It was reported that a new type of coronavirus (2019-nCoV) was the cause of repeated pneumonia cases in Wuhan, Hubei Province, China in December 2019. The aim of this study was to evaluate the laboratory results of the patients diagnosed with COVID-19 as confirmed by RT-PCR test or presence of ground-glass opacities on CT imaging. 51 people with various exclusion criteria were included in the single-center study planned retrospectively. Comparison was made by measuring the laboratory findings of the patients who underwent RT-PCR test, who underwent computed lung tomography and who were divided into four groups according to their results. All data were accessed via electronic health records. Fisher Exact Test, One Way ANOVA, Kruskall Wallis Test were used as statistical analysis method in addition to percentages and frequencies and adjusted Bonferonni test were used for post-hoc analysis. In laboratory findings, significant data were obtained in favor of COVID-19 at WBC, NEU, EOS, Prothrombin time, INR and Troponin levels. It was concluded that laboratory findings can be used as a preliminary diagnosis in the admission of the patient to the clinic, and also help to determine the clinical severity of the disease and predict the prognosis. Aralık 2019’da Wuhan, Hubei Çin’de arda arda görülen pnömoni vakalarının sebebinin yeni bir tip koronovirus (2019-nCoV) olduğu bildirilmiştir. Bu çalışmada COVID-19 tanısı RT-PCR testi ile doğrulanmış veya BT görüntülemesinde buzlu cam alanları tespit edilmiş hastaların laboratuar bulgularının değerlendirilmesi amaçlanmıştır. Tek merkezli retrospektif olarak planlanan çalışmaya çeşitli dışlama kriterleri ile 51 kişi dahil edilmiştir. Karşılaştırma, RT-PCR testi yapılan, bilgisayarlı akciğer tomografisi yapılan ve sonuçlarına göre dört gruba ayrılan hastaların laboratuvar bulguları ölçülerek yapıldı. Tüm verilere elektronik sağlık kayıtları aracılığıyla erişildi. COVID-19 tedavi protokollerinde kortikosteroidler yaygın olarak kullanılmasına rağmen bağışıklığın baskılaması nedeniyle fırsatçı enfeksiyonlar ve invaziv pulmoner aspergillozis yönünden tartışılmaktadır. Bu çalışma COVID-19 tanısı almış iki olguda yüksek doz kortikosteroid tedavisi ile ilişkili invaziv pulmoner aspergillozis olgusunu sunmak amacıyla yapılmıştır. Bu çalışmada yer alan her iki olgu daha önce invaziv pulmoner aspergillozis yönünden risk faktörü olmayan, hipertansiyon tanısı almış ve herhangi bir bağımlılıkları bulunmayan genç erkek hastalardır. Her iki olguda COVID-19 tedavisi süreçlerinde yüksek doz metilprednizolon tedavisi almış ve post-covid dönemde invaziv pulmoner aspergillozis enfeksiyonu gelişmiştir. Vakaların biri COVID-19 tanısın 81. ve IPA tanısının 26. gününde gelişen arrestin ardından yanıtsız kardiyopulmoner resustasyon sonucunda exitus kabul edilirken, diğeri COVID-19 tanısının 58. ve IPA tanısının 19. gününde oksijen desteği ile taburcu edilmiştir. IPA COVID-19 tanılı ve immünosupresif tedavi gören hastalarda yaşamı tehdit eden bir komplikasyon olarak düşünülmelidir. COVID-19 hastalarına uzun süreli ve yüksek doz kortikosteroid kullanılması halinde IPA yönünden dikkatli olunması ve bu hastaların bu amaçla taranması endike olabilir., Although corticosteroids are widely used in COVID-19 treatment protocols, they are discussed in terms of opportunistic infections and invasive pulmonary aspergillosis due to immune suppression. This study was conducted to present a case of invasive pulmonary aspergillosis associated with high-dose corticosteroid therapy in two cases diagnosed with COVID-19.Both cases included in this study were young male patients who had no previous risk factor for invasive pulmonary aspergillosis, were diagnosed with hypertension, and did not have any addictions. Both cases received high-dose methylprednisolone treatment during the treatment of COVID-19 and invasive pulmonary aspergillosis infection developed in the post-covid period. One of the cases was accepted as exitus as a result of unresponsive cardiopulmonary resuscitation following the arrest that developed on the 81st day of the COVID-19 diagnosis and the 26th day of the IPA diagnosis, while the other was discharged with oxygen support on the 58th day of the COVID-19 diagnosis and the 19th day of the IPA diagnosis.IPA should be considered as a life-threatening complication in patients diagnosed with COVID-19 and receiving immunosuppressive therapy. If long-term and high-dose corticosteroids are used in COVID-19 patients, it may be indicated to be careful in terms of IPA and to screen these patients for this purpose.

