (Spain). This is the second record of the species in Europe, after being cited on a Japanese zelkova bonsai in England in 1974. Alate viviparous females and sexuales (winged males and apterous oviparae) are here described, and we present the molecular sequence of the mitochondrial cytochrome oxidase I, which allows comparison with the recently described T. (T.) latifoliae on in Zelkova serrata var. latifolia. It is the fifth species of Tinocallis genus that is known in territories of Iberian Peninsula, three of which are also native to Southeast Asia.

Scientific research to date has not focused on the issue of bilingual advertising creation. As a contribution to this line of research, this article uses a specific sociolinguistic approach, taking the Basque country as its model. Thus, using semi-structured in-depth interviews, it describes the internal processes that major advertising agencies follow in order to create advertisements in two official Languages, Basque and Spanish. The study highlights the fact that the creation process is primarily in Spanish, adapting to Basque at a later date, a fact which has repercussions on the effectiveness of the campaigns in the second language. The main conclusion shows a need to define and design a prototype model of bilingual development, which at the present time is nonexistent.

The optimum dispersion and rheological properties of red clay-based ceramic suspension loaded with unary and binary starch were investigated in aqueous medium. The aqueous ceramic suspension was prepared consisting of red clay, quartz, feldspar, and distilled water. Using a polyelectrolyte dispersant (Darvan 821A), the ternary ceramic powder was initially optimized to give the smallest average particle size at 0.8 wt. (%) dispersant dosage as supported by sedimentation test. This resulted into an optimum high solid loading of 55 wt. (%). The addition of either unary or binary starches to the optimized ceramic slurry increased the viscosity but maintained an acceptable fluidity. The mechanism of such viscosity increase was found to be due to an adsorption of starch granules onto ceramic surfaces causing tolerable agglomeration. Correspondingly, the rheological evaluations showed that the flow behaviors of all starch-loaded ceramic slurries can be described using Herschel-Bulkley model. The parameters from this model indicated that all ceramic slurries loaded with starch are shear thinning that is required for direct casting process.

Agricultural development in Indonesia had been conducted in a top-down manner since its independence, which has limited its effectiveness due to the gap between the reality faced by the development actors and actors who are involved in the agricultural supply chain. This paper provides evidence of a more participative model design by involving most of the actors in its process. The study was performed in action research by using a co-creation approach, involving actors who contributed their thoughts in designing the most suitable model that can support them in reaching a more sustainable supply chain for coffee agribusiness in Bandung Regency, West Java, Indonesia. The results show that a co-creation approach has managed to improve the performance of the coffee supply chain by the formation of a cooperative, which enhanced coordination among stakeholders. Furthermore, the involvement of farmers provided significant contributions in the design of the model.

Systems Training for Emotional Predictability and Problem Solving (STEPPS) is a group psychotherapy approach that is effective for patients with borderline personality disorder (BPD) in a public health care setting. The sample in this study comprised 118 outpatients with BPD who were asked to participate in a Spanish-adapted version of STEPPS for 18 months, a psychotherapy program that could be added to their usual psychiatric intervention. They were divided into an experimental group who participated in STEPPS, and a control group, who received treatment as usual. Several variables were collected and the Borderline Evaluation of Severity Over Time (BEST) scale was administered at pretest, Months 3 and 6, posttest (Month 18), and 2-year follow-up (Month 42), after which a post hoc data analysis was carried out. The STEPPS program improved the as-usual treatment provided previously, and the results were cost-effective. A higher educational level and good patient collaboration predicted better outcome.

BackgroundIn contrast with the setting of acute myocardial infarction, there are limited data regarding the impact of diabetes mellitus on clinical outcomes in contemporary cohorts of patients with chronic coronary syndromes. We aimed to investigate the prevalence and prognostic impact of diabetes according to geographical regions and ethnicity. Methods and resultsCLARIFY is an observational registry of patients with chronic coronary syndromes, enrolled across 45 countries in Europe, Asia, America, Middle East, Australia, and Africa in 2009–2010, and followed up yearly for 5 years. Chronic coronary syndromes were defined by ≥1 of the following criteria: prior myocardial infarction, evidence of coronary stenosis >50%, proven symptomatic myocardial ischaemia, or prior revascularization procedure.Among 32 694 patients, 9502 (29%) had diabetes, with a regional prevalence ranging from below 20% in Northern Europe to ∼60% in the Gulf countries. In a multivariable-adjusted Cox proportional hazards model, diabetes was associated with increased risks for the primary outcome (cardiovascular death, myocardial infarction, or stroke) with an adjusted hazard ratio of 1.28 (95% confidence interval 1.18, 1.39) and for all secondary outcomes (all-cause and cardiovascular mortality, myocardial infarction, stroke, heart failure, and coronary revascularization). Differences on outcomes according to geography and ethnicity were modest.ConclusionIn patients with chronic coronary syndromes, diabetes is independently associated with mortality and cardiovascular events, including heart failure, which is not accounted by demographics, prior medical history, left ventricular ejection fraction, or use of secondary prevention medication. This is observed across multiple geographic regions and ethnicities, despite marked disparities in the prevalence of diabetes.ClinicalTrials identifierISRCTN43070564

