Abstract

Sağır, Özlem (Balikesir Author), Introduction More than 300 million surgical procedures are performed each year. Acute kidney injury (AKI) is a common complication after major surgery and is associated with adverse short-term and long-term outcomes. However, there is a large variation in the incidence of reported AKI rates. The establishment of an accurate epidemiology of surgery-associated AKI is important for healthcare policy, quality initiatives, clinical trials, as well as for improving guidelines. The objective of the Epidemiology of Surgery-associated Acute Kidney Injury (EPIS-AKI) trial is to prospectively evaluate the epidemiology of AKI after major surgery using the latest Kidney Disease: Improving Global Outcomes (KDIGO) consensus definition of AKI. Methods and analysis EPIS-AKI is an international prospective, observational, multicentre cohort study including 10 000 patients undergoing major surgery who are subsequently admitted to the ICU or a similar high dependency unit. The primary endpoint is the incidence of AKI within 72 hours after surgery according to the KDIGO criteria. Secondary endpoints include use of renal replacement therapy (RRT), mortality during ICU and hospital stay, length of ICU and hospital stay and major adverse kidney events (combined endpoint consisting of persistent renal dysfunction, RRT and mortality) at day 90. Further, we will evaluate preoperative and intraoperative risk factors affecting the incidence of postoperative AKI. In an add-on analysis, we will assess urinary biomarkers for early detection of AKI. Ethics and dissemination EPIS-AKI has been approved by the leading Ethics Committee of the Medical Council North Rhine-Westphalia, of the Westphalian Wilhelms-University Münster and the corresponding Ethics Committee at each participating site. Results will be disseminated widely and published in peer-reviewed journals, presented at conferences and used to design further AKI-related trials. Trial registration number NCT04165369. ©

4. Guillain-Barré Syndrome After COVID-19 Infection.

Abstract

Introduction: Guillain-Barré syndrome (GBS) is an immune-mediated polyradiculoneuropathy characterized by ascending paralysis, loss of DTR, sensory, motor and autonomic symptoms. COVID-19 can trigger GBS, although the mechanism is not clear. Our aim is to discuss a patient with new GBS in the post-COVID period. Case: A 59 y/M was admitted to the respiratory ward with dyspnea and difficulty in walking. Medical history revealed no chronic diseases, medications or drugs/smoking except of COVID-19, 4 months prior admission. He was on Nintedanib for persistant cough, and chest images (Figure 1) revealed post-COVID pulmonary fibrosis. The patient was admitted to the ICU 10 days later due to progressive motor weakness in his lower extremities. GBS was diagnosed with EMG, which revealed acute axonal degeneration. At admission GCS was 15, SpO2 : 96% (with 4 lt/min O2 via nasal cannula), HR: 107 beats/min, BP: 107/76 mmHg and muscle strength was 4/5 in lower, and 5/5 in upper extremities. DTR were hypoactive. SARS-COV-2 PCR was negative and CSF sample showed elevated protein levels. He received IVIG 175 grams/day for 5 days. Muscle strength in his lower extremities improved and was 5/5 on day 5, but respiratory symptoms progressively worsened. NIV was started on day 4 as his SpO2 was <91%. On day 7 hypoxemia worsened (pO2 : 52 mmHg), and chest CT revealed a progression in ground glass density (Figure 2). The patient was intubated and conservative MV was started. The patient further detoriated and died on the same day due to respiratory failure. Discussion: GBS has been reported simultaneously during and after COVID-19 although the mechanism is not clear. Neurotoxicity may develop by targeting of the nerves directly by the virus or the antibodies; formed by the activated immune system. Abstract

Amaç: Çalışmamızda; genel anestezi uygulanan hastalarda anestezi indüksiyonunda uygulanan lidokainin etkisini ve entübasyona bağlı gelişen hemodinamik yanıt üzerine etkilerini ve bu etkilerin PI ile kıyasının retrospektif olarak değerlendirilmesi amaçlanmıştır.Yöntem: Etik kurul onayı alındıktan sonra retrospektif olarak ASA I-II, 18-65 yaş arası elektif cerrahi geçiren, laringoskopi yapılarak endotrakeal entübasyon ile opere olan 108 hasta çalışmaya alındı. İndüksiyon esnasında 1,5 mg/kg dozunda lidokain uygulanmış olan hastalar Grup L, uygulanmamış hastalar Grup P olarak adlandırıldı. Hastaların yaş, cinsiyet, boy, kilo, ASA skoru, Mallampati skoru, lidokain kullanımı gibi bilgileri kaydedildi. Kalp atım hızı (KAH), sistolik arteryel basıncı (SAB), diyastolik arteryel basıncı (DAB), ortalama arteryel basıncı (OAB), Massimo® pulse oksimetri monitorizasyonundan elde edilen PI değerleri operasyona girmeden önce (bazal), TOF=0 anında, entübasyon anında, 1. dk, 2.dk, 3. dk. 4. dk, 5. dk anlarında kaydedildi. Bulgular: Yaş, cinsiyet, vücut kitle indeksi (VKİ), ASA skoru, Mallampati sınıflandırması açısından gruplar istatistiksel olarak benzerdi (p>0,05). Hemodinamik parametrelerin hepsi (KAH, SAB, DAB, OAB) bazal değerlerde ve TOF=0 anında, hem Grup L'de hem de Grup P'de istatistiksel olarak birbiriyle benzer seyretti. Bu parametreler entübasyon, 1. dk, 2.dk, 3. dk, 4. dk, 5. dk anlarında Grup L'de, Grup P'ye göre anlamlı olarak düşük bulundu (p 0.05). Abstract

