4. Vitamin D deficiency, nutritional rickets and osteomalacia : hidden disease burden and implications for public health prevention policies

Abstract

Vitamin D deficiency remains the most common cause of nutritional rickets in the UK with considerable rise in reported cases in the last decade, however the true extent of disease burden remains unexplored. The thesis contains three prospective studies investigating the hidden burden of vitamin D deficiency in: 1) infants presenting with symptomatic deficiency and their family members, 2) a large multi-ethnic newborn cohort and 3) Sudden Unexpected Death in Infancy and Childhood (SUDIC). In infants with severe deficiency, significant cardiac dysfunction, growth plate abnormalities and osteomalacia was uncovered in infants with severe deficiency; and biochemical osteomalacia in 77% of their asymptomatic family members. Only a third of the UK newborns (n=919/2999) had sufficient 25 hydroxyvitamin D (25OHD) levels, with babies born in the winter to dark-skinned mothers most affected. At post-mortem examination (n=33, 29 infants) 70% had histological rickets at the growth plate but only 40% manifest biochemical vitamin D deficiency and only 9% radiological rickets. Lower 24,25 dihydroxyvitamin D3 [24,25(OH)2D3] levels and elevated ratio of 25OHD3:24,25(OH)2D3 were better indicators of histological rickets when compared to 25OHD3. Elevated osteoid surface, volume and thickness were noted in roughly 33%, 62% and 14% respectively. On three-point bending test, rib samples with histological rickets displayed disproportionate displacement in response to load indicating increased plasticity and had a tendency towards higher fracture toughness.

Abstract

Context Denosumab is an effective treatment for many receptor activator of nuclear factor kappa-B ligand (RANKL)-mediated disorders but there are potential safety considerations and limited data to guide its use in children and adolescents. Objective This document seeks to summarize the evidence and provide expert opinion on safe and appropriate use of denosumab in pediatric RANKL-mediated disorders. Participants Ten experts in pediatric bone and mineral medicine from 6 countries with experience in the use of denosumab participated in the creation of this document. Evidence Data were sourced from the published literature, primarily consisting of case reports/series and review articles because of the lack of higher level evidence. Expert opinion of the authors was used substantially when no published data were available. Conclusion Denosumab is an effective treatment for RANKL-mediated disorders in children and adolescents but is often not curative and, in some cases, is best used in conjunction with surgical or other medical treatments. Careful multidisciplinary planning is required to define the goals of treatment and expert oversight needed to manage the risk of mineral abnormalities. This study aimed to explore the effect of maternal socioeconomic status (SES) on newborn 25-hydroxyvitamin D (25OHD) concentrations. Design: Over two 1-week periods (winter and summer of 2019), 3000 newborn heel prick dried blood spots (DBS) and additional data from newborns, from a regional newborn screening laboratory (52° N) in the West Midlands, UK, were gathered. Post code was replaced with lower layer super output area (LSOA). Index of Multiple Deprivation (IMD) quintiles for the corresponding LSOA was used to assess SES [quintile one (Q1): most deprived 20%, quintile five (Q5): least deprived 20%]. Each of the seven domains of deprivation were examined (income, employment, education, health, barriers to housing and services, crime and living environment). 25OHD was measured on 6mm sub-punch from DBS using quantitative liquid chromatography tandem mass spectrometry and equivalent plasma values were derived.
