The first successful lung transplant in Colombia was performed on October 28, 1997 in Medellín by Alberto Villegas Hernández at the "Clínica Cardiovascular Santa María" today called the Cardio VID Clinic. Here we present both survival outcomes and characteristics of the oldest and most experienced lung transplant program in Colombia. We conducted a retrospective study of all patients taken to lung transplantation at the Cardio VID Clinic in Medellín, Colombia from October 1997 to October 2022. Patient information from our institutional database and transplant archives were retrieved and reviewed. From October 1997 to October 2022, a total of 153 patients underwent orthotopic lung transplantation at our institution in Medellín, Colombia. Mean recipient age was 48 ± 13 years, the youngest patient was 15 years old and the oldest patient was 73 years old at the time of transplant. Seventy-four (48.4%) patients were men and seventy-nine (51.6%) were women. Uncensored lung transplant survival in Medellin at 1 month, 1 year, 5 years, and 10 years were 68%, 50%, 31%, and 12%, respectively. Although health care coverage in Colombia reaches nearly 100%, socioeconomic hurdles during post-transplant care, nonreturning patients, infections, and traumatic donor deaths lead to high mortality rates. Introduction: Data on extra-corporeal membrane oxygenation (ECMO) therapy for pregnant patients with Coronavirus 2019 (COVID-19) infection are limited. Here we report a case of an emergency cesarean section performed while the COVID-19 positive mother was on ECMO support. Case report: A 36-year-old COVID-19 positive patient at 26 weeks gestational age presented with respiratory failure requiring extra-corporeal membrane oxygenation therapy. Nine days later fetal distress demanded an emergency C-section. After 5 weeks on ECMO, the patient was weaned off. Both mother and child were discharged. Discussion: The decision to perform an urgent C-section is one that requires meticulous thought from the attending team. Pulmonary maturation is key as pregnancy may need to be terminated at any time during ECMO. Conclusion: Data on ECMO support for pregnant patients with COVID-19 infection are scarce. Best results can be achieved ensuring adequate anticoagulation, meticulous choice of cannulas, continued fetal monitoring, early lung maturation, and precision timing of delivery. The appearance of atomic structures of nanoscopic dimensions, with new and interesting physical properties, requires revisiting various aspects of particle interaction with solid matter. For this purpose in this work we present a study of electronic energy loss of H+ and protons (fragments) from the dissociation of H2+ ions interacting with ultra-thin amorphous silicon films for incident beam energies from 1 to 10 keV/u. We report measurements of energy distributions of transmitted protons and molecular fragments through these films, from which we derive the average energy losses and the energy loss straggling. Our experimental findings turn out to be in good agreement with nonlinear electronic stopping power models and some previous experimental data. [ABSTRACT FROM AUTHOR]

Gamma Knife Icon is the latest version of the Gamma Knife - a stereotactic radiosurgery instrument known for its accuracy and efficiency in treating disorders and lesions of the brain. As an alternative to surgery the Icon device enables the treatment of patients immobilized not only in a traditional frame but also in a customized "face mask." Mask-based treatments are convenient and permit delivery of multifraction treatments that are suitable for larger lesions (>3cm/10cc) and lesions abutting critical brain structures, which would otherwise be unsuited for single-fraction treatment. Also, on-board cone beam CT capability and infra-red camera incorporation increase confidence in the precision of treatment deliver): The Icon increases the flexibility of the treatment process while ensuring the same level of daily accuracy. In this article, we discuss the unique capabilities of the Icon that make it a complete intracranial stereotactic radiation treatment solution and share our experience with its use at the University of Mississippi Medical Center (UMMC) over the past two years. Carbon nanotube properties can be modified by ion irradiation; therefore it is important to know the manner in which ions deposit energy (how much and where) in the nanotubes. In this work, we have studied, experimentally and with a simulation code, the irradiation of multi-walled carbon nanotubes (MWCNT), supported on a holey amorphous carbon (a-C) substrate, with low energy (2–10 keV/u) H+ and H2+ molecular beams, impinging perpendicularly to the MWCNT axis. The energy distribution of