- Abstract
Data on deformation profile of athlete's heart are still scarce. However, strain values of athletes can reveal several correspondence between training and systolic or diastolic function and can deepen our understanding of their cardiac physiology. We sought to investigate the left ventricular (LV) longitudinal and circumferential deformation parameters in a large cohort of athletes and to determine the related factors. Elite athletes (EA; n=147, mean age 26±6 years, 20% women) competing in sport disciplines of combined exercise nature, were investigated and compared to age- and gender matched, healthy, sedentary volunteers (NC; n=44, 27±8 years, 25% women). Beyond standard two-dimensional echocardiographic protocol (Philips iE33), parasternal short-axis and apical views optimized for speckle tracking analysis were obtained. LV global longitudinal strain (GLS) and global circumferential strain (GCS) were calculated by averaging the values of the standard 16 LV segments. Data are presented as mean±SD, p values under 0.05 were considered as statistically significant. EA had increased LV mass (EA vs. NC; 113±22 vs. 95±30 g/m2, p<0.01). Ejection fraction did not differ between groups (59.6±6.3 vs. 60.5±4.5 %). GLS of athletes was significantly lower (-21.5±2.3 vs. -22.9±2.9 %, p<0.01), while GCS was similar compared to controls (-29.7±4.8 vs. -29.9±4.9 %). Strain values did not differ between genders (EA group, females vs. males; GLS: -22.2±1.6 vs. -21.3±2.4; GCS -29.9±5.1 vs. -29.7±4.7 %). In contrast to NC, in the EA group GLS correlated with age (r=-0.29), weight (r=0.36), height (r=0.41) and also with early diastolic mitral inflow velocity (r=-0.28) and isovolumic relaxation time (r=0.31, p<0.05). In athletes, GCS inversely correlated with LV mass (r=0.38) and was found to be the only independent predictor of ejection fraction among strain parameters (β=-0.31, p<0.01). LV longitudinal strain is decreased, circumferential strain is preserved in elite athletes, which might have a key role in maintaining ejection fraction at resting conditions. LV longitudinal strain is related to anthropometric features and early diastolic function, whereas circumferential strain is affected by LV mass. Our results can represent reference values for athlete's heart using the current platform.

- Abstract
Purpose: Atrial fibrillation (AF) is the most prevalent sustained arrhythmia. It is a disease of the elderly and it is common in patients (pts) with structural heart disease. Hypertension (HA), hypertensive heart disease (HHD), diabetes mellitus (DM), coronary artery disease (CAD), heart failure (HF), and valvular heart disease (VHD) are recognized predisposing factors to AF. Objectives: To echocardiographicly disclose the most common predisposing morbidities to AF in our population sample. Methods: From June 2000 to February 2013, 3755 consecutive pts with AF were studied during echocardiographic check-up. According to transthoracic echo, pts were divided in groups based on dominative underlying heart diseases. Electrocardiographically documented AF was subdivided in two groups: transitory and chronic. Transitory AF fulfilled criteria for paroxysmal or persistent AF. Chronic AF were cases of long-standing persistent or permanent AF. Results: The median age was 72 years, age range between 16 and 96 years. There were 51.4% of females. Chronic AF was observed in 68.3% pts. Distribution of underlying heart diseases is shown in figure. Lone AF was diagnosed in only 25 pts, mostly in younger males (median age 48 years, range 29–59, men 80%). Chronic AF was predominant in groups with advanced cardiac remodeling such as dilatative cardiomyopaty (DCM) and VHD, mostly in elderly. HA and DM were found in 75.4% and 18.8%, respectively. Almost 1/2 of pts with AF had HF and 59.2% had diastolic HF. Conclusion: Up to now, echocardiographic categorization of the predisposing factors to AF was not reported. Echocardiographic evaluation of patients with AF could facilitate in identification and well-timed treatment of predisposing comorbidites.

- Abstract
Background: Differences in definitions and measurements of parameters describing left ventricular (LV) mechanics among vendors hamper the use of 3D deformation analysis for clinical and research purposes. Our aim was to identify the reference values for 3D LV function parameters using a vendor-independent DICOM-based software. Methods: In 235 healthy volunteers uniformly distributed across decades (44±14 years, range 18–76 years), 3D LV full volume data sets (35±6 vps) were acquired using a GE Vivid E9 scanner. Exclusion criteria were athletic training, pregnancy, body mass index > 30 kg/m2, and poor apical acoustic window. LV 3D parameters (including ejection fraction, strain parameters, systolic dyssynchrony index - SDI, twist and torsion) were analyzed offline using 4D LV Analysis software 3.1 (TomTec, D). Results: Physiologic ageing was associated with a significant increase in LV ejection fraction and torsion, LV circumferential and 3D strain, and with a less synchronous regional LV function (p<0.0001 for all, except p=0.01 for torsion). Women had higher LV ejection fractions and absolute values of longitudinal and 3D strain than men (Table), but also smaller LVs (EDV 55±9 vs 64±11 ml/m2) and lower blood pressure values (SBP 114±14 vs 128±13 mmHg, p<0.0001). LV torsional mechanics (twist 7.7 vs 6.4°, torsion 0.91 vs 0.83°/cm, p=NS) and synchronicity (SDI 5.2±1.3 vs 5.2±1.3%) were similar between genders. LV deformation also showed significant regional variations, strain values in LV basal region being significantly lower than in mid or apical regions (p<0.001 for all strains). Conclusions: This study is the first to report age- and gender-specific normative values for global LV systolic function and deformation assessed by a vendor-independent software. Our data may foster the use of 3DE for assessing LV myocardial function in multi-vendor echo labs.
