14. Essays on human capital interventions in developing countries

Abstract

In the first chapter, co-authored with Emanuela Galasso, we evaluate an original large-scale intervention in Madagascar (SEECALINE) that focuses on promoting correct breast-feeding, complementary feeding and hygiene practices. We find that the program helped 0-5 year old children, in the participating communities to bridge their gap in weight-for-age z-score and the incidence of underweight. The program also had significant effects in protecting height-for-age and reducing the incidence of stunting. We also show that SEECALINE can have very different effects on the anthropometric status of children, depending on the educational level of the mother. We find that the program improved height-for-age only for children whose mothers had at least secondary level education. We propose an explanation based on interaction effects between proxies of birth conditions and maternal education. More educated mothers meet the necessary conditions that reinforce the behavioral change enabling program effects. We provide evidence that access to public health facilities during birth and early childhood is necessary for translating behavioral change into improvements in children‟s health status. Chapter 1 leaves the question of differential take-up by maternal education unanswered. The heterogeneous effects could be due to lack of adoption of practices by the least educated mothers. In the second chapter I apply difference-in-difference and propensity score weighting techniques to identify causal impact of the program availability on behavioral change and show that least educated mothers adopted the recommended practices. This complements the evidence presented in chapter 1 that although improved knowledge of child-care may be necessary it is not sufficient to translate into improvements in nutritional outcomes. In the third chapter co-authored with Emanuela Galasso and Jeffrey Yau, we estimate the returns to differential lengths of exposure to SEECALINE. We address this question using information available only on program participants. To that end, we develop a methodology that circumvents this data hurdle and estimate returns to differential lengths of exposure using administrative data. We find that the differential returns are decreasing over time, though they do not dissipate to zero. These results provide suggestive evidence that the returns to the program reflect learning effects from the intervention.

Abstract

Thermal energy storage using phase change material (PCM) capsules is a prevalent method used in high-temperature applications such as steam generation and concentrated solar power. However, the research works on the development of latent heat storage PCM encapsulations are limited. In the present study, experimental and numerical investigations conducted on high-temperature cylindrical encapsulations filled with sodium nitrate that were using air as the heat transfer fluid are presented. A modification of the cylindrical capsule known as the annular capsule is proposed to improve the heat transfer characteristics inside the PCM by providing a heat transfer surface at the center of the capsule. Experimental investigations were conducted utilizing a high-temperature (723 K) test facility to determine the discharging characteristics of both capsule designs in the temperature range between 603 and 473 K. A two-dimensional axisymmetric numerical model is developed and validated with the results obtained from the experiments. The performances of both the basic and annular capsules are compared, and it is observed that the discharging time reduced by 24% for the annular capsule. The heat transfer enhancement occurs due to an increase in the contact surface area and the improvement in flow characteristics of the air.

Abstract

Therapeutic plasma exchange (TPE) is an effective and affordable treatment option in most parts of Southeast Asia (SEA). In 2018, the SEA TPE Consortium (SEATPEC) was established, consisting of regional neurologists working to improve outcome of various autoimmune neurological diseases. We proposed an immunotherapeutic guideline prioritizing TPE for this region. We reviewed disease burden, evidence-based treatment options, and major guidelines for common autoimmune neurological disorders seen in SEA. A modified treatment algorithm based on consensus agreement by key-opinion leaders was proposed. Autoimmune antibody diagnostic testing through collaboration with accredited laboratories was established. Choice of first-line immunotherapies (IVIg/corticosteroid/TPE) is based on available evidence, clinicians’ experience, contraindications, local availability, and affordability. TPE could be chosen as first-line therapy for GBS, CIDP, MG (acute/short term), IgG, A paraproteinemic neuropathy, and NMDAR encephalitis. Treatment is stopped for acute monophasic conditions such as GBS and ADEM following satisfactory outcome. For chronic immune disorders, a therapy taper or long-term maintenance therapy is recommended depending on the defined clinical state. TPE as second-line treatment is indicated for IVIg or corticosteroids refractory cases of ADEM, NMOSD (acute), MG, and NMDAR/LGI1/CASPR2/Hashimoto’s encephalitis. With better diagnosis, treatment initiation with TPE is a sustainable and effective immunotherapy for autoimmune neurological diseases in SEA.

