Consumers often know nothing about the person(s) who made the products they consume. This manuscript shows that firms can benefit from changing this status quo. We demonstrate that “personizing” the producer, that is, exposing consumers to personal information about a given product's producer, can significantly increase product preference and willingness to pay. We find this effect even if the related information is not tied to the producer's production competencies which is surprising from an economic perspective because the added information is non-diagnostic. We propose that this effect unfolds because the personizing treatment makes consumers feel more socially connected to the producer, which the consumer rewards through increased product demand. A series of studies documents the existence of the personizing effect, provides process evidence for the effect by mediation and moderation, and rules out several alternative explanations. More broadly, this research advances our understanding of why and when making producers personal can be beneficial in commercial transactions.

Zusammenfassung Metaphern können helfen, eine Sprache für das Neue zu finden. In neutestamentlichen Texten wird die Erfahrung des Christusgläubig-Werdens häufig metaphorisch beschrieben, zum Beispiel anhand geburts- und zeugungsmetaphorischer Aussagen. Als »Wiedergeburt« ist diese Metapher zur Katachrese geworden, weist im gegenwärtigen Gebrauch aber auch Bedeutungsverschiebungen auf und stellt keine geeignete Kategorie dar, um die ursprünglich lebendige Metaphorik der neutestamentlichen Texte zu erfassen.

Zusammenfassung Hintergrund und Zielsetzung Eine frühe Versorgung von Patient:innen, die Schmerzen und Risikofaktoren einer Chronifizierung aufweisen, ist sinnvoll; diese Patient:innen können von einer frühen interdisziplinären multimodalen Schmerztherapie (IMST) profitieren. Angesichts der unzureichenden Versorgung wurden im Rahmen von PAIN2020 zwei ambulante Therapiemodule für die Sekundärprävention einer Schmerzchronifizierung entwickelt werden: die edukative bzw. begleitende IMST (E‑IMST/B‑IMST). Material und Methoden Der Entwicklungsprozess der beiden IMST wird vorgestellt. Es wurden zwei Zielgruppen von Patient:innen definiert, für die in Abhängigkeit von Chronifizierung, Beeinträchtigung und Komplexität der Störung eine Sitzung (E‑IMST) bzw. 10 Interventionstermine (B‑IMST) vorgesehen waren. Die Konzeption erfolgte in 5 Schritten: Erarbeitung der Zielstellungen; Erarbeitung der Hauptinhalte; Workshop zur inhaltlichen und konzeptionellen Ausgestaltung (Inhalte, Vermittlung, Übungen); Erstellung eines Ablaufplans; Aufbereitung der Ergebnisse (Manual, Präsentationen, Arbeitsblätter, Handbuch). Zunächst wurde die B‑IMST entwickelt, woraus Inhalte für die E‑IMST extrahiert wurden. Daneben sollte ein Konzept zur Überprüfung der Umsetzbarkeit und ein Wirkmodell für eine Pilotierung entwickelt werden. Ergebnisse Zielstellungen für beide IMST-Formen sind die Verbesserung des Verständnisses von Schmerz und mitbedingenden Faktoren, die Erhöhung des Kontroll- und Selbstwirksamkeitserlebens und die Erhöhung der Eigenverantwortung hinsichtlich schmerzreduzierender Strategien. Unterschiede zwischen den beiden Therapiemodulen ergeben sich aus den Bedarfen und Rahmenbedingungen. Für beide IMST-Module wurden ärztliche, physio- und psychotherapeutische Inhalte und Abläufe ausgestaltet. Die B‑IMST setzt sich aus 5 Modulen mit je 2 Terminen als Gruppenintervention zusammen (biopsychosoziales Modell; Aktivierungsplanung; Bedürfnisregulation; Schlaf und Medikamente; Alltagstransfer). Die 3‑stündige E‑IMST-Gruppenintervention vermittelt in erster Linie Wissen über Schmerz und das biopsychosoziale Schmerzmodell. Es kommen theoretische und praktische Interventionen bzw. erfahrungs- und erlebnisorientierte Methoden zur Anwendung. Schlussfolgerung Es existieren nun zwei interdisziplinär ausgerichtete Manuale für die sekundärpräventive Behandlung von Patient:innen mit wiederkehrenden Schmerzen und Risikoprofil für eine Chronifizierung. Diese Ansätze müssen nun hinsichtlich Machbarkeit und Wirksamkeit überprüft werden.

