Introduction Primary spinal cord tumors are amenable to total surgical excision but the outcomes are still diverse. Objective The purpose of this study is to identify the significant factors which influence the long-term outcome of spinal cord tumors. Materials and Methods A total of 84 consecutive spinal cord tumor patients who underwent microsurgical excision during April 2003 to 2014 were retrospectively studied. Patients operated after April 2014 were not considered. Ten factors were taken and correlated with the outcome. Multiple regression analysis was used. Patients were periodically evaluated at 1, 6 months, 1, 2, and 5 years. In 72 cases, postoperative MRI scans were performed. The follow-up period ranged between 5 to 15 years. Results In our study, we had 64 extramedullary tumors and 21 intramedullary tumors with varied pathology. Axial location of the tumour (20.294/0.000), extent of the resection of the tumour (13.827/0.001), preoperative Nurick grade (11.349/0.023), and location of the tumour in the spine with respect to vertebral segments (8.151/0.017) were significant predictive factors. We had good outcomes in 65 cases and poor outcomes in 19 cases. Conclusion Our results show location of the tumor with respect to axial plane and vertebral segments were the main contributing factors. They influence the extent of excision of the spinal cord tumor. Preoperative neurological status is the fourth factor which independently determines the long-term outcome. It is one of the few papers where patients have been followed-up for a long period ranging from 5 to 15 years.

STUDY DESIGN It is a case series involving clinical presentation, radiological findings, surgical technique and long term outcome of Non-dysraphic intramedullary spinal cord lipomas in adults along with the review of the literature. PURPOSE The purpose of the study is to find out from our series as well as from literature what determines the long term outcome and how it can be improved in patients diagnosed to have intramedullary spinal cord lipomas. OVERVIEW OF LITERATURE Non-dysraphic spinal intramedullary lipomas in adults are extremely rare. Majority of cases occur in children and in cervico-dorsal regions. Only eight cases of dorso-lumbar spinal lipomas without spinal dysraphism in adults have been reported in the English literature till 2013. METHODS Here we report our experience with three such cases in the dorsolumbar region and discuss the surgical technique and the long term outcome of such cases. RESULTS Review of literature and from our own cases we conclude that long term outcome after surgery is determined by the preoperative neurological status. CONCLUSIONS Earlier surgical intervention with preserved neurological status results in better outcome. Radical subtotal excision without producing iatrogenic postoperative neurological deficit should be the goal of the surgery and it stabilizes the disease process in the long run. When early clinico-radiological signs of recurrence develop, such patient's to be reoperated immediately to prevent them from developing a fixed neurological deficit.

Introduction: In cervical disc disease, the exact location of the posterior osteophyte with relationship to the bodies of the adjacent cervical vertebra at each disc space level, as observed during surgery under operating microscope and correlating it with the histopathological development of the posterior osteophyte has not been documented in literature. A detailed review of literature on the development of posterior osteophytes and its impact on cervical spinal cord is also being reviewed. Materials and Methods: 1st Phase: In a prospective study conducted over first 5 years (2007-2012), intraoperative observations of 294 disc spaces in 201 patients who were operated for cervical disc disease using the standard anterior cervical microdiscectomy were analyzed. We observed under operating microscope 32 C3-C4 disc spaces, 62 C4-C5, 123 C5- C6, 74 C6-C7, 2 C7- D1 and 1 C2- C3 disc space during the study period. 2nd Phase: The above prospective study was extended into the 2nd phase over next 3 1/2 years (2009-2012), and clinico-radiological (MRI findings) - intraoperative observations of 118 disc spaces in 70 patients who were operated for cervical disc disease were analyzed. These observations were correlated with the histopathological characteristics of the excised disc material. Results: 1st Phase study: At C3-C4 disc space, posterior osteophyte originates from the upper vertebral body of C3 in 78.13% [25/32] disc spaces. In mobile segments of C4-C5 in 70.96% [44/62] and C5-C6 in 84.55% [104/123] disc spaces, the osteophyte arises from posterior margins of both the vertebral bodies. At C6 –C7, it arises from lower vertebral body of C7 in 71.62% [53/74]) disc spaces. 2nd Phase: It was observed histopathologically and intraoperatively, that posterior osteophytes formation goes through three stages. Posterior osteophyte formation is of Fibrocartilage in 10.17% (12/118), Mixed variety 7.62% (9/118) and Bony type in 82.20% [97/118]. In patients who had Bony type, 89.4% had myelopathy, 75 % had radiculopathy while 89.4% patients had hyperintense signal within the spinal cord. Discussion: Our results show that there is a definite pattern in the formation of the posterior osteophyte within the cervical disc spaces. At junctional areas like C3-C4 and C6 –C7 the posterior osteophyte originates from the relatively fixed vertebra like C3 and C7. In mobile segments like C4-C5 and C5- C6 the posterior osteophyte originates from both the bodies of adjacent vertebra. We also observed that the posterior osteophyte formation in cervical disc disease goes through the following three stages. 1st stage: Fibrocartilage, 2nd stage: Mixed type consisting of both the fibrocartilage and partially bony and 3rd stage: Bony type. These stages we feel evolve over a span of few years. Patients presenting with myeloradiculopathy and hyperintense signal within the spinal cord are likely to harbor bony posterior osteophytes compressing the thecal sac and requires surgical intervention. This is the first document of its kind in literature. Detailed reviews of literature on experimental models showing the development of posterior osteophyte supporting our observations, the pathological and radiological impact of the posterior osteophytes on cervical spinal cord, natural history of cervical spondylotic myelopathy and fate of the posterior osteophytes after anterior fusion surgery are being dealt upon in this paper.