15. Depression, Anxiety and State Guilt in Individuals under Quarantine in an Institution due to COVID-19 and the Related Factors

INTRODUCTION: This study identifies depression, anxiety and state guilt of Turkish citizens kept in quarantine for 14 days in an institution where they are brought upon their request within the scope of infection control measures and the related affecting risk factors during the coronavirus pandemic. METHODS: A total of 385 individuals under quarantine for 14 days between the dates of April 20-May 21, 2020 were included in the study. The data were collected using a socio-demographic data form (age, gender, employment status, having sleep disorder, duration of television review and Internet use, etc.), Beck Anxiety Inventory (BAI), Beck Depression Inventory (BDI) and Guilt Scale (GS) after the participants were informed about the study objective and they signed the informed consent form. RESULTS: A total of 385 participants, 84 of whom were women, with a mean age of 35.32±11.7 were included in the study. According to BAI and BDI, 14.5% of the participants had anxiety and 17.1% had depression symptoms. The regression analysis found that female gender, having sleep disorder, using Internet for 8 hours or more and having chronic illness were risk factors for anxiety; while female gender, being aged 18-30, having sleep disorder, having psychiatric illness, and using Internet for 8 hours or more were risk factors for depression. The state guilt did not show any significant correlation with any socio-demographic factor. CONCLUSION: Female gender, being young, having a history of psychiatric illness, having chronic illness, having sleep disorder, using Internet for 8 hours or more were found to be risk factors for more anxiety and depression symptoms. It is important to educate people about the methods of how to maintain healthy sleep during quarantine, to effectively fight against the excessive circulation of misinformation and to provide sufficient medical care to those with psychiatric and chronic diseases, being more vulnerable against infections.

Bu çalışma, dijitalleşen uygulamalarla beraber hemşire iş yükü dağılımında, zaman, hareket, maliyet ve hata gibi israflardan arındırılan uygulamaların, hastaya ayrılan zamana, hasta ve çalışan güvenliğine etkisini yalın yaklaşım ile incelemek amacıyla yapılmıştır. Çalışma, Tanımlayıcı tipte gözlemsel bir araştırmadır.Araştırmanın verileri yalın yöntemler olan Gemba Yürüyüşü, Değer Akış Haritalama, Yedi İsraf Tablosu ve araştırmacı tarafından hazırlanan Faaliyet Çizelgesi ve Sosyodemografik Veri Formu ile toplanmıştır. Çalışmaya Bolu'da bir devlet hastanesinin iki farklı dahili branşlar kliniklerinde çalışan 15 hemşire katılmıştır. Dahili kliniklerden biri hemşire iş süreçlerinde dijital uygulamaları kullanırken, diğerinde dijital uygulamalar sınırlı bir şekilde kullanmaktadır. Verilerin analizinde, değer akış haritalama yöntemi kullanılmıştır. Mevcut durumda sistem ölçütleri, kısmi dijital klinikte toplam akış süresi 24 saat, değer katan süre 17.5 saat, değer katmayan süre 6.5 saattir. Dijital klinikte ise toplam akış süresi 24 saat, değer katan süre 19.6 saat, değer katmayan süre 4.4 saattir. Her iki klinik için belirlenen gereksiz üretim, stok, bekleme, işlem, hareket, taşıma ve hata israf odakları yedi israf tablosuna yerleştirilmiştir. Yalın teknikler olan heijunka, kaikau, 5S, kanban, jidoka, andon ve poka yoke yöntemleriyle iyileştirme önerileri yapılarak gelecek durumda sistem ölçütleri toplam akış süresi 8 saat, değer katan süre 7.5 saat, değer katmayan süre 0.5 saat olarak öngörülmüştür.Sonuç olarak, dijital uygulamaların, hemşire iş akışlarını iyileştireceği, doğrudan hasta bakımına ayrılan sürenin artacağı, dokümantasyona harcanan sürenin azalacağı, hasta, çalışan güvenliğinin ve memnuniyetinin artacağı, böylece israfın değere dönüşeceği görülmüş ve hemşire iş akışlarına süreçlerin analizi ve iyileştirme çalışmalarında yalın felsefenin kullanılmasının faydalı olacağı kanısına varılmıştır. The aim of this study is to examine the effects of nursing practices, which are free of wastes such as time, movement, cost and error with digitalized workflows, on patient time, patient and employee safety with a lean approach. The study is an observational study of descriptive type.In this study, Gemba Walk, Value Stream Mapping and Seven Wastes Table which are lean techniques for analysis were used. In addition, the Work Analysis Chart and Sociodemographic Data Form prepared by the researcher was used. The study included 15 nurses working in two different internal medicine clinics of a state hospital in Bolu. One of the internal medicine clinics uses digital applications in nursing work processes, while the other uses digital applications in a limited way.In the data analysis, value stream mapping method was used. For the current situation, the system criteria were measured as L/T 24 hours, value adding time 17.5 hours and time that does not add value is 6.5 hours in the partial digital clinick. Redundant production, stock, waiting, processing, movement, transport and defects foci determined for both clinics are placed in seven waste tables. Heijunka, kaikau, 5S, kanban, jidoka, andon and poka yoke which are lean methods have been proposed to improve. System criteria for future status L/T 8 hours, value adding time 7.5 hours and time that does not add value was estimated as 0.5 hour. As a result, it has been seen that digital applications will improve nurse workflows, increase the time allocated to direct patient care, decrease in the time spent on documentation, increase in patient, employee safety and satisfaction, thus wasting becomes value. It was concluded that the use of lean philosophy in the analysis and improvement of processes in nurse workflows would be beneficial. 71