- Abstract
Background: Patients with cancer are at increased risk of developing symptoms of depression and anxiety. However, data on the variables associated with these symptoms in the long term are scant. This study aims to evaluate rumination and thought suppression as explanatory variables of depressive and anxiety symptoms at one‐ and five‐year follow‐up in patients diagnosed with cancer. Methods: A total of 131 patients with cancer were assessed at baseline (≤4 months of diagnosis), and at 1 and 5 years after diagnosis. A battery of self‐reported measures was used to evaluate anxiety and depressive symptoms, rumination, thought suppression, social support, and self‐efficacy. The associations among these variables were assessed with linear mixed‐effects models. Results: The models for depressive and anxiety symptoms explained 43.5% and 44.2% of the variance, respectively. Rumination was a significant explanatory variable of both depressive and anxiety symptoms over the five‐year follow‐up period, while thought suppression was only associated with anxiety symptoms. Female gender was associated with a higher risk of presenting anxiety symptoms but this same variable was also protective against depressive symptoms. Conclusions: The assessment and treatment of rumination and thought suppression in patients diagnosed with cancer is advisable, as these cognitive domains seem to be associated to symptoms of emotional disorders in the long term.

- Abstract

OBJECTIVE This study aims to analyse the association between rumination and thought suppression and the intensity of psychological distress in a sample of patients with recently diagnosed cancer. METHODS A total of 131 patients with a recent diagnosis of cancer were assessed within 4 months of diagnosis. All participants completed standardized questionnaires to assess psychological distress, rumination, and thought suppression. Multiple regression and logistic regression models were conducted to determine the associations between these variables. RESULTS The multiple regression model explained 50% of variance in psychological distress, with rumination independently explaining more than 16% of the variance. Thought suppression, however, was not significantly associated with distress. The logistic model for caseness of depression and anxiety explained, respectively, 33% and 48% of the variance. Anxiety caseness was also associated with social support and self-efficacy. CONCLUSIONS The findings of this study indicate that rumination is an important explanatory variable of psychological distress in patients with recently diagnosed cancer. Statistical models that include rumination could explain a significant proportion of variance of clinical caseness for both anxiety and depression. These findings suggest that assessing rumination in recently diagnosed patients could improve clinical practice by detecting individuals at greater risk of developing more severe psychological distress. These findings suggest that treating rumination as a key component of the therapeutic objectives of psychological interventions could improve clinical outcomes.

Goi-mailako teknologian sarturiko sektoretan, aeronautika adibidez, konpainien arteko lankidetza ezinbestekoa da arrisku teknologiko eta finantzario handiak dituzten proiektu berriak burutzeko.Guzti hau kontuan hartuta, mende hasieran hurrengo lanean zehar ondo azalduko den Fabrikazio Globalaren Sare Birtualak (FGSB)delako kontzeptu berria atera zen. Estrategia honen bidez, sektore berdinean lan egiten duten konpainiek elkarrekin lan egiten dute, lehiakideak izan arren, bakarrik burutzeko gai ez diren proiektuak aurrera eraman ahal izateko.Guzti hau Industria 4.0 edo Laugarren Iraultza Industrialaren testuinguruan. Iraultza hauen ezaugarriak hurrengoak dira: elektronika, informazio eta komunikazio teknologien erabilera, sistema ziber-fisikoen ezarpena eta interneten erabilera prozesu industrialetan. The globalization process experimented by the World in the last years has forced the companies to be more and more competitives so they can survive in the current market. For the high-tecnology sectors, as the aeronautical is,the cooperation between companies has became essential for developing new risky projects, in the technological and financial sense.Keeping this in mind, at the begining of this century was born the following concept: Golbal Manufacturing Virtual Network. This strategy consist on the cooperation between companies belonging to the same sector, even if they are competitors, so they can afford projects that would be imposible to afford individually. All that in a 4.0 Industry context, wich is characterized bythe use of the electronics, information anda comunication technologies, the establishment of the ciber-physical sistems and the use of the internet for the industrial procedures.