Sleep disorders constitute major nonmotor features of Parkinson's disease (PD) that have a substantial effect on patients' quality of life and can be related to the progression of the neurodegenerative disease. They can also serve as preclinical markers for PD, as it is the case for rapid eye movement (REM)-associated sleep behavior disorder (RBD). Although the etiology of sleep disorders in PD remains undefined, the assessment of the components of the circadian system, including melatonin secretion, could give therapeutically valuable insight on their pathophysiopathology. Melatonin is a regulator of the sleep/wake cycle and also acts as an effective antioxidant and mitochondrial function protector. A reduction in the expression of melatonin MT(1) and MT(2) receptors has been documented in the substantia nigra of PD patients. The efficacy of melatonin for preventing neuronal cell death and for ameliorating PD symptoms has been demonstrated in animal models of PD employing neurotoxins. A small number of controlled trials indicate that melatonin is useful in treating disturbed sleep in PD, in particular RBD. Whether melatonin and the recently developed melatonergic agents (ramelteon, tasimelteon, agomelatine) have therapeutic potential in PD is also discussed. peerReviewed

Study Design A retrospective study was undertaken to delineate the characteristics of non-traumatic sequestrated epidurally migrated cervical disc prolapse. Purpose To present first case series of eight such cases diagnosed preoperatively and to discuss their magnetic resonance imaging (MRI) characteristics and their management. Overview of Literature Non-traumatic spontaneous migration of the sequestrated disc fragment epidurally behind cervical vertebral body is rare. Only ten cases have been reported in literature. Methods Detailed clinico-radiological profiles of these 8 cases are presented. In six cases their clinical picture was suggestive of cervical myelopathy. MRI scan showed single level epidural migrated disc behind body of C4, C6, and C7 in six patients and two cases with multiple levels (C5-C6). In six cases, anterior corpectomy with excision of the disc was performed and the seventh patient underwent dorsal laminectomy. The eighth patient chose not to undergo surgery. Results T1 images of the MRI scan showed an isointense signal in all the 8 cases. T2 images revealed a varying intensity. In six cases who underwent anterior corpectomy, there was a rent in the posterior longitudinal ligament. Among those in two cases multiple disc fragments were seen. In the rest four cases, a single large fragment was observed. These patients improved after anterior corpectomy and disc excision. There was no improvement in the patient who had undergone dorsal laminectomy. The eighth patient who refused surgery progressively deteriorated. Conclusions We opine that MRI scan especially T1 images are useful in these cases. We prefer to treat these cases through anterior corpectomy with excision of the sequestrated disc which proved to result in excellent outcome.

Introduction and Aim: The management of posterior fossa arachnoid cyst (PFAC) in adults is controversial. To review our cases and literature, propose a practically useful surgical strategy, which gives excellent long-term outcome in management of PFAC. Materials and Methods: We analyzed our case records of 26 large intracranial arachnoid cysts in adults treated over 12 years. Of them, we had 7 patients with symptomatic PFAC. Reviewed the literature of 174 PFAC cases (1973-2012) and added 7 of our new cases with a follow-up ranging from 3 to 12 years. Results: In 6 cases the PFAC was located in the midline. In the 7 th case, it was located laterally in the cerebello-pontine (CP) angle. All patients were treated surgically. Excision of the cyst was performed in 5 of these cases. Among the two intra-fourth ventricular cysts, in both the cases cysto-peritoneal shunt was performed. Postoperative computed tomography/magnetic resonance imaging showed variable decrease in size of the cyst even though clinically all patients improved. We propose a surgical strategy for the management of these cases which would aid the surgeon in decision making. Discussion: We observed that these PFACs can occur either in the midline within the fourth ventricle or retroclival region or extra-fourth ventricular region. It can also develop laterally in the CP angle or behind the cerebellum or as intracerebellar cyst. Importance of this is except for Midline Intra-fourth ventricular cyst/retroclival cyst, the rest all can be safely excised with excellent long term outcome. The treatment strategy for Midline Intra-fourth ventricular cyst/retroclival cyst can be either cysto-peritoneal shunt or endoscopic fenestration of the cyst.