Objective:This cross section study has been conducted to determine the burnout level ofthe nurses working in a university hospital and to analyze the relationshipbetween burnout and socio-demographic variables. Methods: The study has been done on 192 nurses who agreed toparticipate. Data has been collected by a sociodemographic data form consistingof 31 questions and consisting of 22 questions Maslach Burnout Inventory (MBI).Results: The nurses’ MBI emotionalexhaustion average subscale points were determined as 20.3±7.0, depersonalization subscaleaverage points were determined as 6.0±3.2 and personal accomplishment averagesubscale points were determined as 19.3±4.1. Gender, perceived income level,presence of chronic disease, history of receiving psychiatric help, totallength of service in clinic, problem life while taking annual leave,occupational compliance, adherence to the profession, love of the professionand the possibility of professional development of the institution were foundto affect burnout. Bu araştırma üniversite hastanesinde çalışan hemşirelerde tükenmişlik düzeylerini belirlemek ve tükenmişliğin sosyo-demografik değişkenlerle ilişkisini incelememesi amacıyla kesitsel olarak yapılmıştır.Araştırmada örneklem seçilmesine gidilmemiş evrenin tamamı örnekleme alınmıştır. İlgili hastanede çalışıp araştırmamıza katılmaya gönüllü olan 192 hemşire ile çalışma tamamlanmıştır. Araştırmanın verileri Mayıs 2015 tarihinde toplanmıştır.Veri toplamada 31 sorudan oluşan sosyodemografik veri formu ve Maslach Tükenmişlik Ölçeği (MTÖ) kullanılmıştır. Verilerin değerlendirilmesinde Mann-Whitney U ve Kruskall Wallis Test ile T-test ve One-way ANOVA testi kullanılmıştır.Hemşirelerin MTÖ 'den aldıkları puan ortalamalarına göre tükenmişlik düzeyi yüksek bulunmuştur. Tükenmişlik alt boyutlarında katılımcıların duygusal tükenme yüksek ve kişisel başarı algısı düşük olarak belirlenmiştir. Duyarsızlaşma alt boyutunda da tükenme orta düzeyde seyretmiştir.Üniversite hastanesinde çalışan hemşirelerde tükenmişlik düzeyi ile ilişkili bulunan başlıca faktörler cinsiyet, aile yaşantısı, gelir düzeyi algısı, tanısı konulmuş hastalık durumu, psikolojik destek öyküsü, çalışılan klinikteki toplam hizmet süresi, yıllık izin kullanım durumu, mesleki yönden gelişim faaliyetleri, iş yerindeki sorun algısı, mesleki adanmışlık düzeyidir. Tükenmişlik düzeyleri ile ilişkili bulunmayan bazı değişkenler ise yaş, medeni hal, eğitim düzeyi, sürekli kullanılan ilaç varlığı, mesleki esaslar (mesai çalışma şekli, nöbet sayısı, çalışılan klinik vb.), kişisel alışkanlıklardır.Sonuç olarak; tükenmişlik sendromu hemşirelerin kişisel özellikleri ile iş yeri ve mesleki özelliklerinin etkileşimleri sonucunda yaşanabilmektedir. Tüm bu değişkenler tükenme düzeylerini ve tükenme alt boyutlarını doğrudan veya dolaylı olarak etkileyebilmektedir.Anahtar Kelimeler: Tükenmişlik, Profesyonel, Hemşireler, Risk Faktörleri. The present study performed cross-sectional to investige burnout levels of the nurses working in university hospital and and the relationship between burnout and sociodemographic variables.A sample selection was not prefered and the whole universe was accepted as sample instead. 192 nurses working in the related hospital were included in the study. The data collection of the study was completed in May 2015. A sociodemographic data form including 31 questions and Maslach Burnout Scale (MBS) were used in data collection. The evaluation of the data was performed with Mann-Whitney U and Kruskall Wallis Test , T-test and One-way ANOVA.Burnout levels were found increased in the study group. In burnout sub-dimension, emotional burnout of the subjects were found increased while personal success sense was found decreased. In desensetization sub-dimension burnout was found as moderate levels. The factors that have been found related with burnout levels of the nurses in universty hospital are sexuality, family life, sense of income levels, diagnosed diseases, story of psychological support, total time of service in the department, annual leave use, professional development activities, problem sense at work, levels of professional commitment, In concusion, burnout syndrome may be seen as a result of interaction of personal properties and workplace and Professional properties. 23. Adaptation of the Adverse Childhood Experience Questionnaire for Adults Into Turkish: A Validity and Reliability Study.