"Approximate computing" is a design technique that sacrifices computational correctness in order to obtain high performance and low power consumption. Approximate Booth multiplier structures are the focus of this investigation in this thesis. The approximate multiplier for errors resulting from the approximate radix-4 Booth encoding, the approximate regular partial product array, and the approximate 4-2 compressor is estimated using a probabilistic error model. The NMEDs of 8-bit and 16-bit approximation designs can be determined using the proposed methodology. Because it constantly increases the sensitivity to the statistical delay of a path, a test pattern has typically been considered a random variable with a Gaussian distribution. In various circuit configurations, the pattern-created delay is non-Gaussian, and its sensitivity varies between test patterns. Probability mass functions are used to express it (PMFs). By employing many uncorrelated test patterns per path, this article explains how autonomous test pattern generation (ATPG) can increase defect coverage (DC). The ATPG method's impact is evaluated by comparing it to previous methods. It has been shown that critical pathways can be identified for further exploration using the ATPG supplied. Abstract

Some degree of calculation inaccuracy is common in statistical applications, such as machine learning and data sensing. Approximate computations can be utilised to conserve energy and increase performance. The output error rate of some approximation solutions is uncontrollable, therefore they can only be employed in a limited number of situations. An approximation floating point multiplier called the CFPU is the primary purpose of this research. Our approach approximates multiplication by omitting the operation's most energy-intensive step and replacing it with a less energy-intensive one. CFPU dynamically picks the inputs that create the biggest approximation error and processes them in exact CFPU mode in order to fine-tune the amount of approximation. When completing at least 4% of multiplications in approximate mode, our CFPU outperforms a typical FPU. These multiplications save a large amount of energy in our evaluated applications. We found that by replacing the CFPU with FPUs on the AMD Southern Island GPU, we were able to reduce power consumption by 77% and improve latency by 3.5 percent for eight common Open CL applications. Abstract

An X-DADDA multiplier that is both energy-efficient and accurate is the subject of this article. Voltage scaling and approximate width setting approximation controls boost the multiplier's energy usage and dependability while extending its lifespan. The latter can only be utilised before and after design, unlike the former, which can be modified at any time. A certain degree of precision is required in order to calculate the partial product columns and the over scaled voltage in order to maximise energy. Columns with lower bit significances and more switching activity are commonly used to keep the inaccuracy within a reasonable range. Level shifters are used rarely in this design due of their low cost. Every column following the first one is connected to each other. If you want more efficiency from your multiplier, you should reduce the output from it to only four bits. 15-nm FinFET technology is used to investigate the X-Dadda structure's efficiency. Approximation based on the typical relative error distance of 0.11 can save up to 43% of the power consumed in a home. Bias temperature instability (BTI) is reduced by up to 9.9 percent in this case compared to the precise mode when the delay degradation is 50%. The X-accuracy Dadda's also investigate the influence of process adjustments. Finally, the X-Dadda multiplier is put to the test in neural networks for image categorization and processing. Abstract

Reports of cerebral venous thrombosis (CVT) after messenger RNA (mRNA)-based SARS-CoV-2 vaccination has caused safety concerns, but CVT is also known to occur after SARS-CoV-2 infection. Comparing the relative incidence of CVT after infection vs vaccination may provide a better perspective of this complication.To compare the incidence rates and clinical characteristics of CVT following either SARS-CoV-2 infection or mRNA-based SARS-CoV-2 vaccines.Between January 23, 2020, and August 3, 2021, this observational cohort study was conducted at all public acute hospitals in Singapore, where patients hospitalized with CVT within 6 weeks of SARS-CoV-2 infection or after mRNA-based SARS-CoV-2 vaccination (BNT162b2 [Pfizer-BioNTech] or mRNA-1273 [Moderna]) were identified. Diagnosis of SARS-CoV-2 infection was based on quantitative reverse transcription-polymerase chain reaction or positive serology. National SARS-CoV-2 infection data were obtained from the National Centre for Infectious Disease, Singapore, and vaccination data were obtained from the National Immunisation Registry, Singapore.SARS-CoV-2 infection or mRNA-based SARS-CoV-2 vaccines.Clinical characteristics, crude incidence rate (IR), and incidence rate ratio (IRR) of CVT after SARS-CoV-2 infection and after mRNA SARS-CoV-2 vaccination.Among 62 447 individuals diagnosed with SARS-CoV-2 infections included in this study, 58 989 (94.5%) were male; the median (range) age was 34 (0-102) years; 6 CVT cases were identified (all were male; median [range] age was 33.5 [27-40] years). Among 3 006 662 individuals who received at least 1 dose of mRNA-based SARS-CoV-2 vaccine, 1 626 623 (54.1%) were male; the median (range) age was 50 (12-121) years; 9 CVT cases were identified (7 male individuals [77.8%]; median [range] age: 60 [46-76] years). The crude IR of CVT after SARS-CoV-2 infections was 83.3 per 100 000 person-years (95% CI, 30.6-181.2 per 100 000 person-years) and 2.59 per 100 000 person-years (95% CI, 1.19-4.92 per 100 000 person-years) after mRNA-based SARS-CoV-2 vaccination. Six (66.7%) received BNT162b2 (Pfizer-BioNTech) vaccine and 3 (33.3%) received mRNA-1273 (Moderna) vaccine. The crude IRR of CVT hospitalizations with SARS-CoV-2 infection compared with those who received mRNA SARS-CoV-2 vaccination was 32.1 (95% CI, 9.40-101; P .001).The incidence rate of CVT after SARS-CoV-2 infection was significantly higher compared with after mRNA-based SARS-CoV-2 vaccination. CVT remained rare after mRNA-based SARS-CoV-2 vaccines, reinforcing its safety.