This post-hoc analysis of the AIRFLOW-2 trial investigated the changes in airway CT-parameters after targeted lung denervation (TLD) and whether these changes are associated with treatment response. In the treatment group (n = 32), an improvement in air trapping was significantly associated with an improvement in residual volume (RV). Furthermore, improvements in Pi10 and airway lumen were significantly associated with an improvement in both RV and FEV1. Our results could suggest that when improving airway characteristics like decreasing airway wall thickness and increasing the airway lumen, this leads to less air trapping and an improvement in clinical outcomes.

Although psychosocial factors have a profound impact on the experience of pain and pain recovery, the transfer to clinical application has so far been insufficient. With this article, a task force of the special interest group "Psychosocial Aspects of Pain" of the German Pain Society (Deutsche Schmerzgesellschaft e. V.) would like to draw attention to the considerable discrepancy between existing scientific evidence on the importance of psychosocial factors in the development of chronic pain disorders and the translation of these findings into the care of pain patients. Our objective is a stronger integration of psychological and psychosomatic expertise in pain treatment and research, as well as the improvement of structural and institutional conditions, to achieve an increased consideration of psychosocial aspects. In this way, modern, integrative and complex pain concepts can reach the patient. Based on these fundamental findings on the importance of psychosocial factors in pain and pain treatment, implications for the transfer to clinic and further research will be shown.Psychosoziale Faktoren beeinflussen Schmerzerleben und Schmerzgenesung weitreichend, trotzdem ist der Transfer in die klinische Anwendung bisher unzureichend. Mit diesem Beitrag möchte eine Arbeitsgruppe des Arbeitskreises „Psychosoziale Aspekte bei Schmerz“ der Deutschen Schmerzgesellschaft e. V. auf die erhebliche Diskrepanz zwischen bestehender wissenschaftlicher Evidenz zur Bedeutung psychosozialer Faktoren bei der Entstehung chronischer Schmerzstörungen und der Translation dieser Ergebnisse in die Versorgung von Schmerzpatienten aufmerksam machen. Unsere Ziele sind eine stärkere Integration psychologischer und psychosomatischer Expertise in die Schmerzbehandlung und -forschung sowie die Verbesserung der strukturellen und institutionellen Voraussetzungen, um zu einer vermehrten Berücksichtigung psychosozialer Aspekte zu kommen. Nur so können die modernen, integrativen und komplexen Schmerzkonzepte beim Patienten ankommen. Basierend auf diesen grundlegenden Erkenntnissen zur Bedeutung psychosozialer Faktoren bei Schmerz und Schmerzbehandlung sollen Implikationen für den Transfer in die Klinik und die weitere Forschung aufgezeigt werden.

In allen theologischen Studiengängen ist die Fähigkeit zur methodengeleiteten Auslegung biblischer Texte eine zu erlangende Kernkompetenz. Diese Einführung verbindet die traditionellen Schritte der historisch-kritischen Exegese des Neuen Testaments mit neueren Ansätzen aus der Linguistik und Literaturwissenschaft. Das Buch motiviert die Lesenden durch eine direkte Ansprache, nimmt viele Fragen aus der Lehrpraxis auf und stellt die praktische Umsetzung in den Vordergrund. Viele Beispiele der neutestamentlichen Exegese illustrieren das methodische Vorgehen und den damit verbundenen Erkenntnisgewinn. Insbesondere die Voraussetzungen von Studierenden ohne Kenntnisse des Altgriechischen werden durchgehend beachtet. Eingehend behandelt wird auch der zielführende Umgang mit digital verfügbaren Ressourcen, wie Online-Lexika, Textdatenbanken, digitalisierten Handschriften, Bibelsoftware etc.

Patient*innen vor einer Chronifizierung der Schmerzen bewahren: Hier führt das Projekt PAIN2.0 weiter, was als vom Innovationsfonds gefördertes Projekt PAIN2020 begonnen hatte. Erprobt wird nun eine innovative Versorgungsform, bei der vor allem interdisziplinäre und multimodale Inhalte im Vordergrund stehen: die ambulante interdisziplinäre multimodale Schmerztherapie (A-IMST) in der Gruppe.