- Abstract
Background and Purpose: Questioning the adverse childhood experiences of adults may facilitate their search for help for their physical, social, and psychological well-being. The aim of this study is to determine the validity and reliability of the Adverse Childhood Experience Questionnaire for Adults in Turkish. Methods: This methodological study was conducted with 290 participants. The data collection tools consisted of a personal information form, the Adverse Childhood Experience Questionnaire for Adults, and the Childhood Trauma Experiences Questionnaire Short Form. Results: The number of items in the original questionnaire was 29 and became 24 in the Turkish adaption, which showed content validity, construct validity, and internal consistency reliability (Cronbach's alpha (α) = 0.928). Conclusions: The Adverse Childhood Experience Questionnaire for Adults was found to be valid and reliable for the Turkish population., (© 2024 Springer Publishing Company.)

24. Perineal care incontinence training for caregivers: randomised controlled trial.

- Abstract
Objectives: The aim of this study was to implement training to increase the perineal care knowledge of relatives caring for incontinent palliative care patients and to examine the effectiveness of this training., Methods: This study was designed as a pretest-post-test randomised controlled trial. The study included 84 relatives of patients with incontinence (experimental group, n=42; control group, n=42). The experimental group was given one-on-one face-to-face training on perineal care for patients with incontinence, while the control group was given training through a brochure. Data were collected using a personal information form and the Perineal Care Knowledge Test. The Wilcoxon signed-rank and Mann-Whitney U tests were used for statistical analysis., Results: The difference between the pretest and post-test scores of the experimental group was found to be statistically significant (p<0.05), while the difference between the pretest and post-test scores of the control group was not significant (p>0.05). The post-test scores of the experimental group were higher than those of the control group (p<0.05)., Conclusions: It was found that one-on-one face-to-face perineal care training was a more effective method for increasing the perineal care knowledge of patients' relatives than brochures., Competing Interests: Competing interests: None declared., (© Author(s) (or their employer(s)) 2023. 25. Depression, Anxiety and State Guilt in Individuals under Quarantine in an Institution due to COVID-19 and the Related Factors.

- Abstract
Introduction: This study identifies depression, anxiety and state guilt of Turkish citizens kept in quarantine for 14 days in an institution where they are brought upon their request within the scope of infection control measures and the related affecting risk factors during the coronavirus pandemic., Methods: A total of 385 individuals under quarantine for 14 days between the dates of April 20-May 21, 2020 were included in the study. The data were collected using a socio-demographic data form (age, gender, employment status, having sleep disorder, duration of television review and Internet use, etc.), Beck Anxiety Inventory (BAI), Beck Depression Inventory (BDI) and Guilt Scale (GS) after the participants were informed about the study objective and they signed the informed consent form., Results: A total of 385 participants, 84 of whom were women, with a mean age of 35.32±11.7 were included in the study. According to BAI and BDI, 14.5% of the participants had anxiety and 17.1% had depression symptoms. The regression analysis found that female gender, having sleep disorder, using Internet for 8 hours or more and having chronic illness were risk factors for anxiety; while female gender, being aged 18-30, having sleep disorder, having psychiatric illness, and using Internet for 8 hours or more were risk factors for depression. The state guilt did not show any significant correlation with any socio-demographic factor., Conclusion: Female gender, being young, having a history of psychiatric illness, having chronic illness, having sleep disorder, using Internet for 8 hours or more were found to be risk factors for more anxiety and depression symptoms. It is important to educate people about the methods of how to maintain healthy sleep during quarantine, to effectively fight against the excessive circulation of misinformation and to provide sufficient medical care to those with psychiatric and chronic diseases, being more vulnerable against infections., Competing Interests: Conflict of Interest: The authors state no conflict of interest., (Copyright: © 2021 Turkish Neuropsychiatric Society.)