Amaç: Diz artroplastisi olan hastaların anestezi yöntemleri, postoperatif kanama miktarları, kullanılan sıvı miktarları, kullanılan hemostatik ajanlar, postoperatif kan transfüzyonu miktarları, uygulanan turnike süreleri, postoperatif ağrı modaliteleri, mobilizasyon süreleri ve postopertaif 1. ve 3. ayda gelişen komplikasyonların retrospektif incelenmesi amaçlanmıştır.Yöntem: Etik kurul onayı alındıktan sonra 2016 - 2017 tarihleri arasında Ortopedi ve Travmatoloji Kliniğinde; diz artroplasti cerrahisi uygulanan 564 hasta çalışmaya alındı. Hastalara ait tüm veriler INNOVIAN klinik bilgi sistemi, MIAMED hastane verileri ve hasta dosyalarından elde edilmiştir. Hastalar uygulanan cerrahi tekniğe göre total ve unikondiler diz artroplastisi olarak iki ana gruba ayrılıp her grup da kendi için anestezi yönetimine göre alt gruplara bölünmüştür. Hastaların demografik verileri, postoperatif kanama miktarları, intraoperatif kullanılan sıvı miktarları, peroperatif kullanılan hemostatik ajanlar (TxA kullanımı), postoperatif gelişen anemi oranları, postoperatif kan transfüzyonu oranları, intraoperatif cerrahi turnike süreleri, postoperatif ağrı modaliteleri, mobilizasyon süreleri ve postoperatif komplikasyon değerleri kaydedilmiştir.Bulgular: Demografik veriler açısından her iki cerrahi gruptaki hastalar benzerdi (p>0,05). Uygulanan anestezi yöntemleri karşılaştırıldığında kullanılan sıvı miktarı, turnike süresi, dren miktarı, mobilizasyon süresi, taburculuk süresi postoperatif 6. saat Hgb ve postoperatif anemi açısından fark yoktu. Total diz artroplastisinde postoperatif kanama daha çok saptandı (p0,05). Postoperatif ağrı yönetiminde tüm hastalarda avantajları nedeniyle multimodal ağrı yönetimi tercih edildi. Unikondiler diz artroplastilerinde taburculuk süresi TDA'ya göre daha kısaydı (p 0.05). When the anesthesia methods were compared, there was no difference in the amount of fluid used, tourniquet duration, amount of drain, mobilization time, duration of discharge from hospital, and postoperative 6th hour Hgb and postoperative anemia. Postoperative bleeding was more common in total knee arthroplasty (p 0.05). Postoperative bleeding was lower in patients using TxA as a hemostatic agent regardless of surgical procedure (p 0.05). Multimodal pain management was preferred because of the advantages of postoperative pain management in all patients. Abstract

Objectives: This study aims to evaluate the effects of two different doses of intraarticular ketamine on visual analog scale (VAS) scores at rest and movement, time to first analgesic requirement, and 24-h morphine consumption in patients undergoing arthroscopic meniscectomy as well as to assess the frequency of postoperative nausea&vomiting, respiratory depression, pruritus, urinary retention, and constipation and to compare the time to discharge., Patients and Methods: This prospective randomized double-blind study was performed between August 2013 and August 2014 on 75 patients (32 males, 43 females; mean age 46.7±13 years; range, 18 to 75 years) with American Society of Anesthesiologists scores of I-II scheduled for unilateral meniscectomy. Patients were randomized to receive 0.5 mg.kg -1 ketamine (group K1), 1 mg.kg -1 ketamine (group K2) or saline (group S) to a total volume of 20 mL intraarticularly at the end of the surgery. All patients were performed periarticular 10 mL 0.5% bupivacaine infiltration. Visual analog scale at rest and during passive knee movement was used to evaluate pain both preoperatively and at postoperative 0, 30 min, and 1, 2, 4, 6, 12, and 24 h. Time to first analgesic requirement and morphine consumption were recorded., Results: Visual analog scale scores at rest and during movement at postoperative 0 were significantly reduced in group K2 compared with group S (p<0.05). The first analgesic requirement time was significantly longer in group K1 (76.9±25.2 min) and group K2 (93.4±26.1 min) than group S (29.3±7.1 min). Morphine consumption was lower in group K2 compared to group K1 and group S at postoperative 30 min, and 1 and 2 h. However, 24-h morphine consumption was similar in all groups., Conclusion: Intraarticular injection of 0.5 mg.kg -1 and 1 mg.kg -1 ketamine for postoperative pain management provided similar analgesic efficacy. However, high dose ketamine more noticeably decreased opioid requirement in the early postoperative period.