protons traversing the nanotubes (either from the H+ beam or dissociated from the H2+ beam) was measured by the transmission technique in the forward direction. Two well-differentiated peaks appear in the experimental energy-loss distribution of the fragments dissociated from the molecular H2+ beam, in correspondence to the ones detected with the proton beam. One is the low-energy loss peak (LELP), which has a symmetric width; the other is the high-energy loss peak (HELP), which shows an asymmetric broadening towards larger energy loss than the corresponding proton energy distribution. A semi-classical simulation, accounting for the main interaction processes (both elastic and inelastic), of the proton trajectories through the nanotube and the supporting substrate has been done, in order to elucidate the origin of these structures in the energy spectra. Regarding the H+ energy spectrum, the LELP corresponds to projectiles that travel in quasi-channelling motion through the most outer walls of the nanotubes and then pass through the substrate holes, whereas the HELP results mostly from projectiles traversing only the a-C substrate, with the asymmetry broadening being due to a minor contribution of those protons that cross the a-C substrate after exiting the nanotube. The broadening of the peaks corresponding to dissociated fragments, with respect to that of the isolated protons, is the result of vicinage effects between the fragments, when travelling in quasi-channelling conditions through the outer layers of the nanotube, and Coulomb explosion just after exiting the target. The excellent agreement between the measured and the simulated energy spectra of the H+ beam validates our simulation code in order to predict the energy deposited by ion beams in carbon nanotubes. An infrequent yet known complication of ECMO is abdominal compartment syndrome requiring emergency laparotomy. Also, the need for prolonged enteral nutrition while on ECMO may require endoscopic gastrostomy to maintain adequate nutritional status. Here we describe our experience with emergency laparotomy and endoscopic gastrostomy in patients on ECMO support.We retrieved patient histories from our clinical archives and performed a retrospective description of all patients taken to an emergency laparotomy or endoscopic gastrostomy while on ECMO support at our cardiovascular referral center from July 2019 through June 2024.During the research period of 5 years a total of 401 patients were placed on ECMO support for either cardiogenic shock or respiratory failure. A total of 27 (7%) patients required an abdominal intervention while on ECMO. 14 (3.5%) patients required emergency laparotomy and 13 (3.2%) of patients required endoscopic gastrostomy tube placement. Overall 30-day mortality of all patients requiring a general surgery procedure while on ECMO support was 33%.ECMO support can result in many complications despite its many benefits. Patients who require emergency laparotomy while on ECMO have lower survival-to-discharge and higher mortality at 30 days. Endoscopic gastrostomy however, can be safely performed on ECMO with little to no bleeding complications despite anticoagulation. [ABSTRACT FROM AUTHOR]

Background: Brain shift and pneumocephalus are major concerns regarding deep brain stimulation (DBS)., Objective: To report the extent of brain shift in deep structures and pneumocephalus in intraoperative magnetic resonance imaging (MRI)., Methods: Twenty patients underwent bilateral DBS implantation in an MRI suite. Volume of pneumocephalus, duration of procedure, and 6 anatomic landmarks (anterior commissure, posterior commissure, right fornix [RF], left fornix [LF], right putaminal point, and left putaminal point) were measured., Results: Pneumocephalus varied from 0 to 32 mL (median = 0.6 mL). Duration of the procedure was on average 195.5 min (118-268 min) and was not correlated with the amount of pneumocephalus. There was a significant posterior displacement of the anterior commissure (mean = -1.1 mm, P < .001), RF (mean = -0.6 mm, P < .001), LF (mean = -0.7 mm, P < .001), right putaminal point (mean = -0.9 mm, P = .001), and left putaminal point (mean = -1.0 mm, P = .001), but not of the posterior commissure (mean = 0.0 mm, P = .85). Both RF (mean = -.7 mm, P < .001) and LF (mean = -0.5 mm, P < .001) were posteriorly displaced after a right-sided burr hole. There was a correlation between anatomic landmarks displacement and pneumocephalus after 2 burr holes (rho = 0.61, P = .007), but not after 1 burr hole (rho = 0.16, P = .60)., Conclusion: Better understanding of how pneumocephalus displaces subcortical structures can significantly enhance our intraoperative decision making and overall targeting strategy.