The management of acute postoperative pain remains suboptimal. Systematic reviews and Cochrane analysis can assist with collating evidence about treatment efficacy, but the results are limited in part by heterogeneity of endpoints in clinical trials. In addition, the chosen endpoints may not be entirely clinically relevant. To investigate the endpoints assessed in perioperative pain trials, we performed a systematic literature review on outcome domains assessing effectiveness of acute pain interventions in trials after total knee arthroplasty. We followed the Cochrane recommendations for systematic reviews, searching PubMed, Cochrane, and Embase, resulting in the screening of 1590 potentially eligible studies. After final inclusion of 295 studies, we identified 11 outcome domains and 45 subdomains/descriptors with the domain "pain"/"pain intensity" most commonly assessed (98.3%), followed by "analgesic consumption" (88.8%) and "side effects" (75.3%). By contrast, "physical function" (53.5%), "satisfaction" (28.8%), and "psychological function" (11.9%) were given much less consideration. The combinations of outcome domains were inhomogeneous throughout the studies, regardless of the type of pain management investigated. In conclusion, we found that there was high variability in outcome domains and inhomogeneous combinations, as well as inconsistent subdomain descriptions and utilization in trials comparing for effectiveness of pain interventions after total knee arthroplasty. This points towards the need for harmonizing outcome domains, eg, by consenting on a core outcome set of domains which are relevant for both stakeholders and patients. Such a core outcome set should include at least 3 domains from 3 different health core areas such as pain intensity, physical function, and one psychological domain.

ZusammenfassungDie Versorgung von Patienten mit Schmerzen und Chronifizierungsrisiko ist nach wie vor gekennzeichnet durch Über‑, Fehl- und Unterversorgung. Das Projekt PAIN2020 (Innovationsfonds 01NVF17049) hat zum Ziel, durch die Einführung eines frühzeitigen, auf Schmerz spezialisierten interdisziplinären diagnostischen Ansatzes die ambulante Versorgung von Patienten im Hinblick auf Schmerzen und das Funktionsniveau zu verbessern. Im Rahmen einer randomisierten kontrollierten Studie werden bundesweit in 31 Einrichtungen der Regelversorgung (schmerzspezialisiertes Angebot) Patienten mit Risikofaktoren bei bestehenden Schmerzen einer frühen Schmerzdiagnostik zugeführt. Die Interventionsbedingung besteht dabei in einem interdisziplinären multimodalen Assessment mit den beteiligten Disziplinen Schmerzmedizin, Physiotherapie und Psychologie. Die Kontrollbedingung umfasst einen einmaligen Termin bei einem Schmerztherapeuten der Qualitätssicherungsvereinbarung Schmerzmedizin bzw. mit Zusatzbezeichnung spezielle Schmerztherapie. Patienten und Vorbehandler erhalten entsprechend der Befunde detaillierte Empfehlungen für eine weitere bedarfsgerechte Behandlung. Es sind 2 Evaluationsansätze geplant. Für den ersten beträgt die zu erreichende Nettofallzahl 3840 Patienten, deren klinische Daten (Deutscher Schmerzfragebogen, zusätzliche Skalen) längsschnittlich erhoben (Einschluss, 3 und 6 Monate nach Diagnostik) und auf Grundlage eines Mehr-Ebenen-Modells ausgewertet werden. Im Rahmen eines 2. Ansatzes werden diese klinischen Daten einerseits um Sekundärdaten der BARMER ergänzt sowie die Patienten des ersten Evaluationsansatzes mit BARMER-Versicherten gematcht, die an dem Projekt nicht teilgenommen haben. Die Auswertung übernimmt ein externes Evaluationsinstitut. Das Projekt startete im April 2018.

ZusammenfassungSchmerz ist ein multikausales biopsychosoziales Phänomen. Das bedeutet für eine angemessene Versorgung ein entsprechendes interdisziplinäres Vorgehen in Diagnostik und Therapie. Faktoren, die bei bestehenden Schmerzen zu einer Chronifizierung beitragen können, sollten frühzeitig im Versorgungsalltag berücksichtigt werden. Dafür sind sektorenübergreifende Vernetzungen sowie interdisziplinäre Versorgungsangebote von großer Bedeutung.