Abstract

In December 2017, 79 delegates attended the 2nd regional plasmapheresis conference and workshop for Southeast Asia (SEA) on the immunomodulatory role of plasma exchange in central and peripheral nervous system disorders in Kuala Lumpur, Malaysia. This meeting featured 6 plenary lectures, interactive sessions dedicated for experience sharing, case presentations, and a practical session for paramedics. Clinical experts and researchers from 7 SEA countries and India shared experience and challenges in treating autoimmune neurological disorders. While the spectrum of diseases and neurology practice remained largely similar, there was great disparities in accessibility of therapeutic plasma exchange (TPE) within SEA countries and between urban or rural settings. Costs, human resources, and healthcare policies are common challenges in providing sustainable TPE services. Novel techniques and innovative ideas in performing TPE were explored. A working consortium comprising of key opinion leaders was proposed to improve standards of TPE and enhance future research.

Abstract

Treatment-induced neuropathy of diabetes (TIND) is an acute painful autonomic small-fiber neuropathy that develops following an abrupt improvement in glycaemia control. Recent reports suggest TIND is a significant problem in tertiary neuropathy clinics. TIND in hospitalized patients with poor initial glycaemia control, that we refer to as nosocomial TIND, has not been well-studied. We describe the demographic, clinical features and indices of glycaemia control in 5 consecutive nosocomial TIND patients. TIND was defined using recently published criteria. Pre-meal capillary blood glucose recordings performed during the period of HbA1c decline was used to calculate glycaemic variability. All the nosocomial TIND patients were hospitalized for prolonged periods for serious medical conditions that warranted good glycaemia control, namely severe sepsis, diabetic ketoacidosis, stroke, heart failure and traumatic head injury. They had raised, double-digit, HbA1c levels at admission that subsequently dropped precipitously with tight in-patient glycaemia control protocols. These patients had multiple, largely asymptomatic, hypoglycaemic episodes. Glycaemic variability also appeared to be high in this cohort. TIND may be a significant cause of morbidity in hospitalized diabetic patients with poor glycaemia control. Not all patients developed both autonomic and painful neuropathies, raising the possibility of forme-fruste TIND. Published version

Abstract

An innovative heterocyclic biologically active chalcone 1,2,3-triazole analogs (6a–j) were prepared to extract excellent yields by coupling the substituted aryl azides (5a–5j) and 5-ethynyl-1,2,3-trimethoxybenzene, by using the method of Huisgen azide–alkyne cycloaddition. The typically synthesized analogs were elucidated by IR, 1H-nuclear magnetic resonance (NMR), 13C-NMR, and Electron spray ionization (ESI)-mass spectroscopy and tested for their cytotoxicity effectiveness in MTT assays against the A549 lung cancer cells. The cytotoxic studies suggested that a few analogs showed moderate to good activities. The compounds 6i and 6c showed low cytotoxicity against the A549 cell line among 12 analogs, the values of IC50 were displayed in the range of 65.05 ± 1.12 and 71.56 ± 1.29 µM, respectively. The compound 6j showed slightly less cytotoxicity but showed good selectivity against A549 cell lines.

48. Improving nutritional status through behavioral change: lessons from Madagascar

Abstract

This paper provides evidence of the effects of a large-scale intervention that focuses on the quality of nutritional and child care inputs during the early stages of life. The empirical strategy uses a combination of double-difference and weighting estimators in a longitudinal survey to address the purposive placement of participating communities and estimate the effect of the availability of the program at the community level on nutritional outcomes. The authors find that the program helped 0-5 year old children in the participating communities to bridge the gap in weight for age z-scores and the incidence of underweight. The program also had significant effects in protecting long-term nutritional outcomes (height for age z-scores and incidence of stunting) against an underlying negative trend in the absence of the program. Importantly, the effect of the program exhibits substantial heterogeneity: gains in nutritional outcomes are larger for more educated mothers and for villages with better infrastructure. The program enables the analysis to isolate responsiveness to information provision and disentangle the effect of knowledge in the education effect on nutritional outcomes. The results are suggestive of important complementarities among child care, maternal education, and community infrastructure.