Postoperative pain management is still insufficient, leading to major deficits, including patient suffering, impaired surgical recovery, long-term opioid intake, and postsurgical chronic pain. Yet, identifying the best treatment options refers to a heterogeneous outcome assessment in clinical trials, not always reflecting relevant pain-related aspects after surgery and therefore hamper evidence synthesis. Establishing a core outcome set for perioperative pain management of acute pain after surgery may overcome such limitations. An international, stepwise consensus process on outcome domains ("what to measure") for pain management after surgery, eg, after total knee arthroplasty, sternotomy, breast surgery, and surgery related to endometriosis, was performed. The process, guided by a steering committee, involved 9 international stakeholder groups and patient representatives. The face-to-face meeting was prepared by systematic literature searches identifying common outcome domains for each of the 4 surgical procedures and included breakout group sessions, world-cafe formats, plenary panel discussions, and final voting. The panel finally suggested an overall core outcome set for perioperative pain management with 5 core outcome domains: physical function (for a condition-specific measurement), pain intensity at rest, pain intensity during activity, adverse events, and self-efficacy. Innovative aspects of this work were inclusion of the psychological domain self-efficacy, as well as the specific assessment of pain intensity during activity and physical function recommended to be assessed in a condition-specific manner. The IMI-PROMPT core outcome set seeks to improve assessing efficacy and effectiveness of perioperative pain management in any clinical and observational studies as well as in clinical practice.

BACKGROUND AND OBJECTIVES Although Interdisciplinary Multimodal Pain Treatment (IMPT) programmes share a biopsychosocial approach to increase the wellbeing of patients with chronic pain, substantial variation in content and duration have been reported. In addition, it is unclear to what extent any favourable health outcomes are maintained over time. Therefore, our first aim was to identify and analyse the change over time of patient-related outcome measures in cohorts of patients who participated in IMPT programmes. Our second aim was to acquire insight into the heterogeneity of IMPT programmes. DATABASES AND DATA TREATMENT The study protocol was registered in Prospero under CRD42018076093. We searched Medline, Embase, PsycInfo and Cinahl from inception to May 2020. All study selection, data extraction and risk of bias assessments were independently performed by two researchers. Study cohorts were eligible if they included adult patients with chronic primary musculoskeletal pain for at least 3 months. We assessed the change over time, by calculating pre-post, post-follow-up and pre-follow-up contrasts for seven different patient-reported outcome domains. To explore the variability between the IMPT programmes, we summarized the patient characteristics and treatment programmes using the intervention description and replication checklist. RESULTS The majority of the 72 included patient cohorts significantly improved during treatment. Importantly, this improvement was generally maintained at follow-up. In line with our expectations and with previous studies, we observed substantial methodological and statistical heterogeneity. CONCLUSIONS This study shows that participation in an IMPT programme is associated with considerable improvements in wellbeing that are generally maintained at follow-up. The current study also found substantial heterogeneity in dose and treatment content, which suggests different viewpoints on how to optimally design an IMPT programme. SIGNIFICANCE The current study provides insight into the different existing approaches regarding the dose and content of IMPT programs. This analysis contributes to an increased understanding of the various approaches by which a biopsychosocial perspective on chronic pain can be translated to treatment programs. Furthermore, despite theoretical and empirical assertions regarding the difficulty to maintain newly learned health behaviors over time, the longitudinal analysis of health outcomes did not find a relapse pattern for patients who participated in IMPT programs.

Based on the fundamental concept of the biopsychosocial model, interdisciplinary multimodal pain therapy (IMPT) has developed to one of the most important components in the treatment of patients suffering from chronic pain. The process criteria for IMPT in Germany are described in the German OPS catalogue and IMPT is mainly offered as an inpatient treatment only. This article updates some of the fundamental criteria for IMPT for adult inpatient treatment and the task force defines basic structural and process criteria for the implementation of IMPT for outpatients.

Health care provision for patients with pain and risk factors for chronicity is still insufficient and characterized in particular by over-, under- and misuse of existing approaches. The PAIN2020 project is funded by the German Federal Ministry of Health Care (Innovationsfonds 01NVF17049) and aims to improve health care delivery for this group of patients by implementing an early ambulatory diagnostic approach by a pain specialist, resulting in an improvement in pain and restored or maintained function. A randomized clinical trial in 31 facilities Germany-wide will identify eligible patients and guide them into early specialized pain diagnostics. The interventional arm provides an interdisciplinary multimodal assessment, delivered by pain medicine specialists, physiotherapists and clinical psychologists. The control arm contains a single assessment by a pain specialist. Patients and caregivers receive detailed recommendations for evidence-based interventions tailored to the needs of the individual patient. Two evaluation approaches will be merged. The first requires a net sample size of 3840 patients, assessed (admission) and followed-up (3 and 6 months) by clinical data (German Pain Questionnaire, additional scales) and analyzed by a multi-level approach. In a second evaluation arm the clinical data of the included patients will be supplemented by secondary data from a statutory health insurance (BARMER) and compared to a sample of policyholders not addressed by the study. Data analyses will be performed by an external evaluation institute. Pain is agreed to be understood as a multi-causal, biopsychosocial phenomenon. A sufficient health care delivery shall therefore contain a corresponding interdisciplinary approach in diagnostic and therapy, respectively. Factors contributing to the process of chronification should be considered early in treating patients suffering from recurrent or persistent pain. Close collaboration of multidisciplinary protagonists in health care as well as interdisciplinary comprehensive medical treatment offer are two of the prerequisites for good practice in health care treating patients with pain.The article introduces into existing knowlegde about pain and risk factors for chronicity. The background (evidence and theoretical) of corresponding health care approaches and a detailed concept of interdisciplinary pain diagnostic by a multiprofessional team consisting of pain physician, physiotherapist and clinical psychologist are described and discussed.

The current healthcare provision in Germany is established, in particular, for the diagnostics and treatment of chronic pain conditions; however, the current aim is to initiate the diagnostic and therapeutic approaches oriented towards the biopsychosocial pain model in the early stages of pain, i.e. before the onset of chronification, for patients with pain and a risk of chronification in order to actively avoid chronification processes. In this context, multiple risk factors play an important role for the diagnostic and therapeutic approaches as well as for the interdisciplinary multimodal pain therapy developed for this purpose. The Global Year of the International Association for the Study of Pain (IASP) 2020 addressed the prevention of (chronic) pain, a welcome opportunity to provide a short review of the evidence for and clinical experiences with timely diagnostic and therapeutic options and to summarize the current framework conditions and scientific recommendations for Germany. At the end of this article the implications for future research are summarized, particularly for the treatment of patients with pain and risk of chronification.Bestehende Versorgungsangebote in Deutschland sind vor allem für die Diagnostik und Therapie chronischer Schmerzen etabliert. Aktuell wird jedoch versucht, am biopsychosozialen Schmerzmodell ausgerichtete diagnostische und therapeutische Ansätze frühzeitiger, also noch vor Beginn der Chronifizierung, für Patienten mit Schmerzen und Chronifizierungsrisiko einzurichten, um Chronifizierungsverläufe aktiv zu vermeiden. Dabei spielen multiple Risikofaktoren für diagnostische und therapeutische Ansätze sowie für die dazu entwickelte interdisziplinäre multimodale Schmerztherapie eine große Rolle. Das Global Year der International Association for the Study of Pain (IASP) adressierte 2020 die Prävention (chronischer) Schmerzen – ein willkommener Anlass, einen kurzen Überblick über Evidenz für und klinische Erfahrungen mit frühzeitigen diagnostischen und therapeutischen Angeboten zu geben sowie die derzeitigen Rahmenbedingungen und wissenschaftlichen Empfehlungen für Deutschland zusammenzustellen. Am Ende des Beitrags werden Implikationen für weitere Forschungen zusammengefasst, insbesondere für die Versorgung von Patienten mit Schmerz und Chronifizierungsrisiko.

With the implementation of the German diagnosis-related groups (DRG) reimbursement system in hospitals, interdisciplinary multimodal pain therapy was incorporated into the associated catalogue of procedures (OPS 8‑918). Yet, the presented criteria describing the procedure of interdisciplinary multimodal pain therapy are neither precise nor unambiguous. This has led to discrepancies in the interpretation regarding the handling of the procedure-making it difficult for medical services of health insurance companies to evaluate the accordance between the delivered therapy and the required criteria. Since the number of pain units has increased in recent years, the number of examinations by the medical service of health insurance companies has increased. This article, published by the ad hoc commission for interdisciplinary multimodal pain therapy of the German Pain Association, provides specific recommendations for correct implementation of interdisciplinary multimodal pain therapy in routine care. The aim is to achieve a maximum level of accordance between health care providers and the requirements of the medical examiners from health insurance companies. More extensive criteria regarding interdisciplinary multimodal pain treatment in an in-patient setting, especially for patients with chronic and complex pain, are obviously needed. Thus, the authors further discuss specific aspects towards further development of the OPS-code. However, the application of the OPS-code still leaves room regarding treatment intensity and process quality. Therefore, the delivery of pain management in sufficient quantity and quality still remains the responsibility of each health care provider.

In 2009 the diagnosis chronic pain disorder with somatic and psychological factors (F45.41) was integrated into the German version of the International Classification of Diseases, version 10 (ICD-10-GM). In 2010 Paul Nilges and Winfried Rief published operationalization criteria for this diagnosis. In the present publication the ad hoc commission on multimodal interdisciplinary pain therapy of the German Pain Society now presents a formula for a clear validation of these operationalization criteria of the ICD code F45.41.

Das Konstrukt der Schmerzstarke wird haufig in Effektivitatsstudien der interdisziplinaren multimodalen Schmerztherapie (IMST) als Ergebnisparameter erhoben. Fraglich ist jedoch zum einen, ob dieses Kriterium, gerade in der IMST, welche als Ziel eher den Umgang mit chronischen Schmerzen als die Reduzierung dieser hat, ein geeignetes Bewertungskriterium darstellt. Zum anderen wird die Frage, inwieweit dieses Konstrukt mit den gangigen Verfahren uberhaupt reliabel und valide gemessen werden kann, kritisch diskutiert. Um die Relevanz der Schmerzstarke als Kriterium des Therapieerfolgs sowie die Interpretierbarkeit und Handhabbarkeit gangiger Erhebungsverfahren zu bewerten, ist eine Partizipation der Patienten unabdingbar. In einer qualitativen, multizentrischen Studie wurden im Rahmen von Fokusgruppen festgelegte Fragestellungen zum Konstrukt sowie der Erhebung der Schmerzstarke mit IMST-Patienten (N = 69; 18–77 Jahre; 80 % weiblich) in vier deutschen Einrichtungen diskutiert. Es zeigte sich, dass das Konstrukt der Schmerzstarke von den Patienten als relevantes, aber nicht primares Therapieoutcome gesehen wird. Hinsichtlich der Messung der Schmerzstarke merkten die Patienten deutliche Probleme an, beispielsweise bei der Erhebung von Attackenschmerz oder auch bei der Abgrenzung der korperlichen Schmerzkomponente von den affektiven und sozialen Aspekten des Schmerzerlebens. Die Auserungen der Patienten legen nahe, dass eine Messung der Schmerzstarke aufgrund der hohen Subjektivitat, der multidimensionalen Natur des Schmerzerlebens, aber auch wegen (sprachlicher) Ungenauigkeiten und fehlender Standardisierung der Messinstrumente problematisch ist. Dies sollte in zukunftigen Messungen der Schmerzstarke Berucksichtigung finden.

This research demonstrates that the self-expressive customization of a product can improve performance on tasks performed using the customized product. Five studies show that the effect is robust across different types of tasks (e.g., persistence tasks, concentration tasks, agility tasks). The evidence further shows that the effect is not due to changes in product efficacy beliefs, feelings of competence, feelings of accomplishment, mood, task desirability, goal activation, or goal attainability. Instead, the self-expressive customization of a product extends an identity (e.g., personal identity, group identity) into the product. When the product is subsequently used to pursue a goal whose desired outcome can affirm the extended identity, performance improves.

In den letzten Jahrzehnten haben sich innerhalb des ­konzeptionellen Rahmens der Verhaltenstherapie verschiedene Ansatze fur die ­Behandlung ­chronischer Schmerzen entwickelt, die sich in Theorie und Behandlungspraxis deutlich unterscheiden. ­Psychotherapeutische Interventionen bei diesen Erkrankungen sind idealerweise in ein ­interdisziplinares und multimodales Vorgehen eingebettet und ­basieren auf einer bio-psycho-sozialen Sichtweise.
